Abstract:Background Lower urinary tract symptoms (LUTS) are common complaint in urology practice and affecting the quality of life for patients. This article aims to perform a systematic review and meta-analysis on the global prevalence of LUTS overall, and according to different patient characteristics. Methods We searched MEDLINE and Embase for population-based epidemiological studies reporting the prevalence of LUTS from inception to 1 Jan 2021. Studies which: (1) have enough information on sample size and prevalence; (2) investigate individuals aged 15 or above; and (3) have clear diagnostic criteria for LUTS. We extracted the following information: year of publication; name of the first author; study period; region of recruitment; race; age range; sex; severity; symptoms; and criteria. We pooled rate estimates with exact binomial and test score-based confidence intervals (CIs) using proportions with a random-effects model. Results We included 222 studies from 36 countries involving 1,692,110 samples and 632,933 patients with LUTS. The overall prevalence of any and moderate-to-severe LUTS was 63.2% (95% CI = 58.0–68.1) and 31.3% (95% CI = 28.8–33.8), respectively. The most common symptom was storage symptoms (56.7%; 95% CI = 51.0–62.4), followed by voiding symptoms (36.4%; 95% CI = 27.8–45.4) and post-micturition symptoms (30.7%; 95% CI = 19.2–43.6). A higher prevalence of moderate-to-severe LUTS was observed in male subjects (35.2%; 95% CI = 32.1–38.5) and individuals aged ≥60 (39.0%; 95% CI = 33.4–44.8; I 2 = 99.9%). Its prevalence increased from 27.4% (95% CI = 24.5–30.3) in 1990–1999, to 31.9% (95% CI = 27.3–36.7) in 2000–2009 and 36.2% (95% CI = 30.7–41.9) in 2010–2019. Conclusions This study was the first comprehensive meta-analysis examining the global prevalence of LUTS. We identified a high level of LUTS prevalence in the general population, with a higher burden in male subjects, older individuals, and the Asian population. There has been an increasing trend in the prevalence of LUTS since the 1990s.

Abstract:Study design, materials and methods The population-based, cross-sectional field survey was conducted between July 2011 and February 2012 in five cities of China using questionnaires regarding demographics, the prevalence and the HRQoL. A random sample of men and women aged≥18 years residing in the five cities who were representative of the general populations was selected for demographic questionnaires, Overactive Bladder Symptom Score (OABSS) and International prostate symptom score(IPSS) first. The individuals meeting the diagnostic criteria of LUTS were further interviewed through King’s health questionnaire (KHQ) and ICIQ-MLUTS/ICIQ -FLUTS to estimate the impact of LUTS on HRQoL. Using 2002 ICS definitions, the prevalence estimates of storage, voiding, and postmicturition LUTS were calculated. Data were weighted by city, age cohort, and gender.

Abstract:Urinary incontinence is a common but understudied health problem in adolescents. This study aimed to investigate the prevalence of and associated factors for urinary incontinence in high-school-aged Chinese adolescents. A stratified two-stage cluster sampling procedure was adopted, yielding a sampling frame of 15,055 participants from 46 high schools in Henan province, China. Self-reported questionnaires were used to collect data. The urinary incontinence variable was assessed using the International Consultation of Incontinence Questionnaire-Short Form. The prevalence of urinary incontinence was 6.6%, with a female predominance (7.2% versus 6.0% in males; p < 0.05), and it increased with age, from 5.8% at 14–15 years to 12.3% at 19–20 years old (p < 0.001). The most common subtype of urinary incontinence was urgency urinary incontinence (4.4%), followed by stress urinary incontinence (1.7%) and mixed urinary incontinence (0.5%). Female sex, higher grades, more frequent sexual behavior, physical disease, chronic constipation, mental health problems, and residence in nonurban areas were significantly associated with higher odds of having urinary incontinence (p < 0.05). Public health programs, such as health education and school-based screening, should be established for early detection and appropriate management of urinary incontinence. Furthermore, individualized interventions targeting associated factors should be developed through collective efforts by adolescents, families, schools, and policymakers.

Abstract:BACKGROUND Pediatric lower urinary tract symptoms (LUTS) are a set of common childhood problems. Community‐level interventions that target behavioral change among children with LUTS can improve symptoms outside of the clinic environment. Parents, navigating the home and school environments, are key in supporting healthy bladder behaviors. Thus, we asked parents about their perceptions and barriers related to pediatric bladder health. METHODS English‐speaking parents (n = 30) of children ages 5‐10 years with and without LUTS were interviewed. Transcripts were coded iteratively by two independent coders using deductive and inductive approaches that emphasized consensus coding and peer debriefing. RESULTS Ninety‐three percent of participants were women, 50% were 30‐39 years old, and 60% held a graduate degree. Parents identified school‐, classroom‐, and child‐based barriers to bladder health. These included the bathroom environment, restrictive policies for bathroom use, and anxiety on how and when to use the bathroom. CONCLUSIONS Addressing school‐, classroom‐, and child‐based barriers is necessary to promote healthy bladder habits among children in the school environment and beyond. Recommended school‐based interventions include bathroom use and sanitation policies that support students' voiding needs, teachers' professional development, and school readiness initiatives. Limitations include participation of English‐speaking parents only.

Abstract:BackgroundWe conducted this study to estimate the prevalence of pediatric lower urinary tract symptoms (pLUTS) in a US privately insured pediatric population who are 6–20 years old by age, sex, race/ethnicity from 2003–2014. This has not been previously described in the literature.MethodsWe retrospectively reviewed Optum’s de-identified Clinformatics® Data Mart Database between 2003–2014. A pLUTS patient was defined by the presence of ≥ 1 pLUTS-related ICD-9 diagnosis code between the age of 6–20 years. Neurogenic bladder, renal transplant and structural urologic disease diagnoses were excluded. Prevalence by year was calculated as a proportion of pLUTS patients among the total population at risk. Variables reviewed included age, sex, race, geographic region, household factors and clinical comorbidities including attention-deficit/hyperactivity disorder (ADHD), constipation, and sleep apnea. Point of service (POS) was calculated as a proportion of pLUTS-related claims associated with a POS compared to the total claims at all POS in the time period.ResultsWe identified 282,427 unique patients with ≥ 1 claim for pLUTS between the ages of 6–20 years from 2003 to 2014. Average prevalence during this period was 0.92%, increasing from 0.63% in 2003 to 1.13% in 2014. The median age group of patients was 6–10 years. More patients were female (59.80%), white (65.97%), between 6 and 10 years old (52.18%) and resided in the Southern US (44.97%). Within a single household, 81.71% reported ≤ 2 children, and 65.53% reported ≥ 3 adults. 16.88% had a diagnosis of ADHD, 19.49% had a diagnosis of constipation and 3.04% had a diagnosis of sleep apnea. 75% of pLUTS-related claims were recorded in an outpatient setting.ConclusionsFamilies consistently seek medical care in the outpatient setting for pLUTS. The demographic and clinical characteristics of our cohort reflect prior literature. Future studies can help define temporal relationships between household factors and onset of disease as well as characterize pLUTS-related healthcare resource utilization. Additional work is required in publicly insured populations.

Abstract:Abstract To determine the prevalence of bladder and bowel dysfunction (BBD) and its relationship with delayed elimination communication (EC) in children. A cross-sectional study was carried out in kindergartens and primary schools in mainland China. A total of 10,166 children ranging from 4 to 10 years old were included. A total of 10,166 valid questionnaires were collected, and 409 children were diagnosed with BBD. The overall prevalence was 4.02% (409/10,166) and decreased with age, from 6.19% at age 4 to 1.96% at age 10. With the prolonged use of disposable diapers (DDs), the commencement of usage of EC in a child was significantly put off or delayed by parents, and the prevalence of BBD amongst these children increased ( P < 0.001). The prevalence of BBD among children who stopped using DDs within the first 12 months and after more than 24 months was 2.79% and 4.38% respectively. Additionally, the prevalence among children who started EC within 12 months after birth and those who never engaged in EC was 1.36% and 15.71% respectively. Early introduction of EC and weaning of DD usage has a positive correlation with lower prevalence of BBD in children in China.

Abstract:Aims To investigate the clinical characteristics of health care-seeking men presenting with lower urinary tract symptoms (LUTS) in China and to reveal risk factors for symptom severity. Methods This multicenter, hospital-based, cross-sectional study recruited 1477 eligible male subjects, who were at least 45 years, seeking health care at 9 participating hospitals across the mainland China. The general medical information and subjective symptoms were recorded, followed by the measurement of prostate volume, urodynamic indices, and laboratory tests for kidney function, plus glucose/lipid metabolism. Univariate and multivariate linear regression were employed for the detection of risk factors for symptom severity. Results The proportion of mild, moderate, and severe LUTS was 14.6%, 32.6%, and 52.8%, respectively, with 62.2% reporting the triple combination of storage, voiding, and postmicturition symptoms. Median prostate volume was 44.6 ml, and 71.1% were experiencing comorbidities. Thirteen independent risk factors for LUTS severity were identified, namely, nocturnal voiding episodes and the presence of straining and weak steam; the triple combination of symptom subtypes; general and nocturia quality of life; Q(max) and bladder outlet obstruction index; and numbers of comorbidities, hypertension, estimated glomerular filtration rate, and cholesterol and glycosylated hemoglobin. Conclusions The majority of health care-seeking LUTS men present with moderate-to-severe and overlapping symptoms, with a high prevalence of both lower urinary tract dysfunction and systemic comorbidities. The evidence from both urological and nonurological independent risk factors demonstrate the multifactorial nature of LUTS, for which a multidisciplinary management is essential.

Abstract:Objective:To analyze the incidence and related risk factors of lower urinary tract symptoms in young female.Methods:We designed a questionnaire according to the international consultant on incontinence questionnaire female lower urinary tract symptoms module(ICIQ-FLUTS),IPSS questionnaire and bristol female lower urinary tract symptoms(BFLUTS) questionnaire and qudity of life(QoL) questionnaire.A few parts of the questionnaire were adjusted based on living habits and culture background in our country.The questionnaires were completed independently by respondents.Results:A total of 1 500 copies of questionnaires were distributed,and 1 387 copiesreturned were completed by young female.The prevalence of storage symptoms was 91.2% and was associated with age and mode of delivery.The prevalence of voiding symptoms was 65.2% and was associated with age,condition of marriage,mode of delivery,and education level.The QoL scores of symptoms of stimulation and obstructive symptoms were 2.87±1.32 and 2.23±1.05,respectively.The QoL scores correlation coefficient of symptoms of stimulation and obstructive symptoms were 0.51 and 0.23,respectively.Conclusions:The prevalence of LUTS in young female of this research was considerable high.The LUTS affected the quality of life of young female to various degree.

Abstract:OBJECTIVE:Functional constipation (FC) is a common complaint in childhood but the prevalence still remains unclear, the aim of this study was to explore the prevalence of FC in childhood in the northern parts of China.METHODS:A screening program on FC in childhood was carried out in 5 northern cities (Beijing, Tianjin, Shenyang, Changchun, Harbin) of China according to symptoms under the Rome III criteria. Random clustered sampling of the inhabitants was carried out under stratification of cities, geographic zones, schools or nurseries. Sample size of each area was in proportion to the population of the area. The range of age was 4-14. All subjects under study were requested to fill in a questionnaire distributed by the teachers. The screening program was carried out immediately after a section survey was completed. All together, 20 000 questionnaires were distributed with 19 638 retrievals. According to the result of the screening, a small number of patients who met the criteria were further selected to undergo a detailed clinical examination in the hospital including laboratory examination, colonic transit time, defecography or/and barium enema, electromyologram and anorectal manometry to exclude organic disease of the colon. Simultaneously, a detailed questionnaire was requested to fill under the assistance of trained doctors or medical students. Prevalence of FC of the population was adjusted by the rate of correct diagnosis from the detailed study.RESULTS:The adjusted point prevalence of FC in 5 northern cities of China according to Rome III criteria was 4.73% with higher prevalence rate of FC seen in the Beijing area (5.02%) than in other cities (4.82%, 4.76%, 4.27%, 4.40%, with P < 0.001). Male to female ratio was 1.26:1 with majority of the FC fell in the 4 - 6 year olds (5.76%).CONCLUSION:FC appeared a common disorder in childhood in the northern parts of China which called for greater attention. Higher prevalence was noted in the age groups of 4-6.

Abstract:Objective To investigate the prevalence and impact of lower urinary tract symptoms(LUTS)on life quality of non-urological outpatients.Methods Male participants over 40 years old and female participants over 18 years old were randomly selected from non-urological outpatients.Questionnaires were administered,items including sociology of population data,international prostate symptoms score(IPSS),overactive bladder symptom score(OABSS),quality of life assessment,ICIQ-MLUTS Long form,and ICIQ-FLUTS Long Form for patients who had symptoms.Sociology of population contained age,career,birth,dietary habit,drinking habit and so on.Results A total of 304 copies of questionnaires were distributed,and 193 copies returned were valid,including 93 filled by men and100 by women.The prevalence of LUTS was in sequence of nocturia(43.0%),urinary frequency(26.9%),hesitancy(26.9%)and incomplete emptying(25.8%)in men;urgency(18%),stress urinary incontinence(18%),urinary frequency(15%),and urge urinary incontinence(8%)in women.Nocturia was the greatest influence on quality of life than other symptoms,followed by frequency.Conclusions The prevalence of lower urinary tract symptoms in non-urological outpatients is high and should be arouse enough attention from both urologists and gynecologists.
Adverse childhood experiences and lower urinary tract symptoms in adolescence: the mediating effect of inflammation

Abstract:Background: There is evidence that adverse childhood experiences (ACEs) are associated with lower urinary tract symptoms (LUTS) in adulthood, but few studies have explored these associations in adolescence. Little is known about the biological mechanisms that could explain these associations. We examine whether inflammatory biomarkers mediate the relationship between ACEs and LUTS in adolescence. Methods: We used data from 4,745 participants from the Avon Longitudinal Study of Parents and Children on ACEs (10 'classical' ACEs assessed from birth to age 8), LUTS at age 14 (any urinary incontinence (UI), daytime and bedwetting, urgency, nocturia, frequent urination, voiding postponement, and low voiding volume) and inflammatory biomarkers interleukin-6 (IL-6) and C-reactive protein (CRP) measured at age 9. We first examined associations between the (i) ACE score (summed score [scale of 0 to 10] of total ACEs) and LUTS and (ii) inflammation and LUTS using multivariable logistic regression. We evaluated the mediating effects of IL-6 and CRP on the association between the ACE score and LUTS using the parametric g-formula whilst adjusting for baseline and intermediate confounders. Findings: Higher ACE scores were associated with increased odds of LUTS, e.g. a one-unit increase in ACE score was associated with an increase in the odds of any UI (odds ratio [OR] 1.16, 95% confidence interval [CI] 1.03-1.30). Higher levels of IL-6 were associated with increased odds of LUTS, e.g. any UI (OR 1.24, 95%CI 1.05-1.47). There was weak evidence that the associations between ACE score and any UI, daytime wetting, bedwetting, urgency, and frequency were mediated by IL-6 (e.g. any UI ORnatural indirect effect 1.03, 95%CI 1.00-1.06, proportion mediated 21%). There was no evidence that CRP was associated with LUTS, nor mediated the association between ACE score and LUTS. Interpretation: This study reports novel findings that could shed light on the biological mechanisms that underlie the link between ACEs and LUTS. Lower Urinary Tract Symptoms in Older Chinese American Women: Prevalence and Risk Factors

Abstract:The aim was to examine the prevalence of and risk factors for lower urinary tract symptoms (LUTS) in a community-dwelling cohort of older Chinese American women. We performed a secondary analysis of a prospective cross-sectional population-based survey of community-dwelling Chinese Americans aged 60 and older in English, Mandarin, Cantonese, Taishanese, or Teochew between 2011 and 2013. A clinical Review of Systems (ROS) was used to assess LUTS, which included urinary frequency, urgency, burning or pain, blood in urine, and urinary incontinence. Of the 1,829 women queried, 28.6% reported LUTS. The cohort of women who reported LUTS had a mean ± SD age of 74.7 ± 8.7 years, with a BMI of 23.4 ± 3.7 kg/m2 and had lived in the USA for 21.5 ± 13.1 years. Nearly 90% earned less than $10,000 a year, 50% were married, and 50% used traditional Chinese medicine (TCM) once monthly or more. In comparison with women without LUTS, women with LUTS had significantly higher rates of medical comorbidities and a poorer perception of their general health and quality of life. In multivariate regression analysis, any LUTS were significantly associated with older age (aOR 1.03, 95% CI 1.01–1.04), TCM use (aOR 1.67, 95% CI 1.34–2.07), anxiety (aOR 1.45, 95% CI 1.02–2.06), depression (aOR 2.00, 95% CI 1.53–2.61), and a history of stroke (aOR 1.90, 95% CI 1.19–3.02). Lower urinary tract symptoms are common among older Chinese American women, particularly in those who are older, use TCM regularly, or report a history of anxiety, depression, or stroke.
Prevalence and risk factors of nocturnal enuresis among children ages 5-12 years in Xi'an, China: a cross-sectional study

Abstract:Background: Nocturnal enuresis (NE) has a negative impact on children's health and imposes a long-term burden on families. With economic development and cultural improvements, parents and medical professionals pay more attention to NE. The aim of this study was to investigate the prevalence and risk factors of NE among children ages 5-12 years in Xi'an, China. Methods: A stratified cluster sampling method was used to conduct a cross-sectional study of NE in 10 kindergartens and 20 primary schools in Xi'an. We used univariate analysis to compare the prevalences of characteristics such as gender, duration of disposable diaper (DD) use, toilet training onset time, daily living habits, academic performance, and family history of NE. Logistic regression analysis was used to calculate odds ratio and to determine risk factors of NE. Results: The study included 6568 children ages 5-12 years, of which 262 (3.99%) had NE. The prevalence rates of NE decreased with age, with the highest prevalence at age 5 (9.09% for boys; 6.03% for girls). However, the prevalence increased with duration of DD use. Children experienced more NE if they never accepted toilet training (7.83%) or if they drank sugary beverages during the day (5.36%). Sleep disorders, sweets intake, drinking low amounts of plain water during the day, and family history of NE, were statistically associated with NE. Conclusion: NE was closely associated with a family history of NE, being male, long-term use of DD, delayed toilet training, drinking sugary beverages and/or consuming little plain water, and sleep disorders. A supportive parental attitude towards NE and timely medical treatment can improve the quality of life of enuretic children.

Abstract:The possibility that controllable environmental factors such as passive smoking and non‐breast feeding contribute substantially to wheezing has implications for prevention. Effects of passive smoking and non‐breast feeding on wheezing in children aged 6 to 10 years were explored in a historical cohort study of 223 children. Family history of respiratory allergy or asthma, male sex, maternal smoking, and non‐breast feeding were significantly associated (p < 0.05) with wheezing in bivariate analysis. In multivariate loglinear analyses, predictors of wheezing included non‐breast feeding (p = 0.05, odds ration = 2.1), male sex (p< 0.03, odds ratio = 3.1), and family history of respiratory allergy (p < 0.03, odds ratio = 2.6). In a second model, predictors included an interaction of maternal smoking and family history (p < 0.005, odds ratio = 4.6) in addition to male sex and family history of respiratory allergy. In further exploration based on tabular analysis, maternal smoking appeared to increase wheezing among children in whom the family history of respiratory allergy was positive (p < 0.001). Among children in whom the family history of respiratory allergy was negative, non‐breast feeding appeared to increase wheezing (p = 0.01). Promotion of breast feeding and reduction of maternal smoking might reduce childhood wheezing.

Abstract:The objective is to review the literature related to lower urinary tract (LUT) conditions in children to conceptualize general practice guidelines for the general practitioner, pediatrician, pediatric urologist, and urologist. PubMed was searched for the last 15-year literature by the committee. All articles in peer-review journal-related LUT conditions (343) have been retrieved and 76 have been reviewed extensively. Prospective trials were few and the level of evidence was low. Most of the recommendations have been done by committee consensus after extensive discussion of literature reports. History taking is an integral part of evaluation assessing day- and nighttime urine and bowel control, urgency, and frequency symptoms. Exclusion of any neurogenic and organic cause is essential. Uroflowmetry and residual urine determination are recommended in all patients to evaluate bladder emptying. Urodynamic studies are reserved for refractory or complicated cases. Urotherapy that aims to educate the child and family about bladder and bowel function and guides them to achieve normal voiding and bowel habits should initially be employed in all cases except those who have urinary tract infections (UTI) and constipation. Specific medical treatment is added in the case of refractory overactive bladder symptoms and recurrent UTIs.Conclusion: Producing recommendations for managing LUTS in children based on high-quality studies is not possible. LUTS in children should be evaluated in a multimodal way by minimal invasive diagnostic procedures. Urotherapy is the mainstay of treatment and specific medical treatment is added in refractory cases.What is Known:• Symptoms of the lower urinary tract may have significant social consequences and sometimes clinical morbidities like urinary tract infections and vesicoureteral reflux. Relationship among Diet Habit and Lower Urinary Tract Symptoms and Sexual Function in Outpatient-Based Males with LUTS/BPH: a Multiregional and Cross-Sectional Study in China

Abstract:Objectives This study assessed the effect of diet habits on lower urinary tract symptoms (LUTS) and sexual function in Chinese men with LUTS/benign prostatic hypertrophy (LUTS/BPH). Setting Multicentre study conducted between July 2013 and December 2013 in 11 hospitals in 3 geographic regions in China. Participants Overall, participants with LUTS/BPH accounted for 61.4% (2584/4208) of the respondents, whose data were processed in the following statistical analysis. Primary and secondary outcome measures LUTS and sexual function were assessed based on the International Prostate Symptom Score (IPSS) and the International Index of Erectile Function 5 (IIEF-5) score. Prostate volume (PV) was determined by ultrasound. Results A total of 4208 participants met the inclusion criteria. The average age of the whole participants was 65.8±7.7 years. Overall, participants with LUTS/BPH accounted for 61.4% (2584/4208) of the respondents, whose data were processed in the following statistical analysis. Generally, prostate enlargement was greatest in south China. LUTS and male sexual dysfunction (MSD) were most severe in northwest China. Based on multivariable analysis, PV enlarged as the age (p<0.001), body mass index (BMI; p<0.001) and vegetable intake (p<0.001) increased. Age (p<0.001) and BMI (p<0.05) independently increased the IPSS. A higher level of education (p<0.001) and more frequent meat, fish and egg intake (p<0.05) decreased the IPSS. Age (p<0.001), BMI (p<0.001), low education level (p<0.05), vegetable intake (p=0.001), and milk and dairy product intake (p=0.001) decreased the IIEF-5 score. Conclusions In addition to factors including age, obesity and level of education, dietary habits and geographic difference might also play an important role in the variation of PV, LUTS and MSD for Chinese men with LUTS/BPH.

Abstract:Background: Even though occupational women have a high incidence of lower urinary tract symptoms (LUTS), which seriously affect their daily work life, few large scale sample studies have provided empirical evidence to support this phenomenon among female nurses in China. Consequently, this article investigated female nurses who was presupposed to have a high prevalence of LUTS, which adversely exposes their health and patient safety to these risks. Additionally, it is considered important to explore the factors associated with LUTS in female nurses for patient care safety and nurse bladder health practice. Objectives: The purpose of this study was to assess the incidence of LUTS and symptoms-related risk factors among female nurses, to provide evidence for the prevention and control of LUTS. Methods: An online survey recruiting 23,066 participants was carried out in a multicenter cross-sectional study in 42 hospitals from December 2020 to November 2022. Stepwise multivariate logistic regression analysis and nomogram were used to identify the factors associated with lower urinary tract symptoms. Besides, SPSS version 26.0, R version 4.2.2, and GraphPad Prism Version 8.3 software packages were used for statistical analysis. Results: Based on the completion rate of the questionnaire which was 84.1% (n = 19,393), it was found that among 19,393 female nurses, the prevalence of LUTS was 67.71% and this rate was influenced by age, Body Mass Index (BMI), marital status, years of working, menstrual status, mode of delivery, history of breastfeeding, history of miscarriage, history of alcohol and coffee or tea consumption ( p < 0.05). Interestingly, we also find that in addition to the above mentioned factors, anxiety, depression, and perceived stress were also related to LUTS in female nurses ( p < 0.05). Conclusion: Given the high prevalence of LUTS among female nurses and their potential influencing factors, female nurses should focus on their reproductive health and develop good lifestyle habits. Thus, nursing managers should provide a warm and harmonious work environment and sensitize female nurses to increase their awareness about the importance of drinking clean water and urinating during work in a hygienic environment.
