Effect of Domestic Povidone Iodine on Conjunctival Sac Disinfection

Abstract:OBJECTIVE To analyze the effect on disinfecting conjunctival sac by 1% domestic povidone iodine.METHODS A total of 200 patients(200 eyes) receiving intraocular surgery were randomly and equally divided into 2 groups(group A and group B).1% Domestic povidone iodine solution was used to irrigate the conjunctival sac before intraocular surgery in group A while 0.8% gentamicin solution was used in group B.Conjunctival cultures were obtained before and after the conjunctival sac was irrigated.RESULTS Before the conjunctival sac was irrigated,the positive rates of conjunctival cultures were 33% in group A and 32% in group B vs 2% in group A and 11% in group B after irrigation.CONCLUSION Irrigating the conjunctival sac with 1% domestic povidone iodine solution before operation is more effective in reducing the rate of contamination than with 0.8% gentamicin solution.Irrigating the conjunctival sac with 1% domestic povidone iodine solution is a good measure to prevent postoperative endophthalmitis.
Conjunctival Sac Bacterial Culture of Patients Using Levofloxacin Eye Drops Before Cataract Surgery: a Real-World, Retrospective Study

Abstract:Background The use of antibiotics preoperatively is effective to decrease the incidence of ocular bacterial infections but may lead to high resistance rate, especially on patients with multi-risk clinical factors. This study systematically analyzed real-world data (RWD) of patients to reveal the association between clinical factors and conjunctival sac bacterial load and offer prophylaxis suggestions. Methods We retrieved RWD of patients using levofloxacin eye drops (5 mL: 24.4 mg, 4 times a day for 3 days) preoperatively. Retrieved data included information on the conjunctival sac bacterial culture, sex, presence of hypertension and diabetes mellitus (DM), and history of hospital-based surgeries. Data was analyzed using SPSS 24.0. Results RWD of 15,415 cases (patients) were retrieved. Among these patients, 5,866 (38.1%) were males and 9,549 (61.9%) females. 5,960 (38.7%) patients had a history of hypertension, and 3,493 (22.7%) patients had a history of DM. 7,555 (49.0%) patients had a history of hospital-based operations. There were 274 (1.8%) positive bacterial cultures. Male patients with hypertension and DM may be at increased risk of having positive bacterial cultures (P < 0.05). Staphylococcus epidermidis (n = 56, 20.4%), Kocuria rosea (n = 37, 13.5%), and Micrococcus luteus (n = 32, 11.7%) were the top 3 isolated strains. Most bacterial strains were resistant to various antibiotics except rifampin, and 82.5% (33 of 40 isolates) of Staphylococcus epidermidis isolates had multidrug antibiotic resistance. Numbers of culture-positive Staphylococcus epidermidis isolates in the male group and non-DM group were greater than those in the female and DM groups, respectively. Micrococcus luteus (n = 11, 8.8%) was found less frequently in non-hypertension group than in hypertension group. Conclusion Sex (Male) and the presence of hypertension and DM are risk factors for greater conjunctival sac bacterial loads. We offer a prophylactic suggestion based on the combined use of levofloxacin and rifampin. However, this approach may aggravate risk of multidrug resistance.
Inhibitory Effect of Short-Term Palpebral Margin Cleaning with Antibiotic Eye Drops on Ocular Surface Flora Before Cataract Extraction: A New Preoperative Antibacterial Method

Abstract:Objective To explore the clinical significance of the combined application of palpebral margin cleaning and antibiotic eye drops in inhibiting bacterial growth in the palpebral margin and conjunctival sacs before cataract extraction. Methods In this study, 61 patients (97 eyes) with age-related cataract who underwent phacoemulsification and intraocular lens implantation were selected, and randomly grouped. In the experimental group, the combined application of palpebral margin cleaning with cotton pads and levofloxacin eye drops was given for three days before the surgery. In the control group, levofloxacin eye drops alone were applied for three consecutive days. Bacteria samples from the conjunctival sac and eyelid margins were cultivated and identified before and three days after taking antimicrobial measures, respectively. Results In the experimental group, the positive rates of the two bacteria samples were 100％ (50/50) and 40％ (20/50) before and 10％ (5/50) and 0％ (0/50) after the treatment. In the control group, the positive rates of the two bacteria samples were 97.9％ (46/47) and 29.8％ (14/47) before and 40.4％ (19/47) and 10.6％ (5/47) after the treatment. The positive rates between the two groups were not significantly different before taking antimicrobial measures (P = 0.485 and 0.395), while they were significantly different after taking antimicrobial measures (P = 0.001 and 0.024). Conclusion Combined application of eyelid and palpebral margin cleaning with cotton pads and antibiotic eye drops before cataract extraction imparted excellent antibacterial effects.
Combination Effect of Cefuroxime and Levofloxacin Against Bacteria Isolated from the Healthy Conjunctival Sac and Endophthalmitis Cases Using a Fractional Inhibitory Concentration Index

Abstract:Purpose: Intracameral cefuroxime (CXM) and antibiotic eye drops such as fluoroquinolone are widely used for preventing endophthalmitis after cataract surgery. However, few reports are available regarding their interactions. This study was conducted to examine the in vitro interaction of CXM and levofloxacin (LVFX) against bacterial isolates from the healthy conjunctival sac before cataract surgery and from endophthalmitis cases. Methods: The activity of each drug was determined using a broth dilution method. Checkerboard synergy testing was performed for 43 isolates from the healthy conjunctival sac before cataract surgery and 15 isolates from endophthalmitis cases, respectively. Antimicrobial combinations were classified as synergistic, additive, indifferent, or antagonistic based on their fractional inhibitory concentration (FIC). Results: Minimum inhibitory concentrations of CXM and LVFX against endophthalmitis isolates were compared against conjunctival sac isolates. The average FIC indices of combined CXM and LVFX used in endophthalmitis isolates were significantly lower than those in conjunctival sac isolates (P < 0.001). The synergistic activities of CXM and LVFX combinations were observed in 50% of endophthalmitis isolates and no conjunctival sac isolates. No consistent additive or synergistic effect was observed in Enterococcus faecalis isolates from endophthalmitis cases. Conclusions: CXM and LVFX resistance was frequently identified in the endophthalmitis isolates. However, CXM and LVFX combination showed a synergistic effect against endophthalmitis isolates and increased the antibiotic activity of each drug, suggesting that the combination may improve effects of both antibiotics and combat drug-resistant bacteria that cause endophthalmitis.
Optimal Duration for the Use of 0.5% Levofloxacin Eye Drops Before Vitreoretinal Surgery

Abstract:Purpose: To investigate various regimens of prophylactic antibiotic therapy for vitreoretinal surgery.Design: This study compared different prophylactic therapies with 0.5% levofloxacin eye drops.Methods: Two hundred nine patients from 7 hospitals scheduled to undergo vitreoretinal surgery were randomized into 3 groups to receive 0.5% levofloxacin eye drops for 1 day (6 times/d), 2 days (3 times/d), or 3 days (3 times/d) before surgery (groups 1D, 2D, and 3D, respectively). All patients received 3 applications of levofloxacin eye drops at 15-minute intervals beginning 1 hour before surgery and conjunctival sac disinfection with 5.0% povidone-iodine 15 minutes before surgery. Conjunctival swabs were cultured before levofloxacin therapy (T0), on the morning of surgery (T1), after povidone-iodine disinfection (T2), immediately after surgery (T3), 1 day postoperatively (T4), and 1 week postoperatively (T5).Results: The positive bacterial culture rates in groups 1D, 2D, and 3D fell, respectively, from 37.33%, 30.77%, and 31.88% at T0 to 10.67%, 12.31%, and 11.59% at T1 and 1.33%, 0%, and 0% at T2. At each time point (T0-T5), there were no significant differences among the groups in positive bacterial culture rate. The bacterial eradication rates in groups 1D, 2D, and 3D were, respectively, 100%, 94.74%, and 90.00% at T1 (after levofloxacin) and 100% in all groups at T2 (after povidone-iodine).Conclusions: The efficacy of levofloxacin in preventing postoperative infection was similar in the 3 treatment groups. It is recommended that 0.5% levofloxacin be used for only 1 day before vitreoretinal surgery (6 times/d) to minimize the use of prophylactic antibiotics.
[Comparative Research of the Efficacy of the Gatifloxacin and Levofloxacin for Bacterial Conjunctivitis in Human Eyes].

Abstract:OBJECTIVE:To evaluate the different efficacy and safety to the treatment of bacterial conjunctivitis between the fluoroquinolone antibiotics gatifloxacin and levofloxacin.METHODS:A multi-centre, random, double-blind and control trial was performed in six centers including Eye Ear Nose and Throat Hospital of Fudan University, Henan Eye Institute, Eye Hospital Affiliated to Wenzhou Medical College, the First Affiliated Hospital Of Nanjing Medical University, Xijing Hospital Affiliated to the Fourth Military Medical University, the Second Affiliated Hospital of Xi'an Jiaotong University between August 2006 and October 2007. The levofloxacin was set as the efficient control. Two hundred and thirty-five patients (235 eyes) that diagnosed as bacterial conjunctivitis were randomly divided into two groups by the method of randomized blocks, the test group (gatifloxacin) had 118 eyes and the control (levofloxacin) group had 117 eyes. The drug delivery into conjunctival sac was performed at a 7-day period (two drops per time, eight times per day at the first two days and two drops per time, four times per day at the following 5 days). All participants were given the conjunctival sac germ culture and drug sensitive test before and after the study. The combination score of signs and symptoms and evaluation of safety were conducted at the pre-delivery day, the (4 ± 1) and (7 ± 1) delivery day. The statistic analysis was conducted by CMH χ(2) test, Pearson χ(2) test and Fisher's exact probabilities test.RESULTS:The efficacy of the two groups was 94.0% (110/117 eyes) in gatifloxacin group and 93.8% (106/113 eyes) in levofloxacin group with no significant difference (χ(2) = 0.052, P = 0.8201). There was also no difference in the bacteria clearance between the two groups [gatifloxacin versus levofloxacin, 94.1% (80/85 eyes) versus 92.5% (74/80 eyes), P = 0.3470]. The decrease of combination score of signs and symptoms at the (4 ± 1) delivery day was 4.436 ± 2.310 in the gatifloxacin group and 3.814 ± 1.962 in the levofloxacin group, the difference of which was significant (F = 7.280, P = 0.0075). This trend was also proved at the (7 ± 1) delivery day (gatifloxacin versus levofloxacin, 7.487 ± 2.821 versus 6.912 ± 2.911, F = 4.060, P = 0.0452). The visual acuity and the tolerance after local application of the eye drops between the two groups had no difference (the visual acuity F = 1.04, P = 0.3080; the tolerance after local admission χ(2) = 0.1372, P = 0.7111). According to the result of the germ culture, the major pathogenic bacteria were Gram-positive bacteria (totally 20 kinds of Gram-positive bacteria and 8 kinds of Gram-negative bacteria). The MIC and drug resistance of gatifloxacin to the Gram-positive bacteria was lower than that of the levofloxacin (Staphylococcus Epidermidis, Staphylococcus Aureus, coagulase negative Staphylococcus, α-hemolytic Streptococcus).CONCLUSIONS:The gatifloxacin eye drop has a good therapeutic effect to the bacterial conjunctivitis. It can effectively clear the pathogen with fast and strong effect. Moreover, it has a low MIC in vitro, advance a prospect in drug resistance, safety and ocular tolerance.
[Human Aqueous Humor Levels of Levofloxacin 0.5%, Gatifloxacin 0.3% and Levofloxacin 0.3% Ophthalmic Solution after Topical Dosing].

Abstract:OBJECTIVE:To compare the aqueous humor concentration of levofloxacin 0.5%, gatifloxacin 0.3% and levofloxacin 0.3% ophthalmic solution after topical dosing in human eyes.METHODS:A randomized, double-blind and parallel study was conducted. Senile cataract patients (150 eyes in 150 cases) receiving phacoemulsification in eye center affiliated second hospital of Zhejiang university were randomized into three groups: levofloxacin 0.5%, 50 eyes;gatifloxacin 0.3%, 50 eyes and levofloxacin 0.3%, 50 eyes. Each group was randomized into five sub-groups, with 10 eyes in each sub-group. Each group received either levofloxacin 0.5% or topical gatifloxacin 0.3% or levofloxacin 0.3% preoperatively. One drop of antibiotic was administered every 15 minutes for 4 doses. More than 100 microl aqueous samples were obtained from different sub-groups during surgery, 15.0, 30.0, 60.0, 120.0 and 180.0 min after the last drop, respectively. Then 100 microl aqueous samples were put into test tube by sample injector accurately. Concentration of the antimicrobial agents was determined by the HPLC (high pressure liquid chromatography). Variance analysis was performed to detect differences among the antibiotic treatment groups. The differences between two groups were assumed by t-test.RESULTS:Humor concentration of levofloxacin 0.5% (1.61 +/- 0.48), (2.41 +/- 0.80), (2.93 +/- 0.50), (2.56 +/- 0.63), (1.87 +/- 0.88) mg/L was significantly higher than that of gatifloxacin 0.3% (0.70 +/- 0.18), (1.29 +/- 0.54), (1.59 +/- 0.67), (1.41 +/- 0.50), (1.13 +/- 0.28) mg/L and levofloxacin 0.3% (0.55 +/- 0.39), (1.15 +/- 0.42), (1.38 +/- 0.49), (1.02 +/- 0.33), (0.55 +/- 0.31) mg/L at the same period after the last drop (F = 23.64, 12.82, 21.13, 25.00, 12.22;P < 0.05).CONCLUSION:Topically applied levofloxacin 0.5% had the higher aqueous humor drug level than gatifloxacin 0.3% and levofloxacin 0.3%.
Culture positivity and distribution of the conjunctival sac bacteria in the perioperative period of corneal refractive surgery

Abstract:The objective of this study was to investigate the culture positivity and distribution of the conjunctival sac bacteria in the perioperative period of corneal refractive surgery. The selected time points of the perioperative period included before the use of antibiotic eye drops, before eye wash (after the use of antibiotic eye drops), after eye wash, and immediately after surgery. Conjunctival specimens obtained at the four time points were cultured to detect the positivity and distribution of bacteria. Before prophylactic antibiotic eye drops were administered, 49 eyes (50%) had positive bacterial culture results, with 45 isolates (91.8%) identified as Staphylococcus epidermidis. The culture positivity rates of the conjunctival sac specimens before eye wash, after eye wash, and immediately after surgery were 19.4%, 3.1%, and 4.1%, respectively. The difference was significant before and after the use of antibiotics and before and after eye wash (both P < 0.001). Staphylococcus epidermidis was the major pathogen in the conjunctival sac before corneal refractive surgery, and the culture positivity rate of the conjunctival bacteria was higher in males. Sixteen of 37 eyes (43.2%) with contact lenses had positive culture results, compared to 33 of 61 eyes (54.1%) without contact lenses (P > 0.05). The judicious preoperative use of antibiotic eye drops combined with the surgical sterile eye wash procedure maximised the removal of conjunctival sac bacteria. Skilled surgical manipulations generally did not increase the risk of infection.
[Clinical Study on the Reduction of Ocular Surface Bacteria in Pre-Operative Cataract Surgery Patients by Cleansing the Eyelid Margins].

Abstract:Objective: To explore the clinical significance of ocular surface bacteria inhibition by cleaning eyelid margins before surgery in cataract patients. Methods: Case-control study. Thirty-five patients (45 eyes) with age-related cataracts who were proposed to receive treatment of Phacoemulsification combined with cataract extraction and intraocular lens implantation at Peking University 3(rd) hospital were included. The experimental group consisted of 9 males and 15 females with the average age of (69.8±11.5). The control group consisted of 6 males and 15 females with the average age of (61.8±16.2). In the experimental group, bacteria samples from the conjunctival sac and eyelid margins were collected at three times separately and were cultured for identification: first sampling was taken prior to application of any type of antimicrobial measures; second sampling was taken after application of lid scrub twice daily for 7 days to cleanse their eyelid margins by the patients;the last sampling was taken after patients' application of Levofloxacin antibiotic eye drops 3 times per day continuously for 3 days prior to surgery. In the control group, bacteria samples from the conjunctival sac and eyelid margins were collected at two times separately and were cultured for identification: first sampling was taken before application of any antimicrobial measures;second sampling was taken after patients' application of only Levofloxacin antibiotic eye drops successively for 3 days before surgery. Then the results of bacteria culture of the two groups above were compared. Results: In the experimental group, according to the eyelid margins bacteria cultures results of the first sampling (taken before application of any antibacterial measures), the total positive rate, positive rate of common bacteria and positive rate of rare bacteria found in the cultures were 83.3% (20/24), 70.8% (17/24), 45.8% (11/24), respectively; according to the bacteria cultures results of the second sampling (tanken after application of lid scrub for cleaning the eyelid margins for 7 days), the total positive rate, positive rate of common bacteria and positive rate of rare bacteria found in the cultures were 58.3% (14/24), 45.8% (11/24), 25% (6/24), respectively;According to the bacteria cultures results of the third sampling (taken after application of antibiotic eye drops for three days), the total positive rate, positive rate of common bacteria and positive rate of rare bacteria found in the cultures were 20.8% (5/24), 20.8% (5/24), 4.2% (1/24), respectively. In the experimental group, we compared the bacterial positive rate of the samples taken after patients' application of antibiotics for 3 days to the bacterial positive rate of the samples taken prior to application of any antibacterial measures to have found that the total positive rate, the positive rates of common bacteria and rare bacteria decreased significantly (P<0.05) and the difference in between are of statistical significance. In the control group, according to the eyelid margins bacteria cultures results of the first sampling (taken before application of any antibacterial measures, the total positive rate, positive rate of common bacteria and positive rate of rare bacteria found in the cultures were 81% (17/21), 66.7% (14/21), 19% (4/21), respectively;According to the bacteria cultures results of the second sampling (taken after the application of antibiotic eye drops for three days), the total positive rate, positive rate of common bacteria and positive rate of rare bacteria found in the cultures were 38.1% (8/21), 14.3% (3/21), 33.3% (7/21) respectively. When comparing the experimental group to the control group, the experimental group yielded significantly better results than the control group for the rate of reduction of rare bacteria in the eyelid margin after using antibiotics for 3 days. This is due to the lid scrub being used with the antibiotics. The difference are of statistical significance (χ(2)=6.518, P=0.017). There was no statistically significant difference between the experimental and control group in terms of the total positive rate as well as the positive rate of common bacteria and rare bacteria in the conjunctival sac. Conclusions: The cleaning of the eyelid and eyelid margin with lid scrub prior to cataract surgery combined with antibiotics eye drops will achieve excellent antibacterial effects for common and rare bacteria in the eyelid and conjunctival sac. The antibacterial effect of using lid scrub with antibiotic eye drops is more favorable than that of using antibiotic eye drops only. However, the lid scrub eyelid hygiene products cannot replace preoperative antibiotics independently. (Chin J Ophthalmology, 2018, 54: 445-451).
Rinsing the Conjunctival Sac with Povidone-Iodine Solution Before Cataract Surgery: A Meta-Analysis

Abstract:Objective Rinsing the conjunctival sac before cataract surgery is a primary prevention of endophthalmitis.How to properly evaluate and select the appropriate solution is a question worth exploring.The purpose of this study is to compare the effects of ocular rinse with povidone-iodine solution,sodium chloride solution and antibiotic solution for cataract surgery by meta-analysis.Methods We conducted both computer-aided and hand searches of the Medline/PubMed,Ovid,Cochrane Library and EMBASE for the relevant control studies assessing the effects of ocular rinse with povidone-iodine solution,sodium chloride solution and antibiotic solution for cataract surgery.Six experiments in 4 studies were included and analyzed in two groups.Results Povidone-iodine solution was found to be significantly more effective than sodium chloride solution in reducing the positive rate of bacteria in the conjunctival sac(P0.0001,pooled RR: 0.15 and 95% CI: 0.05,0.45),as well as than antibiotic solution(P0.05,pooled RR: 0.38 and 95% CI: 0.17,0.83).Conclusion Rinsing the conjunctival sac with povidone-iodine solution before cataract surgery can effectively reducing postoperative infection.
Conjunctival Microbiota in Patients With Type 2 Diabetes Mellitus and Influences of Perioperative Use of Topical Levofloxacin in Ocular Surgery

Abstract:Background: Patients with type 2 diabetes mellitus (T2DM) are prone to ocular surface infections. We therefore characterized the conjunctival microbiome of T2DM patients and the influence of topical levofloxacin to investigate whether a dysbiosis is associated with this phenomenon. Methods: Conjunctival microbiome of 79 T2DM patients and 113 non-diabetic controls was profiled using the 16S rDNA sequencing approach. Furthermore, 21 T2DM and 14 non-diabetic patients who underwent cataract surgeries were followed up perioperatively and the influence of pre- and post-operative levofloxacin on the conjunctival microbiome was further investigated prospectively and compared longitudinally. Results: The α-diversity of the conjunctival microbiota was significantly higher in T2DM patients than in controls ( P < 0.05). Significant differences in both composition and function of the conjunctival microbiome were identified on the ocular surface of T2DM patients as compared to non-diabetic controls. Particularly, phylum Bacteroidetes and Fusobacteria , genus Pseudomonas, Haemophilus , and Empedobacter were enriched, while genus Streptococcus was reduced on the T2DM ocular surface. Microbial genes functioning of bacterial chemotaxis was elevated in the conjunctival microbiome of T2DM patients. Furthermore, compared to the initial status, several genera including Staphylococcus were more abundant in the conjunctival microbiome of T2DM patients after 3-days use of preoperative levofloxacin topically, while no genus was more abundant in the non-diabetic follow-up group. No difference was observed between initial status and 7 days after ceasing all postoperative medications in both diabetic and non-diabetic follow-up groups. Conclusions: The conjunctival microbiome of T2DM patients was more complex and may respond differently to topical antibiotics.

Abstract:OBJECTIVE: To retrospectively identify epidemiological trends of infection on the ocular surface and investigate trends of resistance to bacterial antibiotics compared with 10-years previous for Staphylococcus aureus, coagulase-negative staphylococci (CNS), and Corynebacterium in Japan.MATERIALS AND METHODS: Bacterial isolate samples were collected from the conjunctival sacs of eyes afflicted with conjunctivitis, keratitis, dacryocystitis, and hordeolum from September 2004 through November 2005 (n = 145 isolates) and September 2014 through November 2015 (n = 195 isolates) at the Baptist Eye Institute, Kyoto, Japan. The prevalence of methicillin-resistant S. aureus (MRSA), methicillin-resistant CNS (MR-CNS), and fluoroquinolone-resistant Corynebacterium were examined, and susceptibility of isolated bacteria to levofloxacin (LVFX), cefmenoxime (CMX), chloramphenicol (CP), erythromycin (EM), vancomycin (VCM), and arbekacin (ABK) were compared between both time periods using the disc susceptibility method.RESULTS: Over the 10-year period from initial to final examination, the prevalence of MRSA and MR-CNS significantly decreased from 52% to 22% (P < 0.05) and from 47% to 25% (P < 0.05), respectively, yet there was no change in the prevalence of fluoroquinolone-resistant Corynebacterium (60% and 54%; P = 0.38). Antibiotic-resistance trend analysis revealed that susceptibility to antibiotics in 2014-2015 was similar to that in 2004-2005. MRSA and MR-CNS were susceptible to CP (88%), VCM (100%), and ABK (100%), while fluoroquinolone-resistant Corynebacterium was susceptible to CMX (100%), VCM (100%), and ABK (96%).CONCLUSION: The prevalence of MRSA and MR-CNS significantly decreased between the two time periods, yet more than 50% of the Corynebacterium isolates were still resistant to LVFX. Although no increase in bacterial resistance to antibiotics was found, a cautionary use of fluoroquinolone eye drops should be considered.

Abstract:OBJECTIVE:To compare the aqueous humor penetration of topically applied ofloxacin, ciprofloxacin and tobramycin in human eyes.METHODS:Senile cataract patients (125 eyes in 125 cases) receiving phacoemulsification were randomized into three groups: ofloxacin, 42 eyes; ciprofloxacin, 41 eyes and tobramycin, 41 eyes. Each group was randomized into five sub-groups, with 8 or 9 eyes in each sub-group. Each group received either 0.3% ofloxacin, ciprofloxacin or tobramycin topical drops preoperatively. Eyedrops were instilled one drop every 15 minutes for six times. Aqueous samples were obtained from different sub-groups during surgery, 7.5, 15.0, 30.0, 60.0 and 120.0 min after the last drop, respectively. Concentration of the antimicrobial agents was determined by the HPLC (high pressure liquid chromatography).RESULTS:Aqueous humor concentration of ofloxacin was significantly higher than that of ciprofloxacin at the same period after the last drop. The bioavailability of ofloxacin was 6 times that of ciprofloxacin. Tobramycin could not be detected by HPLC, so its concentration was lower than the minimum detectable level by HPLC (0.05 micro g/ml).CONCLUSION:Ofloxacin has the best aqueous penetration in human eyes, as compared with ciprofloxacin and tobramycin, which suggests ofloxacin could be the preferred topical medicine for the prevention and treatment of endophthalmitis.

Abstract:AIM: To learn about the current condition about bacteria contamination of aqueous humor during cataract operation.METHODS: Bacterial culture was conducted of the samples of aqueous humor and conjunctival sac collected from 807 patients during cataract operation from December 2010 to December 2012. Then, the bacterial culture results were analyzed. RESULTS: On average, the bacterial culture positive rate was 0.93%, with no significant difference in the positive rates of samples collected at different times. The average bacterial culture positive rate of conjunctival sac was 9.54%, with significant difference between the positive rates before and after the operation(χ2=37.21, P<0.01). Most of the bacteria types in aqueous humor were the same as those in the conjunctival sac, with most frequent strain being staphylococcus epidermidis(60.00% for aqueous humor and 91.56% for the conjunctival sac). During continuous operations, >6 times of patients' aqueous humor and the positive rate of conjunctival sac bacteria culture is significantly greater than that of ≤5 times of patients. The result of the drug sensitive test indicated that most bacteria in aqueous humor and conjunctival sac were sensitive to Cephalosporins while they were highly resistant to penicillin, erythromycin, tetracycline, and Lomefloxacin. CONCLUSION:Because of the bacterial pollution in the water from most of the conjunctival sac, preoperative conjunctival sac aseptic preparation is essential. Because intraoperative conjunctival sac, even real water may have some bacteria, strengthen the local application of antibiotics can effectively prevent postoperative intraocular infection.
The Importance of Patient Preferences in the Measurement of Health Care Satisfaction

Abstract:The idea that patients will be more satisfied with health care services that are delivered to meet their preferences is central to the concept of health care marketing. Health care providers increasingly use market segmentation and target marketing to optimize the fit between their services and the consumers who receive them. This study evaluates one model for incorporation of patient preferences into the measurement of satisfaction. Using multiple regression analysis, evaluations of three dimensions of health care satisfaction, interpersonal care, technical quality, access to care accounted for 63% of the variance in overall satisfaction. Inclusion of preferences, defined as importance ranks of each dimension, did not improve ability to predict satisfaction. Four preference segments were identified: interpersonal care seekers, access/quality seekers, access seekers and quality seekers. These four subgroups differed significantly on a number of sociodemographic, health status and health service use characteristics but no significant differences were found in satisfaction between preference segments. Patient satisfaction can best be measured as quality evaluations of dimensions without regard to preferences. In considering the merits of market segmentation and target marketing, alternative satisfaction models that link preferences to health care satisfaction or the possibility that preference targeting does not lead to greater satisfaction should be evaluated.
Bacterial Culture for Conjunctival Sac Before Intraocular Lens Implantation for Cataract

Abstract:OBJECTIVE To explore the effective measures for the prevention of endophthalmitis after intraocular lens implantation for the patients with age-related cataract so as to reduce the incidence of endophthalmitis.METHODS The bacterial culture for the conjunctiva and the drug susceptibility testing were performed for 400 eyes that took cataract intraocular lens implantation in three stages,and the results were analyzed.RESULTS The culture of conjunctival sac was performed for totally 1200 cases among which there were 661 cases cultured positive with the positive rate of 55.08%.There were totally 528(79.88%) strains of Staphylococcus epidermidis isolated.Of 400 cases who did not use antibacterial eye drops,there were 306 cases cultured positive with the positive rate of 76.60%.After using of antibacterial eye drops and washing the conjunctival sac of the lacrimal for 1-3 days,the conjunctival sac bacteria were reduced significantly,the positive rate was less than 50.00%.the difference was statistically significant.Most of the strains were sensitive to cephalosporins,but were highly resistant to penicillins,macrolides and aminoglycosides;the difference in the result of conjunctival bacterial culture between the patients with different genders and age was statistically significant,the positive rate of the male(64.43%) was significantly higher than that of the female,the positive rate of the patients aged between 71 and 80 years old was the highest(65.16%),the difference was statistically significant.CONCLUSION According to the distribution of the conjunctival bacterial species and the drug susceptibility testing in difference stages,we can research on the reduction of the conjunctival bacteria in age-related cataract patients undergoing the intraocular lens implantation so as to effectively prevent the postoperative endophthalmitis.
Compound electrolyte intraocular irrigating solution produces better effects on vision recovery of cataract patients after surgery than Ringer lactate solution

Abstract:Purpose: Intraocular irrigating solution is extensively applied in cataract surgery. This paper explored the difference and relationship between optical coherence tomography (OCT) and optical quality analysis system (OQAS) parameters induced by compound electrolyte intraocular irrigating solution (CEIIS) or Ringer lactate (RL) solution during uncomplicated cataract surgery. Methods: Totally 200 senior cataract patients were randomly divided into the CEIIS and RL groups (N = 100 patients/group). The anterior chamber was irrigated by CEIIS or RL during phacoemulsification. Patients were subdivided into diabetes mellitus (DM)+ and DM- groups. The central macular thickness (CMT), hyper reflective foci (HF), modulation transfer function cutoff frequency (MTF cutoff), Strehl ratio (SR), objective scatter index (OSI), and OQAS values (OVs) at 100%, 20%, and 9% contrast levels were measured preoperatively and 1 day and 1 week after operation using spectral-domain optical coherence tomography and OQAS II, respectively. Best-corrected visual acuity (BCVA) was assessed using the Snellen scale, followed by statistical analysis of its logarithm of the minimal angle of resolution. Results: There were no significant differences in clinical characteristics between the CEIIS and RL groups. Both groups exhibited notably increased postoperative CMT, MTF cutoff, SR, OV at 100%, 20%, and 9% contrast levels, and reduced OSI, indicating CEIIS and RL improved postoperative visual quality. CEIIS surpassed RL solution in improving postoperative visual quality, decelerating the increase of macular HF numbers and CMT in DM+ patients and postoperative BCVA. There was no difference between CEIIS and RL in long-term vision improvement. Conclusion: CEIIS surpasses RL in postoperative visual recovery and retards increases of macular HF numbers and CMT in senior DM+ cataract patients.
