Abstract:Introduction: This is a case of a popliteal swelling which was clinically misdiagnosed as a simple popliteal cyst, which turned out to be a benign fibroblastic tumor featuring nodular fasciitis (NF). A swelling in the popliteal fossa by and large is considered to be a Baker's cyst. However, an array of other lesions which can be encountered such as fibroma featuring NF, meniscal cysts, lipoma, aneurysms, bursitis of the biceps femoris tendon, and schwannoma should also be borne in mind since clinical and radiological evaluation may not always be conclusive. This case report presents fibroma featuring NF masquerading as Baker's cyst, which has not been described in any literature that the authors could find hitherto, making it a unique case. Case report: A 20-year-old male man presented to the outpatient department with a solitary palpable mass on the posterior aspect of his right knee for the past 3 years associated with difficulty in squatting for the past 3 months. The mass gradually was well defined and increased gradually in size to the current size of 10*9 cm, irregular soft to firm in consistency, spherical, non-tender, and non-reducible on the posterior aspect of the popliteal fossa. Local ultrasonography showed evidence of heterogeneous hypoechoic lesion with signs of inflammation; underlying bony cortices appeared to be normal. A clinical diagnosis of Baker's cyst was made and an excisional biopsy was performed. Microscopically, histologic sections showed fibroblasts arranged in loose fascicular patterns intermixed with small amount of collagen and myxoid stroma. Conclusion: Popliteal masses need not always be simple Baker's cyst, and careful evaluation of the mass in the popliteal fossa is always mandatory. Radiologic investigations may not always be conclusive and biopsy is the gold standard for diagnosis.

Abstract:We report a case of a 67-year-old woman suffering from intermittent claudication secondary to severe popliteal stenosis due to compression by a popliteal ganglion cyst extending into the adventitia of the popliteal artery. After declining vein bypass grafting, this patient was successfully treated using ultrasound-guided aspiration of the cyst, which restored normal flow to the popliteal artery. Whilst aspiration of Baker's cysts causing claudication has been attempted before, this represents the first reported case of successful recanalization of the popliteal artery by ganglion cyst aspiration and further supports an important possible aetiology and treatment for cystic adventitial disease.
A Giant Ganglion Cyst of Hip Joint Causing Lower Limb Edema

Abstract:Lower limb edema caused by a giant ganglion cyst of hip joint, rather than deep venous insufficiency or lymphedema, is unusual particularly for guiding clinical practice. In this study, we present a rare case of a 76-year-old man with a 3-month history of progressive swelling of right lower extremity and 2-month history of a known inguinal mass, which was initially thought as hemangioma by using ultrasound. But, subsequent computed tomography examination and exploration surgery revealed a giant ganglion cyst compressing the right femoral vein. Resection of the cyst led to complete resolution of symptoms postoperatively. To the best of our knowledge this is a very rare case of giant ganglion cyst of hip joint origin causing lower limb edema in the literature.
Popliteal Cyst Compressive Tibial Neuropathy and Venous Insufficiency: A Case Report

Abstract:Popliteal cysts are a collection of synovial fluid found in the popliteal fossa that typically form in adults in association with traumatic injuries, degenerative conditions, or inflammatory arthritis of the knee. While often asymptomatic, popliteal cysts may become problematic as enlarging and ruptured cysts may compress surrounding neurovascular structures, resulting in lower extremity edema or peripheral neuropathy. We report a unique case of a symptomatic popliteal cyst in a patient with both compressive neuropathy and venous congestion in the setting of a non-ruptured popliteal cyst after a surgically repaired intraarticular injury. Magnetic resonance imaging (MRI) showed a synovial cyst abutting the posterior neurovascular bundle and evidence of avascular necrosis. An open posterior cyst decompression was done, and the patient was able to report significant symptomatic improvement over the course of two weeks postoperatively. The previously noted varicose veins also demonstrated noticeable resolution. While relatively common, popliteal cysts may require prompt surgical decompression in order to provide effective symptomatic relief.

Abstract:Context: Popliteal synovial cysts, also known as Baker’s cysts, are commonly found in association with intra-articular knee disorders, such as osteoarthritis and meniscus tears. Histologically, the cyst walls resemble synovial tissue with fibrosis evident, and there may be chronic nonspecific inflammation present. Osteocartilaginous loose bodies may also be found within the cyst, even if they are not seen in the knee joint. Baker’s cysts can be a source of posterior knee pain that persists despite surgical treatment of the intra-articular lesion, and they are routinely discovered on magnetic resonance imaging scans of the symptomatic knee. Symptoms related to a popliteal cyst origin are infrequent and may be related to size. Evidence Acquisition: A PubMed search was conducted with keywords related to the history, diagnosis, and treatment of Baker’s cysts—namely, Baker’s cyst, popliteal cyst, diagnosis, treatment, formation of popliteal cyst, surgical indications, and complications. Bibliographies from these references were also reviewed to identify related and pertinent literature. Study Design: Clinical review. Level of Evidence: Level 4. Results: Baker’s cysts are commonly found associated with intra-articular knee disorders. Proper diagnosis, examination, and treatment are paramount in alleviating the pain and discomfort associated with Baker’s cysts. Conclusion: A capsular opening to the semimembranosus–medial head gastrocnemius bursa is a commonly found normal anatomic variant. It is thought that this can lead to the formation of a popliteal cyst in the presence of chronic knee effusions as a result of intra-articular pathology. Management of symptomatic popliteal cysts is conservative. The intra-articular pathology should be first addressed by arthroscopy. If surgical excision later becomes necessary, a limited posteromedial approach is often employed. Other treatments, such as arthroscopic debridement and closure of the valvular mechanism, are not well studied and cannot yet be recommended.

Abstract:Popliteal cyst is a common peri-knee cyst, also known as a Baker's cyst. With the current development of popliteal cysts, the pathogenesis is mainly due to increased pressure in the knee joint caused by various reasons, leading to a fluid-filled sac that can form behind the knee as a result of enlargement of the gastrocnemius-semimembranosus bursa. The current diagnostic methods include X-ray computed, ultrasound, and magnetic resonance. Among them, magnetic resonance imaging is considered the gold standard for the diagnosis of popliteal cysts. There are various treatments, including conservative treatment, traditional surgical resection and arthroscopic surgery. In recent years, the focus of the treatment of popliteal cysts has gradually shifted from the simple removal of cysts to arthroscopic treatment of intra-articular lesions and the treatment of joint-cyst communication, and achieved a good effect. However, there are still controversies about the most effective treatment, and the best surgery method is still uncertained. It is hoped that high quality prospective studies will be able to directly compare different surgery methods, so as to select the best treatment for popliteal cyst. This article reviews past literature research and describes in detail the epidemiology, pathological mechanism, clinical manifestations and signs, auxiliary examination, diagnosis and differential diagnosis and clinical treatment of popliteal cysts. Different diagnosis methods and treatment methods are compared and summarized to provide basis for clinical diagnosis and treatment.
Cystic adventitial disease of the popliteal artery: A case report with review of literature

Abstract:Introduction and importance: Cystic adventitial disease (CAD) is a rare vascular disorder marked by occlusion stemming from the development of a cystic mass within the outer (subadventitial) layer, with a predominant impact on the popliteal artery. The significance of the case presented herein lies in shedding light on a distinct clinical manifestation involving a 40-year-old man who exhibited sporadic calf claudication during ambulation. This instance contributes to the broader understanding of CAD and its diverse clinical presentations, emphasizing the need for further exploration and awareness within the medical community. Case presentation: A 40-year-old man, with no significant past medical history, was referred to the cardiovascular department for the evaluation of a new onset of left calf pain persisting over the past three months. An arterial lower limb Doppler ultrasound was performed, revealing a focal hypoechoic image around the popliteal artery with a regular arterial wall, indicative of extrinsic compression. This resulted in significant stenosis of the popliteal artery during plantar flexion of the foot. The diagnosis of CAD of the left popliteal artery was established after limb computed tomography angiography, and a complete resection of the cyst was scheduled. The postoperative course was uneventful, with the patient experiencing relief from left calf claudication. Clinical discussion: CAD is an uncommon vascular anomaly, representing merely 0.1 % of all vascular conditions. This condition predominantly afflicts men aged between 40 and 50 years old. The etiology of CAD remains a subject of debate, with pathological findings typically involving intramural cysts containing gelatinous material between the media and the adventitia. Surgical intervention becomes necessary when symptoms arise. Conclusion: CAD of the popliteal artery, though rare, is a significant contributor to peripheral vascular insufficiency in young patients without typical atherosclerotic risk factors.

Abstract:INTRODUCTION: A popliteal cyst, also called a Baker cyst, is a benign uncontrolled swelling of the gluteus medius sac in the popliteal fossa at the back of the knee described this excess fluid and subsequent sac formation outside of the kneecap in the commencement.A Baker's (popliteal) cyst is a swelling of the knee's gastrocnemius-semimembranous bursa, which connects to the back of the synovial membrane. It commonly presents as a painless swelling in the popliteal fossa. Adults almost always develop an edema as a result of pathological changes to the knee joint. Sometimes, the cyst enlarges to the position in which it ruptures, releasing synovial fluid into the surrounding tissues and present physiologically as acute thrombophlebitis. Popliteal cyst infection is a rare complication that is often related to septic arthritis. Popliteal joint cysts, also known to as Baker's cysts, are sporadically together with intra-articular knee conditions like osteoarthritis and dislocated patella. Anatomically, the borders of the cyst resemble synovial tissue, and there may be chronic, non-specific inflammation. Even if they are not visible in the knee joint, osteocartilaginous loose bodies may be present within the cyst.CLINICAL FINDING: Pain in the back of knee were the main complaints of a 48-year-old female admitted to Tertiary Rural Hospital.DIAGNOSTIC EVALUATION: Chest x-ray and blood test including all tests are done. Were all performed as part of the physical evaluation to prevent acute and chronic post-surgical discomfort following Excision of Bakers cyst.THERAPEUTIC INTERVENTION: Post operative treatment advised 6 hours iv fluids normal saline, ringer lactate 500ml, antacids, antiemetic, analgesic given frequently advised as the first line of treatment following excision of baker cyst . It gives superior pain relief and allows for a speedier recovery.OUTCOMES: Patient condition was poor.CONCLUSION: Baker cyst is a benign uncontrolled enlargement of the bursa in the popliteal fossa at the back of the knee that is diagnosed in the hospital on the IPD based on the patient having symptoms of swelling and pain in the back of knee. Based on a patient admitted to Tertiary Rural Hospital for diagnostic tests are done as per doctor order blood test, surgery of the excision of cyst . To reduce acute and chronic post-surgical pain after excision baker cyst. Post operative medications advised.

Abstract:Popliteal cysts, often referred to as Baker's cysts, are a common occurrence in the adult knee. Although controversy exists as to the exact indications for treatment, these structures can cause extreme discomfort and morbidity, including pain from rupture and symptoms from neurovascular compromise. Prior to the development of the arthroscope, open treatment of popliteal cysts was not uncommon. Complications such as poor wound healing, cyst recurrence, and knee flexion contractures were reported after such treatment. Owing to the presence of a valve-type structure, also called the posterior transverse synovial infold, there is 1-way flow of synovial fluid into the cyst. Although seldom described, there is a reproducible and relatively straightforward arthroscopic treatment for this pathology. This technical report will describe the arthroscopic treatment of popliteal cysts and clarify the posterior knee anatomy that gives the surgeon the landmarks to perform safe and effective arthroscopic treatment of popliteal cysts.

Abstract:Exertional leg pain occurs with notable frequency among athletes and poses diagnostic challenges to clinicians due to overlapping symptomatology. In this case report, we delineate the clinical presentation of a young collegiate soccer player who endured two years of progressive bilateral exertional calf pain and ankle weakness during athletic activity. The initial assessment yielded a diagnosis of chronic exertional compartment syndrome (CECS), predicated on the results of compartment testing. However, her clinical presentation was suspicious for concurrent type VI popliteal artery entrapment syndrome (PAES), prompting further radiographic testing of magnetic resonance angiography (MRA). MRA revealed severe arterial spasm with plantarflexion bilaterally, corroborating the additional diagnosis of PEAS. Given the worsening symptoms, the patient underwent open popliteal entrapment release of the right leg. Although CECS and PAES are both known phenomena that are observed in collegiate athletes, their co-occurrence is uncommon owing to their different pathophysiological underpinnings. This case underscores the importance for clinicians to be aware that the successful diagnosis of one condition does not exclude the possibility of a secondary, unrelated pathology. This case also highlights the importance of dynamic imaging modalities, including point-of-care ultrasound, dynamic MRA, and dynamic angiogram.

Abstract:Objective To explore the diagnosis and surgical procedure of soft tissue tumors in the popliteal fossae.Methods A retrospective study was conducted to evaluate surgical excision of 117 cases of popliteal soft tissue tumors.The preoperative diagnosis was compared with pathologic diagnosis and surgical complications was analyzed.Results Totally 117 cases included 96 popliteal cysts and 21 non-popliteal cysts were related to 11 disease.The correct rate of preoperative diagnosis was 85.5%(popliteal cysts 94.8%,non-popliteal cysts 42.9%).All cases were followed up 8 to 38(36±3.6) years except 4 cases,109 cases were cured,4 cases were recurrence and 2 died.Complications were seen in 6 cases.Conclusions Soft tissue tumors in the popliteal fossae are related to much broader spectrum of diseases.On curing popliteal cyst,it is important to master surgical indications and crucial points of operation procedure.Because histological features of non-popliteal cyst is difficult to determine,surgical treatment should be adviced for initial choise.
Compression syndromes of the popliteal neurovascular due to Baker cyst: A case report

Abstract:Introduction and importance: Baker's cyst, or popliteal cyst, is typically arising in the popliteal fossa located between the semi-membranous tendon and the medial head of the gastrocnemius. Asymptomatic Baker's cyst does not require treatment. Surgical treatment may be considered after the failure of conservative measures when the cyst causes persistent pain and rarely compresses the adjacent neurovascular structure. Case presentation: We report an unusual presentation of Baker's cyst in a 43-year-old patient who complained of knee pain and after several months of conservative treatment. Following a physical and radiological examination, a Baker cyst was confirmed that compresses the tibial nerve. A surgical decompression and excision of the cyst was performed using the posterior approach to allow complete removal of the cyst's stalk and wall. Histopathological report confirms dense collagen without true epithelial lining. The patient was asymptomatic at 6 months of follow-up. Clinical discussion: It is important to recognize that a large Baker's cyst can be symptomatic and present as a compression syndrome of the adjacent neurovascular structure. The current case report described an early surgical management to treat a tibial nerve compression syndrome caused by a Baker cyst. A surgical decompression through a posterior approach may facilitate complete removal of the cyst wall. Conclusions: Baker's cyst is a common knee pathology that could rarely compress the adjacent neurovascular structures. The surgical decompression through a posterior approach results in favorable outcomes in symptomatic patients with failed conservative measures.
Pigmented Villonodular Synovitis Presenting as a Popliteal Cyst

Abstract:Introduction: Pigmented villonodular synovitis is an uncommon disease characterized by hyperplastic synovium, large effusions and bone erosions. It commonly mimics other conditions. As a result, the diagnosis may remain elusive, as the classic signs of tenderness and effusion are not specific. Occasionally, PVNS presents as a popliteal cyst, which may divert attention from intra-articular pathology. In this article, we describe a case of pigmented villonodular synovitis that presented with a popliteal cyst as the chief problem. Case report: A twelve year old boy presented to us with a history of a gradually enlarging mass in the left popliteal fossa since eighteen months. He complained of restriction of terminal knee flexion, but he had no pain. After clinical examination and imaging the mass was interpreted as being either a popliteal cyst or chronic hematoma without ruling out the possibility of soft tissue sarcoma. An open excisional biopsy of the lesion was performed which revealed a reddish-brown cystic-nodular mass communicating with the knee joint. The histology was consistent with that of pigmented villonodular synovitis. We performed an arthroscopic synovectomy 3 weeks later. Abnormal synovium found in all compartments of the knee was removed. Histology from the intra-articular lesion was also consistent with a diagnosis of pigmented villonodular synovitis. Conclusion: A swelling in the popliteal region may be due to various etiologies. The cause should be thoroughly investigated. Along with clinical examination and imaging the tissue must be subjected to histopathological examination. Pigmented villonodular synovitis can rarely present as a popliteal cyst. In such a situation, treatment consists of dealing with intra-articular and extra-articular pathology.
Complex Interplay: A Gigantic Recurrent Popliteal Cyst Following Total Knee Arthroplasty in a Patient with Refractory Rheumatoid Arthritis Case Report

Abstract:Introduction: A popliteal cyst, or Baker's cyst, is often associated with joint diseases such as osteoarthritis and rheumatoid arthritis (RA). It is rare for these cysts to develop following total knee arthroplasty (TKA), but understanding when and why they might can optimize patient care. Presented here is a unique case of a massive, chronically recurring infected popliteal cyst in a patient with RA and prior TKA, shedding light on an unusual complication worth attention in the orthopedic literature. Case report: In this case, the patient had longstanding, difficult-to-treat RA. Following left TKA, the patient developed a painful popliteal cyst, leading to hospitalization in 2023. Microbiological analysis identified Staphylococcus lugdunensis as the infectious agent despite negative mycobacterial and mycological cultures. Surgical intervention involved a one-stage procedure, encompassing resection of the extensive thigh cyst and prosthesis replacement. Notably, the cyst reached an unprecedented size, measuring 32 cm at its peak, presenting a unique challenge in management. Conclusion: This case report contributes significantly to orthopedic literature by highlighting the intricate interplay between joint pathologies, surgical interventions, and infections. It highlights the importance of multidisciplinary collaboration in managing complex musculoskeletal conditions. The rarity of a massive and infected popliteal cyst post-TKA emphasizes the need for heightened vigilance in patient care post-surgery. Furthermore, this case report serves as a valuable addition to the understanding of potential complications associated with TKA, offering insights that may inform future treatment strategies and optimize patient outcomes in orthopedic practice.
Infected popliteal pseudoaneurysm in a youth basketball player: A case report and brief review of the literature

Abstract:Introduction: An infected popliteal pseudoaneurysm has never been described in the pediatric population. Physicians need to be aware of its presentation and management, in order to diagnose and treat this medical condition adequately. Methods: We describe the case of a 14-year-old boy who developed myositis and cellulitis centered at the popliteal fossa after playing basketball. A treatment of intravenous cefazolin was started. 5 days later, he experienced a knee pain flare-up, which turned out to be a popliteal pyomyositis with a pseudoaneurysm of the popliteal artery. A saphenous vein graft bypass of the popliteal artery and an excision of the popliteal pseudoaneurysm were performed. Intravenous cefazolin was continued for 6 weeks and prophylactic acetylsalicylic acid for 6 months. Results and conclusion: This case highlighted the importance of repeating radiologic investigations if a patient suffering from soft tissue infection has persistent pain after several days of appropriate antibiotics. A popliteal pseudoaneurysm can be diagnosed with ultrasound imaging and treated with a popliteal-popliteal bypass. Our patient needed a catheter-guided dilation of the anastomosis at the vein graft 6 months post-surgery, and then evolved favorably and went back to playing basketball 6 months post-dilation.
Unusual Presentation of Popliteal Cyst on Magnetic Resonance Imaging

Abstract:Popliteal cyst commonly presents as an ellipsoid mass with uniform low signal intensity on T1-weighted magnetic resonance images and high signal intensity on T2-weighted images. Here, we describe a popliteal cyst with unusual appearance on magnetic resonance imaging, including heterogeneous intermediate signal intensity on T2-weighted images. Arthroscopic cyst decompression revealed that the cyst was filled with necrotic synovial villi, indicative of rheumatoid arthritis. Arthroscopic enlargement of unidirectional valvular slits with synovectomy was useful for the final diagnosis and treatment.
Popliteal Cysts: A Systematic Review of Nonoperative and Operative Treatment

Abstract:Background: Treatment methods for popliteal cysts have varied over the past several decades and have posed challenges to providers as recurrences were frequent. With greater understanding of relevant anatomy, both operative and nonoperative treatment methods have evolved to appropriately target relevant pathology and improve outcomes. The purposes of this review were to outline the evolution of treatment methods and to qualitatively summarize clinical outcomes. Methods: We performed a systematic review on treatments for popliteal cysts to include publications from 1970 to 2019. Other inclusion criteria consisted of studies with ≥10 patients enrolled, studies with a patient age of ≥16 years, studies with an adequate description of the treatment technique, and studies with a Level of Evidence of IV or higher. The review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, and literature quality was assessed using a modified Coleman methodology score. Results: Thirty studies met inclusion criteria in this review. Nine studies discussed nonoperative treatment, and 21 studies discussed operative treatment. Eight of the 9 nonoperative treatment studies utilized corticosteroid injections. The most recent studies have advocated for ultrasound-guided intracystic injection with possible cyst fenestration. Most operative studies utilized an arthroscopic approach to enlarge the communication with the joint space. However, alternative treatment techniques are still utilized. Conclusions: The current literature on the treatment of popliteal cysts indicates that intracystic corticosteroid injection with cyst fenestration is an effective nonoperative treatment method. Arthroscopic surgical procedures with enlargement of the communication have been most widely studied, with positive results; however, further studies are needed to confirm superiority over other treatment methods. Delayed Presentation of Popliteal Artery Laceration Mimicking Baker Cyst After Arthroscopic Meniscectomy: A Case Report

Abstract:Case: We report a case of delayed presentation of popliteal artery laceration after arthroscopic meniscectomy. Three weeks after the procedure, the patient developed severe calf pain, swelling over the popliteal fossa, and foot drop, despite a normal initial postoperative period. Clinical examination and radiological investigations suggested a massive hematoma in the popliteal fossa. A decision was made to explore the popliteal artery and drain the hematoma after consultation with the vascular surgeon. A laceration was noted, and it was repaired. Conclusion: Orthopaedic surgeons should maintain a high index of suspicion regarding the possibility of this postoperative complication, especially after an arthroscopic meniscectomy.

Abstract:Popliteal cysts can be an oppressive symptom in patients with concomitant intra-articular knee pathologies. Because isolated treatment of intra-articular lesions is usually not sufficient to resolve the problems associated with a large symptomatic popliteal cyst, a popliteal cyst should be concurrently addressed with other knee pathologies to maximize patient outcomes. Conservative treatment and open surgical excision are associated with high rates of recurrence, so arthroscopic techniques are the preferred treatment options for recalcitrant cases. Arthroscopic communication-enlargement surgery with cyst wall removal seems to be the most effective. We present a simple and effective basic technique for arthroscopic popliteal cyst treatment and some approaches that allow management of the more demanding cases.

Abstract:Objective To summarize the sonographic features of the different types of popliteal cyst rupture,and to investigate the value of high frequency color Doppler ultrasound in the diagnosis and differential diagnosis of popliteal cyst rupture.Methods The ultrasonographic features of 15 cases diagnosed by ultrasound and confirmed by puncture or MRI were analyzed retrospectively.Results Among the 15 cases of ruptured popliteal cyst,8 cases were proximal rupture,effusion were accumulated in the soft tissue behind semimembranosus;7 cases were distal rupture,with 5 cases of effusion accumulated between the gastrocnemius and soleus muscle,2 cases of effusion accumulated between the gastrocnemius muscle and subcutaneous fat.Conclusions High frequency color Doppler ultrasound to diagnosis of popliteal cyst rupture,diagnosis and differential diagnosis is very high.Proximal rupture of popliteal cyst is not easy to find,and need to be vigilant.
