Abstract:BACKGROUND:Giant cell tumors (GCTs) and giant cell granulomas (GCGs) are giant cell-rich lesions that occur extremely rarely in the temporal bone and have similar clinical presentations.OBJECTIVES:We aimed to analyze the clinical features and introduce our staging system and surgical treatment.METHODS:Forty-six patients pathologically diagnosed with a giant cell lesion involving the temporal bone between October 2001 and October 2020 were reviewed retrospectively. The clinical characteristics, surgical approaches, and risk factors for recurrence were analyzed.RESULTS:GCTs and GCGs presented as masses centered on the temporomandibular joint with similar imaging features, including a thin, calcified shell and central scattered calcifications on a computed tomography scan. Differences were detected on magnetic resonance imaging in 29.6% (4/14) of GCG and 50% (16/32) of GCT cases; the remaining cases were not distinguishable. Based on our staging system and surgical strategy, 31.8% (7/22) of GCT and 10% (1/10) of GCG cases experienced recurrence, which compares to recurrence rates of 60% in GCT cases and 20% in GCG cases in previous studies.CONCLUSIONS:Specific clinical and preoperative imaging features help to make a diagnosis of temporal giant cell-rich lesions. Our staging system and surgical strategy could help surgeons tailor the surgical strategy.
Giant Cell Tumors of the Skull: a Series of 18 Cases and Review of the Literature

Abstract:Giant cell tumors (GCTs) are generally benign, locally aggressive lesions mostly located in the metaphysis of long bones. GCTs of the skull are rare and the majority of the cases have been presented as case reports. The authors retrospectively reported 18 patients with GCTs of the skull at a single institution from April 1994 to February 2012 and summarized the clinical, radiological, pathological characteristics and management of the disease. Meanwhile, a systematic review of 94 case reports of GCTs of the skull was performed. Headache and symptoms related to the involvement of intracranial nerves were the most common symptoms. Over 90 % of the tumors originated from sphenoid and temporal bones. On MRI, very low signal on T2-weighted images were found highly indicative of GCTs of the temporal bone. Univariate analysis revealed that extent of tumor resection and post-operative radiation therapy (RT) were prognostic factors significantly influencing the survival of the patients. We concluded that complete tumor resection is the optimal goal in treating this disease and adjuvant RT should be given once tumor residual is inevitable.
Granular cell tumor:a clinicopathologic analysis of 15 cases

Abstract:Purpose To investigate the histogenesis and clinicopathologic features of granular cell tumor(GCT).Methods The clinical and pathologic profiles of 15 GCT cases were evaluated. Immunohistochemical study was performed on all cases.Results Our study included 13 cases of benign granular cell tumor(BGCT)and 2 malignant granular cell tumors(MGCT). There were 5 male patients and 10 female. Ages ranged from 19 to 69 years (mean, 41.6 years), and two patients with MGCT were 67 and 69 years old in this group. The tumor size of BGCT ranged from 0.4 to 5.3 cm (mean 2.3 cm), and the tumor sizes of 2 MGCTs were 6 cm and 14 cm respectively. All cases were mainly manifested as a solitary painless nodule located in the dermal, subcutaneous and submucosal tissues. Three cases of tumors occurred in the waist, two in the subaxillary region, two in the chest wall, one each in the breast, upper arm, uterus, perianal region, vocal cords, esophagus, colon and tongue. Microscopically, the tumor cells of BGCT usually possessed abundant eosinophilic granular cytoplasm, small and dark nuclei. The tumor cells of MGCT usually exhibited the following features: vesicular nuclei with prominent nucleoli, nuclear pleomorphism and spindling of some tumor cells. Immunohistochemical study showed strongly positive staining for NSE, S-100 protein and CD68 in all cases, but negative for SMA and MG. Conclusions GCT is a rare tumor showing an origin from Schwann cell. The majority of MGCT occur in the elderly, and volume of the tumor usually is relatively large.

Abstract:Objective To explore the imaging features of giant cell tumor of tendon sheath(GCTTS) so as to improve the accuracy of imaging diagnosis. Methods Thirty?nine cases of GCTTS confirmed by pathological examination were retrospectively analyzed.Of the 39 cases, 32 were diffuse form and 7 were localized form.Seventeen patients underwent X?ray examination,18 had CT pain examination,37 had pain MR imaging and 17 had contrast?enhanced MR imaging.Results X?ray showed local soft tissue masswith slightly high density, and the adjacent bones were shown destruction or erosion tovarious degrees(10/17). No obvious calcification and periosteal reaction were shown.CT scans could provide images better than X?ray films.CT showed local soft tissue masses in the peri?articular muscle gaps. Some tumors were lobulated with cross?jointed growth(8/18),and the adjacent bones were shown destruction or erosion tovarious degrees(11/18).No obvious calcification and periosteal reaction were shown. On MRI,most cases were isointense on T1WI(32/37),and hyperintense on T2WI(27/37). Most cases tended to be heterogeneously enhanced following Gd?DTPA administration(14/17).The lesions were typically located adjacent to a tendon or partially/completely encasing it. Conclusions The imaging findings of GCTTS have some certain characteristics. MRI is able to depict the characteristic internal signal of GCTT and its relationship with adjacent tissues, which is valuable for diagnosis, treatment and follow?up.

Abstract:Giant cell tumor (GCT) of bone is a locally aggressive benign neoplasm that is associated with a large biological spectrum ranging from latent benign to highly recurrent and occasionally metastatic malignant bone tumor. It accounts for 4-10% of all bone tumors and typically affects the meta-epiphyseal region of long bones of young adults. The most common site involved is the distal femur, followed by the distal radius, sacrum, and proximal humerus. Clinical symptoms are nonspecific and may include local pain, swelling, and limited range of motion of the adjacent joint. Radiographs and contrast-enhanced magnetic resonance imaging (MRI) are the imaging modalities of choice for diagnosis. Surgical treatment with curettage is the optimal treatment for local tumor control. A favorable clinical outcome is expected when the tumor is excised to tumor-free margins, however, for periarticular lesions this is usually accompanied with a suboptimal functional outcome. Local adjuvants have been used for improved curettage, in addition to systematic agents such as denosumab, bisphosphonates, or interferon alpha. This article aims to discuss the clinicopathological features, diagnosis, and treatments for GCT of bone.

Abstract:Malignant giant cell tumors of bone (MGCTB) are rare, and the diagnosis can be difficult due to the occurrence of a variety of malignant tumors containing giant cells. To better understand its clinicopathological features, we have reviewed our experience with 17 cases of MGCTB. Five cases were primary malignant giant cell tumor of bone (PMGCTB), and 12 cases were giant cell tumors of bone initially diagnosed as benign but malignant in a recurrent lesion (secondary MGCTB, SMGCTB). The patients included six women and 11 men (age ranged from 17 to 52 years; mean, 30.5 years). The tumor arose in the femur (six cases), the tibia (seven cases), the humerus (three cases), and the fibula (one case). Microscopically, PMGCTB showed both conventional giant cell tumor and malignant sarcoma features. SMGCTB were initially diagnosed as conventional giant cell tumor of bone, the recurrent lesion showing malignant features. Histologically, the malignant components included osteosarcoma (11 cases), undifferentiated high-grade pleomorphic sarcoma (two cases), and fibrosarcoma (four cases). SMGCTB cases showed strong expression of p53. Follow-up information revealed that four patients died of lung metastasis, two patients are alive with lung metastases, and 11 patients are alive without tumor. MGCTB should be considered as a high-grade sarcoma. It must be distinguished from GCTB and other malignant tumors containing giant cells. p53 might play a role in the malignant transformation of GCTB.
Multimodality imaging of an unusual giant cell tumor of thoracic spine with mediastinal invasion: a case report

Abstract:Giant cell tumor (GCT) is a benign yet locally aggressive bone neoplasm typically situated in the juxta-articular metaphysis of long bones. Although spinal involvement is rare, it is predominantly reported in the axial skeleton, with the sacrum being the primary location. Conversely, GCTs are notably uncommon in the thoracic spine. In this report, we present computed tomography (CT), magnetic resonance imaging (MRI), and 2-Deoxy-2-[fluorine-18]-fluoro-D-glucose (18F-FDG) positron emission tomography combined with computed tomography (PET/CT) multimodality imaging findings of a 36-year-old woman diagnosed with a GCT of the thoracic spine. CT scans provide a precise evaluation of cortical thinning and penetration. While MRI lacks specific diagnostic indicators for GCT, it remains invaluable for delineating the extent of soft tissue expansion and the tumor's relationship with intraspinal neural elements -critical information for surgical planning. 18F-FDG PET/CT effectively illustrates the lesion's hypermetabolic and locally aggressive characteristics. It is noteworthy that GCT occasionally exhibits metastatic malignant potential, underscoring the value of FDG PET as a pivotal modality for staging, restaging, or assessing therapy response, and for monitoring the efficacy of radiotherapy. Familiarity with the imaging features of GCT is essential for physicians to avoid misinterpretation. This tumor should be considered in the differential diagnosis of spinal tumors, distinguishing it from bone metastases or neurogenic tumors.

Abstract:Objectives To review the clinical and imaging data of spinal giant cell tumour of the tendon sheath (GCTTS) to improve our understanding of the disease. Methods The imaging findings, clinicopathological features and clinical outcomes of 14 patients with pathologically confirmed spinal GCTTS were analysed retrospectively. Results All 14 patients had a single spinal lesion, including ten cervical vertebra lesions and four thoracic vertebra lesions. CT scan findings: The lesions showed osteolytic bone destruction and were centred on the facet joint, eroding the surrounding bone with a paravertebral soft tissue mass. MRI scan findings: all the lesions manifested predominantly as isointense or hypointense on T1-weighted imaging (T1WI). On T2-weighted imaging (T2WI), eight lesions were hypointense, and four were isointense. The remaining two lesions showed slight hyperintensity. The enhanced scans of eight lesions showed moderate to marked homogeneous or heterogeneous enhancement. PET/CT findings: Among the five patients who underwent PET/CT, three presented lesions with well-defined, sclerotic borders, and the uptake of 18 F-FDG was markedly increased. One lesion showed an ill-defined border and an uneven increase in 18 F-FDG uptake with an SUVmax value of 8.9. A recurrent lesion was only found on PET/CT 45 months after surgery and the SUVmax was 5.1. Conclusions Spinal GCTTS is extremely rare. Osteolytic bone destruction in the area of the facet joint with a soft tissue mass and hypointensity on T2WI images are indicative of the spinal GCTTS. GCTTS shows high uptake of 18 F-FDG, and PET/CT is helpful in detecting recurrent lesions.
Giant cell tumors of the skull base: case series and current concepts

Abstract:Objective: To study the clinical features, tumor characteristics and outcomes of giant cell tumors (GCTs) in the skull base based on long-term follow-up. We also report the largest series of GCTs in the temporal bone and the lateral skull base. Materials and methods: A retrospective study was conducted of all GCTs managed at the Gruppo Otologico, a quaternary referral skull base institute, in Italy from 1993 to 2013. The clinical features, investigations, surgical management and follow-up were recorded. The surgical approaches used were infratemporal fossa approach (ITFA) type B and D and middle cranial fossa (MCF) approaches. Results and observations: A total of 7 patients with GCTs of the skull base were treated at our institution. The principal complaints were hearing loss reported in 6 (85.71%) patients, tinnitus in 5 (71.43%) and swelling in 3 (42.9%). Pure-tone audiometry showed conductive hearing loss in 5 (71.43%) patients. High-resolution CT scan and MRI with gadolinium enhancement were done in all patients. Radiology showed involvement of the ITF and middle ear in 6 (85.71%) patients each, temporomandibular joint in 4 (57.14%) patients, invasions of the squamous part of the temporal bone, mastoid, MCF and greater wing of sphenoid in 3 (42.9%) patients each and the petrous bone in 2 (28.6%) patients. ITFA type B was applied as an approach for tumor removal in 5 (71.43%) patients, including a case where an additional MCF approach was employed, and ITFA type D and the transmastoid approach were applied in 1 (14.3%) patient each. Total tumor removal and successful cure was achieved in 6 (85.71%) patients. Subtotal removal leading to recurrence and eventual mortality was the result in 1 (14.3%) patient. Conclusions: A thorough knowledge of the anatomy of the skull base and the various skull base approaches is necessary to tackle GCTs. ITFA type B and D combined with MCF approaches provide good exposure of the tumor with minimal postoperative sequelae and good locoregional control. Recurrence due to either subtotal removal or suboptimal treatment may have disastrous consequences for the patient.

Abstract:Objective To analyze the common MRI signs and typical cases of bone giant cell tumors,in order to improve its diagnostic accuracy.Materials and Methods The MR findings of 96 cases with bone giant cell tumors proved pathologically were analyzed retrospectively.The common signs were defined as the signs appeared in 60% cases.The typical cases were the cases with all the common signs.Results The common signs included:patient with 20-40 ages(66.67%),in the end of tubular bone(67.71%),predominantly intermediate signal intensity with hypo and/or hyperintensity foci in T1 weighted image(63.54%),predominantly intermediate signal intensity with patchy hyperintensity signal foci in T2 weighted image(70.83%),liquefy and necrosis(71.88%),without fluid-fluid levels(87.50%),without liquid in the adjacent joint(78.13%),without soft tissue mass(60.42%),extending to subchondral bone or terminal plate of vertebral body(90.63%),with integrated or unintegrated hypointensity margin(10%),with different degree edema around the lesions(63.54%),diffuse inhomogenerous enhancement(82.76%).All the common signs were appeared at the same time in 10 cases(10.41%).Conclusion Patient with 20-40 ages,in the end of long tubular bone,predominantly intermediate signal in T1WI and T2WI,liquefy and necrosis,no fluid-fluid levels and soft tissue mass,no liquid in the adjacent joint,extending to subchondral bone,with hypointensity signal margin and edema,and diffuse inhomogenerous enhancement are the common MRI signs,typical cases are rare.
Imaging Features of Aggressive Giant Cell Tumors of the Mobile Spine: Retrospective Analysis of 101 Patients From Single Center

Abstract:Study Design: Retrospective study. Objectives: Giant cell tumors (GCTs) of the mobile spine can be locally aggressive. This study described and classified the typical and atypical appearance of aggressive spinal GCTs according to imaging findings to help the imaging diagnosis, especially for patients with rapid neurological deficit that may require emergent surgery without biopsy. Methods: Computed tomography (CT) and magnetic resonance imaging (MRI) scans of patients diagnosed with aggressive spinal GCTs at single center were reviewed. Results: Overall, 101 patients with 100 CT images and 94 MR images were examined. All lesions were osteolytic with cortical destruction; 95 lesions showed epidural extension; 90 were centered in the vertebral body; 82 showed pathological fracture and/or collapse of the vertebral body; 78 had pseudotrabeculation on CT; 80 showed low-to-iso signal intensity or heterogeneous high-signal intensity with cystic areas on the T2-weighted images; 9 showed fluid-fluid level on T2-weighted images; and 61 patients showed marked enhancement on contrast-enhanced CT and/or MRI. Forty-one lesions (40.6%) had at least 1 atypical radiographic feature: 19 involved >= 2 segments; 11 were centered in the posterior neural arch; 10 had a paravertebral mass over 2 segments; 16 showed partial margin sclerosis with partial cortical destruction on CT scans; and 3 showed mineralization within the tumor on CT. Eighty-eight patients underwent CT-guided biopsy with a diagnostic accuracy rate of 94.3%. Conclusions: Spinal GCTs might appear more radiologically atypical, and about 40% of the lesions may have at least 1 atypical feature. CT-guided biopsies are recommended for definitive diagnosis.

Abstract:Giant cell tumor of bone (GCTB) is classified as an intermediate malignant tumor due to its locally aggressive behavior, burdened by high local recurrence rate. GCTB accounts for about 4–5% of all primary bone tumors and typically arises in the metaphysis and epiphyses of the long tubular bones. Mutation of gene H3F3A is at the basis of GCTB etiopathogenesis, and its immunohistochemical expression is a valuable method for practical diagnosis, even if new biomarkers have been identified for early diagnosis and for potential tumor recurrence prediction. In the era of computer-aided diagnosis, imaging plays a key role in the assessment of GCTB for surgical planning, patients’ prognosis prediction and post treatment evaluation. Cystic changes, penetrating irregular margins and adjacent soft tissue invasion on preoperative Magnetic Resonance Imaging (MRI) have been associated with a higher rate of local recurrence. Distance from the tumor edge to the articular surface and thickness of unaffected cortical bone around the tumor should be evaluated on Computed Tomography (CT) as related to local recurrence. Main features associated with local recurrence after curettage are bone resorption around the graft or cement, soft tissue mass formation and expansile destruction of bone. A denosumab positive response is represented by a peripherical well-defined osteosclerosis around the lesion and intralesional ossification. Radiomics has proved to offer a valuable contribution in aiding GCTB pre-operative diagnosis through clinical-radiomics models based on CT scans and multiparametric MR imaging, possibly guiding the choice of a patient-tailored treatment. Moreover, radiomics models based on texture analysis demonstrated to be a promising alternative solution for the assessment of GCTB response to denosumab both on conventional radiography and CT since the quantitative variation of some radiomics features after therapy has been correlated with tumor response, suggesting they might facilitate disease monitoring during post-denosumab surveillance.

Abstract:Objective To explore the pathological basis of the magnetic resonance imaging( MRI) features of giant cell tumors( GCTs) in the mobile spine. Methods The MRI images and pathological appearances of 21 patients with GCT were retrospectively reviewed. Results Twenty tumors showed homogeneous low to intermediate signal intensity on T1WI images and one tumor showed heterogeneous signal intensity on T1WI. 11 cases showed iso to low signal intensity on T2WI.Calcification,bone like matrix,collagen matrix and hemosiderin deposition made low signal intensity. Eight tumors showed heterogeneous signal intensity mainly due to secondary cystic changes,necrosis,and hemorrhage. Abundant mononuclear and multinucleated giant cells without calcification,bone-like matrix,collagen matrix and hemosiderin deposition made tumors as relatively high signal on T2WI. Curvilinear low signal was observed in six cases on T1WI and T2WI images within the vertebral body,which corresponded to thickened trabeculae,fibrous setae,or hemosiderin deposit. The low-signal-intensity pseudocapsule was observed in one tumor due to surrounding fibrous proliferation. Abundant thin walled capillaries,blood sinus were the basement of the enhancement pattern. Contrast enhanced scan showed significant homogeneous enhancement in six cases and nine cases showed heterogeneous enhancement due to cystic changes,necrosis. One case showed no significant enhancement due to rare thin-walled capillaries in the stroma. Conclusion MRI appearances can reflect the pathological characteristic of GCTs. The proportion of parenchyma and interstitial led to tumor signal diversity.
Diagnosis and treatment of giant cell tumors of spine: Report of 36 cases

Abstract:Objective: Giant cell tumors of spine often present difficult problems because of their location and unpredictable course. In order to sum up the experience of diagnosis and treatment, thirty six cases were analyzed and reported. Methods: Thirty six cases of giant cell tumor of spine (19 males and 17 females with ages ranging from 12 to 67 years) were reviewed. Sixteen lesions involved the cervical spine, six in thoracic spine, five in lumbar spine, and another nine lesions occupied the cervical thoracic and thoracic lumbar segments or sacrum respectively. Before 1990,the treatments were carried out with two varieties: i.e. radiation therapy alone were done in 3 cases of cervical spine lesion, and 7 cases were treated by surgical excision as well as radiotherapy. Since 1991,a subgroup of 23 cases for whom preoperative computed tomography scans and MRI were available was classified using the Enneking's staging system and Weinstein Boriani Biagini staging system. Six cases of stage 2 lesion and 3 cases of stage 3 lesion were treated by somectomy along the pseudocapsure of the tumor and spinal instrumentation. Thirteen cases of stage 3 lesion were treated by total spondylectomy with spinal instramentation. Results:Radiotherapy alone for 3 cases of cervical spine lesion and radiotherapy with surgical excision for other 7 cases were followed up for 7 to 30 years,and there was no evidence of local recurrence. Somectomy for 9 cases and total spondylectomy for 13 cases were followed up for 2 to 8 years, 3 cases were found with local recurrence because of uncompleted resection, and other 19 cases with no evidence of local recurrence. Conclusion: These preliminary results suggest that completed resection is an effective procedure in control of local recurrence of spine giant cell tumors. Radiotherapycould be considered as an profitable alternative for the treatment of spine giant cell tumors.

Abstract:Aims:Giant cell tumor of the bone (GCTB) is associated with considerable morbidity. As GCTB epidemiological data for China are limited, this study is aimed at describing the disease characteristics of GCTB in China and establishing the historical context for its treatment before recent advances in treatment options.Methods:The disease characteristics, treatment patterns, and local GCTB recurrence rate after primary surgery for GCTB were evaluated in this single-center, retrospective, noninterventional, observational study of patients treated for GCTB at Ji Shui Tan Hospital, Beijing, from 2009 to 2016 based on medical chart review. Patients with unmet need were defined as those whose surgical treatment was difficult or who had to undergo high-morbidity surgery.Results:Among the 668 patients with a primary GCTB diagnosis, 578 (86.5%) of target lesions were in the extremities, and 89 (13.3%) were in the pelvic or axial bone. Of these, 173 (25.9%) were characterized as having an unmet need. Almost all GCTB patients received surgical treatment at both primary diagnosis (666/668 (99.7%)) and last disease recurrence (196/200 (98.0%)). Additionally, about one-third of patients received nonsurgical treatment at primary diagnosis (205/668 (30.7%)) and disease recurrence (67/200 (33.5%)), with neoadjuvant therapy being the most common treatment. The rate of high-morbidity surgery increased for recurrent disease (65/200 (32.5%)) compared with primary diagnosis (111/668 (16.6%)). The 2-year cumulative incidence of postoperative disease recurrence was 29.2%, in line with rates observed in prior studies.Conclusion:As many patients with primary and recurrent disease received high-morbidity surgery, more effective treatments are needed.
Three-micron imaging of the Hubble Deep Field

Abstract:Images of the Hubble Deep Field at 3.2 μm, taken with the 10-m Keck Telescope, are presented. The images cover a total area of ∼ 2.5 arcmin2. To a 5-sigma limit of [3.2]total ≈ 17.5 mag (Vega-relative), 11 sources are detected, 9 of which are extragalactic. The integrated galaxy number counts are therefore ∼ 1.3 × 104 deg−2 to this depth. The galaxies detected at 3.2 μm have a median redshift of z = 0.56. All 3.2 μm sources have 1.6 μm, 1.1 μm and visual counterparts, all of fairly regular morphologies; several also have 6.7 μm, 15 μm, 850 μm, 8.5 GHz, or 1.4 GHz counterparts. No sources are found which are either anomalously red or anomalously blue in their H − [3.2] color, and there are significant detections of most of the known near-infrared HDF sources for which detection in these 3.2 μm data seemed likely. Abstract:Diffuse tenosynovial giant cell tumor (D-TSGCT) is a benign but locally destructive tumor of synovium that may involve joints, tendon sheaths, and bursae. Its occurrence in the temporomandibular joint (TMJ) is extremely rare. The authors reported a case of 48-year-old man with an extra-articular D-TSGCT in the TMJ with medial cranial fossa extension. computed tomography (CT) and magnetic resonance imaging (MRI) features are described. The lesion was a cystic-solid mass centered at the temporal bone without involvement of the condylar head, and its solid component presented high-density on CT and hypointensity on MRI. No signs of recurrence or metastasis was observed during 12-months of follow-up. The present report suggested the potential characteristics of radiologic imaging of D-TSGCT in TMJ.

Abstract:The present study aimed to investigate the value of magnetic resonance imaging (MRI) in the diagnosis of giant cell tumor of the tendon sheath (GCTTS), including localized (L-) and diffuse (D-) types. A total of 38 patients with GCTTS, including 31 with L-GCTTS and 7 with D-GCTTS, diagnosed by surgery and pathology, were retrospectively analyzed. All patients underwent MRI examination. Of the 31 patients with L-GCTTS, the tumors were located in the hand and wrist (18 patients), the ankle and foot (10 cases), the knee joint (2 cases) and the temporomandibular joint (1 case). All 31 lesions were either located in relation to a tendon or were partially/completely enveloping it and all were well marginated. With respect to the 7 D-GCTTS patients, the tumors were located in the ankle and foot (6 cases) or the hand and wrist (1 cases). All 7 lesions presented as an aggressive soft tissue mass infiltrating the tendon sheath and adipose tissue around the joint. The characteristic internal signal of GCTTS, including L-GCTTS and D-GCTTS, was demonstrated by MRI examination. MRI is currently the optimal modality for preoperative assessment of tumor size, extent and invasion of adjacent joint and tenosynovial space.
Giant Cell Tumor in the Thyroid Area: a Case Report in the Novel Location and Review of Literature.

Abstract:Giant cell tumor of soft tissue (GCT-ST) is a rare benign tumor of low malignant potential. It is thought to be the soft tissue counterpart of giant cell tumors of the bone due to its pathological resemblances. GCT-ST is most commonly found in superficial soft tissue of thigh, trunk and upper extremities. The head and neck region is rarely affected. Here for the first time, we describe a case of GCT-ST in the thyroid region. A 70-year-old female patient presented with a painless swelling in her left neck for the previous three weeks. The condition was initially diagnosed as thyroid goiter and left lobectomy was arranged. Intraoperative findings showed an irregular mass invading the strap muscles and trachea. Complete tumor resection was difficult, and part of the tumor was left in the thyroid bed. Histopathology of the resected specimen showed a mixture of mononuclear round to oval cells and multinucleated osteoclast-like giant cells. The giant cells were CD 68 positive. The patient received a revision surgery 3 months after the first operation to achieve complete resection. There was no recurrence in the first 3-month follow-up. However, 6 months after the revision surgery, the tumor recurred on both sides of the neck. The patient suffered from dysphagia and breathlessness. As further surgery and radiation therapy were not considered, denosumab was used as a novel agent After three months of treatment, the patient showed symptom-relief and tumor-regression. The patient continued to have tumor-regression after 1 year of the denosumab treatment. GCT-ST is a benign tumor, although in this case, it was showing features of malignancy. A review of the literature was conducted to identify previous studies on GCT-ST in the head and neck. We present this case for its rare location and novel treatment with denosumab.
