Abstract:Background and introduction: Cardiac imaging is crucial for diagnosing heart disorders. Methods like X-rays, ultrasounds, CT scans, and MRIs provide detailed anatomical and functional heart images. AI can enhance these imaging techniques with its advanced learning capabilities. Method: In this scoping review, following PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses) Guidelines, we searched PubMed, Scopus, Web of Science, and Google Scholar using related keywords on 16 April 2024. From 3679 articles, we first screened titles and abstracts based on the initial inclusion criteria and then screened the full texts. The authors made the final selections collaboratively. Result: The PRISMA chart shows that 3516 articles were initially selected for evaluation after removing duplicates. Upon reviewing titles, abstracts, and quality, 24 articles were deemed eligible for the review. The findings indicate that AI enhances image quality, speeds up imaging processes, and reduces radiation exposure with sensitivity and specificity comparable to or exceeding those of qualified radiologists or cardiologists. Further research is needed to assess AI's applicability in various types of cardiac imaging, especially in rural hospitals where access to medical doctors is limited. Conclusions: AI improves image quality, reduces human errors and radiation exposure, and can predict cardiac events with acceptable sensitivity and specificity.

Abstract:Echocardiography is an integral part of the diagnosis and management of cardiovascular disease. The use and application of artificial intelligence (AI) is a rapidly expanding field in medicine to improve consistency and reduce interobserver variability. AI can be successfully applied to echocardiography in addressing variance during image acquisition and interpretation. Furthermore, AI and machine learning can aid in the diagnosis and management of cardiovascular disease. In the realm of echocardiography, accurate interpretation is largely dependent on the subjective knowledge of the operator. Echocardiography is burdened by the high dependence on the level of experience of the operator, to a greater extent than other imaging modalities like computed tomography, nuclear imaging, and magnetic resonance imaging. AI technologies offer new opportunities for echocardiography to produce accurate, automated, and more consistent interpretations. This review discusses machine learning as a subfield within AI in relation to image interpretation and how machine learning can improve the diagnostic performance of echocardiography. This review also explores the published literature outlining the value of AI and its potential to improve patient care.

Abstract:Background/Objective: Atrial fibrillation [AF] is the most common arrhythmia encountered in clinical practice and significantly increases the risk of stroke, peripheral embolism, and mortality. With the rapid advancement in artificial intelligence [AI] technologies, there is growing potential to enhance the tools used in AF detection and diagnosis. This scoping review aimed to synthesize the current knowledge on the application of AI, particularly machine learning [ML], in identifying and diagnosing AF in clinical settings. Methods: Following the PRISMA ScR guidelines, a comprehensive search was conducted using the MEDLINE, PubMed, SCOPUS, and EMBASE databases, targeting studies involving AI, cardiology, and diagnostic tools. Precisely 2635 articles were initially identified. After duplicate removal and detailed evaluation of titles, abstracts, and full texts, 30 studies were selected for review. Additional relevant studies were included to enrich the analysis. Results: AI models, especially ML-based models, are increasingly used to optimize AF diagnosis. Deep learning, a subset of ML, has demonstrated superior performance by automatically extracting features from large datasets without manual intervention. Self-learning algorithms have been trained using diverse data, such as signals from 12-lead and single-lead electrocardiograms, and photoplethysmography, providing accurate AF detection across various modalities. Conclusions: AI-based models, particularly those utilizing deep learning, offer faster and more accurate diagnostic capabilities than traditional methods with equal or superior reliability. Ongoing research is further enhancing these algorithms using larger datasets to improve AF detection and management in clinical practice. These advancements hold promise for significantly improving the early diagnosis and treatment of AF.

Abstract:Echocardiography (Echo), a widely available, noninvasive, and portable bedside imaging tool, is the most frequently used imaging modality in assessing cardiac anatomy and function in clinical practice. On the other hand, its operator dependability introduces variability in image acquisition, measurements, and interpretation. To reduce these variabilities, there is an increasing demand for an operator- and interpreter-independent Echo system empowered with artificial intelligence (AI), which has been incorporated into diverse areas of clinical medicine. Recent advances in AI applications in computer vision have enabled us to identify conceptual and complex imaging features with the self-learning ability of AI models and efficient parallel computing power. This has resulted in vast opportunities such as providing AI models that are robust to variations with generalizability for instantaneous image quality control, aiding in the acquisition of optimal images and diagnosis of complex diseases, and improving the clinical workflow of cardiac ultrasound. In this review, we provide a state-of-the art overview of AI-empowered Echo applications in cardiology and future trends for AI-powered Echo technology that standardize measurements, aid physicians in diagnosing cardiac diseases, optimize Echo workflow in clinics, and ultimately, reduce healthcare costs.

Abstract:Artificial intelligence (AI) is revolutionizing cardiac imaging, including echocardiography. However, AI has scarce penetration in resource‐limited regions. The implementation of AI‐aided echocardiography (AIE) poses unique challenges and opportunities in resource‐limited areas. Some obvious advantages of AIE include aiding image acquisition, interpretation, and triaging patients based on severity. The challenges AIE faces in resource‐limited regions include a lack of data accessibility for model development, physician apprehension, and an outdated regulatory framework. Based on our early experience with AI, we believe AIE in resource‐limited regions will enhance health equity, improve access to the technology, and lead to cost savings. However, significant efforts are needed to realize these objectives.

Abstract:Artificial intelligence (AI) is evolving in the field of diagnostic medical imaging, including echocardiography. Although the dynamic nature of echocardiography presents challenges beyond those of static images from X-ray, computed tomography, magnetic resonance, and radioisotope imaging, AI has influenced all steps of echocardiography, from image acquisition to automatic measurement and interpretation. Considering that echocardiography often is affected by inter-observer variability and shows a strong dependence on the level of experience, AI could be extremely advantageous in minimizing observer variation and providing reproducible measures, enabling accurate diagnosis. Currently, most reported AI applications in echocardiographic measurement have focused on improved image acquisition and automation of repetitive and tedious tasks; however, the role of AI applications should not be limited to conventional processes. Rather, AI could provide clinically important insights from subtle and non-specific data, such as changes in myocardial texture in patients with myocardial disease. Recent initiatives to develop large echocardiographic databases can facilitate development of AI applications. The ultimate goal of applying AI to echocardiography is automation of the entire process of echocardiogram analysis. Once automatic analysis becomes reliable, workflows in clinical echocardiographic will change radically. The human expert will remain the master controlling the overall diagnostic process, will not be replaced by AI, and will obtain significant support from AI systems to guide acquisition, perform measurements, and integrate and compare data on request.

Abstract:The accurate assessment of left ventricular systolic function is crucial in the diagnosis and treatment of cardiovascular diseases. Left ventricular ejection fraction (LVEF) and global longitudinal strain (GLS) are the most critical indexes of cardiac systolic function. Echocardiography has become the mainstay of cardiac imaging for measuring LVEF and GLS because it is non-invasive, radiation-free, and allows for bedside operation and real-time processing. However, the human assessment of cardiac function depends on the sonographer's experience, and despite their years of training, inter-observer variability exists. In addition, GLS requires post-processing, which is time consuming and shows variability across different devices. Researchers have turned to artificial intelligence (AI) to address these challenges. The powerful learning capabilities of AI enable feature extraction, which helps to achieve accurate identification of cardiac structures and reliable estimation of the ventricular volume and myocardial motion. Hence, the automatic output of systolic function indexes can be achieved based on echocardiographic images. This review attempts to thoroughly explain the latest progress of AI in assessing left ventricular systolic function and differential diagnosis of heart diseases by echocardiography and discusses the challenges and promises of this new field.

Abstract:Precision medicine has the potential to revolutionize the way cardiovascular diseases are diagnosed, predicted, and treated by tailoring treatment strategies to the individual characteristics of each patient. Artificial intelligence (AI) has recently emerged as a promising tool for improving the accuracy and efficiency of precision cardiovascular medicine. In this scoping review, we aimed to identify and summarize the current state of the literature on the use of AI in precision cardiovascular medicine. A comprehensive search of electronic databases, including Scopes, Google Scholar, and PubMed, was conducted to identify relevant studies. After applying inclusion and exclusion criteria, a total of 28 studies were included in the review. We found that AI is being increasingly applied in various areas of cardiovascular medicine, including the diagnosis, prognosis of cardiovascular diseases, risk prediction and stratification, and treatment planning. As a result, most of these studies focused on prediction (50%), followed by diagnosis (21%), phenotyping (14%), and risk stratification (14%). A variety of machine learning models were utilized in these studies, with logistic regression being the most used (36%), followed by random forest (32%), support vector machine (25%), and deep learning models such as neural networks (18%). Other models, such as hierarchical clustering (11%), Cox regression (11%), and natural language processing (4%), were also utilized. The data sources used in these studies included electronic health records (79%), imaging data (43%), and omics data (4%). We found that AI is being increasingly applied in various areas of cardiovascular medicine, including the diagnosis, prognosis of cardiovascular diseases, risk prediction and stratification, and treatment planning. The results of the review showed that AI has the potential to improve the performance of cardiovascular disease diagnosis and prognosis, as well as to identify individuals at high risk of developing cardiovascular diseases. However, further research is needed to fully evaluate the clinical utility and effectiveness of AI-based approaches in precision cardiovascular medicine. Overall, our review provided a comprehensive overview of the current state of knowledge in the field of AI-based methods for precision cardiovascular medicine and offered new insights for researchers interested in this research area.

Abstract:The electronic stethoscope combined with artificial intelligence (AI) technology has realized the digital acquisition of heart sounds and intelligent identification of congenital heart disease, which provides objective basis for heart sound auscultation and improves the accuracy of congenital heart disease diagnosis. At the present stage, the AI based cardiac auscultation technique mainly focuses on the research of AI algorithms, and the researchers have designed and summarized a variety of effective algorithms based on the characteristics of cardiac audio data, among which the mel-frequency cepstral coefficients (MFCC) is the most effective one, and widely used in the cardiac auscultation. However, the current cardiac sound analysis techniques are based on specific data sets, and have not been validated in clinic, so the performance of algorithms need to be further verified. The lack of heart sound data, especially the high-quality, standardized, publicly available heart sound database with disease labeling, further restricts the development of heart sound diagnostic analysis and its application in screening. Therefore, expert consensus is necessary in establishing an authoritative heart sound database and standardizing the heart sound auscultation screening process for congenital heart disease. This paper provides an overview of the research and application status of auscultation algorithm and hardware equipment based on AI in auscultation screening of congenital heart disease, and puts forward the problems to be solved in clinical application of AI auscultation screening technology.
Using artificial intelligence in the development of diagnostic models of coronary artery disease with imaging markers: A scoping review

Abstract:Background: Coronary artery disease (CAD) is a progressive disease of the blood vessels supplying the heart, which leads to coronary artery stenosis or obstruction and is life-threatening. Early diagnosis of CAD is essential for timely intervention. Imaging tests are widely used in diagnosing CAD, and artificial intelligence (AI) technology is used to shed light on the development of new imaging diagnostic markers. Objective: We aim to investigate and summarize how AI algorithms are used in the development of diagnostic models of CAD with imaging markers. Methods: This scoping review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guideline. Eligible articles were searched in PubMed and Embase. Based on the predefined included criteria, articles on coronary heart disease were selected for this scoping review. Data extraction was independently conducted by two reviewers, and a narrative synthesis approach was used in the analysis. Results: A total of 46 articles were included in the scoping review. The most common types of imaging methods complemented by AI included single-photon emission computed tomography (15/46, 32.6%) and coronary computed tomography angiography (15/46, 32.6%). Deep learning (DL) (41/46, 89.2%) algorithms were used more often than machine learning algorithms (5/46, 10.8%). The models yielded good model performance in terms of accuracy, sensitivity, specificity, and AUC. However, most of the primary studies used a relatively small sample ( n < 500) in model development, and only few studies (4/46, 8.7%) carried out external validation of the AI model. Conclusion: As non-invasive diagnostic methods, imaging markers integrated with AI have exhibited considerable potential in the diagnosis of CAD. External validation of model performance and evaluation of clinical use aid in the confirmation of the added value of markers in practice. Electrocardiogram-based artificial intelligence for the diagnosis of heart failure: a systematic review and meta-analysis.

Abstract:BACKGROUND:The electrocardiogram (ECG) is an inexpensive and easily accessible investigation for the diagnosis of cardiovascular diseases including heart failure (HF). The application of artificial intelligence (AI) has contributed to clinical practice in terms of aiding diagnosis, prognosis, risk stratification and guiding clinical management. The aim of this study is to systematically review and perform a meta-analysis of published studies on the application of AI for HF detection based on the ECG. METHODS:We searched Embase, PubMed and Web of Science databases to identify literature using AI for HF detection based on ECG data. The quality of included studies was assessed using the Quality Assessment of Diagnostic Accuracy Studies 2 (QUADAS-2) criteria. Random-effects models were used for calculating the effect estimates and hierarchical receiver operating characteristic curves were plotted. Subgroup analysis was performed. Heterogeneity and the risk of bias were also assessed. RESULTS:A total of 11 studies including 104,737 subjects were included. The area under the curve for HF diagnosis was 0.986, with a corresponding pooled sensitivity of 0.95 (95% CI: 0.86-0.98), specificity of 0.98 (95% CI: 0.95-0.99) and diagnostic odds ratio of 831.51 (95% CI: 127.85-5407.74). In the patient selection domain of QUADAS-2, eight studies were designated as high risk. CONCLUSIONS:According to the available evidence, the incorporation of AI can aid the diagnosis of HF. However, there is heterogeneity among machine learning algorithms and improvements are required in terms of quality and study design.

Abstract:Developments in artificial intelligence (AI) have led to an explosion of studies exploring its application to cardiovascular medicine. Due to the need for training and expertise, one area where AI could be impactful would be in the diagnosis and management of valvular heart disease. This is because AI can be applied to the multitude of data generated from clinical assessments, imaging and biochemical testing during the care of the patient. In the area of valvular heart disease, the focus of AI has been on the echocardiographic assessment and phenotyping of patient populations to identify high-risk groups. AI can assist image acquisition, view identification for review, and segmentation of valve and cardiac structures for automated analysis. Using image recognition algorithms, aortic and mitral valve disease states have been directly detected from the images themselves. Measurements obtained during echocardiographic valvular assessment have been integrated with other clinical data to identify novel aortic valve disease subgroups and describe new predictors of aortic valve disease progression. In the future, AI could integrate echocardiographic parameters with other clinical data for precision medical management of patients with valvular heart disease.

Abstract:The quantification of left ventricular ejection fraction (LVEF) has important clinical utility in the assessment of cardiac function and is vital for the diagnosis of cardiovascular diseases. A transthoracic echocardiogram serves as the most commonly used tool for LVEF assessment for several reasons, including, its noninvasive nature, great safety profile, real-time image processing ability, portability, and cost-effectiveness. However, transthoracic echocardiogram is highly dependent on the clinical skill of the sonographer and interpreting physician. Moreover, even amongst well-trained clinicians, significant interobserver variability exists in the quantification of LVEF. In search of possible solutions, the usage of artificial intelligence (AI) has been increasingly tested in the clinical setting. While AI-derived ejection fraction is in the preliminary stages of development, it has shown promise in its ability to rapidly quantify LVEF, decrease variability, increase accuracy, and utilize higher-order processing capabilities. This review will delineate the latest advancements of AI in evaluating LVEF through echocardiography and explore the challenges and future trajectory of this emerging domain.

Abstract:Cardiovascular diseases remain the leading cause of global mortality, underscoring the critical need for accurate and timely diagnosis. This narrative review examines the current applications and future potential of artificial intelligence (AI) and machine learning (ML) in cardiovascular imaging. We discuss the integration of these technologies across various imaging modalities, including echocardiography, computed tomography, magnetic resonance imaging, and nuclear imaging techniques. The review explores AI-assisted diagnosis in key areas such as coronary artery disease detection, valve disorders assessment, cardiomyopathy classification, arrhythmia detection, and prediction of cardiovascular events. AI demonstrates promise in improving diagnostic accuracy, efficiency, and personalized care. However, significant challenges persist, including data quality standardization, model interpretability, regulatory considerations, and clinical workflow integration. We also address the limitations of current AI applications and the ethical implications of their implementation in clinical practice. Future directions point towards advanced AI architectures, multimodal imaging integration, and applications in precision medicine and population health management. The review emphasizes the need for ongoing collaboration between clinicians, data scientists, and policymakers to realize the full potential of AI in cardiovascular imaging while ensuring ethical and equitable implementation. Abstract:Echocardiography frequently serves as the first-line treatment of diagnostic imaging for several pathological entities in cardiology. Artificial intelligence (AI) has been growing substantially in information technology and various commercial industries. Machine learning (ML), a branch of AI, has been shown to expand the capabilities and potential of echocardiography. ML algorithms expand the field of echocardiography by automated assessment of the ejection fraction and left ventricular function, integrating novel approaches such as speckle tracking or tissue Doppler echocardiography or vector flow mapping, improved phenotyping, distinguishing between cardiac conditions, and incorporating information from mobile health and genomics. In this review article, we assess the impact of AI and ML in echocardiography.

Abstract:Purpose of Review Development of artificial intelligence (AI) models, particularly the application of machine learning (ML) and deep learning (DL) architectures to enhance the current diagnostic armoury for the detection of cardiovascular diseases (CVD) has expanded exponentially over the past five years. We review the current AI landscape in CVD and the impact of applying AI models to the electrocardiogram (ECG). Recent Findings Researchers continue to explore innovative methods utilising ML and DL to automate ECG diagnosis and gain insights into underlying cardiovascular physiology. The results from numerous studies including two randomised control trials (EAGLE and BEAGLE) demonstrates the wide-ranging potential for the application of AI models to both 12-lead and single lead ECG recordings. This allows a fixed timepoint recording to effectively be extrapolated into a continuous monitoring device to predict outcomes for CVD diseases such as left ventricular systolic dysfunction (LVSD), atrial fibrillation (AF), hypertrophic obstructive cardiomyopathy (HOCM), cardiac amyloidosis, valvular heart disease, pulmonary hypertension and channelopathies. A significant proportion of models developed utilise DL architectures such as convolutional neural networks (CNN). The area under the curve (AUC) for published studies range from 0.66–0.99 with sensitivities and specificities between 26.29—95% and 79.5—96.6% respectively. These results suggest a possible role for AI-ECG as a screening tool for cardiovascular pathology. Summary Initial research is promising and demonstrates the potential transformative impact of AI to streamline workflow, aid clinical decision making and prognosticate future cardiovascular outcomes. Future randomised controlled trials are needed to provide an evidence base for the implementation of AI tools into the clinician's workflow.
Prospective Human Validation of Artificial Intelligence Interventions in Cardiology: A Scoping Review

Abstract:Background: Despite the potential of artificial intelligence (AI) in enhancing cardiovascular care, its integration into clinical practice is limited by a lack of evidence on its effectiveness with respect to human experts or gold standard practices in real-world settings. Objectives: The purpose of this study was to identify AI interventions in cardiology that have been prospectively validated against human expert benchmarks or gold standard practices, assessing their effectiveness, and identifying future research areas. Methods: We systematically reviewed Scopus and MEDLINE to identify peer-reviewed publications that involved prospective human validation of AI-based interventions in cardiology from January 2015 to December 2023. Results: Of 2,351 initial records, 64 studies were included. Among these studies, 59 (92.2%) were published after 2020. A total of 11 (17.2%) randomized controlled trials were published. AI interventions in 44 articles (68.75%) reported definite clinical or operational improvements over human experts. These interventions were mostly used in imaging (n = 14, 21.9%), ejection fraction (n = 10, 15.6%), arrhythmia (n = 9, 14.1%), and coronary artery disease (n = 12, 18.8%) application areas. Convolutional neural networks were the most common predictive model (n = 44, 69%), and images were the most used data type (n = 38, 54.3%). Only 22 (34.4%) studies made their models or data accessible. Conclusions: This review identifies the potential of AI in cardiology, with models often performing equally well as human counterparts for specific and clearly scoped tasks suitable for such models. Nonetheless, the limited number of randomized controlled trials emphasizes the need for continued validation, especially in real-world settings that closely examine joint human AI decision-making.

Abstract:Leveraging artificial intelligence (AI) for the analysis of electrocardiograms (ECGs) has the potential to transform diagnosis and estimate the prognosis of not only cardiac but, increasingly, noncardiac conditions. In this review, we summarize clinical studies and AI-enhanced ECG-based clinical applications in the early detection, diagnosis, and estimating prognosis of cardiovascular diseases in the past 5 years (2019-2023). With advancements in deep learning and the rapid increased use of ECG technologies, a large number of clinical studies have been published. However, most of these studies are single-centre, retrospective, proof-of-concept studies that lack external validation. Prospective studies that progress from development toward deployment in clinical settings account for < 15% of the studies. Successful implementations of ECG-based AI applications that have received approval from the Food and Drug Administration have been developed through commercial collaborations, with approximately half of them being for mobile or wearable devices. The field is in its early stages, and overcoming several obstacles is essential, such as prospective validation in multicentre large data sets, addressing technical issues, bias, privacy, data security, model generalizability, and global scalability. This review concludes with a discussion of these challenges and potential solutions. By providing a holistic view of the state of AI in ECG analysis, this review aims to set a foundation for future research directions, emphasizing the need for comprehensive, clinically integrated, and globally deployable AI solutions in cardiovascular disease management.
