Abstract:Objective: To examine the likelihood of head acceleration events (HAEs) as a function of previously identified risk factors: match time, player status (starter or substitute) and pitch location in elite-level men's and women's rugby union matches. Methods: Instrumented mouthguard data were collected from 179 and 107 players in the men's and women's games and synchronised to video-coded match footage. Head peak resultant linear acceleration (PLA) and peak resultant angular acceleration were extracted from each HAE. Field location was determined for HAEs linked to a tackle, carry or ruck. HAE incidence was calculated per player hour across PLA recording thresholds with 95% CIs estimated. Propensity was calculated as the percentage of contact events that caused HAEs across PLA recording thresholds, with a 95% CI estimated. Significance was assessed by non-overlapping 95% CIs. Results: 29 099 and 6277 HAEs were collected from 1214 and 577 player-matches in the men's and women's games. No significant differences in match quarter HAE incidence or propensity were found. Substitutes had higher HAE incidence than starters at lower PLA recording thresholds for men but similar HAE propensity. HAEs were more likely to occur in field locations with high contact event occurrence. Conclusion: Strategies to reduce HAE incidence need not consider match time or status as a substitute or starter as HAE rates are similar throughout matches, without differences in propensity between starters and substitutes. HAE incidence is proportional to contact frequency, and strategies that reduce either frequency or propensity for contact to cause head contact may be explored.

Abstract:Objectives: To examine the propensity of tackle height and the number of tacklers that result in head acceleration events (HAEs) in elite-level male and female rugby tackles. Methods: Instrumented mouthguard data were collected from women (n=67) and men (n=72) elite-level rugby players from five elite and three international teams. Peak linear acceleration and peak angular acceleration were extracted from HAEs. Propensities for HAEs at a range of thresholds were calculated as the proportion of tackles/carries that resulted in an HAE exceeding a given magnitude for coded tackle height (low, medium, high) and number of tacklers. Propensity ratios with 95% CIs were calculated for tackle heights and number of tacklers. Results: High tackles had a 32.7 (95% CI=6.89 to 155.02) and 41.2 (95% CI=9.22 to 184.58) propensity ratio to cause ball carrier HAEs>30 g compared with medium tackles for men and women, respectively. Low tackles had a 2.6 (95% CI=1.91 to 3.42) and 5.3 (95% CI=3.28 to 8.53) propensity ratio to cause tackler HAEs>30 g compared with medium tackles for men and women, respectively. In men, multiple tacklers had a higher propensity ratio (6.1; 95% CI=3.71 to 9.93) than singular tacklers to cause ball carrier HAEs>30 g but a lower propensity ratio (0.4; 95% CI=0.29 to 0.56) to cause tackler HAEs>30 g. No significant differences were observed in female tacklers or carriers for singular or multiple tacklers. Conclusion: To limit HAE exposure, rule changes and coaching interventions that promote tacklers aiming for the torso (medium tackle) could be explored, along with changes to multiple tackler events in the male game.

Abstract:Objectives: To investigate head kinematic variables in elite men's and women's rugby union and their ability to predict player removal for an off-field (HIA1) head injury assessment. Methods: Instrumented Mouthguard (iMG) data were collected for 250 men and 132 women from 1,865 and 807 player-matches, respectively, and synchronised to video-coded match footage. Head peak resultant linear acceleration (PLA), peak resultant angular acceleration (PAA) and peak change in angular velocity (dPAV) were extracted from each head acceleration event (HAE). HAEs were linked to documented HIA1 events, with ten logistical regression models for men and women, using a random subset of non-case HAEs, calculated to identify kinematic variables associated with HIA1 events. Receiver operating characteristic curves (ROC) were used to describe thresholds for HIA1 removal. Results: Increases in PLA, and dPAV were significantly associated with an increasing likelihood of HIA1 removal in the men's game, with an OR ranging from 1.05-1.12 and 1.13-1.18, respectively. The optimal values to maximise for both sensitivity and specificity for detecting an HIA1 were 1.96krad.s-2, 24.29g, and 14.75rad.s-1 for PAA, PLA, and dPAV respectively. Only one model had any significant variable associated with increasing the likelihood of a HIA1 removal in the women's game - PAA with an OR of 8.51 (1.23-58.66). The optimal values for sensitivity and specificity for women were 2.01krad.s-2, 25.98g, and 15.38rad.s-1 for PAA, PLA, and dPAV respectively. Conclusion: PLA and dPAV were predictive of men's HIA1 events. Further HIA1 data are needed to understand the role of head kinematic variables in the women's game. The calculated spectrum of sensitivity and specificity of iMG alerts for HIA1 removals in men and women present a starting point for further discussion about using iMGs as an additional trigger in the existing HIA process.

Antigen stimulation of prostaglandin synthesis and control of immune responses.

Abstract:Within 2 min following the intravenous injection of sheep erythrocytes (sRBC) there occurs 20 to 80-fold increase in prostaglandin (PG) F2alpha in the spleen. This burst of synthesis is followed by a slow decline to control levels over the next 1-4 hr. No increase in splenic PGF2alpha levels is observed between 24 and 72 hr after injection. Injection of colloidal carbon results in a small increase, approximately 20% of the increase in PGF2alpha observed with sRBC. The early increase in splenic PGF2alpha levels stimulated by sRBC is also dependent upon thymus-derived (T) cells, since the increase is small or nonexistent in athymic mice and NZB mice. Also, the elevation of splenic PGF2alpha levels is blocked by the administration of indomethacin or Ro 20-5720, both of which block the synthesis of prostglandin. A small increase (2-fold) in PGF2alpha levels occurs in the thymus. A soluble antigen, bovine gamma globulin, stimulated a bimodal increase in splenic PGF2alpha levels, the early peak occurring at 2 hr and the later increase occurring at 48 hr. Using inhibitors of prostaglandin synthesis, it is possible to enhance the appearance of cells forming 19S antibody against sRBC, both in vivo and in vitro. Furthermore, inhibition of prostaglandin synthesis enhances DNA synthesis induced in a two-way mixed-lymphocyte reaction only in whole spleen cell cultures and not in cultures of spleen cells purified by passage over glass wool. Based on this evidence, it is proposed that the prostaglandins represent a major soluble mediator in the control of T cell-T cell interactions and also play an important part in T-B (bone-marrow derived) cell interactions.
Dosimetric quality assurance for intensity-modulated radiotherapy feasibility study for a filmless approach.

Abstract:PURPOSE Test and comparison of various 2-D real-time detectors for dosimetric quality assurance (QA) of intensity-modulated radiotherapy (IMRT) with the vision to replace radiographic films for 2-D dosimetry. MATERIAL AND METHODS All IMRT treatment plans were created with the Konrad software (Siemens OCS). The final dose calculation was also carried out in Konrad. A Mevatron Primus (Siemens OCS) linear accelerator which provides 6-MV and 15-MV highenergy photon beams was used for the delivery of segmented multileaf-modulated IMRT. Three different 2-D detectors, each based on a different physical (interaction) principle, were tested for the field-related IMRT verification: (1) the MapCheck diode system (Sun Nuclear), (2) the I'mRT QA scintillation detector (Scanditronix/Wellhöfer), and the Seven29 ionization chamber array (PTW). The performance of these detector arrays was evaluated against IMRT dose distributions created and calculated with Konrad and the results obtained were compared with film measurements performed with radiographic films (EDR2, Kodak). Additionally, measurements were performed with point detectors, such as diamond, diodes (PTW) and ionization chambers (PTW, Scanditronics/ Wellhöfer) and radiochromic films (GafChromic film MD55, ISP). RESULTS The results obtained with all three 2-D detector systems were in good agreement with calculations performed with the treatment-planning system and with the standard dosimetric tools, i.e., films or various point dose detectors. It could be shown that all three systems offer dosimetric characteristics required for performing field-related IMRT QA with relative dose measurements. The accuracy of the 2-D detectors was mostly +/- 3% normalized to dose maximum for a wide dynamic range. The maximum deviations did not exceed +/- 5% even in regions with a steep dose gradient. The main differences between the detector systems were the spatial resolution, the maximal field size, and the ability to perform absolute dosimetric measurements. CONCLUSION Commercial 2-D detectors have the potential to replace films as an "area detector" for field-related verification of IMRT. The on-line information provided by the respective systems can even improve the efficiency of the QA procedures.
Identifying a severity measure for head acceleration events associated with suspected concussions

Abstract:Objectives: To identify a head acceleration event (HAE) severity measure associated with HIA1 removals in elite level rugby union. Methods: HAEs were recorded from 215 men and 325 women with 30 and 28 HIA1 removals from men and women, respectively. Logistical regression were calculated to identify if peak power, maximum principal strain (MPS) and or Head Acceleration Response Metric (HARM) were associated with HIA1 events compared to non-cases. Optimal threshold values were determined using the Youden Index. Area under the curve (AUC) were compared using a paired sample approach. Significant differences were set at p<0.05. Results: All three severity measures were associated with HIA1 removals in both the mens and womens game. Power performed greatest for HIA1 removals in both the mens and womens games, based on overall AUC, sensitivity, and specificity values. HARM and MPS were found to perform lower than PLA in the womens game based on AUC comparisons (p=0.006 and 0.001, respectively), with MPS performing lower than PAA (p=0.001). Conclusion: The findings progress our understanding of HAE measures associated with HIA1 removals. Peak power, a measure based on fundamental mechanics and commonly used in sports performance, may be a suitable HAE severity measure.

Abstract:The study aimed to illustrate how contact (from match‐event data) and head acceleration event (HAE) (from instrumented mouthguard [iMG]) data can be combined to inform match limits within rugby. Match‐event data from one rugby union and rugby league season, including all competitive matches involving players from the English Premiership and Super League, were used. Playing exposure was summarised as full game equivalents (FGE; total minutes played/80). Expected contact and HAE exposures at arbitrary thresholds were estimated using match‐event and iMG data. Generalised linear models were used to identify differences in contact and HAE exposure per FGE. For 30 FGEs, forwards had greater contact than backs in rugby union (n = 1272 vs. 618) and league (n = 1569 vs. 706). As HAE magnitude increased, the differences between positional groups decreased (e.g., rugby union; n = 34 and 22 HAE >40 g for forwards and backs playing 30 FGEs). Currently, only a relatively small proportion of rugby union (2.5%) and league (7.3%) players exceeded 25 FGEs. Estimating contact and HAEs per FGE allows policymakers to prospectively plan and model estimated overall and position‐specific loads over a season and longer term. Reducing FGE limits by a small amount would currently only affect contact and HAE exposure for a small proportion of players who complete the most minutes. This may be beneficial for this cohort but is not an effective HAE and contact exposure reduction strategy at a population level, which requires individual player management. Given the positional differences, FGE limits should exist to manage appropriate HAE and contact exposure.

Abstract:Soccer is a unique sport where players purposefully and voluntarily use their unprotected heads to manipulate the direction of the ball. There are limited soccer head impact exposure data to further study brain injury risks. The objective of the current study was to combine validated mouthpiece sensors with comprehensive video analysis methods to characterize head impact exposure and biomechanics in university varsity women’s soccer. Thirteen female soccer athletes were instrumented with mouthpiece sensors to record on-field head impacts during practices, scrimmages, and games. Multi-angle video was obtained and reviewed for all on-field activity to verify mouthpiece impacts and identify contact scenarios. We recorded 1307 video-identified intentional heading impacts and 1011 video-verified sensor impacts. On average, athletes experienced 1.83 impacts per athlete-exposure, with higher exposure in practices than games/scrimmages. Median and 95th percentile peak linear and peak angular accelerations were 10.0, 22.2 g, and 765, 2296 rad/s2, respectively. Long kicks, top of the head impacts and jumping headers resulted in the highest impact kinematics. Our results demonstrate the importance of investigating and monitoring head impact exposure during soccer practices, as well as the opportunity to limit high-kinematics impact exposure through heading technique training and reducing certain contact scenarios.

Abstract:Head acceleration events (HAEs) are acceleration responses of the head following external short-duration collisions. The potential risk of brain injury from a single high-magnitude HAE or repeated occurrences makes them a significant concern in sport. Instrumented mouthguards (iMGs) can approximate HAEs. The distinction between sensor acceleration events, the iMG datum for approximating HAEs and HAEs themselves, which have been defined as the in vivo event, is made to highlight limitations of approximating HAEs using iMGs. This article explores the technical limitations of iMGs that constrain the approximation of HAEs and discusses important conceptual considerations for stakeholders interpreting iMG data. The approximation of HAEs by sensor acceleration events is constrained by false positives and false negatives. False positives occur when a sensor acceleration event is recorded despite no (in vivo) HAE occurring, while false negatives occur when a sensor acceleration event is not recorded after an (in vivo) HAE has occurred. Various mechanisms contribute to false positives and false negatives. Video verification and post-processing algorithms offer effective means for eradicating most false positives, but mitigation for false negatives is less comprehensive. Consequently, current iMG research is likely to underestimate HAE exposures, especially at lower magnitudes. Future research should aim to mitigate false negatives, while current iMG datasets should be interpreted with consideration for false negatives when inferring athlete HAE exposure.

Exploring concussion prevalence, knowledge and reporting behaviors in women playing rugby union in the United Kingdom

Abstract:Objectives: The aim of this study was to capture information regarding concussion prevalence, knowledge, and reporting behaviors in women rugby players across all competitive levels in the UK. We also investigated whether these outcomes were influenced by the prevailing concussion awareness programme (HEADCASE). Measures: 230 players from elite (Premiership), Championship, British Universities and College Sport and club level completed (n = 133) or partially completed (n = 97) a mixed-methods online questionnaire. Results: Over a mean period of 8 years, 159 players (74%) experienced at least one suspected or diagnosed rugby-related concussion. In total, 408 rugby related concussions were reported in these players during this time. Prevalence of concussion did not differ across playing level or position. Being tackled was the most common cause of rugby-related concussions, and the act of tackling another player was second. Players across all levels demonstrated a good knowledge of concussion symptoms, but only 12% identified that concussion can occur without direct impact to the head. 146 players completed the HEADCASE training (mandatory for Premiership players). 41% of players admitted to deliberately failing to report a suspected rugby concussion, and this behavior was not influenced by HEADCASE training. These individuals were more likely to avoid reporting a suspected concussion during an important (e.g. semifinal or final) than unimportant (e.g. training) scenario. Those who underwent the graduated return to play pathway (n = 41) were significantly more likely to have completed HEADCASE training. Conclusion: The prevalence of concussion in women's rugby union is high, but does not differ by playing level or position. All players had a good knowledge of concussion symptoms, but many were unaware that concussion may occur without direct impact to the head. Almost half of all players admitted to deliberately failing to report a suspected concussion, and this was not significantly influenced by the completion of HEADCASE training.

Abstract:Association football, also known as soccer in some regions, is unique in encouraging its participants to intentionally use their head to gain a competitive advantage, including scoring a goal. Repetitive head impacts are now being increasingly linked to an inflated risk of developing long-term neurodegenerative disease. This study investigated the effect of heading passes from different distances, using head acceleration data and finite element modelling to estimate brain injury risk. Seven university-level participants wore a custom-fitted instrumented mouthguard to capture linear and angular acceleration-time data. They performed 10 headers within a laboratory environment, from a combination of short, medium, and long passes. Kinematic data was then used to calculate peak linear acceleration, peak angular velocity, and peak angular acceleration as well as two brain injury metrics: head injury criterion and rotational injury criterion. Six degrees of freedom acceleration-time data were also inputted into a widely accepted finite element brain model to estimate strain-response using mean peak strain and cumulative strain damage measure values. Five headers were considered to have a 25% concussion risk. Mean peak linear acceleration equalled 26 ± 7.9 g, mean peak angular velocity 7.20 ± 2.18 rad/s, mean peak angular acceleration 1730 ± 611 rad/s 2 , and 95th percentile mean peak strain 0.0962 ± 0.252. Some of these data were similar to brain injury metrics reported from American football, which supports the need for further investigation into soccer heading.

Abstract:Background: Repeated head impacts sustained by athletes have been linked to short-term neurophysiologic deficits; thus, there is growing concern about the number of head impacts sustained in sports. Accurate head impact exposure data obtained via head impact sensors may help identify appropriate strategies across sports and between genders to mitigate repetitive head impacts. Purpose: To quantify sport- and gender-based differences in head impact rate and mechanism for adolescents. Study Design: Cohort study; Level of evidence, 2. Methods: High school female and male varsity soccer, basketball, lacrosse, and field hockey (female only) teams were instrumented with headband-mounted impact sensors during games over 2 seasons of soccer and 1 season of basketball, lacrosse, and field hockey. Video review was used to remove false-positive sensor-recorded events, and the head impact rate per athlete-exposure (AE) was calculated. Impact mechanism was categorized as equipment to head, fall, player to head, or head to ball (soccer only). Results: Male players had significantly higher head impact rates as compared with female players in soccer (3.08 vs 1.41 impacts/AE; rate ratio, 2.2 [95% CI, 1.8-2.6]), basketball (0.90 vs 0.25; 3.6 [2.6-4.6]), and lacrosse (0.83 vs 0.06; 12.9 [10.1-15.8]). Impact mechanism distributions were similar within sports between boys and girls. In soccer, head to ball represented 78% of impacts, whereas at least 88% in basketball were player-to-player contact. Conclusion: Across sports for boys and girls, soccer had the highest impact rate. Male high school soccer, basketball, and lacrosse teams had significantly higher head impact rates than did female teams of the same sport. For girls, basketball had a higher head impact rate than did lacrosse and field hockey, and for boys, basketball had a similar impact rate to lacrosse, a collision sport. Sport differences in the distribution of impact mechanisms create sport-specific targets for reducing head impact exposure.

Abstract:Background: Rugby league is a full-contact collision sport with an inherent risk of concussion. Purpose: To verify the impacts recorded by the x-patch™ with video analysis. Study design: Observational case series. Methods: The x-patch™ was used on eight men's semi-professional rugby league players during the 2016 Newcastle Rugby League competition (five forwards and three backs). Game day footage was recorded by a trained videographer using a single camera located at the highest midfield location to verify the impact recorded by the x-patch™. Videographic and accelerometer data were time synchronized. Results: The x-patch™ sensors recorded a total of 779 impacts ≥ 20 g during the games, of which 732 (94.0%) were verified on video. In addition, 817 impacts were identified on video that did not record an impact on the sensors. The number of video-verified impacts ≥ 20 g, per playing hour, was 7.8 for forwards and 4.8 for backs (range = 3.9-19.0). Impacts resulting in a diagnosed concussion had much greater peak linear acceleration (M = 76.1 g, SD = 17.0) than impacts that did not result in a concussion (M = 34.2g, SD = 18.0; Cohen's d = 2.4). Conclusions: The vast majority (94%) of impacts ≥ 20 g captured by the x-patch™ sensor were video verified in semi-professional rugby league games. The use of a secondary source of information to verify impact events recorded by wearable sensors is beneficial in clarifying game events and exposure levels.
Padded Headgear in Junior and Youth Australian Football: Player Insights From a National Survey

Abstract:Objectives: To investigate beliefs and factors associated with padded headgear (HG) use in junior (<13 years) and youth (≥13 years) Australian football. Design: Online survey. Setting: Junior and youth athletes in Australia. Participants: Australian football players aged U8 to U18. Assessment of variables: Survey questions regarding demographics, HG use, concussion history, beliefs about HG, and risk-taking propensity. Main outcome measures: Rates of padded HG use, and beliefs associated with HG use. Results: A total of 735 players (including 190, 25.9% female) representing 206 clubs participated. Headgear was worn by 315 players (42.9%; 95% CI: 39.3-46.4). Most (59.5%) HG users wore it for games only and wore it voluntarily (59.7%), as opposed to being mandated to do so. Junior players were more likely than youth players to agree to feeling safer ( P < 0.001) and being able to play harder while wearing HG ( P < 0.001). Median responses were "disagree" on preferring to risk an injury than wear HG, and on experienced players not needing to wear HG. Beliefs did not differ between males and females. Headgear use was associated with players belonging to a club where HG was mandated for other age groups (OR 16.10; 95% CI: 7.71-33.62, P < 0.001), youth players (OR 2.79; 95% CI: 1.93-3.93, P < 0.001), and female players (OR 1.57; 95% CI: 1.07-2.30, P = 0.019). Conclusions: Club HG culture, older age and being female were prominent variables associated with voluntary HG use. Players reported believing that HG offers protection. The rate of voluntary and mandated HG use identified is at odds with current scientific evidence that does not support HG as effective concussion prevention.

Abstract:This study investigated the attitudes, preferences, motivations and acceptance of softshell headgear among youth rugby players. Female and male rugby players (ages 13-17) were surveyed regarding headgear use during training and matches, discontinuation reasons, preferred brands, motivations for use, and reasons for non-use. We assessed confidence without headgear, head injuries, familiarity with specifications, and awareness of benefits/risks. Most (86%) didn't wear headgear during training; 74.4% abstained in matches. Reasons for discontinuation included discomfort and perceived ineffectiveness. Parental advice (78%) and injury protection (52%) drove headgear use. Non-use reasons: lack of ventilation (67%), bulkiness (50%), discomfort (44%), non-compulsory use (36%), and lack of consideration (36%). 44.2% believed headgear protects against head injuries; 30.2% were unsure. The results of this study indicate a range of attitudes among youth rugby players towards the use of headgear. Understanding their motivations and concerns is crucial for improving player safety. While some players see headgear as a valuable protective measure, others are deterred by factors such as discomfort and lack of ventilation. There is a need for greater awareness and education about headgear benefits and risks among rugby players, potential modifications to headgear design to enhance comfort and ventilation should be explored and further research conducted to explore the benefits that headgear has for head impact protection.

Abstract:Instrumented mouthguards (iMGs) are a novel technology being used within rugby to quantify head acceleration events. Understanding practitioners' perceptions of the barriers and facilitators to their use is important to support implementation and adoption. This study assessed men's and women's rugby union and league iMG managers' perceptions of staff and player interest in the technology, data and barriers to use. Forty‐six iMG managers (men's rugby union and league n = 20 and n = 9 and women's rugby union and league n = 7 and n = 10) completed an 18‐question survey. Perceived interest in data varied across staff roles with medical staff being reported as having the most interest. The iMG devices were perceived as easy to use but uncomfortable. Several uses of data were identified, including medical applications, player monitoring and player welfare. The comfort, size and fit of the iMG were reported as the major barriers to player use. Time constraints and a lack of understanding of data were barriers to engagement with the data. Continued education on how iMG data can be used is required to increase player and staff buy‐in, alongside improving comfort of the devices. Studies undertaken with iMGs investigating player performance and welfare outcomes will make data more useful and increase engagement.
