Abstract:Polymethylmethacrylate (PMMA) bone cement extensively utilized for the treatment of osteoporotic vertebral compression fractures due to its exceptional handleability and mechanical properties. Nevertheless, the clinical application of PMMA bone cement is restricted by its poor bioactivity and excessively high modulus of elasticity. Herein, mineralized small intestinal submucosa (mSIS) was incorporated into PMMA to prepare a partially degradable bone cement (mSIS-PMMA) that provided suitable compressive strength and reduced elastic modulus compared to pure PMMA. The ability of mSIS-PMMA bone cement to promote the attachment, proliferation and osteogenic differentiation of bone marrow mesenchymal stem cells was shown through cellular experiments carried out in vitro, and an animal osteoporosis model validated its potential to improve osseointegration. Considering these benefits, mSIS-PMMA bone cement shows promising potential as an injectable biomaterial for orthopedic procedures that require bone augmentation.
Bioactive poly (methyl methacrylate) bone cement for the treatment of osteoporotic vertebral compression fractures

Abstract:Rationale: Poly (methyl methacrylate) (PMMA) bone cement is one of the most commonly used biomaterials for augmenting/stabilizing osteoporosis-induced vertebral compression fractures (OVCFs), such as percutaneous vertebroplasty (PVP) and balloon kyphoplasty (BKP). However, its clinical applications are limited by its poor performance in high compressive modulus and weak bonding to bone. To address these issues, a bioactive composite bone cement was developed for the treatment of osteoporotic vertebral compression fractures, in which mineralized collagen (MC) was incorporated into the PMMA bone cement (MC-PMMA). Methods: The in vitro properties of PMMA and MC-PMMA composite bone cement were determined, including setting time, compressive modulus, adherence, proliferation, and osteogenic differentiation of rat bone mesenchymal stem cells. The in vivo properties of both cements were evaluated in an animal study (36 osteoporotic New Zealand female rabbits divided equally between the two bone cement groups; PVP at L5) and a small-scale and short-term clinical study (12 patients in each of the two bone cement groups; follow-up: 2 years). Results: In terms of value for PMMA bone cement, the handling properties of MC-PMMA bone cement were not significantly different. However, both compressive strength and compressive modulus were found to be significantly lower. In the rabbit model study, at 8 and 12 weeks post-surgery, bone regeneration was more significant in MC-PMMA bone cement (cortical bone thickness, osteoblast area, new bone area, and bone ingrowth %; each significantly higher). In the clinical study, at a follow-up of 2 years, both the Visual Analogue Score and Oswestry Disability Index were significantly reduced when MC-PMMA cement was used. Conclusions: MC-PMMA bone cement demonstrated good adaptive mechanical properties and biocompatibility and may be a promising alternative to commercial PMMA bone cements for the treatment of osteoporotic vertebral fractures in clinical settings. While the present results for MC-PMMA bone cement are encouraging, further study of this cement is needed to explore its viability as an ideal alternative for use in PVP and BKP.

Abstract:Poly(methyl methacrylate) (PMMA) is widely used in orthopedic applications, including bone cement in total joint replacement surgery, bone fillers, and bone substitutes due to its affordability, biocompatibility, and processability. However, the bone regeneration efficiency of PMMA is limited because of its lack of bioactivity, poor osseointegration, and non-degradability. The use of bone cement also has disadvantages such as methyl methacrylate (MMA) release and high exothermic temperature during the polymerization of PMMA, which can cause thermal necrosis. To address these problems, various strategies have been adopted, such as surface modification techniques and the incorporation of various bioactive agents and biopolymers into PMMA. In this review, the physicochemical properties and synthesis methods of PMMA are discussed, with a special focus on the utilization of various PMMA composites in bone tissue engineering. Additionally, the challenges involved in incorporating PMMA into regenerative medicine are discussed with suitable research findings with the intention of providing insightful advice to support its successful clinical applications.

Abstract:Polymethyl methacrylate (PMMA) bone cement is a commonly used bone adhesive and filling material in percutaneous vertebroplasty and percutaneous kyphoplasty surgeries. However, PMMA bone cements have been reported to cause some severe complications, such as secondary fracture of adjacent vertebral bodies, and loosening or even dislodgement of the set PMMA bone cement, due to the over-high elastic modulus and poor osteointegration ability of the PMMA. In this study, mineralized collagen (MC) with biomimetic microstructure and good osteogenic activity was added to commercially available PMMA bone cement products, in order to improve both the mechanical properties and the cytocompatibility. As the compressive strength of the modified bone cements remained well, the compressive elastic modulus could be significantly down-regulated by the MC, so as to reduce the pressure on the adjacent vertebral bodies. Meanwhile, the adhesion and proliferation of pre-osteoblasts on the modified bone cements were improved compared with cells on those unmodified, such result is beneficial for a good osteointegration formation between the bone cement and the host bone tissue in clinical applications. Moreover, the modification of the PMMA bone cements by adding MC did not significantly influence the injectability and processing times of the cement.

Abstract:Artificial bone graft with osteoconductivity, angiogenesis, and immunomodulation is promising clinical therapeutics for the reluctant healing process of bone defects. Among various osteogenic substitutes, polymethyl methacrylate (PMMA) bone cement is a quit competitive platform due to its easy deployment to the bone defects with irregular shape and biomimetic mechanical properties. However, the biologically inert essence of PMMA is reliant on the passive osseointegration and cannot provide sufficient biologic cues to induce fast bone repair. Bioactive glass could serve as an efficient platform for the active osteogenesis of PMMA via ionic therapy and construction of alkaline microenvironment. However, the direct of deployment of bioactive glass into PMMA may trigger additional cytotoxicity and hinder cell growth on its surface. Hence we incorporated ionic therapy as osteogenic cue into the PMMA to enhance the biomedical properties. Specifically, we synthesized core-shell microspheres with a strontium-doped bioactive glass (SrBG) core and hydroxyapatite (HA) shell, and then composited them with PMMA to introduce multifunctional effects of HA incorporation, alkaline microenvironment construction, and functional ion release by adding microsphere. We preparedxSrBG@HA/PMMA cements (x= 30, 40, 50) with varied microsphere content and evaluated impacts on mechanical/handling properties, ion release, and investigated the impacts of different composite cements on proliferation, osteogenic differentiation, angiogenic potential, and macrophage polarization. These findings provide new perspectives and methodologies for developing advanced bone biomaterials to promote tissue regeneration.

Abstract:PMMA-based bone cements are used for anchoring artificial joints. The cements are offered as two-component systems. During mixing, a liquid paste is formed by free-radical polymerization, which completely hardens into a solid cement matrix as polymerization progresses with an increase in viscosity. Polymerization from MMA to PMMA is an exothermic process, energy is released in the form of heat. After fixation of the prosthesis and curing of the cement, the cement fills the space between the prosthesis and the bone. With the filler PMMA, a strong force-locking and interlocking mechanical bond is created. The essential properties of PMMA cements are dictated by the powder component. In vivo, the hard and brittle bone cements absorb body fluids and become more elastic and softer. The properties of various PMMA bone cements differ significantly, although the chemical acrylate base is identical.

Abstract:<p>Bioinert polymethylmethacrylate (PMMA) bone cement is the most conventional approach for quick fixation of the orthopedic implant during hip replacement surgery. However, problems with implant loosening occur frequently and particularly under osteoporosis due to inadequate osteointegration between the cement and host bone. In order to enhance osteointegration ability of the PMMA cement, mineralized collagen (MC) was incorporated into the PMMA (PMMA-MC) in the current study. An improved wettability and dynamic mechanical performance of the PMMA-MC was demonstrated in vitro, compared to pure PMMA. Then, the effect of PMMA and PMMA-MC on osteoporotic bone marrow stromal cells were investigated. The results revealed that the addition of MC could significantly up-regulate osteoblastic gene expressions (Runx2, Osx, and Alp) and down-regulate the expressions of adipogenic markers (C/EBPα, PPARγ-2, and adiponectin). The osteointegration ability of bone cement was evaluated in an osteoporotic rat model with bone defect. After 8 and 12 weeks of implantation, PMMA-MC group had significantly more new bone formation around the implant and a higher push-out force at the bone-cement surface than that of the PMMA group. Moreover, to simulate clinical hip replacement, prosthesis was fixed with either PMMA or PMMA-MC bone cement in osteoporotic cadaveric femurs. Compared to the PMMA group, more torque was required in the PMMA-MC group to achieve the same rotation degree in torsion test. These findings revealed the potential of MC to induce an efficient fixation between the modified cement surface and osteoporotic host bone. PMMA-MC can be a promising candidate biomaterial for clinical hip replacement.</p>

Abstract:Polymethyl-methacrylate (PMMA) bone cement is used extensively in hip and knee arthroplasty. A thorough understanding of the basic chemistry underlying PMMA is important for orthopaedic surgeons because this underscores the specific way bone cement is used during surgery. Recently, clinical research has shed light on the various types of PMMA regarding the viscosity of the mixture and the effect of cement additives. These variations in composition may alter the clinical efficacy of implanted bone cement in hip and knee arthroplasty. Understanding these key differences will allow the surgeon to tailor the PMMA composition as needed to maximize outcomes of hip and knee arthroplasty. This review will summarize the preclinical feature of PMMA, evaluate current and past commercially available bone cement options, analyze preclinical results and clinical outcomes of various bone cement types, and highlight future areas of research.

Abstract:The utilization of polymethyl methacrylate (PMMA) bone cement is employed for the purpose of stabilizing fractured vertebral bodies. The existence of a mechanical imbalance in hard polymethylmethacrylate (PMMA) bone cement has the potential to increase the likelihood of a fracture occurring in the neighbouring vertebral body. In order to reduce potential difficulties, the primary goal of this study is to investigate the potential benefits of increasing PMMA bone cement's bioactivity and lowering its elastic modulus. The incorporation of a 10% volume fraction of hyaluronic acid (HyA) and polyethylene glycol (PEG) into the bone cement led to an improvement in the bioactivity and decreasing of elastic modulus of polymethylmethacrylate (PMMA). The integration of HyPE gel phase presents several advantages over pure PMMA bone cement, including enhanced setting parameters, improved degradability, and increased biocompatibility. The gel phase is additionally accountable for a reduction in the elastic modulus of polymethylmethacrylate (PMMA) bone cement. In addition, the existence of a porous structure that arises from the degradation of the HyPE gel phase delivers a significant amount of room, thereby enhancing the process of bone regeneration when implanted in the femur of rabbits. The utilization of HyPE in PMMA has been shown through comprehensive μ-CT analysis to enhance bone formation, thereby promoting osteointegration at the implantation site. Furthermore, the histological analysis demonstrated the existence of osteogenic activity in the PMMA polyethylene glycol supplemented with 10% HyA and 10% PEG after a 2-month period subsequent to implantation.

Abstract:Polymethyl methacrylate (PMMA) bone cement is one of the most successful biomaterials in orthopaedic fixation, however, the poor osteogenic/osteointegration and high exothermic temperature limited their wide applications especially the complex clinical bone diseases. In this study, a multifunctional bioactive bone cement reinforced with the poly(citrate-silicon) (PCS) nanospheres was developed for overcoming the limitation of PMMA bone cement (PMMA-PCS). Compared with traditional PMMA, PMMA-PCS cement showed the increased injectability (about 5.56 times), extended setting time (about 6.4 min), increased hydrophilicity (46.9°) and reduced polymerization heat (about 41.9℃), without affecting the stability and plasticity. The mechanical strength was also more matched to the bone, with a compression modulus of about 61.8% of PMMA, which was beneficial for reducing stress concentration at the bone implantation site. PMMA-PCS bone cement also had good degradability, with a degradation rate of about 34.79% in 6 weeks. PMMA-PCS bone cement possessed the antioxidant, in vitro fluorescence imaging, in vitro antibacterial, and in vitro biomineralization properties. The cellular experiments showed that PMMA-PCS bone cement showed the improved osteogenic differentiation, mineral deposition, good anti-inflammatory and pro-angiogenic properties. In a rat femoral condyle defect model, the results demonstrated that PMMA-PCS bone cement could significantly promote the new bone formation and bone ingrowth both at the cement-bone contact surface and inside the cement, through improving the osteogenic proteins and angiogenesis.

Abstract:The objective of the current study was to design a bioactive and mechanically stable ceramic/polymer cement for repairing bone defects. Solvent casting method was utilized to prepare porous diopside/PMMA scaffolds. The characteristic functional groups associated with diopside and PMMA in the composites were confirmed by FT-IR spectroscopy. Scanning electron microscopy revealed that the surface of composites covered by small non-uniform pores having dimensions below 4 μm. It was found that the porosity of the composites can be altered by varying the content of filler (diopside) in the composites. The biomineralization ability and mechanical strength of PMMA were enhanced by fabricating it as a composite with diopside. The presence of apatite peaks on the surface of diopside/PMMA composites was observed after 4 weeks of immersion in SBF solution. The mechanical strength of diopside/PMMA composites was found to match with cancellous bone. Hence, the compositional ratio and constituents played a key role in determining the activity of the composites. Bioactive silicates can be used as a filler in biomedical polymers for imparting ductility to ceramics for achieving advanced functionalities.

Abstract:ObjectiveThis research investigated the mechanical properties and bioactivity of polymethylmethacrylate (PMMA) bone cement after addition of the nano-hydroxyapatite(HA) coated bone collagen (mineralized collagen, MC).Materials & MethodsThe MC in different proportions were added to the PMMA bone cement to detect the compressive strength, compression modulus, coagulation properties and biosafety. The MC-PMMA was embedded into rabbits and co-cultured with MG 63 cells to exam bone tissue compatibility and gene expression of osteogenesis.Results15.0%(wt) impregnated MC-PMMA significantly lowered compressive modulus while little affected compressive strength and solidification. MC-PMMA bone cement was biologically safe and indicated excellent bone tissue compatibility. The bone-cement interface crosslinking was significantly higher in MC-PMMA than control after 6 months implantation in the femur of rabbits. The genes of osteogenesis exhibited significantly higher expression level in MC-PMMA.ConclusionsMC-PMMA presented perfect mechanical properties, good biosafety and excellent biocompatibility with bone tissues, which has profoundly clinical values.

Abstract:Commercial poly methyl methacrylate (PMMA)-based cement is currently used in the field of orthopedics. However, it suffers from lack of bioactivity, mechanical weakness, and monomer toxicity. In this study, a PMMA-based cement nanocomposite reinforced with hydroxyapatite (HA) nanofibers and two-dimensional (2D) magnesium phosphate MgP nanosheets was synthesized and optimized in terms of mechanical property and cytocompatibility. The HA nanofibers and the MgP nanosheets were synthesized using a hydrothermal homogeneous precipitation method and tuning the crystallization of the sodium-magnesium-phosphate ternary system, respectively. Compressive strength and MTT assay tests were conducted to evaluate the mechanical property and the cytocompatibility of the PMMA-HA-MgP nanocomposites prepared at different ratios of HA and MgP. To optimize the developed nanocomposites, the standard response surface methodology (RSM) design known as the central composite design (CCD) was employed. Two regression models generated by CCD were analyzed and compared with the experimental results, and good agreement was observed. Statistical analysis revealed the significance of both factors, namely, the HA nanofibers and the MgP nanosheets, in improving the compressive strength and cell viability of the PMMA-MgP-HA nanocomposite. Finally, it was demonstrated that the HA nanofibers of 7.5% wt and the MgP nanosheets of 6.12% wt result in the PMMA-HA-MgP nanocomposite with the optimum compressive strength and cell viability.

Abstract:As the population ages, so does the demand for bone loss treatments. The main components of these medicines must be able to endure longer and perform more effectively. Bone cement made of poly (methyl methacrylate) (PMMA), which is often used in damaged bone replacement surgery, is a vital biological material. As a result, the impact of additional nanoparticles such as zirconium dioxide (Zr02) and magnesium oxide (MgO) on polymer binary blends (Acrylic bone cement: 15% PMMA) for a bone scaffold was studied in this research. ZrO2 and MgO nanoparticles were introduced in various weights present to the polymer mix matrix 0, 0.5, 1, 1.5, 2). Hand lay-up molding using two different types of PMMA material was utilized to create the polymer. The reinforcement materials were mixed individually with a binary polymer blend material according to the reinforcement material selection ratio, and then heat-treated at 55°C for 3 hours to complete polymerization and remove any residual stress. Mechanical characteristics such as tensile strength and Young's modulus were evaluated for all of the prepared samples. The chemical bonding of nanoparticles and synthetic binary polymeric mix composites was evaluated using Fourier transform infrared spectroscopy (FTIR). The tensile strength and Young's modulus of a binary polymeric blend reinforced with (1.5wt% ZrO2, and 1wt% MgO) both dramatically increased. A scanning electron microscope (SEM) was used to examine the surface morphology of the fracture surface of tensile specimens. SEM images demonstrated that nanoparticles (ZrO2 and MgO) were distributed uniformly throughout the polymeric mix matrix.

Abstract:At a time of unpredictable challenges for health, one trend is certain: there is an exceedingly high demand for functional implants, particularly bone grafts. This has encouraged the emergence of bone tissue engineering substitutes as an alternative method to conventional bone grafts. However, the current approaches in the field face several limitations that have prevented the ultimate translation into clinical settings. As a result, many attempts have been made to fabricate synthetic bone implants that can offer suitable biological and mechanical properties.Light curable methacrylate-based polymers have ideal properties for bone repair. These materials are also suitable for 3D printing which can be applicable for restoration of both function and aesthetics. The main objective of this research was to investigate the role of calcium phosphate (CaP) incorporation in a mechanically stable, biologically functional and 3D printable polymer for the reconstruction of complex craniofacial defects. The flow behaviour of the polymer as a function of CaP addition, surface remineralisation potential, in vitro cell culture, using MC3T3 and Adipose-Derived Mesenchymal Stem Cells (ADSCs) and ex ovo angiogenic response was assessed. Finally, in vivo studies were carried out to investigate neo-bone formation at 4- and 8-weeks post-implantation. Quantitative micro-CT and histological evaluation did not show a higher rate of bone formation in CaP filled CSMA composites compared to CSMA itself. Therefore, such polymeric systems hold promising features by allowing more flexibility in designing a 3D printed scaffold targeted at the reconstruction of maxillofacial defects.
Improved Bone Ingrowth of Tricalcium Phosphate Filled Poly(methyl Methacrylate) (PMMA) Bone Cements in Vivo

Abstract:The cement augmentation technique using poly(methyl methacrylate) (PMMA) is considered to be the most effective method for fixation of osteoporotic fracture because of its outstanding mechanical properties. However, the low bio-degradation of PMMA, which hinders bone ingrowth, limits it potential for a broad range of applications. Previous studies have shown blending β-tricalcium phosphate (β-TCP) with PMMA was able to improve the osteoconduction while the material still suffered poor mechanical properties due to the poor compatibility between two components. In this study, we explored the use of hydroxyethyl methacrylate (HEMA) as a compatibilizer for improving the affinity of TCP/PMMA blends system. The resultant TCP/HEMA/PMMA(THP) composite cements exhibited similar setting properties to PMMA cement but much better mechanical properties than composite systems without HEMA as a compatibilizer. No cytotoxicity or hemolysis was observed in the THP bone cement system by biochemical assays. Bone ingrowth was significantly improved at 12 weeks. The reinforced pull-out strength of screws was observed at 4, 8, 12 weeks through in vivo studies performed on rabbit femurs. The THP system, as an injectable bone cement material, shows great potential for use in the cement augmentation technique.
The "magnesium Sacrifice" Strategy Enables PMMA Bone Cement Partial Biodegradability and Osseointegration Potential

Abstract:Poly (methyl methacrylate) (PMMA)-based bone cements are the most commonly used injectable orthopedic materials due to their excellent injectability and mechanical properties. However, their poor biocompatibility and excessive stiffness may cause complications such as aseptic implant loosening and stress shielding. In this study, we aimed to develop a new type of partially biodegradable composite bone cement by incorporating magnesium (Mg) microspheres, known as “Mg sacrifices” (MgSs), in the PMMA matrix. Being sensitive to the physiological environment, the MgSs in PMMA could gradually degrade to produce bioactive Mg ions and, meanwhile, result in an interconnected macroporous structure within the cement matrix. The mechanical properties, solidification, and biocompatibility, both in vitro and in vivo, of PMMA–Mg bone cement were characterized. Interestingly, the incorporation of Mg microspheres did not markedly affect the mechanical strength of bone cement. However, the maximum temperature upon setting of bone cement decreased. This partially biodegradable composite bone cement showed good biocompatibility in vitro. In the in vivo study, considerable bony ingrowth occurred in the pores upon MgS degradation. Together, the findings from this study indicate that such partially biodegradable PMMA–Mg composite may be ideal bone cement for minimally invasive orthopedic surgeries such as vertebroplasty and kyphoplasty.
Gelatin/gentamicin sulfate-modified PMMA bone cement with proper mechanical properties and high antibacterial ability

Abstract:Abstract With the aging of the population, the risk of osteoporotic vertebral compression fractures (OVCF) caused by osteoporosis increases rapidly. Surgeons often fill the bone defect with injectable polymethylmethacrylate (PMMA) bone cement through vertebroplasty. However, compared with cancellous bone, the higher mechanical properties of PMMA bone cement can easily lead to the fracture of the adjacent cone. Besides, the wound infection caused by surgery is also a serious problem. In order to solve these problems, we designed a new type of PMMA bone cement, by adding gelatin as a pore former, 5% (w/w) gentamicin sulfate (GS) for antibacterial purpose, and 30% (w/w) barium sulfate (BaSO 4 ) to provide excellent radiopacity. Compared with the traditional PMMA bone cement, with the dissolution of gelatin after being immersed in phosphate buffered saline (PBS) for 14 d, the mechanical properties of modified PMMA bone cement decreased by approximately 67%, which is close to the human cancellous bone. Besides, the release of GS increased 3.8 times, and the GS concentration remained above the minimum inhibitory concentration (MIC) for 12 d. In addition, the setting properties, contact angle, antibacterial ability, and cell compatibility of PMMA bone cement also maintained well. The integration and dissolution of gelatin were observed by a scanning electron microscope (SEM). All results indicate that the new type of gelatin-modified PMMA bone cement is a potential candidate material for vertebroplasty.

Mechanical properties and antibiotic release characteristics of poly(methyl methacrylate)-based bone cement formulated with mesoporous silica nanoparticles

Abstract:The influence of mesoporous silica nanoparticles (MSNs) loaded with antibiotics on the mechanical properties of functional poly(methyl methacrylate)-(PMMA) based bone cements is investigated. The incorporation of MSNs to the bone cements (8.15wt%) shows no detrimental effects on the biomechanical properties of the freshly solidified bone cements. Importantly, there are no significant changes in the compression strength and bending modulus up to 6 months of aging in PBS buffer solution. The preserved mechanical properties of MSN-functionalized bone cements is attributed to the unchanged microstructures of the cements, as more than 96% of MSNs remains in the bone cement matrix to support the cement structures after 6 months of aging. In addition, the MSN-functionalized bone cements are able to increase the drug release of gentamicin (GTMC) significantly as compared with commercially available antibiotic-loaded bone cements. It can be attributed to the loaded nano-sized MSNs with uniform pore channels which build up an effective nano-network path enable the diffusion and extended release of GTMC. The combination of excellent mechanical properties and sustainable drug delivery efficiency demonstrates the potential applicability of MSN-functionalized PMMA bone cements for orthopedic surgery to prevent post-surgery infection.

Abstract:The incidence of osteoporotic vertebral compression fractures (OVCFs) increases gradually with age, resulting in different degrees of pain for patients, even possible neurological damage and deformity, which can seriously affect their quality of life. Vertebral augmentation plays an important role in the surgical treatment of OVCFs. As the most widely used bone cement material, polymethyl methacrylate (PMMA) offers inherent advantages, such as injectability, ease of handling, and cost-effectiveness. However, with its application in the clinic, some disadvantages have been found, including cytotoxicity, high polymerization temperature, high elastic modulus, and high compressive strength. To improve the mechanical properties and the biological performance of conventional PMMA bone cement, several studies have modified it by adding bioceramics, bioglass, polymer materials, nanomaterials, and other materials, which have exhibited some advantages. In addition, other alternative materials, such as calcium phosphate, calcium sulfate, and calcium silicate cements—including their modifications—have also been explored. In this review, we examined the existing research on the side-effects of conventional PMMA bone cement, modified PMMA bone cement, and other alternative materials designed to improve properties in OVCFs. An overview of various modified bone cements can help further scientific research and clinical applications.
