Abstract:Background An early and accurate diagnosis is crucial to provide optimal patient care in neurodegenerative diseases. Although an EEG shows advantages in availability and cost compared to the current diagnostic tools, it is not routinely used in clinical practice. Previous reviews have either focused on single disease populations and/or solely on resting state EEG. To evaluate the utility of EEG for early diagnosis and differential diagnosis, we conducted a systematic review across Alzheimers disease (AD), Frontotemporal Dementia (FTD) and Lewy Body Dementia (DLB). Methods We searched databases Pubmed, Cochrane, Web of Science, and Scopus for articles published from 2000 to 2023 investigating resting-state and task-based EEG-markers in biomarker-proven AD, FTD and DLB. Results Our search yielded a total of 12010 studies, of which 71 papers were eligible: 34 on AD, 18 on DLB and 9 on FTD. Slowing of the frequency spectrum was a common observation across diseases, achieving excellent sensitivity in AD and DLB. Research on FTD was limited and with varying results in the discrimination from healthy controls, although connectivity analysis and microstates are promising avenues. In differential diagnosis, both spectral and connectivity metrics show encouraging results. Task-based EEG emerges as a promising tool in early AD. Conclusion EEG shows promise as a cost-effective, non-invasive tool for early detection and differential diagnosis. Future research should aim to collect standardized data from multicentric cohorts, across multiple diseases and stages, and explore the neural underpinnings of these diseases, to improve interpretability of the findings.

Abstract:Background and Objectives Mild Cognitive Impairment (MCI) and Alzheimer's Disease (AD) are progressive neurological disorders that significantly impair the cognitive functions, memory, and daily activities. They affect millions of individuals worldwide, posing a significant challenge for its diagnosis and management, leading to detrimental impacts on patients' quality of lives and increased burden on caregivers. Hence, early detection of MCI and AD is crucial for timely intervention and effective disease management. Methods This study presents a comprehensive systematic review focusing on the applications of deep learning in detecting MCI and AD using electroencephalogram (EEG) signals. Through a rigorous literature screening process based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, the research has investigated 74 different papers in detail to analyze the different approaches used to detect MCI and AD neurological disorders. Results The findings of this study stand out as the first to deal with the classification of dual MCI and AD (MCI+AD) using EEG signals. This unique approach has enabled us to highlight the state-of-the-art high-performing models, specifically focusing on deep learning while examining their strengths and limitations in detecting the MCI, AD, and the MCI+AD comorbidity situations. Conclusion The present study has not only identified the current limitations in deep learning area for MCI and AD detection but also proposes specific future directions to address these neurological disorders by implement best practice deep learning approaches. Our main goal is to offer insights as references for future research encouraging the development of deep learning techniques in early detection and diagnosis of MCI and AD neurological disorders. By recommending the most effective deep learning tools, we have also provided a benchmark for future research, with clear implications for the practical use of these techniques in healthcare.

Abstract:Alzheimer disease (AD) remains a significant global health concern. The progression from preclinical stages to overt dementia has become a crucial point of interest for researchers. This paper reviews the potential of neurophysiological biomarkers in predicting AD progression, based on a systematic literature search following PRISMA guidelines, including 55 studies. EEG-based techniques have been predominantly employed, whereas TMS studies are less common. Among the investigated neurophysiological measures, spectral power measurements and event-related potentials-based measures, including P300 and N200 latencies, have emerged as the most consistent and reliable biomarkers for predicting the likelihood of conversion to AD. In addition, TMS-based indices of cortical excitability and synaptic plasticity have also shown potential in assessing the risk of conversion to AD. However, concerns persist regarding the methodological discrepancies among studies, the accuracy of these neurophysiological measures in comparison to established AD biomarkers, and their immediate clinical applicability. Further research is needed to validate the predictive capabilities of EEG and TMS measures. Advancements in this area could lead to cost-effective, reliable biomarkers, enhancing diagnostic processes and deepening our understanding of AD pathophysiology.

An Automated Approach for the Detection of Alzheimer's Disease From Resting State Electroencephalography

Abstract:Early detection is crucial to control the progression of Alzheimer's disease and to postpone intellectual decline. Most current detection techniques are costly, inaccessible, or invasive. Furthermore, they require laborious analysis, what delays the start of medical treatment. To overcome this, researchers have recently investigated AD detection based on electroencephalography, a non-invasive neurophysiology technique, and machine learning algorithms. However, these approaches typically rely on manual procedures such as visual inspection, that requires additional personnel for the analysis, or on cumbersome EEG acquisition systems. In this paper, we performed a preliminary evaluation of a fully-automated approach for AD detection based on a commercial EEG acquisition system and an automated classification pipeline. For this purpose, we recorded the resting state brain activity of 26 participants from three groups: mild AD, mild cognitive impairment (MCI-non-AD), and healthy controls. First, we applied automated data-driven algorithms to reject EEG artifacts. Then, we obtained spectral, complexity, and entropy features from the preprocessed EEG segments. Finally, we assessed two binary classification problems: mild AD vs. controls, and MCI-non-AD vs. controls, through leave-one-subject-out cross-validation. The preliminary results that we obtained are comparable to the best reported in literature, what suggests that AD detection could be automatically detected through automated processing and commercial EEG systems. This is promising, since it may potentially contribute to reducing costs related to AD screening, and to shortening detection times, what may help to advance medical treatment.

Abstract:Alzheimer's disease (AD) is a neurodegenerative disease characterized by dementia and mental illness. The main pathological changes of AD are the accumulation of amyloid β (Aβ) and phosphorylated tau. With the increase in the aging population, the prevalence among adults aged ≥60 years will be growing from 5.4% (2015) to 6.7% (2050) in China.<a class="ejp-citation-link js-ejp-citation-link">[1]</a> At the present stage, the diagnosis of AD is primarily based on the long term clinical observation. Neuro-imaging and invasive cerebrospinal fluid (CSF) measurement of Aβ and tau may help making a differential diagnosis, but the cost to conduct these examinations is high and they are only applicable for the middle to late stages of the disease.<a class="ejp-citation-link js-ejp-citation-link">[2]</a> Furthermore, there is still no effective treatment to reverse the clinical symptoms and the pathological changes of AD because most of the patients when they are seen by specialists have already lost the majority of AD-related cortical and hippocampal neurons.<a class="ejp-citation-link js-ejp-citation-link">[3]</a> Therefore, the early detection of AD is of importance in order to monitor the AD progression and provide early intervention of the disease. Accordingly, there is a great need to develop low-cost, noninvasive measurement for the early detection of AD. Among many recent research advances, electroencephalography (EEG) has become an effective measurement aiding AD diagnosis and monitoring the disease progression. It has been reported that sleep architecture alteration, decreased θ and δ power could manifest before AD neuropathology.<a class="ejp-citation-link js-ejp-citation-link">[4]</a> In this review, we update the recent findings on EEG spectrum analysis, sleep architecture measurement, brain networks research, and their correlation with AD disease progression. We also discuss the potential application of EEG and sleep monitoring in the research of AD pathogenesis, and use them to evaluate the possibility of early AD detection.

Abstract:BACKGROUND:The Alzheimer's disease (AD) population increases worldwide, placing a heavy burden on the economy and society. Presently, there is no cure for AD. Developing a convenient method of screening for AD and mild cognitive impairment (MCI) could enable early intervention, thus slowing down the progress of the disease and enabling better overall disease management.METHODS:In the current study, resting-state electroencephalography (EEG) data were acquired from 113 normal cognition (NC) subjects, 116 amnestic MCI patients, and 72 probable AD patients. After preprocessing by an automatic algorithm, features including spectral power, complexity, and functional connectivity were extracted, and machine-learning classifiers were built to differentiate among the 3 groups. The classification performance was evaluated from multiple perspectives, including accuracy, specificity, sensitivity, area under the curve (AUC) with 95% confidence intervals, and compared to the empirical chance level by permutation tests.RESULTS:The analysis of variance results (P<0.05 with false discovery rate correction) confirmed the tendency to slow brain activity, reduced complexity, and connectivity with AD progress. By combining the features, the ability of the machine-learning classifiers, especially the ensemble trees, to differentiate among the 3 groups, was significantly better than that of the empirical chance level of the permutation test. The AUC of the classifier with the best performance was 80.08% for AD vs. NC, 70.82% for AD vs. MCI, and 63.95% for MCI vs. NC.CONCLUSIONS:The current study presented a fully automatic procedure that could significantly distinguish NC, MCI, and AD subjects via resting-state EEG signals. The study was based on a large data set with evidence-based medical diagnosis and provided further evidence that resting-state EEG data could assist in the discrimination of AD patients.

Abstract:IntroductionDementia in its various forms represents one of the most frightening emergencies for the aging population. Cognitive decline—including Alzheimer's disease (AD) dementia—does not develop in few days; disease mechanisms act progressively for several years before clinical evidence.MethodsA preclinical stage, characterized by measurable cognitive impairment, but not overt dementia, is represented by mild cognitive impairment (MCI), which progresses to—or, more accurately, is already in a prodromal form of—AD in about half cases; people with MCI are therefore considered the population at risk for AD deserving special attention for validating screening methods.ResultsGraph analysis tools, combined with machine learning methods, represent an interesting probe to identify the distinctive features of physiological/pathological brain aging focusing on functional connectivity networks evaluated on electroencephalographic data and neuropsychological/imaging/genetic/metabolic/cerebrospinal fluid/blood biomarkers.DiscussionOn clinical data, this innovative approach for early diagnosis might provide more insight into pathophysiological processes underlying degenerative changes, as well as toward a personalized risk evaluation for pharmacological, nonpharmacological, and rehabilitation treatments.

Abstract:Background: The growing number of older adults in recent decades has led to more prevalent geriatric diseases, such as strokes and dementia. Therefore, Alzheimer's disease (AD), as the most common type of dementia, has become more frequent too. Background: Objective: The goals of this work are to present state-of-the-art studies focused on the automatic diagnosis and prognosis of AD and its early stages, mainly mild cognitive impairment, and predicting how the research on this topic may change in the future. Methods: Articles found in the existing literature needed to fulfill several selection criteria. Among others, their classification methods were based on artificial neural networks (ANNs), including deep learning, and data not from brain signals or neuroimaging techniques were used. Considering our selection criteria, 42 articles published in the last decade were finally selected. Results: The most medically significant results are shown. Similar quantities of articles based on shallow and deep ANNs were found. Recurrent neural networks and transformers were common with speech or in longitudinal studies. Convolutional neural networks (CNNs) were popular with gait or combined with others in modular approaches. Above one third of the cross-sectional studies utilized multimodal data. Non-public datasets were frequently used in cross-sectional studies, whereas the opposite in longitudinal ones. The most popular databases were indicated, which will be helpful for future researchers in this field. Conclusions: The introduction of CNNs in the last decade and their superb results with neuroimaging data did not negatively affect the usage of other modalities. In fact, new ones emerged.

Abstract:Alzheimer’s Disease (AD) is a neurogenerative disorder and the most common type of dementia with a rapidly increasing world prevalence. In this paper, the ability of several statistical and spectral features to detect AD from electroencephalographic (EEG) recordings is evaluated. For this purpose, clinical EEG recordings from 14 patients with AD (8 with mild AD and 6 with moderate AD) and 10 healthy, age-matched individuals are analyzed. The EEG signals are initially segmented in nonoverlapping epochs of different lengths ranging from 5 s to 12 s. Then, a group of statistical and spectral features calculated for each EEG rhythm (δ, θ, α, β, and γ) are extracted, forming the feature vector that trained and tested a Random Forests classifier. Six classification problems are addressed, including the discrimination from whole-brain dynamics and separately from specific brain regions in order to highlight any alterations of the cortical regions. The results indicated a high accuracy ranging from 88.79% to 96.78% for whole-brain classification. Also, the classification accuracy was higher at the posterior and central regions than at the frontal area and the right side of temporal lobe for all classification problems.

Abstract:Mild cognitive impairment (MCI) is often considered a precursor to Alzheimer's disease (AD) and early diagnosis may help improve treatment effectiveness. To identify accurate MCI biomarkers, researchers have utilized various neuroscience techniques, with electroencephalography (EEG) being a popular choice due to its low cost and better temporal resolution. In this scoping review, we analyzed 2310 peer-reviewed articles on EEG and MCI between 2012 and 2022 to track the research progress in this field. Our data analysis involved co-occurrence analysis using VOSviewer and a Patterns, Advances, Gaps, Evidence of Practice, and Research Recommendations (PAGER) framework. We found that event-related potentials (ERP), EEG, epilepsy, quantitative EEG (QEEG), and EEG-based machine learning were the primary research themes. The study showed that ERP/EEG, QEEG, and EEG-based machine learning frameworks provide high-accuracy detection of seizure and MCI. These findings identify the main research themes in EEG and MCI and suggest promising avenues for future research in this field.

Abstract:Background Electroencephalogram (EEG) has emerged as a non-invasive tool to detect the aberrant neuronal activity related to different stages of Alzheimer’s disease (AD). However, the effectiveness of EEG in the precise diagnosis and assessment of AD and its preclinical stage, amnestic mild cognitive impairment (MCI), has yet to be fully elucidated. In this study, we aimed to identify key EEG biomarkers that are effective in distinguishing patients at the early stage of AD and monitoring the progression of AD. Methods A total of 890 participants, including 189 patients with MCI, 330 patients with AD, 125 patients with other dementias (frontotemporal dementia, dementia with Lewy bodies, and vascular cognitive impairment), and 246 healthy controls (HC) were enrolled. Biomarkers were extracted from resting-state EEG recordings for a three-level classification of HC, MCI, and AD. The optimal EEG biomarkers were then identified based on the classification performance. Random forest regression was used to train a series of models by combining participants’ EEG biomarkers, demographic information (i.e., sex, age), CSF biomarkers, and APOE phenotype for assessing the disease progression and individual’s cognitive function. Results The identified EEG biomarkers achieved over 70% accuracy in the three-level classification of HC, MCI, and AD. Among all six groups, the most prominent effects of AD-linked neurodegeneration on EEG metrics were localized at parieto-occipital regions. In the cross-validation predictive analyses, the optimal EEG features were more effective than the CSF + APOE biomarkers in predicting the age of onset and disease course, whereas the combination of EEG + CSF + APOE measures achieved the best performance for all targets of prediction. Conclusions Our study indicates that EEG can be used as a useful screening tool for the diagnosis and disease progression evaluation of MCI and AD.

Abstract:Clinical EEG and Neuroscience, Ahead of Print. Introduction: The aim was to examine the differences in electroencephalography (EEG) findings by visual and automated quantitative analyses between Alzheimer's disease (AD) and dementia with Lewy bodies (DLB) and Parkinson's disease with dementia (PDD). Methods: EEG data of 20 patients with AD and 24 with DLB/PDD (12 DLB and 12 PDD) were retrospectively analyzed. Based on the awake EEG, the posterior dominant rhythm frequency and proportion of patients who showed intermittent focal and diffuse slow waves (IDS) were visually and automatically compared between the AD and DLB/PDD groups. Results: On visual analysis, patients with DLB/PDD showed a lower PDR frequency than patients with AD. In patients with PDR <8 Hz and occipital slow waves or patients with PDR <8 Hz and IDS, DLB/PDD was highly suspected (PPV 100%) and AD was unlikely (PPV 0%). On automatic analysis, the findings of the PDR were similar to those on visual analysis. Comparisons between visual and automatic analysis showed an overlap in the focal slow wave commonly detected by both methods in 10 of 44 patients, and concordant presence or absence of IDS in 29 of 43 patients. With respect to PDR <8 Hz and the combination of PDR <8 Hz and IDS, PPV and NPV in DLB/PDD and AD were not different between visual and automatic analysis. Conclusions: As the noninvasive, widely available clinical tool of low expense, visual analysis of EEG findings provided highly sufficient information to delineate different brain dysfunction in AD and DLB/PDD, and automatic EEG analysis could support visual analysis especially about PD.

Abstract:Alzheimer’s disease is a common brain disorder affecting many people worldwide. It is the primary cause of dementia and memory loss. The early diagnosis of Alzheimer’s disease is essential to provide timely care to AD patients and prevent the development of symptoms of this disease. Various non-invasive techniques can be utilized to diagnose Alzheimer’s in its early stages. These techniques include functional magnetic resonance imaging, electroencephalography, positron emission tomography, and diffusion tensor imaging. They are mainly used to explore functional and structural connectivity of human brains. Functional connectivity is essential for understanding the co-activation of certain brain regions co-activation. This systematic review scrutinizes various works of Alzheimer’s disease detection by analyzing the learning from functional connectivity of fMRI datasets that were published between 2018 and 2024. This work investigates the whole learning pipeline including data analysis, standard preprocessing phases of fMRI, feature computation, extraction and selection, and the various machine learning and deep learning algorithms that are used to predict the occurrence of Alzheimer’s disease. Ultimately, the paper analyzed results on AD and highlighted future research directions in medical imaging. There is a need for an efficient and accurate way to detect AD to overcome the problems faced by patients in the early stages.

Early Detection of Alzheimer’s disease by using the Latest Techniques

Abstract:: Alzheimer's disease (AD) is a prevalent neurodegenerative disease affecting cognitive functions and is particularly common among elderly people worldwide. It is considered incurable, and its symptoms progressively deteriorate over time. Early detection of AD is critical for developing new and effective treatment strategies. Dementia causes irreversible damage to brain neurons, leading to changes in brain structure that can be analyzed through multifractal frameworks. This study focuses on developing an efficient computing technique to pre-process and classify AD, especially in the early stages. The development of additional non-invasive and cost-effective tools for identifying individuals in the preclinical or early clinical stages of AD is necessary. These tools can aid in early detection and potentially more effective therapeutic and preventative strategies for AD. Large clinical trials are necessary to validate these tools before implementing them in clinical practice. This review will summarize and highlight the most promising screening tools, including neuropsychiatric, clinical, blood, and neurophysiological tests.

Abstract:Alzheimer's disease is primarily characterized by the gradual progression of neurodegeneration that leads to cognitive decline, and its early diagnosis remains the cornerstone for effective interventions. This systematic review of the current literature will investigate and summarize the available knowledge regarding the emerging biomarkers for the early diagnosis of AD. Many databases were searched across a wide range, using articles from the last ten years, all dealing with biochemical, imaging, and genetic biomarkers in the preclinical and prodromal stages of Alzheimer's disease. More key biomarkers are those in cerebrospinal fluid, which are CSF markers like amyloid-beta (Aβ42) and tau proteins. Novel biomarkers are within the blood, through neuroimaging studies that use techniques such as PET and MRI. Genetic markers include the one APOE ε4. Findings hold promise for using biomarker profiles, especially fluid biomarkers combined with advanced neuroimaging, to enhance diagnostic accuracy at early stages. Such biomarkers require validation, however, through large-scale and longitudinal studies before their routine use in clinical settings. Standardized assessment protocols on the biomarker domain take on importance in this light to highlight the potential of new biomarkers in changing the landscape of diagnosis for AD.

Abstract:Alzheimer's Disease (AD) is a severe brain disorder, destroying memories and brain functions. AD causes chronically, progressively, and irreversibly cognitive declination and brain damages. The reliable and effective evaluation of early dementia has become essential research with medical imaging technologies and computer-aided algorithms. This trend has moved to modern Artificial Intelligence (AI) technologies motivated by deeplearning success in image classification and natural language processing. The purpose of this review is to provide an overview of the latest research involving deep-learning algorithms in evaluating the process of dementia, diagnosing the early stage of AD, and discussing an outlook for this research. This review introduces various applications of modern AI algorithms in AD diagnosis, including Convolutional Neural Network (CNN), Recurrent Neural Network (RNN), Automatic Image Segmentation, Autoencoder, Graph CNN (GCN), Ensemble Learning, and Transfer Learning. The advantages and disadvantages of the proposed methods and their performance are discussed. The conclusion section summarizes the primary contributions and medical imaging preprocessing techniques applied in the reviewed research. Finally, we discuss the limitations and future outlooks.
EEG and ERP biosignatures of mild cognitive impairment for longitudinal monitoring of early cognitive decline in Alzheimer's disease

Abstract:Cognitive decline in Alzheimer's disease is associated with electroencephalographic (EEG) biosignatures even at early stages of mild cognitive impairment (MCI). The aim of this work is to provide a unified measure of cognitive decline by aggregating biosignatures from multiple EEG modalities and to evaluate repeatability of the composite measure at an individual level. These modalities included resting state EEG (eyes-closed) and two event-related potential (ERP) tasks on visual memory and attention. We compared individuals with MCI (n = 38) to age-matched healthy controls HC (n = 44). In resting state EEG, the MCI group exhibited higher power in Theta (3-7Hz) and lower power in Beta (13-20Hz) frequency bands. In both ERP tasks, the MCI group exhibited reduced ERP late positive potential (LPP), delayed ERP early component latency, slower reaction time, and decreased response accuracy. Cluster-based permutation analysis revealed significant clusters of difference between the MCI and HC groups in the frequency-channel and time-channel spaces. Cluster-based measures and performance measures (12 biosignatures in total) were selected as predictors of MCI. We trained a support vector machine (SVM) classifier achieving AUC = 0.89, accuracy = 77% in cross-validation using all data. Split-data validation resulted in (AUC = 0.87, accuracy = 76%) and (AUC = 0.75, accuracy = 70%) on testing data at baseline and follow-up visits, respectively. Classification scores at baseline and follow-up visits were correlated (r = 0.72, p<0.001, ICC = 0.84), supporting test-retest reliability of EEG biosignature. These results support the utility of EEG/ERP for prognostic testing, repeated assessments, and tracking potential treatment outcomes in the limited duration of clinical trials.
Classifying Alzheimers Disease and Dementia Patients Using Non-invasive EEG Biomarkers

Abstract:Researchers are currently exploring methods to detect early-stage Alzheimer's Disease (AD) and other forms of dementia such as frontotemporal dementia (FTD), especially through non-invasive biomarkers, i.e., measurements that reflect bio- logical processes. This paper utilizes a dataset of electroencephalogram (EEG) recordings, a noninvasive biomarker, to distinguish individuals with Alzheimer's or frontotemporal dementia from healthy control subjects. This paper explores the usage of machine learning methods to more accurately predict the cognitive status of patients from these non-invasive EEG Biomarkers. We found that AD patients could be easily separated from healthy controls based on their EEG features using simple linear classifiers with an accuracy of 77% and the AD, FTD, and healthy controls with an accuracy of around 57% (randomly selecting the right class is about one third or 33% in the 3-way classification).

Abstract:Abstract The dynamics of the resting brain exhibit transitions between a small number of discrete networks, each remaining stable for tens to hundreds of milliseconds. These functional microstates are thought to be the building blocks of spontaneous consciousness. The electroencephalogram (EEG) is a useful tool for imaging microstates, and EEG microstate analysis can potentially give insight into altered brain dynamics underpinning cognitive impairment in disorders such as Alzheimer’s disease (AD). Since EEG is non-invasive and relatively inexpensive, EEG microstates have the potential to be useful clinical tools for aiding early diagnosis of AD. In this study, EEG was collected from two independent cohorts of probable AD and cognitively healthy control participants, and a cohort of mild cognitive impairment (MCI) patients with four-year clinical follow-up. The microstate associated with the frontoparietal working-memory/attention network was altered in AD due to parietal inactivation. Using a novel measure of complexity, we found microstate transitioning was slower and less complex in AD. When combined with a spectral EEG measure, microstate complexity could classify AD with sensitivity and specificity > 80%, which was tested on an independent cohort, and could predict progression from MCI to AD in a small preliminary test cohort of 11 participants. EEG microstates therefore have potential to be a non-invasive functional biomarker of AD.
