Secondary Trauma by Internet Content Moderation: a Case Report

Abstract:Introduction In recent years, a global debate has emerged regarding the protection of Internet users from exposure to harmful content. Content moderation is defined as the organized practice of filtering user-generated content posted on internet, social networks, and media to determine the appropriateness of the content for a site, locality, or jurisdiction. The growing volume of this content along with the psychological impact of this activity have promoted the application of automated approaches based on artificial intelligence and machine-learning. However, the changing characteristics of content, as well as the cultural differences that influence its appropriateness, mean that human moderation of Internet content currently continues to exist. Psychological effects of this activity such as symptoms of post-traumatic stress disorder (PTSD) could represent an example of secondary trauma. Objectives Our aim is to describe a clinical case of post-traumatic stress disorder presenting with specific traumatic exposure idiosyncrasy that could lead to a better consequence characterization of a recent social phenomena such as internet content moderation. Methods We expose the clinical case of a woman with emotional distress who was reffered to our outpatient psychiatric unit in Barcelona in 2022 after five years working as an internet content moderator. Results We describe the case of a 35-year-old woman without relevant medical, toxicologic or psychiatric record that presents to our out-patient psychiatric clinic with post-traumatic stress disorder after five years of working as an internet content moderator and being exposed to visual traumatic content such as sexual assault and paedophilia. The clinical presentation consisted with one year of recurrent daily panic attacks, intrusive images about the traumatic exposure, intrusive thoughts, insomnia, vivid nightmares, avoidance of exposure to her son, distrust of the environment and intense fear for her son security. The disorder interfered in her capacity to work. The patient received psychological treatment and ISRS (Sertraline) was prescribed, however only partial response was reached with persistence of the majority of symptoms. Conclusions The presented case suggests a temporal and symptom content relationship between the described work exposure and the appearance of emotional distress in a patient without PTSD history. Although previous evidence of secondary trauma in people exposed to indirect traumatic experiences has been reported, for example in healthcare professionals, the exposure to alien trauma through digital exposure as a workactivity is yet to be specifically examined. It is necessary to expand knowledge on the clinical expression of this phenomenon due to the observed recurrence of anxious and depressive symptomatology related to repeated exposure to traumatic content. Abstract:Emergency responders (police, fire, ambulance and defence force personnel) risk exposure to dangerous and traumatic events, and the possible subsequent development of post-traumatic stress disorder. Consequently, partners of these emergency responders risk developing secondary traumatic stress (STS) from vicarious exposure to the trauma through communication and engagement with their responders. A mixed-methods study of the partners of emergency responders in New Zealand examined the extent of such partner-associated STS. This article focuses on two research questions: to what extent were risk factors for STS identified within that population, and what did the participants believe may help them to mitigate the impact of STS. An online anonymous survey was developed and eligible participants completed a 17-item STS scale, a social support measure, and answered several open-ended questions. Of the 646 participants, twenty percent appear to be experiencing intrusion, arousal, and avoidance symptoms related to the trauma experienced by their responder. Almost half stated they have little or no emotional/informational support related to their responder’s work. Thematic analysis of free-text responses identified the need for additional support and more direct communication/engagement from the organisations for partners to navigate their experiences of STS and the level of social support received and required. The authors conclude with recommendations to emergency responder organisations.

SummarySocial workers have had a pivotal role in handling the challenges faced by people dealing with the coronavirus disease 2019 (COVID-19) pandemic. Supporting clients who have undergone traumatic experiences during a global pandemic may increase the risk of experiencing secondary traumatic stress. This study examined two protective factors that may be key in the association between stress and social workers' secondary traumatic stress: Psychological capital (internal protective factor) and satisfaction with supervision (external factor). The study sample included 104 Israeli social workers who worked in their profession during the first year of the COVID-19 pandemic. A regression-based path analysis was conducted to test the moderation model with the PROCESS software.FindingsSelf-efficacy significantly moderated the association between perceived stress and intrusion; increasing self-efficacy decreases perceived stress's effect on intrusion. Although no significant moderating effect was found for the existence of supervision, the interaction between perceived stress and satisfaction with supervision was significant in predicting secondary trauma.ApplicationsSocial workers' unions must work to improve the quality of supervision and raise awareness of the importance of supervision in general and particularly during times of crisis. Manageable workloads, a reasonable balance between work and home, and verbal and financial encouragement to seek professional psychological assistance would all be to the advantage of social workers at risk for secondary trauma.

Abstract:Summary Working with abused children can have positive and negative implications for social workers. Some of these implications have been conceptualized as secondary traumatic stress and vicarious post-traumatic growth. The current study examined the question of whether these two phenomena share similar contributing factors. Based on the social-ecological theory and previous studies on secondary traumatic stress, we examined the contribution of personal factors and level of exposure to trauma (i.e. years of work experience, professional exposure to child abuse, mastery) and environmental factors (i.e. social support, supervision, and role stress) to the explained variance of secondary traumatic stress and post-traumatic growth, with the goal of understanding these two potential outcomes. This cross-sectional study comprised 255 social workers who worked with abused children. Findings The results showed that despite the correlation between the two outcomes—post-traumatic growth and secondary traumatic stress—they shared only one common predictive factor: the extent of social workers’ exposure to abused children. Role stress was correlated positively, and mastery was correlated negatively, with secondary traumatic stress only. Years of work experience was correlated positively with post-traumatic growth only. Applications The current study shows the importance of continuing efforts to identify the factors that contribute to post-traumatic growth. At the same time, efforts should be made to foster certain personal and environmental factors to reduce secondary traumatic stress levels.

Abstract:Purpose Digital forensic investigators (DFIs) encounter traumatic material, and this is associated with the development of secondary traumatic stress (STS). Limited research has been conducted with UK DFIs, a significant concern given their role. This study aims to explore levels of self-reported STS by DFIs, the relationship to gender, years as a DFI, frequency of exposure to traumatic material and difficulty coping with such material. This study also aims to provide insight into the psychological impact, identify potential risk groups and explore coping strategies within this specific group of professionals. Design/methodology/approach A correlational cross-sectional design was conducted at a fixed point in time, with a sample of 114 currently employed DFIs, recruited through the Forensic Capability Network. Findings Mean STS scores fell within the moderate range, although 29.9% of participants reported high to severe levels. Significant correlations were found between STS total and subscales with difficulty viewing child sexual abuse material (CSAM). While females reported higher mean STS scores, the difference was not statistically significant, however, females did score significantly higher on the arousal sub-scale of the STS. Regression analysis included the variables (COPE scale and subscales, frequency of viewing child abuse material, years spent working as a DFI, age, gender) that identified mental disengagement, difficulty viewing CSAM and younger age as predictors of increased STS. Research limitations/implications Primarily, the reliance on self-report instruments lacks external validation of STS. Furthermore, possible response or selection basis could have stemmed from participants experiencing high stress. Hosting the study online hindered exploring this further, suggesting the potential for future research. Although the regression model explained 28% of STS variance, unaccounted factors remain, constituting 72% unexplained variance. A mixed method approach might unveil additional variables, addressing potential limitations. Additionally, this study was cross-sectional meaning that the authors cannot infer causation. Practical implications These findings underscore the need for educational efforts targeting DFI's to raise awareness about potential mental health risks associated with CSAM-related work. Equally crucial is emphasising the hazards associated with adopting negative coping strategies. Equipping DFI's with this knowledge may enable them to make informed decisions aimed at minimising the impact of job-related stressors. Moreover, it highlights the necessity of recognising DFIs as a group deserving access to professional and mental health support. It is pertinent to consider recent research highlighting the stigma of therapy and a prevailing perception of a "critical or judgemental workplace culture" among UK investigators. Social implications To the best of the authors' knowledge, this study represents the first known exploration of STS in UK-based DFI's, encompassing an investigation into potential risk and protective factors. A proportion of the sample reported experiencing mild to severe levels of STS, with the mean STS score falling within the moderate range. Notably, 29.9% of participants reported high to severe levels of STS. Originality/value The findings provide an inaugural exploration of STS among UK-based DFI's, offering crucial insight into the psychological impact, vulnerable demographics and coping strategies within this unique professional context. Practical implications based on the findings are considered.
Examining the Impact of Organizational Support on the Secondary Traumatic Stress of Mental Health Professionals Exposed to Workplace Violence

Abstract:Mental health professionals are at a heightened risk of secondary traumatic stress (STS) due to the higher prevalence of trauma-exposed individuals seeking clinical help compared to the general population. The aims of this study were as follows: (1) to explore the association between exposure to workplace violence (WPV) and secondary traumatic stress, and the potential mitigating effects of organizational support and (2) to examine how the workplace setting (inpatient vs outpatient) affects the experience of STS. The study was cross-sectional. Over 58 days, survey data was collected from mental health professionals working in both inpatient and outpatient settings. A total of 2,549 professionals responded. The overall prevalence of STS across settings was 87%. The highest reported secondary traumatic stress levels were among nurse practitioners/APRNs (49.29%), followed by nurses (47.94%) and psychiatrists (46.85%). Levels of STS occurred at slightly higher levels among professionals in outpatient settings (48.39%) than those in inpatient settings (45.11%). Increased exposure to WPV correlated with higher levels of STS, while higher levels of perceived organizational support (POS) had a moderating effect on STS levels. This study suggests that STS is a significant phenomenon and that mental health professionals are at risk for developing the condition. Organizational support can play a role in mitigating the effects of WPV and STS. Interventions aimed at improving organizational support and building psychological resilience may help reduce the prevalence of STS among this population.

A Socioecological Approach to Understanding Secondary Trauma in Professionals Working with Survivors of Sex Trafficking: A Hierarchical Regression Analysis

Abstract:Individuals who are trafficked for sex have high rates of trauma exposure prior to and while being trafficked; therefore, professionals who work with this population are potentially exposed to high levels of trauma details increasing their risk of developing secondary traumatic stress (STS). This study investigated the STS symptoms of professionals working with survivors of sex trafficking utilizing a socioecological framework to guide the design and analysis. An online survey was completed by 583 respondents from a broad range of organizational settings who completed measures tapping into STS symptoms, lifetime trauma exposures, history of being sex-trafficked, dose of direct and indirect trauma exposure at work, use of emotional and instrumental support to cope, state report cards on sex trafficking policies, and organizational-level practices toward being STS informed. STS scores among professionals working with survivors of sex trafficking were high, with those in child welfare settings reporting the highest levels of STS. Hierarchical regression analysis indicates higher STS was associated with variables at all levels of the socioecological model except the macrosystem, with fewer years of experience, a history of being sex trafficked, higher dose of indirect exposure, less use of emotional support, and lower organizational STS scores predictive of higher distress. Together, study findings indicate that STS is a significant concern in the anti-trafficking workforce and that a socioecological framework is useful for understanding STS impacts, highlighting the value of multiple response strategies across levels. This analysis suggests that organizational-level strategies to ameliorate/buffer impacts of occupation-related trauma exposure among these professionals can be especially impactful.

Abstract:South Africa is a particularly violent and traumatised society. Occupational and forensic social workers employed by the South African Police Service (SAPS) are confronted with many occupational stressors, including secondary traumatic stress (STS). STS can result in reduced functioning and/or performance in a social worker's professional and/or personal life. A mixed methods approach was adopted to explore the phenomenon of STS in the social workers employed at SAPS. In the first stage of the study, the Secondary Traumatic Stress Scale (STSS) was completed by 128 social workers. During the second stage of the study, 30 participants were purposively selected and interviewed about their responses to the constant exposure to traumatic material. Quantitative data were analysed through descriptive and inferential statistics and qualitative data were analysed through thematic analysis. Results showed that the level of STS among these social workers was a concern as many participants experienced symptoms of intrusion, avoidance and arousal. Differences in STS levels were found depending on the race and rank of the participants. This study highlights the need for social workers to adopt self-care practices and for policies to be implemented to ameliorate the effects of STS.
The role of cognitive trauma processing in the occurrence of secondary traumatic stress symptoms in professionals working with people after traumatic experiences

Abstract:Objectives: The aim of the study was to establish the relationship between cognitive trauma processing and secondary traumatic stress (STS) in professionals working with people after traumatic experiences. Methods: The results obtained from 500 persons representing five professional groups (therapists, paramedics, nurses, social workers and probation officers) were analyzed. The were more women (76.4%) than men (23.6%). The average age of the respondents is 44.09 years (SD = 9.85). The study used two standard measurement tools, i.e., the Secondary Traumatic Stress Inventory (STSI), and the Cognitive Processing of Trauma Scale (CPOTS), allowing assessment of five remedial strategies (positive cognitive restructuring, downward comparison, resolution/acceptance, denial, regret) and a survey developed for the use of the research. Results: The obtained results indicated that medical personnel, including paramedics and nurses, revealed the highest intensity of STS, and therapists - the lowest. STS was associated mainly with negative coping strategies, such as regret and denial, which play the predictive role for STS. Conclusions: There is a need to provide psychological assistance to professionals working with people after traumatic experiences, especially medical staff.

Abstract:OBJECTIVE: Secondary traumatic stress (STS), vicarious trauma (VT), and burnout (BO) are work-related outcomes commonly ascribed to mental health workers, given their exposure to clients' traumatic experiences. It is theorized that a worker's own history of trauma increases the occurrence of these outcomes, through retraumatization/activation of threat cues during client interactions and overinvolvement with a client's progress. Given the inconsistencies in the literature and the ubiquity of trauma among workers, a systematic review was conducted to examine the association of personal trauma and the 3 related, but separate, work outcomes.METHOD: A systematic search strategy was used across relevant research databases (Cochrane, JSTOR, PsycINFO, PubMed) for empirical studies conducted from 2000-2021. In accordance with PRISMA guidelines, a four-phase selection process was used, resulting in 39 studies identified meeting the inclusion criteria.RESULTS: A clear (positive) association between personal trauma history and STS and VT were identified, whereas mostly null findings were observed in regard to BO. The majority of studies were conducted in Western countries, adopted questionnaires as the primary means of data collection, and all but one were cross-sectional in design.CONCLUSION: In addition to a lack of diversity in study design, there were conceptual limitations to the research conducted (e.g., treating victims as a unitary group, neglecting the inclusion of mechanisms). To assist in moving the field forward, five research recommendations are outlined with the goal of creating greater clarity in the work-outcomes literature and increased nuance in how personal trauma is understood. Are parents at a higher risk for secondary traumatic stress?: How interviewing child victims impacts relationships with forensic interviewer's friends and family

Abstract:Background: Forensic interviewers are at a heightened risk for secondary traumatic stress (STS) due to their frequent interactions with victims of child maltreatment (Bonach & Heckert, 2012). To date, however, few studies have examined the negative effects of this work on interviewers' social and emotional well-being. Objective: The present study seeks to explore the effect of STS on the relationships of forensic interviewers, including those with friends, family, and their respective children. Participants and setting: Data are derived from a sample of 367 forensic interviewers (FIs) recruited from across the United States. Methods: The current study used a cross-sectional research design to obtain qualitative and quantitative data from an online survey of certified forensic interviewers. Results: Personal-level predictors of STS included interviewers' sex (β = 0.11, p = 0.02), trauma history (β = 0.13, p = 0.004), and frequency of socializing with family members outside of work (β = -0.12, p = 0.01). Work-related predictors included the frequency of direct (β = 0.10, p = 0.04) and indirect exposures to graphic details of child maltreatment (β = 0.09, p = 0.05), burnout (β = 0.58, p = 0.000), and years of experience investigating crimes against children (β = 0.10, p = 0.03). Factors such as parental status and external social support were not significantly associated with STS in the qualitative analysis. Conclusions: Forensic interviewers experience both positive and negative effects of exposure to crimes against children, with work-related factors being particularly impactful on the potential for STS.
Characterization of a Highly Conserved Histone Related Protein, Ydl156w, and Its Functional Associations Using Quantitative Proteomic Analyses*

Abstract:A significant challenge in biology is to functionally annotate novel and uncharacterized proteins. Several approaches are available for deducing the function of proteins in silico based upon sequence homology and physical or genetic interaction, yet this approach is limited to proteins with well-characterized domains, paralogs and/or orthologs in other species, as well as on the availability of suitable large-scale data sets. Here, we present a quantitative proteomics approach extending the protein network of core histones H2A, H2B, H3, and H4 in Saccharomyces cerevisiae, among which a novel associated protein, the previously uncharacterized Ydl156w, was identified. In order to predict the role of Ydl156w, we designed and applied integrative bioinformatics, quantitative proteomics and biochemistry approaches aiming to infer its function. Reciprocal analysis of Ydl156w protein interactions demonstrated a strong association with all four histones and also to proteins strongly associated with histones including Rim1, Rfa2 and 3, Yku70, and Yku80. Through a subsequent combination of the focused quantitative proteomics experiments with available large-scale genetic interaction data and Gene Ontology functional associations, we provided sufficient evidence to associate Ydl156w with multiple processes including chromatin remodeling, transcription and DNA repair/replication. To gain deeper insights into the role of Ydl156w in histone biology we investigated the effect of the genetic deletion of ydl156w on H4 associated proteins, which lead to a dramatic decrease in the association of H4 with RNA polymerase III proteins. The implication of a role for Ydl156w in RNA Polymerase III mediated transcription was consequently verified by RNA-Seq experiments. Finally, using these approaches we generated a refined network of Ydl156w-associated proteins.

Abstract:OBJECTIVE: This study examined the reliability and factor structure of the Secondary Traumatic Stress Scale (STSS) and the prevalence and correlates of secondary traumatic stress (STS) among home visitors.METHOD: Survey data were collected between 2015 and 2020 from 301 home visitors with caseloads. Participants completed the 17-item STSS, which assesses intrusion, avoidance, and arousal symptoms using the <i>DSM-IV-TR</i> diagnostic criteria. Internal reliabilities of the scale and subscales were measured and confirmatory factor analyses were performed to validate hypothesized model solutions. Symptom prevalence among the sample was calculated and linear regressions were conducted to examine whether personal and workplace factors were associated with STS.RESULTS: Analyses confirmed that the STSS had sound internal consistency and that both 3- and single-factor measurement models fit the data. Approximately 10% of home visitors met the clinical criteria for PTSD, though prevalence decreased to 8% after omitting an intrusion item that was endorsed by most respondents. Increased exposure to adverse childhood experiences and poorer work environment ratings were associated with increased STS. Non-Hispanic White race was associated with elevated arousal symptoms. No other personal or workplace factors were associated with scores on the STSS full scale or subscales.CONCLUSION: This study reaffirms that the STSS has sound psychometric properties, but it also raises questions about the prevalence and etiology of STS. Given the likely costs of PTSD to personal well-being and professional efficacy, further research is needed to advance the measurement and prediction of secondary traumatic stress. Abstract:The authors have developed, in dogs, a nonsuture vascular anastomosis utilizing plastic adhesive and a water-soluble spindle tube composed of 80% saccharose, 16% glucose, and 4% dextrin. The patency rate of end-to-end arterial anastomosis was 70% in arteries with an external diameter of 2.5 to 4.0 mm. The device is easily applied, saves time, and does not require any special instruments.

Abstract:In addition to high-risk and high-stress events that police officers routinely encounter, many are also exposed to extreme traumatic exposures or disasters caused by nature (eg. Hurricanes) and human action (e.g., terrorist attacks or plane crashes). These exposures can result in a variety of adverse reactions including post-traumatic stress disorder (PTSD), acute stress disorder, major depressive disorder and anxiety disorders. Understanding and accurately measuring the burden of disease arising from involvement in extreme events, on policing organizations and individual police officers is critical for policy makers and those who plan and deliver services. This systematic review synthesizes existing research on large-scale disasters, in order to further our understanding of how extreme events impact the mental health of police officers. The results found variability in the reported rates of mental disorder; however, there are some clear trends. Overall, the rates of PTSD among police officers that are consistently lower than those of civilians affected by the same disaster, and are lower than other occupations. This undoubtedly speaks to the resilience and training of members of policing organizations that prepare them for this work. Studies also demonstrate that reported distress in terms of acute stress disorder, anxiety and depression, continues to rise in some groups as time-elapsed from the event lengthens; suggesting a need to ensure that mental health supports are provided at later stages after the event.

Abstract:Police officers are subjected, daily, to critical incidents and work-related stressors that negatively impact nearly every aspect of their personal and professional lives. They have resisted openly acknowledging this for fear of being labeled. This research examined the deleterious outcomes on the mental health of police officers, specifically on the correlation between years of service and change in worldviews, perception of others, and the correlation between repeated exposure to critical events and experiencing Post-Traumatic Symptoms. The Cumulative Career Traumatic Stress Questionnaire- Revised (Marshall in J Police Crim Psychol 21(1):62−71, 2006) was administered to 408 current and prior law enforcement officers across the United States. Significant correlations were found between years of service and traumatic events; traumatic events and post-traumatic stress symptoms; and traumatic events and worldview/perception of others. The findings from this study support the literature that perpetual long-term exposure to critical incidents and traumatic events, within the scope of the duties of a law enforcement officer, have negative implications that can impact both their physical and mental wellbeing. These symptoms become exacerbated when the officer perceives that receiving any type of service to address these issues would not be supported by law enforcement hierarchy and could, in fact, lead to the officer being declared unfit for duty. Finally, this research discusses early findings associated with the 2017 Law Enforcement Mental Health and Wellness Act and other proactive measures being implemented within law enforcement agencies who are actively working to remove the stigma associated with mental health in law enforcement.

Abstract:Firefighters are trained to respond to a wide range of emergencies, and over the course of responding to emergencies, are exposed to a variety of potentially traumatic events. Repeated exposure to traumatic events may adversely impact firefighters' mental health. Combined with stigma surrounding help-seeking, lack of systematic resources and referral programs, and other barriers to care, firefighters do not routinely receive a continuum of trauma informed, assessment and treatment services from trained professionals. The purpose of this paper is to describe an evidence-informed approach to a reciprocal, collaborative model among social work and public health practice, policy, and research; and to offer a framework for researchers, clinicians, and policy advocates to more effectively achieve goals of preventing and treating trauma exposure in firefighters. This paper provides a comprehensive literature review, as well as describes a case study of a successful public health intervention for mitigating exposure to trauma among a population of firefighters. The significant contribution of mental health issues to the burden of disease among our firefighters is a matter of considerable public health concern. Effective approaches to mitigating behavioral health needs must bridge both mental health disciplines and public health to minimally, include access to evidence-based assessment and treatment, peer support capacity building, widely available mental health education, and leadership development to promote culture change. When considering a model for services with firefighters, reciprocal collaboration among researchers, health care professionals, first responder organizations, families, and other relevant community stakeholders is essential for success.
