Abstract:Skull defect is one of the major sequelae of traumatic brain injury and decompressive craniectomy,which affects the prognosis of neurological function.Cranioplasty is the main method to treat skull defect.The surgical technique is relatively simple and has been widely performed in neurosurgical departments.Although the optimal timing of cranioplasty is still controversial,the author advocates early cranioplasty when the patent's condition permits.Different cranioplasty materials have their own advantages and disadvantages,so we should choose appropriate materials according to the specific conditions of patients.Full preoperative evaluation,reasonable operation and proper postoperative management are important means to prevent complications related to cranioplasty.In view of these problems,the author reviews the latest literature and progress in order to provide reference for standardizing clinical treatment.
Cranioplasty for patients developing large cranial defects combined with post-traumatic hydrocephalus after head trauma.

Abstract:BACKGROUND:Large cranial defects combined with hydrocephalus after decompressive craniectomy are a common, harsh reality among patients with head trauma. Typically, a shunt is first used to relieve the hydrocephalus. However, subsequently the patients may develop a severe sinking scalp flap over the skull defect before cranioplasty, which would make the procedure difficult. METHODS:This problem was overcome by temporarily adjusting the shunt pressure using a programmable ventriculoperitoneal shunt tube, which allowed expansion of the depressed scalp flap and facilitated the subsequent cranioplasty. This study describes two patients who were treated for this problem after severe head trauma. RESULTS:When performing a titanium mesh cranioplasty after a shunt, this new method facilitated the separation of the scalp from the underlying muscle or dura and obliterated the dead space between the titanium mesh and the underlying tissue. Both patients had satisfactory outcomes without complications. CONCLUSIONS:This method is easy and safe and it facilitates the cranioplasty, reducing the potential complications, including intracranial haematoma, effusions and infection, and thereby improving the patient outcome.
Timing and material for cranioplasty in patients with posttraumatic skull defects

Abstract:Skull defect is commonly seen after traumatic brain injury and is often caused by decompressive craniectomy. Despite from pressure induced deformation or translocation, traumatic skull defect can also lead to multiple pathological influences to the brain, including cerebral blood flow or metabolism reduction, cerebrospinal fluid circulation disturbance, contralateral hematoma or effusion, and cerebral electrical activity changes. Cranioplasty can improve cerebral perfusion and metabolism as well as to support the rectification of cerebrospinal fluid circulation. Currently, early cranioplasty has been well accepted by neurosurgeons for the treatment of skull defect after decompressive surgery for traumatic brain injury, but the exact time point is still under debate and requires further clinical evidence. Therefore, the authors review the research progress of timing and material choices for cranioplasty of patients with skull defect after traumatic current researches, so as to provide possible inspiration for clinical practice.
Case report: Gross total resection of a primary fourth ventricular meningioma using the telovelar approach in a dog

Abstract:An 11-year-old spayed female Maltese dog presented with a 2-month history of gait alterations, wide-based stance, and chronic vomiting. Neurological examination revealed cerebellovestibular signs, including head tilt, nystagmus, strabismus, intentional tremor, and hypermetric gait. MRI showed a mass with iso- to hypointensity on T1-weighted (T1W) images and heterogeneous hyperintensity on T2-weighted (T2W) images, with marked non-uniform contrast enhancement. The tumor was removed via a telovelar approach without intraoperative complications. Postoperatively, the dog developed non-ambulatory paraparesis with the rigidity of the pelvic limbs but recovered ambulation within 6 days. Preoperative neurological signs progressively improved, and the patient was discharged without complications 10 days after surgery. Histological examination revealed dense spindle cells with an abundant collagen matrix and oval-shaped nucleated cells with small whorls, leading to a diagnosis of transitional meningioma of the fourth ventricle. MRI follow-up at 8 months postoperatively showed no definitive evidence of recurrence. At the final follow-up, 15.4 months postoperatively, mild neurological signs, including a slight head tilt and subtle strabismus, remained, but the rest of the neurological examination was normal. This is the first reported case of a meningioma in the fourth ventricle of a dog successfully removed using the telovelar approach.
Clinical and Diagnostic Imaging Features of Brain Herniation in Dogs and Cats

Abstract:BACKGROUND: Quantification of brain herniation on MRI and its immediate clinical implications are poorly described.OBJECTIVES: Define the normal position of caudal fossa structures on brain MRIs in dogs and cats utilizing morphometry, compare this to dogs and cats with caudal transtentorial herniation (CTH), foramen magnum herniation (FMH) or both identified on MRI, and investigate associations between herniation severity, clinical signs, and 24-hour outcome.ANIMALS: Ninety-two controls (66 dogs, 26 cats), 119 cases with herniation (88 dogs, 31 cats).METHODS: Retrospective case series. The MRI database was searched for controls with normal brain anatomy and cases with brain herniation. Morphometry in controls established TTX (transtentorial to rostroventral cerebellum) to quantify CTH and FMX (caudoventral cerebellum to foramen magnum) to quantify FMH. Measurements were compared between cases and controls. Correlations with specific clinical variables and outcome were investigated.RESULTS: Measurements in medium/large control dogs versus small dog and cat controls were significantly different (P < .001, TTX: -0.46, -0.305, -0.3, FMX: 0.695, 0.27, 0.25, respectively). 119/1564 (7.6%) cases that underwent brain imaging had brain herniation. TTX and FMX were significantly different between controls and cases with CTH or FMH (P < .001). 67/89 (75%) cases with supratentorial lesions had no signs directly attributable to herniation. 71/119 (60%) had a normal anesthetic recovery. TTX was significantly associated with 24-hour survival (P < .001).CONCLUSIONS AND CLINICAL IMPORTANCE: Brain herniation can be quantified on MRI. Clinical signs directly attributable to brain herniation commonly are absent, and more severe CTH based on TTX is associated with a worse short-term outcome.

Abstract:An 8-month-old, 3.4 kg, castrated male Toy Poodle was referred for progressive tetraparesis and respiratory disorder without a history of trauma. Repeated computed tomography (CT) and magnetic resonance imaging (MRI) with different positions of the neck revealed concurrent atlanto-occipital dislocation (AOD) and atlantoaxial instability (AAI) with spinal cord compression. This case was unique due to its congenital nature and the absence of trauma. The surgical treatment involved precise removal of the C1 vertebra's ventral articular facet, which was compressing on the spinal cord, attributed to its fixed and malaligned position within the atlantooccipital joint. Following facetectomy, the stabilization of the occipital bone to the C2 vertebra was achieved by screws, wire, and polymethyl methacrylate. Two days after surgery, the dog recovered ambulation and showed gradual improvement in gait, despite mild residual ataxia. Postoperative CT and radiographs showed successful decompression of the spinal cord. The screw loosening was confirmed at 114 days, which was managed successfully by extracting the affected screws. Through the 21-month monitoring period, the dog showed a normal gait with a wide-based stance of the pelvic limbs when standing and experienced no pain. This case represents the first report of concurrent congenital AOD and AAI treated with a ventral surgical approach, contributing new insights to the understanding and management of such complex cranio-cervical junction disorders in veterinary neurosurgery.
Case report: Reconstruction of a complex maxillofacial gunshot defect using a titanium patient-specific implant in a dog

Abstract:This report describes the surgical reconstruction of large maxillofacial defect caused by a short-range gunshot injury in a dog using titanium patient-specific implant (PSI). A 3-year-old male Wolf Shepherd was admitted for a large right facial defect with right nasal cavity exposure caused by a gunshot injury. Radiographic examination revealed severe loss of the right maxillary, nasal, and incisive bones, multiple fractures of both left and right palatine bones, and a comminuted fracture of the right mandible. Initial surgical procedure included computed tomography (CT) imaging for three-dimensional (3D) implant design. Open wound management was maintained for 18 days until the fresh granulation tissue fully covered the wound bed. The implant was designed in a "hand grasping shape" to cover the defect, align multiple fractured palatine bones, and make a snap fit function. Multiple holes, including cortical screw holes, were added to the final design. The implant was printed on a titanium alloy. Surgical application of titanium PSI was performed 19 days after the primary surgery. A free sublingual mucosal graft was used to reconstruct the mucosal layer of the right nasal cavity. The mucosa was then covered with collagen membrane to strengthen the structure of the nasal cavity. Blunt dissection of the hard palate mucoperiosteum above the palatine process and palatine bones, soft tissue above the maxilla was performed, and the 3D printed titanium implant was fastened in a preplanned position. The facial soft tissue defect was reconstructed, and the titanium PSI was covered using an angularis oris cutaneous flap. Partial flap necrosis occurred in the rostral aspect, and the wound was managed to heal by a second intension. Flap dehiscence at the junction of the flap and hard palate mucoperiosteum occurred with exposure of the implant 2 days postoperatively. Multiple attempts to close the defect failed, and the owner wanted to stop treatment. Healthy granulated tissue was observed proximal to the implant. The defect no longer increased in size and did not show any noticeable complications related to the defect at 60 days after titanium PSI application, and the dog was discharged. Six months post-operatively, the dog remained active with great appetite, gained weight, and showed acceptable facial symmetry without enlargement of the implant exposure or any implant-related problems.

Reconstruction of a Combined Frontal Bone and Orbital Roof Defect With Associated Meningoencephalocele Using 3D Modeling and 3D Navigation

Abstract:Introduction: Orbital roof fractures are uncommon and normally associated with high energy trauma in which multiple other injuries are present. Most can be managed non-operatively with close observation. However, in a small proportion the defect is such that it permits the development of a meningoencephalocele, which can cause exophthalmos, a reduction in visual acuity and pain, all of which are unlikely to improve without surgical treatment. In light of their scarcity and the potential of serious risks with surgery that includes meningitis and visual disturbance (or even loss), thorough planning is required so that the meningoencephalocele can be reduced safely and the orbital roof adequately reconstructed. Methods: We report a case of a patient with a frontal bone defect, orbital roof fracture and associated meningoencephalocele that presented years after being involved in a road traffic accident in another country, who complained of a significant headache and orbital pain. The use of 3D modeling to help plan the surgery, and intraoperative 3D navigation to help negotiate the anterior skull base are described along with the reconstruction of the frontal bone and orbital roof using titanium mesh contoured on the 3D model. Conclusions: Although conservative management of orbital roof fractures predominates; those that are symptomatic, have associated neurologic symptoms or pose a risk to the eyesight warrant a surgical approach. The methods of repair, which center around separating the intracranial and intraorbital contents, are described in the context of this patient and previous cases, and a treatment algorithm is proposed.

This disorder of adrenal biosynthesis can now be classified unequivocally on the basis of molecular biological techniques. In its classical form, this disease is characterized by loss of the gene region which codes for adrenal 21-hydroxylase. The techniques of gene analysis enable unequivocal identification of carriers of heterozygotic and homozygotic features and hence affords the basis for a rational concept for therapy of this disease.

Abstract:Surgery is one of the primary options for the management of traumatic brain injury(TBI). We focused on operative techniques, additional options, and potential pitfalls of surgical intervention for intracranial hematomas, such as acute subdural hematoma(ASDH), acute epidural hematoma(AEDH), cerebral contusion, and intracerebral hematoma. A wide craniotomy covering the hematoma was recommended for a case of AEDH to evacuate the hematoma, control bleeding, and prevent blood reaccumulation. Combined multiple craniotomies leaving a bone bridge over the sinus for dural tenting sutures enabled safe surgical intervention in a case of AEDH with sinus injuries. Different surgical techniques have been advocated for the evacuation of ASDH. Large craniotomy is often chosen as it can easily be shifted to decompressive craniectomy in case of brain swelling. It is important to pay attention to injuries of dural sinuses and bridging veins, and to expose the floor of the middle cranial fossa. Small craniotomy or endoscopic burr-hole evacuation of ASDH has been accepted as a way to avoid large craniotomies and additional morbidity, particularly for patients who are poor surgical candidates. Contusion necrotomy is performed for satisfactory control of progressive elevation in intracranial pressure and clinical deterioration.
Management of Intracranial Hemorrhage and Skull Fracture After Blunt Intrauterine Trauma

Abstract:Background: Fetal trauma during pregnancy can have a significant impact on the developing brain. Fetal trauma can lead to several intracranial pathologies including hypoxic-ischemic injury, skull fractures, and intracranial hemorrhages. Blunt trauma to a fetus resulting in the need for neurosurgical intervention is a rare occurrence and seldom described in the literature. Case description: Here we present the case of a 28-year-old, 36-week pregnant woman who was brought to the hospital by ambulance following a high-speed motor vehicle collision as the restrained driver. On computed tomography of the abdomen, the fetus was found to have a left-sided skull fracture with intracranial hemorrhage. The fetus was emergently delivered by way of caesarean section for lack of fetal movement and indeterminate heart tracings. Postnatally, the neonate had a Glasgow Coma Scale of 7. A postnatal head computed tomography better defined the skull fractures and multiple areas of intracranial hemorrhage. The baby was taken to the operating room for evacuation of the hematomas. At 16 months of age, the baby was well with only mild developmental delay, although a ventriculoperitoneal shunt was needed in a delayed fashion at 3 months of age. Conclusions: We present a rare situation where emergent caesarean section delivery followed by neonatal craniotomy was necessary. Our case illustrates that good outcomes can be achieved with rapid identification of fetal intracranial injury and intervention.

Traumatic pericardial rupture with secondary cardiac herniation in a dog

Abstract:Objective: To describe the unique finding and treatment of a dog with cardiac herniation due to traumatic pericardial rupture. Case summary: A 6.5-year-old entire male Yorkshire Terrier was presented for further management after being hit by a car. Despite suspected significant intrathoracic trauma at that time, the patient regained hemodynamic stability and had orthopedic surgery to correct a right iliac fracture. The patient was readmitted to the hospital 12 days following the initial visit due to considerable respiratory difficulty after accidentally being dropped several feet. Thoracic radiographs revealed an unusual severe mediastinal shift to the left with an atypical position of the cardiac silhouette against the left lateral thoracic wall. Due to the severe respiratory compromise of the patient and newly developed pneumothorax, an exploratory thoracotomy was recommended, where a complete rupture of the pericardium was identified, with secondary left-sided prolapse of the heart. Other more common intrathoracic injuries (ie, lung perforation, rib fractures) were also identified and partially repaired. The patient recovered successfully and was discharged 4 days postoperatively. New or unique information provided: This is the first case report in the veterinary literature of traumatic pericardial rupture and cardiac herniation. According to human case descriptions, this is a rare and often fatal occurrence, which can be significantly challenging to diagnose preoperatively or antemortem. Emergency veterinary clinicians should be aware of this rare but important complication of blunt thoracic trauma. Surgical intervention may be necessary in cases with suspected or confirmed entrapment of great vessels or cardiac chambers, although these abnormalities were not present in this case.

Abstract:The presence of air in the vertebral canal (extracranial epidural emphysema) has been described in veterinary medicine in isolated case reports mainly associated with intervertebral disc disease. This case of a nine-year-old male neutered greyhound describes a different aetiology where the presence of extensive epidural emphysema was associated with pneumomediastinum, pneumothorax and subcutaneous emphysema following thoracotomy for a lung lobectomy. The extracranial epidural emphysema was managed conservatively by sealing of the thoracotomy site and treating the pneumothorax. Extracranial epidural emphysema should be considered as a possible complication of thoracic surgery if extensive subcutaneous emphysema and/or pneumomediastinum develops.

Abstract:Introduction: Facial bone fractures triggered by low-height falls are rare in toddlers, while severe intracranial injuries resulting from minor trauma are extremely rare. Case: Herein, we report the case of a 2-year-old girl who fell from a baby chair, striking her chin, who rapidly developed impaired consciousness 3 h later. The patient subsequently presented with a mandibular fracture and acute obstructive hydrocephalus due to a traumatic isolated subarachnoid hemorrhage in the posterior cranial fossa. She was successfully treated with ventricular drainage, which achieved a favorable outcome. Conclusion: Maxillofacial trauma and head injuries are closely associated. Even in minor cases of maxillofacial trauma, vigilant monitoring and prompt intervention are crucial to prevent fatal outcomes in toddlers.

Abstract:Objective: To report a digital workflow for use and long-term outcome of cranioplasty with a 3D-printed patient-specific Polyetheretherketone (PEEK) implant in a 12-y-old German Shepherd dog after surgical removal of an extensive occipital bone multilobular osteochondrosarcoma (MLO). Study design: Retrospective case report. Animal: A 12-year-old neutered female German Shepherd dog was presented with facial deformity, blindness, tetraparesis, and ataxia. Magnetic resonance imaging (MRI) and computed tomography (CT) identified a large skull-based mass extending extra-and intracranially with severe compression of the cerebellum and occipital lobes of the cerebrum. Methods: One-stage decompressive craniectomy using virtual surgical planned 3D-printed craniotomy cutting guides and the Misonix BoneScalpel® and reconstruction with a patient-specific 3D-printed PEEK cranial implant. Results: 3D-printed craniectomy cutting guides allowed an adequate fit of the cranial implant to the original skull. Misonix BoneScalpel® allowed performing a safe and extensive craniectomy. Postoperative CT (8 weeks after surgery) confirmed the PEEK cranial implant to be in place and without implant rejection. Clinically, the neurological examination identified only a right-hind limb delay in proprioception 8 weeks postoperatively, which remained unchanged at 18 months after surgery. Adjunctive treatment included metronomic chemotherapy. Eighteen months after surgery the dog passed away for reasons unrelated to the MLO, no implant-related complications were reported. Conclusion: 3D-printed craniectomy cutting guides, patient-specific PEEK cranial implant, and metronomic chemotherapy can lead to a successful long-term outcome in dogs with extensive skull MLO. Clinical significance: PEEK is an alternative biomaterial that can be used successfully for skull reconstruction.
Management of migrating intracranial bullet fragments in a 13-year-old female after firearm brain injury: technical and surgical nuances

Abstract:INTRODUCTION: We present the challenges and nuances of management in a rare case of multiple migrating intracranial fragments after pediatric gunshot wound to the head (GSWH).CASE PRESENTATION: A 13-year-old girl suffered left parietal GSWH, with new neurologic decline 3 days after initial debridement. Serial imaging showed the largest intracranial fragments had migrated into the left trigone, and descended further with head of bed (HOB) elevation. HOB was iteratively decreased, with concurrent intracranial pressure monitoring. After extubation, with an alert and stable neurologic exam, HOB was decreased to -15 degrees, allowing gravity-assisted migration of the fragments to an anatomically favorable position within the left occipital horn. The patient underwent occipital craniotomy for fragment retrieval on hospital day 27. Two large and >20 smaller fragments were retrieved using neuronavigation and intraoperative ultrasound. Forensics showed these to be .45 caliber handgun bullet fragments. The patient recovered well after 2-months of intensive inpatient rehabilitation.DISCUSSION: During new neurologic decline after GSWH, bullet migration must be considered and serial cranial imaging is requisite. Surgical retrieval of deep fragments requires judicious planning to minimize further injury. Tightly controlled HOB adjustments with gravity assistance for repositioning of fragments may have utility in optimizing anatomic favorability prior to surgery.

Abstract:Objective: To review our single-institution experience in the surgical management of foramen magnum tumors via a far-lateral approach using an oblique straight incision. Methods: From October 2023 to January 2024, four cases of tumors in the foramen magnum area treated at the Capital Medical University-affiliated XuanWu hospital neurosurgery department were involved in this study. All cases were managed with a far-lateral approach using an oblique straight incision. We retrospectively reviewed the clinical and imaging data, as well as the surgical strategies employed. Results: Three cases of foramen magnum meningiomas and one case of glioma of the ventral medulla. All cases underwent a far-lateral approach using an oblique straight incision; all cases had a gross total resection, and the wounds healed well without cerebral fluid leakage or scalp hydrops. Except for one case of right foramen magnum meningioma, which had dysphagia and pneumothorax, the other cases were without any postoperative complications. Conclusion: A far-lateral approach using an oblique straight incision can preserve muscle integrity and minimize subcutaneous exposure, allowing for complete anatomical reduction of muscles. This craniectomy method is simple and replicable, making it worthy of further clinical practice.

Abstract:Objective To analyze and discuss the diagnosis and treatment of bilateral frontal brain contusion with cerebral centre hernia in order to improve the curative effect of severe brain injury .Methods The 126 patients suffering from bilateral frontotemporal brain contusion with cerebral centre hernia were enrolled in .They were admitted in the department of neurosurgery in the affiliated hospital of Hebei University from June 2008 to June 2013.All patients were given comprehensive treatment including surgery . The surgical procedure included bilateral decompressive craniectomy skull and internal decompression surgery .Among all the cases , 63 cases surgery during diencephalon period , 54 cases survived; 49 cases surgery during nondiencephalon period, 36cases survived .112 cases were performed surgery , of which 93 survived; while 14 cases received conservative treatment , 2 cases survived.We explored the diagnosis and treatment of cerebral centre hernia in the method of retrospect review Results The survival rate of surgical treatment group was higher than that of the combining with relevant literatures . conservative treatment group , the difference was statistically significant ( p<0.05) .The survival rate of the diencephalon period of brain centre hernia surgery group was higher than that of nondiencephalon period surgery group , the difference was statistically significant ( p<0.05) Conclusion The patient with brain centre hernia should be paid more attention to , the importance of early diagnosis should be clearly elucidated and the monitoring of patient during diencephalon period should be reinforced .So undergoing surgery during diencephalon period may achieve better therapeutic effect .
