Abstract:Active collaboration between speech-language therapists (SLTs) and persons with aphasia (PWA) is essential for personalizing aphasia treatment and optimizing clinical outcomes during aphasia rehabilitation. Studies have demonstrated the feasibility and effectiveness of collaboration between SLTs and PWA during the goal setting component of the intervention process in aphasia rehabilitation. However, it remains unclear whether this collaboration as well as PWA's feedback can be applied and be beneficial to all components of aphasia rehabilitation. Here we introduce a theoretical framework of self-regulated learning (SRL) for aphasia rehabilitation that spotlights the role of PWA's feedback in all components of the rehabilitation process. We review studies on models of SRL in educational psychology and evidence from clinical aphasiology and neurolinguistics. Based on the evidence, we argue that PWA's feedback is beneficial for optimizing all components of the intervention process in aphasia rehabilitation. To make this work, communicative barriers to collaborative, patient-centered goal setting must be minimized or eliminated to facilitate alternative channels for receiving PWA's feedback which will promote their engagement in aphasia rehabilitation. Also, insights from PWA's feedback can help SLTs to select treatments and tasks that promote PWA's engagement in the intervention process, which, in turn, can maximize aphasia recovery and increase quality of life. PWA can be active participants in the whole intervention process and SLTs can use information from PWA's feedback to improve the personalization of the complete intervention process to maximize clinical outcomes.

What do speech-language pathologists do to support families' active involvement in early intervention? Exploring moment-to-moment interactions in sessions

Abstract:Purpose: There is growing emphasis on the importance of involving families in all aspects of the early speech-language pathology intervention process, however, both speech-language pathologists (SLPs) and families have reported that this can be challenging. To better understand how SLPs work together with families in intervention, this study aimed to (a) explore what SLPs do to support families to be actively involved in intervention, both inside and outside sessions, and (b) explore the perspectives of families regarding how SLPs supported their active involvement. Method: Video-reflexive ethnography (VRE) was used to explore engagement with 21 matched SLP-family dyads in early intervention services. Up to three intervention appointments for each participating family were video recorded. Short video segments were selected and played to participants in individual semi-structured interviews where participants were invited to reflect on the interactions captured (i.e. "video-reflexive sessions"). Interview data were analysed using template analysis. Result: Participants reported that SLPs supported families to be actively involved in sessions by (1) planning to involve families; (2) setting expectations; (3) modelling and teaching strategies; (4) inviting families to "have a go"; (5) providing feedback; and (6) problem solving together. Participants also reported that families were supported to be involved outside sessions when SLPs: (1) got them to practice in sessions; (2) made home practice achievable and provided resources; (3) asked about home practice; (4) linked home practice to progress; and (5) helped other family members get involved. Conclusion: The results of this study provide an explicit understanding of how SLPs can support families to take an active role in early intervention, and support the importance of a reflective and individualised approach to supporting engagement.
Person-centered care for people with aphasia: tools for shared decision-making

Abstract:Shared decision-making is a fundamental aspect of person-centered care, and can and should be part of many different aspects of the rehabilitation process. Communication disabilities like aphasia, which affects people's ability to use and understand spoken and written language, can make shared decision-making especially challenging to the resources and skills of rehabilitation practitioners. The purpose of this narrative review is to provide a comprehensive description of tools that can support successful shared decision-making with people with aphasia in the rehabilitation environment. These tools and strategies are appropriate for use by physicians, nurses, social workers, physical therapists (also referred to as physiotherapists), occupational therapists, and other service or care providers. The important role of speech-language pathologists as consultants is also described. Case scenarios throughout the paper illustrate the application of recommended tools and strategies along with best practices.
Topic generation in aphasia language therapy sessions: Issues of identity

Abstract:Background: Aphasia language therapy sessions have a general “order of phases” and usually open with a period of casual conversation. It has been noted that the overall structure of participation in therapy, characterised by therapist control, is anticipated in this opening phase. The detailed mechanisms through which control is achieved and sustained have not been fully examined. The processes through which these mechanisms operate and their relationship with participant roles and identities appear to be especially relevant for study if the overall goal of aphasia therapy is to address life participation and the social context of communication (e.g., Shadden & Agan, 2004). Aims: The concern of this paper is to examine in detail some of the mechanisms through which professional control is achieved and sustained, with a particular focus on ways in which speech and language therapists and people with aphasia work at generating and maintaining the topics that are observed to develop in the opening phase of aphasia language therapy sessions. In addition, there is a specific interest in how, in certain instances, the activity of topic generation and maintenance may be shown to relate to the participants' identities as “professional” and “patient”. Methods & Procedures: The paper focuses on analysis of data from the opening phase of five sessions, one each from three therapist/person with aphasia dyads, and two from separate sessions of a fourth dyad. Data from these sessions are taken from a larger study of aphasia language therapy in day‐to‐day practice. The turn‐by‐turn process of topic generation is examined in detail using techniques from Conversation Analysis in conjunction with a consideration of “membership categorisation” and “category‐bounded activities”. Outcomes & Results: Close examination of the process of topic generation and maintenance in five sessions from four different dyads revealed how therapists selected certain topics for topicalisation and rejected others. Selected topics were related to activities closely tied to the person with aphasia's identity as a person with communication impairment. In so doing the therapists established a particular type of therapeutic identity for themselves. Conclusions: Managing the person with aphasia's identity by addressing communication activities that are bound to the aphasic impairment and rejecting other types may, perversely, hamper a speech and language therapist's understanding of that person with aphasia's communicative competence, including broader issues that may be germane to intervention. Understanding and recognising where control resides and through which mechanisms it operates is of potential benefit to therapists and people with aphasia alike—for example, enabling more focused and realistic goal setting or more accurate insights into the impact of aphasia, and aphasia language therapy on everyday social participation. The challenge for therapists and for people with aphasia is to be able to shift roles appropriately and act as equals in the enterprise of therapy.

Conversational Therapy in Aphasia: From Behavioral Intervention to Neuromodulation

Abstract:This article describes the conversational therapy approach for the treatment of persons with aphasia (PWAs). Around 1970s, this approach was inspired by a series of pragmatic principles and techniques to aphasia rehabilitation whose main objective was to set up a condition of communicative exchange with the PWA using his/her own available communicative resources. Indeed, although language represents the most powerful behavior that humans use for communicating, within the conversational approach any intentional action (i.e., gestures, body movements, facial expression, drawing) can be used to communicate. For this reason, its application is particularly suitable for severe PWAs whose damage has compromised all the modalities of language (i.e., production, comprehension, reading, and writing). In this perspective, the speech-language pathologist's (SLP's) goal is not necessarily focused on restoring the damaged linguistic functions, still today pursued by the cognitive approach, but to ameliorate the use of language by teaching the PWA compensatory, productive strategies, and strengthening his/her residual communicative abilities. In this review, the fundamental principles of the conversational approach together with its modalities of treatment, which emphasize the importance of an active interaction between the SLP and the PWA, are reported. A brief summary of recent experimental evidence which combines conversational therapy with a noninvasive brain stimulation technique, transcranial direct current stimulation, is also included.

Abstract:Primary progressive aphasia (PPA) describes a group of language-led dementias. Speech and language therapy is the main available intervention for people with PPA. Despite best practice recommendations for speech and language therapy to include access to group therapies (Volkmer et al, 2023a), research evidence to date has predominantly focused on delivery in individual sessions. The aim of this study was to gather the collective intelligence of expert speech and language therapists/pathologists delivering group therapy for people with PPA to synthesize guidance for clinicians. This paper describes a qualitative study using narrative synthesis methods. Data were collected using the Template for Intervention Description and Replication - TIDiER. Eight respondents described a total of 17 different groups. Respondents worked across healthcare, research clinics and third sector organizations in Australia, Canada, Spain, the USA and the UK. For the purposes of analysis, groups were divided into two main types: (1) groups delivering specific therapy interventions; and (2) groups providing broader opportunities for conversational practice and support. This initial synthesis of the current state of the art in PPA therapy groups highlights several important considerations around candidacy, content and ecological validity of delivering group intervention for people with PPA.

Abstract:Aphasia, a communication disability commonly caused by stroke, can profoundly affect a person's mood and identity. We explored the experiences of stroke survivors with aphasia and depression who received a modified cognitive behavioral therapy (CBT)-based psychological intervention. The therapy is manualized with a flexible treatment protocol, including 10 individually based therapy sessions (+2 booster sessions) either via telehealth or in person. Six participants with chronic aphasia (60% of the total sample) participated in in-depth interviews that were analyzed using reflexive thematic analysis. Two core themes were derived from the data: the first theme, helpful elements of therapy—doing enjoyable activities, new ways of thinking, problem solving, working with the experienced therapist, and using telehealth; and the second theme, making progress—mood, communication, acceptance of the 'new me', and improving relationships. All participants found the therapy to be helpful in managing mood problems with various elements being beneficial depending on the individual, highlighting the importance of tailoring the intervention. Therefore, delivering modified CBT to individuals with aphasia is likely to be acceptable both in person and through telehealth. Further evaluation of the intervention and its impact on mood would be beneficial.

Abstract:Background Persons with aphasia (PWA) experience a number of communicative and social‐emotional challenges. Reported experiences of PWA include but are not limited to, being misunderstood, isolated, frustrated, and infantilised. Aims The aim of this pilot study, involving a Life Participation Approach to Aphasia (LPAA), conducted over the course of 2 years, was to understand community perceptions of aphasia while PWA engaged in an interprofessional treatment program involving speech and drama therapy. Methods & Procedures The interprofessional treatment program involved PWA participating in a therapeutic theatre program using the CoActive Therapeutic Theater (CoATT) while also receiving speech‐language therapy. Each year, the PWA performed a different, original therapeutic theatre production for a public audience, at the culmination of their interprofessional treatment program. In this paper, we share data obtained from perspectives of audience members who witnessed the theatre production and aphasia education during the first year of the study and friends and family of PWA who participated in the therapeutic theatre process during the second year of the study. Outcomes & Results Responses from audience members who participated in aphasia education and witnessed the therapeutic theatre performance by the PWA during the first year, indicated an increased knowledge of aphasia. Friends and family members of PWA who witnessed their loved ones engaging in the CoATT process through interprofessional treatment, in the second year, reported that their loved ones benefited from the CoATT process, which was distinct from other therapeutic processes to their knowledge and that they were impacted by watching their loved one perform. Conclusions & Implications These initial findings create footing towards understanding impact of therapeutic theatre in combination with speech‐language therapy in the lives of PWA. They help us to obtain an initial appreciation of how therapeutic theatre and aphasia education help connect PWA and their community. WHAT THIS PAPER ADDS What is already known on this subject Caregivers and communities at large play a significant role in and substantially impact the recovery of their loved ones (Dalemans et al., 2010; Grawburget et al., 2013; Kniepmann & Cupler, 2014) with aphasia. However, existing research suggests that persons with aphasia (PWA) are often misunderstood, isolated and infantilised by their communities. What this paper adds to existing knowledge The findings of our study reveal that friends, families and extended communities of PWA gain a positive and deep understanding of challenges experienced by PWA through therapeutic theatre supported by speech language therapy, based in a new CoActive Therapeutic Theatre (CoATT) model. This PWA community also agrees that therapeutic theatre in combination with speech‐language therapy provides confidence and camaraderie between PWAs and strengthen connection between all constituencies. These results support the need for interprofessional intervention within the framework of a Life Participation Approach to Aphasia (LPAA). What are the potential or actual clinical implications of this work? Treatment paradigms that bring PWAs in contact with their communities using an LPAA approach can increase confidence and social engagement for PWAs potentially leading to better outcomes for their individual speech‐language therapy as well as create means of educating communities about PWA, and their stories.

Participatory Development of a Speech-Language Telerehabilitation Intervention combined with Home-based Transcranial Direct Current Stimulation for Primary Progressive Aphasia: a Qualitative Study

Abstract:Purpose Intensive speech-language therapy (SLT) delivered as telerehabilitation in combination with transcranial direct current stimulation (tDCS) may be an effective treatment option for people with primary progressive aphasia (pwPPA). However, such combined treatment approaches are technically challenging and feasibility for pwPPA has not yet been established. We aimed to involve stakeholders in the development of a novel approach combining remotely-supervised but independently self-administered home-based tDCS with an intensive aphasia telerehabilitation (naming and communicative-pragmatic therapy). Method Eight pwPPA and 16 caregivers participated in two semi-structured interviews to identify the needs, preferences, and perceived barriers and challenges with regard to SLT, telerehabilita-tion and tDCS, and the combination of all components. Based on the results, a step-by-step manual was developed and tested by means of home-based usability tests and follow-up inter-views involving four dyads. Interview data were transcribed and analysed qualitatively and quantitatively. Competency checklists used in the usability tests were analysed quantitatively. Results PwPPA and their caregivers provided valuable insights into all aspects of the planned treat-ment programme. Overall, the findings suggest a high level of acceptance and perceived need for an intensive telerehabilitation SLT approach combined with tDCS. Using the developed step-by-step manual and training, pwPPA were able to independently perform more than half of the actions required for telerehabilitation but needed assistance with technically more de-manding aspects of tDCS. To ensure feasibility, caregiver-assistance is needed to support pwPPA during technically challenging interventions. Conclusions This mixed-methods study identified needs and preferences of pwPPA and their caregivers with regard to speech-language telerehabilitation, as well as barriers and challenges regarding telerehabilitation and home-based tDCS. We demonstrate high acceptability and initial feasibil-ity of such combined programmes. Our findings highlight the importance of stakeholder in-volvement in intervention development, which will inform future development and optimisation of technologically demanding intervention programmes.
Preliminary outcomes from a pilot study of personalised online supported conversation for participation intervention for people with Aphasia

Abstract:Aphasia negatively impacts face-to-face social participation and the difficulties that people experience using the phone exacerbate these challenges in staying in touch with family and friends. Videoconferencing enables multimodal communication, and teamed with supported conversation, could facilitate access to conversation and thereby increase social participation for people with chronic aphasia.This pilot study examined whether supported conversation provided over Skype could improve people's social participation. It reports on preliminary outcomes of this intervention on people's social network, communication confidence, aphasia-related quality of life, and mood.29 participants with chronic aphasia received an initial 2-h technology training session followed by 16 h of online-supportedThe intervention was personalised by individualising goals in technology, communication, and participation. An observational prospective cohort study design was used with baseline, immediately post-intervention, and 8-week follow-up assessments. Measures of social network and communication confidence (primary outcome measures), and aphasia-related quality of life, life participation, and mood (secondary outcome measures) were undertaken. Shapiro-Wilk tests were conducted to examine normality of distribution of each variable. Where data were normally distributed, one-way repeated-measures ANOVAs were used to examine the effect of time. Where data were not normally distributed, Wilcoxon Signed Ranks test was used.Outcomes & Results: 27 participants completed the intervention. As a group, participants reported significantly more social contacts, more life participation, and higher aphasia-related quality of life post-intervention, which were maintained. There was a group gain on the measure of communication confidence post-intervention, although this was not maintained. As a group, the participants' mood did not significantly change through intervention and follow-up. Individual variability was noted across all outcome measures.Conclusions: These preliminary findings suggest that relatively low dose and non-intensive online supported conversation for participation intervention delivered by qualified or student speech and language therapists improved social participation in some people with aphasia and improved their quality of life. Communication confidence also improved for some, although benefits were short term. Findings make novel contributions to the existing supported conversation evidence base with positive social participation and quality of life outcomes, likely achieved by the explicit participation focus. Whilst preliminary findings are positive, study limitations need addressing. Further investigations are merited to refine the intervention and outcome measure choice and capture feasibility data. Finally, a definitive controlled trial is needed to explore the clinical efficacy and cost-effectiveness.

Abstract:Introduction Treatment intensity for people with aphasia (PWA) is a significant factor in enhancing recovery. Personal factors such as fatigue, physical endurance and motivation as well as clinician availability have been described as barriers to increased intensity. The use of student therapists has been shown to assist with addressing service gaps. Methods The aim of the study was to explore the experiences of PWA who received enhanced community-based treatment delivered by Speech Language Pathology (SLP) students. Enhanced community-based treatment was defined as three or more treatment sessions per week targeting communication. Semi-structured interviews were conducted with 10 PWA living in New Zealand. Interview data was analysed with reflexive thematic analysis. Results Two themes and six sub-themes were developed. Theme 1 related to intensity and included more treatment is better than less (sub-theme 1), there’s a “right time” for more intensive treatment (sub-theme 2), the hard work is worth the effort (sub-theme 3). Theme 2 related to working with SLP students; it didn’t feel like they were students (sub-theme 4), we just got on so well (sub-theme 5), and they listened to what I wanted (sub-theme 6). Conclusions The findings confirm PWA value access to more intensive treatment and desire involvement in decisions about flexible treatment schedules. PWA have positive experiences when treatment is provided by SLP students. Implications for clinical practice and future research directions are discussed.

Abstract:Background: The progressive interest in social activities within the life and clinical efforts of persons with aphasia makes it necessary to focus more clinical attention on conversation as a site for therapy and as a goal for intervention. A number of innovative approaches have been designed, but most fall sort when authentic conversation is considered. Aims: This article provides one alternative to those intervention approaches. It aims to improve the interactional abilities of persons with aphasia by addressing authentic conversation. The intervention approach, Facilitating Authentic Conversation (FAC), is described in detail by providing its theoretical basis, the practical guidelines for its organisation and design, and its implementation procedure. Additionally, a case study is provided that addresses two questions: does this approach reduce targeted problematic behaviours in conversations and does this approach increase the utilisation of effective strategies during conversations. Methods & Procedures: Constructivism and conversation analysis are used to design a therapy approach that includes an analysis procedure and various strategies to achieve a therapeutic effect during authentic conversation. Using qualitative and quantitative techniques derived from conversation analysis, evidence for change within conversations is provided. Outcomes & Results: Evidence suggests that this intervention approach assisted in changing the conversational behaviours of the person with aphasia (PWA) who served as the case study. Two conversational strategies that were highlighted via several specific therapeutic techniques were found to have reduced specific problematic conversational behaviours 19 months post-onset. Conclusions: The article describes an innovative therapeutic approach that employs authentic conversation as the primary vehicle for intervention. The case study provided with this description provides evidence that identified problematic behaviours can be modified through constructivist principles that expose the PWA to beneficial strategies used to overcome conversational barriers due to aphasia.

Qualitative Exploration of Speech Pathologists' Experiences and Priorities for Aphasia Service Design: Initial Stage of an Experience-Based Co-Design Project to Improve Aphasia Services

Abstract:Introduction and aims: Stroke survivors with aphasia (impaired language/communication) have poor outcomes and gaps in the clinical implementation of best practice contribute to this. Little is known, however, about speech pathologist perspectives on the touchpoints (key moments shaping experiences) in the clinical care pathway that have the greatest impact on service delivery nor how this varies by geographical location. We explored the experiences of speech pathologists who provide aphasia services to establish priorities for improvement and design. Methods and analysis: This is the initial experience gathering and priority identification stage of an experience-based co-design (EBCD) project. Speech pathologists were recruited from 21 geographically diverse Hospital and Health Services in Queensland, Australia. Speech pathologists working in acute, rehabilitation and community services shared positive and negative experiences of delivering aphasia care in interviews and focus groups. Experiential data were analysed using qualitative thematic analysis to determine touchpoints. Priorities for service design were identified using an adapted nominal group technique. Results: Speech pathologists (n = 62) participated in 16 focus groups and nine interviews and shared 132 experiences of delivering aphasia care. Providing care in teams with poor awareness of the impacts of aphasia was identified as a key challenge, as poor patient-provider communication was perceived to increase risk of adverse outcomes for patients. Speech pathologists identified areas for improvement related to their own professional needs (e.g., greater access to clinical supervision); collaborative health care (e.g., better coordination and interdisciplinary care to increase therapy time); and the service context and environment (e.g., psychological services able to support diverse communication needs). Conclusions: Speech pathologist delivery of aphasia services could be improved through increased access to clinical supervision, opportunities for peer debriefing and interdisciplinary care. Priorities for service design varied by geographical location and included: education to support care transitions (remote areas), improved referral pathways and service linkage (regional areas) and dedicated aphasia staffing (metropolitan areas). Patient or public contribution: A consumer advisory committee comprising people with aphasia (n = 3, authors K.M., K.D. and B.A.), their significant others (n = 2, authors J.D. and P.M.), and a Cultural Capability Officer (author G.B.) guided this research. Abstract:This research was designed to deepen the understanding of contributing factors to disparities in speech-language pathology (SLP) service delivery to children of diverse backgrounds with language and communication delays, as well as to initiate community connections. We sought to understand the perspectives and experiences of those who provide or oversee SLP services to children with communication disorders in early intervention settings in Washington State. Individual semistructured interviews were completed with six SLPs and three administrators across five organizations. The interviews were designed to understand both individual- and systems-level contributing factors to service delivery challenges. Interviews were analyzed using an iterative coding process with multiple rounds of coding and multiple coders. In addition to synthesizing participants' understandings of, and preparation for, culturally responsive practice, three overarching themes are presented: (1) assets of early intervention as a service delivery model, (2) the distance between families and the practitioners and systems that serve them, and (3) compromised access and quality of services due to a predominantly monolingual-English system. Findings inform the next steps for building community partnerships with early intervention organizations. The long-term goal of this work is to collaboratively develop a participatory research program on equitable service delivery that addresses children's, families', and SLPs' needs. Abstract:The objective of the present research was to evaluate the potential of NIR diffuse reflectance spectroscopy as a way for nondestructive measurement of loquat soluble solid content of three varieties harvested from two orchards. According to the analysis, it was shown that the correlation coefficient of curves was relatively high in the two regions of 1400-1500 nm and 1900- 2000 nm. Statistical models were developed using partial least square regression (PLSR), stepwise multiple linear regression (SMLR) and principal component regression (PCR) methods in the full spectral region from 800 to 2500 nm and in the combined region of 1400-1500 nm and 1900-2000 nm. The results of PLSR model in the full spectral region were better than those of other models. The modeling results based on derivative spectra were not as good as those based on original spectra. Finally, a model was established based on the original spectra with 17 points smoothing for all the three varieties in the full spectral region by PLSR method. The correlation coefficients of calibration and validation were 0.96 and 0.95, respectively. The results indicate that it is feasible to use NIR spectroscopy technique for quantitative analysis of loquat soluble solid content.
'It's been an extraordinary journey': Experience of engagement from the perspectives of people with post-stroke aphasia

Abstract:Background: Engagement is recognized as an important factor in aphasia treatment response and outcomes, yet gaps remain in our understanding of engagement and practices that promote engagement from the client perspective. Aims: The purpose of this phenomenological study was to explore how clients with aphasia experience engagement during their inpatient aphasia rehabilitation. Methods & procedures: An interpretative phenomenological analysis approach guided the study design and analysis. Data were collected through in-depth interviews with nine clients with aphasia, recruited through purposive sampling, during their inpatient rehabilitation admission. Analysis was completed using a variety of analytic techniques including coding, memoing, triangulation between coders and team discussion. Outcomes & results: The analysis revealed that for clients with aphasia in the acute phrase of recovery, the rehabilitation process resembles travelling on a journey through a foreign land. Successful engagement in the journey was accomplished when one had a therapist who served as a trusted guide and was able to be a friend, invested, adaptable, a co-creator, encouraging and dependable. Conclusions & implications: Engagement is a dynamic, multifaceted and person-centred process involving the client, provider and rehabilitation context. Findings from this work have implications for measuring engagement, training student clinicians to be skilled facilitators in engaging their clients and implementing person-centred practices that promote engagement within clinical settings. Capturing the client perspective can provide novel insights regarding practices to foster and maintain engagement in aphasia rehabilitation. Future work is needed to explore barriers and facilitators to enacting engagement practices, in order to develop and test strategies to support practice change.
More than one way to improve a CAT: Outcomes and reflections on two iterations of the Queen Square Intensive Comprehensive Aphasia Programme

Abstract:Background: The field of human expert performance teaches us that high quality, high-dose guided practice is required to make large gains in cognitively driven acts. The same also seems to be true for people with acquired brain injury, yet therapy services for people with aphasia (PWA) have traditionally not been designed with this in mind. Intensive Comprehensive Aphasia Programmes (ICAPs) are one way to address the chronic under-dosing of therapy that most PWA experience. Aims: There are several ways to deliver an ICAP; here we describe two iterations of our Queen Square ICAP. There was a 20-month COVID-induced pause between the Year 1 (Y1) and Year 2 (Y2) ICAP groups. We analyse ICAP-induced changes in both groups of PWA on a series of key outcome measures that span the International Classification of Functioning, Disability and Health, covering language impairment and function as well as mood and social participation. Methods & procedures: Forty-six PWA took part in Y1 and 44 in Y2. The PWA were all in the chronic stage post stroke and varied in aphasia severity from mild to severe, with the Y2 group being more impaired than Y1. Quantitative data was collected before and after the ICAP. The Y2 therapy team provided independent reflections on their experiences of delivering an ICAP. Outcomes & results: ICAP-related changes in outcome measures (impairment, function and goal attainment) were generally comparable for the Y1 and Y2 groups, with both groups' speech production abilities improving the most. Both groups made clinically and statistically significant gains on the main quality of life measure. Participation in the ICAP made a big difference to PWAs' self-confidence ratings. Their mood ratings also improved significantly, although they were not, on average, in the depressed range at baseline (directly pre-ICAP). All improvements achieved in both groups were maintained at the 3-month follow-up, highlighting the lasting effects that ICAPs can provide. Conclusions: Evidence continues to accrue that ICAPs are an efficient way of increasing the dose of expert coaching required for people with chronic aphasia to make clinically meaningful improvements in their communicative abilities and quality of life. The main challenge remaining is convincing health-care providers to invest in them.
Development of a self-management intervention for stroke survivors with aphasia using co-production and behaviour change theory: An outline of methods and processes

Abstract:Background Self-management is a promising approach to improve quality of life after stroke. However, evidence for the appropriateness and effectiveness of self-management for stroke survivors with aphasia is limited. This article reports on the process used to develop a supported self-management intervention for stroke survivors with aphasia (SSWA) using co-production and behaviour change theory. Preparatory research included systematic reviews, and qualitative interviews and focus groups with SSWA, family members and speech and language therapists (SLTs). Materials and methods We conducted six, 2 hour long intervention development workshops with key stakeholders. The workshops were informed by principles of co-production and the intervention development process outlined by the Behaviour Change Wheel (BCW). We also incorporated the findings of our preparatory research within workshops. Each workshop included an introduction, 1–2 co-production tasks and time for feedback at the end of the session. Data were analysed on an ongoing basis so that findings could be used to feed in to subsequent workshops and intervention development. Results Workshop participants (n = 12) included; SSWA (n = 5), family members (n = 3) and SLTs (n = 4). Together, participants engaged with accessible and participatory co-production tasks which aligned with the BCW framework. Participants engaged in discussion to define self-management in behavioural terms (behavioural diagnosis) and to identify what needed to change to support self-management. Participant’s co-produced solutions for supporting self-management and discussed options to implement these in practice. Prototype materials were generated by the research team and evaluated by participants. Intervention functions and behaviour change techniques (BCTs) were mapped to the solutions generated by participants by the research team, after the final workshop. A supported self-management intervention for SSWA was developed which will be delivered by SLTs through community stroke services. Conclusions This paper reports the process we used to integrate co-production work with behaviour change theory to develop a complex self-management intervention. This is of relevance for researchers looking to harness the strengths of co-production methods and theory in intervention design. Future research will feasibility test the supported self-management intervention developed. This paper provides transparency to our intervention development process which will help others to better interpret the findings of our feasibility work.

Abstract:Aphasia, a communication disability prevalent among stroke survivors, significantly impacts psychosocial well-being and quality of life. Intensive Comprehensive Aphasia Programs (ICAPs) provide intensive and comprehensive treatment, incorporating individual and group therapy, patient and family education, and technology utilization. Despite positive outcomes, global implementation of ICAPs remains limited. In Brazil, preliminary evidence supports the development of intensive and comprehensive aphasia programs, such as the Brazilian Aphasia House—a program inspired by the successful Aphasia House model in the United States. The Brazilian Aphasia House offers personalized and intensive therapy in a simulated domestic environment, encompassing individual and group therapy sessions, and patient and family education, with the goal of enhancing functional communication skills and overall quality of life. Operating as a teaching clinic within a public university, it also serves as a training ground for future therapists to expand ICAP availability in Brazil. Challenges in implementing ICAPs in Brazil include socioeconomic barriers, limited public aphasia awareness, a shortage of specialized professionals, transportation, and session costs. Sustainable implementation requires committed leadership and financial resourcing, with involvement from public institutions and universities. By increasing awareness and nationwide implementation of ICAPs, access to aphasia rehabilitation services can be improved, potentially leading to better outcomes for individuals with aphasia and their caregivers. The Brazilian Aphasia House serves as an exemplar of an ICAP, offering intensive and personalized therapy to enhance communication abilities and promote social participation for people with aphasia.
