Abstract:Janus kinase (JAK) inhibitors are disease-modifying agents with efficacy in treating a spectrum of burdensome dermatologic conditions. The US Food and Drug Administration (FDA) recently placed a black box warning on this class of medications due to safety concerns based on data from studies investigating tofacitinib in patients with rheumatoid arthritis. Here we provide an overview of the timeline of FDA approval of JAK inhibitors in dermatology. We also discuss the available safety profiles of approved oral JAK1 inhibitors, namely abrocitinib and upadacitinib, oral baricitinib, a JAK1/2 inhibitor, deucravacitinib, a Tyk2 inhibitor, and the topical JAK1/2 inhibitor ruxolitinib in dermatology patients. Additionally, we offer suggestions for initial screening and laboratory monitoring for patients receiving JAK inhibitors. We found that the rates of venous thromboembolism reported in trials ranged from no events to 0.1-0.5% in dermatology-specific phase 3 clinical trials compared with no events in the placebo. The rates of cardiovascular events ranged from no events to 0.4-1.2% compared with no events to 0.5-1.2% in the placebo. The rates of serious infections were 0.4-4.8% compared with no events to 0.5-1.3% in the placebo. The rates of nonmelanoma skin cancer (NMSC) ranged from no event to 0.6-0.9% compared with no events in the placebo. The rates of non-NMSC ranged from no event to 0.2-0.7% compared with no event to 0.6% in the placebo. Most patients who developed these adverse events had risk factors for the specific event. The most common adverse events of oral JAK inhibitors included upper respiratory infections, nasopharyngitis, nausea, headache, and acne. Dermatologists should consider patients' baseline risk factors for developing serious complications when prescribing oral JAK inhibitors.

Abstract:Juvenile idiopathic arthritis(JIA) is one of the most common chronic connective tissue diseases characterized by unknown etiologic arthritis with the onset before the age of 16 years and disease course for more than 6 weeks.JIA may be accompanied by impairment of multiple organ function.Recent studies have shown the important role of the Janus kinase (JAK)-signal transducer and activator of transcription (STAT) pathway in the pathogenesis of JIA.Tofacitinib is an oral Janus kinase(JAK) inhibitor approved by Food and Drug Administration (FDA) in 2012 for the effective treatment of rheumatoid arthritis.However, there is little clinical evidence for the use of Tofacitinib in pediatrics.This review aims to clarify the mechanisms, efficacy and safety of Tofacitinib on the treatment of JIA.

Abstract:Abstract Background Malignant skin tumor (MST) is one of the most concerning possible adverse events of Janus kinase (JAK) inhibitors, while the risk of drug-related MST events remains unproven. Aim This study aimed to evaluate the association between JAK inhibitors and adverse events of MST and to characterize the main features of drug-related MSTs. Method Data (2012 to 2021) were collected using the US Food and Drug Administration Adverse Event Reporting System (FAERS). Adverse event cases of JAK inhibitors as the primary suspected (PS) drug were extracted explicitly for further analysis. Disproportionality analysis evaluated the association between JAK inhibitors and MST events by estimating the reporting odds ratio (ROR) and the information component (IC) with a 95% confidence interval (95% CI). Results A total of 142,673 cases with JAK inhibitors as a PS drug were collected from FAERS, including 1,400 cases of MST events. Ruxolitinib, upadacitinib, tofacitinib, and baricitinib were included in the disproportionality analysis. Three JAK inhibitors were associated with MST events, including ruxolitinib (ROR 5.40, 95%CI 5.03–5.81; IC 2.39, 95%CI 2.14–2.62), upadacitinib (ROR 4.79, 95%CI 4.03–5.71; IC 2.24, 95%CI 1.62–2.77), and tofacitinib (ROR 1.67, 95%CI 1.53–1.83; IC 0.73, 95%CI 0.43–1.02). The median time to onset of MST events was 378.5 days. Conclusion The data mining of FAERS suggested an association between MSTs and ruxolitinib, upadacitinib, and tofacitinib. More attention should be paid to MST events when prescribing JAK inhibitors in clinical practice.

Abstract:Janus kinase (JAK) inhibitors are immunomodulatory agents with broad potential for use within dermatology. However, the US Food and Drug Administration has recently placed additional warning labels on JAK inhibitors given concern for an increased risk of major adverse cardiovascular events, malignancy, venous thromboembolism, and mortality. Here, we summarize recent efficacy and safety data of multiple JAK inhibitors including tofacitinib, upadacitinib, baricitinib, and abrocitinib. JAK inhibitors have high efficacy in treating psoriatic arthritis and atopic dermatitis, but carry an increased risk of venous thromboembolism and cardiovascular events relative to other approved treatments. Here, we provide current considerations on balancing the benefits of JAK inhibitors with potentially serious, but low-absolute risk, safety concerns.
P629 JAK inhibitor safety in Inflammatory Bowel Disease: real-world data comparing the effect of JAK inhibitors on blood parameters and their clinical significance

Abstract:Abstract Background Janus kinase inhibitors (JAKi) licensed for the treatment of inflammatory bowel disease (IBD) now include tofacitinib, upadacitinib and filgotinib. JAKi are known to cause abnormalities in a multitude of blood tests. However, whether there are differences in their effects in a ‘real-world’ setting is not known. Methods Blood test results for 118 patients treated with a JAKi were collected. A total of 13 parameters known to be affected were included. Percentage change at 12 and 24 weeks from baseline was calculated and compared amongst the different JAKi. The difference between the proportion of patients with abnormal results at each time-point was also reviewed. Results The JAKi breakdown included: tofacitinib-64, upadacitinib-35, filgotinib-18. JAKi treatment was associated with a significant rise in cholesterol, bilirubin, ALP and AST at 12 and 24 weeks (table 1), while rises in triglyceride and LDL cholesterol, and haemoglobin reduction were only apparent at 24 weeks. Other parameters showed transient or non-significant changes. Individual analysis of each parameter highlighted only two significant differences amongst JAKi: upadacitinib significantly reduced WCC (p=0.048) and neutrophil count (p=0.044) within the group at 24 weeks, and when directly compared to tofacitinib (p=0.047). However, no patients were neutropenic at 24 weeks and only 5 patients had a low WCC (upadacitinib: 3, tofacitinib: 2, filgotinib: 1). As per Common Terminology Criteria for Adverse Events (CTCAE), adverse events (AEs) in these parameters were low despite the significant percentage change in multiple variables. 1 patient (1.1%) was found to be anaemic (Hb <80g/L, at 12 weeks which resolved at 24 weeks), 1 (1.4%) was noted to be lymphopenic (lymphocyte count <0.5x109/L, at 24 weeks), and 1 (limited data) with CK >5 times upper limit of normal. No patients had a significant hepatic disorder as per CTCAE, though mild liver function changes above the upper limit of normal were seen in 9% of patients. On comparing the proportion of patients with abnormal results at 12 and 24 weeks to baseline, it was only cholesterol (p=0.027) and HDL-cholesterol (p=0.041) that had a significantly higher proportion of patients within the abnormal range at 12 weeks. This did not persist at 24 weeks (p=0.13 and p=0.37 respectively). Two patients were commenced on a statin during JAKi therapy. No abnormalities in these blood parameters caused drug withdrawal. Conclusion In this cohort the majority of changes in blood test parameters were transient and/or mild with no AE-related drug withdrawals during JAKi therapy. No clinically significant differences have been appreciated amongst the three JAKi.

Abstract:The efficacy of JAK-kinase inhibitors in autoimmune diseases is due to the peculiarities of their pathogenesis, the involvement of various cytokines that trigger the cascade of inflammatory reactions, enhance the transcriptional response of disease-induced or disease-associated genes and potentiate the production of proinflammatory cytokines. Only tofacitinib is currently approved for use in pediatric rheumatic diseases. Given the literature data on the efficacy of other JAK-kinase inhibitors in adult rheumatic diseases, we assume the possibility of their use in pediatric patients. Objective. To evaluate the efficacy and safety of selective JAK-kinase inhibitor upadacitinib in children with rheumatic diseases. Patients and methods. Data on 85 pediatric patients with various variants of juvenile idiopathic arthritis (JIA) (n = 64) and systemic connective tissue lesions (SCTL) (n = 21) treated with upadacitinib were included in this retrospective study. Initiation of therapy was carried out on the basis of the rheumatology department of the Research Institute of Pediatric Rheumatology of the Federal State Autonomous Institution "Children's Health Research Center" of the Ministry of Health of Russia (Moscow) in the period from May 2022 to July 2023. Descriptive statistics methods were used to assess the efficacy of upadacitinib therapy, comparative analysis was performed using the Wilcoxon criterion. Therapy outcomes in the group of patients with JIA were evaluated by the percentage of improvement according to pediatric criteria of the American College of Rheumatology, achievement of inactive disease stage according to C.Wallace criteria and JADAS-71 activity index, in the group of patients with SCTL – by the dynamics of clinical and laboratory activity parameters, as well as steroid-saving effect. Results. In patients with JIA without systemic manifestations treated with upadacitinib for 3 months (15/54), 30% improvement in ACR Pedi criteria was recorded in 13/15 (87%), 50% in 12/15 (80%), 70% in 11/15 (73%), and 90% in 10/15 (66%) patients; after 6 months. – 30th; 50th; 70th; 90% improvement in 14/15 (93%), 14/15 (93%), 13/15 (87%), 13/15 (87%) patients, respectively. Inactive disease stage according to C.Wallace criteria after 3 months was achieved in 5/15 (33.3%) patients, after 6 months. – у 11/15 (73,3%). In patients with systemic JIA the efficacy of therapy after 3 months was evaluated in 3/10 patients: 2/3 achieved JADAS remission (1 of them – stage of inactive disease according to C.Wallace criteria), in 1 patient the disease activity increased against the background of hip arthroplasty. After 6 months (assessment of outcomes is available in 3/10 patients): 1 patient achieved JADAS remission, 1 patient achieved 50% improvement according to ACR Pedi criteria, 1 patient showed increased disease activity when attempting to reduce the dose of glucocorticoids (GCS). In the group of patients with SCTL, the majority of patients were able to reduce clinical and laboratory disease activity and the demand for oral GCS. Conclusion. The use of upadacitinib in children in rheumatic diseases is highly effective and safe. In patients with refractory course of the disease it is possible to achieve a decrease in disease activity and remission, to reduce the dose of GCS. Abstract:Abstract Background/Aims JAK inhibitors (JAKi) were launched in the UK for rheumatoid arthritis in 2017. Licenced indications have subsequently expanded to include other inflammatory arthritides, inflammatory bowel disease, psoriasis and atopic dermatitis. Since then, there have been numerous safety warnings from regulatory bodies, with signals for cancer, venous thromboembolism and cardiovascular events. We set out to compare how these safety warnings have influenced prescribing behaviour for individual JAKi medications in England. Methods We analysed aggregate prescribing data from all NHS hospitals in England between January 2019 and July 2023. We compared monthly prescriptions issued for individual JAKi (tofacitinib, baricitinib, upadacitinib, filgotinib) across all licenced indications. Standardisation was performed using WHO Daily Defined Doses. We conducted interrupted time-series analyses to evaluate whether changes in prescribing behaviour occurred after: i) the initial press release for the ORAL Surveillance trial data (January 2021); and ii) the European Medicines Agency (EMA) safety committee warning for the JAKi class as a whole (October 2022). Results Overall, JAKi prescribing has increased year-on-year since 2019; however, striking differences exist for individual JAKi medications. Pre-2021, an estimated additional 93 patients per month were newly initiated on tofacitinib. After the ORAL Surveillance press release (January 2021), tofacitinib initiations decreased to 26 new patients/month (p < 0.001). Following the EMA safety warning (October 2022), tofacitinib prescribing fell sharply (71 fewer patients/month; p < 0.001). A similar pattern was observed for baricitinib - pre-2021: 145 new patients/month; post-Jan 2021: 15 new patients/month; post-Oct 2022: 60 fewer patients/month (all, p < 0.001). In contrast, prescriptions for the JAK1-selective inhibitors (upadacitinib and filgotinib) have increased markedly since 2021, with no apparent impact of the pan-JAKi safety warnings. In July 2023, baricitinib remained the most prescribed JAKi in England across all indications (175,245 defined daily doses [DDD]) followed by upadacitinib (154,510 DDD), tofacitinib (105,215 DDD), and filgotinib (72,445 DDD). Conclusion Prescribing of JAKi has changed markedly in association with emerging safety warnings, with significant reductions in prescriptions for tofacitinib and baricitinib. It is notable that warnings have emerged for the JAKi class as a whole, yet prescriptions for the JAK1-selective inhibitors, upadacitinib and filgotinib, have continued to increase. Explanations could include clinicians' interpretation of risk differences within the JAKi class, as well as external pressures such as price and licenced indications. Adverse cardiovascular events in rheumatoid arthritis patients treated with JAK inhibitors: an analysis of postmarketing spontaneous safety reports.

Abstract:Objectives The ORAL Surveillance trial, a postmarketing safety clinical trial, found an increased risk of adverse cardiovascular events and venous thromboembolism (VTE) in patients treated with Janus Kinase (JAK) inhibitors compared to tumor necrosis factor (TNF) inhibitors. However, additional studies yielded mixed results and data on other JAK inhibitors are limited. Methods A retrospective, pharmacovigilance study using the FDA adverse event reporting system (FAERS) to assess reporting of adverse cardiovascular events following treatment with JAK inhibitors in rheumatoid arthritis (RA) patients between January 2015 and June 2023. To identify disproportionately increased reporting, an adjusted reporting odds ratio (adj.ROR) was calculated with a multivariable logistic regression model. Results We identified safety reports of 75,407 RA patients treated with JAK inhibitors (tofacitinib, n=52,181; upadacitinib, n=21,006; baricitinib, n=2,220) and 303,278 patients treated with biologic disease-modifying antirheumatic drugs (bDMARDs; TNF inhibitors, rituximab, and tocilizumab). The mean age was 61.2(±12) and 59.0(±13), respectively; 82% and 81% were women. Compared to bDMARDs, JAK inhibitors were associated with an increased reporting of VTE [n=1,393, adj.ROR=2.11 (1.97−2.25)], stroke [n=973, adj.ROR=1.25 (1.16−1.34)], ischemic heart disease [IHD, n=999, adj.ROR=1.23 (1.13−1.33)], peripheral edema [n=2699, adj.ROR=1.22 (1.17−1.28)], and tachyarrhythmias [n=370, adj.ROR=1.15 (1.00-1.33)]. Most of the events occurred in the first year after treatment initiation. When different JAK inhibitors were compared, VTE, stroke, and IHD were more frequently reported with upadacitinib and baricitinib than tofacitinib. When stratified by age category, all safety signals were statistically significant in patients aged≤65 years. Conclusion In this global postmarketing study, JAK inhibitors are associated with increased reporting of VTE, stroke, IHD, and tachyarrhythmias. These adverse events were reported following all JAK inhibitors that were studied, suggesting a class effect.

Janus Kinase and Tyrosine Kinase Inhibitors in Dermatology: A Review of Their Utilization, Safety Profile and Future Applications

Abstract:Janus kinase inhibitors, also commonly referred to as JAK inhibitors, are a novel drug class that target and block cytokine signaling mediated by the Janus kinase-signal transducer and activator of transcription (JAK-STAT) pathway, thereby regulating immune response and cell growth. Although JAK inhibitors are mainly used for rheumatological conditions such as rheumatoid arthritis, their application in the field of dermatology is actively being investigated. Tofacitinib is US FDA-approved for psoriatic arthritis and showing promise for treating psoriasis. Most recently, regulatory approvals for the US were gained by ruxolitinib as a first-inclass, selective, topical therapy for atopic dermatitis and oral upadacitinib for active psoriatic psoriasis. Additionally, abrocitinib and upadacitinib have demonstrated efficacy in atopic dermatitis and are pending FDA approval for this indication. The therapeutic potential of JAK inhibitors in dermatological conditions such as alopecia areata, psoriasis, atopic dermatitis, vitiligo, and dermatomyositis are showing promising results in clinical trials. Adverse events for JAK inhibitors seem to be similar to that of biologic drugs. Common adverse effects include increased risk of infections and thromboembolic events. Further investigation is needed to not only better understand the safety profile of JAK inhibitors, but also their full utility within the field of dermatology.
A Comprehensive Overview of Globally Approved JAK Inhibitors

Abstract:Janus kinase (JAK) is a family of cytoplasmic non-receptor tyrosine kinases that includes four members, namely JAK1, JAK2, JAK3, and TYK2. The JAKs transduce cytokine signaling through the JAK-STAT pathway, which regulates the transcription of several genes involved in inflammatory, immune, and cancer conditions. Targeting the JAK family kinases with small-molecule inhibitors has proved to be effective in the treatment of different types of diseases. In the current review, eleven of the JAK inhibitors that received approval for clinical use have been discussed. These drugs are abrocitinib, baricitinib, delgocitinib, fedratinib, filgotinib, oclacitinib, pacritinib, peficitinib, ruxolitinib, tofacitinib, and upadacitinib. The aim of the current review was to provide an integrated overview of the chemical and pharmacological data of the globally approved JAK inhibitors. The synthetic routes of the eleven drugs were described. In addition, their inhibitory activities against different kinases and their pharmacological uses have also been explained. Moreover, their crystal structures with different kinases were summarized, with a primary focus on their binding modes and interactions. The proposed metabolic pathways and metabolites of these drugs were also illustrated. To sum up, the data in the current review could help in the design of new JAK inhibitors with potential therapeutic benefits in inflammatory and autoimmune diseases.

EEGs during High‐Dose Fentanyl‐, Sufentanil‐, or Morphine‐Oxygen Anesthesia

Abstract:In 49 patients undergoing open-heart surgery we compared the electroencephalographic (EEG) effects of high-dose morphine, fentanyl, or sufentanil with O2, using two computerized analysis and display techniques: a period analysis (the Klein method) and an aperiodic analysis (the Neurometrics monitor). During fentanyl or sufentanil anesthesia, both techniques revealed a general decrease in frequency, shown by the aperiodic analysis primarily as a marked increase in the very low frequency range: an increase in the 1-Hz bin (TP1, in μv2) from 2.80 × 104 ± 3.20 × 104 (SD) to 45.1 × 104 ± 27.2 × 104 for fentanyl and from 3.11 × 104 ± 2.83 × 104 to 52.8 × 104 for sufentanil. The cumulative percent power at 3 Hz (CP3) increased from 27.2 ± 6.8 to 83.0 ± 11.0 for fentanyl and from 22.7 ± 5.2 to 85.1 ± 10.4 for sufentanil, while the frequency at 90% cumulative percent power (F90, in Hz) decreased from 17.8 ± 2.9 to 7.9 ± 2.8 for fentanyl and 16.4 ± 5.2 to 5.6 ± 4.3 for sufentanil. The changes with morphine were less obvious, with some attenuation of high-frequency power shown by the Klein method, and an increase from 24.1 ± 8.6 to 59.3 ± 20.7 with CP3, but no change in TP1. Low-frequency power with the period analysis and TP1 with the aperiodic analysis decreased between laryngoscopy and the incisions with fentanyl and sufentanil. We conclude that these narcotics given in high doses markedly alter the EEG, that the changes with fentanyl and sufentanil are greater than those with morphine, that computerized analysis and display techniques can quantitate these changes, that--- using the EEG of other general anesthetics as a guide---the patients were adequately anesthetized, and that, given the appropriate variables, the EEG probably reflects the depth of anesthesia with high-dose narcotics.
Safety of Janus Kinase Inhibitors in Inflammatory Bowel Diseases

Abstract:In recent years, better knowledge of the pathophysiology of inflammatory bowel diseases (IBD) has led to a relevant expansion of the therapeutic arsenal for these conditions. Janus kinase (JAK) inhibitors are a family of small molecules that block one or more of the intracellular tyrosine kinases, including JAK-1, JAK-2, JAK-3 and TYK-2. Tofacitinib, a non-selective small molecule JAK inhibitor, and upadacitinib and filgotinib, which are selective JAK-1 inhibitors, have been approved by the US Food and Drug Administration (FDA) for moderate-to-severe active ulcerative colitis. Compared to biological drugs, JAK inhibitors have a short half-life, rapid onset of action, and no immunogenicity. Both clinical trials and real-world evidence support the use of JAK inhibitors in the treatment of IBD. However, these therapies have been linked with multiple adverse events (AEs) including infection, hypercholesterolemia, venous thromboembolism, major adverse cardiovascular events, and malignancy. While early studies recognized several potential AEs, post-marketing trials have shown that tofacitinib may increase the risk of thromboembolic diseases and major cardiovascular events. The latter are seen in patients aged 50 years or older with cardiovascular risk factors. Hence, the benefits of treatment and risk stratification need to be considered when positioning tofacitinib. Novel JAK inhibitors with a more selective effect on JAK-1 have proven to be effective in both Crohn's disease and ulcerative colitis, offering a potentially safer and efficacious therapeutic option to patients, including those with previous non-response to other therapies such as biologics. Nevertheless, long-term effectiveness and safety data are required.

Abstract:Introduction: The number of treatment opportunities for patients with moderate-severe atopic dermatitis (AD) is increasing, however with varying treatment safety profiles. The safety profile of oral JAKis remains a concern because of their ubiquitous mechanism of action inhibiting many cytokines. Safety warnings for these products have combined data across indications, making it difficult to assess risk for specific populations. Objective: We aimed to estimate the incremental risk of pre-specified adverse events (AEs) per patient-year (PY) of oral JAKi compared with standard of care (SoC), incl. topical corticosteroids, biologic agents, and other treatments, among those with AD or with other diagnoses. Methods: A systematic literature review was conducted on 17 preselected AEs based on randomized controlled trials of oral JAKis. Meta-analyses (MA) estimated the incidence rate difference (IRD) for each AE between oral JAKis and SoC, both in AD and non-AD populations. Number needed to harm (NNH) was calculated as the inverse of the IRD for each AE. Results: We identified 82 studies, including 19 in AD population, for use in the MA. In the AD population, the estimated NNH in PY for acne was 5, indicating 1 additional acne event for every 5 patients treated with oral JAKis vs SoC per year. Moreover, the NNH was 33 for herpes zoster, 62 for anemia, 94 for serious adverse events (SAEs), and 452 for malignancies. In the non-AD population, the NNH was 52 for HZ, 56 for hyperlipidemia, 58 for SAEs, 113 for anemia, 153 for acne, and 254 for malignancies. Conclusions: The MA identified an increased risk with oral JAKis compared with SoC for multiple AEs, spanning from less to more severe AE, including malignancies in both AD and non-AD populations. As safety risks are observed in both the AD and non-AD populations, the use of oral JAKis should be carefully considered.
National and interprovincial prescribing patterns of JAK-inhibitors in Canada: a repeated cross-sectional analysis

Abstract:Janus Kinase (JAK) inhibitors have emerged as a novel category of medications to treat a variety of immune-mediated conditions. However, limited insight exists regarding the impact of safety concerns on their usage and prescribing practices. Therefore, the objective of this study was to describe the utilization of JAK-inhibitors in Canada, both nationally and within individual provinces. We used data from IQVIA's Compuscript database. We conducted a repeated cross-sectional study of all JAK-inhibitor units dispensed in retail pharmacies (tofacitinib, ruxolitinib, baricitinib, and upadacitinib) within the ten Canadian provinces from July 1, 2016, to June 30, 2022. Throughout Canada, outpatient pharmacies dispensed an estimated total of 26,126,409 JAK-inhibitor units between 2016 and 2022, averaging 9,431 units dispensed per 100,000 population. All provinces had increasing rates of JAK-inhibitor units dispensed over time, whereby between July 2021 to June 2022, New Brunswick exhibited the highest rates (27,696 units per 100,000), and Prince Edward Island demonstrated the lowest rates (10,065 units per 100,000). In this study, utilization of JAK-inhibitors increased in Canada over the study period, evident at both provincial and national levels. Variability in JAK-inhibitor utilization between provinces underscores the necessity for further investigations to ascertain appropriate usage practices.

JAK inhibitors in refractory juvenile rheumatic diseases: Efficacy, tolerance and type-I interferon profiling, a single center retrospective study

Abstract:Objectives: - Janus Kinase inhibitors (JAKi) are a new class of drugs available for pediatric rheumatic diseases. This study aimed to describe the safety and effectiveness of JAKi in these diseases, with a focus on longitudinal interferon-stimulated genes (ISG) assessment. Methods: - We present a single-center retrospective study of children with refractory pediatric rheumatic diseases including connective tissue diseases, monogenic type I interferonopathies or juvenile idiopathic arthritis, receiving JAKi. According to physicians' assessment, treatment effectiveness was classified at 12 months as a complete response in the total absence of disease activity, partial response in case of significant (>50%) but incomplete improvement or no response in the case of non-response or improvement of less than 50% of the clinical and biological parameters. ISG were monitored longitudinally using Nanostring technology. Results: - 22 children were retrospectively included in this study, treated either by baricitinib or ruxolitinib. Complete response was achieved at 12 months in 9/22 (41%) patients. 6/22 (27%) patients were non-responders and treatment had been discontinued in five of them. Within the interferon (IFN)-related diseases group, ISG-score was significantly reduced 12 months after JAKi onset (p = 0.0068). At 12 months, daily glucocorticoid doses had been reduced with a median dose of 0.16 mg/kg/day (IQR 0.11; 0.33) (p = 0.0425). 7/22 (32%) patients had experienced side effects, infections being the most common. Increase of the body mass index was also recorded in children in the first 6 months of treatment. Conclusion: - JAKi represent a promising treatment of immune-mediated pediatric diseases, enabling to decrease type-I IFN transcriptomic signature in responding patients, especially in the context of juvenile dermatomyositis. JAKi represent steroid-sparing drugs but they induce metabolic changes linked to weight gain, posing a concern in the treatment of young patients and teenagers. More data are required to define the efficacy and safety of JAKi in the management of refractory pediatric rheumatic diseases.

Comparative safety of oral Janus kinase inhibitors versus dupilumab in patients with atopic dermatitis: A population-based cohort study

Abstract:Background: Systemic Janus kinase inhibitors (JAKi) and dupilumab both have emerged as promising therapeutics for atopic dermatitis (AD). Dupilumab has a favorable safety profile, but oral JAKi therapy has been established in other diseases that carry potential comorbid susceptibilities that influence safety. Objective: We sought to provide real-world evidence of the comparative safety of oral JAKi versus dupilumab in patients with AD. Methods: The study used observational data from multiple healthcare organizations in the US. Patients with AD treated with either oral JAKi (upadacitinib, abrocitinib, and baricitinib) or dupilumab were enrolled. The 2 treatment groups were propensity score matched 1:1 on the basis of demographics, comorbidities, and prior medications. Safety outcomes within 2 years after the initiation of medications were measured by hazard ratios (HRs) with 95% confidence intervals (CIs). Results: A total of 14,716 patients were included, with 942 patients treated with oral JAKi and 13,774 with dupilumab. The 2 treatment groups respectively included 938 patients after matching. Treatment with oral JAKi was not associated with increased risks of mortality, malignancies, major adverse cardiovascular events, venous thromboembolism, renal events, or serious gastrointestinal events. However, patients receiving oral JAKi showed significantly higher risks of skin and subcutaneous tissue infection (HR = 1.35, 95% CI = 1.07-1.69), herpes infection (herpes simplex, HR = 1.64, 95% CI = 1.03-2.61; herpes zoster, HR = 2.51, 95% CI = 1.14-5.52), acne (HR = 2.09, 95% CI = 1.54-2.84), cytopenia (anemia, HR = 1.83, 95% CI = 1.39-2.41; neutropenia, HR = 4.02, 95% CI = 1.91-8.47; thrombocytopenia, HR = 1.76, 95% CI = 1.08-2.89), and hyperlipidemia (HR = 1.45, 95% CI = 1.09-1.92); the risk of ophthalmic complications was higher in those receiving dupilumab (HR = 1.49, 95% CI = 1.03-2.17). Conclusion: Oral JAKi did not exhibit concerning safety issues in treating patients with AD but increased the risk of infections and abnormalities in laboratory findings. Long-term follow-up data are required to validate these results.
The effects of endotoxin administration on blood amino acid concentrations: similarities with sepsis.

Abstract:Following an acute endotoxin (LPS) administration (1 mg/kg body weight) to rats, there was a decrease in the blood concentration of most gluconeogenic amino acids, alanine, glycine, serine, threonine and proline. While the administration of the endotoxin induced no changes in the concentrations of aromatic amino acids, it decreased the concentration of both branched-chain amino acids (leucine, isoleucine and valine) and basic amino acids (lysine, arginine, histidine and ornithine). On a global basis, the endotoxin significantly decreased the total blood amino acid concentration 2 hrs. after the administration, the effect lasting as long as 8 hrs. after endotoxin treatment. This decrease was mainly associated with a lower blood concentration of essential amino acids. Rats induced septic, by cecal ligature and puncture, showed a blood amino acid pattern most similar to those acutely treated with LPS.
Managing Cardiovascular and Cancer Risk Associated with JAK Inhibitors

Abstract:Janus kinase inhibitors (JAKi) have enormous appeal as immune-modulating therapies across many chronic inflammatory diseases, but recently this promise has been overshadowed by questions regarding associated cardiovascular and cancer risk emerging from the ORAL Surveillance phase 3b/4 post-marketing requirement randomized controlled trial. In that study of patients with rheumatoid arthritis with existing cardiovascular risk, tofacitinib, the first JAKi registered for chronic inflammatory disease, failed to meet non-inferiority thresholds when compared with tumor necrosis factor inhibitors for both incident major adverse cardiovascular events and incident cancer. While this result was unexpected by many, subsequently published observational data have also supported this finding. Notably, however, such a risk has largely not yet been demonstrated in patients outside the specific clinical situation examined in the trial, even in the face of many studies examining this. Nevertheless, this signal has practically re-aligned approaches to both tofacitinib and other JAKi to varying extents, in other patient populations and contexts: within rheumatoid arthritis, but also in psoriatic arthritis, axial spondyloarthritis, inflammatory bowel disease, atopic dermatitis, and beyond. Application to individual patients can be more challenging but remains important to harness the substantive potential of JAKi to the maximum extent safely possible. This review not only explores the evolution of the regulatory response to the signal, its informing data, biological plausibility, and its impact on guidelines, but also the many factors that clinicians must consider in navigating cardiovascular and cancer risk for their patients considering JAKi as immune-modulating therapy.

Abstract:Abstract Background Janus kinase inhibitors (JAKi) are the first orally administered treatment for Ulcerative Colitis (UC). The JAK family comprises of four intracellular tyrosine kinases inhibitors (JAK1, JAK2, JAK3 and Tyrosine Kinase 2). Inhibition of the JAK kinases lead to dampening of the inflammatory cytokines which drive Inflammatory Bowel Disease (IBD). Tofacitinib was the first generation pan-JAKi. This was followed by second generation JAKi: Filgotinib and Upadacinib which preferentially inhibit JAK1. We report on the real-world evidence on the efficacy and safety of JAKi in the treatment of UC in a tertiary IBD treatment centre. Methods Patients attending the IBD clinic at weeks 8-12 of initiation of therapy were included. Baseline demographics, disease activity and previous therapies were recorded. Clinical response was measured using the SCCAI and biochemical response was measured using calprotectin and CRP. We defined clinical response as ≥3 points reduction from baseline SCCAI, and clinical remission as SCCAI Score of <2.5, and / or biochemical remission as a CRP of less than 5 mg/L and a calprotectin level of < 250 mg/g. We also assessed treatment persistence, and the reasons for treatment cessation. Results The UC populations are shown in Table 1. 52 patients were included and an additional 24 patients who discontinued therapy early on were assessed to review secondary outcomes. Table 1 summarises the comparative outcomes of the JAKi. 81% and 63% of patients achieved clinical and biochemical remission respectively. Overall, 31% discontinued treatment due to lack of efficacy or adverse events. 60% of patients reported adverse events (AEs), 25 % of patients discontinued treatment due to AEs and 84% of patients continued treatment despite reporting AEs. The most common AEs were hypercholesteremia and cold and flu like symptoms and the most common AEs related with treatment discontinuation were recurrent infection or persistent deranged liver functions contributed to patients’ history of primary sclerosing cholangitis. No Serious Adverse Events were reported. 60% of patients on tofacitinib developed side effects and persisted with therapy, which included hypercholesteremia (67%), shingles (11%), flu like symptom (11%) and hair loss (11%). 53% had side effects with Filgotinib; hypercholesteremia (25%), cold like symptoms (25%), chest infection (13%), shingles (13%), acne (13%). 68% developed side effects with Upadacitinib; reports of acne (20%), cold and flu (33%), shingles and mood swings (7%) and high cholesterol (67%). Conclusion In this real-world cohort of UC patients, JAKi was effective in inducing remission and had an acceptable safety profile. The observed safety profile was as expected compared to clinical data results.
