Abstract:OBJECTIVE:To analyze the clinical characteristics of unilateral conductive hearing loss with intact tympanic membrane, and summarize the key diagnostic points, differential diagnosis and observe the effects of surgical treatment.METHODS:We reviewed data from 82 patients with unilateral conductive hearing loss with intact tympanic membranes who accepted the exploratory tympanotomy from April 2011 to September 2013. There were 41 males and 41 females, aged from 7 to 66( averaged 26.5±13.7)years, with a history of one month to 50 years. The history, clinical symptoms, audiological evaluation, high resolution temporal bone CT, the results of surgical exploration and hearing reconstruction were analyzed.RESULTS:The exploratory tympanotomy revealed 43 cases of congenital middle ear malformations (52.4%), 22 cases of otosclerosis (26.8%), eight cases of congenital cholesteatoma (9.8%), six cases of trauma induced conductive hearing loss (7.3%), three cases of congenital ossicular malformations with congenital cholesteatoma (3.7%). Progressive hearing loss was common in patients with otosclerosis and congenital cholesteatoma, and patients with congenital middle ear malformations described their hearing loss since childhood. High resolution temporal bone CT of congenital middle ear malformation, trauma induced conductive hearing loss, congenital cholesteatoma diagnosis rate was 40.0%, 50.0%, and 83.3% respectively. The preoperative air-conductive threshold of patients with absence of the oval window were increased to (66.9±1.1)dBHL, the preoperative bone-conductive threshold achieved (28.3±10.4)dBHL at 2 000 Hz. While patients with stapes fixation and that with ossicular chain discontinuity were (27.2±9.7)dBHL and (17.8±8.8)dBHL(P=0.000)respectively. Through the tympanic exploration with endaural incision under the microscope, different hearing reconstruction were applied according to different lesions. After the operation, the hearing level of 52 patients with return visit were improved, the mean air-conductive threshold were decreased from (60.0±11.4)dBHL to (32.2±12.1)dBHL(P=0.000); and the mean ABG were decreased from (43.2±12.0)dB to (16.3±9.4)dB(P=0.000).CONCLUSIONS:Congenital middle ear malformations, otosclerosis, congenital cholesteatoma are the most common causes in unilateral conductive hearing loss with an intact tympanic membrane. The diagnosis rate can be improved by analyzing the clinical features. Through exploratory tympanotomy and hearing reconstruction, we can clarify the diagnosis and achieve a satisfying hearing recover.
Intraoperative CT-guided cochlear implantation in congenital ear deformity.

Abstract:Intraoperative computed tomography (iCT)-guided cochlear implantation is practical and effective for correct electrode placement in the cochlea of patients with congenital inner ear and/or complex middle ear malformation.The operation in patients with inner ear and/or complex middle ear malformation including abnormal facial nerve course is difficult. This study evaluated the efficacy of cochlear implantation under the guidance of iCT to insure correct electrode placement.This was a prospective interventional case series. Ten patients with severe to profound sensorineural hearing loss due to ear malformations were enrolled, and iCT was used to confirm the right placement of electrodes.Intraoperative CT was performed three times in one patient, twice in two, and once in the others. Interruption of the surgical process for each iCT until resumption of surgery was 9.64 ± 0.63 min. iCT revealed incorrectly positioned cochlear implants in two patients, which were immediately corrected. There were no reoperations due to misplacement of electrodes. iCT helped locate the cochlea in the middle ear of one patient with an abnormal facial nerve course. The overall intervention rate based on iCT findings was 30%.level 4.
Imaging assessment of profound sensorineural deafness with inner ear anatomical abnormalities

Abstract:Objective: To explore the value of a combined computed tomography (CT) and magnetic resonance imaging (MRI) in evaluating profound sensorineural deafness patients before cochlear implant (CI) surgery. Methods: A retrospective analysis of 1012 cases of profound sensorineural deafness that received CI was performed. Results: A total of 96 cases were diagnosed with inner ear abnormalities including large vestibular aqueduct syndrome (LVAS, n = 61), Michel deformity (n = 3), cochlear incomplete partition I (n = 2), cochlear incomplete partition II (n = 6), cochlear hypoplasia with vestibular malformation (n = 3), cochlear ossification (n = 3), bilateral internal auditory canal obstruction (n = 5) and internal auditory canal stenosis (n = 2). Conclusion: High resolution CT (HRCT) can display bony structures while MRI can image the membranous labyrinth in preoperative evaluation for cochlear implantation. The combination of these two modalities provides reliable anatomical information regarding the bony and membranous labyrinths, as well as the auditory nerve.
[Functional Evaluation of Eustachian Tube in Patients with Otitis Media Using Multi-Slice Spiral CT and Impedance Audiometry].

Abstract:OBJECTIVE:To assess the value of multi-slice spiral CT (MSCT) and impedance audiometry in preoperative evaluation of eustachian tube function in patients with otitis media.METHODS:MSCT and impedance audiometry were performed in 61 patients with otitis media (63 ears), and the results were graded to analyze the correlation between the two examinations. The pathological changes at the tympanic orifice were recorded during surgical exploration of the eustachian tube, and the results were compared with those by MSCT and impedance audiometry.RESULTS:MSCT revealed normal findings in 8 ears, mild pathological changes in 19 ears, and severe changes in 36 ears. Soft tissue shadows at the tympanic orifice was found in 16 ears, and 47 ears were free of the shadows. Impedance audiometry showed normal findings in 20 ears, mild pathological changes in 15 ears, and severe changes in 28 ears. In surgical exploration, tympanic orifice lesions were detected in 22 ears, and were not detected in 41 ears. The rank correlation coefficient between MSCT and impedance audiometry was 0.706, suggesting a correlation between the two examinations. Goodness-of-fit test showed good match of the results of MSCT and surgical exploration (P=0.000, kappa=0.627).CONCLUSION:MSCT combined with impedance audiometry provides a means for comprehensive evaluation of eustachian tube function and lesions, which greatly facilitates surgical planning and treatment of the lesions.

Abstract:The aim of this study is to evaluate the usefulness of high-resolution computed tomography (HRCT) densitometry in the diagnosis of otosclerosis and to investigate the relationship between CT densitometry and audiometry.HRCT findings and audiometry were compared among 34 patients (34 ears, the otosclerosis group) with surgically confirmed otosclerosis between January 2007 and December 2007 and 33 patients (33 opposite normal ears, the control group) with facial paralysis diagnosed at the same period of time. Seven regions of interest (ROI) were set manually around the otic capsule on the axial slice of 0.75-mm-thick CT image. The mean CT values of these seven regions were measured. In each ROI, the mean CT value of the otosclerosis group and that of the control group were compared. Based on the CT findings, the ears with otosclerosis were classified into two groups: Group A showed no pathological CT findings; Group B showed low density around the cochlea. In the otosclerosis group, the relationship between the findings of CT and the results of audiometry was analyzed.The mean CT values in the area posterior to the oval window and anterior to the oval window were significantly lower for the otosclerosis group compared with the control group (the former t=-2.030, p=0.046; the latter Z=-4.979, p<0.01). Group A consisted of 30 patients, 7 of which (23.33%) exhibited conductive hearing loss, and 23 of which (76.67%) exhibited mixed hearing loss; Group B had 4 patients, all with mixed hearing loss. For the otosclerosis group, the mean CT value in the area posterior to the oval window was positively correlated with the mean air conduction threshold (r=0.4273, p=0.0117) and with the mean air-bone gap (r=0.3995, p=0.0192).Quantitative evaluation of CT with slices less than 1mm in thickness may provide important information for the diagnosis and assessment of otosclerosis which are unattainable through other methods.
Virtual Quality Control in Middle Ear Surgery: Is Image-guided Tympanoscopy a Valuable Tool for Depicting Borderline Situations?

Abstract:Hypothesis: Before modern imaging was introduced, revision surgery was the only way to evaluate possible reasons for inadequate improvement in hearing after ossicular replacement during reconstructive middle ear surgery. Background: The aim of this study was to evaluate freely navigable virtual tympanoscopy using different computed tomographic modalities. We compared cone-beam computed tomography (CBCT), flat panel computed tomography (FPCT), and conventional computed tomography in helical mode (CTH), volume mode (CTV), and ultra high resolution mode (CTD). Methods: Four temporal bone specimens were reconstructed with partial or total ossicular replacement prostheses. The best functional results for prosthetic coupling were achieved under the control of laser Doppler vibrometry (LDV). Afterward, a progressive step-by-step decoupling of the prostheses was carried out. Different prosthesis positions were evaluated by LDV as well as different computed tomographic modalities with 3D reconstruction of each dataset. Results: Anatomical structures were better depicted and the best position and coupling of inserted prostheses were achieved using CBCT. All imaging techniques could be used to control the position of middle ear prostheses, but CBCT provided the highest resolution and the best image quality in both 2D and 3D reformations and in 3D-animated video representation. Conclusion: Compared with several other imaging modalities, CBCT was best at depicting miscellaneous coupling problems. Noninvasive detection of coupling problems caused by minimal loss of contact between prostheses and middle ear ossicles will influence the clinical outcome. This early detection will help to determine whether revision surgery is needed.
The value of preoperative CT scan of tympanic facial nerve canal in tympanomastoid surgery

Abstract:CONCLUSIONS: The combined analysis of axial, coronal, and sagittal planes can increase the positive rate of diagnosis for dehiscence of the facial nerve canal.OBJECTIVE: The aim of this study was to evaluate the diagnostic value of high resolution computed tomography (HRCT) scanning of the condition of the tympanic portion of the facial nerve canal in patients with chronic otitis media.METHODS: A total of 76 ears of 72 inpatients with chronic suppurative otitis media and with cholesteatoma or granulation tissues in the attic were examined by routine HRCT. The condition (including dehiscence) of the tympanic portion of the facial nerve canal observed by CT on multi-planar images and surgical findings were recorded.RESULTS: The condition of the tympanic portion of the facial nerve canal observed by CT could be confirmed by surgical findings in 67/76 ears, but was not confirmed in 9/76 ears. Axial-transverse images could clearly show the position and length of dehiscence on the lateral wall of the facial nerve, while coronal images were superior to axial-transverse images in showing the bony circumference of the facial nerve, and sagittal images could show the inferior wall of the facial nerve canal more clearly than images on the former planes.

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Objective: To analyze the etiology, diagnosis, and treatment of unexplained conductive hearing loss (UCHL) with intact tympanic membrane. Methods: A systematic review was conducted based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. A total of 642 articles were retrieved from databases such as PubMed, Embase, Web of Science, and Cochrane. Fifty-four research articles and 21 case reports were screened out according to the inclusion and exclusion criteria for analysis of the etiology of UCHL. Seven research articles with UCHL who underwent exploratory tympanotomy were selected for data extraction and analysis of clinical characteristics. Results: UCHL is a common manifestation of various diseases, including congenital ossicular anomalies (COA), otosclerosis (OTS), congenital middle ear cholesteatoma (CMEC), oval window atresia, superior semicircular-canal dehiscence, congenital stapedial footplate fixation, middle ear osteoma or adenoma, congenital ossification of stapedial tendon, and so on. A total of 522 patients were included in the 7 articles; among whom OTS showed a tendency to increase with age. The main symptoms were hearing loss, followed by tinnitus, dizziness, ear fullness, ear pain, facial paralysis. A total of 87.5% to 93.0% patients with COA manifested as nonprogressive deafness that occurred since childhood, with tinnitus incidence of 15.6% to 30.2%, and 86.4% to 96.4% patients with OTS presented with progressive hearing loss, with tinnitus incidence of 60.1% to 90.9%. The diagnosis positive rate of high-resolution computed tomography (HRCT) was 33.8% to 87.1%, and CMEC was higher than that of COA (83.3%-100% vs 28.6%-64%). All the articles reported good hearing recovery. The most common surgical complications included taste abnormalities, tinnitus, and dizziness. Conclusion: UCHL presents with similar clinical manifestations and poses challenges in preoperative diagnosis. Exploratory tympanotomy is the primary method for diagnosis and treatment, with good prognosis after removing the lesion and reconstructing hearing during the operation. Children can also safely undergo the surgery.

Abstract:Objective:To study high-resolution CT(HRCT) findings of chronic sup p urative otitis media and evaluate their clinical significance.Me thods:HRCT find ings of 118 ears with chronic suppurative otitis media confirmed by surgery were analysed retrospectively and compared with surgical findings.Re sults:The accur acy of HRCT in diagnosis of cholesteatoma, granuloma, and simple otitis media wa s 95.8%, 94.1%, and 96.6%, respectively. The specificity was 98.4%, 96.1%, and 95.8%, respectively. The sensitivity was 929%, 86.7%, and 97.9%, respectivel y.There is significant difference in location and shape of soft tissue mass, invol vement facial nerve canal, tympanic sinus and destruction of bony labyrinth bet ween cholesteatoma, granuloma and simple otitis media, but no difference between cholesteatoma and granuloma. The incidence of destruction of auditory ossicles in cholesteatoma was higher than that in granuloma. No destruc tion of auditory ossicles was demonstrated in simple otitis media. The incidence of destruction of tympano-scutum or tegmen in cholesteatoma was highe r than that in granuloma and simple otitis media. Remodeling of bony wall surrounding the so ft tissue mass was found in 52 ears(92.9%) with cholesteatoma, and bony erosion in 12 ears(80%) with granuloma.Conclusion: HRCT can accurately show location and shape of soft tissue mass of tympanum and/or mastoid antrum, destruction of aud itory ossicles and important structures of temporal bone, which can provide evid ence in diagnosis of choronic suppurative otitis media and in its classification .
Reliability of high-resolution CT scan in diagnosis of ossicular tympanosclerosis

Abstract:Objectives: To evaluate the reliability of high-resolution CT scans (HRCT scans) in the diagnosis of tympanosclerosis and to determine its benefit to predict the post-surgical hearing outcome based on comparing radiological and surgical findings. Methods: A retrospective study at a tertiary institute included 940 ears that underwent tyampanoplasty for chronic suppurative otitis media (CSOM) between January 2013 and March 2017. Preoperative temporal bone HRCT scans were analyzed to check for the prediction of tympanosclerosis and ossicular fixation. Intraoperatively, ossicular chain integrity was checked. Preoperative and postoperative audiometric evaluations using air-bone gap (ABG) were compared. A postoperative pure-tone ABG of 20 dB or less was considered as a successful hearing result. Results are compared with historical control groups, the study has been reviewed and approved by the IRB at the medical research center in Hamad Medical Corporation; however, it is a retrospective study so no informed consent was obtained from the patients. Results: The study included 940 ears that underwent tympanoplasties due to CSOM, where 238 out of 940 (25.3%) of ears showed tympanosclerosis during tympanoplasty, intraoperatively, tympanosclerosis was localized to the eardrum in 174 of the 238 involved ears (73.1%), A 64 out of 238 (26.9%) of the ears with tympanosclerosis showed ossicular fixation, divided as 45 ears with Incudo-malleal fixation, 14 ears with stapes fixation, and 5 ears with triple ossicular fixation. HRCT scan of the temporal bone was suggestive of ossicular chain fixation in 79 cases distributed as 55 incudo-malleal fixations, 19 stapes fixation, and 5 complete ossicular fixations, with a sensitivity of 96.8%, and specificity of 98%. The audiological results were analyzed, with a patient follow-up after 6 months. Conclusions: Our study showed that CT scans when combined with the clinical findings can be an informative guide to otolaryngologists for preoperative evaluation and counseling of tympanosclerosis surgeries.
Radiological Study of Temporal Bone in Children with Profound Deafness Before Cochlear Implant: CT Vs Magnetic Resonance Imaging

Abstract:OBJECTIVE:To evaluate the clinical application of magnetic resonance imaging (MRI) and CT in children with profound deafness before cochlear implant and their impact on surgical decision.METHODS:The pre-operative CT and MRI data from 71 children with bilateral profound deafness planned for cochlear implant were studied.RESULTS:12 patients showed enlarged vestibular aqueduct (16.9%); 14 patients had Mondini malformation (19.7%); 3 patients demonstrated enlarged internal auditory meatus (4.2%); and 2 patients were suspected to have fistulae (2.8%) on both CT and MRI examinations. In comparison between MRI and CT, unilateral cochlear fibrosis was detected in 5 patients(7%) by MRI, but none by CT scans. Bilateral dehiscent facial nerve was detected in 1 patient(1.4%) by CT scan, while MRI was normal.CONCLUSION:CT and MRI are essential for the pre-operative planning of cochlear implant, especially in patients with enlarged vestibular aqueduct, Mondini malformation, enlarged internal auditory meatus and suspected fistulae. These two imaging modalities are complementary to each other in cases with cochlear fibrosis and facial nerve dehiscence.
[High-Resolution CT with Multi-Planar Reformation in Diagnosis of Stapedial Otosclerosis].

Abstract:To evaluate the value of HRCT with multi-planar reformation (MPR) in the diagnosis of otosclerosis.138 ears in 129 patients with otosclerosis confirmed by surgery were evaluated retrospectively, using the MPR image along stapes as standard image, and 132 normal ears were collected as the control group.In the otosclerosis group, HRCT-MPR was positive in 108 ears, suspicious positive in 12 ears, and negative in 18 ears.In the control group, HRCT-MPR was positive in 4 ears, suspicious positive in 9 ears, and negative in 119 ears. The sensitivity for HRCT-MPR was 87.0% and the specificity was 90.2%. However, with 2 mm axial CT alone, the sensitivity was only 47.8%.The positive findings were mainly fenestral foci which related to stapes.HRCT-MPR showed high rate of sensitivity and specificity.We recommend the MPR image along stapes as standard image for otosclerosis diagnosis.
THE ROLE OF HIGH-RESOLUTION MSCT IN THE DIAGNOSIS OF OSSICULAR CHAIN DISORDER TYMPANOSCLEROSIS

Abstract:To review the effectiveness of high-resolution computed tomography in making a diagnosis of tympanosclerosis and its reliability in predicting the postoperative period based on the result of hearing restoration, also based on radiological and surgical data. Methods: A retrospective survey was conducted in diagnostic and treatment institutions, and tympanoplasty was performed for chronic suppurative otitis media (CSOM) 200 ear. In March 2020 and 2021 of the same month, preoperative MSCT images of the temporal bone were reviewed, to verify the prognosis of tympanosclerosis and the integrity of the auditory ossicles. The integrity of the ossicular chain was checked both preoperatively and postoperatively using audiometry (air-bone conduction (ABG)) and the results were assessed: postoperative successful outcome was a tone frequency and sound volume level of 20 dB or less. Results: The study was conducted on 200 ears that underwent tympanoplasty for CSOM, of which 130 out of 200(49%) ears had tympanosclerosis localized in the eardrum, discovered during surgery (tympanoplasty), ears were affected in 41 out of 130 (30, 1%), Incus-malleus ossicular ligament disorders occurred in 19 of 41 ears. 11 ears with a disorder of the stapes bone, and 6 with a disorder of the connection of all three bones. In the remaining 70 cases, a scan of the temporal bone revealed the integrity of all hearing bones and their conductivity. After 6 months, during repeated observation of patients, audiological results of fixation with sensitivity were obtained (95.1%) and specificity (93.8%). Conclusions: Based on the conducted research, it is possible to draw the following conclusion: for otolaryngologists, MSCT in combination with clinical data serves to effectively assess the extent of the lesion, as well as inform for the postoperative prognosis for tympanosclerosis.

Abstract:Objective To investigate and contrastive analyse the respective value of tympanograms and high resolution CT scan of temporal bone in the diagnosis of secretory otitis media.Methods The clinical data of 150hospitalized patients with secretory otitis media in Department of ENTHNS,Tongji Hospital,Wuhan from 2009to 2012were retrospectively analyzed.Based on the surgical finding of tympanic cavity fluid,the diagnostic accordance rate of tympanograms and temporal bone CT for middle ear effusion were calculated respectively,and their diagnostic value were evaluated.Results The diagnostic accordance rate of tympanogram B for the middle ear effusion in secretory otitis media was 94.5%(138/146ears),the rate of abnormal tympanograms was 91.6%(206/225ears),while temporal bone CT with a rate of 99.2%(117/118ears).The difference between the latter two data was statistically significant(P<0.01).Conclusion The temporal bone CT scan,with a higher diagnostic value for middle ear effusion in secretory otitis media,could be used as a supplement to the acoustic immittance measurement,especially to non-B tympanograms for the diagnosis of secretory otitis midia.
Do CT Values Change over Time and Correlate with Hearing in the Non-Surgical Ears of Otosclerosis Patients?

Abstract:Background High-resolution computed tomography (HRCT) is often used to diagnose otosclerosis. Aims/Objectives To investigate whether CT values change over time and correlate with hearing in the non-surgical ears of otosclerosis patients. Materials and methods 32 patients with bilateral otosclerosis who had undergone unilateral stapedectomy were enrolled in the study. HRCT examination and pure tone audiometry were performed before and after the surgery. CT values of different regions of interest (ROIs) were measured manually and the data were collected for analysis. Results The CT value of anterior to the inner auditory meatus (AIAM) decreased by 88.5 HU (p < .05), and the changes in CT values in other ROIs showed no statistical difference. Regarding hearing, the value of air bone gap (ABG) increased by 3.6 dB (p = .020), and the average hearing deterioration rate of ABG was 3.1 dB/year. The CT value of the mid-point of the stapes footplate (MPSF) showed a certain correlation with the change of ABG (p < .05). Conclusions and significance The CT value of AIAM may change over time. The overall hearing of non-surgical ears in otosclerosis patients shows a deteriorating trend. Moreover, the changes in the CT value of MPSF may be related to the hearing, which needs further research.
Observation of osseous labyrinth high-resolution computed tomography densitometry in patients with otosclerosis

Abstract:Objective To investigate the value of high-resolution computed tomography(CT) densitometry in the diagnosis of otosclerosis.Methods CT scanning and pure tone test results of 34 patients(34 ears,the otosclerosis group) with surgically confirmed otosclerosis and 33 patients(33 opposite normal ears,the control group) with facial paralysis diagnosed at the same period were analyzed retrospectively.Seven regions of interest(ROI) were set manually around the otic capsule on the axial slice of 0.75-mm thick.The mean Hounsfieldunit(HU) values of the seven regions were measured.Based on the CT imaging,the ears with otosclerosis were classified into group A(no pathological CT findings) and group B(low density around the cochlea).In each ROI,the difference of mean HU values between the otoscerosis group and the control group was compared.In the otosclerosis group,the relationship between the findings of CT and the results of the audiometry was analyzed.Results The mean HU values in the area posterior and anterior to the oval window were significantly lower in the otosclerosis group than in the control group(the former: t=-2.030,P=0.046;the latter: Z=-4.979,P<0.01).The patients(30 cases in group A and 4 cases in group B) showed a conductive hearing loss[9 cases(30%) in group A and 1 case(25%) in group B] or a mixed hearing loss[21 cases(70%) in group A and 3 cases(75%) in group B].Conclusions The bone density around the oval window might change in patients with otosclerosis.

Reliability of high-resolution CT scan in diagnosis of otosclerosis

Abstract:Objective: To assess the reliability of high-resolution computed tomographic scan (HRCT scan) for the diagnosis of otosclerosis and to determine its usefulness to predict hearing deterioration and surgical difficulties. Study design: Prospective study. Setting: Tertiary reference center. Patients: Two hundred nine ears (200 patients) presenting progressive conductive hearing loss with normal tympanic membrane, abnormal stapedial reflex, and scheduled for stapes surgery. The mean age was 47.3 years. Intervention: All patients underwent HRCT scan before surgery (slice thickness of 0.6 to 1 mm). Stapedotomy was performed in 99% of cases. Main outcome measures: High-resolution computed tomographic scan results were categorized as positive, doubtful, or negative. We classified a CT scan as positive for otosclerosis when a hypodense focus was seen around the otic capsule. Preoperative and postoperative air- and bone-conduction thresholds were collected. Results: Of 209 HRCT scans, 84.2% were classified positive, 8.6% doubtful, and 7.2% negative. In all patients with positive CT scan, otosclerosis was confirmed in surgery. Among 15 negative cases, we found 4 minor malformations and 1 fracture of the stapes. Footplate incidents (mobilized, floating, or fractured footplate; 5.3%) occurred significantly more frequently when an HRCT scan was negative or doubtful (p = 0.05). Mean preoperative air-bone gap was 27.7 dB (standard deviation, 10). Mean postoperative air-bone gap was within 10 dB in 65% and within 20 dB in 92% of cases. Greater than 10 dB deterioration of bone-conduction thresholds occurred in 2% of cases. Mean preoperative and postoperative bone-conduction thresholds were significantly lower in cases of round window obliteration, pericochlear, or internal auditory canal hypodensities (p < 0.005 and p < 0.0001, respectively). Conclusion: In our series, the sensitivity of HRCT scan to otosclerosis was 95.1%. Hypodense otosclerotic foci were mostly localized at the anterior part of footplate. Negative or doubtful cases were associated with the highest incidence of stapes footplate complications. Foci involving otic capsule, internal auditory canal, or round window led to a significantly higher risk of sensorineural hearing loss.

Abstract:The aim of this study was to examine and assess the comparative values of HRCT-based multiplanar reformation (MPR), volume rendering (VR) and virtual endoscope built on three-dimensional shaded-surface display (SSD-based CTVE) for evaluations of the ossicular chain. The normal pure tone audiograms, type-A tympanogram, and normal HRCT characteristics of 32 human ears of 18 patients were reviewed, whose ossicular chains were reconstructed with the three aforementioned protocols and assessed via the 3-point scoring system. The HRCT-based protocols could demonstrate a 3D image of the ossicular chain, except that of the footplate on the SSD-based CTVE. On the qualitative assessment, the efficacy of the MPR and VR, which were both superior to the SSD-based CTVE (P < 0.05), presented no statistical significance among the major and/or hyperdense structures (P > 0.05). As regards the lateral process of the malleus, VR was found to be significantly superior to the MPR and SSD-based CTVE (P < 0.05), both of which, however, showed no significant comparative differences (P > 0.05). Moreover, the three protocols in terms of efficacy were comparatively different in their representations of the anterior crus and footplates of the stapes, respectively (P < 0.05). On the MPR images, not all the images of the lenticular process were ideal; 20 of 32 cases were detected, but not defined. VR could be the more valuable protocol for the 3D reconstruction of the ossicular chain and ought to be more employed in future, especially in education.
