Abstract:Objective To investigate the clinical effectiveness of anterior cervical discectomy and fusion (ACDF)plus plating in the treatment of unstable Hangman's fractures.Methods A retrospective case series study was carried out to analyze 12 patients who received ACDF plus plate internal fixation for unstable Hangman's fracture admitted to Sir Run Run Shaw Hospital affiliated to Zhejiang University School of Medicine from July 2014 to July 2018.There were nine males and three females,aged 24-70 years with an average of 46.2 years.According to Levine-Edwards typing,there were seven patients of type Ⅱ,four patients of type Ⅱ A and one patient of type Ⅲ.There were two patients of grade D and 10 patients of grade E according to the American Spinal Injury Association (ASIA) classification.The operation time,intraoperative blood loss and the operational complications were recorded.At 3 days,1,3,6,12 and 24 months after operation,the height of intervertebral disc,physiological curvature of cervical spine,fracture healing and bone graft fusion were evaluated.The recovery of neurological function was assessed by ASIA,and the neck pain relief and function improvement was assessed by visual analogue scale (VAS) and the neck disability index (NDI).Results All patients were followed for 6-48 months(mean,27.7 months).The operation time ranged from 50-100 minutes,with an average of 72.5 minutes.The intraoperative blood loss was 30-120 ml (mean,65.8 ml).No serious complications occurred during and after operation.Two patients had dysphagia after operation,and the symptoms were relieved at the follow-up of 3 months.All patients achieved bony fusion of C2-3 within the 6 month follow-up.No subsidence or migration of cages and significant loss of intervertebral space height were observed.The patient's neck and occipital pain was relieved obviously,and the flexion,extension and rotation of cervical spine were good.The neurologic dysfunction of two patients of ASIA Grade D recovered after surgery,and the final ASIA grade was E.The VAS score was decreased from 5.0 (4.0,6.0)points preoperatively to 1.0(0,1.0)points at the last follow-up (P ＜0.01).The NDI score was improved from (28.5 ± 6.4) points preoperatively to (5.3 ± 2.7) points at the last follow-up (P＜0.01).Conclusion ACDF combined with plate internal fixation can provide immediate postoperative stability,high fusion rate,less complications,less pain,and satisfactory function recovery.
Early Self-directed Home Exercise Program After Anterior Cervical Discectomy and Fusion: A Pilot Study

Abstract:Study design: Pilot randomized controlled trial. Objective: To examine the acceptability and preliminary safety and outcome effects of an early self-directed home exercise program (HEP) performed within the first 6 weeks after anterior cervical discectomy and fusion (ACDF). Summary of background data: Little is known regarding optimal postoperative management after ACDF. Methods: Thirty patients (mean ± standard deviation, age = 50.6 ± 11.0 years, 16 women) undergoing ACDF were randomized to receive an early HEP (n = 15) or usual care (n = 15). The early HEP was a 6-week self-directed program with weekly supportive telephone calls to reduce pain and improve activity. Treatment acceptability was assessed after the intervention period (6 weeks after surgery). Safety (adverse events, radiographic fusion, revision surgery) was determined at routine postoperative visits. Disability (Neck Disability Index), pain intensity (Numeric Rating Scale for neck and arm pain), physical and mental health (SF-12), and opioid use were assessed preoperatively, and at 6 weeks and 6 and 12 months after surgery by an evaluator blinded to group assignment. Results: Participants reported high levels of acceptability and no serious adverse events with the early HEP. No difference in fusion rate was observed between groups (P > 0.05) and no participants underwent revision surgery. The early self-directed HEP group reported lower 6-week neck pain than the usual care group (F = 3.3, P = 0.04, r = 0.3, mean difference = -1.7 [-3.4; -0.05]) and lower proportion of individuals (13% vs. 47%) using opioids at 12 months (P = 0.05). No other between-group outcome differences were observed (P > 0.05). Conclusion: An early self-directed HEP program was acceptable to patients and has the potential to be safely administered to patients immediately after ACDF. Benefits were noted for short-term neck pain and long-term opioid utilization. However, larger trials are needed to confirm safety with standardized and long-term radiograph assessment and treatment efficacy. Level of evidence: 2.

Abstract:Introduction To improve the efficacy of anterior cervical decompression and fusion (ACDF) and reduce postoperative complications in degenerative cervical myelopathy, our team established a set of perioperative care of enhanced recovery guidelines of ACDF based on the concept of enhanced recovery after surgery. In addition, a prospective, multicentre, randomised clinical trial was designed. Methods and analysis A total of 260 patients aged 18–65 years will be included. Preoperative MRI and CT will be used to confirm the typical manifestations of cervical spondylosis, such as cervical disc herniation and spinal cord compression. The patient presents with neck and shoulder pain, numbness of upper limbs, weakened grip strength and cotton sense of foot tread. Patients received normal conservative treatment for 3 months with no obvious relief or even aggravation of symptoms. Patients will be assigned to the group in strict accordance with the random allocation table. Patients in groups A and B will receive conventional perioperative care and perioperative care for enhanced recovery, respectively. The main outcome indicators are the Karnofsky Performance Scale score and the Japanese Orthopaedic Association scale. Secondary outcome indicators are pain assessment by Numeric Rating Scale, Neck Disability Index, quality of life index (QL-Index) and postoperative complications. Follow-up will be conducted at 3, 6 and 12 months postoperatively. Ethics and dissemination Ethical approval has been granted by the Ethics Committee of Fujian Medical University Union Hospital, Fuzhou, China (2020YF034-01). Results of the research will be published in an international peer-reviewed scientific journal and disseminated through presentation at scientific conferences. Abstract:Single-crystal ultrathin films of Gd, Tb, Dy, Ho, and Er were grown epitaxially on W{l brace}110{r brace} in ultrahigh vacuum. Comparison with single-crystal data available for Tb shows that while the purity of the films may be higher, the crystallinity, i.e., the long-range order, is less well developed in the films than in bulk single crystals.
Efficacy of anterior cervical discectomy and fusion using exoscope in the treatment of cervical spondylopathy

Abstract:Objective:To investigate the effect and safety of anterior cervical discectomy and fusion (ACDF) using exoscope in the treatment of cervical spondylopathy.Methods:From March 2017 to March 2019, 15 patients with cervical spondylopathy were admitted to Department of Neurosurgery, Zhongshan Hospital Affiliated to Fudan University underwent ACDF using exoscope whose data were retrospectively analyzed. Odom scale and Japanese Orthopaedic Association (JOA) scale were used after operation to evaluate the effect of ACDF using exoscope.Results:All 15 patients successfully underwent the surgeries. The operation time was 100±6 mins (80-160 mins). The bleeding volume was 75±10 ml (40-150 ml). The disc fusion ratio was 19/19.One patient had transient postoperative hoarseness and there was no death post operation. Fifteen patients were followed up for 3-27 months with an average of 11.3±1.7 months. The assessment result based on the Odom scale showed that 12 patients were very satisfied with the operation and 3 patients were satisfied with the operation. The mean JOA score of 15 patients was 15.8±0.2 points at 3 months post operation, which was higher than that prior to operation (13.3±0.4 points) ( t=5.8, P<0.01). The symptoms of all patients were relieved and the spinal cord function was improved significantly. Conclusions:The efficacy of ACDF using exoscope in the treatment of cervical spondylopathy seems good with few complications, which is worthy of being widely used.

Abstract:BACKGROUND: Anterior cervical discectomy and fusion (ACDF) is a common surgical procedure for cervical radiculopathy. While postoperative physical therapy (PT) has been shown to be effective after many orthopaedic surgeries, there is limited evidence available regarding postoperative PT after ACDF for cervical radiculopathy. CASE PRESENTATION: A 61-year-old male initiated early in-person PT 2 weeks following 2-level ACDF, which utilized a multimodal PT program (manual therapy, patient education, therapeutic exercise, self-management, HEP, modalities) guided by tissue healing, achievement of clinical objective milestones, and clinical reasoning. OUTCOME AND FOLLOW-UP: Significant improvement in outcome measures (numeric pain-rating scale, neck disability index, 36-item short-form health survey, global rating of change, fear avoidance beliefs questionnaire, cervical active range of motion) was achieved 12 weeks postoperatively and maintained up to 2 years with no adverse events. DISCUSSION: Early initiated in-person multimodal postoperative PT may be safe and effective for patients at all stages of healing after ACDF. Anterior cervical decompression with fusion for treating multilevel cervical myelopathy

Abstract:Objective To evaluate the clinical value of the segemental methods of anterior cervical decompression with fusion for treating multilevel cervical myelopathy. Methods A retrospective review of 20 patients who had undergone segemental method of anterior cervical decompression with fusion for treating multilevel cervical myelopathy. All patiens'cervical plain films,magnetic resonance imagings(MRI) and computed tomographies(CT) on preoperation and postoperation were observed. The mJOA(modified JOA) scores of preoperation and 3 months postoperatively were collected.The implant graft fusion rate ,the degree of correcting kyphotic deformity in cervical vertebrae and the complications were recorded. Results 20 cases were followed up from 14 months to 42 months. All patients improved their functional status in different degree. The mean preoperative mJOA score of all patients was 9.2, the postoperative was 13.8,the average mJOA recovery rate was 59.4%(p<0.05).All patients'kyphotic deformity was corrected and some restored normally. No complications such as graft dislocation,screw and plates loosing or broken were happened.Conclusions The segemental method of anterior cervical decompression with fusion for treating multilevel cervical myelopathy appears to be effectively decompressing spinal cord,improving nerve functional status and restoring lordosis to the cervical spine with high fusion rate and fewer complications.

Abstract:Study design: Retrospective study. Objective: This study aims to investigate the effect of cervical brace utilization on postoperative axial symptoms in patients undergoing single-segment anterior cervical discectomy and fusion (ACDF). Summary of background data: Anterior cervical discectomy and fusion (ACDF) is the most commonly used surgical method in the treatment of cervical spondylosis. For patients with single-segment ACDF. The absence of a neck brace after surgery is safe and does not affect the outcome of surgery. However, the effect on the incidence of AS is unclear. Methods: Patients who underwent anterior cervical single-segment ACDF between May 2020 and August 2021 were retrospectively analyzed. Participants were divided into brace group and nonbraced groups. The incidence of axial symptoms, cervical mobility, and postoperative quality of life were then compared between the 2 groups. Results: A total of 121 patients were included in this study: 62 in the brace group and 59 in the nonbraced group. There were no statistically significant variations observed in the overall demographic characteristics, including age, sex, body mass index, smoking status, and disease duration. The study findings showed that there was a significant decrease in the occurrence of axial symptoms among patients in nonbraced group, in addition to a considerable increase in cervical mobility 1 month following the surgery. Conclusions: The omission of a cervical brace following surgery in patients undergoing single-segment ACDF reduced the incidence of early postoperative axial symptoms, improved their overall quality of life, and facilitated the recovery of postoperative cervical mobility.

Abstract:Objective: To investigate the clinical effect of anterior cervical discectomy and fusion (ACDF) in the treatment of cervical spondylosis of vertebral artery type(CSA). Methods: The clinical data of 42 patients with CSA from January 2020 to January 2022 were retrospectively analyzed. There were 25 males and 17 females, aged from 30 to 74 years old with an average of (53.9±11.0) years old. There were 18 cases with single-segment lesions, 17 cases with two-segment lesions, and 7 cases with three-segment lesions. The American Academy of Otolaryngology-Head and Neck Surgery's Hearing and Balance Committee score (CHE), the Neck Disability Index (NDI) and the cervical curvature Cobb angle were recorded before surgery and after surgery at 6 months. Results: All 42 ACDF patients were followed up for 6 to 30 months with an average of (14.0±5.2) months. The operative time ranged from 95 to 220 min with an average of (160.38±36.77) min, the intraoperative blood loss ranged from 30 to 85 ml with an average of (53.60±18.98) ml. Tow patients had mild postoperative dysphagia, which improved with symptomatic treatment such as nebulized inhalation. CHE score decreased from (4.05±0.96) preoperatively to (2.40±0.70) at 6 months postoperatively (t=12.97, P<0.05). The number of improved vertigo at 6 months postoperatively was 38, with an improvement rate of 90.5%. NDI score was reduced from (34.43±8.04) preoperatively to (20.76±3.91) at 6 months postoperatively (t=11.83, P<0.05). The cervical curvature Cobb angle improved from (8.04±6.70)° preoperatively to (12.42±5.23)° at 6 months postoperatively (t=-15.96, P<0.05). Conclusion: The ACDF procedure has outstanding clinical efficacy in treating CSA. The operation can rapidly relieve patients' episodic vertigo symptoms by relieving bony compression and reconstructing cervical curvature. However, it is necessary to strictly grasp the indications for surgery and clarify the causes of vertigo in patients, and ACDF surgery is recommended for CSA patients for whom conservative treatment is ineffective.
Comparative study of outcomes between allograft intervertebral disc transplantation and anterior cervical discectomy and fusion: a retrospective cohort study at least 5 years of follow-up

Abstract:Purpose: Adjacent segment degeneration (ASDeg) after anterior cervical discectomy and fusion (ACDF) seriously affects the long-term efficacy of the operation. Therefore, our team has done a lot of research on allograft intervertebral disc transplantation (AIDT) to prove its feasibility and safety. This study will compare the efficacy between AIDT and ACDF in the treatment of cervical spondylosis. Methods: All patients who received ACDF or AIDT in our hospital from 2000 to 2016 and followed up for at least 5 years were recruited and divided into ACDF and AIDT groups. The clinical outcomes including functional scores and radiological data of both groups were collected and compared preoperatively and postoperatively at 1 week, 3 months, 6 months, 12 months, 24 months, 60 months and last follow-up. Functional scores included Japanese Orthopedic Association score (JOA), Neck Disability Index (NDI), Visual Analog Scale of Neck (N-VAS) and Arms (A-VAS) pain, the Short Form Health Survey-36 (SF-36) and imaging dates including digital radiographs in the lateral, hyperextension and flexion positions to assess the stability, sagittal balance and mobility of the cervical spine and magnetic resonance imaging (MRI) scans to assess the degeneration of adjacent segment. Results: There were 68 patients with 25 in AIDT group and 43 in ACDF group. Satisfactory clinical results were obtained in both groups, but the long-term NDI score and N-VAS score in the AIDT group were better. The AIDT obtained the same stability and sagittal balance of the cervical spine as fusion surgery. The range of motion of adjacent segments can be restored to the preoperative level after transplantation, but this increases significantly after ACDF. There were significant differences in the superior adjacent segment range of motion (SROM) between two groups at 12 months (P = 0.039), 24 months (P = 0.035), 60 months (P = 0.039) and the last follow-up (P = 0.011). The inferior adjacent segment range of motion (IROM) and SROM had a similar trend in the two groups. The ratio value of the greyscale (RVG) of adjacent segments showed a downward trend. At the last follow-up, the RVG decreased more significantly in the ACDF group. At the last follow-up, there was a significant difference in the incidence of ASDeg between the two groups (P = 0.000). And the incidence of adjacent segment disease (ASDis) is 22.86% in the ACDF group. Conclusion: The allograft intervertebral disc transplantation may be as an alternative technique to traditional anterior cervical discectomy and fusion for the management of cervical degenerative diseases. For the more, the results showed it would improve cervical kinematics and reduce the incidence of adjacent segment degeneration.

Abstract:Abstract Background The efficacy and safety of anterior cervical discectomy and fusion (ACDF) through mini-incision and posterior laminoplasty for long-level cervical spondylosis were investigated. Method From January 2018 to September 2019, clinical patients data with 3–4 segments (C3–7) cervical spondylotic radiculopathy, cervical spondylotic myelopathy, or mixed cervical spondylosis who received ACDF (42 cases) throughwith mini-incision or LAMP (36 cases) treatment were retrospectively collected and analyzed. The operative time, bleeding volume, incisive length, and hospital stay were recorded. Moreover, the intervertebral height, functional segment height, cervical lordosis, cervical hyperextension and hyperflexion range-of-motion (ROM) and ROM in all directions of the cervical spine before and after the operation were measured. Additionally, all relevant postoperative complications were also recorded. Then, the therapeutic effects of both surgical methods were investigated. Results Patients in the ACDF group had less bleeding, shorter incision, and fewer hospitalization days than the LAMP group. There was no significant difference in JOA, VAS score of the upper limb, NDI score after surgery between two groups. Postoperative intervertebral height and functional segment height in the ACDF group were significantly higher than those before the operation, and postoperative functional segment height of the ACDF group was significantly higher than that of the LAMP group. Moreover, the postoperative cervical lordosis angle in the ACDF group was significantly larger than the LAMP group. There was no significant difference between preoperative and postoperative ROM in all directions of the cervical spine for the two groups. Conclusions Both ACDF through mini-incision and LAMP are effective treatments for long-level cervical spondylosis. However, ACDF through mini-incision shows minor trauma, less bleeding, fast recovery, and it is beneficial for cervical lordosis reconstruction.

Abstract:Study design This is a meta-analysis and systematic review of the available literature. Objective In the case of severe foraminal stenosis, conducting uncinate process resection (UPR) during ACDF could achieve complete nerve root decompression and significant relief of neurological symptoms for CR. However, there is some controversy regarding its necessity and safety. This study aims to compare the safety and efficacy of ACDF with UPR and ACDF. Methods The following electronic databases were searched: Medline, PubMed, Embase, the Cochrane Central Register of Controlled Trials, Evidence Based Medicine Reviews, VIP, and CNKI. And the following data items were considered: baseline demographics, efficacy evaluation indicators, radiographic outcome, and surgical details. Results 10 studies were finally identified, including 746 patients who underwent ACDF with UPR compared to 729 patients who underwent ACDF. The group of ACDF with UPR had statistically longer intraoperative time (95% CI: 4.83, 19.77, P = .001) and more intraoperative blood loss (95% CI: 12.23, 17.76, P < .001). ACDF with UPR obtained a significantly better improvement of Arm VAS at postoperative first follow-up (95% CI: −1.85, −.14 P = .02). There was no significant difference found in improvement of Neck VAS at postoperative latest follow-up (95% CI: −.88, .27, P = .30), improvement of Arm VAS at postoperative latest follow-up (95% CI: −.59, −.01, P = .05), improvement of NDI (95% CI: −2.34, .33, P = .14), JOA (95% CI: −.24, .43, P = .56), change of C2-C7 lordosis (95% CI: −.87, 1.33, P = .68), C2-C7 SVA (95% CI: −.73, 5.08, P = .14), T1 slope (95% CI: −2.25, 1.51, P = .70), and fusion rate (95% CI: .83, 1.90 P = .29). Conclusion ACDF with UPR is an effective and necessary surgical method for CR patients with severe foraminal stenosis.

Abstract:Objective:To explore the feasibility as well as mid‐term and long‐term clinical efficacy of self‐locking and zero‐profile cage for the treatment of multi‐segmental cervical spondylosis .Methods : A total of 34 patients with cervical spondylosis ,who received anterior cervical discectomy and fusion (ACDF) with new fusion cage for at least three segments during Oct .2010 and Apr .2013 ,were enrolled .The neural and cervical function was evaluated with Japanese Orthopedic Association(JOA) scores and neck disability index(NDI) before surgery ,3 months after surgery ,12 months after surgery and at the last follow‐up .Meanwhile ,the cervical curvature and intervertebral height were evaluated by X‐ray at each time point and then compared with that before surgery .And the condition of cervical fusion was assessed by three‐dimensional computed tomography scan .Furthermore ,dom evaluation system was used to evaluate patients′satisfaction after operation .Results:The follow‐up averaged 24 .3 months (range 18‐30 months) .There were significant differences between the JOA and NDI scores at the 3rd ,12th month after operation and the last follow‐up and that at preoperative point (P< 0 .05) .The angles of cervical curvature and intervertebral height were improved postoperatively (P< 0 .05) and maintained throughout the follow‐up .CT scan demonstrated that the precise fusion rate and the total fusion rate were 62 .4% and 85 .1% respectively at the 6th month after operation .And the precise fusion rate ,the total fusion rate ,the excellent and good rate were 93 .1% ,100% ,and 88 .2%(30/34) ,respectively ,at the 12th month after operation .Conclusions :The self‐locking and zero‐profile cervical cage could be applied in multi‐segmental ACDF .It could maintain the cervical curvature as well as the intervertebral height after wide rage decompression ,and assure the success of fusion as well as the satisfactory outcome .

Abstract:ObjectiveTo compare the efficacy and safety of the postoperative long‐term effect of the treatment of single‐level cervical spondylosis through anterior cervical discectomy and fusion (ACDF) and artificial cervical disc replacement (ACDR). MethodsThis is a retrospective contrastive study, which was conducted for the period of January 2007 and January 2009 at the Department of Spine Surgery of the First Affiliated Hospital of Xinjiang Medical University. A total of 113 patients were divided into two groups depending on the operation method: ACDF group (fusion group, n = 66) and ACDR group (replacement group, n = 47). The ACDR group comprised of 23 males and 24 females. The age of these patients ranged from 31–60 years, with an average age of 42.89 ± 6.30 years. The ACDF group comprised of 38 males and 28 females. The age of these patients ranged from 28–73 years old, with an average age of 49.38 ± 9.89 years old. The evaluation index included the visual analogue scale (VAS), neck disability index (NDI), range of motion, dysphagia, adjacent vertebral disease, and related complications (prosthesis displacement, heterotopic ossification, etc.). ResultsA total of 113 patients met the inclusion criteria, and these patients receive more than 96 months of follow‐up. The VAS and NDI of these two groups of patients significantly improved, when compared with those before the operation. In the last follow‐up visit, the range of motion in the ACDR group and ACDF group was 43.22 ± 3.58 and 32.54 ± 2.82, respectively, and both are significantly different comparing to the values measured before the operation (P < 0.05). The dysphagia incidence of the ACDR group was higher than that of the ACDF group at the 36th month, but was lower than that of the ACDF group in other points time. In the last follow‐up visit, six patients (12.77%) in the ACDR group and 18 patients (27.27%) in the ACDF suffered from adjacent segment degeneration (ASD). The general complication rate in the replacement group and fusion group was 38.31% and 37.88%, respectively, but the difference between the two groups was not statistically significant (P > 0.05). ConclusionOverall, the clinical efficacy and related complication rate of single‐level cervical spondylosis after an anterior cervical approach operation was superior in the ACDR group when compared to the ACDF group.

Abstract:Background: We aimed to analyze the clinical outcomes and effectiveness of cervical biportal endoscopic spine surgery (C-BESS) and anterior cervical discectomy and fusion (ACDF) in patients with symptomatic cervical disc herniation. Methods: This study was a retrospective chart review of four-year clinical data involving 318 cases of symptomatic cervical disc herniation, with 156 patients undergoing the ACDF and 162 patients receiving the C-BESS. Preoperative and postoperative one-year data were collected. Results: The numeric rating scale and neck disability index showed statistically significant improvement for both ACDF and C-BESS groups. While showing a longer operation time and more blood loss during surgery compared to the ACDF group, the C-BESS group demonstrated a learning effect as the surgeon's proficiency increased with more cases. There was no significant difference in the postoperative length of hospitalization between the two methods. The subgroup with predominant arm pain revealed the statistical difference in arm pain intensity changes between the two groups (p < 0.001). The rates of complication were 2.6% for the ACDF group and 1.9% for the C-BESS group. Conclusions: C-BESS and ACDF are effective surgical treatments for patients with symptomatic single-level cervical disc herniation in relieving relevant pain intensities and pain-related disabilities.

Abstract:Objective:To assess the effecacy and safety of multilevel cage-assisted anterior cervical decompression and fusion as a treatment for cervical degenerative disease.Method:From June 2002 to March 2006,13 cases with cervical degenerative disease underwent three-level cage-assisted anterior cervical depression and fusion.The clinical results were assessed by JOA score and the radiological results including the cervical spine curvature and the anterior colomn height of fusion segments with respect to preoperation,postoperation and follow-up were evaluated.The fusion rate,incidence of cage subsidence and migration、adjacent degeneration were evaluated.Result:Spinal curvature increased by 3.3±2.9mm(P<0.01) and the anterior colomn height of fusion segments increased by 4.2±2.7mm(P<0.01) one week after operation.JOA score increased by 4.2±1.0 points one month after operation.All patients were followed up for an avarage of 31.3 months(range,12-59 months).At the final follow up,spinal curvature decreased by 1.1±1.9mm(P>0.05) and the anterior colomn height of fusion segments decreased by 1.8±2.4mm(P<0.05).The fusion rate was 91.7%,cage subsidence rate was 35.9%.Aadjacent degeneration was not found.Conclusion:Multilevel cage-assisted anterior cervical decompression and fusion is an effective and safe way if the indication is appropriate.
Efficacy of an allograft cellular bone matrix as an alternative to autograft in anterior cervical discectomy and fusion: radiological results & safety

Abstract:Background: The predominant surgical procedure employed for patients with symptomatic cervical radiculopathy is anterior cervical discectomy and fusion (ACDF). ACDF typically involves the use of an interbody cage augmented with iliac crest bone graft (ICBG) or local autograft to enhance fusion rate. Substantial complications can arise from autograft use, including donor site morbidity, difficulties with ambulation, and diminished quality of life. This study aims to evaluate the effectiveness and safety of an allograft cellular bone matrix (ACBM) as an osteopromotive bone, in ACDF procedures. Methods: This retrospective, single-center, consecutive case series included 73 patients who underwent an ACDF procedure. The surgical procedure involved the placement of an interbody cage supplemented with anterior plate fixation and an ACBM within the interbody spacer. Patient charts were reviewed to gather demographic information, radiographic findings, as well as perioperative and post-operative complications. Radiographic fusion was assessed at 6 and 12 months by a blinded, musculoskeletal-trained radiologist and a board-certified spinal surgeon reviewer. Any discrepancies were settled by a third, senior reviewer. Complete fusion was defined as: evidence of bridging bone across the disc space on CT, angular motion <3 degrees, and translational motion <2 mm on lateral radiographs. Complications were analyzed at 6, 12, and 15+ months post-operatively to assess clinical outcomes and device performance. Results: A total of 73 patients (50 males, 23 females) with an average age of 54.6 (range, 31-77) years underwent an ACDF procedure between C3-T1 with an ACBM. The breakdown of levels operated on was 26%, 32%, 34%, and 8% for one, two, three, and four level procedures, respectively. There were three patients who received spinal injections for pain within the first year post-operatively. There were two patients who required secondary surgery within the first 12 months where supplemental posterior hardware was needed. Notably, there were no instances of cage subsidence, cage migration, cage/graft removal, or reoperation. There were no cases of chronic dysphasia. At 6 months, 45% of patients with available imaging demonstrated complete fusion, while 97.4% of patients with available imaging demonstrated complete fusion at 12 months. Conclusions: At the 12-month follow-up, our study demonstrates a high fusion rate in a real-world population of up to 4 operative levels. There were no bone graft related complications or incidences of cage migration/subsidence. It is noteworthy that the study involved a significant number of multilevel cases (74% of cases). Despite this, our results align with historical fusion rates and provide support for the utilization of ACBMs as a fusion adjunct in ACDF procedures up to 4 levels.

Abstract:This is part two of a review of the status of biologically based complementary and alternative medicine (CAM) therapies that have potential anticancer activity, with a focus on those agents that may be active in childhood cancer. The review, presented in two parts, includes descriptions of the mechanisms that underlie their effects, the manner by which these agents are identified and evaluated, and discussion of some of the limitations of current research. The first part 1–81 introduced the topic of biologically based CAM therapies. The discussion included a description of the discovery of biologically based products for cancer, the classification of these compounds, and their use, from the perspectives of the Ayurvedic and traditional Chinese medicine (TCM) systems. Issues associated with evidence-based research of compounds used in these practices are addressed. This part focuses on preclinical and clinical studies of specific herbs or natural products and their bioactive constituents that demonstrate potential in cancer therapeutics. Turmeric, mistletoe, green tea, ginseng, mushrooms, and micronutrients are representative of compounds that exert their effects through mechanisms attributed to specific chemical classes. The section concludes with a discussion of the status of clinical trials of biologically based CAM therapeutics for cancer and the future direction of research.
