Abstract:Objective To investigate the experience and value of retroperitoneal laparoscopic nephron-sparing excision.Methods Between September 2005 and May 2009,35 cases underwent the retroperitoneal laparoscopic nephron-sparing surgery.Results All procedures were technically successful.The mean operating time was 85 min,and mean ischemic time was 25 min.The mean estimated blood loss was 240 mL.The mean hospital stay after surgery was 9d.No intraoperative complications occurred.No local recurrence or trocar site metastasis was observed during a mean follow-up period of 1-30 months.Conclusion Retroperitoneal laparoscopic nephron-sparing surgery represents a feasible option for the patients with localized small renal tumors.It offers a new effective and minimally invasive treatment for selected patients.

Abstract:Objective To evaluate the clinical effect of retroperitoneal laparoscopic nephron-sparing surgery.Methods From March 2007 to May 2008,15 patients with local renal tumors underwent retroperitoneal laparoscopic partial nephrectomy by a single surgeon.Hilar control was achieved with bulldog clamps.Hook-cautery is used to circumscribe the tumor for resection by scoring the renal capsule circumferentially with a 2-10 mm margin around the tumor.Specimens are randomly extracted through tumor edges and immediately given to pathological frozen sections.The operative time,blood loss,postoperative stay,complications and the time of complete convalescence were recorded.Results Mean operative time was 110 minutes and mean time of hilar control was 26 minutes.Mean blood loss was 200ml.Mean postoperative stay was 9 days.Final pathological evaluation revealed negative positive margins in these cases.During 2-14 months follow-up there was no local recurrence.Conclusions Retroperitoneal laparoscopic partial nephrectomy for small renal tumors can be performed safely and effectively.Extended follow-up period is required to determine its long-term functional and oncological outcomes.

Abstract:OBJECTIVE: To report the outcome of nephron-sparing surgery (NSS) for renal cell carcinoma (RCC) with elective and imperative indications, with analysis of complications, renal function and long-term oncological follow-up.MATERIAL AND METHODS: We analyzed 91 patients undergoing NSS for RCC, 26 with imperative and 65 with elective indications. Variables analyzed included patient age and gender, comorbidities per patient and preoperative renal function. Pathological features included tumor size, WHO 2002 stage and grade. Peri- and postoperative data included complication and re-intervention rates, tumor recurrence and the mortality rate.RESULTS: RCC was found in 81.3% of cases in the imperative group and in 73.9% in the elective group. Median tumor size was 3.2 and 2.5 cm (p = 0.02) in the imperative and elective groups, respectively. Operation time was longer in the imperative group (p < 0.01). There was no difference between the two groups regarding intraoperative complication rate (p = 0.37), whereas postoperative complications occurred more frequently in the imperative group (p = 0.02). The median follow-up period was 48 months for both groups. Mean serum creatinine during follow-up was significantly higher in the imperative group (238.4 vs 97.2 Micromol/L). However, no patient required hemodialysis. There was no local recurrence or tumor-specific death in either group.CONCLUSIONS: NSS for RCC is a reasonable treatment option for patients with an imperative indication as it prevents the need for hemodialysis as well as ensuring excellent oncological follow-up. A higher postoperative complication rate has to be balanced against improved quality of life.

Abstract:OBJECTIVE: To report our experience in managing various benign and malignant renal tumors with nephron-sparing surgery.MATERIALS AND METHODS: Records of patients who underwent nephron-sparing surgery (NSS) either through open or laparoscopic approach between May 1997 and June 2006 at our institution were reviewed. Patient and tumor-related characteristics, treatment modality and complications were noted.RESULTS: There were 26 patients (29 renal units), including three with bilateral lesions who underwent nephron-sparing surgery. Mean age at surgery was 47.0 years (range 16-67 years). Mean tumor size was 4.7 cm (range 2-7.5 cm). Mean warm ischemia time was 41 min and 32.5 min, operative time 158 min and 186 min and blood loss 200ml and 85 ml in open (n=24) and laparoscopic approach (n=2) respectively. Complications were seen in five (19.2%) patients of whom two had postoperative bleeding requiring nephrectomy in one and angioembolization in another. One patient with persistent urinary leak required intervention. Local wound infection in one patient and incisional hernia in another were surgically managed. Histopathological profile revealed 13 (44.8%) benign lesions which included angiomyolipoma (eight), simple cyst (two), cortical adenoma (one), metanephric adenoma (one) and myelolipoma (one). The remaining 16 (55.2%) malignant lesions included renal cell carcinoma (15) and metastatic adenocarcinoma (one). At a mean follow-up of 38.6 months (range 1-91) no patient had local recurrence or distant metastasis. Cancer-specific survival was 100% and overall survival was 92.3%.CONCLUSIONS: Nephron-sparing surgery is a safe and effective alternative to nephrectomy in both benign and malignant lesions of the kidney.
Surgical Approach for Partial Nephrectomy in the Management of Small Renal Masses: A Systematic Review and Network Meta-Analysis

Abstract:Background: A variety of surgical and nonsurgical management options for small renal masses (SRMs) now exist. Surgery in the form of partial nephrectomy (PN) has three different approaches. It is unclear which PN approach, if any, offers superior clinical outcomes. Aim: The aim of this study is to compare outcomes in patients with SRMs <4 cm undergoing PN through the open partial nephrectomy (OPN), laparoscopic partial nephrectomy (LPN), or robotic partial nephrectomy (RPN) approach and to establish the advantages and disadvantages of the various approaches. Methods: A systematic literature search was conducted for studies comparing at least two of the above techniques. Eighteen studies and 17,013 patients were included in our study. A network meta-analysis with a frequentist framework was performed. OPN was used as the baseline comparator. The prespecified primary outcome was R0 resection rates. Secondary outcomes included operating time, ischemia time, blood loss, transfusion rates, urine leak rates, significant morbidity, length of stay, and recurrence. Results: There was no significant difference between the techniques in terms of R0 rates, tumor recurrence, urine leak rates, renal function, and >3a Clavien-Dindo complications. LPN had a longer ischemic time and operating time. OPN had a longer length of stay and higher average intraoperative blood loss. RPN had lower blood transfusion rates. Discussion: All approaches are acceptable from an oncological perspective. The minimally invasive approaches (i.e., RPN and LPN) offer advantages in terms of morbidity; however, LPN may increase ischemic time and operative duration. Variations between perioperative outcomes may influence the choice of approach on a case-by-case and institutional basis.

Reoperation for ipsilateral local recurrence following prior nephron-sparing surgery: innovative surgical insights from a high-volume urological center with cross-sectional study

Abstract:Background: The main aim of this study was to examine the perioperative results of reoperations and suggest novel surgical approaches. Based on a substantial number of robotic and laparoscopic nephron-sparing surgery (NSS), we aim to propose novel surgical strategies that offer practical recommendations to surgeons. Methods: Renal cell carcinoma patients with ipsilateral recurrent tumors, without evidence of metastasis, and who underwent primary NSS at our center between 2013 and 2023 were enrolled in this study, and all received the second time surgery. We conducted an analysis to evaluate perioperative outcomes and observed trends over a decade. Additionally, based on the findings from this study, we developed our surgical strategies. Results: In the past decade, our center has successfully conducted a total of 2546 surgeries for renal cell carcinoma, out of which this study includes 15 patients who met the specified criteria. For reoperation, robotic-assisted surgery was applied in 5 cases (33.3%), laparoscopic surgery in 6 cases (40%), and open surgeries in 4 cases (26.7%). While 4 (26.7%) patients underwent NSS while radical nephrectomy was performed on 11 patients (73.3%). The median operative time was 215 minutes (IQR: 135-300), and the median estimated blood loss was 50 ml (IQR: 50-100). The median length of postoperative hospitalization was 6 days (IQR: 5-9). Furthermore, there has been a yearly increase in the application of robotic-assisted NSS at our institution. Conclusion: Reoperation following the pNSS is a secure and effective surgical approach. We introduce novel surgical strategies for primary surgery and reoperation, which offer valuable insights to surgeons in current study.

Abstract:Background: The enhanced recovery after surgery (ERAS) program is focused on improving surgical outcomes and enhancing the patient experience before, during and after surgery. We performed this study to evaluate the effect of ERAS in perioperative period of laparoscopic nephron sparing surgery (LNSS). Methods: A retrospective analysis of 287 consecutive patients with localized renal cell carcinoma (RCC) who underwent LNSS from December 2015 to June 2017 was conducted. Our study design included two cohorts. Patients underwent conventional care in one group (n=136), and ERAS protocol in the other group (n=151). The data of recovery of gastrointestinal function, catheter and drainage tube removal time, length of stay (LOS), hospitalization expenses and incidence of postoperative complications were compared between the two groups. Results: Compared with conventional group, cases in ERAS group presented with shorter time of recovery of gastrointestinal function (P<0.05) and LOS (P<0.05), shorter catheter and drainage tube removal time (P<0.05), lower hospitalization expenses and lower perioperative complications (P<0.05). Conclusions: Program of ERAS used in perioperative period of LNSS can apparently accelerate the time of postoperative recovery, reduce complications, shorten time stay in hospital and cut down the total cost, finally improve patients' satisfaction.
Comparison of the Postoperative Renal Function Between Nephron-Sparing Surgery and Radical Nephrectomy

Abstract:Objective To compare the changes of renal function in patients with early localized renal cell carcinomaafter nephron sparing surgery and radical nephrectomy, and to explore the guiding role of different renal function indexes in clinical practice. Methods Clinical data and postoperative follow-up data of 63 patients with early renal tumor who received treatment in Nanjing Gulou Hospital between June 2013 and October 2014 were analyzed retrospectively. A total of 35 patients underwent nephron sparing surgery (Group NSS); 28 patients underwent radical nephrectomy (Group RN), and all the patients were followed up for two years. In addition, serum creatinine, uric acid, estimated glomerular filtration rate, cystatin C, urinary microalbumin and urinary β2microglobulin of these patients were collected and recorded during the follow-up period. Results Renal function of the 63 patients changed in different degrees after operation, and damaged more obviously in the early stage. With the extension of follow-up length, renal function of most patients could recover gradually. The incidence rate of postoperative acute renal insufficiency was 34%in Group NSS and 79% in Group RN, which presented statistically significant differences (P<0.01). The overall postoperative follow-up results showed there were statistically significant differencesin changes of estimated glomerular filtration rate, cystatin C and β2microglobulin (P<0.05), but no statistically significant differences in serum creatinine, uric acid and urinary microalbumin. Although levels of uric acid and microalbumin presented no statistically significant differences between the two groups, the average level in Group NSS was lower than that of Group RN. Early detection results of postoperative renal dysfunction showed the positive rate of independent detection of all indexes was low, especially the serum creatinine, uric acid, estimated glomerular filtration rate and other traditional renal function indexes. When the indexes were combined, the positive rate of renal dysfunction was significantly improved, and the combined positive rate was 37.1% and 71.4% in Group NSS and Group RN, respectively. Conclusion Nephron sparing surgery causes less damage to renal function in patients with early localized renal tumors than that of radical nephrectomy. Cystatin C, urinary microalbumin and urinaryβ2microglobulin are more sensitive to early detection ofpostoperative renal dysfunction, and may play a synergistic role with creatinine, uric acid and estimated glomerular filtration rate.
Surgical Options for Localized Renal Cell Carcinoma, A Retrospective Clinical Study Based on Tumor Size

Abstract:Abstract PurposeAlthough many studies have explored the options of radical nephrectomy (RN) and nephron sparing surgery (NSS) for localized renal cell carcinoma (RCC), the answer to this question remains unclear. This study aims to compare the long-term prognostic differences between RN and NSS among different sizes of localized RCC.MethodsThis study retrospectively included 80,439 T1-T4 N0 M0 patients who underwent RN or NSS based on the Surveillance, Epidemiology, and End Results database. We calculated the 10-year overall survival (OS) and cancer specific survival of patients with RCC. We also evaluated the risk of cardiovascular death in patients using competing risk models for RN and NSS.ResultsOur analysis showed that patients who underwent NSS had a more prolonged OS of 5 and 10 years when the tumor size was less than 8.5cm and 7.2cm. Compared to RN, NSS does not appear to improve OS in large (> 7.2cm) RCC patients. And stratified analysis showed that NSS for RCC less than 9.2cm may be more likely to benefit from long-term OS in younger patients (<60 years), while RCC above 7.3cm may be more suitable for RN in older patients (>=60 years). The gender-stratified results suggested male and female patients may be more suitable for NSS for RCC below 6.4 and 7.7cm, respectively. Besides, competing risk models showed patients receiving RN have higher cumulative cardiovascular mortality.ConclusionsFor large RCC, NSS may be very carefully selected unless there are clear indications such as isolated congenital kidney and bilateral kidney cancer.
A “3s+f” Nephrometry Score System to Predict the Clinical Outcomes of Laparoscopic Nephron-Sparing Surgery

Abstract:BackgroundWhen we treat renal cell carcinoma by laparoscopic nephron-sparing surgery (NSS), it is essential to use an evaluation system to predict clinical outcomes. Hitherto, there are more than a dozen nephrometry score systems. In this study, through assessing the correlations between nephrometry score systems and clinical outcomes, we aim to provide a novel nephrometry score system—the “3S+f” score system—to simplify the evaluation of technical complexity of partial nephrectomy.MethodsWe retrospectively collected the data of 131 patients who underwent NSS, which was performed by a single surgeon (SZ) from January 2013 to July 2018 at Peking University Third Hospital. The “3S+f” score system contains four parameters: “size, side, site, and fat”, all of which can be obtained from preoperative imaging data. We evaluated the correlations between the “3S+f” score and clinical outcomes, and compared R.E.N.A.L. score and PADUA score.ResultsAll the three nephrometry score systems were related to some clinical outcomes in univariate analyses. In multivariate regression models, the “3S+f” score, the R.E.N.A.L. score, and the PADUA score were significantly associated with operative time (p = 0.016, p = 0.035, and p = 0.001, respectively) and warm ischemia time (all p = 0.008, p < 0.001, and p < 0.001, respectively). “3S+f” was also significantly related to extubation time > 5 days (p = 0.018). In predicting operative time > 120 min and extubation time >5 days from ROC curves, the AUCs of the “3S+f” score (0.717 and 0.652, respectively) were larger than both the R.E.N.A.L (0.598 and 0.554, respectively) and PADUA (0.600 and 0.542, respectively) score systems.ConclusionA novel nephrometry score system—the “3S+f” score system—shows equivalent correlation and the ability in predicting clinical outcomes when compared to the R.E.N.A.L. score system and the PADUA score system, which can describe renal tumors.

Abstract:Capsule endoscopy is a new technique for the study of small intestine particularly in cases of obscure gastro-intestinal bleeding or chronic iron-deficiency anemia. In our study we present a case of 62 years-old man with a chronic iron-deficiency anaemia developed at least six months before and refractory to iron therapy. The patient was submitted to upper gastro-intestinal endoscopy and to partial colonscopy due to abdominal adhesions for previous abdominal surgery. A capsule endoscopy was indicated to achieve a definitive diagnosis. Wich discosed an intestinal obstruction. So far a surgical resolution was indicated. The surgery revealed a stricturing neoplasia of the cecum in which the capsule endoscopy was retained.

Abstract:Introduction: The aim of our study was to examine the influence of tumor complexity and operative variables on the degree and rate of post-nephron sparing surgery (NSS) acute kidney injury (AKI). Methods: We retrospectively reviewed the records of 477 patients who underwent NSS for enhancing renal masses in our institution. AKI was determined using the latest definition by AKIN and RIFLE criteria. Serum creatinine was assessed daily starting from day 1 post-surgery and until discharge (usually on postoperative day 3). Estimated glomerular filtration was determined using the Modification of Diet in Renal Disease equation. Results: Overall, 191 patients (40%) developed postoperative AKI, and most of them (88%) were grade 1. Multivariate analysis revealed that the most significant and independent variables associated with AKI were operation time (p = 0.02), ischemia time (p = 0.02), and the use of tissue adhesive for tumor bed closure (p = 0.02). Other important factors (by univariate analysis) were the need for blood transfusion (p = 0.003) and estimated blood loss (p = 0.007). Conclusions: Operative time, ischemia, and tumor bed closure method are independent predictors of post-NSS AKI. Efforts should be made to limit prolonged ischemia and to reduce viable parenchymal loss. Further studies concerning the functional impact of AKI in these patients will be required.
Endoscopic Robot-assisted Simple Enucleation Versus Laparoscopic Simple Enucleation with Single-layer Renorrhaphy in Localized Renal Tumors: A Propensity Score-matched Analysis from a High-volume Centre.

Abstract:OBJECTIVE To compare perioperative results and early oncological outcomes of endoscopic robot-assisted simple enucleation (ERASE) and laparoscopic simple enucleation (LSE) by using a propensity score-matched analysis. METHODS We evaluated 383 patients who underwent transperitoneal ERASE or LSE for renal tumors from November 2012 to October 2016. Propensity score matching was performed on age, gender, body mass index, Eastern Cooperative Oncology Group score, tumor side and size, preoperative estimated GFR and PADUA score. RESULTS In total, 278 and 105 patients underwent ERASE and LSE, respectively. The PADUA score was >= 10 for 61 (21.9%) and 13 (12.4%), respectively (P = .034). After matching, mean operative time and warm ischemic time were significantly lower with ERASE than LSE (171.9 vs 188.2 minutes; P = 0.016 and 20.9 vs 24.2 minutes; P = .001). The estimated mean blood loss was similar (167.7 vs 183.3 mL; P = .315). The conversion rate to open surgery or radical nephrectomy was similar with ERASE and LSE (1.0% vs 5.0%, P = .214) and the rate of intraoperative complications was lower (2.0% vs 8.9%, P = .030). The overall incidence of positive surgical margins was similar (P = .614). The median follow-up was less for ERASE than LSE patients (22 vs 38 months). Recurrence did not differ between the 2 groups: 2 ERASE cases (2.0%) versus 4 LSE cases (4.0%) (P = .679). CONCLUSION ERASE is a safe and acceptable alternative to LSE. ERASE appears to confer shorter operative time, shorter warm ischemic time and lower rate of intraoperative complication. (C) 2018 Elsevier Inc.

Abstract:Objective To explore the experience of laparoscopic nephron sparing surgery (LNSS) in treatment of endophytic renal tumors.Methods The clinical data of 26 patients (23 male and 3 female),aged from 29 to 73,with central endogenous renal tumors which underwent LNSS between December 2013 and August 2016 were studied retrospectively,with 19 tumors in the right kidney and 7 in the left.Three cases were accompanied with renal insufficiency.The level of pre-operative serum creatinine was 56.4-127 μmol/L,averaged 79.1 μmol/L.Maximum diameters of the tumors were 1.5-4.6 cm,averaged 2.9 cm.subtypes of the tumors were classified by Computed Tomography Angiography (CTA) and 3-Dimensional Imaging before operation,including 13 pre-pedicle,5 post-pedicle and 8 sinus central cases.All of 26 patients underwent RLPN via deliberately designed accesses and using special suture techniques according to different central endogenous subtypes.Icy saline was introduced to para-renal space and intrarenal pelvis following renal artery blockage in order to reducing ischemic damage to kidneys.Results The tumors were successfully removed by RLPN.Median operation time was 130 min (55-310 min).Median duration of renal artery clamping was 28min (13-39 min).Average blood loss during the operation was 52.4ml (20-370 ml).Time of removal of the drainage tube was 3-12 d (median 5 d).Postoperative hospital-stay duration was 3-26 d (median 9 d).Two patients showed postoperative complications with severe hemorrhage,controlled by interventional artery embolization.All patients were followed up for 1 to 32 months (median 18 months) by 3 month interval.There was no significant change in the serum creatinine level 3 months after the operation compared with that of pre-operation.Conclusion LNSS for the central endogenous renal tumors is technologically safe and feasible.
Propensity-matched Pair Analysis of Safety and Efficacy Between Laparoscopic and Open Radical Nephrectomy for the Treatment of Large Renal Masses (>10 Cm): a Retrospective Cohort Study

Abstract:Background:Open radical nephrectomy (ORN) is a practical procedure for treating patients with large renal carcinomas >10 cm in size, and few studies have focused on feasibility and safety of laparoscopic radical nephrectomy (LRN). The current study was to assess the safety and effectiveness of LRN and ORN in large renal carcinoma patients by propensity matched pair analysis.Methods:In this cohort study, a retrospective review of radical nephrectomy data from October 2010 to October 2018 at Changhai Hospital was conducted. Patients with renal carcinomas >10 cm in size by pre-operative images were included. Patients' demographics including age, gender, body mass index (BMI), tumor size, operation time, hospitalization days, etc. were collected. Renal tumor patients undergoing LRN or ORN were match-paired by gender, BMI, age, and tumor size. Peri-operative outcomes including estimated blood loss and complications were compared. The follow-up contents included survival time, disease progression, and cause of death, and cancer-specific and progression-free survival were estimated via Kaplan-Meier curve analysis.Results:Among 92 patients with clinical T2b renal masses, 37 pairs were matched. The average tumor sizes of the LRN and ORN groups were 11.37±0.30 and 11.67±0.33 cm (P=0.375), respectively. The average operating time for LRN was slightly longer (204.32±11.17 vs. 192.78±8.50 min, P=0.414). Estimated blood loss (EBL) (336.49±63.58 mL for LRN vs. 545.95±74.52 mL for ORN, P=0.036), the length of postoperative stay [6.0 (5.0-9.0) for LRN vs. 9.0 (6.0-11.5) days for ORN, P=0.015], and removal time of the drainage tube [4.0 (3.0-5.0) days for LRN vs. 5.0 (4.0-6.0) for ORN, P<0.001] were less than in the LRN group. The pathological subtype and Fuhrman grade were comparable. Both groups were followed up for a similar period, and no difference was observed in 5-year survival rates.Conclusions:Considering the conversion rates and overall complication rates, it seems that LRN for large renal carcinomas demonstrated equivalent peri-operative safety and effectiveness compared with ORN, with no adverse effects on midterm oncological outcomes.
Efficacy of Laparoscopic Nephron-Sparing Surgery Without Renal Wound Suturing

Abstract:Objective To evaluate the feasibility and clinical efficacy of laparoscopic nephron-sparing surgery without renal wound suturing.Methods From December 2012 to February 2014,9 patients (4 male,5 female) with renal tumor underwent laparoscopic nephron-sparing surgery without suturing in our hospital.The mean age in those patients was 56 years (range 42-72 years).The mass size ranged from 1.1to 3.8 cm (mean 2.4 cm).The location of tumor included 2 in left kidney,7 in right kidney.Meanwhile,6 tumors were found in the middle part of kidney,2 tumors were demonstrated in the lower part of kidney,and 1 tumor located in the upper part of kidney.The imaging examination confirmed that all tumors protruded the renal surface and infiltrated into the renal parenchyma about 0.1-1.2 cm,which could be defined as the peripheral renal tumor without the invasion of collective system.The retroperitoneal approach was used in all patients with 3 trocars.During the operation,the renal artery was exposed and fats surrounding or on the surface of the tumor were resected.The renal artery was occluded.Tumor excision was then performed using scissors.An incision was made at the point 0.5 cm away from the margin of the tumor,extending deeply to completely cut the tumor.Meanwhile,bleeding was controlled with bipolar diathermy.Then,the clamping of the renal artery was removed and the retroperitoneal pressure was reduced in order to check the site of bleeding.If the bleeding occurred severely,the kidney wound was sutured with 2-0 absorbable sutures.Otherwise,the coagulative glue was sprayed on the wound surface.Results In all the patients,the procedures were done without conversion to open surgeries.The renal wound suturing appeared in one case because the tumor infiltrated into the deep of the kidney and the bleeding could not be completely controlled by bipolar diathermy.Eight of 9 cases were successfully performed without suturing.The mean time of renal artery occlusion was 8 min (range 3-18 min).In our patients,the mean operation time was 120 min (90-180 min),and the mean blood loss was 60 ml (10-200 ml).None of the patients received blood transfusion during the surgery.The mean postoperative hospital stay was 7 days (5-9 days).No complications,such as urine leakage,bleeding occurred after the operation.All the patients were followed up with no recurrence of tumor.Conclusions Laparoscopic nephron-sparing surgery without renal wound suturing is feasible and safe for patients with the small and the depth of infiltration less than 0.5 cm tumor in the kidney.

Abstract:Objective To report our experience of nephron sparing surgery (NSS) with mini-incision approach.Methods Sixty-one mini-incision NSS were analyzed retrospectively from 2005 to 2011 in our department.Demographic and clinical data,complications,histologic and oncologic data were analyzed.There were 35 males and 26 females with average age of 41 years.B ultrasound and CT scan were performed in all cases and MR was performed in 23 cases.CT angiography was performed in 10 cases.Renal cell cancer was diagnosed in 38 cases,angiomyolipoma in 14 cases,complicated renal cyst or duplicated kidney in 6 cases and no definite diagnosis in 3 cases.All the lesions were single with 21 in the right side and 40 in the left side.The average diameter of the lesions was 3.2 (2.3-8.8) cm.The mini-flank supra-12th rib incision was performed in all cases.The renal artery was clamped with bulldog clamp,all transected blood vessels on the renal incisal surface were sutured with 3-0 Vicryl sutures,and collecting system was closed with continuous' 3-0 Vicryl suture.Results The average length of incision was 8.1 (6.5-11.0) cm.The average operative time was 93 (70-140) min.The average warm ischemia time was 19 (12-32) min.The average blood loss was 72 (20-300) ml and the average hospital stay was 6 (5-15) d.The delayed hemorrhage was occurred in 1 patient who was treated with conservative treatment.Clear cell carcinoma was found in 28 cases,chromophobe carcinoma was found in 6 cases,papillary carcinoma was found in 5 cases,angiomyolipoma was found in 16 cases,complicated cyst was found in 3 cases and renal duplication was found in 3 cases.Fortyfive patients were followed up from 28 to 82 months with no delayed complication and no recurrence or metastasis in 35 malignant cases.Conclusions The approach of mini-incision NSS can duplicate the procedure of traditional open NSS.This approach can also couple the benefits of both laparoscopic and open NSS techniques with a smaller incision which can be used to remove the specimen.
Nephron-sparing Surgery Versus Radical Nephroureterectomy in the Treatment of Organ-localized Upper Urinary Tract Urothelial Carcinoma: A Population-based Study of 1581 Patients

Abstract:Abstract Purpose: Nephron-sparing surgeries (NSS) have been widely used for patients with low-risk upper tract urothelial carcinoma (UTUC). We compared the prognosis of patients with organ-localized UTUC after NSS and radical nephroureterectomy (RN) through a population-based study and a propensity score matching (PSM) method. We further developed prognostic models for these patients using the Cox analysis and the competing-risk models.Materials and methods: The follow-up dataset of 1580 patients with organ-localized UTUC were screened out from the Surveillance, Epidemiology, and End Results (SEER) registry. A PSM process with a caliper width of 0.05 was performed to match the patients accepting NSS with those accepting RN. The Cox analysis was used for overall survival (OS) and competing-risks models based on Fine-Gray regression for cancer-specific survival (CSS). A forward stepwise selection process was applied to select independent predictors for OS and CSS. After selecting the predictors, two nomograms were developed based on the Cox analysis and the competing-risk model and internal validations were performed using 1000 bootstrap resamples.Results: Before PSM, RN was associated with better OS compared with NSS in the overall cohort (5-year OS rate: 65.1% vs. 55.6%, p<0.001) and the patients with renal pelvic lesions (5-year OS rate: 68.6% vs. 52.8%, p=0.001). Baseline variables were well balanced using the PSM method. After PSM, there was no significant difference in OS between the RN and NSS groups. Competing-risks models revealed that NSS was associated with poorer CSS compared with RN in the post-PSM cohort (n=766; subdistribution hazard ratio/ SHR=1.39, 95% confidence interval/ CI 1.03-1.89, p=0.033) and in patients with ureteral lesions from the post-PSM cohort. Four independent predictors, which were age at diagnosis, AJCC T stage, histologic grade, and surgery methods, were selected out for OS and CSS through forward stepwise selection methods. Two nomograms for OS and CSS were respectively developed. Conclusions: Patients accepting RN had a comparative or better outcome in each sample group. NSS achieved a similar prognosis for selective patients with organ-localized UTUC. Two nomograms were developed to predict OS and CSS with C-indexes of 0.672 and 0.643.

Abstract:OBJECTIVE:To compare the safety and efficacy of laparoscopic and open partial nephrectomy for small renal cell carcinoma. METHODS:To collect and analyze the data such as operation time, warm ischemia time, complications, recovery and surgical margins of 110 patients of small renal carcinoma (T1aN0M0) from January 2004 to March 2009, 52 of which underwent laparoscopic partial nephrectomy (LPN) and the other 58 patients underwent open partial nephrectomy (OPN). RESULTS:The mean operation time of LPN group and OPN group were 177.8 min and 126.7 min (t = 3.973, P < 0.01), respectively. The warm ischemia time of the two groups were 28.3 min and 21.9 min (t = 4.627, P > 0.05), respectively. Two cases in LPN group and 1 case in OPN group (3.8% vs 1.7%, chi(2) = 0.010, P > 0.05) needed blood transfusion. The increment of creatine after operation were 4 micromol/L in LPN group and 6 micromol/L in OPN group (t = -2.018, P > 0.05). Six cases (11.5%) in LPN group and 8 cases (13.8%) in OPN group needed collection system repairing (chi(2) = 0.130, P > 0.05)and the same data of hematuria after operation was observed. There was no urinary fistula or other severe complications in all patients. The hospital stay after operation was 10.6 d and 12.9 d (t = -3.244, P < 0.01) in the two groups, respectively. All surgical margins were negative. CONCLUSION:LPN and OPN have the same safety and efficacy, LPN primary treatment can be used for selected patients of T1aN0M0 renal cell carcinoma because of its fast recovery.
