Abstract:Introduction: To assess the effect of a hands-on ultrasound training session to teach urologic trainees ultrasound-guided percutaneous needle placement. Materials and methods: University of California, San Francisco (UCSF) urology residents completed a time trial, placing a needle into a phantom model target under ultrasound guidance. Participants were randomized into three educational exposure groups: Group 1's time trial occurred prior to any teaching intervention, group 2's after experiencing a hands-on training module, and group 3's after exposure to both the training module and one-on-one attending feedback. Needle placement speed and accuracy as well as trainees' perceived confidence in utilizing ultrasound were measured. Results: The study cohort consisted of 15 resident trainees. Seven were randomized to group 1, three to group 2, and five to group 3. All residents reported minimal prior ultrasound experience. Their confidence in using ultrasound improved significantly after completing the training module with the most significant improvement seen among junior residents. Time to needle placement was fastest after receiving attending feedback (46.6sec in group 3 vs. 82.7sec in groups 1 and 2, p<0.01). Accuracy also improved with attending feedback, though the number of repositioning attempts did not differ significantly between groups. Conclusions: A hands-on training module and use of an abdominal phantom trainer increased resident confidence and skill in their use of ultrasound to guide percutaneous needle positioning. Attending feedback is critical for improving accuracy in needle guidance toward a target. Ultrasound-guided needle positioning is a teachable skill and can be applicable to multiple urologic procedures.
Training novice in ultrasound-guided venipuncture: A randomized controlled trial comparing out-of-plane needle-guided versus free-hand ultrasound techniques on a simulator

Abstract:Background: Peripheral intravenous access is a common medical procedure, however, it can be difficult to perform in some patients. Success rates have proved greater with ultrasound guidance. Peripheral intravenous access using ultrasound requires specific training, especially for new ultrasound users. To overcome these difficulties, guidance devices on ultrasound probes are able to control the angle of penetration into tissues. We hypothesized that, and particularly for new ultrasound users, the use of a needle guide (NG) paired with the out-of-plane approach would facilitate puncture of a simulation model of vessel more effectively than similar free hand (FH) techniques. Methods: A prospective controlled randomized study was conducted of new ultrasound users using a guide wire introducer needle on gelatine phantom. After a 30-min lecture, one group performed the FH technique and the other group performed the NG technique both in an out-of-plane approach. The main criterion was the number of attempts before success of catheterization of this model of vessel. Results: Thirty-four nurse anesthetist students participated in the study. The number of attempts before success using the NG technique was significantly lower: 3.7 (±0.9) in the NG group versus 6.7 (±3.3) in the FH group ( p = 0.01). In the NG group, 100% of the participants achieved success after the sixth attempt. In the FH group, only 81.25% ( n = 13/16) reached success. Conclusion: NG technique has been proved to have a steeper learning curve compared with the FH technique. A study on a learning curve in clinical practice is needed to confirm these results.
Application of simulation-based training in the ultrasound-guided regional anesthesia teaching program

Abstract:Objective:To evaluate the effectiveness of simulation-based training in the ultrasound-guided regional anesthesia teaching program.Methods:This prospective assessor-blind randomly controlled trial was conducted in the hospital from November 2019 to October 2020. Thirty anesthesia trainees from clinical rotation program of anesthesiology residence training project were randomly divided into workshop group (WG) or the control group (CG) for pre-technical procedure training on UGRA. Following didactic lectures, simulation-based workshops were performed only for the WG. All trainees were assessed by written examination, ultrasound scanning and anatomy recognition in human body and ultrasound-guided target injections into pork mass. The scores of examination, drawing anatomy, the sonographic proficiency , nerve recognition and the time taken to perform the injections as well as the errors were all recorded and analyzed. Difference of measurement data was compared by t-test or rank-sum test.Results:The simulation-based training program significantly decreases the time taken for target injections in the WG as compared with that in the CG [188.0(53.5) s vs. 219.0(70.5) s, P=0.006]. The total errors score was lower in WG than in CG [5.5(4.5) vs. 10.0 (8.5), P=0.001]. The drawing anatomy score (33.8±5.4 vs. 28.5±6.1), sonographic proficiency score (37.8±5.9 vs. 24.5±7.4) and the nerve recognition score (7.1±1.9 vs. 4.6±2.3) were all higher in the WG than that in the CG (all P<0.05). Conclusions:For anesthesia trainees, the simulation-based training program can improve the UGRA skills and support the building of clinical competence.
Mixed reality navigation system for ultrasound-guided percutaneous punctures: a pre-clinical evaluation

Abstract:Image-guided surgery is growing in importance with each year. Various imaging technologies are used. The objective of this study was to test whether a new mixed reality navigation system (MRNS) improved percutaneous punctures. This system allowed to clearly visualize the needle tip, needle orientation, US probe and puncture target simultaneously with an interactive 3D computer user inferface. Prospective pre-clinical comparative study. An opaque ballistic gel phantom containing grapes of different sizes was used to simulate puncture targets. The evaluation consisted of ultrasound-guided (US-guided) needle punctures divided into two groups, standard group consisted of punctures using the standard approach (US-guided), and assisted navigation group consisted of punctures using MRNS. Once a puncture was completed, a computed tomography scan was made of the phantom and needle. The distance between the needle tip and the center of the target was measured. The time required to complete the puncture and puncture attempts was also calculated. Total participants was n = 23, between surgeons, medical technicians and radiologist. The participants were divided into novices (without experience, 69.6%) and experienced (with experience > 25 procedures, 30.4%). Each participant performed the puncture of six targets. For puncture completion time, the assisted navigation group was faster (42.1%) compared to the standard group (57.9%) (28.3 s ± 24.7 vs. 39.3 s ± 46.3—p 0.775). The total punctures attempts was lower in the assisted navigation group (35.4%) compared to the standard group (64.6%) (1.0 mm ± 0.2 vs. 1.8 mm ± 1.1—p 0.000). The assisted navigation group was more accurate than the standard group (4.2 ± 2.9 vs. 6.5 ± 4.7—p 0.003), observed in both novices and experienced groups. The use of MRNS improved ultrasound-guided percutaneous punctures parameters compared to the standard approach.
The Effectiveness of Simulation-based Ultrasound-guided Regional Anesthesia Training Programs for Anesthesia Residents: a Prospective Randomized Controlled Trial

Abstract:Abstract BackgroundOptimizing educational programs for obtaining and maintaining competency in ultrasound-guided regional anesthesia (UGRA) is needed for anesthesia residents. This study aimed to assess the effectiveness of simulation-based training programs on the UGRA skills of senior anesthesia residents.MethodsThis prospective assessor-blind randomized controlled trial was conducted in a tertiary hospital in November 2019. Twenty anesthesia residents who had been in a clinical rotation in anesthesiology for 3-5 years were randomly allocated to either the workshop group (WG) or the control group (CG) for pretechnical procedure training on UGRA. Following didactic lectures for both groups, simulation-based workshops were performed only for the WG. All participating trainees were assessed by written examination, ultrasound scanning and anatomy recognition in live human models and ultrasound-guided target injections in porcine meat models. The written examination score, sonographic proficiency scores, time taken to perform the injections and errors were recorded and analyzed.ResultsThe simulation-based training program significantly reduced the time taken for target injections performed by trainees in the WG (183.9±44.8 seconds) compared with those in the CG (239.6±64.2 seconds): mean difference -55.8 (-107.7, -3.7), P=0.037. The error score was also significantly lower in the WG than in the CG. Methods A systematic literature search was performed in Medline, Web of Science, Embase, the Cochrane Central Register of Controlled Trials, reference lists of articles, and other sources. Only randomized controlled trials were included. Two reviewers independently selected and assessed each study for quality and extracted data. Results Twelve randomized controlled trials were included in the meta-analysis. The results indicated that ultrasound-guided intraarticular and periarticular joint injections were more accurate than the landmark-guided injections (odds ratio, 0.36; 95% confidence interval, 0.22–0.60). Ultrasound-guided joint injections significantly decreased the visual analog scale scores at both 2 wks (mean difference, −9.57; 95% confidence interval, −13.14 to −5.99) (P < 0.00001) and 6 wks (mean difference, −14.21; 95% confidence interval, −18.20 to −10.21) (P < 0.00001) after injection. There was no statistically significant difference in visual analog scale score at 12 wks between ultrasound-guided and landmark-guided intraarticular and periarticular joint injections (mean difference, −4.42; 95% confidence interval, −11.71 to 2.87) (P = 0.23). Conclusions Intraarticular and periarticular injections using ultrasound guidance significantly improves the accuracy of joint injections, and there is a significant decrease in visual analog scale scores for up to 6 wks after injection. The effect of ultrasound guidance on the long-term outcome of joint injections is inconclusive. The improved accuracy of injections was associated with pain relief. The authors recommend routine ultrasound guidance for intraarticular and periarticular injections.

Abstract:Objective: The aim of this study was to investigate the advantages of the double-screen contrast method compared with the short-axis ultrasound display method for teaching ultrasound-guided femoral vein puncture during the standardized training of resident doctors. Methods: Sixty resident doctors undergoing standardized training were randomly divided into a test group (short–long axis switching double-screen contrast, n = 30) and a control group ( n = 30). These two groups of physicians underwent teaching and training of ultrasound-guided femoral vein puncture, and the success rate of the first puncture attempt, the total catheterization time, and any accidental femoral artery punctures were recorded and compared between the two groups. Results: The success rate of the first puncture attempt in the test group was significantly higher than that in the control group ( P < 0.05). In the control group, two doctors accidentally punctured the artery, while in the test group, no arterial punctures occurred. The puncture time for the test group was longer than for the control group ( P < 0.05). Conclusion: In the standardized training of ultrasound-guided femoral vein puncture for resident doctors, the double-screen contrast method has significant advantages. It enables resident doctors to quickly understand and apply the technique, so it is worth making this the method of choice.

Abstract:Introduction Ultrasound is recommended to guide insertion of peripheral intravenous vascular cannulae (PIVC) where difficulty is experienced. Ultrasound machines are now common-place and junior doctors are often expected to be able to use them. The educational standards for this skill are highly varied, ranging from no education, to self-guided internet-based education, to formal, face-to-face traditional education. In an attempt to decide which educational technique our institution should introduce, a small pilot trial comparing educational techniques was designed. Methods Thirty medical students were enrolled and allocated to one of three groups. PIVC placing ability was then observed, tested and graded on vascular access phantoms. Results The formal, face-to-face traditional education was rated best by the students, and had the highest success rate in PIVC placement, the improvement statistically significant compared to no education (p = 0.01) and trending towards significance when compared to self-directed internet-based education (p<0.06). Conclusions The group receiving traditional face-to-face teaching on ultrasound-guided vascular access, performed significantly better than those not receiving education. As the number of ultrasound machines in clinical areas increases, it is important that education programs to support their safe and appropriate use are developed.

Abstract:Background Radial artery cannulation is a crucial investigative procedure for measuring patients’ blood pressure invasively and serial blood gases. However, radial artery cannulation can be challenging for medical residents, and it is necessary to establish a facile and straightforward teaching strategy. This study aimed to evaluate the efficiency of acoustic shadowing-facilitated ultrasound guidance on radial artery cannulation teaching for medical residents. Methods A total of 116 medical postgraduates who underwent standardized residency training programs in the Department of Anesthesiology were randomly divided into a new ultrasound-guided teaching group and a traditional ultrasound-guided teaching group. In the new ultrasound-guided teaching group, radial artery puncture technique was taught by acoustic shadowing-facilitated ultrasound guidance. The training included both theoretical and practical components. After the training, the success rate of the first puncture attempt, the success rate of the catheterization, the ultrasonic positioning time, and the catheterization time of the two groups were compared in a unified manner. A questionnaire on the subjective evaluation of the various aspects of the program by participants was conducted at the end of the training period. Results The study included 101 medical residents. The success rate for radial artery puncture at the first attempt in the new ultrasound-guided teaching group was 78.43%. It was significantly higher than that of the traditional ultrasound-guided group (58.00%, odds ratio = 0.380; 95% CI = 0.159 to 0.908; p = 0.027). The success rate for the first arterial catheterization in the new ultrasound-guided teaching group was significantly higher than that of the traditional ultrasound-guided group (74.51% vs. 52.00%, odds ratio = 0.371; 95% CI = 0.160 to 0.858; p = 0.019). The ultrasonic positioning time and catheterization time in minutes in the new ultrasound-guided teaching group were significantly shorter than that of the traditional ultrasound-guided group (14.36 ± 3.31 vs . 18.02 ± 4.95, p < 0.001; 10.43 ± 2.38 vs . 14.78 ± 8.02, p = 0.012). However, no significant differences were observed in the incidence of local hematomas and teaching satisfaction scores between the two groups. Conclusion Acoustic shadowing facilitates ultrasound-guided radial artery puncture and catheterization is beneficial in the standardized training and teaching of residents. It improves the success rate of the first attempt at radial artery puncture and catheterization and shortens the time of ultrasound location and catheterization. Analyzing the Evolution of Needle and Ultrasound Probe Manipulation Skills of Interventional Radiology Trainees With Time and Experience

Abstract:Purpose: To test the hypothesis that hand motion analysis can measure the progression of needle and ultrasound probe manipulation skills of interventional radiology trainees in central venous line placement. Materials and methods: An expert cohort of 6 interventional radiologists and 4 anesthesiologists and a trainee cohort of 6 novice trainees (<50 central lines) and 5 experienced trainees (>50 central lines) performed simulated central venous access. Four novices and 1 experienced trainee repeated the task 1 year later. An electromagnetic motion tracking system tracked the needle hand and ultrasound probe. Path length, translational, and rotational movements were calculated separately for the needle hand and probe sensor. These metrics were used to calculate motion metrics based scores on a scale of 0 to 3 for each sensor. Nonparametric statistics were used, and the data are reported as median ± interquartile range. Results: Comparing novice and experienced trainees, there was a significant difference in probe scores (experienced vs. novice: 1 ± 2 vs. 0 ± 0, P = 0.04) but not in needle-hand scores (1 ± 1.5 vs. 0 ± 1, P = 0.26). Trainees showed a significant increase in probe scores at the 1-year follow-up (baseline vs. follow-up: 0 ± 1 vs. 2.5 ± 1.8, P = 0.003), but no significant difference was observed in the needle manipulation metrics. Experts differed significantly from experienced trainees for all metrics for both sensors (P < 0.05), with the exception of the path length of the probe. Conclusions: Acquisition of improved dexterity of the probe may occur before improvement in the dexterity with the needle hand for interventional radiology trainees.
Ultrasound probe tilt impedes the needle-beam alignment during the ultrasound-guided procedures

Abstract:Abstract The objective of this study was to identify the factors that complicate the needle visualization in ultrasound-guided in-plane needling procedures. Forty-nine residents were recruited and randomized to insert the simulated blood vessel with four different views including Neutral (the long axis of the probe along the visual axis and the ultrasonic beam vertical to the surface of gel phantom), 45°-rotation (45° angle between the long axis of probe and the operator’s visual axis), 45°-tilt (45° angle between the ultrasonic beam and the surface of gel phantom) and 45°-rotation plus 45°-tilt of probe. Number of needle redirections, insertion time, and needle visibility were documented and compared for each procedure. When the residents faced with 45°-tilt view, the needle redirections (2 vs 0) and insertion time increased significantly (39 vs 16) compared with that of the Neutral view. When faced with 45°-rotation plus 45°-tilt view, the residents’ performance decreased further as compared with that of the 45°-tilt view and the Neutral view. However, there was no performance difference between the Neutral view and 45°-rotation view. In conclusion, during ultrasound-guided in-plane procedures, tilting the ultrasound probe may increase the difficulty of needle-beam alignment.

Abstract:Background: Anesthesia, nerve block, therapeutic injections, and biopsies all require an acupuncture intervention. However, traditional two-dimensional (2D) ultrasound-guided needle puncture is often challenging and therefore requires the use of three-dimensional (3D) ultrasound images to accurately identify and evaluate the patient's anatomical structure.Methods: In this study, a 3D multi-modal intelligent intervention system using electromagnetic navigation for real-time positioning and ultrasound images was described. A total of 190 cases requiring puncture were randomly divided into control (conventional 2D ultrasound instrument) and experimental (novel 3D ultrasound imedis9000) groups. The advantages and disadvantages of the two puncture methods were prospectively analyzed in the 190 cases, and the feasibility of electromagnetic navigation real-time positioning was compared to ultrasound imaging.Results: This study included 190 cases from two centers that required puncture treatment and were randomly assigned to the control (conventional 2D ultrasound instrument; n=95) or the experimental (novel 3D ultrasound imedis9000; n=95) groups. Percutaneous vascular puncture, percutaneous biopsy, percutaneous bile duct puncture, thoracic paravertebral nerve block, and sciatic nerve block operations were performed separately. The results indicated that the puncture time and number of trials in the experimental group were significantly lower than those in the control group. No significant difference was identified in the basic vital signs between the two groups before and after surgery. The success rate of the novel 3D ultrasound imedis9000 was 100%, and the success rate of the conventional 2D ultrasound instrument was 95.7%. Furthermore, the results also showed that the novel 3D ultrasound imedis9000 and the matching coaxial positioning channel puncture needle had low pain, good toughness and strength, and great convenience.Conclusions: The new 3D multi-modal intelligent intervention system using electromagnetic navigation real-time positioning and ultrasound images has significant advantages compared with conventional 2D ultrasound in terms of puncture time, number of trials, operation difficulty, and convenience, and is worthy of further promotion and use in clinics.Trial Registration: Beijing Municipal Drug Administration, 20190015.

Abstract:INTRODUCTION Today, the "gold standard" of surgical management of benign thyroid disease is still controversy. There are different surgical approaches to the thyroid gland, from a radical, total thyroidectomy to a conservative treatment, lobectomy with or not isthmectomy. In consideration of the more frequent incidence of small carcinomas accidentally founded in the context of the thyroid parenchyma removed for another pathology, the aim of this study is to bring a contribute to resolve the debate on the therapeutic choice in the surgical management of the thyroid benign disease. PATIENTS AND METHODS From January 2000 to January 2006 502 thyroidectomy were performed in the Section of General and Thoracic Surgery of University of Palermo: 458 total thyroidectomy (91.3%), 2 partial thyroidectomy (0.3%), 24 (4.8%) lobectomy with isthmectomy and 18 (3.6%) reinterventions for relapse; 34 patients (6.8%) were preoperative suspected (cytological or ultrasonography) for thyroid cancer. RESULTS After total thyroidectomy in 17 patients (3.4%) we founded a papillary incidental "microcarcinoma", diameter range 2-10 mm (mean 6.9 mm). In 2 cases (11.7%) we found infiltrative and multifocal tendency. Histologically, were 11 cases of classical papillary (64.7%), 4 cases (23.6%) of follicular and 2 (11.7%) sclerosing, in 9 cases of multinodular goiter, 3 of follicular adenoma, 3 cases of follicular carcinoma, 1 case of Graves disease and 1 case of Hashimoto thyroiditis. DISCUSSION Papillary microcarcinoma is a "thyroid papillary cancer with a diameter < or = 1 cm?". From the analysis of literature, we observed a progressive increasing of papillary microcarcinoma from 12% (1980) to 25% (1990), with a prevalence of diameters < or = 5. Actually, there are no univocal data on natural history, nodal metastasis, biological and clinical behaviour, morbidity and mortality of these neoplasms; the certain data is that only the radical surgery can prevent relapse and incidentalomas that modify surgical approach. CONCLUSIONS In our opinion, the surgical management of the all thyroid disease must be the more radical since the first time, because we think other approaches not correct to improve the complete health from the benign thyroid disease and to prevent (secondary prevention) papillary microcarcinoma not pre-operative diagnosed, because there are no preoperative pattern to make a correct diagnosis of this tumour.

Abstract:The interest in the hydraulic architecture of trees, and the interplay between xylem anatomical characteristics and dimensions, its hydraulic conductance and water transport through the plant, stretches back to Leonardo da Vinci’s observations and Huber’s (1928) first systematic measurements of a constant area of conductive sapwood across the entire length of the tree and of a close correspondence with the amount of leaf area supported. Apart from the pure interest in the inner workings of plant self-organization, the study of hydraulic architecture has since demonstrated its role in key tree and ecosystem processes as a result of (1) the close association between gaseous-phase and liquid-phase conductances (i.e. between stomatal and hydraulic conductances, responsible for leaf water loss and replenishment, respectively), and important effects on CO2 availability and photosynthetic rates (Brodribb et al., 2005), (2) the requirements imposed by hydraulic constraints on growth allocation between tree parts, which affects both primary production (through allocation to transpiring foliage) and net ecosystem production (through allocation to shortlived, easily decomposed fine roots; Litton et al., 2007) and (3) xylem and foliage vulnerability to extreme events, whenever the limits imposed by plant hydraulic architecture and stomatal behaviour are exceeded, resulting in extreme tissue dehydration and foliage dieback (Martı́nezVilalta & Piñol, 2002). Several studies have demonstrated the variability of plant hydraulic architecture both between and within species, which is reflected in the huge variability in tree form and function across scales. It has been suggested that the observed differences could mirror the variability in environmental conditions experienced by different species and individuals, resulting in an optimal behaviour under the pressure of evolutionary processes (Magnani et al., 2002). In this issue of New Phytologist, Martı́nez-Vilalta et al., (pp. 353–364) explore the geographic pattern of hydraulic architecture in Pinus sylvestris, which, because of its wide natural range as well as its ecological and productive relevance, has been the subject of a number of related studies and can be therefore viewed as a de facto model species in forest tree functional and evolutionary ecology. Seen in the context of this large body of studies on the geographic and genetic variability of functional traits, the article provides not only new valuable information (in terms of parameters explored and their correlation and trade-offs), but also important hints of how evolutionary strategies could differ at the intraspecific and interspecific levels, and for different traits and processes. The authors did not find evidence at the intraspecific level of some of the associations and trade-offs between hydraulic traits that have been commonly reported across species. An interesting case in point is the observation of a lack of variability across the entire latitudinal range explored (from Finland to Spain and southern Italy) in xylem vulnerability to embolism, which is determined by tracheid fine anatomical features (Hacke & Jansen, 2009) and appears to determine the minimum water potential the plant can tolerate (Jacobsen et al., 2007). By contrast, large interspecific differences in vulnerability to xylem cavitation have been found among coniferous and other evergreen species (Maherali et al., 2004), which were related to mean annual precipitation. A similar pattern has been recently observed among a range of shrub species (Bhaskar et al., 2007), after accounting for phylogenetic effects. Can the limited variability reported here for P. sylvestris populations (and for P. ponderosa, described by Maherali & DeLucia, 2000) be extended to the entire genus, as suggested by Martı́nez-Vilalta et al. Abstract:BACKGROUND:Arterial cannulation is an important procedure for hemodynamic monitoring and blood sampling. Traditional radial artery cannulation is performed by using anatomical knowledge and pulse palpation as a guide. Arterial cannulation using ultrasound (US) requires specific training, especially for new US users. We hypothesized that even for new US users, US guidance would facilitate the successful puncture by lower attempts before successful intraluminal cannulation of a simulation model of the radial artery.METHODS:A prospective randomized controlled crossover study was conducted with new US users on a gelatin phantom wrist. Three sessions of training were proposed: US-guided technique with low blood pressure (BP), palpation-guided technique with high BP, and one secondary comparison with low BP. For the 2 first sessions, all volunteers performed each technique but not in the same order. The main criterion was the number of attempts before successful catheterization of the model artery. A secondary criterion was the number of needle movements (the number of attempts plus the number of needle directional changes).RESULTS:Twenty new US users participated in the study. Numbers of attempts before successful catheterization were significantly lower when using the US technique: 1.1 (± 0.4) for US versus 1.6 (± 0.8) for palpation high BP (P = .02) versus 2.5 (± 1.4) for the secondary comparison, palpation low BP (P < .001). All of the participants achieved success after the 12th needle movement for US technique, after the 19th needle movement for palpation high BP, and after the 25th needle movement for the secondary comparison, palpation low BP. The total time before success was not significantly different between the 2 first sequences (US vs palpation high BP).CONCLUSIONS:US technique was more successful than traditional palpation technique for novice US users performing arterial cannulations for the first time. A study in the clinical practice is needed to confirm these results.

Abstract:The Journal of Vascular Access, Ahead of Print. Introduction:Ultrasound-guided placement of Midlines catheters (MCs) is a standard procedure with many benefits for patients. Even if there are some guidelines worldwide, this invasive technique is still taught at the patient's bed and relies on mentoring in many care centers. The performance of this care by novice practitioners raises ethical and quality of care issues mainly because of its risk of complications. This study aimed to propose and assess a simulation-based learning method for the placement of MCs in novice practitioners.Methods:A single-center prospective observational study was conducted with anesthesia residents who had no prior experience of Midline placement. Two workshops were planned. The first one consisted of a theoretical training and a simulated practical phase. The second workshop included an assessment of theoretical memorization, a practical exercise and adherence to the training program.Results:The median score of the theoretical memorization was 14.6 (interquartile range [IQR]: 13.5–15.8). The MCs placement time was significantly higher (Med: 12.23 min; IQR: 12.21–12.80) for novice practitioners who did not successfully complete solo MCs placement in simulation versus novice practitioners who successfully completed solo MCs placement in simulation 6.66 min (IQR: 5.92–8.93) (p = 0.002). The number of attempts was significantly higher (p = 0.034) for the novice practitioners who did not successfully complete solo MCs placement in simulation with 67% having performed three punctures, against 0% for the novice practitioners who successfully completed solo MCs placement in simulation. All novice practitioners found this training model efficient for learning how to place MCs and considered it allows for reproducibility in care situations.Conclusion:This ultrasound-guided MCs training on simulation is an agile and fast alternative to traditional bedside training for anesthesia novice practitioners.

Team-based learning for teaching musculoskeletal ultrasound skills: a prospective randomised trial

Abstract:<h3 class="Heading">Objective</h3><p class="Para">The aim of this prospective randomised trial was to assess the impact of the team-based learning approach on basic musculoskeletal ultrasound skills in comparison to both peer-assisted and conventional teaching and to examine the influence of gender and learning style on learning outcomes.</p><h3 class="Heading">Methods</h3><p class="Para">In this prospective randomised trial, we randomly assigned 88 students to 3 groups: team-based learning (<em class="EmphasisTypeItalic">n</em> = 19), peer-assisted learning (<em class="EmphasisTypeItalic">n</em> = 36) and conventional teaching (<em class="EmphasisTypeItalic">n</em> = 33). Pre-existing knowledge was assessed using a multiple-choice (MC) exam. Student performance after completing the course was measured using an Objective Structured Clinical Examination (OSCE) and a second MC exam. Students were asked to complete Kolb's Learning Style Inventory and to evaluate the course.</p><h3 class="Heading">Results</h3><p class="Para">There was a significant gain in theoretical knowledge for all students (<em class="EmphasisTypeItalic">p</em> < 0.001). The team-based learning groups' performance proved to be significantly superior on the OSCE (<em class="EmphasisTypeItalic">p</em> = 0.001). As gender had no significant effect on practical or theoretical performance, learning style was linked to differences in the practical outcome. An evaluation showed overall satisfaction with the course and with the respective teaching methods.</p><h3 class="Heading">Conclusion</h3><p class="Para">Team-based learning proved to be superior to peer-assisted and conventional teaching of musculoskeletal ultrasound skills.</p>

Does Test-enhanced Learning Improve Success Rates of Ultrasound-guided Peripheral Intravenous Insertion? A Randomized Controlled Trial

Abstract:Introduction: Teaching procedural skills in medicine is time- and resource-intensive, and it is therefore important to determine which educational strategies are most effective. Test-enhanced learning (TEL) has been demonstrated to be effective in improving learner retention; however, there is little research evaluating the testing effect on the acquisition of procedural skills in medicine. The objective of this study was to examine the impact of TEL on learning ultrasound-guided intravenous (USG IV) insertion on a simulated model. Methods: We conducted a prospective randomized controlled trial of medical students at a single tertiary care academic hospital. Participants were randomized to either the TEL group (TEG) or control group (CG). Each group received an USG peripheral IV teaching session that included a didactic portion and hands-on skills training. The training sessions were identical except that the TEG was informed at the outset that there would be an assessment at the end of the session. The TEG then received a formal assessment of the skill during the last 15 minutes of the session, whereas the CG had continued practice time. Subjects in both groups were evaluated 10-14 days later to compare skill performance using a simulation-based assessment tool consisting of a global rating scale (GRS) and checklist items. Results: Thirty medical students completed the study, 15 in the TEG and 15 in the CG. There were no significant differences between the two groups at baseline based on year of medical training or prior IV or ultrasound experience. The overall procedural success rate was 93.3% (95% CI = 79.0%-100.0%) in the TEG and 80.0% (95% CI = 57.1%-100%) in the CG (p = 0.60). The first-attempt failure rate was 13.3% (95% CI = 0.0%-32.8%) in the TEG and 33.3% (95% CI = 6.3%-60.4%) in the CG (p = 0.39). There were no statistically significant differences between the means of the GRS (TEG = 4.7, CG = 4.2; p = 0.53, r = 0.11) or checklist scores (TEG = 78.6%, CG = 75.3%; p = 0.20, r = 0.24). Conclusions: In this study of novice learners, both TEL with structured practice and structured practice alone lead to high success rates performing USG IV insertion on a simulated model. While we noted a trend toward higher procedural success rates and lower first-attempt failure rates in the TEG, these did not meet statistical significance. Further studies with larger sample sizes are required to determine whether the beneficial effects of TEL can be transferred to procedural skills teaching.
