Abstract:Spacecraft observations of turbulence within a magnetic reconnection (guide field approximately 0) ion diffusion region are presented. In the inertial subrange, electric and magnetic fluctuations both followed a -5/3 power law; at higher frequencies, the spectral indices were -1 and -8/3, respectively. The dispersion relation was found to be consistent with fast-mode-whistler waves rather than kinetic Alfvén-ion cyclotron waves. Lower hybrid waves, which could be enhanced by whistler mode conversion, were observed, but the associated anomalous resistivity was not found to significantly modify the reconnection rate.

Abstract:INTRODUCTION: Mouth breathing (MB) is an etiological factor for sleep-disordered breathing (SDB) during childhood. The habit of breathing through the mouth may be perpetuated even after airway clearance. Both habit and obstruction may cause facial muscle imbalance and craniofacial changes.OBJECTIVE: The aim of this paper is to propose and test guidelines for clinical recognition of MB and some predisposing factors for SDB in children.METHODS: Semi-structured interviews were conducted with 110 orthodontists regarding their procedures for clinical evaluation of MB and their knowledge about SDB during childhood. Thereafter, based on their answers, guidelines were developed and tested in 687 children aged between 6 and 12 years old and attending elementary schools.RESULTS: There was no standardization for clinical recognition of MB among orthodontists. The most common procedures performed were inefficient to recognize differences between MB by habit or obstruction.CONCLUSIONS: The guidelines proposed herein facilitate clinical recognition of MB, help clinicians to differentiate between habit and obstruction, suggest the most appropriate treatment for each case, and avoid maintenance of mouth breathing patterns during adulthood.

Breathing mode influence on craniofacial development and head posture

Abstract:Objective: The incidence of abnormal breathing and its consequences on craniofacial development is increasing, and is not limited to children with adenoid faces. The objective of this study was to evaluate the cephalometric differences in craniofacial structures and head posture between nasal breathing and oral breathing children and teenagers with a normal facial growth pattern. Method: Ninety-eight 7-16 year-old patients with a normal facial growth pattern were clinically and radiographically evaluated. They were classified as either nasal breathing or oral breathing patients according to the predominant mode of breathing through clinical and historical evaluation, and breathing respiratory rate predomination as quantified by an airflow sensor. They were divided in two age groups (G1: 7-9) (G2: 10-16) to account for normal age-related facial growth. Results: Oral breathing children (8.0±0.7 years) showed less nasopharyngeal cross-sectional dimension (MPP) (p=0.030), whereas other structures were similar to their nasal breathing counterparts (7.6±0.9 years). However, oral breathing teenagers (12.3±2.0 years) exhibited a greater palate length (ANS-PNS) (p=0.049), a higher vertical dimension in the lower anterior face (Xi-ANS-Pm) (p=0.015), and a lower position of the hyoid bone with respect to the mandibular plane (H-MP) (p=0.017) than their nasal breathing counterparts (12.5±1.9 years). No statistically significant differences were found in head posture. Conclusion: Even in individuals with a normal facial growth pattern, when compared with nasal breathing individuals, oral breathing children present differences in airway dimensions. Among adolescents, these dissimilarities include structures in the facial development and hyoid bone position.

Abstract:Objective: To assess the association between self-reported symptoms of pediatric OSA and clinical signs in MB children. Methods: Seventy-three MB children aged 7-14 years answered an interview questionnaire on OSA symptoms in childhood, focusing on chewing, nasal, and sleep disturbances. MB children were checked for changes on the craniofacial, occlusion, TMJ, upper airway, and body posture by a multi-disciplinary team, consisting of medical residents, dental, and psychology postgraduate students. Multiple logistic regression analysis verified the association between clinical signs and self-reported symptoms. Results: Reported symptoms of chewing disturbance as TMD noise, muscle pain, and morning headache were significantly associated with the presence of lip incompetence and Class II malocclusion. Sleep disturbances as snoring, waking up at night, daytime sleepiness, and sleeping with the mouth open were significantly associated with the presence of hypertrophic tonsils and obstructive Mallampati score. Conclusion: Symptoms of pediatric OSA were found in mouth-breathing children, which should be carefully investigated in order to prevent OSA in the future.

Subjects and Methods: A structured questionnaire consisting of 10 questions was framed. Out of 110 children, 30 parents who responded to a questionnaire-based study were included after obtaining their consent. After obtaining their responses, all the questions were evaluated to understand the parental responses to associate their knowledge about SDB of their children with the presence of MB in their children. Chi-square test was used as the study consisted of qualitative questions. Results: Responses in order of chronological sequence received from parents for their children were as follows: child feels thirsty (25 [83.33%]), strange position of child during sleep (24 [80%]), child struggles to breathe (16 [53.33%]), child feels dry mouth on waking (14 [46.66%]), child has upper respiratory tract infections (6 [20%]), child unusually sleepy during day time (5 [16.66%]), and child appears to be more hyperactive (5 [16.66%]). Other less common responses were related to restless sleep of child and snoring. Conclusions: Parental responses for its association with MB in children are important to help us diagnose early symptoms; further, it can be correlated with clinical predictors to confirm the MB habit in their children.

Abstract:AIMS AND OBJECTIVES: Persistent mouth breathing affects stomatognathic functions along with effects on the academics and social life of a child. Sleep-related problems and behavioral symptoms similar to that found in attention-deficit hyperactivity disorder (ADHD) can be present in mouth breathers. This study aims at assessing the sleep disturbances and pattern of symptoms of ADHD in children with mouth breathing.MATERIALS AND METHODS: A cross-sectional study was carried out on 100 children of mouth breathing (consecutively selected) in 7-12 years of age using semi-structured proforma, children's sleep habit questionnaire (CSHQ), and diagnostic and statistical manual of mental disorders, version 5 (DSM 5). Statistical analysis was done using SPSS software version 21. Mean, standard deviation, Chi-square, and Pearson's correlation coefficient test were utilized during the analysis. <i>p</i> value of <0.05 was considered significant.RESULTS: Out of 100 children, 70 were males and 30 were females. On the CSHQ scale, the highest score was found on sleep-disordered breathing followed by sleep onset delay and daytime sleepiness. The commonest symptom of inattention was "failing to give close attention in school" (73%) whereas the commonest symptom of hyperactivity was "trouble waiting for his/her turn" (66%). Seven participants satisfied complete criteria for ADHD. Sleep duration and daytime sleepiness had a significant negative correlation with hyperactivity (<i>p</i> < 0.05). A positive correlation was observed between daytime sleepiness and inattention (<i>p</i> < 0.01).CONCLUSION: Children with sleep disturbances or ADHD should be assessed for the presence of mouth breathing, as early identification and correction of mouth breathing may help to prevent unnecessary exposure to the medication.CLINICAL SIGNIFICANCE: Children with ADHD or sleep disturbances should always be assessed for the presence of mouth breathing. Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Mouth breathing syndrome (MBS) is defined as a set of signs and symptoms that may be completely or incompletely present in subjects who, for various reasons, replace the correct pattern of nasal breathing with an oral or mixed pattern. Better understanding of the characteristics of MBS at each stage of growth will be helpful for the accurate diagnosis and treatment of MBS.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objective</h3><p>This review aimed to clarify the relevant factors affecting MBS in children and to examine the influence of an incompetent lip seal (ILS) on facial soft tissue form in preschool-aged children.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Recent data have shown that an ILS affects MBS in children. In addition, the factors associated with MBS in preschool-aged children were not mutually unrelated, and multiple factors such as general conditions, lifestyle, and eating habits may interact with one another. Furthermore, children with an ILS already tended to have a flattened nose and an anteriorly prominent lip at 3 years of age, implying that the adverse effects of an ILS on the growth of the craniofacial region appear relatively early.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>These results suggest the importance of early diagnosis and treatment of MBS. Early treatment of MBS may prevent possible dental and physical problems in the future.</p>
Oropharyngeal obstruction and respiratory system compliance are linked to ventilatory control parameters in pediatric obstructive sleep apnea syndrome

Abstract:Instable ventilatory control is an endotypic trait of obstructive sleep apnea syndrome (OSAS). This study aimed to evaluate the relationships between the anatomical compromise of the upper (oro- and naso-pharynx) and lower airways and ventilatory control (measured by chemical loop gain) in otherwise healthy children suffering from moderate to severe OSAS (apnea hypopnea index ≥ 5/hour). The children underwent ear, nose and throat examination, measurement of impedance of the respiratory system that allowed characterizing peripheral lung mechanics using the extended Resistance-Inertance-Compliance model. Physiologically constrained analytical model based on tidal breathing analysis allowed for the computation of steady-state plant gain, steady-state controller gain (CG0) and steady-state loop gain (LG0). Medium-frequency components of the feedback control system were then deduced. Fifty children (median age 11.2 years) were enrolled. Oropharyngeal obstruction was associated with decreased CG0 (0.6 [0.2; 1.0] vs 1.5 [0.5; 6.6] L.s- 1.mmHg- 1, p = 0.038) and LG0 (0.4 [0.2; 1.1] vs 1.2 [0.4; 9.3], p = 0.027), while nasal obstruction did not modify ventilatory control parameters. In a multivariate analysis Medium-Frequency PG was negatively related to minute ventilation and respiratory system compliance. Both upper (tonsil hypertrophy) and lower (compliance of respiratory system) airways are linked to ventilatory control in children with moderate to severe OSAS.

Abstract:After more than a century of conjecture and heated argument, the orthodontic relevance of nasal obstruction and its assumed effect on facial growth continues to be debated. Oral respiration disrupts those muscle forces exerted by tongue, cheeks and lips upon the maxillary arch. The main characteristics of the respiratory obstruction syndrome are presence of hypertrophied tonsils or adenoids, mouth breathing, open-bite, cross-bite, excessive anterior face height, incompetent lip posture, excessive appearance of maxillary anterior teeth, narrow external nares, "V" shaped maxillary arch. The purpose of this study is to evaluate relationship between nasal obstruction and severity of malocclusion. The sample analyzed in this article consisted of 49 children aged from 7 to 15 years, who pronounced difficulty in breathing through the nose. Patients and their parents were interviewed, clinical examination was performed, and measurements from dental casts and panoramic radiograph were obtained. All patients were examined by otorhinolaryngologist, and the nasal obstruction was confirmed by posterior rhinomanometry test. This study showed the significant association between nasal resistance and increased overjet (p = 0.042), open bite (p = 0.033) and maxillary crowding (p = 0.037). The tendency of greater nasal resistance was observed for the patients with the first permanent molars relationship Angle II and posterior cross-bite.

Abstract:(1) Background: Sleep-disordered breathing represents a growing public health concern, especially among children and adolescents. The main risk factors for pediatric sleep-disordered breathing in school-age children are tonsillar and adenoid hypertrophy. Adenoidectomy, often in combination with tonsillectomy, is the primary treatment modality for pediatric sleep-disordered breathing. This study aims to comprehensively investigate various risk and protective factors in children with sleep-disordered breathing undergoing adenotonsillar or adenoidal surgeries. We also aim to explore the differences in neuropsychological profiles. (2) Methods: This is an observational, retrospective cohort study. We collected information on adenoidectomy or adenotonsillectomy in children referred to our center. We reviewed the clinical history and preoperative visits and collected data through a telephone questionnaire. The Pediatric Sleep Questionnaire (PSQ) and the Pediatric Quality of Life Inventory (PedsQL) screen sleep-disordered breathing and quality of life, respectively. The data were statistically analyzed using SPSS version 22.0 for Windows (SPSS Inc., Chicago, IL, USA). (3) Results: The study involved 138 patients, but only 100 children participated. A higher percentage of patients with sleep-disordered breathing were observed to have mothers who smoked during pregnancy. A smaller proportion of patients with sleep-disordered breathing habitually used a pacifier. A rise in physical score was associated with a reduced PSQ at follow-up (p = 0.051). An increase in the overall academic score was related to a decrease in the PSQ at follow-up (p < 0.001). A more significant proportion of patients undergoing adenotonsillectomy were observed to have a history of prematurity and cesarean birth. (4) This comprehensive study delves into the intricate interplay of risk and protective factors impacting children with sleep-disordered breathing undergoing adenotonsillectomy and adenoidectomy.

Mouth breathing: adverse effects on facial growth, health, academics, and behavior

Abstract:The vast majority of health care professionals are unaware of the negative impact of upper airway obstruction (mouth breathing) on normal facial growth and physiologic health. Children whose mouth breathing is untreated may develop long, narrow faces, narrow mouths, high palatal vaults, dental malocclusion, gummy smiles, and many other unattractive facial features, such as skeletal Class II or Class III facial profiles. These children do not sleep well at night due to obstructed airways; this lack of sleep can adversely affect their growth and academic performance. Many of these children are misdiagnosed with attention deficit disorder (ADD) and hyperactivity. It is important for the entire health care community (including general and pediatric dentists) to screen and diagnose for mouth breathing in adults and in children as young as 5 years of age. If mouth breathing is treated early, its negative effect on facial and dental development and the medical and social problems associated with it can be reduced or averted.

Abstract:Although humans breathe naturally through the nostrils, mouth breathing in children has recently gathered attention. In this study, we hypothesized that tongue function and its related maxillofacial morphology would affect breathing in adolescence. To verify this hypothesis, we examined the association between breathing patterns, including mouth and nasal breathing; oral functions, including tongue motor function; and craniofacial morphology during adolescence, which has not been investigated till date. C3-H, which indicates the anteroposterior position of the hyoid bone in relation to the third cervical vertebra, was significantly smaller in mouth-breathers than in nasal-breathers. Lip-closing force, tongue pressure, and masticatory efficiency were lower in the order of nasal-breathers, oronasal-breathers, and mouth-breathers, and the values for mouth-breathers were significantly lower than those for nasal-breathers. Tongue pressure alone was identified as a significant independent variable, with an odds ratio of 1.063 (95% confidence interval, 1.006–1.123; p < 0.05). Our results indicate a relationship between mouth breathing and the lip-closing force, tongue pressure, and masticatory efficiency, as well as the significance of tongue pressure on mouth breathing in adolescents. The findings highlight the importance of clarifying the pathophysiology of mouth breathing and its underlying causes.

The influence of body mass index on airway resistance in children with sickle cell disease: A longitudinal study based on impulse oscillometry

Abstract:BACKGROUND: Impulse oscillometry (IOS) is an effective tool for assessing airway mechanics and diagnosing obstructive airway disease (OAD) in children with sickle cell disease (C-SCD). Obesity is known to be associated with OAD, and untreated OAD often leads to hypoxia-related complications in C-SCD. Considering the increasing prevalence of obesity in C-SCD, it is important to explore the influence of body mass index (BMI) on OAD in this disease population.METHODS: A longitudinal retrospective chart review was conducted on 55 C-SCD (161 IOS observations) and 35 non-SCD asthmatic children (C-Asthma) (58 observations), primarily to investigate the association between BMI and airway resistance in C-SCD and C-Asthma. We conducted generalized linear mixed models (GLMM), adjusted for pharmacotherapies, to demonstrate the influence of BMI on total (R<sub>5</sub>), central (R<sub>20</sub>), and peripheral (R<sub>5-20</sub>) airway resistance and reactance (X<sub>5</sub>, resonant frequency (Fres)). We further compared age, BMI, and IOS indices between C-SCD and C-Asthma using the Mann-Whitney test.RESULTS: Age and BMI were not statistically different between the two groups. In C-SCD, BMI was associated with R<sub>5</sub> (GLMM t-statistics:3.75, 95%CI:1.01,3.27, p-value<0.001*) and R<sub>20</sub> (t-statistics:4.01, 95%CI:1.04,1.15, p-value<0.001*), but not with R<sub>5-20</sub> or airway reactance. In asthmatics, BMI was not associated with IOS estimates except Fres (t-statistics: 3.93, 95%CI: -0.06, -0.02, p-value<0.001*). C-SCD demonstrated higher airway resistances (R<sub>5</sub> and R<sub>20</sub>) and reactance (Fres) compared to C-Asthma (Mann-Whitney: p-values<0.05).CONCLUSION: BMI significantly influenced total and central airway resistance in C-SCD. While higher airway resistances reflected increased OAD in C-SCD than asthmatics, higher Fres perhaps indicated progressive pulmonary involvement in C-SCD.

Abstract:Background: The relationship between malocclusion and respiration has been debated for decades. Aim: The aim of this study is to assess dental occlusion in relation to mouth breathing and different types of pharyngeal lymphoid tissue obstruction in children. Settings and Design: This was a cross-sectional study of 200 children aged between 6 and 12 years who were clinically examined and divided into two groups: mouth breathers and nasal breathers. Materials and Methods: The children were subjected to otorhinolaryngologic examination to identify the type of pharyngeal obstruction. Dental interarch relationship and pharyngeal tissue obstruction were diagnosed and appropriate cross tabulations were done. Statistical Analysis Used: The data collected were statistically analyzed using the SPSS version 15.0 software. Results: Statistically significant association was found between type of breathing and pharyngeal lymphoid tissue obstruction ( P = 0.001), dental occlusion and type of breathing ( P = 0.001), and pharyngeal lymphoid tissue obstruction and dental occlusion ( P = 0.001). Higher prevalence of crossbite, deep bite, and Class II malocclusion was seen in children with adenotonsillar hypertrophy as well as mouth breathing habit. Conclusion: Obstructive tonsils and adenoids are risk factors for the development of malocclusion. Early detection and correction of airway obstruction can help in proper growth of the dentofacial region.

Abstract:Abstract Objective This retrospective study analyzed breathing patterns and age subgroups effect on cortical bone quality of the mandible in growing subjects, aiming to explore the application value of facial skeletal pattern combined with cortical bone density detection in early screening diagnosis of mouth breathing. Methods One hundred twenty-six participants were divided into four groups: mouth breathing group (7–9, 10–12 years old) and nasal breathing group (7–9, 10–12 years old). The mandibular anterior, middle, and posterior cortical bone mineral density (CBMD), cortical bone width (MCW), ANB, and FMA values were measured. Independent T-test and Mann–Whitney U test were used to compare the measured values. Binary logistic regression was employed to analyze the correlation between measured variables and the children’s breathing patterns. ROC analysis was used to determine the ability of the cortical bone density measurements in early screening diagnosis of MB. Results Mouth breathing had a negative impact on CBMD and MCW of the pre-mandibular (Pog) in subjects aged 7–9 years and also impacted the development of (Pog) and submandibular (Me) sites in subjects aged 10–12 years. Older children in the nasal breathing group have higher CBMD, MCW, and SNB values and lower FMA values. Single-factor and multiple-factor logistic binary regression analysis showed that FMA, MSPogCBMD, MSPogMCW, and ANB are correlated factors for children at risk of mouth breathing. Conclusion Mouth breathing pattern is closely associated with decreased mandibular CBMD and MCW values in children aged 7–12, where the anterior (Pog) and inferior (Me) sites of anterior mandible are more significantly affected. Furthermore, in combination with facial skeletal pattern, it provides a basis for the early warning diagnosis of mouth breathing.

Preschool Impulse Oscillometry Predicts Active Asthma and Impaired Lung Function at School Age

Abstract:Background Asthmatic symptoms often start during early childhood. Impulse oscillometry (IOS) is feasible in preschool children who may be unable to reliably perform spirometry measurements. Objective We evaluated the utility of IOS in a multicenter multiethnic high-risk asthma cohort entitled the Vitamin D Antenatal Asthma Reduction Trial (VDAART). Methods The VDAART recruited pregnant women whose children were followed from birth to age 8 years. Lung function was assessed with IOS at ages 4, 5, and 6 years and spirometry at ages 5, 6, 7, and 8 years. Asthma status, respiratory symptoms, and medication use were assessed with repeated questionnaires from birth to age 8 years. Results In total 220 children were included in this secondary analysis. Recent respiratory symptoms and short-acting β2-agonist (SABA) use were associated with increased respiratory resistance at 5 Hz (R5) at age 4 years (β 2.6; 95% CI 1.0, 4.4; P=0.002 and β 3.4; 95% CI 0.7, 6.2; P=0.015, respectively). Increased R5 at age 4 years was also associated with decreased lung function from ages 5 to 8 years (β -0.3; 95% CI -0.5, -0.1; P<0.001 for FEV 1 at 8 years) and active asthma at age 8 years (β 2.0; 95% CI 0.2, 3.8; P=0.029). Conclusion Increased respiratory resistance in preschool IOS is associated with frequent respiratory symptoms as well as school-age asthma and lung function impairment. Our findings suggest that IOS may serve as a potential objective measure for early identification of children who are at high risk of respiratory morbidity.
