Abstract:Background: Animal study found that soluble guanylate cyclase (sGC) plays an important role in development of hypertension (HT) by affecting the NO-sGC-CGMP signaling pathway. This study aims to evaluate the association of sGC with essential hypertension (EH) in the Han Chinese population.Methods: This case-control study included 2,012 hypertensive cases and 2,210 controls, and 6 tagging single nucleotide polymorphisms (SNPs) were selected (rs3806777, rs3806782, rs3796576 and rs7698460 in GUCY1A3, as well as rs2229202 and rs1459853 in GUCY1B3). Then the association of the six SNPs with EH was further evaluated in this study.Results: The results indicated that the A/A genotype of rs1459853 in GUCY1B3 was associated with higher HT risk, and the odds ratio (OR) of its recessive model was 1.191 (P=0.044). After adjusting for covariates, the association was still significant. Further stratification analyses showed that rs1459853 in non-drinking subjects and rs7698460 in women were associated with EH. In the follow-up study, rs1459853 were related to increased HT risk in men and smoker subjects. In adolescents, rs2229202 that in GUCY1B3 had significant association with prehypertension (Pre-HT), HT, and prehypertension with hypertension (Pre-HT + HT). After adjusted for covariates, the association was remaining significant. And in girls, rs3806782 was significantly connected with HT and Pre-HT + HT.Conclusions: Overall, our findings suggest that sGC may contribute to the genetic susceptibility to EH, and it was validated for the first time in adolescents.

Abstract:Most single nucleotide polymorphisms (SNPs) associated with human blood pressure (BP) are in noncoding regions of the genome and have small effect sizes in the general population. Functional studies in a controlled model system are essential to ascertain the effect of these noncoding loci on blood pressure. Such direct testing is challenging and largely absent, contributing to a critical gap between genetic discoveries and physiological understanding. We reasoned that physiological studies in genetically engineered and phenotypically permissive animal models would help to address this critical gap. We tested the hypothesis by studying the noncoding SNP rs1882961. The T allele of rs1882961, which has a frequency of approximately 30% in the European population, is associated with 0.23 mmHg higher systolic blood pressure in humans. We mapped the human rs1882961 SNP to the rat genome and used CRISPR/Cas9 to delete a 4-bp segment encompassed the rs1882961 orthologous site in the genome of the Dahl salt-sensitive (SS) rat (SS-Δrs1882961). Male SS-Δrs1882961 -/- rats exhibited significantly higher mean arterial pressure (MAP) and systolic and diastolic blood pressures (SBP and DBP) than wild-type littermates after the rats were fed a high-salt (4% NaCl) diet for 14 days (156.6 ± 5.9 vs 149.3 ± 5.1 mmHg for MAP, 179.4 ± 7.8 vs 171.9 ± 6.0 mmHg for SBP, and 133.2 ± 4.6 vs 125.1 ± 4.5 mmHg for DBP; n=7 for SS-Δrs1882961 -/- and 9 for wild-type, p<0.05). In contrast, MAP, SBP, DBP, and pulse pressure of female SS-Δrs1882961 -/- rats were significantly lower than wild-type littermates on a high-salt diet (139.8 ± 1.8 vs 149.5 ± 4.7 mmHg for MAP, 159.3 ± 2.5 vs 172.3 ± 6.0 mmHg for SBP, 116.8 ± 1.5 vs 124.5 ± 3.7 mmHg for DBP, and 42.5 ± 1.6 vs 47.7 ± 2.3 mmHg for pulse pressure; n=8 for SS-Δrs1882961 -/- and 7 for wild-type, p<0.05). The difference in systolic blood pressure reached more than 10 mmHg in female rats. Urinary albumin and protein levels of SS-Δrs1882961 -/- rats after 14 days on a high-salt diet tended to be higher in male and lower in female compared with wild-type littermates. The remarkable effects on BP as well as the sex-divergent changes resulted from a 4-bp noncoding segment deletion around rs1882961 orthologous site indicate that noncoding genomic segment may have profound impact on phenotypes on permissive genomic background and with informative stressors.
Genetic Predisposition and Salt Sensitivity in a Chinese Han Population: the EpiSS Study

Abstract:Objectives. Genome-wide association studies and candidate gene studies have found many single nucleotide polymorphisms (SNPs) that affect salt sensitivity (SS). We constructed a polygenic risk score (PRS) to estimate the joint effect of these SNPs on SS. Methods. We recruited 762 Chinese participants into the study. An unweighted PRS was constructed using 42 known genetic risk variants associated with SS or salt sensitivity blood pressure. A modified Sullivan’s acute oral saline load and diuresis shrinkage test was used to detect salt sensitivity. Logistic regression was used to estimate the joint effect of the SNPs on SS both overall and after stratification by hypertension. Results. The mean age of the participants was 57.1 years, and most of them were female (77.4%). The prevalence of SS was 28.7%. Both the continuous PRS and PRS tertiles were significantly associated with the risk of SS and a BP increase of more than 5 mmHg during acute salt loading but were not associated with a BP decrease of more than 10 mmHg during the diuresis shrinkage process. In the normotensive group, participants with PRSs in the middle and top tertiles had a more than twofold increased risk of SS (OR = 2.18, 95% CI: 1.15–4.12, P=0.016, and OR = 2.28, 95% CI: 1.19–4.38, P=0.013, respectively) compared with participants with PRSs in the first tertile. In the normotensive group, participants with PRSs in the middle tertile (OR = 1.94, 95% CI: 1.01–3.71, P=0.046) and top tertile (OR = 2.30, 95% CI: 1.19–4.44, P=0.013) had an increased risk of a greater than 5 mmHg increase in BP during acute salt loading than those with PRSs in the first tertile. In the hypertension group, neither the continuous PRS nor PRS tertile was significantly associated with the risk of SS. Conclusion. The 42 investigated SNPs were jointly and significantly associated with SS, especially in the normotensive Chinese population. These findings may provide genetic evidence for identifying target populations that would benefit from salt restriction policies.

Abstract:Most SNPs associated by GWAS with blood pressure (BP) phenotypes are located in noncoding regions of DNA. Investigating the physiological effects of these SNPs and their underlying mechanisms will advance our understanding of how genetic and epigenetic mechanisms regulate BP. The BP-associated SNP rs1173771 is in linkage disequilibrium (LD) with 10 other SNPs (r 2 >0.80) forming a haplotype spanning 17.5 kb of noncoding DNA. We explored the effects of the rs1173771 LD orthologous region on BP on the genetic background of Dahl salt-sensitive (SS) rats and the physiological and molecular mechanisms involved. A ~29.5 kb noncoding segment on rat chromosome 5 orthologous to the human rs1173771 LD region was identified using LiftOver. The model SS-rs1173771LD -/- was generated using CRISPR-Cas9 to delete this region in SS rats. BP was not significantly different between SS-rs1173771LD -/- rats and wildtype (WT) littermates on a 0.4% NaCl diet. After 14 days of a 4% NaCl diet, 24-hour mean arterial pressure (MAP) and systolic blood pressure (SBP) of SS-rs1173771LD -/- male rats were significantly lower compared with WT (149.1 ± 2.7 vs 155.9 ± 2.5 mmHg for MAP and 169.6 ± 4.3 vs 179.0 ± 3.0 mmHg for SBP). No difference in diastolic blood pressure was observed. No significant difference of BP was observed between genotypes in females. The transcription start sites of human natriuretic peptide receptor 3 (NPR3) and rat Npr3 are located ~129-kbp and ~80-kbp from the human rs1173771 LD region and the rat orthologous region, respectively. RNA-seq indicated that Npr3 expression was upregulated in the mesenteric artery in SS-rs1173771LD -/- rats. Vasodilation of mesenteric arteries in response to C-type natriuretic peptide was significantly enhanced in SS-rs1173771LD -/- male rats compared with WT littermates. A selective NPR-C antagonist abolished vasodilation in both groups, suggesting the vasorelaxation was NPR-C dependent. In conclusion, deletion of the rs1173771 LD orthologous region increased mesenteric artery vasorelaxation and attenuated salt-induced hypertension in male SS rats. Our findings provided evidence that evolutionarily conserved noncoding DNA regions located far from protein-coding genes can influence BP in vivo.

Abstract:Objective: Salt-sensitive hypertension (SSH) affects approximately half of the hypertensive population, increasing the risk of vascular complications. The underlying pathophysiological mechanisms of SSH remain complex and need to be fully elucidated. Our prior research has identified genetic factors contributing to the salt sensitivity of blood pressure (SSBP), particularly involving genes regulating volume and blood pressure. We also observed enhanced peripheral vascular response to angiotensin II in humans with salt-sensitive hypertension. Given the pivotal role of the angiotensin II receptor type-1 (AT1R or AGTR1) in blood pressure and intravascular volume regulation, we hypothesized a genetic association between AGTR1 and SSBP. Methods: Our study involved 240 individuals of European ancestry from the HyperPATH cohort, examined under restricted and high dietary salt conditions. We employed a tagging single nucleotide variant approach to genotype participants at AGTR1 . Results: Our regression model revealed a significant association between the rs2638355 (A/G) variant and salt-sensitive systolic blood pressure (SS-SBP), and rs2638355 increased AGTR1 gene expression. Notably, carriers of the risk-allele of the noncoding regulatory variant rs2638355 exhibited higher systolic blood pressure under high salt diet conditions than nonrisk allele individuals. A sex-stratified analysis showed this salt-driven effect on systolic blood pressure was significant only in females, underscoring the role of dietary salt in modulating genetic effects in this group. Furthermore, a restricted salt diet in these individuals diminished blood pressure and negated the blood pressure phenotype–genotype association. Conclusion: Overall, our findings could aid in pinpointing individuals with salt-sensitive blood pressure among hypertensive patients, especially considering dietary and sex-specific factors.

Abstract:Most Blood Pressure (BP)-associated single nucleotide polymorphisms (SNPs) are in intronic or intergenic regions. For the noncoding SNP rs28451064, allele A is associated with elevated diastolic BP as well as increased risk of myocardial infarction and angina pectoris. rs28451064 is mapped to the nearby protein coding gene KCNE2 and is an expression quantitative trait locus for RCAN1 . We performed a series of studies to experimentally identify genes influenced by rs28451064 and to begin to understand the mechanism. We differentiated a human induced pluripotent stem cell (iPSC) line to endothelial cells (iECs). The iECs were purified by sorting with VE-cadherin. Chromatin conformation analysis using Micro-C identified several genes including MRPS6 and SLC5A3 as sharing a chromatin loop with rs28451064. We used CRISPR-Cas9 to generate isogenic iPSC lines with either homozygous low BP (G) or high BP allele (A) of rs28451064. Three clones of iPSCs with each genotype were established by screening several hundred clones. The edited iPSCs were differentiated into iECs for RNA-seq analysis, which showed that several genes that are genomically close to rs28451064 tended to be differentially expressed, including KCNE2 , SMIM34 , and RCAN1 . The edited iPSC lines were subjected to six rounds of differentiation to iECs for targeted real-time PCR analysis of genes near rs28451064, including the genes mentioned above. The results indicated significantly increased expression of SMIMI11 (1.93±0.25 fold) and SMIM34 (1.45±0.13 fold) (n=34) and decreased expression of RCAN1 (0.90 ± 0.03 fold) and SLC5A3 (0.90 ± 0.03 fold) (n=36) in iECs with the high BP allele compared to the low BP allele of rs28451064 (p<0.05). SMIM11 and SMIM34 encode small integral membrane proteins with unknown functions. RCAN1 encodes regulator of calcineurin 1, and SLC5A3 encodes sodium/myo-inositol cotransporter 1. Interestingly, the closest genes, MRPS6 and KCNE2 , along with MRPL23 , a control gene on a different chromosome, were not significantly differentially expressed. In summary, we have shown definitively that rs28451064 influences the expression of several local genes in an allele-specific manner. The results provide new insights into the function of a BP-associated noncoding SNP.

Abstract:Dietary salt intake has significant effects on arterial blood pressure and the development of hypertension. Mechanisms underlying salt-dependent changes in blood pressure remain poorly understood. The goal of the present study was to utilize urine samples from the Dietary Approaches to Stop Hypertension (DASH)-Sodium trial and a targeted metabolomic approach to identify associations of urinary metabolites with blood pressure phenotypes on low- or high-sodium intake. We examined urinary levels of metabolites in 103 participants of the DASH-Sodium trial after 30 days on a defined diet containing high sodium (targeting 150 mmol sodium intake per day) or low sodium (50 mmol sodium per day). Systolic or diastolic pressure was 10 or more mmHg higher on the high-sodium intake in 51 of the 103 participants. Targeted chromatography/mass spectrometry analysis was performed in 24-h urine samples for 10 metabolites related to the tricarboxylic acid cycle and 47 amino metabolites. Urinary metabolite levels improved the prediction of classification of blood pressure salt-sensitivity based on race, age and sex. Random forest and generalized linear mixed model analyses identified significant (false discovery rate <0.05) associations between 24 h excretions of β-aminoisobutyric acid, cystine, citrulline, homocysteine and lysine and systolic blood pressure and between cystine and diastolic blood pressure. The differences of 24 h excretion of homocysteine between low- and high-sodium intakes were significantly associated with the differences in diastolic blood pressure. These associations were significant with or without considering demographic factors. Treatment with β-aminoisobutyric acid significantly attenuated high-salt-induced hypertension in Dahl salt-sensitive rats. These findings support the presence of new mechanisms of blood pressure regulation involving metabolic intermediaries.

Abstract:A 1.37Mbp region of the Brown Norway rat Chr13 confers protection from salt‐sensitivity to Dahl salt‐sensitive (SS) rats. Of five candidate genes identified in this region, only Rfwd2 was highly expressed in the renal medulla. A pathway‐based Bayesian graphical model analysis utilizing RNA‐seq and GeneChip data was used to identify relationships between Rfwd2 and genes in pathways related to blood pressure. GeneChip data was used to assess whether the genes with a high marginal posterior probability of interdependence with Rfwd2 were differentially expressed in the renal medulla of SS vs. salt‐resistant congenic SS.13BN26 rats fed 0.4% (LS) or 4% NaCl (HS) diet for 7‐days. Of the 9 genes with the highest absolute partial correlations with Rfwd2, 4 were quantifiable by GeneChip analysis and significantly different (P<0.05) between SS and SS.13BN26 rats: F8 (Factor VIII) (lower in SS on LS/HS); Csnk1d (Casein kinase 1 delta) (higher in SS rats on LS/HS); Pik3ca (Phosphatidylinositol‐4,5‐bisphosphate 3‐kinase) (lower in SS rats on LS, suppressed in both on HS diet); Acly (ATP citrate lyase) (increased with HS in both). These analyses have allowed the unbiased identification of genes associated with our candidate gene providing new insights into salt‐sensitivity.

Abstract:Previous analysis of 13 overlapping subcongenic strains led to the identification of a 1.37 Mbp region on chromosome 13 (positions 80.92 to 82.29 Mbp in the Rn5 genome assembly) that influenced the mean arterial blood pressure of the Dahl salt-sensitive (SS) rat on a high-salt diet by more than 20 mmHg. The goal of the present study was to identify biological pathways that could mediate the blood pressure effect of this genomic region. RNA-seq analysis was performed for the renal outer medulla tissue in five selected subcongenic strains, the SS, and a congenic strain from which the subcongenic strains were derived. Rats were fed a 0.4% salt diet or switched to a high-salt diet for 7 days. Affymetrix GeneChip data for SS and three additional congenic or consomic strains were obtained from a previous study. The RNA-seq and microarray data were merged using a cross-platform normalization method to generate a transcriptome dataset containing 90 observations for each gene. A Bayesian model analysis was performed for 243 biological pathways to assess their likelihood to discriminate blood pressure levels across experimental groups. Seven pathways showed posterior probabilities greater than 0.4. These pathways involved neuroactive ligand-receptor interaction, phenylalanine, tyrosine and tryptophan biosynthesis, and protein degradation. A Bayesian approach was used to estimate undirected graphical models among the three known genes located in the 1.37 Mbp region (Astn1, Fam5b, and Rfwd2) and genes in each of the 7 pathways identified above and 11 additional pathways known to be involved in blood pressure regulation. The analysis identified several previously unknown relationships between the three candidate genes and genes in pathways that could regulate blood pressure. The study demonstrated a new, unbiased approach for identifying biological pathways mediating the effect of a candidate genomic region on hypertension.

Abstract:Epigenetic modifications of the genome play a key role in the regulation of gene expression. It has been reported that epigenetic modifications of several genes are associated with hypertension. To investigate the potential role of genome-wide changes in DNA methylation in salt-induced hypertension, experiments were performed on inbred Dahl SS rats obtained from two colonies maintained at the Medical College of Wisconsin (i.e. MCWSS) and Charles River Laboratory (CRLSS). The colonies are genetically identical, but CRLSS rats were maintained on a whole grain diet containing 1% NaCl (CRLSS_LS) while MCWSS rats were fed casein-based AIN-76A chow containing 0.4% NaCl (MCWSS_LS) until both colonies were switched to an AIN-76A chow containing 4% NaCl for 14 days starting at 6 weeks of age (CRLSS_HS and MCWSS_HS, respectively). Mean arterial pressure and albumin excretion rate in MCWSS_HS rats were significantly greater (142±14 mmHg and 100±16 mg/day, n=6) than in CRLSS_HS rats (118±2 mmHg and 20±2 mg/day, n=7). Reduced representation bisulfite genome sequencing (RRBS) measured 5-Methylcytosine levels at single-base resolution in the renal outer medulla in the above groups, each with four biological replicates. For genomic regions located within CpG islands (CGI’s) and exhibiting differential methylation between LS and HS in each colony, HS diet increased median methylation levels several-fold in both MCWSS (7.45% vs. 0.35% for MCWSS_HS vs. MCWSS_LS, respectively, p = 2.84E-31) and CRLSS rats (7.62% vs. 1.21% for CRLSS_HS vs. CRLSS_LS, respectively, p = 1.65E-32). For genomic regions exhibiting differential methylation between MCWSS and CRLSS, MCWSS_HS rats (which exhibited higher blood pressure) had higher median methylation levels than CRLSS_HS rats (7.56% vs. 2.75%, p = 2.12E-9). We observed 156 hypermethylated and 241 hypomethylated regions within CGI’s of MCWSS_LS compared to CRLSS_LS. Examples of differentially methylated genes include the serine protease Prss2 , the transcription factor E2f1 , and the matrix protein Spock2 . These results suggest that sodium-dependent and independent dietary components could induce changes in DNA methylation that may predispose and participate in the development of hypertension and renal damage.

Abstract:The goal of the present study was to narrow a 1.37 Mbp region containing five genes on chromosome 13 (positions 80.92-82.29 Mbp in the Rn5 genome assembly) that we have previously found to influence the mean arterial pressure (MAP) by at least 25 mmHg in SS rats fed a high salt diet (8.0% NaCl for 14 days). The creation of 6 overlapping subcongenic strains that cover this region identified a 0.71 Mbp region (positions 81.01-81.72 Mbp) in which substitution of SS alleles with BN alleles reduced salt-induced hypertension in congenic SS rats by nearly 30 mmHg and significantly reduced urinary albumin excretion (UalbV) (see figure below; n=10-15/strain; * significant difference from SS; p<0.05). Analysis of the narrow candidate region revealed the presence of two protein-coding genes ( Pappa2 and Astn1 ) and a microRNA ( miR488 ), none of which are known to be mechanistically involved in hypertension. Pappa2 mRNA in these rat strains fed 0.4% NaCl diet was expressed at 6-10 fold higher levels in the renal cortex of the salt-resistant congenic strains with the BN allele (26-N, -O, and [[Unable to Display Character: –]]P) compared to strains with the SS allele (26-Q, -R, and [[Unable to Display Character: –]]S). A Pappa2 coding sequence variant of unknown functional importance was identified. Immunohistochemistry and fluorescence in situ hybridization studies localized Pappa2 to intercalated cells of the cortical and medullary collecting duct. Astn1 and miR-488 were not differentially expressed in renal tissue. Together, these findings point towards Pappa2 as a candidate gene for salt-induced hypertension in SS rats.

Abstract:BACKGROUND: Hypertension is a key risk factor for major adverse cardiovascular events but remains difficult to treat in many individuals. Dietary interventions are an effective approach to lower blood pressure (BP) but are not equally effective across all individuals. BP is heritable, and genetics may be a useful tool to overcome treatment response heterogeneity. We investigated whether the genetics of BP could be used to identify individuals with hypertension who may receive a particular benefit from lowering sodium intake and boosting potassium levels. METHODS: In this observational genetic study, we leveraged cross-sectional data from up to 296 475 genotyped individuals drawn from the UK Biobank cohort for whom BP and urinary electrolytes (sodium and potassium), biomarkers of sodium and potassium intake, were measured. Biologically directed genetic scores for BP were constructed specifically among pathways related to sodium and potassium biology (pharmagenic enrichment scores), as well as unannotated genome-wide scores (conventional polygenic scores). We then tested whether there was a gene-by-environment interaction between urinary electrolytes and these genetic scores on BP. RESULTS: Genetic risk and urinary electrolytes both independently correlated with BP. However, urinary sodium was associated with a larger BP increase among individuals with higher genetic risk in sodium- and potassium-related pathways than in those with comparatively lower genetic risk. For example, each SD in urinary sodium was associated with a 1.47–mm Hg increase in systolic BP for those in the top 10% of the distribution of genetic risk in sodium and potassium transport pathways versus a 0.97–mm Hg systolic BP increase in the lowest 10% ( P =1.95×10 −3 ). This interaction with urinary sodium remained when considering estimated glomerular filtration rate and indexing sodium to urinary creatinine. There was no strong evidence of an interaction between urinary sodium and a standard genome-wide polygenic score of BP. CONCLUSIONS: The data suggest that genetic risk in sodium and potassium pathways could be used in a precision medicine model to direct interventions more specifically in the management of hypertension. Intervention studies are warranted.

Abstract:Intermediary metabolism, including renal metabolism, is increasingly recognized as an important contributor to the regulation of blood pressure and the development of hypertension, a leading risk factor for disease burden worldwide. Dietary salt intake has significant effects on blood pressure. We test the hypothesis that human blood pressure on low- or high-sodium intake is associated with divergent changes of serum and urinary metabolites. A gas chromatography and mass spectrometry analysis of 192 serum samples from the Dietary Approaches to Stop Hypertension 2 (DASH2 or DASH-Sodium) trial detected 384 metabolites and identified 116. Serum levels of citraconic acid, 1-monostearin and serotonin were associated with systolic or diastolic blood pressure on low- or high-sodium intake after adjustment for covariates (FDR < 0.05). Serum levels of 2-ketoisocaproic acid, glyceric acid, 1-monostearin, ornithine, and β-aminoisobutyric acid were associated with blood pressure differences between low- and high-sodium intakes (FDR < 0.05). An integrated analysis of 33 amino compounds and tricarboxylic acid cycle intermediates indicated that serum and urinary levels were not significantly correlated (FDR < 0.05) for the majority of these metabolites. Blood pressure appeared to be associated with serum, but not urinary, levels of leucine and cystine and urinary, but not serum, levels of β-aminoisobutyric acid. These novel findings support a role for several serum metabolites and renal handling of metabolites in the regulation of human blood pressure on low- or high-sodium intake.
Genetic Variants at PSMD3 Interact with Dietary Fat and Carbohydrate to Modulate Insulin Resistance

Abstract:PSMD3 encodes subunit 3 of the 26S proteasome, which is involved in regulating insulin signal transduction, and dietary factors could potentially regulate the function of this gene. We aimed to investigate the associations of PSMD3 variants with glucose-related traits and the interactions of those variants with dietary fat and carbohydrate for glucose-related traits in the Genetics of Lipid Lowering Drugs and Diet Network (GOLDN) study and to replicate the findings in the Boston Puerto Rican Health Study (BPRHS). Ten single nucleotide polymorphisms (SNPs) were selected, covering 90% the genetic variations in or near PSMD3. Minor allele (C) carriers of rs4065321 had higher homeostasis model assessment of insulin resistance (HOMA-IR) than noncarriers in males of both the GOLDN (P = 0.022) and BPRHS (P = 0.036). Minor allele (T) carriers of rs709592 had significantly higher HOMA-IR (P = 0.032) than C homozygotes in the GOLDN, whereas the T allele carriers of rs709592 tended to have higher HOMA-IR (P = 0.08) than C homozygotes in the BPRHS. In the GOLDN, there was an interaction between rs709592 and dietary carbohydrate on HOMA-IR (P = 0.049). Subjects carrying the T allele of rs709592 had higher HOMA-IR compared only with noncarriers with low carbohydrate intake (≤49.1% energy; P = 0.004). SNPs rs4065321 and rs709592 both significantly interacted with dietary MUFAs and carbohydrate on glucose concentrations in the GOLDN. Our study suggests that PSMD3 variants are associated with insulin resistance in populations of different ancestries and that these relationships may also be modified by dietary factors.

Abstract:Essential hypertension (EH) is a major risk factor for cardiovascular disorders, the leading cause of death in the United States. Given its great public health impact, it is crucial to understand the genetic basis of EH. EH is highly heterogeneous and to use an intermediate phenotype of EH, sodium lithium countertransport (SLC), will provide substantial advantage for disease genes discovery. We proposed two approaches to explore the genes for SLC.The first study examined the relationship between SLC and a positional candidate gene, SLC34A2, which is linked to SLC in baboon. We sequenced gene SLC34A2 in baboon and human. Strong homology was established in exonic organization and sequence between human and baboon SLC34A2 genes and extensive variation in both species was identified. Association studies between SLC and SLC34A2 were carried out in 1856 RFHS phase II individuals and 634 baboons. Significant association of SLC with human SNP rs3775909 (p=0.03) in SLC34A2 and haplotype block 2 (p<0.005) were observed. Strong evidence for association of SLC with SLC34A2 was found for baboon SNP Asn136Asn (p=0.0001). Consistent findings in two different species implied that SLC34A2 may be one of the genes involved in SLC. However, linkage analyses conditional on genotypes of baboon Asn136Asn suggest that Asn136Asn is not the primarily functional site for SLC. We conclude that SLC34A2 is associated with SLC, though it may not be the major effect gene.In second study, we integrated gene expression micrarray with linkage analysis to search for genes for SLC. Two independent microarrays (U133A and U133_plus_2.0) were used to identify the differentially expressed genes in high verse low SLC groups. Five genes, IER3, ARHGAP15, CD47, CDKAL1 and PRKRA, were among top 1% of differentially expressed genes in both arrays and also mapped to linkage region for SLC in RFHS Phase II population. A follow-up association study for IER3 shows that SNP rs8512 is significantly associated with SBP (p=0.002) and DBP (p=0.0008), and SNP rs2284174 has marginal association with SLC (p=0.055) and SBP (p=0.085). In conclusion, we identified some interesting susceptible genes for SLC by combining gene expression profiling and linkage study.

Abstract:Introduction: Salt Sensitivity (SS) of blood pressure (BP) is an independent predictor of death due to cardiovascular disease, but its pathogenesis is poorly understood. Sodium (Na + ) is stored in the skin and muscle interstitium. This hyperosmolar Na + activates monocytes in vitro via oxidative stress with generation of isolevuglandin (isoLG) protein adducts that are immunogenic and activate the adaptive immune system. Methods: Five subjects with essential hypertension discontinued all anti-hypertensive therapy for two weeks before the study. SS was assessed by an inpatient protocol of salt loading (460 mmoL/24h) and salt depletion (10 mmoL/24h, plus furosemide 40 mg x 3). Muscle and skin Na + contents were measured at baseline (BA) by 23 Sodium magnetic resonance imaging ( 23 NaMRI). Urine and serum electrolytes, glomerular filtration rate and the % CD14 + monocytes containing isoLG adducts using flow cytometry were obtained at BA, after salt-loading (HI) and after salt-depletion (LO). All continuous data are displayed as median (interquartile range). Spearman’s correlation was used to test associations. Results: Median age was 54 years (44-55), 60% of subjects were female, screening systolic BP (SBP) was 140 mmHg (134-148), diastolic BP was 88 mmHg (84-99) and BMI was 35 kg/m 2 (30-39). SBP response to salt-depletion (salt-sensitivity index, SSI) varied from -13.8 to +1.8 mmHg. %isoLG + CD14 + cells were 48 (27-65) at BA, 55 (31-56) at HI, and 70 (33-72) at LO (p=0.594, by the Kruskal-Wallis test). The correlation between SSI and delta (Δ) %isoLG LO minus HI, was 0.86, [95% confidence interval (CI), -0.07-0.99] which may suggest conclusively as we gather more data that the greater the SSI, the larger the decrease in isoLGs by salt depletion. Muscle Na + content correlated with 24h urine Na + (BA) (r=0.90, 95% CI, 0.11-0.99), however, the correlation with BP, SSI or isoLGs was inconclusive, potentially due to the small sample size. Skin Na + content correlated with baseline %CD14IsoLG + (r=0.91; 95% CI, 0.12-0.99). Conclusions: Na + intake is a component of the determinants of muscle Na + . Skin Na + is associated with increased isoLGs in monocytes, a marker of immune cell activation. Variability in ΔCD14isoLG may serve as a biomarker for SS of BP in humans.

Genetic Variation in Sodium-glucose Cotransporter 2 and Heart Failure

Abstract:Inhibition of sodium-glucose cotransporter 2 (SGLT2) represents an emerging pharmaceutical approach for the treatment of heart failure. The mechanisms by which SGLT2 inhibitors reduce the risk of heart failure are not well understood. The objective of this study was to investigate the association between single nucleotide polymorphisms (SNPs) in the SLC5A2 gene, encoding SGLT2, and heart failure, and to assess potential mediators of this association. Regression and mediation analyses were conducted with individual participant data of the UK Biobank (n = 416,737) and validated in the cardiovascular high-risk cohort of the LUdwigshafen RIsk and Cardiovascular Health study (LURIC; n = 3316). Two intronic SNPs associated with SLC5A2 expression were included in a genetic score, which was associated with lower risk of heart failure in UK Biobank (odds ratio 0.97, 95% confidence interval, 0.95-0.99, P = 0.016). This association was also present in participants without type 2 diabetes or coronary artery disease (CAD). The associations of the genetic score with HbA1c, high-density lipoprotein cholesterol, uric acid, systolic blood pressure, waist circumference, and body composition mediated 35% of the effect of the score on heart failure risk. No associations of the genetic SGLT2 score with atherosclerotic cardiovascular disease outcomes or markers of volume status were observed, which was confirmed in the LURIC study. Variations in the gene encoding SGLT2 were associated with the risk of prevalent or incident heart failure. This association was mediated by several mechanisms and did not depend on the presence of type 2 diabetes or previous CAD events.
Sex Differences in Gut Microbiota Fatty Acid Metabolism in Human Salt Sensitivity of Blood Pressure

Abstract:Salt sensitivity is a trait in which high dietary sodium (Na+) intake causes an increase in blood pressure. We previously demonstrated that in the gut, elevated dietary Na+ causes dysbiosis. The mechanistic interplay between excess dietary Na+-induced alteration in the gut microbiome and sex differences is less understood. The goal of this study was to identify novel metabolites in salt sensitivity and sex differences in response to a high dietary Na+ intake. We hypothesized that excess dietary salt-induced dysbiosis is associated with distinct metabolomic changes in men and women. We performed plasma metabolomics analysis and measured blood pressure of human volunteers who completed a validated 3-day food record to estimate dietary Na+ intake. Based on the recommendations by the American Heart Association, we classified daily Na+ intake of <2.3g as normal salt and high salt for subjects consuming ≥ 2.3g Na+. Spearman correlation was used to assess the relationship between Na+ intake and blood pressure. We also performed RNA sequencing on human (N=11) monocytes treated with high salt in vitro. The relationship between dietary Na+ intake and mean arterial pressure was different in women (r=0.3422, p=0.0022, N=81) and men (r=0.4552, p=0.0010, N=49). Network analysis of human plasma untargeted metabolome revealed fatty acids as top subnetworks differentially changed with salt intake. In subjects consuming a high Na+ diet, levels of linoleate (1.211 ± 0.330 vs. 0.869 ± 0.170; p<0.001); palmitate (1.155 ± 0.292 vs. 0.924 ± 0.233; p= 0.003); arachidonic acid (1.119 ± 0.242 vs. 0.965 ± 0.201; p=0.015); and 12-hydroxyeicosatetraenoic acid (1.329 ± 0.925 vs. 0.902 ± 0.520; p=0.0469) were higher in women (28) than men (25). In contrast, we observed no sex differences in these parameters between men and women consuming a normal salt diet. RNA sequencing analysis revealed that exposure of human monocytes to high salt in vitro upregulated transcription of fatty acid receptors and arachidonic acid-related genes. Specifically, ALOX12 (277.8 ± 13.8 vs. 169.5 ± 16.7, p<0.0001) which encodes for arachidonate 12-lipoxygenase, ALOX5AP (6459 ± 1553 vs. 2598 ± 30, p=0.0310) which encodes for 5-lipoxygenase activating protein, and FFR2 (418.1 ± 97.8 vs. 115.9 ± 29.4, p=0.0078) which encodes for free fatty acids receptor 2 were increased in human monocytes, and this was associated with a pro-inflammatory phenotype. These findings suggest that excess dietary Na+ is associated with fatty acid metabolism dysregulation, and that the arachidonic acid pathway may potentially play a role in sex differences of salt-sensitive hypertension. Abstract:Objective: Arterial hypertension (AHT) is a multifactorial pathology in which environmental and genetic factors interact leading to the development of this pathology. The genetic influence on the onset of AHT has been addressed in candidate gene studies. Polymorphisms that interfere with sodium and water balance are described in the literature as associated with increased risk of AHT. However, there are few studies that study the synergism between them in the risk of AHT. Objective: Evaluate whether there is a synergistic effect between two polymorphisms that interfere with sodium and water balance, alpha adducin (ADD1 G460W rs4961) and sodium epithelial channel (SCNN1G G(-173)A rs5718), in the increased risk of AHT. Design and method: A case-control study was conducted with a population of 1712 individuals (mean age of 51.0±7.9), depending on whether or not they had AHT. We obtained two groups: 860 hypertensive patients and 852 controls. All subjects collected blood for analytical tests and DNA for genetic analysis. The polymorphisms rs4961 and rs5718 were evaluated in both groups. The Odds Ratio (OR) with 95% confidence intervals were calculated. Data analysis was performed using SPSS for Windows, version 25.0. The significance threshold was set at p<0.05. Results: The WW genotype of ADD1 was more frequent in the hypertensive group than in the control group (OR 2.76; 95%CI:1.38-5.55; p=0.004). The GG genotype of SCNN1G was not statistically significant between both groups (OR 0.95; 95%CI: 0.78-1.16; p=0.597). However, when we associate the two genetic variants, there is an increased risk of AHT, with an OR of 7.68 (95% CI 1.75-33.73) with statistical significance (p=0.007). Conclusions: With our results, we can conclude that the alpha adducin polymorphism (ADD1 G460W) is a statistically significant risk factor for AHT, and the sodium epithelial channel polymorphism (SCNN1G G(-173)A) alone is not associated with AHT onset. However, when together, these two polymorphisms increase the risk of AHT to na OR of 7.7. These results lead us to conclude that AHT is not explained in a simple way with a single polymorphism, since there are synergistic effects between genes that favor the development of AHT.

Abstract:Salt sensitivity of blood pressure is an independent cardiovascular risk factor as powerful as hypertension, yet it goes untreated due to the lack of a feasible diagnostic tool. We previously found that sodium enters antigen-presenting cells (APCs) via ENaC, leading to the formation of isolevuglandins (IsoLGs). IsoLGs are highly reactive oxidative products of fatty acids and act as neoantigens to trigger inflammation in hypertension. We found that activation of APCs via IsoLG is highly variable in humans. Thus, we hypothesized that this variability in IsoLG-mediated APC activation contributes to the variability observed in human salt sensitivity of blood pressure and that IsoLGs can be used as a diagnostic tool. We measured systolic blood pressure (SBP) and IsoLG-containing APCs (dendritic cells [DC], classical, intermediate and non-classical monocytes) by flow cytometry in 14 hypertensive subjects who were off therapy for 2 weeks, before (B) and after in-patient 24 hr salt loading (HI, 460 mmol Na + ) and salt depletion (LO, 10 mmol Na + and furosemide 40 mg x 3). Salt sensitivity was assessed by ΔSBP (LO minus HI). Median age was 49, with 5 females, median BMI (IQR) of 29.8 (25.2, 39.5) kg/m 2 and screening SBP of 135.1 (130, 144.9) mmHg. The median urinary Na + excretions (IQR) were 169.6 (125, 203.3), 360.9 (324.1, 421.2) and 54.8 (36.8, 76) mmol/24h at baseline, HI and LO days, respectively. Baseline percentages of classical and non-classical monocytes with IsoLGs were positively correlated with ΔSBP (r=0.64, p=0.01 and r=0.71, p<0.01 respectively). Changes in IsoLG-containing DCs and nonclassical monocytes positively correlated with ΔSBP (r=0.60, p=0.03 and r=0.69, p=0.01 respectively). Both the baseline levels and changes in IsoLG containing APCs associated with salt sensitivity. Greater decrease in IsoLG-containing APCs predicted greater decline in blood pressure in response to salt depletion, suggesting that oxidative stress in APCs is implicated in the pathogenesis of salt sensitivity. Our results further suggest that baseline percentage of IsoLG-containing APCs may be the first predictor of salt sensitivity that does not require a protocol of salt loading and depletion. This would enable the diagnosis of salt sensitivity in clinic practice.
