Rare Cardiac Myxoma with A Hard Texture: A Case Report

Abstract:Background: Primary tumors of the heart are uncommon but not rare. Myxoma comprises 50% of all benign cardiac tumors in adults and 15% of such tumors in children. Cardiac myxoma is usually soft in texture and has a friable or villous surface, which tends to be associated with embolic events. It is also rare for myxoma to be full of calcium and metastatic bone deposits.Case presentation: We herein present a 58-year-old female patient with cardiac space-occupying lesion within the left atrium, and the lesion is lubricous and hard. Her only symptoms were hypertension, dizziness, and chest stuffiness. After surgical resection, we confirmed that the lesion was atrial myxoma by histopathology examination. The patient recovered uneventfully and was discharged. A 2-year follow-up exam showed no evidence of tumor recurrence.Conclusions: Cardiac myxoma is rare and has various clinical manifestations. It is usually friable and has an irregular surface, making embolic manifestations one of the most common complications. Consequently, immediate surgical excision is suggested upon diagnosis. In our case, the patient had a rare type of cardiac myxoma, which had a hard texture and lubricous surface. In our opinion, this case has a low risk of embolism. We prefer elective surgery rather than emergency surgery to perfect the preoperative examination for this kind of patient.
Hibernoma--two patients with a rare lipoid soft-tissue tumour

Abstract:Background: Hibernomas are rare benign soft-tissue tumours arising from brown fat tissue. Although imaging characteristics are not specific certain imaging features, common locations and patient demographics may suggest hibernoma as a differential diagnosis. Case presentation: We report on two 48-year-old male patients with hibernoma. The tumour presented with local swelling of the inguinal region in the first patient and was an incidental imaging finding in the second patient. Imaging included magnetic resonance imaging in both patients and computed tomography as well as 18F-fluorodeoxyglucose positron emission tomography-computed tomography in the second patient. In both cases histological diagnosis was initially based on excisional and needle core biopsy, respectively. Complete surgical resection confirmed the diagnosis of hibernoma thereafter. Conclusion: In soft tissue tumours with fatty components hibernoma may be included into the differential diagnosis. Because of the risk of sampling errors in hibernoma-like tissue components of myxoid and well-differentiated liposarcoma, complete resection is mandatory. This article also reviews the current imaging literature of hibernomas.
A large thigh hibernoma in a 23-years-old man: Case report

Abstract:Introduction and importance: Hibernomas are benign soft tissue tumors containing prominent brown adipocytes that resemble normal brown fat, they occur in a wide age range (from 2 to 75 years) and make up for about 1 % of all adipocytic tumors <2 % of benign fatty neoplasms. They have traditionally been regarded as benign tumors with no potential for malignancy; due to their similarity in clinical and radiographical presentation, they are often misdiagnosed as malignant tumors like liposarcomas. While these tumors are generally considered non-malignant, their distinctive characteristics and uncommon occurrence make them an intriguing subject for medical study. Case presentation: This article describes the clinical, radiographic, and histologic features of a young patient with a thigh hibernoma compressing the obturator nerve. Clinical discussion: Hibernomas are more likely to develop in brown fat-enriched areas in newborns, such as the neck, scapular area, armpit, chest, and retroperitoneum. Increased awareness among clinicians and pathologists, coupled with advances in imaging and immunohistochemistry, has enhanced our ability to accurately identify and treat these intriguing tumors. Conclusion: The rarity of hibernomas together with the close resemblance of their radiological features to those of malignant tumors like liposarcomas or atypical lipomatous tumors, make these lesions extremely difficult to identify and often misdiagnosed. Continued research is essential to further our understanding this neoplasm and refine diagnostic and therapeutic approaches.
Primary non-myxomatous cardiac tumor: clinicopathologic analysis of 19 cases

Abstract:Objective:To investigate the clinicopathologic and diagnostic features of non-myxomatous cardiac tumors for improving the diagnostic level of the disease. Methods: Nineteen patients with pathologically confirmed primary non-myxomatous cardiac tumor during January 2009 to July 2016 at Ruijin Hospital were enrolled in this study. The clinical and pathologic features, imaging findings and immunophenotype of these cases were analyzed retrospectively and relevant literatures were reviewed. Results: Nine of the 19 cases were benign tumor (9/19), including 3 hemangioma, 3 lipoma, 2 papillary fibroelastoma and 1 schwannoma. Eight cases were malignant tumors (8/19), including 5 angiosarcoma, 1 malig-nant mesothelioma, 1 synovial sarcoma and 1 diffuse large B cell lymphoma.Two cases were intermediate tumors (2/19), including 1 fibromatosis and 1 composite hemangioendothelioma. Clinically, most of the patients presented with recurrent chest tightness and dyspnea. In patients with benign tumor, symptoms was mild or even absent and some patients were dis-covered during routine physical examination. Serious symptoms were seen in patients with malignant tumor. Conclusions:Primary cardiac non-myxomatous tumors are extremely rare,consisting various types of tumors and lacking specific clini-cal manifestations. The diagnosis of benign tumor is relatively easier. The malignant tumors are mainly sarcoma ,especially angiosarcoma. Other types of malignant tumors are rare and their histomorphology are of diversive state; the definite di-agnosis depends on histopathology and immunohistochemical examination and molecular detection.
Invasive Cardiac Lipoma: a case report and review of literature

Abstract:Background: Cardiac lipomas are rare benign tumors of the heart. They are usually asymptomatic and are thus most often diagnosed on autopsies. Symptoms, when present, depend upon the location within the heart. Typical locations are the endocardium of the right atrium and the left ventricle. Diagnostic modality of choice is cardiac MRI. Treatment guidelines have not yet been established due to the very low prevalence of these tumors and are thus guided by the patient's symptomatology. Case presentation: We describe a case of an invasive cardiac lipoma, wherein the initial symptom of the patient was shortness of breath. Although the echocardiogram visualized the tumor in the right atrium, a cardiac MRI was performed for better tissue characterization. The MRI revealed a large, fat containing, septated mass in the right atrium with invasion into the inter-atrial septum and inferior cavoatrial junction. There was also invasion of the coronary sinus along the inferior and left lateral aspect of the posterior atrioventricular groove. Although the mass appeared to represent a lipoma by imaging characteristics, the unusual extension into the coronary sinus led to consideration of a low-grade liposarcoma in the differential. Thus a pre-operative biopsy was performed along with MDM2 gene amplification to rule out a liposarcoma preceding surgical excision. Conclusion: Cardiac lipomas are well-characterized on cardiac MRI, which is the diagnostic modality of choice. Typical locations are the right atrium and the left ventricle. However, in those with atypical features such as invasion of the coronary sinus, pre-operative biopsy for histopathologic confirmation is imperative to exclude well-differentiated liposarcoma. Our patient with a simple lipoma underwent partial resection to relieve symptoms. We discuss prognosis and treatment of symptomatic cardiac lipomas.
Intraosseous hibernoma mimicking sclerotic bone metastasis-a case report

Abstract:Intraosseous hibernoma is an uncommon presentation of brown adipose tissue benign tumors. Imaging differential diagnoses include bone island, sclerotic metastasis, lymphoma, hemangioma, and sclerotic myeloma. In this report, a 72-year-old patient presented with right hip pain following a fall injury, leading to an extensive diagnostic workup. Initial CT of the pelvis without contrast suggested potential sclerotic metastatic disease. MRI findings could not be definitive. Further assessment with CT-guided biopsy and S-100 immunohistochemical staining confirmed a rare diagnosis of intraosseous hibernoma. This case describes multimodality imaging characteristics of a rare intraosseous hibernoma with discussion of imaging features of differential diagnostic considerations of related benign and malignant bone lesions.

Abstract:Background and Purpose. The difficult transtemporal ultrasound window is a relatively frequent occurrence. The authors assessed if the thickness of the temporal bone squama as measured in the “bone window” of the head computerized tomography (CT) scan can predict the transtemporal acoustic window. Methods. The authors retrospectively reviewed the head CTs on their bone window setting of patients in which nonimaging transcranial Dopplers (TCDs) had been performed. The thickness of the temporal squama in its thinnest portion was measured. The temporal TCD windows were graded in three classes: class 1 as good, class 2 when only a partial study is possible, and class 3 as an impossible ultrasonic window. In a case‐control design, for every patient with any class 2 and 3 TCD temporal window, a patient with a class 1 window was randomly included from the same time period. Results. Fifty‐five temporal bones (56%) were class 1, 17 (17%) were class 2, and 27 (27%) were class 3. Bone thicknesses (in mm, mean ± SD) were greater in those with poorer windows: class 1 = 2.67 ± 0.70, class 2 = 4.06 ± 0.56, and class 3 = 5.04 ± 1.06, P= .0001y Cusick's nonparametric test of trend. Temporal squama thickness of = 5 mm portends 86% sensitivity, 90% specificity,0% positive predictive value, and a positive likelihood ratio of 8.6 for a class 3 transtemporal ultrasound window. Conclusion. Measurement of temporal bone thickness on the bone window setting of the head CT scan may be useful in identifying patients who are poor candidates for transcranial ultrasound.
A huge pancreatic lipoma mimicking a well-differentiated liposarcoma: A case report and systematic literature review

Abstract:BACKGROUND Pancreatic lipomas are thought to be very rare. Lipomas are usually easy to identify on imaging, particularly via computed tomography (CT). But sometimes it's quite difficult to distinguish a lipoma from a well-liposarcoma without histologic result. CASE SUMMARY Here, we present a case of pancreatic lipoma in a 59-year-old female. She was asymptomatic and had no medical history of note. CT and magnetic resonance imaging revealed a mass like well-differentiated liposarcoma in the pancreatic head, positron emission tomography/CT showed a low fluorodeoxyglucose uptake and laboratory tests revealed elevated transaminase and carbohydrate antigen-199 levels. Finally, the patient underwent a pancreaticoduodenectomy. Histologically, mature adipocytes were noted in the bulk of the tumor. Accordingly, the pathologic diagnosis of the pancreatic neoplasm was lipoma. To our knowledge, this case is the first example of a suspected well-differentiated liposarcoma that was actually a pancreatic lipoma. We also highlight the radiological features distinguishing a pancreatic lipoma from a pancreatic liposarcoma and briefly review the literature. CONCLUSION Pancreatic lipomas show no obvious gender bias and most commonly occur in the head of the pancreas, of which the maximum diameters are often less than 5 cm, and small, asymptomatic non-compressed lipomas require follow-up only. Surgical excision should be considered when the tumor has compressed important tissues or is difficult to distinguish from a liposarcoma, the choice of surgery depends on the intraoperative presentation.
Adipose and myxoid tumors of childhood and adolescence

Abstract:Adipose and myxoid tumors in children are an unusual and challenging group of neoplasms that have some unique aspects in contrast to these tumors in adults. Less than 10% of soft tissue neoplasms in the 1st 2 decades of life have an adipose phenotype and most are benign. The most common are various types of lipoma and lipoblastoma. Liposarcoma in young patients is rare and has a distinctive distribution of histologic subtypes, including classic myxoid liposarcoma, and unusual variants, such as pleomorphic-myxoid liposarcoma. Pathologic examination enhanced by adjunct techniques, such as immunohistochemistry and cytogenetic or molecular genetic studies, is useful for classification of difficult cases. Myxoid tumors can overlap with adipose tumors and are included in this review because of the morphologic similarities and importance of diagnostic accuracy. This article reviews the clinicopathologic features of adipose and myxoid tumors with an emphasis on the unique aspects of these neoplasms in children and adolescents and the differential diagnosis.
Skin Prick Test with Self-Saliva in Patients with Oral Aphthoses: A New Diagnostic Pathergy for Behcet’s Disease and Recurrent Aphthosis

Abstract:There may be some difficulties to differentiate Behcet’s disease (BD), recurrent aphthosis (RA), and herpetic aphthous ulceration, from other mimicking oral disorders. Despite of unexpected sensitivity and responsiveness, the skin pathergy test regarding a non-specific hypersensitivity has long been thought as one of auxiliary diagnostic benefits for BD. To determine the potential usefulness and disease specificity of the prick reaction with saliva, a skin prick test with neat and filter-sterilized saliva was performed on the forearm skin of 26 individuals; 10 patients with BD (8 incomplete type without uveitis, 1 complete type, and 1 neurological type), 5 with RA, 3 with herpetic oral aphthosis, 2 with erythema nodosum alone, and 6 healthy controls. We assessed the skin reaction at 48 hours after pricking, and the pricked skin lesions were biopsied and analyzed immunohistologically. Nine of 10 BD patients (90 %) exhibited an indurative erythema at the skin site pricked with self-saliva, whereas 3 of 5 RA patients (60%) were relatively weak reaction. Pricking with filter-sterilized saliva failed to recapitulate any of positive skin reactions, albeit a faint erythematous dot appeared in a few BD patients, implicating the involvement of causative microorganism(s) in oral bacterial flora. Culture of saliva from 3 randomly chosen BD patients revealed numerous streptococcal colonies on Mitis-Salivarius agar. Histology of the pricked skin sites showed perivasucular inflammatory infiltrates, composed of CD4+ T cells and CD68+ monocyte/macrophage lineage, a feature consistent with a delayed type hypersensitive reaction. Our results suggested that skin prick test using self-saliva (a new diagnostic pathergy) can be a simple and valuable in vivo diagnostic approach for differentiating BD and RA from other mimicking mucocutaneous diseases. The positive skin prick may be triggered by resident intra-oral microflora, particularly streptococci, and may in part address the underlying immunopathology in BD.

Abstract:In this report we present the results of a study of the intellectual functioning and cognitive profile of 26 Prader‐Willi syndrome (PWS) patients. The mean IQ score was 62.3 (range 39–96). In 13 patients a significant difference between verbal and performance IQ was found. In 10 of them the performance IQ was higher than the verbal. The results of subtest analysis indicate that cognitive strengths are more visible than cognitive weaknesses. Highest scores were noted especially in the performance scale, i.e. Block Design (9 children) and Coding or Mazes (5 children). Analysis of all available data indicates that PWS patients score better on visual motor discrimination skills than on auditory verbal processing skills. These results are promising for intervention programs and education stiategies.

Abstract:ABSTRACT: Hibernomas are "pseudolipomas" originating from remnants of fetal brown adipose tissue. These rare benign tumors may occur throughout the body but most commonly in the thigh, shoulder, back, and neck, and are rarely found in the abdominal cavity, retroperitoneum, breast, bones, scrotum, and perirectum. We present a case of a 58-year-old woman with a known mediastinal mass, who was incidentally found to have a very FDG-avid fat-containing lesion in the omentum abutting the stomach. Subsequent endoscopic ultrasound-guided fine-needle aspiration confirmed hibernoma. The review of the literature shows the location is very unusual.

Abstract:Primary cardiac myxoid liposarcoma is an extremely rare disease. We reported a 61-year-old man with a 1-month history of aggressive dyspnea and anorexia due to a giant primary cardiac myxoid liposarcoma. Radical resection, whenever feasible, is considered to be an appropriate surgical strategy for this disease. Heart transplantation may be an alternative therapy for such isolated and unresectable tumor.

Abstract:Benign cardiac neoplasms are relatively uncommon. Cardiac leiomyomas are usually diagnosed as a benign metastasizing leiomyoma or as a part of intravenous leiomyomatosis spectrum. Primary cardiac leiomyomas are extremely rare and should be diagnosed after ruling out the involvement of systemic leiomyomas. Only nine cases were found in the literature that described De novo occurrence of primary intra-cardiac leiomyoma. In this study, we present a case of 60-year-old female patient with a large pedunculated mass located in the left ventricle. Histopathology examination and immunohistochemistry aid confirmed the diagnosis of benign leiomyoma. No evidence of extra cardiac lesions was detected in the patient. The patient remained healthy with no signs of recurrence four years after the surgical resection. Benign cardiac tumors are not often seen, but when they do occur, they can present a serious risk to life. This is particularly significant because these tumors can detach and cause embolization, leading to the development of strokes. Moreover, these individuals do not show any clinical symptoms, making their detection quite challenging. When there is a suspicion, it is advised to utilize echocardiography and other imaging techniques to verify the presence of a tumor. In this report, we present a rare case and provide differential diagnoses, along with a review of the literature.

Abstract:In the present report we describe the characteristics of 2 clones, E2 and C5, isolated from the human colon adenocarcinoma cell line LoVo. When grafted to immunosuppressed newborn rats, these clones formed tumors that varied with regard to differentiation rate, basement‐membrane organization and lung metastatic potential. Production and distribution of laminin by E2, C5 and related tumors was studied by immunohistochemistry with an anti‐laminin monoclonal antibody 4CI2 (MAb 4CI2). In lowly metastatic E2‐derived tumors, strong regular stainings were observed which were strictly peri‐tumoral and corresponded to the basal lamina. Since the antibody interacted with human laminin (the graft) but not with rat laminin (the host), this result indicated that basementmembrane laminin was supplied mainly by tumor‐cell synthesis. In highly metastatic C5‐derived tumors, the staining obtained with MAb 4CI2 was peri‐cellular and unorganized. Laminin synthesis by E2 and C5 cells in sub‐cultures or soon after dissociation from explanted tumors was studied by metabolic labelling with 35S‐methionine under steady‐state conditions followed by immunoprecipitation and SDS‐PAGE. High‐molecular‐weight laminin comprised by disulfide‐linked A and B chains, i.e., heterotrimeric laminin, was found in cell lysates and in the secretion medium of cell lines and tumor cells. In addition, BIB2 dimers and free B chains were observed in cell lysates. Quantitatively, Iaminin expression by E2 and C5 clones or tumor cells was not significantly different. These findings suggest that basementmembrane defects in invasive clone LoVo C5 were not due to laminin under‐expression.
Cardiac liposarcoma and bilateral ovarian mature teratoma in a patient with congenital heart disease and f-anisosplenia

Abstract:A 10-year-old female with atrial septal defect presented with bilateral ovarian mature cystic teratomas. Three spleniculi adjacent to the main spleen were noticed while the endoscopic surgery. Two years later a heart tumor was discovered which led to death. Pathologic examination of samples of the mass proved it to be a liposarcoma. The unusual combination of atrial septal defect, f-anisosplenia (female, congenital heart disease, multiple unevenly-sized spleens, left lung type pulmonary isomerism), bilateral ovarian mature cystic teratomas and liposarcoma of the heart appears to be a unique presentation.
Myxoid Lipoblastoma and Mimickers on Fine-Needle Biopsy in a Child

Abstract:Image-guided percutaneous core needle biopsy is a standard and safe procedure for the diagnosis of both solid and hematological malignancies in children. Despite recent improvements, nondiagnosis biopsies persist. Lipoblastoma is a benign adipocytic tumor composed of embryonal fat admixed with mature adipocytes and occurring before the age of 1 year in one-third of cases. Lipoblastoma is usually easily diagnosed, but in some cases, diagnosis may be difficult on percutaneous biopsies, when the lipoblastic component is not well represented or when the tumor contains a prominent myxoid component mimicking other myxoid tumors. We report here a case of lipoblastoma with a predominant myxoid component and discuss differential diagnosis of myxoid lesions of infancy. In such cases, pathologic examination enhanced by adjunct techniques, such as immunohistochemistry and cytogenetic or molecular genetic studies, is needed to achieve accurate diagnosis, particularly on fine-needle biopsies.
Rare primary cardiac myxofibrosarcoma in adolescence: diagnostic challenges and surgical insights.

Abstract:This article presents a unique case of primary myxofibrosarcoma, an exceptionally rare cardiac neoplasm, in a 16-year-old girl. With fewer than 40 documented cases, this tumor's occurrence in adolescence is particularly uncommon. The patient presented with right-sided paresthesia and subsequent right hemiparesis and dysarthria, along with bilateral pleural effusion. Imaging tests revealed a cerebral ischemic lesion and the presence of a left atrial mass. Initially suspected as a myxoma, the tumor exhibited significant infiltration. Despite prompt excision and cardiac intervention, the patient experienced rapid decompensation and succumbed to heart failure 2 days later. Histopathological analysis revealed an intermediate grade myxofibrosarcoma. This case underscores the diagnostic complexity, emphasizing the need for early identification given the grim prognosis associated with cardiac myxofibrosarcomas, requiring detailed imaging, surgical, and histopathological insights.
