Abstract:Background: Nd: YAG laser posterior capsulotomy is a commonly performed procedure to treat posterior capsular opacification following cataract surgery. While the procedure is known to effectively improve visual acuity, its potential impact on intraocular pressure (IOP), refraction, anterior chamber depth (ACD), and macular thickness remains a subject of debate. This study aims to investigate the effects of Nd: YAG laser posterior capsulotomy on these ocular parameters. Methods: This study was designed as a prospective analysis conducted at a tertiary eye care center on patients who underwent Nd: YAG laser posterior capsulotomy at a tertiary eye care center between January 2020 and December 2020. Preoperative and postoperative measurements of IOP, refraction, ACD, and macular thickness were collected and compared. Statistical analyses were performed using paired t-tests, and p-values <0.05 were considered statistically significant. Results: A total of 30 eyes from 25 patients (mean age, 65.4 ± 8.2 years) were included in the analysis. Following Nd: YAG laser posterior capsulotomy, the mean intraocular pressure (IOP) increased from 15.2 ± 2.3 mmHg to 17.0 ± 2.7 mmHg (p < 0.001). The refractive status showed a mean myopic shift of -0.35 ± 0.16 diopters (p < 0.001,). Additionally, there was a statistically significant reduction in anterior chamber depth (ACD) from 3.15 ± 0.42 mm to 2.90 ± 0.37 mm (p = 0.003,). However, there was no significant change in macular thickness after the procedure (p = 0.287). Conclusion: Nd: YAG laser posterior capsulotomy leads to a mild but statistically significant increase in IOP and a small myopic shift in refraction. Additionally, the procedure results in a decrease in ACD but does not significantly affect macular thickness. Ophthalmologists should consider these effects when performing Nd: YAG laser posterior capsulotomy and carefully monitor patients postoperatively.

Corneal Endothelial Changes Following Early Capsulotomy Using Neodymium:Yttrium–Aluminum–Garnet Laser

Abstract:We aimed to survey whether the timing of neodymium:yttrium–aluminum–garnet (Nd:YAG) laser capsulotomy would alter the corneal endothelial morphology and density. A retrospective cohort study was conducted, and 48 patients with unilateral posterior capsular opacity (PCO) and Nd:YAG laser capsulotomy performance were enrolled. The participants were divided into the early Nd:YAG group (timing ≤ 12 months, n = 20) and late Nd:YAG group (timing > 12 months, n= 28) depending on elapsed months between phacoemulsification and Nd:YAG laser capsulotomy. Endothelial cell density (ECD), coefficient of variant (CV), hexagonality (HEX), and central corneal thickness (CCT) between the two groups were collected. A generalized estimate equation was conducted to evaluate the corneal endothelial parameters between the two groups with an adjusted odds ratio (aOR) and 95% confidence interval (CI). The CDVA was improved after treatment in both groups (both p < 0.001). Chronically, ECD in the early group was significantly decreased one week after treatment (2221.50 ± 327.73/mm2 vs. 2441.55 ± 321.80/mm2, p < 0.001), which recovered to 2369.95 ± 76.37/mm2 four weeks after the treatment but was still lower than the preoperative status (p < 0.001). In addition, the HEX percentage showed a significant reduction at four weeks after treatment (p = 0.028). The ECD in the early group was significantly lower than that in the late group (aOR: 0.167, 95% CI: 0.079–0.356, p = 0.003) in both week 1 (p < 0.001) and week 4 (p = 0.004) after laser treatment. In conclusion, the early application of Nd:YAG laser capsulotomy within one year after cataract surgery may be the reason for postoperative ECD decrement without known etiology.

Patients and Methods: A total of 50 eyes from 50 consecutive patients were included. All patients had visually significant posterior capsular opacification (PCO) after uneventful phacoemulsification surgery with manual capsulorhexis and single-piece C-loop acrylic IOL implantation (AcrySof ® SA60AT) and were treated with Nd:YAG laser. Anterior segment images were captured with Anterion ® in non-dilated conditions before and one month after the procedure. In the "Metrics App", we collected data of the anterior chamber angle (ACA) 3 and 9 o'clock, the anterior chamber depth (ACD), the anterior chamber volume (ACV), and the central corneal thickness (CCT). We also collected demographic and clinical data [age, gender, months from surgery to Nd: YAG capsulotomy, pre- and post-capsulotomy corrected distance visual acuity (CDVA) and spherical equivalent, and axial length]. Results: We did not find a statistically significant difference in ACD after the Nd:YAG capsulotomy (3.96 ± 0.55 vs 3.97 ± 0.55 mm, p = 0.10). In 28 patients, we noted a backward movement of the IOL; in the remaining 22, the IOL moved forward or did not change at all. We did not observe significant changes in ACA at the 3 and 9 o'clock reference points, ACV, or CCT. There was an improvement in CDVA after the procedure (0.37 ± 0.21 vs 0.12 ± 0.27 logMAR, p = 0.015), without statistically significant differences in SE (? 0.15 ± 0.84 vs ? 0.25 ± 0,93, p = 0.42). Axial length was not correlated with ACD variations after the procedure (p = 0.67). Conclusion: Our results show that Nd:YAG laser capsulotomy does not change the axial position of a single-piece C-loop acrylic IOL inside the capsular bag, as well as other anterior chamber parameters. Previous studies have addressed the change in IOL position and anterior chamber parameters following Nd:YAG treatment, with mixed results. 7–12 However, most of these studies present relevant limitations. Some studies included a mixed group of patients submitted to phacoemulsification surgery or extracapsular cataract extraction, while others included eyes that had been implanted with different IOLs. In this study, we evaluated the effective lens position and anterior chamber parameters before and after Nd:YAG capsulotomy in patients with PCO after uneventful phacoemulsification cataract surgery and single-piece C-loop IOL implantation. All parameters were obtained with a novel high-resolution swept-source anterior segment optical coherence tomography (AS-OCT) device (Anterion ® , Heidelberg Engineering GmbH, Heidelberg, Germany). Abstract:We compute the next-to-next-to-leading order QCD corrections to the partonic reaction that dominates top-pair production at the Tevatron. This is the first ever next-to-next-to-leading order calculation of an observable with more than two colored partons and/or massive fermions at hadron colliders. Augmenting our fixed order calculation with soft-gluon resummation through next-to-next-to-leading logarithmic accuracy, we observe that the predicted total inclusive cross section exhibits a very small perturbative uncertainty, estimated at ±2.7%. We expect that once all subdominant partonic reactions are accounted for, and work in this direction is ongoing, the perturbative theoretical uncertainty for this observable could drop below ±2%. Our calculation demonstrates the power of our computational approach and proves it can be successfully applied to all processes at hadron colliders for which high-precision analyses are needed.
Long-term incidence of posterior capsular opacification in patients with non-infectious uveitis

Abstract:Abstract Little is known about the long-term incidence of posterior capsule opacification (PCO) after cataract surgery in patients with uveitis. This retrospective study included 211 eyes of 146 patients with non-infectious uveitis who underwent cataract surgery and implantation of an Acrysof SN60WF (Surface: plasma-treated, Optic and Haptic: hydrophobic acrylic), iSert XY-1 (Surface: UV-ozone-treated, Optic and Haptic: hydrophobic acrylic), or iSert 251/255 (Surface: UV-ozone-treated, Optics: hydrophobic acrylic, Haptic: polymethyl methacrylate). The cumulative incidences of PCO and subsequent yttrium–aluminum-garnet (Nd:YAG) capsulotomy over the 5-year follow-up were analyzed, and patients who were implanted with different intraocular lenses (IOLs) were compared. Mixed-effects Cox proportional hazard models showed that, compared with the Acrysof group, the iSert XY-1 group had higher risks of PCO (adjusted HR, 7.26; 95% CI, 1.82–28.8) and Nd:YAG capsulotomy (adjusted HR, 6.50; 95% CI, 1.55–27.2). Similar results were obtained when the Acrysof group was compared with the iSert 251/255 group for PCO (adjusted HR, 8.22; 95% CI, 2.35–28.7) and Nd:YAG capsulotomy (adjusted HR, 8.26; 1.90–36.0). These data suggest that a plasma-treated surface, hydrophobic acrylic optic and hydrophobic acrylic haptic, of the IOL could enhance biocompatibility even under inflammatory conditions, thus suppressing PCO development.

Abstract:PurposeΤo investigate whether Yag laser capsulotomy, in addition to restoring ocular media transparency, may also significantly alter retinal nerve fiber layer (RNFL) thickness analysis and spectral domain optical coherence tomography (SD OCT) indices and quality parameters, such as signal strength (SS), in patients with mild, diffuse posterior capsular opacification (PCO).MethodsProspective interventional case series study which enrolled 28 patients with mild PCO and good baseline best-corrected visual acuity (BCVA). All patients underwent a complete ophthalmologic examination, including SD OCT RNFL thickness measurement prior to Yag laser capsulotomy. Following laser intervention, all patients repeated the same complete ophthalmic examination and results were recorded and analysed.ResultsMean RNFL thickness showed a significant postoperative increase from 89.6 to 94.1 μm (p = 0.01), which was also confirmed for superior (p = 0.041), nasal (p = 0.011) and inferior quadrants (p = 0.032). SS and BCVA were also significantly increased after laser treatment (p < 0.01). A significant positive correlation was observed between SS and mean RNFL thickness postoperatively (p = 0.043) but not preoperatively (p = 0.059). Similarly, a significant relationship was found between BCVA and average RNFL thickness after (p = 0.017) but not before (p = 0.457) laser intervention.ConclusionsOur study suggests that the presence of even mild PCO may alter the sensitivity of SD-OCT acquisition and lead to inadvertent RNFL thickness underestimation. Clinicians should be alerted to this contingency and take it into consideration when evaluating various optic neuropathies with SD OCT, in pseudophakic eyes.

Abstract:Purpose To assess the risk for pseudophakic cystoid macular oedema (PCME) and posterior capsular opacification (PCO) associated with combined cataract surgery and trabeculectomy compared to cataract surgery alone. Methods Data analysis of subjects who underwent routine cataract surgery without and with concomitant trabeculectomy at the Department of Ophthalmology, Bristol Eye Hospital, the UK, between January 2008 and December 2017. Odds ratios (ORs) for PCME between the types of surgeries were calculated using univariate and multivariate regression analysis. Multivariate Cox regression controlling for age and gender was used to estimate the hazard ratio (HR) for neodymium‐doped yttrium aluminium garnet (Nd:YAG) laser capsulotomies. Results This study included 56 973 cataract surgeries without and 288 with concomitant trabeculectomy (phaco‐trab) with a mean follow‐up time of 6.9 ± 4.2 years. Baseline variables (age and gender, diabetes, pseudoexfoliation, use of pupil expansion device and postoperative follow‐up time) were comparable between the groups. Postoperative rates of PCME remained non‐significant between the cataract surgery and phaco‐trabe groups both in uni‐ and multi‐variate analysis (OR 0.347, 95%CI 0.049–2.477, p = 0.291). Furthermore, in Cox regression analysis adjusted for the patients' age and gender, Nd:YAG laser capsulotomy rates remained non‐significant between the cataract surgery and phaco‐trabe groups (HR 1.250, 95%CI 0.883–1.769, p = 0.209). Conclusions In our large cohort study, combining trabeculectomy with cataract surgery did not predispose to an increased PCME or Nd:YAG laser capsulotomy rates.

Abstract:ABSTRACT A human physical asymmetry is the near 90% clockwise occipitoparietal scalp hair-whorl direction in Europeans, an incidence that approximates the left lateralization of speech and right-handedness. Hair-whorl direction is also asymmetric in horses, Equus ferus caballus and placement is proposed to be related to temperament and lateralized skill in equitation manoeuvres. We describe a hair-whorl asymmetry in the horse, mane direction. Of 526, 3-year-old American Quarter horses, 69% of horses had mane directed to the right and 31% had mane directed to the left. The bias was larger in females, with 74% of females vs. 65% of males having mane directed to the right. Mane direction was unrelated to coat colour. The behavioural significance of mane asymmetry was investigated using judges’ scores from a reining competition requiring symmetrical maneuvers of spin, circle and roll-back to either the left or to the right. There was no relation between mane asymmetry and overall reining performance and no relation between mane direction and scores for left or right manoeuvres. The results are discussed in relation to the significance of morphological asymmetries, neural function and the influence of planar cell polarity genes, such as Frizzled, that influence epidermal hair cell patterning.
Long-Term Outcomes of Posterior Capsular Opacification in Highly Myopic Eyes and Its Influencing Factors

Abstract:Introduction: The purpose of this study was to evaluate the long-term outcomes of posterior capsular opacification (PCO) in highly myopic eyes and its influencing factors. Methods: Patients undergoing phacoemulsification with intraocular lens implantation and followed up for 1-5 years were included in this prospective cohort study. The severity of PCO was evaluated using EPCO2000 software system, with the area of central 3.0 mm (PCO-3 mm) and within the capsulorhexis (PCO-C) both being analyzed. Percentage of eyes after Nd:YAG capsulotomy, as well as clinically significant PCO (defined as eyes with visual-impairing PCO or after capsulotomy), were also included as outcome variables. Results: A total of 673 highly myopic eyes [axial length (AL) ≥ 26 mm] and 224 control eyes (AL < 26 mm) were analyzed. The mean follow-up time was 34.0 ± 9.0 months. PCO was more severe in highly myopic eyes compared with controls with regard to higher EPCO scores (P < 0.001 for both PCO-3 mm and PCO-C), higher capsulotomy rate (P = 0.001), higher clinically significant PCO rate (P < 0.001) and shorter PCO-free survival time (P < 0.001). Extreme myopia (AL ≥ 28 mm) would further aggravate PCO in terms of higher EPCO scores (PCO-3 mm: P = 0.017; PCO-C: P = 0.013) and higher clinically significant PCO rate (P = 0.024) compared with other myopic eyes. In highly myopic eyes, AL [odds ratio (OR) 1.124, P = 0.004] and follow-up duration (OR 1.082, P < 0.001) were independent risk factors for clinically significant PCO after cataract surgery. Conclusion: Highly myopic eyes had more severe PCO in the long term. Longer AL and follow-up duration were associated with higher risk of PCO. Influence of the “Inverted U Method” Nd: YAG Laser Posterior Capsulotomy on Anterior Segment Parameters, Decentration and Tilt of Intraocular Lens in Patients after Phaco-vitrectomy

Abstract:To investigate the effects of the "inverted U method" Nd:YAG laser posterior capsulotomy on anterior segment parameters, decentration and tilt of intraocular lens (IOLs) and visual acuity in posterior capsular opacification (PCO) patients after combined phaco-vitrectomy.Seventy-six patients (76 eyes) were enrolled in this study, who were diagnosed as PCO and underwent "inverted U method" Nd:YAG laser posterior capsulotomy. All patients had undergone previous combined phaco-vitrectomy for rhegmatogenous retinal detachment. The parameters including IOL decentration, angle of IOL tilt, anterior chamber angle (ACA), anterior chamber depth (ACD), anterior chamber volume (ACV), central corneal thickness (CCT), keratometry, pupil size, axial length (AL), intraocular pressure (IOP), refractive status, corrected distance visual acuity (CDVA), objective scattering index (OSI) and Chinese version of Visual Function 14 (VF-14) index were obtained before and 3 months after capsulotomy.After capsulotomy, the tilt angle and decentration of the IOLs at both vertical (tilt: p = .004, decentration: p = .029) and horizontal meridian (tilt: p = .001, decentration: p = .017) were significantly decreased, ACA increased (p = .015), CDVA (p = .000), VF-14 score (p = .000) and OSI (p = .000) were significantly improved. There were a significant decrease in cylindrical error (p = .001) and a myopic shift in spherical error (p = .001) after the capsulotomy. No significant differences in ACD, ACV, CCT, keratometry, pupil size, AL and IOP were detected (p > .05 for all).The "inverted U method" Nd: YAG laser posterior capsulotomy decreases tilt and decentration of the IOL, increases ACA and causes no change in ACD, ACV, CCT and IOP in patients after phaco-vitrectomy. These changes make a decrease in cylindrical error and a myopic shift in spherical error. Laser capsulotomy significantly improves visual acuity.

This study aimed to identify potential risk factors, known at the time of cataract surgery, that influence the development of PCO. Subject/methods: A retrospective study of publicly funded cataract surgery from The Royal College of Ophthalmologists' National Ophthalmology Database. Eligible for analysis were 500,872 cataract operations performed in 41 participating centres. Results: The 500,872 operations were performed on 243,167 (48.5%) left eyes and 257,705 (51.5%) right eyes from 373,579 patients by 2196 surgeons. Post-cataract PCO was recorded for 61,778 (12.3%) eyes and the six month, one, three, five and nine year observed rates of PCO were 2.3%, 4.4%, 19.7%, 34.0% and 46.9% respectively. Different PCO profiles were observed between IOL materials and the identified risk factors that increased the risk of developing PCO included hydrophilic IOL material, axial length >26 mm, the presence of high myopia and implantation of lower IOL powers and previous vitrectomy surgery, along with younger age and female gender. Conclusions: Many factors influence the development of PCO relating to the patient, the eye, the lens and the surgery. Some factors are modifiable such as IOL material, therefore the opportunity exists to attempt to reduce PCO rates, benefitting patients and the UK NHS.
Systemic immune-inflammation index could be associated with pseudophakic cystoid macular edema after an uneventful phacoemulsification surgery in patients without risk factors

Abstract:Background: To evaluate the association between serum biomarkers and pseudophakic cystoid macular edema (PCME) in eyes without risk factors after uneventful phacoemulsification cataract surgery. Methods: This is a case-control study. Patients without risk factors and who developed clinically significant PCME after uncomplicated phacoemulsification surgery were enrolled in the study. The age- and sex-matched control group that had normal fundus examination findings and 10/10 visual acuity in the first week, first month and following postoperative control visits was randomly recruited from the same study cohort. The neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and systemic immune-inflammation index (SII) were obtained from the preoperative complete blood count (CBC) test and compared between the two groups. Linear regression analysis was used to assess the relationship between central macular thickness (CMT) and biomarkers. A binary logistic regression model was generated to evaluate the significance of the biomarkers in predicting PCME. The receiver operating characteristic (ROC) curves of the significant parameters in the logistic regression model were presented to detect the area under the curve (AUC), the cut-off point, the sensitivity and the specificity. Results: The study cohort included 5352 patients. Of these patients, 52 (0.97%) met the inclusion criteria, and 60 age- and sex-matched patients were recruited as the control group. PLR, NLR, and SII were significantly different between the two groups (p = 0.006, p = 0.002, p < 0.001, respectively). According to the linear regression analysis, SII was found to have a significant relationship with CMT (p < 0.001). Only SII was assessed as significant in the logistic regression model (p = 0.046). In the ROC curve, the AUC of SII was 0.709. The sensitivity and specificity of SII for PCME prediction were 65.38% and 75%, respectively, and the cut-off point was 433.70. Conclusion: SII is associated with the occurrence of PCME in eyes without risk factors after uneventful phacoemulsification surgery. SII could be a useful tool to predict PCME in eyes without risk factors.

Abstract:Objective To investigate the effective factors on posterior capsular opacification(PCO) and long-term visual impairment after cataract surgery.Methods Forty-seven eyes(37 patients) with age-related cataract who had received phacoemulsification and intraocular lens implantation were enrolled in this study.The patients’ surgical-related factors such as posterior capsule polishing were documented,and visual acuity,anterior and posterior segment of the eyes were examined before and after the surgery.Degrees of PCO were evaluated under dilated pupil at two years after surgery.Results PCO was observed in 25 eyes of the 47 eyes at two years after surgery,in which PCO involving central 3 mm area was found in 18 eyes,4 eyes underwent Nd:YAG laser posterior capsulotomy,the posterior capsulotomy rate was 8.5%.Statistics showed that there were statistical differences in the incidence of PCO and PCO involving central 3 mm area between posterior capsule polishing group and none-polishing group(both P<0.05).Posterior capsule polishing was found to be the only protective factor among various factors for PCO and PCO involving central 3 mm area.Analysis of the influencing factors on the visual impairment at two years after surgery showed that there were statistically significant associations between PCO involving central 3 mm area,ocular fundus disease,macular disease and post-operative visual impairment(all P<0.05).Conclusion PCO involving central 3 mm area is the main factor that is responsible for the long-term visual impairment after cataract surgery.Posterior capsule polishing significantly decrease the incidence and severity of PCO.

Abstract:PurposeTo describe cytokines, chemokines and growth factors profiles in patients undergoing cataract surgery with femtosecond laser pretreatment and investigate their relationships with the postoperative in vivo inflammation index.MethodsAqueous humor was collected from 22 eyes after femtosecond laser pretreatment and from 22 eyes at the beginning of routine cataract surgery. The levels of 45 inflammation-related mediators were measured using multiplex fluorescent bead-based immunoassays. Laser flare photometry was measured preoperatively and at 1 day, 7 days and 30 days postoperatively.ResultsCompared with the control group, the femtosecond laser treatment group showed significantly higher aqueous humor levels of fibroblast growth factor (FGF-2), tumor necrosis factor (TNF)-alpha, leukemia inhibitor factor (LIF), interleukin (IL)-1ra and IL-18, and significantly lower aqueous humor levels of IL-9, platelet-derived growth factor (PDGF)-BB, eotaxin and TNF-beta. Postoperative aqueous flare was significantly greater in the manual cataract surgery group at 1 day (p<0.001), 7days (p<0.001) and 30 days (p = 0.002). No correlation was found between the analyzed mediators and the aqueous flare values.ConclusionsThe expression profiles of cytokines, chemokines and growth factors and the correlations of these profiles with the in vivo inflammatory indexes for patients undergoing cataract surgery with femtosecond laser pretreatment were identified. Our data indicate a disturbance of postoperative inflammation response after femtosecond laser treatment.

Abstract:Posterior capsular opacification (PCO), also known as "secondary cataract", is a common complication following cataract surgery that can significantly impair visual acuity. The incidence of PCO varies widely in the literature, influenced by intraocular lens (IOL) type and patient risk factors. Neodymium-doped yttrium-aluminum-garnet (YAG) laser posterior capsulotomy is the standard treatment for PCO-related visual impairment. Recurrence of PCO after initial treatment with YAG capsulotomy, though more common in children, is rare in adults. Its underlying pathophysiological mechanisms are similar to that of primary PCO, which includes proliferation, migration, and/or clustering of lens epithelial cells (LECs), with subsequent reclosure of the posterior aperture. Potential risk factors for PCO recurrence that have been speculated through a comprehensive search of the current literature include younger age, female sex, high myopia, diabetes, vitrectomized status, uveitis, low-diopter IOLs, and certain IOL types with higher water content. We present a case of recurrent PCO in a highly myopic 48-year-old male following cataract surgery and implantable collamer lens (ICL) explantation who received a hydrophobic acrylic lens with 4% water content. Surgical techniques that may reduce recurrent PCO occurrence and appropriate postoperative care are emphasized to assist surgeons in their approach to patients at high-risk for this complication.

Abstract:Objective To evaluate the effect of neodymium:YAG(Nd:YAG) laser capsulotomy on ocular wavefront aberrations in pseudophakic eyes with posterior capsule opacification(PCO).Methods The wavefront aberrations in 68 pseudophakic eyes of 57 patients with PCO were measured with Nidek OPD aberrometer/topographer before and after Nd:YAG laser capsulotomy.Root-mean-square(RMS) of total aberration,higher aberration,spherical aberration,coma and trefoil at 5.0 mm pupil diameter was measured.Results Best corrected visual acuity improved in all eyes after Nd:YAG capsulotomy significantly(P<0.05).The RMS of total aberration,higher aberration,spherical aberration,coma and trefoil was 1.849±0.763,0.605±0.247,0.238±0.146,0.166±0.112 and 0.322±0.132,respectively after capsulotomy.The RMS values in all eyes decreased after capsulotomy significantly(P<0.05).Conclusions Nd:YAG laser capsulotomy significantly reduced the wavefront RMS in the pseudophakic eyes with PCO.

Abstract:Aspergillus foetidus ATCC 10254 was selected from nine starch-utilizing microorganisms for its high amylolytic activity. This mould produced high levels of extracellular alpha-amylase in rice starch medium and degraded the available starch efficiently. Optimal conditions for enzyme production on 2.0% rice medium included 28 degrees C, initial pH of 6.6, and supplementations with 0.02% NaNO2, 0.08% KH2PO4, and 0.08% corn steep liquor. Eleven-fold purification of the enzyme was obtained after ammonium sulphate and ethanol precipitations from spent medium. The molecular weight was estimated at 41 500. Optimum pH and temperature for enzyme activity were 5.0 and 45 degrees C. Michaelis-Menten constants were 1.14 mg/ml on amylopectin, 2.19 mg/ml on soluble starch and 7.65 mg/ml on amylose. Amylose produced substrate inhibition while glucose or maltose did not inhibit the enzyme. This alpha-amylase may be used as a saccharifying enzyme for rice starch. Aspergillus foetidus ATCC 10254 also presents a potential for treatment of starch-containing waste waters.
Lens Capsule-Related Complications in Femtosecond Laser-Assisted Cataract Surgery: a Study Based on Video Analysis

Abstract:PurposeTo analyse the occurrence and potential causes of lens capsule-related complications during femtosecond laser-assisted cataract surgery (FLACS).MethodsThis prospective consecutive cohort study included the first 1600 eyes (from 1140 consecutive patients) who received FLACS performed by the same surgeon from May 2015 to December 2018. The potential causes and characteristic signs of capsulotomy-related complications, including incomplete capsulotomies and radial anterior capsule (AC) tears, were summarised based on the agreement of two ophthalmologists after they analysed the surgical videos. Subgroup analysis was conducted to characterise the capsulotomy learning curve.ResultsOf the 1600 eyes, 52 (3.25%) had incomplete capsulotomies and 22 (1.38%) had radial AC tears. The most common causes of incomplete capsulotomies were eye tilt (16 eyes, 30.77%), air bubbles or ocular secretions at the interface (14 eyes, 26.92%) and white cataracts (7 eyes, 13.46%). Additionally, 54.55% (12/22) of AC tears were due to incomplete capsulotomy and secondary capsulorhexis. A significant difference was noted between the first 200 eyes and subsequent groups in terms of the incidence of incomplete capsulotomies. No difference was observed in the incidence of AC tears after the initial 100 procedures.ConclusionThe most common causes of incomplete capsulotomies were eye tilt and air bubbles or ocular secretions at the interface. Secondary capsulorhexis after incomplete capsulotomy is the main risk factor for AC tears. There was a steep learning curve for laser capsulotomy in the first 100 operated eyes, as evidenced by the higher complication rate, but this stabilised after 200 procedures.
