Early Gastric Cancer Presenting As a Submucosal Tumor.

Abstract:A 44-year-old man with a history of acute pancreatitis and hyperlipidemia presented with a chief complaint of paroxysmal epigastric pain and dullness for 2 years. Two weeks before admission, the patient underwent an upper gastrointestinal endoscopy examination at a local facility. The esophagogastroduodenoscopy revealed a 10-mm elevated lesion located at fundus resembling a submucosal tumor. The endoscopic ultrasonography displayed a 0.7 cm × 0.8 cm, well-defined, hypoechogenic lesion originating from the muscularis propria layer (Figure A). Physical examination and laboratory parameters were unremarkable. No gastric wall thickening was evident on the abdominal computed tomography examination. The patient was then admitted to our hospital seeking further endoscopic treatment. Routine upper gastrointestinal endoscopy revealed a round-shaped, sessile polypoid submucosal lesion with erosion in the gastric fundus near the cardia, measuring about 1 cm in diameter (Figure B). The submucosal lesion was completely removed through an endoscopic submucosal dissection procedure (Figure C). The primary pathology showed lymphoid tissue hyperplasia with uniformed cells and atypical nucleus (Figure D). Further immunohistochemistry results confirmed the diagnosis of poorly differentiated adenocarcinoma. Bone marrow aspiration and biopsy ruled out the probable diagnosis of non-Hodgkin lymphoma. Thus, a decision was made to proceed with radical gastrectomy. During the surgical procedure, enlarged lymph nodes were found around the stomach and elevated mucosa was noticed in the fundus of stomach. Postoperative pathology confirmed an early gastric poorly differentiated adenocarcinoma with infiltration to the submucosa (Figures E and F) with no lymphatic or distant metastasis (T1bN0M0, grade IA). Early gastric cancer presenting as a submucosal tumor is rare. On diagnosis, the lesion should be carefully differentiated from other submucosal lesions, such as smooth muscle tumor; lymphoma; stromal tumor; lipoma; heterotopic pancreas; and other unusual cases, such as plexiform angiomyxoid myofibroblastic tumor,1Choi H.S.
