Abstract:Background: Asian New York City (NYC) residents have the lowest screening rates by race across cancer type. Past studies indicate heterogeneity within the Asian American population, yet few have examined differences in screening uptake and related factors among Asian ethnic subgroups in NYC. Further, cancer screening knowledge and cancer fatalism have been found to impact cancer screening uptake, and effects vary by race, ethnicity, immigration status, and cancer type. Objective: Our study aimed to examine differences in cancer screening uptake, knowledge, and fatalism among Chinese, Korean, and South Asian NYC adults and to examine the effects of cancer screening knowledge and cancer fatalism on cancer screening uptake. Methods: We used data from NYU Langone Perlmutter Cancer Center's Cancer Community Health Resources and Needs Assessment survey. The survey was developed and distributed in 10 languages in collaboration with 23 community partners. Data were collected from NYC adults with recruitment strategies focused on immigrant and non-English language preference communities from Oct. 2021 - Dec. 2022. Multivariable logistic and multinomial regression analyses were used to examine differences in breast, cervical, and colorectal cancer (CRC) screening uptake; breast and CRC screening knowledge; and cancer fatalism between Chinese, Korean, and South Asian (Bangladeshi, Pakistani, Asian Indian, and Nepali) adults; and to examine the effects of breast and CRC screening knowledge on breast and CRC screening uptake and cancer fatalism on breast, cervical, and CRC screening uptake (n=1,045). Analyses were restricted by age and sex using United States (U.S.) Preventive Services Task Force guidelines. Results: The sample was 60.0% female with a mean age of 45.7 years; 83.1% of participants were born outside of the U.S., 49.9% of participants were Chinese, 22.8% were Korean, and 27.3% were South Asian. Korean women had 0.52 (95%CI: 0.31, 0.88) times lower odds of Pap test uptake than Chinese women; South Asian adults had 0.44 (95%CI: 0.24, 0.80) times lower odds of CRC screening uptake than Chinese adults. Korean adults had 1.68 (95%CI: 1.17, 2.41) times higher odds of knowing the correct age to begin having mammograms compared to Chinese adults; South Asian adults had 0.66 (95%CI: 0.46, 0.94) times lower odds of knowing the correct age to begin CRC screening compared to Chinese adults. Korean adults had a 0.39 (95%CI: 0.25, 0.63) and 0.22 (95%CI: 0.13, 0.37) times lower odds of some and high cancer fatalism, respectively, compared to Chinese adults. No associations were found between cancer screening knowledge or cancer fatalism and cancer screening uptake. Conclusions: Our study adds to the current literature on the diversity of cancer needs, beliefs, and behaviors among Asian Americans. Determinants of Cancer Screening Disparities Among Asian Americans: A Systematic Review of Public Health Surveys

Abstract:We conducted a systematic analysis of 24 peer-reviewed literary works that examined Asian Americans’ breast, cervical, and colon cancer screening, focusing on empirical findings from large-scale public health surveys (i.e., NHIS, CHIS, HINTS, BRFSS). We provide an overview of relevant research in terms of study characteristics, samples, predictor/covariate of cancer screenings, and key findings. Our analysis indicates that Asian Americans’ cancer screening rates are lower than for non-Hispanic Whites for all cancer types in four large-scale public health surveys throughout 17 study years. Acculturation and healthcare access were two significant factors in explaining Asian Americans’ cancer screening rates. Cancer fatalism and family cancer history emerged as potential factors that may account for more variances. However, the screening disparities between Asian Americans and whites persist even after adjusting all covariates, including SES, acculturation, healthcare access, health status, and health perception/literacy. More individual and cultural factors should be identified to address these disparities.

Colorectal Cancer Screening among Korean American Immigrants: Unraveling the Influence of Culture

Abstract:Screening for colorectal cancer (CRC) is underutilized among ethnic minority groups, particularly among Korean American immigrants. To explore the role of cultural and health beliefs in CRC screening, a structured questionnaire was administered to 281 Korean American immigrants aged between 50 and 88 in the New York metropolitan area. Results showed that 20% of the sample had undergone a fecal occult blood test within the past year, and 35% of the respondents had received a sigmoidoscopy and/or colonoscopy within the previous five years. Binary logistic regression analyses revealed significant predictors including health belief constructs, such as perceived seriousness of cancer and confidence in screening uptake, and gender-specific cultural beliefs and attitudes about CRC screening. Perceived helplessness lowered CRC screening among the women, while fatalism lowered it among the men. The findings reinforce a need for cultural-and gender-specific intervention strategies to increase CRC screening in this particularly vulnerable population.

Abstract:e18568 Background: Foreign-born Asians were less likely to utilize preventive care due to medicine beliefs, language barriers, insurance coverage, and more. Their perceptions of breast cancer risks and screening benefits for asymptomatic conditions may differ from individuals born in the United States. It is urgent for preventive health professionals to understand screening disparities in Asian Americans. Methods: Data from the California Health Interview Survey (CHIS) 2015-2016 were used. Multivariable adjusted logistic regressions were generalized to identify the up-to-date mammography screening in relation to socioeconomic, chronic health condition/health behavior, and preventive care utilization, stratified by race. Results: Of NH-White and Asian American women aged 40 and above (unweighted N = 13,451), 78.41% were NH-White, 25.38% were foreign-born, 56.88% spoke only English at home, and 6.44% were not currently insured. The prevalence for self-reported mammography screening in NH-White and Asian American women were 68.06% and 65.97%, respectively. Among Asian American women, the lowest rate of mammograms was in Koreans (55.29%) and Chinese (72.19%) had the highest rate (p = 0.6438). In NH-White women, place of birth, smoking status, borderline diabetes, high blood pressure, is currently insured, and having a preventive care visit in the past 12 months were significant factors. Within Asian American women, borderline hypertension and having a preventive care visit were significant factors, but not ethnicity. Conclusions: The findings indicate that mammogram disparity in relation to socioeconomics, health behaviors, preventive care utilization, and race. Further development of ethnic-specific cancer prevention strategies and policies that address the subgroup differences within the larger racial/ethnic population are needed. Public health outreach and cancer education should target Asian American women with borderline hypertension and no preventive care visits.

Abstract:This study examined the association between cancer history, social support, and up to date colorectal cancer (CRC) screening among four racial/ethnic groups. We conducted a cross-sectional analysis using data on respondents aged 45-75 years from the 2022 Behavioral Risk Factor Surveillance System. Our outcome of interest was CRC screening and exposures of interest were race/ethnicity, cancer history, and social support. Weighted multivariable logistic regression was performed. Among 73,869 adults, the CRC screening rate was 66.8% with the highest rate in non-Hispanic (NH) Whites (72.2%) and the lowest in Hispanics (52.6%). Screening rates were higher in adults with a cancer history (81.9%) and those having social support (69%). Hispanic adults with a cancer history had lower screening use (50.9% vs. 77.4% in no cancer history group; p-value <0.001). Regardless of race/ethnicity, adults without social support had lower screening utilization (p-value<0.05). In effect modification, NH White adults who reported no cancer history and lack of social support were 12% less likely to have CRC screening than those with social support but without cancer history (OR,0.88;95% CI, 0.79-0.98). Similar results were observed among Hispanic adults without a cancer history and social support, with 37% less likely to have CRC screening than those with social support but no cancer history (OR,0.63;95% CI, 0.42-0.93). NH White and Hispanic adults without a cancer history and limited social support were less likely to have CRC screening uptake. By implementing culturally tailored interventions that address social support needs, greater CRC screening compliance may be increased among these populations.

Obstacles to Colorectal Cancer Screening in Chinese: A Study Based on the Health Belief Model

Abstract:BACKGROUND: Colorectal cancer (CRC) has emerged as the second most common cancer in Asia. This study uses the Health Belief Model (HBM) to examine the factors associated with the uptake of CRC testing. METHODS: A population-based telephone survey in which 1,004 randomly selected Chinese residents of Hong Kong aged between 30 and 65 yr were interviewed in 2006. Instruments were developed to measure the variables pertinent to the HBM, including perceived susceptibility, severity, benefit, barriers, and cues to action. The relationships of these predictors with CRC testing were examined using bivariate and multiple logistic regression analyses. RESULTS: Ten percent of the respondents had undertaken a CRC screening test. In the multiple logistic regression analyses, knowledge factors that were positively associated with CRC testing included knowledge of CRC symptoms (adjusted odds ratio [aOR] 3.33, 95% confidence interval [CI] 1.21-9.11) and knowledge of CRC risk factors (aOR 2.61, 95% CI 1.16-5.88). Five of the variables pertaining to the HBM were significant correlates of CRC testing-perceived severity of CRC (aOR 0.28, 95% CI 0.13-0.65), perceived health and psychological barriers to CRC testing (aOR 0.42, 95% CI 0.21-0.84, 95% CI 0.21-0.85), perceived access barriers to CRC testing (aOR 0.22, 95% CI 0.05-0.85), physician's recommendation (indicator of cues to action) (aOR 23.50, 95% CI 10.66-51.80), and having health insurance (indicator of cues to action) (aOR 2.06, 95% CI 1.01-4.19). CONCLUSIONS: CRC testing compliance among this Asian population is low. Knowledge of CRC symptoms and risk factors are low. Perceived health, psychological, and access barriers to CRC testing are high. The physician's recommendation and having health insurance coverage significantly increase testing, but physicians mainly recommend patients with a family history of CRC for testing, and only around one-third of the population has health insurance.
Colorectal cancer screening disparities in Asian Americans: the influences of patient-provider communication and social media use

Abstract:Purpose: We examined the role of patient-provider communication (PPC) during in-person visits and via electronic communication and social media use on colorectal cancer (CRC) screening among Asian Americans (AAs) and Non-Hispanic Whites (NHWs) aged 50 and older. Methods: Health Information National Trends Survey 2017-2020 data were analyzed. Results: AAs tended to evaluate the quality of PPC during their in-person visits to a health care provider lower than NHWs. AAs' CRC screening rate was lower than the rate of NHWs (78.8% vs. 84.4%). After adjusting for sociodemographics, healthcare access, and health status, the quality of PPC was the only significant predictor associated with a lower probability of CRC screening among AAs (Adjusted OR 0.74; 95% CI 0.56, 0.96); while the Internet to communicate with a health care provider was the only significant predictor of CRC screening among NHWs (Adjusted OR 1.76; CI 1.11, 2.79). AAs were more likely to use YouTube to watch a health-related video than NHWs (43.5% vs, 24%). However, social media use was not associated with CRC screening in both AAs and NHWs. Conclusion: Use of electronic communication technology may contribute to improve health information literacy and reduce the disparity. On-line communication may empower the culturally and linguistically diverse AAs by improving their confidence in communication with health care providers. Thus, communication technologies need to be strategically utilized and tailored to better meet the communication needs of racial/ethnic minorities. Online communication technologies may reduce the disparities in PPC related to cancer screening and cancer burden experienced by AAs.
Decreased colorectal cancer incidence and mortality in a diverse urban population with increased colonoscopy screening

Abstract:Abstract Background Although colorectal cancer screening has contributed to decreased incidence and mortality, disparities are present by race/ethnicity. The Citywide Colon Cancer Control Coalition (C5) and NYC Department of Health and Mental Hygiene (DOHMH) promoted screening colonoscopy from 2003 on, and hypothesized future reductions in CRC incidence, mortality and racial/ethnic disparities. Methods We assessed annual percent change (APC) in NYC CRC incidence, stage and mortality rates through 2016 in a longitudinal cross-sectional study of NY State Cancer Registry, NYC Vital Statistics, and NYC Community Health Survey (CHS) data. Linear regression tested associations between CRC mortality rates and risk factors. Results Overall CRC incidence rates from 2000 decreased 2.8% yearly from 54.1 to 37.3/100,000 population in 2016, and mortality rates from 2003 decreased 2.9% yearly from 21.0 to 13.9 in 2016 at similar rates for all racial/ethnic groups. Local stage disease decreased overall with a transient increase from 2002 to 2007. In 2016, CRC incidence was higher among Blacks (42.5 per 100,000) than Whites (38.0), Latinos (31.7) and Asians (30.0). In 2016, Blacks had higher mortality rates (17.9), than Whites (15.2), Latinos (10.4) and Asians (8.8). In 2016, colonoscopy rates among Blacks were 72.2%, Latinos 71.1%, Whites 67.2%, and Asians, 60.9%. CRC mortality rates varied by neighborhood and were independently associated with Black race, CRC risk factors and access to care. Conclusions In a diverse urban population, a citywide campaign to increase screening colonoscopy was associated with decreased incidence and mortality among all ethnic/racial groups. Higher CRC burden among the Black population demonstrate more interventions are needed to improve equity.

Abstract:Introduction: Advances in treatment have significantly improved the survival rates of adolescent and young adult (AYA) cancer patients, with a five-year survival rate reaching 86%. However, these survivors remain at increased risk for developing secondary cancers later in life. Racial and ethnic minority groups, such as Hispanics, often experience significant healthcare disparities, including lower cancer screening rates. Cancer survivors, particularly those who are Hispanic and diagnosed as AYAs, may face unique barriers in adhering to recommended cancer screening guidelines. This study examines the uptake rates of colon, breast, and cervical cancer screenings among Hispanic survivors of AYA cancer compared to their age- and sex-matched Hispanic non-cancer controls. Methods: The National Health Interview Survey data (years 2000 - 2021) [R1] were analyzed to identify Hispanic survivors of AYA cancer and Hispanic age- and sex-matched non-cancer controls. Data on cancer screening, including whether patients were up- to-date and ever screened for colon, breast, and cervical cancers, were assessed. The US Preventive Services Task Force recommendations were used to determine whether survivors were up-to-date for the cancer screening tests. The sample included 92 colon cancer survivors, 113 breast cancer survivors, and 313 cervical cancer survivors, matched with 920, 1130, and 3130 controls, respectively. Weighted percentages and odds ratios (OR) were calculated, adjusting for age at survey, sex, marital status, education, poverty status, and insurance. Results: Colon cancer screening uptake was significantly higher among survivors, with 26.1% up-to-date versus 11.5% of controls (OR=2.49, 95% CI=1.17-5.30, p=0.019). For breast cancer, 14.6% of survivors were up-to-date compared to 5.1% of controls [R2] (OR=2.51, 95% CI=1.33-4.72, p=0.005), and 82.6% of survivors were ever screened versus 38.7% of controls (OR=7.41, 95% CI=4.43-12.41, p<0.001). Cervical cancer screening showed the most pronounced difference, with 40.6% of survivors up-to-date compared to 19.0% of controls (OR=3.36, 95% CI=2.43- 4.65, p<0.001), and 71.2% of survivors ever screened compared to 28.6% of controls (OR=14.16, 95% CI=8.56-23.40, p<0.001). Conclusion: Hispanic AYA cancer survivors exhibit significantly higher cancer screening rates compared to matched controls. Despite these positive findings, the overall low screening rates highlight the need for targeted interventions to ensure that all survivors adhere to recommended screening guidelines. Enhancing access to healthcare and ensuring continuity of care are critical steps toward reducing disparities and improving health outcomes for this vulnerable population. Risk Factors for Readmission in Patients With Cancer Comanaged by Hospitalists

Abstract:Little is known about which variables put patients with cancer at risk for 30-day hospital readmission. Comanagement of this often complex patient population by specialists and hospitalists has become increasingly common. This retrospective study examined inpatients with cancer comanaged by hospitalists, hematologists, and oncologists to determine the rate of readmission and factors associated with readmission. Patients in this cohort had a readmission rate of 23%. Patients who were discharged to a skilled nursing facility (odds ratio [OR] = 0.34) or hospice (OR = 0.11) were less likely to have 30-day readmissions, whereas patients who had surgery (OR = 3.16) during their index admission were more likely. Other factors, including patient demographics, cancer types, and hospitalization interventions and events, did not differ between patients who were readmitted and those who were not. These findings contribute to a growing body of literature identifying risk factors for readmission in medical oncology and hematology patients.

Abstract:Introduction: Beliefs and perceptions about cancer risks significantly influences a person's cancer screening and overall health behaviors. To better understand the factors associated with perceptions surrounding cancer in Oregon, this study examined racial differences in cancer risk beliefs among Oregon adults as part of the Population Health Assessment in Cancer Center Catchment Areas effort. Methods: This study utilizes data from the Understanding Cancer in Oregon (UCanOR) project, which included a 47-item cross-sectional survey capturing demographic and behavioral information. The survey was administered to participants selected through an area-based sampling strategy (51.8%) and a convenience sample (48.2%). Cancer beliefs and perceptions were assessed through five questions: 1) "There are so many different recommendations about preventing cancer, it's hard to know which ones to follow;" 2) "Cancer is most often caused by a person's behavior or lifestyle;" 3) "It seems like everything causes cancer; 4) When I think about cancer, I automatically think about death;" 5) "There is not much you can do to lower your chances of getting cancer." Multivariable generalized linear regression and logistic regression analyses were used to assess the association between race/ethnicity and cancer beliefs either as a summed continuous variable or five specific binary variables with Agree vs. Disagree as the outcome, adjusting for confounders (age, sex, employment status, marital status, BMI, cancer history, insurance status). Results: The study included 1,357 respondents aged over 18 years old with a mean age of 50.0 (SD = 17.8), with the majority being Non-Hispanic White (87.3%), followed by Asian/Pacific Islanders (8.4%), Hispanic White (3.5%), and Black (0.8%). Asian/Pacific Islanders were significantly more likely than individuals of other racial backgrounds to have negative beliefs about cancer. Specifically, compared to Non-Hispanic White, Asian/Pacific Islanders were 2.5 times as likely to agree that "cancer is most often caused by a person's behavior or lifestyle" [OR (95% CI) = 2.48 (1.64-3.75)] and that "there is not much you can do to lower your chances of getting cancer" [OR (95% CI) = 2.80 (1.70-4.62))], and 1.5 times as likely to agree that "when I think about cancer, I automatically think about death" [OR (95% CI) = 1.61 (1.07-2.41)]. Conclusion: This study found that Asians/Pacific Islanders in Oregon had significantly more negative cancer beliefs. In terms of future directions, interventions aimed at promoting healthy cancer prevention behaviors should particularly target Asian/Pacific Islander communities. Abstract:Background: Native Hawaiians and Pacific Islanders (NHPI) are hindered by sociocultural, geographic, and environmental factors that contribute to cancer health disparities, often masked by aggregated data with Asian Americans. An average of 7,393 Hawai'i residents are diagnosed with invasive cancer yearly, and rates are disproportionate ethnically; . Native Hawaiians and Micronesians suffer higher mortality rates for breast cancer (24.9 and 21.6 per 100,000) than White women (17.0). This study aims to investigate social drivers of health behaviors among NHPI in Hawaiʻi by examining social factors that influence cancer screenings through mixed methods. Methods: This mixed methods study is a follow-up to a cross-sectional survey administered in 2018 to 3 Pacific Islander ethnic subgroups: Native Hawaiians, Chuukese, and Marshallese in Hawaiʻi using Respondent Driven Sampling within public housing (N=1,010 NHPI). Disparate cancer screening practices were uncovered within these groups when compared to Hawaiʻi state data. Logistic regression was used to analyze predictors of colon, breast, and cervical cancer screenings. The Gelberg Behavioral Model for Vulnerable Populations and socio-ecological model served as the theoretical models. To triangulate the findings, 16 semi-structured interviews with adults living within the communities first surveyed were conducted in 2023 to uncover any additional drivers of health behaviors that the regression missed. Results: Quantitative findings include cultural/linguistic misunderstandings between NHPI and healthcare providers. Participants who indicated difficulty understanding a provider's instructions because of language were 51% less likely to be screened for cervical cancer (p = .04). Individuals who hold neutral acculturation to both their ethnic and American culture are less likely to be screened for cancer. Those whose responses fell in a neutral category were 40% less likely than those with a high ethnic identity to have had a colon cancer screening (p = .02). Qualitative findings demonstrate differing views among Pacific Islander groups including competition for resources between groups within public housing. All of the interviewees highlight the role of God and faith in decision making. Informational exchanges take place mainly within the family and cohabitants. From triangulating the findings, there is a lack of strong social ties/networks that could facilitate health-based resources and knowledge. Families are not discussing self-care or family histories of disease, which are instrumental in the early detection of cancers and missed within the regression. Conclusion: This study provides insight into the social drivers of cancer screenings for NHPI communities in Hawaiʻi. Novel insights for the fields of medical sociology and public health are highlighted by influential social processes to NHPI cancer screening behaviors. Recommendations include further research and programs that focus on and foster social dynamics of these communities in relation to cancer screenings. Citation Format: Mark L. Abstract:Introduction: The University of California, San Francisco (UCSF), a National Cancer Institute (NCI) designated comprehensive cancer center implemented NCI's Colorectal cancer (CRC) and Clinical Trials (CT) Education Initiative to foster awareness, knowledge and intent to uptake CRC screening or participate in CT. Racial/ethnic groups have higher death rates and shorter survival rates for most cancers and face additional disparities and inequities. CRC is the third most commonly diagnosed cancer. CRC screening reduces incidence and mortality, however, it is underutilized. CT are studies that evaluate methods to understand prevent, find and treat cancer plus assess safety & effectiveness. Racial/ethnic populations CT participation is low. More diverse enrollment is needed to guide future cancer prevention, control, care and improve health equity. Methods: UCSF Community Health Educators engaged with community partners and the public to conduct CRC & CT education. The education topics included understanding the colon and rectum, colorectal health promotion, risk factors, what is CRC and screening, access to screening, what are clinical trials, how they advance medical progress, diversity & inclusion, types and phases of CT, informed consent, myths and facts, mistrust/other concerns, barriers, and how to find and enroll in CT. Education sessions included administering pre/post surveys- 19-item for CRC and 17-item for CT to measure knowledge and intention to get screened for CRC or participate in CT. Results: There were 772 total surveys (N=411 CRC completed pre/post, 361 completed CT pre/post). CRC respondents: 70% female, 30% male; 54 average age; 53% African American, 0.3% American Indian, 23% Asian, 13% Latino, 4% Pacific Islander, 3% White & 3% Multiracial. Overall (from pre to post education) CRC knowledge significantly increased, as well as intent to get screened, and to ask healthcare provider about CRC. Knowledge improved pre to post by 14% (76% vs 91%). Post-education intent, respondents strongly agreed or agreed: 91% talk to healthcare provider, 92% get screened for CRC, 89% talk about CRC with family/friends, 95% eat healthier, 95% increase physical activity. CT respondents: 77% female, 23% male; 51 average age; 64% African American, 1.2% American Indian, 10% Asian, 17% Latino, 2% Pacific Islander, 4% White & 2% Multiracial. Post CT education knowledge, intent to seek out CT information and intent to join significantly increased. CT knowledge improved pre to post by 7% (83% vs 90%). Post-education intent respondents strongly agreed or agreed: 69% seek out CT information, 59% search for CT, 75% talk to a healthcare provider about CT; 78% importance of people of color CT participation; 48% intent to join a CT and 70% talk to family/friends about joining a CT. Conclusion: CRC and CT education demonstrated efficacy in increasing awareness and knowledge and advanced intent to get colorectal cancer screening and enroll in clinical trials. Abstract:Objective To broaden the currently limited reach of genomic innovations, research is needed to understand how psychosocial and cultural factors influence reactions to genetic testing in diverse subgroups. Cancer fatalism is important in cancer prevention and deserves theoretical and empirical attention in the context of genomics and behavior change. Methods The current study employed data from a randomized controlled trial (N = 593) offering skin cancer genetic testing (using the melanocortin‐1 receptor [MC1R] gene) in primary care in Albuquerque, New Mexico, USA. We examined interrelations of cancer fatalism with demographics, general health beliefs, perceived risk, perceived control, sun protection and skin screening behaviors and cancer worry in the skin cancer context stratified across Hispanic versus non‐Hispanic ethnicity, and examined cancer fatalism as a moderator of intervention effects on study primary outcomes, including 3‐month sun protection, cancer worry and perceived risk. Abstract:In the United States (US), disparities in cervical cancer screening uptake persist by race/ethnicity and socioeconomic status (SES). However, few have examined how race/ethnicity and SES contribute to specific guideline adherence patterns, which can exacerbate existing disparities. We investigated the impact of race/ethnicity and SES (education and household net worth [HHN]) on the probability of over- and under-screening. Optum Clinformatics Data Mart from 2012-2021 was used to identify average-risk females aged 30-64 and ≥1 cervical cancer screening with continuous health plan enrollment and no history of hysterectomy/cervical cancer. Over- and under-screening were defined as intervals between the index and subsequent screening which were shorter and longer than the interval recommended by the 2012 US Preventive Services Task Force guideline, respectively. Multivariate logistic regression was used to evaluate the association of race/ethnicity (Non-Hispanic [NH]-White, NH-Asian, Hispanic, NH-Black) and SES including HHN (< 250,000- 500,000) and education level ( Bachelor's Degree) with over- and under-screening. We tested for interactions between race/ethnicity and SES. Of the 686,884 eligible patients, 72.8% were NH-White, 71.4% had <Bachelor's Degree and 43.3% had HHN < 250,000 (OR, 1.3 95%CI, 1.2-1.3) and 499,000 (OR, 1.1; 95%CI, 1.0-1.1) vs > 250,000 (OR, 0.9; 95%CI, 0.9-1.0) and 499,000 (OR, 0.9; 95%CI, 0.9-1.0) vs >$500,000. For under-screening, the interaction between race/ethnicity and HHN was significant, and the predicted probability (PP) increased with higher HHN among Hispanic but not in other groups (p<0.01). For over-screening, the interaction between race/ethnicity and HHN was significant and the PP increased with higher HHN among Hispanic and NH-White but decreased for NH-Black and NH-Asian. The interaction between race/ethnicity and education was significant and the PP increased with higher education among Hispanic and NH-White but decreased among NH-Black and NH-Asian. Even within a commercially insured population, we observed that the majority of patients received non-guideline adherent screening and SES contributed to disparities differently among racial/ethnic groups. Findings indicate the need to tailor interventions and policies for subpopulations experiencing over- and under-screening to address the overall screening quality and disparities in outcomes. Abstract:1576 Background: Despite the clinical importance of genetic testing and counseling for cancer risk reduction and treatment, minority populations are less likely to receive genetic testing and counseling or participate in genetics research, contributing to health disparities. In a randomized study of three modes of genetic counseling (GC3) conducted in three safety net hospitals, we noted a lower accrual of participants identifying as Chinese despite the availability of language-concordant study documents and bilingual staff. To understand barriers to recruitment, we conducted a mixed methods study using responses to the San Francisco Mammography Registry (SFMR) Breast Health History questionnaire (used for recruitment for GC3) and qualitative interviews with seven participants of the genetic counseling study. Methods: We extracted data from the SFMR from 2014-2018 for patients self-identifying as Asian and a control group of patients self-identifying as White. The primary outcome was the proportion of patients who agreed to be contacted for future breast health research. Survey data included information for personal history of breast cancer, history of biopsy, first-degree relative with cancer, age, and level of education. Z-tests were conducted for comparisons between groups. For qualitative analysis, seven Chinese-identifying participants were interviewed to understand why they chose to participate in the genetic counseling study and identify potential barriers and facilitators. Results: We included 121,415 patients (n=38,483 Asian). Asian patients had significantly lower rates of accepting future contact for breast health research studies (25.4% for Asians vs. 48.3% for White, p<0.0001). Lower rates were observed across Chinese (23.3%), Filipina (30.2%), Japanese (27.8%), and Vietnamese (23.3%) subgroups (p<0.0001 for all). A history of breast cancer (personal or family) or biopsy was associated with higher rates of acceptance within both Asian and White groups (p<0.001 for all comparisons). Education did not change participation rates as much in Asian patients as in White patients. The qualitative study found barriers across three domains: awareness, opportunity, and acceptance, with a referral from the participant’s medical team identified as a facilitator. Conclusions: Individuals identifying as Asian in the SFMR were less likely to agree to be contacted for future breast health research. We identified multilevel barriers through qualitative interviews. This has important implications for designing recruitment strategies for Asian populations.

Abstract:Abstract We examined rural-urban differences in the prevalence of cancer risk factors and screening behaviors across U.S. census regions to better understand variations within and between geographic regions and inform strategies to address rural cancer inequities. Using an ecological cross-sectional design, we examined rural-urban differences in the self-reported prevalence of county-level cancer risk factors (i.e., obesity, physical inactivity, alcohol consumption) and cancer screening behaviors (i.e., breast, colorectal and cervical) ascertained from the Behavioral Risk Factor Surveillance System and National Health Interview Survey (2008-2013) across four U.S. census regions (Northeast, Midwest, South, and West). County-level rurality was defined using 2013 U.S. Office of Management and Budget rural-urban classification codes, and the U.S. Department of Agriculture, Economic Research Service’s rural-urban continuum codes. Chi-square tests assessed differences in the mean prevalence of county-level cancer risk factors and cancer screening behaviors in each geographic region. Analysis of variance (ANOVA) models estimated the p-values for trends in cancer risk factors and screening prevalence across the rural-urban continuum. Within each geographic region, rural counties had a higher prevalence of smoking, obesity and physical inactivity. Yet, rural-urban differences in cancer risk factors were not statistically significant in all geographic regions (e.g., prevalence of obesity for Western rural vs. urban counties was 26.5% vs. 25.7% [p=0.08] and physical inactivity for Northeastern rural vs. urban counties was 23.2% vs. 22.5% [p=0.15]). Moreover, the mean prevalence of obesity was higher in urban areas of the Midwest (31%) and South (31.3%) compared to rural areas in the West (26.5%) and Northeast (29.5%). Binge alcohol use was higher in rural vs. urban counties in the West (19.8% vs. 17.7%; p<0.001) and Midwest (22.3% vs. 21.5%; p=0.006), but lower in the South (13.5% vs. 15.2%; p<0.001). Obesity and smoking prevalence increased with increasing rurality across all regions (ptrend<0.045). Breast, cervical and colorectal cancer screening prevalence were lower in rural vs. urban counties in each geographic region (all p-values<0.04). However, the overall prevalence of screening across all cancer sites was higher in rural Northeast counties compared to both rural and urban counties in the South and West regions. Across all regions, cervical and breast cancer screening decreased with increasing rurality (all ptrend<0.001). A significant inverse trend in prevalence of colorectal cancer screening with increasing rurality was observed in all regions except the Northeast (ptrend=0.17). Our findings suggest notable variations in rural-urban cancer risk factors and screening disparities across U.S. geographic regions. Further exploration of the source of this geographic variation is warranted to ensure the development and implementation of relevant cancer control interventions targeting rural populations most in need. Abstract:Chinese Americans are one of the fastest-growing and largest Asian-American subgroups. Approximately 70% of Chinese Americans are immigrants with 46% being of limited English proficiency. Despite colorectal cancer (CRC) being the second leading cause of cancer death in Chinese Americans, Chinese Americans have lower CRC screening rates than other Asian subgroups, and only 40% of CRC cases among Chinese Americans are diagnosed early. Furthermore, CRC is the second most common cancer among Chinese American women. Race/ethnicity concordant providers may process culturally specific knowledge, skills, and experience that may facilitate better communication. Although using an interpreter can help to overcome language barriers between providers and patients, it may not achieve the same level of communication as a language concordant provider. The purpose of this study was to test a community education intervention, taking into account the racial/ethnic and language concordance of the presenter, to increase CRC screening uptake among foreign-born Chinese American women. This study used a quasi-experimental design and a convenience sample of 198 foreign-born Chinese-American women. Logistic regression was used to examine the effect of racial/ethnic and language concordance of the presenter on the return of completed FOBT kits for testing. The Chinese/English-speaking presenter had the highest return rate (73%), followed by the White/English-speaking presenter (61%), and the Chinese/Chinese-speaking presenter (48%), and the differences were statistically significant. Post-education intention to screen was also a significant predictor for returning the FOBT kit for testing. The use of an interpreter did not decrease the effectiveness of the educational intervention.

Abstract:A simple model electronic Hamiltonian to describe the potential energy surfaces of several low-lying d-d states of the [Fe(bpy)(3)](2+) complex is developed for use in molecular dynamics (MD) simulation studies. On the basis of a method proposed previously for first-row transition metal ions in aqueous solution, the model Hamiltonian is constructed using density functional theory calculations for the lowest singlet and quintet states. MD simulations are then carried out for the two spin states in aqueous solution in order to examine the performance of the model Hamiltonian. The simulation results indicate that the present model electronic Hamiltonian reasonably describes the potential energy surfaces of the two spin states of the aqueous [Fe(bpy)(3)](2+) system, while retaining sufficient simplicity for application in simulation studies on excited state dynamics.
