Abstract:Background . There is an increasing trend to represent domain knowledge in structured graphs, which provide efficient knowledge representations for many downstream tasks. Knowledge graphs are widely used to model prior knowledge in the form of nodes and edges to represent semantically connected knowledge entities, which several works have adopted into different medical imaging applications . Methods . We systematically searched over five databases to find relevant articles that applied knowledge graphs to medical imaging analysis. After screening, evaluating, and reviewing the selected articles, we performed a systematic analysis. Results . We looked at four applications in medical imaging analysis, including disease classification, disease localization and segmentation, report generation, and image retrieval. We also identified limitations of current work, such as the limited amount of available annotated data and weak generalizability to other tasks. We further identified the potential future directions according to the identified limitations, including employing semisupervised frameworks to alleviate the need for annotated data and exploring task-agnostic models to provide better generalizability. Conclusions . We hope that our article will provide the readers with aggregated documentation of the state-of-the-art knowledge graph applications for medical imaging to encourage future research.

Abstract:Medical knowledge graphs (MKGs) are the basis for intelligent health care, and they have been in use in a variety of intelligent medical applications. Thus, understanding the research and application development of MKGs will be crucial for future relevant research in the biomedical field. To this end, we offer an in-depth review of MKG in this work. Our research begins with the examination of four types of medical information sources, knowledge graph creation methodologies, and six major themes for MKG development. Furthermore, three popular models of reasoning from the viewpoint of knowledge reasoning are discussed. A reasoning implementation path (RIP) is proposed as a means of expressing the reasoning procedures for MKG. In addition, we explore intelligent medical applications based on RIP and MKG and classify them into nine major types. Finally, we summarize the current state of MKG research based on more than 130 publications and future challenges and opportunities.

Abstract:A knowledge graph is defined as a collection of interlinked descriptions of concepts, relationships, entities and events. Medical knowledge graphs have been the most recent advances in technology, therapy and medicine. Nowadays, a number of specific uses and applications rely on knowledge graphs. The application of the knowledge graph, another form of artificial intelligence (AI) in cardiology and cardiovascular medicine, is a new concept, and only a few studies have been carried out on this particular aspect. In this brief literature review, the use and importance of disease-specific knowledge graphs in exploring various aspects of Kawasaki disease were described. A vision of individualized knowledge graphs (iKGs) in cardiovascular medicine was also discussed. Such iKGs would be based on a modern informatics platform of exchange and inquiry that could comprehensively integrate biologic knowledge with medical histories and health outcomes of individual patients. This could transform how clinicians and scientists discover, communicate and apply new knowledge. In addition, we also described how a study based on the comprehensive longitudinal evaluation of dietary factors associated with acute myocardial infarction and fatal coronary heart disease used a knowledge graph to show the dietary factors associated with cardiovascular diseases in Nurses’ Health Study data. To conclude, in this fast-developing world, medical knowledge graphs have emerged as attractive methods of data storage and hypothesis generation. They could be a major and effective tool in cardiology and cardiovascular medicine and play an important role in reaching effective clinical decisions during treatment and management of patients in the cardiology department.

Abstract:Computer-aided diagnosis (CAD) has become one of the major research subjects in medical imaging and diagnostic radiology. In this article, the motivation and philosophy for early development of CAD schemes are presented together with the current status and future potential of CAD in a PACS environment. With CAD, radiologists use the computer output as a "second opinion" and make the final decisions. CAD is a concept established by taking into account equally the roles of physicians and computers, whereas automated computer diagnosis is a concept based on computer algorithms only. With CAD, the performance by computers does not have to be comparable to or better than that by physicians, but needs to be complementary to that by physicians. In fact, a large number of CAD systems have been employed for assisting physicians in the early detection of breast cancers on mammograms. A CAD scheme that makes use of lateral chest images has the potential to improve the overall performance in the detection of lung nodules when combined with another CAD scheme for PA chest images. Because vertebral fractures can be detected reliably by computer on lateral chest radiographs, radiologists' accuracy in the detection of vertebral fractures would be improved by the use of CAD, and thus early diagnosis of osteoporosis would become possible. In MRA, a CAD system has been developed for assisting radiologists in the detection of intracranial aneurysms. On successive bone scan images, a CAD scheme for detection of interval changes has been developed by use of temporal subtraction images. In the future, many CAD schemes could be assembled as packages and implemented as a part of PACS. For example, the package for chest CAD may include the computerized detection of lung nodules, interstitial opacities, cardiomegaly, vertebral fractures, and interval changes in chest radiographs as well as the computerized classification of benign and malignant nodules and the differential diagnosis of interstitial lung diseases. In order to assist in the differential diagnosis, it would be possible to search for and retrieve images (or lesions) with known pathology, which would be very similar to a new unknown case, from PACS when a reliable and useful method has been developed for quantifying the similarity of a pair of images for visual comparison by radiologists.

Abstract:Abstract The detailed physiological perspectives captured by medical imaging provides actionable insights to doctors to manage comprehensive care of patients. However, the quality of such diagnostic image modalities is often affected by mismanagement of the image capturing process by poorly trained technicians and older/poorly maintained imaging equipment. Further, a patient is often subjected to scanning at different orientations to capture the frontal, lateral and sagittal views of the affected areas. Due to the large volume of diagnostic scans performed at a modern hospital, adequate documentation of such additional perspectives is mostly overlooked, which is also an essential key element of quality diagnostic systems and predictive analytics systems. Another crucial challenge affecting effective medical image data management is that the diagnostic scans are essentially stored as unstructured data, lacking a well-defined processing methodology for enabling intelligent image data management for supporting applications like similar patient retrieval , automated disease prediction etc. One solution is to incorporate automated diagnostic image descriptions of the observation/findings by leveraging computer vision and natural language processing. In this work, we present multi-task neural models capable of addressing these critical challenges. We propose ESRGAN, an image enhancement technique for improving the quality and visualization of medical chest x-ray images, thereby substantially improving the potential for accurate diagnosis, automatic detection and region-of-interest segmentation. We also propose a CNN-based model called ViewNet for predicting the view orientation of the x-ray image and generating a medical report using Xception net, thus facilitating a robust medical image management system for intelligent diagnosis applications. Experimental results are demonstrated using standard metrics like BRISQUE, PIQE and BLEU scores, indicating that the proposed models achieved excellent performance. Further, the proposed deep learning approaches enable diagnosis in a lesser time and their hybrid architecture shows significant potential for supporting many intelligent diagnosis applications.

Abstract:ABSTRACT Objectives Artificial intelligence (AI) is an emerging field in dentistry. AI is gradually being integrated into dentistry to improve clinical dental practice. The aims of this scoping review were to investigate the application of AI in image analysis for decision‐making in clinical dentistry and identify trends and research gaps in the current literature. Material and Methods This review followed the guidelines provided by the Preferred Reporting Items for Systematic Reviews and Meta‐Analyses Extension for Scoping Reviews (PRISMA‐ScR). An electronic literature search was performed through PubMed and Scopus. After removing duplicates, a preliminary screening based on titles and abstracts was performed. A full‐text review and analysis were performed according to predefined inclusion criteria, and data were extracted from eligible articles. Results Of the 1334 articles returned, 276 met the inclusion criteria (consisting of 601,122 images in total) and were included in the qualitative synthesis. Most of the included studies utilized convolutional neural networks (CNNs) on dental radiographs such as orthopantomograms (OPGs) and intraoral radiographs (bitewings and periapicals). AI was applied across all fields of dentistry ‐ particularly oral medicine, oral surgery, and orthodontics ‐ for direct clinical inference and segmentation. AI‐based image analysis was use in several components of the clinical decision‐making process, including diagnosis, detection or classification, prediction, and management. Conclusions A variety of machine learning and deep learning techniques are being used for dental image analysis to assist clinicians in making accurate diagnoses and choosing appropriate interventions in a timely manner.

Abstract:Abstract Artificial intelligence (AI)–assisted medical imaging technology is a new research area of great interest that has developed rapidly over the last decade. However, there has been no bibliometric analysis of published studies in this field. The present review focuses on AI-related studies on computed tomography imaging in the Web of Science database and uses CiteSpace and VOSviewer to generate a knowledge map and conduct the basic information analysis, co-word analysis, and co-citation analysis. A total of 7265 documents were included and the number of documents published had an overall upward trend. Scholars from the United States and China have made outstanding achievements, and there is a general lack of extensive cooperation in this field. In recent years, the research areas of great interest and difficulty have been the optimization and upgrading of algorithms, and the application of theoretical models to practical clinical applications. This review will help researchers understand the developments, research areas of great interest, and research frontiers in this field and provide reference and guidance for future studies.

Knowledge-based medical image analysis and representation for integrating content definition with the radiological report

Abstract:Technology breakthroughs in high-speed, high-capacity, and high performance desk-top computers and workstations make the possibility of integrating multimedia medical data to better support clinical decision making, computer-aided education, and research not only attractive, but feasible. To systematically evaluate results from increasingly automated image segmentation it is necessary to correlate them with the expert judgments of radiologists and other clinical specialists interpreting the images. These are contained in increasingly computerized radiological reports and other related clinical records. But to make automated comparison feasible it is necessary to first ensure compatibility of the knowledge content of images with the descriptions contained in these records. Enough common vocabulary, language, and knowledge representation components must be represented on the computer, followed by automated extraction of image-content descriptions from the text, which can then be matched to the results of automated image segmentation. A knowledge-based approach to image segmentation is essential to obtain the structured image descriptions needed for matching against the expert's descriptions. We have developed a new approach to medical image analysis which helps generate such descriptions: a knowledge-based object-centered hierarchical planning method for automatically composing the image analysis processes. The problem-solving steps of specialists are represented at the knowledge level in terms of goals, tasks, and domain objects and concepts separately from the implementation level for specific representations of different image types, and generic analysis methods. This system can serve as a major functional component in incrementally building and updating a structured and integrated hybrid information system of patient data.(ABSTRACT TRUNCATED AT 250 WORDS)
K-Diag: Knowledge-enhanced Disease Diagnosis in Radiographic Imaging

Abstract:In this paper, we consider the problem of disease diagnosis. Unlike the conventional learning paradigm that treats labels independently, we propose a knowledge-enhanced framework, that enables training visual representation with the guidance of medical domain knowledge. In particular, we make the following contributions: First, to explicitly incorporate experts' knowledge, we propose to learn a neural representation for the medical knowledge graph via contrastive learning, implicitly establishing relations between different medical concepts. Second, while training the visual encoder, we keep the parameters of the knowledge encoder frozen and propose to learn a set of prompt vectors for efficient adaptation. Third, we adopt a Transformer-based disease-query module for cross-model fusion, which naturally enables explainable diagnosis results via cross attention. To validate the effectiveness of our proposed framework, we conduct thorough experiments on three x-ray imaging datasets across different anatomy structures, showing our model is able to exploit the implicit relations between diseases/findings, thus is beneficial to the commonly encountered problem in the medical domain, namely, long-tailed and zero-shot recognition, which conventional methods either struggle or completely fail to realize.

An Explainable and Personalized Cognitive Reasoning Model Based on Knowledge Graph: Toward Decision Making for General Practice

Abstract:General practice plays a prominent role in primary health care (PHC). However, evidence has shown that the quality of PHC is still unsatisfactory, and the accuracy of clinical diagnosis and treatment must be improved in China. Decision making tools based on artificial intelligence can help general practitioners diagnose diseases, but most existing research is not sufficiently scalable and explainable. An explainable and personalized cognitive reasoning model based on knowledge graph (CRKG) proposed in this article can provide personalized diagnosis, perform decision making in general practice, and simulate the mode of thinking of human beings utilizing patients' electronic health records (EHRs) and knowledge graph. Taking abdominal diseases as the application point, an abdominal disease knowledge graph is first constructed in a semiautomated manner. Then, the CRKG designed referring to dual process theory in cognitive science involves the update strategy of global graph representations and reasoning on a personal cognitive graph by adopting the idea of graph neural networks and attention mechanisms. For the diagnosis of diseases in general practice, the CRKG outperforms all the baselines with a precision@1 of 0.7873, recall@10 of 0.9020 and hits@10 of 0.9340. Additionally, the visualization of the reasoning process for each visit of a patient based on the knowledge graph enhances clinicians' comprehension and contributes to explainability. This study is of great importance for the exploration and application of decision making based on EHRs and knowledge graph.

Abstract:Healthcare knowledge graphs (HKGs) are valuable tools for organizing biomedical concepts and their relationships with interpretable structures. The recent advent of large language models (LLMs) has paved the way for building more comprehensive and accurate HKGs. This, in turn, can improve the reliability of generated content and enable better evaluation of LLMs. However, the challenges of HKGs such as regarding data heterogeneity and limited coverage are not fully understood, highlighting the need for detailed reviews. This work provides the first comprehensive review of HKGs. It summarizes the pipeline and key techniques for HKG construction, as well as the common utilization approaches, i.e., model-free and model-based. Abstract:Abstract Background Knowledge graphs (KGs), especially medical knowledge graphs, are often significantly incomplete, so it necessitating a demand for medical knowledge graph completion (MedKGC). MedKGC can find new facts based on the existed knowledge in the KGs. The path-based knowledge reasoning algorithm is one of the most important approaches to this task. This type of method has received great attention in recent years because of its high performance and interpretability. In fact, traditional methods such as path ranking algorithm take the paths between an entity pair as atomic features. However, the medical KGs are very sparse, which makes it difficult to model effective semantic representation for extremely sparse path features. The sparsity in the medical KGs is mainly reflected in the long-tailed distribution of entities and paths. Previous methods merely consider the context structure in the paths of knowledge graph and ignore the textual semantics of the symbols in the path. Therefore, their performance cannot be further improved due to the two aspects of entity sparseness and path sparseness. Methods To address the above issues, this paper proposes two novel path-based reasoning methods to solve the sparsity issues of entity and path respectively, which adopts the textual semantic information of entities and paths for MedKGC. By using the pre-trained model BERT, combining the textual semantic representations of the entities and the relationships, we model the task of symbolic reasoning in the medical KG as a numerical computing issue in textual semantic representation. Results Experiments results on the publicly authoritative Chinese symptom knowledge graph demonstrated that the proposed method is significantly better than the state-of-the-art path-based knowledge graph reasoning methods, and the average performance is improved by 5.83% for all relations. Conclusions In this paper, we propose two new knowledge graph reasoning algorithms, which adopt textual semantic information of entities and paths and can effectively alleviate the sparsity problem of entities and paths in the MedKGC. As far as we know, it is the first method to use pre-trained language models and text path representations for medical knowledge reasoning. Our method can complete the impaired symptom knowledge graph in an interpretable way, and it outperforms the state-of-the-art path-based reasoning methods.

Abstract:Abstract Background: Tailored knowledge graph-based recommender systems (KGRSs) have been demonstrated to be able to provide accurate and effective health recommendations to users, and thus significantly reduce health care costs. They are now strongly recommended to be applied in the health care field. Objective: This scoping review aims to identify the current application of KGRSs, their target users and performance metrics, and the potential limitations of implementing health recommender systems in clinical practice. Methods: A review of the studies published from inception to November 1, 2022 was conducted, using key search terms in 6 scientific databases to identify health recommender systems based on knowledge graph technology. Key information from the included studies was extracted and charted. The scoping review was reported following the PRISMA Extension for Scoping Reviews. Result: We included 16 studies and 5 grants totally about the health recommender systems based on knowledge graph technology. They were used in different health areas: traditional Chinese medicine, health management, disease-related decision support, diet, and nutrition recommendations. Among them, 6 studies were for the general public and 6 were for physicians. A total of 13 (81.25%) studies evaluated the KGRS using performance metrics, such as accuracy, recall, F1 score, and area under the curve. All studies pointed out the limitations of the recommender systems and provided directions for their subsequent optimization and improvement. Conclusion: This review describes the state-of-the-art and potential limitations of KGRS used in the health care field. This novel approach has been proven to be effective in overcoming the drawbacks of traditional algorithms, helping users filter massive amounts of data to find out the personalized information they need. Its great potential in digital health needs to be further explored.

Knowledge Graph Based Agent for Complex, Knowledge-Intensive QA in Medicine

Abstract:Biomedical knowledge is uniquely complex and structured, requiring distinct reasoning strategies compared to other scientific disciplines like physics or chemistry. Biomedical scientists do not rely on a single approach to reasoning; instead, they use various strategies, including rule-based, prototype-based, and case-based reasoning. This diversity calls for flexible approaches that accommodate multiple reasoning strategies while leveraging in-domain knowledge. We introduce KGARevion, a knowledge graph (KG) based agent designed to address the complexity of knowledge-intensive medical queries. Upon receiving a query, KGARevion generates relevant triplets by using the knowledge base of the LLM. These triplets are then verified against a grounded KG to filter out erroneous information and ensure that only accurate, relevant data contribute to the final answer. Unlike RAG-based models, this multi-step process ensures robustness in reasoning while adapting to different models of medical reasoning. Evaluations on four gold-standard medical QA datasets show that KGARevion improves accuracy by over 5.2%, outperforming 15 models in handling complex medical questions. To test its capabilities, we curated three new medical QA datasets with varying levels of semantic complexity, where KGARevion achieved a 10.4% improvement in accuracy.

Abstract:Biomedical datasets are often modeled as knowledge graphs (KGs) because they capture the multi-relational, heterogeneous, and dynamic natures of biomedical systems. KG completion (KGC), can, therefore, help researchers make predictions to inform tasks like drug repositioning. While previous approaches for KGC were either rule-based or embedding-based, hybrid approaches based on neurosymbolic artificial intelligence are becoming more popular. Many of these methods possess unique characteristics which make them even better suited toward biomedical challenges. Here, we survey such approaches with an emphasis on their utilities and prospective benefits for biomedicine.

A knowledge graph based question answering method for medical domain

Abstract:Question answering (QA) is a hot field of research in Natural Language Processing. A big challenge in this field is to answer questions from knowledge-dependable domain. Since traditional QA hardly satisfies some knowledge-dependable situations, such as disease diagnosis, drug recommendation, etc. In recent years, researches focus on knowledge-based question answering (KBQA). However, there still exist some problems in KBQA, traditional KBQA is limited by a range of historical cases and takes too much human labor. To address the problems, in this paper, we propose an approach of knowledge graph based question answering (KGQA) method for medical domain, which firstly constructs a medical knowledge graph by extracting named entities and relations between the entities from medical documents. Then, in order to understand a question, it extracts the key information in the question according to the named entities, and meanwhile, it recognizes the questions' intentions by adopting information gain. The next an inference method based on weighted path ranking on the knowledge graph is proposed to score the related entities according to the key information and intention of a given question. Finally, it extracts the inferred candidate entities to construct answers. Our approach can understand questions, connect the questions to the knowledge graph and inference the answers on the knowledge graph. Theoretical analysis and real-life experimental results show the efficiency of our approach.

Abstract:Computer Aided Detection (CAD) is a valuable technique for precisely interpreting medical images and it has a global business opportunity of about USD 1.8 billion. The current aspects with reference to the four sub stages such as image pre-processing, segmentation, feature extraction and classification and the future scope of CAD in medical imaging has been discussed in this paper. Many reviewers have emphasized the need for synergy between engineers and medical professionals for successful development of CAD systems and the current work is a move in that direction. The engineering aspects of the above four stages in four imaging modalities viz. computed tomography, magnetic resonance imaging, mammography and bone scintigraphy used in the diagnosis of five critical diseases have been discussed with a clinical background. Automatic classification of image can play an important role in preliminary screening of very critical ailments bringing down the cost of health care. Another recent advancement is using artificial intelligence and machine learning techniques. This paper reviews these engineering aspects with a view to explore the opportunities to researchers as well as the medical industry to offer affordable medical services with accessibility in even remote locations.
