Abstract:Abstract Background Diagnosis and follow-up of retinal diseases may be improved if the thickness of the various retinal layers, in addition to the total retinal thickness, is taken into account. Here we measured the thickness of the macular retinal layers in a population-based study group to assess the normative values and their associations. Methods Using spectral-domain optical coherence tomographic images (Spectralis®, wavelength: 870 nm; Heidelberg Engineering Co, Heidelberg, Germany), we measured the thickness of the macular retinal layers in participants of the population-based Beijing Eye Study without ocular diseases and without systematic diseases, such as arterial hypertension, hyperlipidemia, diabetes mellitus, cardiovascular diseases, previous myocardial infarction, cerebral trauma and stroke. Segmentation and measurement of the retinal layers was performed automatically in each of the horizontal scans. Results The study included 384 subjects (mean age:60.0 ± 8.0 years). The mean thickness of the whole retina, outer plexiform layer, outer nuclear layer,retinal pigment epithelium, inner retinal layer and photoreceptor layer was 259.8 ± 18.9 μm, 19.4 ± 3.9 μm, 93.4 ± 9.6 μm, 17.6 ± 1.9 μm, 169.8 ± 18.6 μm, and 90.0 ± 4.2 μm, respectively. In multivariable analysis, the thickness of the foveola and of all retinal layers in the foveal, parafoveal and perifoveal region decreased with older age (all P < 0.05), except for the thickness of the parafoveal outer plexiform layer which increased with age. Men as compared to women had higher thickness measurements of the photoreceptor layer and outer nuclear layer in all areas, and of all layers between the retinal nerve fiber layer and inner nuclear layer in the parafoveal area (all P < 0.05). The associations between the macular retinal layers thickness and axial length were not consistent. The inner plexiform layer was thicker, and the ganglion cell layer and inner nuclear layer were thinner, in the temporal areas than in the nasal areas, Conclusions The associations between decreasing thickness of most retinal layers with older age and the correlation of a higher thickness of some retinal layers with male gender may clinically be taken into account.

Abstract:Background: To report the thickness of the peripapillary retinal nerve fibre layer (pRNFL) in Chinese children and examine its association with refractive error, axial length (AL) and optic disc parameters. Design: Population-based cross-sectional study. Participants: A total of 2893 seven-year-old children from 11 randomly selected primary schools in Anyang, central China. Methods: Participants underwent ophthalmic examinations including optical biometry, cycloplegic autorefraction and spectral-domain ocular coherence tomography. Main Outcome Measures: Retinal nerve fibre layer thickness in 16 radial sections, cycloplegic spherical equivalent, AL. Results: The mean (SD) average RNFL thickness was 102.01(8.02) mu m. The average RNFL thickness decreased with smaller disc area (r = 0.18, R-2 = 0.03, P < 0.0001), bigger cup area (r = -0.11, R-2 = 0.01, P < 0.0001), smaller rim area (r = 0.28, R-2 = 0.08, P < 0.0001), smaller nerve head volume (r = 0.27, R-2 = 0.07, P < 0.0001), longer AL (r = -0.15, R-2 = 0.02, P < 0.0001) and a negative spherical equivalent (r = 0.11, R-2 = 0.01, P < 0.0001). Hyperopic children had a thicker RNFL than emmetropic children [102.45(8.13) mu m vs. 100.81 (7.18) mu m, P < 0.001]. Myopic children had thinner RNFL than emmetropic children [99.17 (7.69) mu m vs. 100.81 (7.18) mu m, P < 0.05]. Conclusion: Retinal nerve fibre layer thickness decreased with increasing AL, higher myopia, bigger cup area, smaller disc and rimarea, and a smaller nerve head volume, but the coefficient of determination for all these associations was small. The RNFL in myopes was significantly thinner than emmetropes or hyperopes, but with small absolute differences. The study provides RNFL values for healthy 7-year-old Chinese children. Follow up of this cohort to observe the change of RNFL thickness with myopia and possible change in detected associationswith age is planned.

Abstract:Background: Choroidal abnormalities (CAs) visualized on near-infrared reflectance (NIR) imaging are a new diagnostic criterion for neurofibromatosis type 1 (NF1), but the association between the presence of CAs and visual function remains unknown. This study evaluated the relationship between visual acuity (VA) with the presence, number, or total area of CAs visualized by NIR in children with NF1-associated optic pathway gliomas (NF1-OPGs). Methods: Patients ( 0.05). Conclusions: CAs are prevalent but not ubiquitous, in children with NF1-OPGs. Although CAs are a diagnostic criterion for NF1, their presence and size do not appear to be associated with visual function.

Abstract:OBJECTIVES To investigate the change of optic retinal nerve fiber layer (RNFL) thickness in nonproliferative diabetic retinopathy (NPDR) and to evaluate the correlation between the optic RNFL structural change and visual function. METHODS A cross-sectional study. All cases came from ophthalmology department of Zhejiang Province Traditional Chinese Medical Hospital, and the First People's Hospital of Ningbo, and Zhongshan Ophthalmic Center, Sun Yat-Sen University from December 2009 to October 2012. All the disease and control cases were coming from the same hospitals at the same period. Subjects were divided into 3 different groups: patients with NPDR (n = 89, 89 eyes), patients with diabetic mellitus but without diabetic retinopathy (NRD) (n = 96, 96 eyes) and disease-free controls (n = 115, 115 eyes). One eye of each subject was randomly selected for study. Participants aged from 40 to 70 years at baseline and all diabetic patients had a diabetic history of at least 5 years. Optic RNFL thickness of each subject was measured by spectral domain optical coherence tomography(SD-OCT). Visual function examinations including contrast sensitivity test in spatial frequencies of 1.5, 3.0,6.0, 12.0, 18.0 cycles per degree (c/d), pattern electroretinograms (PERG) and best corrected visual acuity (BCVA) assay. The analysis of RNFL thickness in each group was performed at four preset locations of the optic disc (i.e., temporally, superiorly, nasally, and inferiorly). To assess the structure-function relationship, the BCVA,contrast sensitivity, PERG-P50 amplitude and latency to optic RNFL thickness at each quadrant were analyzed in both NPDR and NRD groups. RESULTS The RNFL thickness at four preset locations of the optic disc (meanly, nasally, temporally, superiorly, and inferiorly) were (97.7 ± 13.0), (71.7 ± 10.3), (70.9 ± 13.3), (118.3 ± 19.7), and (123.1 ± 20.8) µm in the NPDR group; (98.6 ± 15.3), (74.8 ± 13.1), (71.8 ± 14.6), (119.5 ± 17.2), and (125.6 ± 19.9) µm in the NRD group;and (99.1 ± 11.8), (77.4 ± 12.6), (72.6 ± 13.2), (119.1 ± 18.1), and (127.1 ± 19.3) µm in the normal group. The nasally optic RNFL thickness was significantly different among the three groups (F = 8.56, P = 0.000). The thickness in the NPDR and in the NRD group were significantly thinner than that in the normal group (SNK-q test:q = 3.16, 3.11, respectively; both P < 0.05). In the NPDR group, the nasally optic RNFL thickness was significantly thinner than that in the NRD group (SNK-q test:q = 3.07, P < 0.05). The inferiorly optic RNFL thickness was also significantly different among the three groups (F = 3.841, P = 0.035). The thickness in the NPDR group was significantly thinner than that in the normal group (SNK-q test:q = 3.090, P < 0.05). Yet, the difference of inferiorly optic RNFL thickness between the NRD group and the normal or the NPDR group did not reach the level of statistical significance (SNK-q test:q = 2.101, 1.955, P > 0.05). The temporally or superiorly optic RNFL thickness,or the average thickness of optic RNFL did not differ significantly among the three groups (F = 0.985, P = 0.375; F = 0.333, P = 0.71; F = 0.975, P = 0.379, respectively). Contrast sensitivities (1.5, 3.0,6.0, 12.0, 18.0 c/d), PERG-P50 amplitude and latency were all significantly correlated with the RNFL thickness of the nasal quadrant (nasal = 0.28, 0.26, 0.3, 0.25, 0.45, 0.65, 0.48, respectively; P < 0.05) and the inferior quadrant (inferior = 0.25, 0.28, 0.27, 0.26, 0.28, 0.37, 0.71; P < 0.05) in the NPDR group. Contrast sensitivities in high spatial frequencies (6.0, 12.0, 18.0 c/d), PERG-P50 amplitude were also significantly correlated with the RNFL thickness of the nasal quadrant (nasal = 0.59, 0.45, 0.66, 0.33, respectively; P < 0.05) and the inferior quadrant (inferior = 0.46, 0.71, 0.52, 0.41, respectively; P < 0.05) in the NRD group. CONCLUSION The optic RNFL thickness of the nasal and inferior quadrant have been already reduced in patients with NPRD and are significantly correlated with the change of vision function.
Optical coherence tomography of the macular ganglion cell layer in children with neurofibromatosis type 1 is a useful tool in the assessment for optic pathway gliomas

Abstract:Background: Optic pathway glioma (OPG) is a feared complication to neurofibromatosis type 1 (NF1) since it can cause visual impairment in young children. The main goal of screening is to detect symptomatic OPGs that require treatment. Optical coherence tomography (OCT) has been suggested as a tool for detection of neuro-retinal damage. Aims: To investigate whether the ganglion cell layer assessed by OCT is a reliable measure to identify and detect relapses of symptomatic OPGs in children with NF1. Methods: Children (3-6 years) with NF1, with and without known OPG and children with sporadic OPG (S-OPG) resident in the Stockholm area, were invited and followed in a prospective study during a three-year period. Brain magnetic resonance tomography (MRI) had been performed in children with symptoms of OPG. Outcome measures were VA in logMAR, visual field index (VFI), average thicknesses of the ganglion cell-inner plexiform layer (GC-IPL), and peripapillary retinal nerve fiber layer (pRNFL). Results: There were 25 children with MRI-verified OPG and 52 with NF1 without symptomatic OPG. Eyes from NF1 patients without symptoms of OPG showed significantly better results in all four analyzed parameters compared to eyes with NF1-associated OPG. Mean GC-IPL measurements seemed stable and reliable, significantly correlated to pRNFL (correlation coefficient (r) = 0.662, confidence interval (CI) = .507 to .773 p<0.001), VA (r = -0.661, CI = -7.45 to -.551, p<0.001) and VFI (r = 0.644, CI = .452 to .774, p<0.001). GC-IPL measurements were easy to obtain and acquired at considerably younger age than pRNFL (5.6±1.5 vs 6.8±1.3; p<0.001). Conclusions: The mean GC-IPL thickness could distinguish well between eyes with OPG and eyes without symptomatic OPG in children with NF1. As thinning of GC-IPL assessed with OCT could indicate underlying OPG, it should be included in the screening protocol of children with questionable VA measurements and in particular in children with NF1.

Abstract:PURPOSE. To study the distribution of peripapillary retinal nerve fiber layer (RNFL) thickness in a population of 12-year-old children in central China using iVue-100 spectral-domain optical coherence tomography (SD-OCT).METHODS. Twelve-year-old students (n = 2105) from four randomly selected middle schools in Anyang, China, participated in the study. Each child underwent ocular examinations, including optical biometry, cycloplegic autorefraction, and SD-OCT (iVue-100). Glaucoma optic nerve head scan was performed to measure RNFL thickness. Only the children with a signal strength index higher than 45 were included in the analyses. Multivariate analyses were performed to examine the association of RNFL with demographic variables (e.g., sex, age, and body mass index [BMI]) and ocular variables (e.g., axial length and refractive error).RESULTS. Optical coherence tomography scans of adequate quality were available for 1955 children (92.9%). The mean (SD) RNFL thickness was 103.08 (9.01) mu m, with the mean (SD) thickest RNFL in the inferior quadrant (129.34 [14.90] mu m), followed by the superior (126.19 [15.24] mu m), temporal (82.98 [10.57] mu m), and nasal (73.82 [13.89] mu m) quadrants. The RNFL was thicker with shorter axial length (beta = -1.53, P < 0.0001) and with higher hyperopia (beta = 0.90, P < 0.0001). Girls had slightly thicker average RNFL thickness than boys (P < 0.0001). The RNFL thickness had no significant correlation with age or BMI.CONCLUSIONS. This study establishes normative peripapillary RNFL values of 12-year-old Chinese children as measured by iVue-100 SD-OCT. The RNFL thickness decreased significantly with increasing axial length and higher myopia.
Distribution and Classification of Peripapillary Retinal Nerve Fiber Layer Thickness in Healthy Young Adults

Abstract:Purpose: To report the distribution of peripapillary retinal nerve fiber layer (RNFL) thickness in healthy young adults, investigate factors associated with RNFL thickness, and report the percentage of outside normal limits (ONL) and borderline (BL) RNFL thickness classifications based on the optical coherence tomography (OCT) manufacturer reference database.Methods: Participants of the Raine Study Generation 2 cohort (aged 18-22 years) underwent spectral domain OCT imaging with an RNFL circle scan. Eyes with inadequate scans or optic nerve pathology were excluded. Linear mixed models were used to analyze associations.Results: Data were available for 1288 participants (mean age, 20.0 years). Mean RNFL thicknesses in right and left eyes, respectively, were global = 100.5 µm, 100.3 µm (P = 0.03); temporal = 73.1 µm, 68.9 µm (P < 0.001); superotemporal = 140.6 µm, 136.3 µm (P < 0.001); superonasal = 104.9 µm, 115.1 µm (P < 0.001); nasal = 79.7 µm, 79.1 µm (P = 0.09); inferonasal = 109.8 µm, 111.5 µm (P < 0.001); and inferotemporal = 143.2 µm, 143.6 µm (P = 0.51). Longer axial length was associated with thinner RNFL globally, nasally, inferotemporally, superotemporally, superonasally, and inferonasally, as well as thicker RNFL temporally. The prevalence of ONL and BL classifications was generally higher than the expected rates of 1% and 4%, respectively, in temporal sectors and lower than expected in nasal sectors. The prevalence of global BL classifications was lower than expected (right eye, 2.3%; left eye, 2.6%).Conclusions: Measured RNFL thickness differs with axial length and between right and left eyes. More reference data are needed to better define the normal limits of RNFL variation in different populations.Translational Relevance: This study provides an improved understanding of normal variation in RNFL thickness in young adults.

The Pattern of Retinal Nerve Fiber Layer and Macular Ganglion Cell-Inner Plexiform Layer Thickness Changes in Glaucoma

Abstract:Background/Aims. To investigate the patterns of retinal ganglion cell damage at different stages of glaucoma, using the circumpapillary retinal nerve fiber layer (RNFL) and macula ganglion cell-inner plexiform layer (GCIPL) thicknesses. Methods . In 296 eyes of 296 glaucoma patients and 55 eyes of 55 healthy controls, the correlations of mean deviation (MD) with the superior and inferior quadrant RNFL/GCIPL thickness (defined as the average of three superior and inferior sectors, resp.) were analyzed. Results . In early to moderate glaucoma, most of the RNFL/GCIPL thicknesses had significant positive correlations with the MD. In advanced glaucoma, the superior GCIPL thickness showed the highest correlation with MD ( r = 0.495 ), followed by the superior RNFL ( r = 0.452 ) (all; P < 0.05 ). The correlation coefficient of the inferior RNFL thickness with MD ( r < 0.471 ) was significantly stronger in early to moderate glaucoma compared to that in advanced glaucoma ( r = 0.192 ; P < 0.001 ). In contrast, the correlations of the superior GCIPL thickness with MD ( r = 0.452 ) in advanced glaucoma was significantly stronger compared to that in early to moderate glaucoma ( r = 0.159 ; P < 0.001 ). Conclusions . The most preserved region in advanced glaucoma appears to be the superior macular GCIPL, whereas the most vulnerable region for initial glaucoma is the inferior RNFL around the optic disc.

Prediction of results from subjective evaluation of real-time-capable tone-mapping operators applied to limited high-dynamic-range images

Abstract:The dynamic range of digital image sensors is growing faster than the reproducible dynamic range of standard display and print devices. In an assessment, we evaluated eight real-time-capable, high-dynamic-range (HDR) tone-mapping operators, using HDR images restricted to 12 bits per channel, and mapping to 8-bit low-dynamic-range (LDR) output data. In our survey, 51 test persons rated the subjective quality of LDR images of 15 scenes. For each scene, the operators were tested against linear tone mapping. From that survey, we derived three operators that performed better than linear mapping and evaluated their impact on exposure when using tone-mapping in an imaging chain. To mathematically predict the subjective quality of tone-mapped images, we tested five figures of merit against the empirical results of the survey. We propose a quality measure derived from the Naka-Rushton equation. We evaluate the proposed metric against four other image quality metrics, with our novel measure matching the subjective results best.
Normative Data and Minimally Detectable Change for Inner Retinal Layer Thicknesses Using a Semi-automated OCT Image Segmentation Pipeline

Abstract:Neurodegenerative and neuroinflammatory diseases regularly cause optic nerve and retinal damage. Evaluating retinal changes using optical coherence tomography (OCT) in diseases like multiple sclerosis has thus become increasingly relevant. However, intraretinal segmentation, a necessary step for interpreting retinal changes in the context of these diseases, is not standardized and often requires manual correction. Here we present a semi-automatic intraretinal layer segmentation pipeline and establish normative values for retinal layer thicknesses at the macula, including dependencies on age, sex, and refractive error. Spectral domain OCT macular 3D volume scans were obtained from healthy participants using a Heidelberg Engineering Spectralis OCT. A semi-automated segmentation tool (SAMIRIX) based on an interchangeable third-party segmentation algorithm was developed and employed for segmentation, correction, and thickness computation of intraretinal layers. Normative data is reported from a 6 mm Early Treatment Diabetic Retinopathy Study (ETDRS) circle around the fovea. An interactive toolbox for the normative database allows surveying for additional normative data. We cross-sectionally evaluated data from 218 healthy volunteers (144 females/74 males, age 36.5 ± 12.3 years, range 18-69 years). Average macular thickness (MT) was 313.70 ± 12.02 μm, macular retinal nerve fiber layer thickness (mRNFL) 39.53 ± 3.57 μm, ganglion cell and inner plexiform layer thickness (GCIPL) 70.81 ± 4.87 μm, and inner nuclear layer thickness (INL) 35.93 ± 2.34 μm. All retinal layer thicknesses decreased with age. MT and GCIPL were associated with sex, with males showing higher thicknesses. Layer thicknesses were also positively associated with each other. Repeated-measurement reliability for the manual correction of automatic intraretinal segmentation results was excellent, with an intra-class correlation coefficient >0.99 for all layers. The SAMIRIX toolbox can simplify intraretinal segmentation in research applications, and the normative data application may serve as an expandable reference for studies, in which normative data cannot be otherwise obtained.

He was diagnosed as hepatocellular carcinoma over 10 cm in diameter with chronic hepatitis type B. Because the tumor concurred with intra-hepatic metastasis along with lymph node metastasis, we planned transhepatic arterial embolization (TAE) as a first choice of treatment and performed twice. However, because of the tail inside part of the tumor was supplied from the gastroduodenal artery (GDA) and right gastroepiploic artery (RGEA) which were nutrient arteries, an effective TAE was impossible. The enhancement lesion that seemed viable was shown by enhanced CT. Therefore, a wrapping therapy (surgical decollateralization+silicon membrane sheeting) was performed under laparotomy for the purpose of cutting off blood supply to the part of the viable lesion in August of the same year. The tail inside part of the tumor that seemed viable fell into necrosis by wrapping alone, and the serum PIVKA-II level decreased within normal limit. As of ten months after the treatment, the patient is still alive without aggravation.

Abstract:Objective To evaluate changes in retinal nerve fiber layer (RNFL) and macular thicknesses, included ganglion cell-inner plexiform layer (GCIPL) thickness, in patients with spontaneous intracranial hypotension (SIH). Methods This was a retrospective, nonrandom, observational case series study. Comprehensive ophthalmic examinations and systemic examinations were performed. Spectral domain optical coherence tomography angiography scanning was used to measure peripapillary RNFL thickness and macular volume. Results In total, 108 eyes in 54 patients with SIH were evaluated; these were compared with 108 eyes in 54 healthy controls. The mean ages were 38.2 ± 9.4 years (patients with SIH) and 38.9 ± 9.4 years (healthy controls). In both groups, 33 patients were women (61.1%). The peripapillary RNFL and GCIPL were thinner in patients with SIH than in healthy controls (100.08 ± 9.94 µm vs 104.83 ± 8.35 µm and 81.46 ± 5.67 µm vs 85.67 ± 4.57 µm, respectively). Among patients with SIH, the GCIPL was thinner in patients with visual field defects (79.81 ± 5.62 µm vs 82.39 ± 5.12 µm). Conclusions The RNFL and GCIPL were thinner in patients with SIH than in healthy controls. The GCIPL was thinner in eyes with visual field defects among patients with SIH.

Abstract:The monokaryotic yeast phase of the heterobasidiomycete Itersonilia perplexans, unlike the hyphal phase, was found to be sensitive to mycocins produced by killer strains of Cryptococcus humicola, Cr. laurentii, Cystofilobasidium bisporidii and Rhodotorula fujisanense. Both the yeast and hyphal phases wer resistant to mycocins of Cr. podzolicus, Filobasidium capsuligenum, Rhodotorula glutinis, Rh. mucilaginosa, Rh. pallida, Sporidiobolus johnsonii, Sb. pararoseus and Sporobolomyces alborubescens. The different sensitivity patterns of yeast and hyphal phases are probably caused by biochemical differences in the cell walls.

Abstract:Objective This study intended to investigate whether retinal nerve fiber layer (RNFL) thickness could become a potential marker in patients with Parkinson’s disease with cognitive impairment (PD-CI). Methods Fifty-seven PD patients and 45 age-matched healthy controls (HCs) were recruited in our cross-sectional study and completed optical coherence tomography (OCT) evaluations. PD with normal cognition (PD-NC) and cognitive impairment (PD-CI) patients were divided following the 2015 Movement Disorder Society criteria. RNFL thickness was quantified in subfields of the 3.0-mm circle surrounding the optic disk; while a battery of neuropsychiatric assessments was conducted to estimate the Parkinsonism severity. General linear models and one-way ANOVA were adopted to assess RNFL thickness between subgroups with different cognitive statuses; logistic regression analyses were applied to determine the relation between RNFL and PD-CI cases. Results Compared with HCs, more thinning of the RNFL was observed in the inferior and temporal sectors in PD patients, especially in the PD-CI group. Inferior RNFL thickness was reduced in PD-CI compared with PD-NC patients. Logistic regression analysis found that inferior RNFL thickness was independently associated with PD-CI cases (odds ratio = 0.923, p = 0.014). Receiver operating characteristic analysis showed that the RNFL-involved combined model provided a high accuracy in screening cognitive deficiency in PD cases (area under the curve = 0.85, p < 0.001). Conclusion Reduced RNFL thickness especially in the inferior sector is independently associated with PD-CI patients. Our study present new perspectives into verifying possible indicators for neuropathological processes or disease severity in Parkinsonians with cognitive dysfunction.

Abstract:PURPOSE: To evaluate the association between macular choroidal thickness and peripapillary RNFL thickness in patients with optic atrophy due to acute idiopathic demyelinating optic neuritis and in normal controls using spectral domain optical coherence tomography (SD-OCT).METHODS: We performed SD-OCT peripapillary RNFL circular scan centered on the optic disc with horizontal and vertical crosshair scans through the fovea using the enhanced depth technique in 62 eyes with optic atrophy due to acute idiopathic demyelinating optic neuritis and 86 eyes of normal controls. The association between RNFL thickness and macular choroidal thickness measurements was assessed.RESULTS: The mean age was 43 ± 14 years (mean ± SD) in patients with optic atrophy and 45 ± 16 years in healthy controls (p = 0.791). There was a significant association between nasal peripapillary RNFL thickness and choroidal thickness at 3.0 mm nasal to the foveal center in patients with optic atrophy in multivariate analysis (estimate = 1.398, p = 0.011). In controls, there were significant associations between global average, superior, and inferior peripapillary RNFL thickness and choroidal thickness at 3.0 mm superior to the foveal center (estimate = -60.112, p = 0.044, estimate = 15.821, p = 7.312, and estimate = 15.203, p = 7.222, respectively).CONCLUSIONS: Our SD-OCT data revealed that there was a significant association between peripapillary RNFL thickness and macular choroidal thickness in patients with optic atrophy due to acute idiopathic demyelinating optic neuritis and in controls, although the mechanism remained unclear. The difference in the pattern of association between patients with optic atrophy and controls suggests that optic atrophy caused by acute idiopathic demyelinating optic neuritis could affect the pattern of association between peripapillary RNFL thickness and macular choroidal thickness.

Abstract:Neurofibromatosis type 1 (NF1) is a rare inherited neurocutaneous disorder with a major impact on the skin, nervous system and eyes. The ocular diagnostic hallmarks of this disease include iris Lisch nodules, ocular and eyelid neurofibromas, eyelid café-au-lait spots and optic pathway gliomas (OPGs). In the last years, new manifestations have been identified in the ocular district in NF1 including choroidal abnormalities (CAs), hyperpigmented spots (HSs) and retinal vascular abnormalities (RVAs). Recent advances in multi-modality imaging in ophthalmology have allowed for the improved characterization of these clinical signs. Accordingly, CAs, easily detectable as bright patchy nodules on near-infrared imaging, have recently been added to the revised diagnostic criteria for NF1 due to their high specificity and sensitivity. Furthermore, subclinical alterations of the visual pathways, regardless of the presence of OPGs, have been recently described in NF1, with a primary role of neurofibromin in the myelination process. In this paper, we reviewed the latest progress in the understanding of choroidal and retinal abnormalities in NF1 patients. The clinical significance of the recently revised diagnostic criteria for NF1 is discussed along with new updates in molecular diagnosis. New insights into NF1-related neuro-ophthalmic manifestations are also provided based on electrophysiological and optical coherence tomography (OCT) studies.

Abstract:OBJECTIVE: This study aimed to evaluate the retinal nerve fiber layer (RNFL) thickness and macular thickness in Parkinson's disease (PD) patients.METHODS: The present study is a comparative cross-sectional, hospital-based study. A total number of 64 PD patients and 64 controls were recruited. Candidates that fulfilled the criteria with normal ocular examinations were undergone optical coherent tomography (OCT) examinations of the right eye. RNFL and macular thickness were evaluated.RESULTS: There was a statistically significant reduction in RNFL thickness in average (adjusted mean 84.32 vs 95.93, p ≤ 0.001), superior (adjusted mean 105.15 vs 118.13, p ≤ 0.010), and inferior (adjusted mean 104.95 vs 126.55, p ≤ 0.001) PD group compared to the control group. The macula thickness also was significantly reduced in average (adjusted mean 266.51 vs 281.34, p = 0.015), central (adjusted mean 236.37 vs 255.55, p = 0.001), outer superior (adjusted mean 269.16 vs 278.19, p = 0.014), outer inferior (adjusted mean 256.34 vs 272.24, p ≤ 0.001), and outer nasal (adjusted mean 287.64 vs 302.84, p = 0.001) PD group compared to the control group. There was a significant positive correlation between RNFL thickness and visual acuity among PD patients in the inferior segment with p = 0.020 and nasal segment with p ≤ 0.001. There was also a significant positive correlation between macular thickness and visual acuity among PD patients in the inner temporal segment with p = 0.006, outer superior segment with p = 0.003, and outer temporal segment with p ≤ 0.001.CONCLUSION: The mean of the average, superior, and inferior RNFL thickness was significantly lower in the PD group compared to the control. The mean of the average, central, outer superior, outer inferior, and outer nasal macular thickness was significantly lower in the PD group compared to the control.

Abstract:Objective To investigate whether there is a thinning of the retinal nerve fiber layer (RNFL) in the eyes of diabetic patients and to analyze the relationship between RNFL thickness and several diabetic risk factors. Methods Forty-two type 2 diabetes mellitus patients aged 47-70 years were enrolled in this study. All the subjects underwent ophthalmologic examinations and glaucoma diagnosis (GDx) nerve fiber analyzer scanning with two different modes: variable cornea compensation (VCC) and enhanced cornea compensation(ECC).The GDx parameters included the temporal-superior-nasal-inferior-temporal average (TSNITave), superior and inferior averages, the TSNIT standard deviation and the nerve fiber indicator (NFI). The receiver operating characteristic (ROC) was applied to study the diagnostic ability of indices. The association between risk factors and the NFI was studied. Results The area under the curve for the ROC of the NFI was the largest for both the ECC and VCC modes. There was a significant relationship between age and RNFL thinning. However, there was no significant association between diabetic duration and fasting blood glucose with RNFL thinning. The influence of age on NFI was greater in diabetic patients than in control subjects. Conclusions Age has an important influence on the NFI of diabetic patients. Diabetic patients with normal ocular fundus may have RNFL thinning.

Abstract:Objectives: The current study aims to compare retinal nerve fiber layer (RNFL) thickness, macular thickness, and macular volume between children with ADHD and a control group. Method: The study group included children with ADHD and the control group consisted of age- and gender-matched participants without any psychiatric disorder. In all participants, RNFL thickness, macular thickness, and macular volume were measured by using spectral domain–optical coherence tomography (SD-OCT). ADHD symptom severity was evaluated by using parent-report measures, including Conners’ Parent Rating Scale–Revised: Short Form (CPRS-R: S) and the Strengths and Difficulties Questionnaire: Parent Form (SDQ: P). Results: We compared 90 eyes of 45 children with ADHD and 90 eyes of 45 controls. ADHD group had significantly lower RNFL thickness only in nasal quadrant than the controls. The remaining RNFL quadrants, macular thickness, and volume were not significantly different between groups. There was a reverse correlation between RNFL thickness and ADHD symptom severity. Conclusion: This is the first study examining the RNFL thickness in ADHD. Our findings showed that nasal RNFL thickness was lower, indicating reduced unmyelinated axons in the retina of children with ADHD. The results of this study support the evidence that ADHD involves a lag in cortical maturation and this is measurable in the retina.
