Abstract:Background: Total hip arthroplasty (THA) and total knee arthroplasty (TKA) are reliable surgical options to treat pain and disability resulting from degenerative conditions around the hip and knee. Obesity is a modifiable risk factor that contributes to significant morbidity. The purpose of this study was to retrospectively compare outcomes in primary hip and knee arthroplasty for patients with increased body mass index (BMI) and those with normal BMI, using data from the registry of the Alberta Bone and Joint Health Institute (ABJHI).Methods: We retrospectively reviewed the data compiled in the ABJHI registry between March 2010 and July 2016. We reviewed outcomes with respect to length of stay, discharge destination, 30-day readmission, postoperative infection, postoperative transfusion requirements, postoperative adverse events and in-hospital postoperative mechanical complications.Results: A total of 10 902 patients (6076 women, 4826 men) who underwent THA and 16 485 patients (10 057 women, 6428 men) who underwent TKA were included in the study. For both THA and TKA, patients with increased BMI had an increased number of in-hospital medical events, had an increased rate of deep infection, were less likely to be discharged home (p < 0.001) and had decreased transfusion requirements (p < 0.001) than patients whose weight was in the normal range. Increased BMI increased the rate of 30-day readmission and length of stay in the THA cohort but not in the TKA cohort. Increased BMI had no effect on acute postoperative dislocation or periprosthetic fractures. Patients with a BMI of 30 kg/m2 or greater required a THA 1.7 years earlier than patients of normal weight, patients whose BMI was 35 kg/m2 or greater required a THA 3.4 years earlier, and patients whose BMI was 40 kg/m2 or greater required a THA 5.8 years earlier. In the TKA cohort, patients with a BMI of 30 kg/m2 or greater required a TKA 2.7 years earlier than patients whose weight was in the normal range, patients with a BMI of 35 kg/m2 or greater required a TKA 4.6 years earlier, and patients whose BMI was 40 kg/m2 or greater required a TKA 7.6 years earlier.Conclusion: Our study quantifies the effects of obesity in primary hip and knee arthroplasty. It provides a greater understanding of the risks in the obese population when contemplating joint arthroplasty.

Abstract:Background Although obesity may be associated with an increased risk of peri- and post-operative complications after total knee arthroplasty (TKA), body mass index (BMI) cutoffs for TKA patient selection remain a controversial topic. This study aimed to investigate patient-reported outcomes, satisfaction, and pain among BMI classes at 3 months and 1 year following TKA. Methods A total of 2,365 patients were categorized into Centers for Disease Control BMI classes according to preoperative values. The BMI distribution over the cohort demonstrated 7 underweight, 340 healthy weight, 731 overweight, 666 Class I, 391 Class II, and 230 Class III obesity. Pre- and post-operative Knee injury and Osteoarthritis Outcome Score for Joint Replacement (KOOS JR), satisfaction, and pain scores were compared by BMI class by Analysis of Variance (ANOVA) with pairwise comparisons. Results Preoperative KOOS JR scores and satisfaction scores were both significantly greater for healthy weight patients compared to Class III patients ( P < 0.0001). However, KOOS JR, satisfaction, and pain did not vary by BMI class postoperatively. Changes in scores from baseline suggest larger improvements with increasing BMI class, where Class III patients reported the greatest improvements in KOOS JR (23.24±15.67, P < 0.0001) and pain scores (-3.56±2.65, P < 0.0001) at 90 days. Significantly greater improvement with increasing BMI was also present at 1-year postoperatively for KOOS JR scores. A total of 85.8% of Class III patients reached the Minimal Clinically Important Difference (MCID) for KOOS JR at 90 days compared to 76.8% of the healthy weight group ( P = 0.03). Differences between BMI groups in the proportion reaching MCID at 1 year were not significant. Conclusion Patients of higher BMI reported greater and earlier improvements in satisfaction, knee function, and pain following TKA. Shared decision-making remains paramount to preoperative surgical evaluation. However, rationing of this surgical intervention based on BMI alone may not be warranted.

Abstract:Background: A high body mass index (BMI) is associated with increased rates of complications after total knee arthroplasty (TKA). However, no study has examined the effect of BMI on lower limb alignment using the World Health Organization's (WHO) BMI classification. We believe that the WHO's BMI classification allows a uniform standard worldwide. We sought to investigate the potential association between a high BMI and the incidence of postoperative misalignment. We also evaluated whether a higher BMI is associated with worse clinical function.Methods: We retrospectively reviewed the data of 671 patients who underwent primary TKA for varus osteoarthritis between January 2010 and December 2015. The patients were divided into the following 5 groups based on their BMI: normal weight (<25.0 kg/m2), overweight (25.0-29.9 kg/m2), class I obese (30.0-34.9 kg/m2), class II obese (35-39.9 kg/m2), and class III obese (>40 kg/m2). Both weight and height were measured by nurses on admission. Patients' preoperative HKA, gender, age, and side of surgery were collected as baseline. All the patients underwent standing, weight-bearing, full-length radiography before and after surgery to measure the mechanical hip-knee-ankle angle (HKA). We followed up patients by telephone. Among the BMI subgroups, we compared the knee function scores, including the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) score, Knee Society-Knee Score (KS-KS), Knee Society-Function Score (KS-FS), Forgotten Joint Score (FJS), and range of motion (ROM). A multivariate linear regression analysis and a logistic regression was conducted to examine the outcomes.Results: The study had a mean follow-up period of 8.16 years. The multivariate and logistic regression analyses revealed that preoperative alignment (P=0.002) and a higher BMI (P=0.015) were associated with a higher risk of postoperative misalignment. The WOMAC scores were higher in the normal and overweight groups than the other groups (P=0.022). The FJS and KS-KS gradually decreased as BMI increased.Conclusions: A higher BMI is associated with a greater risk of misalignment and worse long-term clinical outcome after TKA. When treating patients with high BMI, we should pay more attention to the adjustment of lower limb alignment intraoperatively.

Abstract:Background: Increase body mass index (BMI) is often accompanied by metabolic diseases such as diabetes, which will increase the uncertainty of total knee arthroplasty (TKA) efficacy and the risk of postoperative complications. The present study was to study the relationship between increase BMI and clinical efficacy of knee arthroplasty in patients with knee osteoarthritis.Methods: A total of 97 patients (36 males and 61 females) with knee osteoarthritis (KOA) who underwent TKA surgery were selected. According to the preoperative body mass index (BMI), the patients were divided into a normal group (n=42), overweight group (n=35), and obese group (n=20). All patients received TKA after admission. Seven days after surgery, the American Knee Society (AKS) and the Hospital for Special Surgery (HSS) scales were used to evaluate the recovery of knee function. The recovery was poor if the scores of AKS and HSS were less than 70.Results: Seven days after TKA, the scores of AKS and HSS in different BMI groups were significantly different, and decreased with the increase of BMI (P<0.05). Age, increased BMI, diabetes, preoperative range of motion (ROM), intraoperative blood loss, postoperative C-reactive protein (CRP), postoperative posterior slope angle (PSA), postoperative infection, and postoperative deep vein thrombosis (DVT) of lower extremities were related to AKS score <70 (P<0.05). Diabetes, preoperative ROM, intraoperative blood loss, postoperative CRP, postoperative PSA, and postoperative infection were related to HSS score <70 (P<0.05). Increased BMI, diabetes, postoperative CRP, postoperative infection, and postoperative DVT were independent risk factors for AKS score <70 (HR =3.458, 1.152, 2.960, 1.023, 3.589, P<0.05). Increased BMI, diabetes, postoperative CRP, and postoperative infection were independent risk factors for HSS score <70 (HR =6.891, 1.263, 1.967, 1.235, P<1.235). The area under the curve (AUC) (95% CI) of BMI in diagnosing AKS <70 was 0.740 (0.641-0.839). The AUC (95% CI) of BMI in diagnosing HSS <70 was 0.809 (0.723-0.894).Conclusions: The increase of BMI is an independent risk factor for the poor recovery of knee function after TKA in patients with KOA.

Functional and Radiological Outcome of Simultaneous Bilateral Total Knee Replacement in a Tertiary Care Centre : A Quasi-prospective Study

Abstract:Introduction: Osteoarthritis (OA) is the most frequent joint disease with a prevalence of 5313 per 100,000 persons in India. OA of the knee is a major contributor to mobility impairment, particularly among women. In these patients, total knee replacement (TKR) is widely accepted as the gold standard treatment for pain relief and restoration of function. Now, patients have the option of undergoing either a simultaneous bilateral TKR or a staged TKR. The former has shown significant advantages such as shorter rehabilitation, cost efficacy and shorter hospital stay. This study aimed to assess the functional and radiological outcome of simultaneous bilateral TKR in a tertiary care centre. Materials and methods: 30 adult patients above the age of 55 years with severe OA knee who underwent simultaneous bilateral TKR between 2022-2023 were included in this prospective study. Preoperatively, they were assessed with radiographs including full length scannograms. Mean Anatomical Axis, Mean Mechanical Axis and Mechanical Axis Deviation (Varus) were calculated based on the scannograms by a single observer. The same observer administered the oxford knee score (OKS) preoperatively to all patients. Regular clinical and radiological follow up was done. After 1 year follow up, the same observer administered the Oxford Knee Score (OKS) to the operated knee. Results: With a female preponderance among the study subjects (56.7%), the majority of the cases in our cohort were characterised as Grade IV Kellgren-Lawrence radiological grade of Osteoarthritis (76.66%) and were found to have tricompartmental involvement (70%). The mean anatomical axis for the right and left lower limb were 4.56±5.17 and 9.68±1.72 respectively. The mean mechanical axis deviation (Varus) for the right and left lower limb were 31.6±15.1 and 52.0±13.9 respectively. Preoperatively, mean mechanical axis for the right and left lower limb were 9.95±4.31 and 14.2 ±0.67 respectively. Postoperatively, mechanical axis was restored to near-normal values (3-5 degrees). Significant improvement in mean OKS from a preoperative value of 18.30 ± 3.46 to postoperative value of 33.50 ± 5.32 was noted at 1 year. Conclusion: Treatment of severe bilateral OA knee with simultaneous single staged bilateral TKR ensures good functional outcome , early mobility with minimal complications.

Abstract:BACKGROUND: The goal of this study was to evaluate the relationship between body mass index (BMI) and early complications of total hip replacement (THR) and total knee replacement (TKR).METHODS: This is a retrospective study of patients who underwent primary THR and TKR between January 2011 and December 2015. We included patients between 18 and 90 years of age with BMI less than 40 kg/m2 with a minimal postoperative follow-up time of 45 days. The primary outcomes were the presence of infection, mechanical complications (dislocation, fractures, arthrofibrosis, or neuropraxia), and thromboembolic events. Overweight and obesity were defined as a BMI of 25-29.9 kg/m2 and ≥ 30 kg/m2, respectively.RESULTS: In total 750 patients were included (THR, 268; TKR, 482) with a mean age of 65.0 ± 12.4 years. The percentage of patients with normal weight, overweight, and obesity was 24.9% (n = 187), 41.7% (n = 313), and 33.4% (n = 250), respectively. The early complication rate was 8.9% (95% confidence interval [CI], 7.1% to 11.2%). Infection and mechanical complications were most prevalent. There was no statistically significant relationship between the incidence of complications and BMI (obesity vs. normal weight: hazard ratio [HR], 1.49; 95% CI, 0.72 to 3.06; p = 0.282); however, there was a tendency toward a greater risk of infectious complications in the patients with obesity (HR, 6.08; 95% CI, 0.75 to 49.16; p = 0.090). Patients with diabetes mellitus (DM) had more risk of infectious complications than those without DM (HR, 2.60; 95% CI, 1.00 to 6.79).CONCLUSIONS: There was no statistical relationship between BMI and early complications of THR and TKR. Nonetheless, there was a tendency toward a higher risk of infection in patients with some degree of obesity.

Abstract:Background: Osteoarthritis is the most prevalent form of arthritis causing disability, particularly in elderly population. Aging, female sex, obesity, and possibly nutritional deficiencies trigger the pathogenesis of osteoarthritis. This study aims to identify the association of body mass index (BMI), with knee osteoarthritis grading viewed from Kellgren–Lawrence (KL) criteria in participants with osteoarthritis of the knee joint.Methods: This research is a cross-sectional study using analytical approach in 70 participants diagnosed with Knee Osteoarthritis at Vaidyaratnam Ayurveda College, Ollur, Thrissur. BMI was calculated by measuring the height and weight of participants. Grading of knee OA based on KL criteria was assessed from the antero-posterior X-ray radiograph.Results: Of 70 participants, 81.4% were female and 31.4% were in the age group of 50–55 years, 27.1% in the age group of 61–65 years. Among the total 70 participants, 45.7% belong to overweight category, 28.6% were obese. Knee OA grading based on KL criteria indicates that the most frequent grade is Grade 2 with 34.3% followed by Grade 3 30%. Spearman rank correlation showed a positive correlation between BMI and degree of joint damage, which was statistically significant at 0.01 level (r = 0.491**, P < 0.001).Conclusion: The higher the BMI value, the heavier the grading of OA of the knee
Body Mass Index Improvement Reduces Total Knee Arthroplasty Complications Among Patients Who Have Extreme, but Not Severe, Obesity

Abstract:Background: While morbid obesity has been associated with increased complication risk in primary total knee arthroplasty (TKA), limited evidence is available to attribute decreased surgical complication rates with body mass index (BMI) reduction. Methods: We retrospectively assessed 464 unilateral TKAs performed in morbidly obese patients, including 158 extremely obese (BMI ≥ 45) and 306 severely obese patients (BMI 40 to 44.9). A detailed medical record review identified concurrent modifiable risk factors and successful preoperative BMI reduction, reaching either a contemporary risk target (BMI < 40) or an institutionally accepted threshold (BMI < 45). Postoperative blood glucose levels and 1-year adverse outcomes (periprosthetic joint infection, wound dehiscence, knee manipulation, periprosthetic fracture) were compared to 557 contemporary control subjects with expected slightly lower (moderate obesity, BMI 35 to 39.9) or sufficiently lower complication risk (overweight, BMI 25 to 29.9). Results: Periprosthetic joint infection and postoperative hyperglycemia were identified more frequently among morbidly obese patients in comparison with a moderately obese control group. Extremely obese patients (BMI ≥ 45) whose BMI improved below 45 had no measurable difference in infection risk from the control group (odds ratio [OR] 0.84, 95% confidence interval [CI] 0.04 to 16.88), while those with a nonimproved BMI had a significantly higher risk (OR 7.70, 95% CI 1.89 to 31.41). No significant differences in the risk for infection were observed between severely obese patients (BMI 40 to 44.9) with preoperative BMI improvement (1.5% rate, OR 1.70, 95% CI 0.17 to 16.57) or nonimprovement (1.7% rate, OR 1.87, 95% CI 0.41 to 8.43). Conclusions: Preoperative medical optimization may decrease postoperative TKA complications. The findings of this study support BMI improvement for extremely obese patients (BMI ≥ 45). The assignment of 40 BMI as a threshold for otherwise healthy patients may exclude patients from potential surgical benefits without realizing risk reduction.

Abstract:Total knee arthroplasty (TKA) are increasing worldwide [1]. The rates have almost increased to 60% more in the last decade in theelderly. Total knee replacement or Knee arthroplasty has made a revolution the field of orthopedics as the earlier thought of painfulconditions like osteoarthritis or rheumatoid arthritis could now be solved by replacing the damaged joint with an artificial joint whichdramatically improves the pain of the patient and the person who could barely walk due to pain starts walking again pain free. Whilemost years these joint replacement surgeries were out of reach for common patients, various schemes implemented by the Govt hasmade it possible that even poor needy patients can get the total knee arthroplasty done and can live a better quality of life free of painand suffering.Methodology: This study was a record based observational study conducted retrospectively in the department of Orthopaedics in atertiary care hospital in central India from 1st March 2021 to 28th February 2022.Observation and Results: A total of 40 patients were studied in total. Out of them 22 were males and 18 were females. The mean ageof the patients was 58.2 years. Out of the 40, 15 patients had yellow ration card i.e. belonging to lower socio economic status while 15had saffron ration card and only 10 had white ration card. Majority of the population was Hindu (40%) followed by Muslims (25%),Buddhist (25%) and 10% were Christians, Sikhs and Jains. Majority of the population was educated up to 10th standard while 15%were illiterate. Osteoarthritis was the most common indication for hip joint replacement (70%).Conclusion: Knee joint replacement is being done commonly in the right candidate and helps in alleviating patients from theirsufferings.

Abstract:[Objective]To determine if obesity(according to body mass index,BMI) is a risk factor for total knee replacement(TKR).[Method] A total of 320 patients with 520 knees underwent TKR operation by a single surgeon with the same type of prosthesis for osteoarthritis (OA). All the patients were divided into 3 groups according to the body mass index: group A with obese patients; group B with sub-obese patients; and group C with non-obese patients. Postoperative results were studied respectively.[Result]Patients in group A had lower preoperative function score than those of group B ,C ( P 0.05, respectively), but knee scores were not significantly different from any other group. The postoperative mean HSS score of all the groups rose significantly according to a 28.3 months' (ranged, 12 - 46 months ) follow - up, while differences between each group were not statistically significant ( P 0.1, respectively). The rate of perioperative complications was significantly higher in the obese patients.[Conclusion]Total knee replacement in obese patients can be successful but is associated with an increased rate of perioperative complications, including problems with Wound -healing, infection, and avulsion of the medial ligament. Alteration in the operative technique for soft - tissue closure and protection of the medial collateral ligament have decreased the rates of complications related to woundhealing and the medial collateral ligament.

Abstract:Background: The influence of obesity measured in terms of body mass index (BMI) on the complication rates following total hip arthroplasty (THA) is a matter of debate. Methods: This retrospective study conducted at a tertiary referral centre at Brisbane, Australia, examines the association between BMI and in-hospital postoperative complications, length of operating time and duration of hospital stay in 964 patients, who underwent THA from 2006 to 2010. Results: Amongst patients undergoing primary THA, when compared to the normal weight patients, those with BMI between 25 kg/m 2 and 29.9 kg/m 2 (overweight) and those with BMI between 35 kg/m 2 and 39.9 kg/m 2 (obese class II) had lower odds of perioperative complications (odds ratio [OR]: 0.62 (95% confidence intervals [CI], 0.43–0.92, p = 0.016) and OR: 0.60 (95% CI, 0.36– 0.99, p = 0.047 respectively). Patients with BMI less than or equal to 40 kg/m 2 were also associated with significantly lower odds of cardiac complications ( p = 0.02). With unadjusted regression analysis, it was noted that those with BMI ≥40 kg/m 2 had the highest odds of developing infectious complications (OR 2.68, 95% CI, 1.08–6.65, p < 0.05). As the BMI increased, there was a statistically significant increase in length of operating time ( p < 0.001). Conclusion: There is a significant impact of BMI on the occurrence of perioperative complications following THA. Compared to normal weight category, the overweight and obese class II patients had a lower likelihood of developing overall, especially cardiac complications. Length of operating time increases along with an increase in BMI.

Abstract:Background: When indicating patients for primary total knee arthroplasty (TKA), surgeons and patients must understand the absolute and relative risks of periprosthetic joint infection (PJI) to make an informed decision. We sought to evaluate the long-term risk of PJI following primary TKA stratified by body mass index (BMI) and PJI-related risk factors. Methods: We identified 25,160 primary TKAs performed from 2000 to 2021 at a single institution. Patients were stratified as having 0, 1, or ≥ 2 PJI risk factors (diabetes, chronic kidney disease, nonprimary osteoarthritis, immunosuppression, or active smoking) and into BMI categories. The 15-year cumulative risk of PJI was evaluated by BMI and PJI risk factors. Results: For the entire cohort, the 15-year absolute risk of PJI was 3%. For patients who did not have PJI risk factors, the absolute risk of PJI at 15 years was 3% in normal weight, 4% in class III obesity, and 4% in class IV obesity. Patients who had class III obesity had a 3-time higher relative risk of PJI when than normal-weight patients (P = 0.01). Among patients who had ≥ 2 PJI risk factors, the absolute risk of PJI at 15 years was 5% in normal weight and 6% in patients who have class III obesity. Conclusions: Healthy patients who had class III obesity had a 3-time increased risk of PJI relative to healthy, normal-weight patients; however, the absolute risk of PJI at 15 years after primary TKA was 4% in this group. Surgeons and patients must consider both a 3-time increased relative risk of PJI and a 4% absolute risk of PJI at 15 years after primary TKA when considering surgery in otherwise healthy patients who have BMI ≥ 40. Body Mass Index is Not an Appropriate Proxy for the Condition of Peri-Incisional Adiposity in Primary Total Joint Arthroplasty Patients

Abstract:Background: The American Academy of Orthopaedic Surgeons (AAOS) defined the acceptable threshold for elective safe surgery as a body mass index (BMI) under 40 due to the increased risk of complications. A consequence of this recommendation has been a hard cutoff based on BMI, which restricts access to care for an increasingly large and diverse population. There is an improved understanding that excess adipose tissue confers additional risk for postoperative complications, including infection, through mechanical and physiologic mechanisms. But it is unclear if BMI is an accurate indicator of adiposity in total joint arthroplasty (TJA) patients and, thus, whether BMI is capturing clinically relevant information in obese patients. Our objective was to determine the relationship between peri-incisional adiposity (PIA) and BMI in a consecutive series of diverse primary TJA patients. Methods: A consecutive series of patients indicated for primary TJA were preoperatively evaluated. For each patient, the following variables were collected: BMI, and measures of peri-incisional adiposity (PIA) on radiographs and ultrasounds. Results: In THA patients (N = 99), Pearson's correlation coefficient (r) = 0.436, which indicates a moderate correlation between BMI and adiposity. In TKA patients (N = 271), r = 0.395 for femoral PIA (FPIA) and r = 0.249 for tibial PIA (TPIA), which indicates a weak correlation between BMI and adiposity measured on radiography. In TKA patients, r = 0.560 for FPIA and r = 0.544 for TPIA, which indicates a moderate correlation between BMI and adiposity measured on ultrasound. Conclusion: Quantification of obesity has become common practice in the preoperative evaluation of primary TJA patients. The intent is to determine the magnitude of adipose tissue, which is one of the main drivers of postoperative complications in obesity. The BMI is ubiquitously used as a proxy for obesity due to its simplicity of attainment and calculation. We report that BMI has a weak to moderate association with peri-incisional adiposity in this population. These findings indicate that BMI may not accurately represent the condition of peri-incisional adipose tissue and, thus, is not capturing the relevant obesity data for preoperative risk stratification in primary total joint arthroplasty patients.

Abstract:Background The current research used a new index—adipose to muscle area ratio (AMR)—to measure fatness compared with body mass index (BMI) in elderly osteoarthritis (OA) patients following total knee arthroplasty. Our study aimed to test the relationship between the two indexes (AMR and BMI) and to examine whether AMR was a predictive factor of patient-reported outcome measures (PROMS) for elderly OA patients following total knee arthroplasty (TKA). Methods The retrospective data of 78 OA patients (older than 60 years) following TKA was included in our study. Clinical features of patients included age, BMI, sex, AMR, side of the implant, time of follow-up, complications, the Knee Society Score (KSS score), and the Hospital for Special Surgery knee score (HSS score). The area of adipose tissue and muscle tissue was measured on the cross section (supra-patella, midline of the patella, joint line of the knee) of the knee magnetic resonance imaging (MRI). AMR was calculated as the average of adipose to muscle area ratio at the three levels. The Pearson correlation analysis, simple linear regression, and multiple linear regression were used to study the relationship between BMI, AMR, and PROMS (KSS total-post score and HSS-post score) in the study. Results Of all patients, the mean (± standard deviations (SD)) of age was 67.78 ± 4.91 years. For BMI and AMR, the mean (± SD) were 26.90 ± 2.11 and 2.36 ± 0.69, respectively. In Pearson correlation analysis, BMI had a good correlation with AMR ( r = 0.56, p = 0.000), and AMR ( r = − 0.37, p = 0.001, HSS-post score; r = − 0.43, p = 0.000, KSS total-post score) had better correlations with PROMS postoperatively compared with BMI ( r = − 0.27, p = 0.019, HSS-post score; r = − 0.33, p = 0.003, KSS total-post score). In multivariate linear regression analysis, AMR was negatively correlated with KSS total-post score as well as HSS-post score, while BMI was not. As for patients with complications, AMR values were between the 3rd quartile and 4th quartile of the AMR value in the entire study cohort. Conclusions In this study, the new obesity evaluation indicator—AMR, which was well related with BMI, was found to be a predictor of PROMS (KSS total-post score and HSS-post score) in elderly OA patients following TKA.
