Abstract:Abstract Background The deformation of oral and maxillofacial region leads to not only the damage of morphology and function, but also a series of aesthetic and psychological problems, severely affecting the quality of life of patients. Oral tissue engineering refers to developing biomaterials for repair or regeneration, with the application of tissue engineering technologies. This has become an area of increasing prominence. Current biologically inert materials are insufficient to fulfill clinical requirements. Therefore, tissue-engineered biomaterials with bioactive, even bionic properties are desperately needed. Main body The complexity of the anatomy and the diversity of tissue types of oral and maxillofacial region pose great challenges to the regeneration, in the aspects of both biomaterials and manufacturing technologies. Biomaterials in clinical practice or research have evolved from natural materials to synthetic materials, from homogeneous materials to multiple composite materials. And now composite materials have increasingly demonstrated their advantages in terms of physicochemical and biological properties over conventional materials. In terms of manufacturing, traditional coating, sintering, and milling technologies can no longer satisfy the requirements for high-precision bionic structures of oral-tissue-engineering biomaterials. Scientists have turned to biofabrication technologies such as microfluidics and additive manufacturing. Short conclusion This review aims to summarize the noteworthy advancements made in biomaterials of oral tissue engineering. We outlined the current biomaterials and manufacturing technologies and focused on various applications of these materials that may be connected to clinical treatment and research. We also suggested the future direction of development for biomaterials in oral tissue engineering. In future, biomaterials characterized by precision, functionalization, and individualization will be manufactured through digital, microfluidic, and 3D printing technologies. Graphical abstract
Development and validation of a novel system that combines a new masticatory simulator and analysis method for modeling the human gummy candy masticatory process

Abstract:During mastication, food undergoes state and texture changes influenced by various mechanical properties, including compression and fracturing of the molar teeth, mixing with saliva, and oral temperature. Prior studies have explored mastication simulators, however, no studies have assessed the forces and duration applied to the molars by the food during bolus formation. In this study, we developed a novel system that integrates a masticatory simulator and analysis method to evaluate mechanical properties. We developed ORAL-MAPS which is equipped with 6-axis force sensor, pneumatic pressure control mechanism, vertical movement, molar-like module, artificial saliva injection unit, and temperature control apparatus. A gap exists between the upper and lower unit at the closest point, allowing the sensor to measure vertical upward force and duration from food, while compressed air provides constant downward pressure. We hypothesized a correlation between the total integrated muscle activity ratio obtained from the human masseter muscle electromyography (iEMG). We compared the normalized impulse obtained from ORAL-MAPS with the normalized total iEMG obtained from human studies with four different types of gummy candies. As a result, the normalized total impulse of gummy candies A, B, C, and D were 1.00 ± 0.00, 1.29 ± 0.06, 0.95 ± 0.00, and 0.39 ± 0.0, respectively. The normalized total iEMG of the same gummy candies were 1.00 ± 0.00, 1.23 ± 0.15, 0.98 ± 0.09, and 0.45 ± 0.07, respectively. Thus, no significant difference was observed between the normalized total impulse obtained in vitro and the normalized total iEMG values for masticating the gummy candies B, C, and D (p > .05).

Abstract:With a growing global population and ageing demographics, the food industry stands at a pivotal crossroads, necessitating bespoke solutions and groundbreaking innovations. In vitro experiments can help understanding food oral processing and formulating products meeting the specific needs of different populations. However, current in vitro models do not reproduce well human oral anatomy and tongue biomechanics, essential for assessing the behaviour of novel and texturized foods under physiologically relevant oral conditions. In response, we unveil a novel 3D biomimetic artificial mouth, showcasing a pneumatic multi-degree-of-freedom artificial tongue meticulously crafted to mirror the mechanical properties and wettability of the human tongue. This cutting-edge technology, featuring tongue surface papillae, is capable of performing lifelike movements. The comparison with in vivo data demonstrates that it accurately reproduces oral processing of three, vastly different, soft foods. Textural characteristics (firmness, adhesive and cohesive properties) and shear viscosities-measured at oral and oropharyngeal-relevant shear rates-of in vitro food boli closely mirrored those observed in vivo. This in vitro device presents unprecedented opportunities for studying the dynamics of food transformation in the mouth, to adapt texture towards food that can be swallowed with ease and to improve food palatability, accommodating specific health needs critical for older adults (e.g., reduced salivary secretion, tongue weakness or poor coordination).

Recent Patents on Mechanical Engineering

Abstract:Background: Masticatory robot is a bionic robot, which is the product of the combination of medical technology and robot technology, to simulate human chewing movement. It can be mainly used for rehabilitation training of patients with occlusal disorders, and detecting the texture of food. Objective: To provide an overview of recent masticatory robots and introduce their respective characteristics and development. Method: This paper reviews various representative patents related to masticatory robot. The structural characteristics, control characteristics, applications of the typical masticatory robots are introduced. Results: The differences of different types of masticatory robots are compared and analyzed, and the structural characteristics are concluded. The main problems in its development are analyzed, the development trend is foreseen, and the current and future research of the patents on masticatory robot is discussed. Conclusion: Masticatory robot has important practical significance, and plays an important role in promoting the technology innovation in medical and food industries. And further improvements are needed in the aspects of authenticity, intelligence and security of the masticatory robot. More patents on masticatory robot should be invented. Abstract:The human gastrointestinal microbiota plays a vital role in maintaining host health and preventing diseases, prompting the creation of simulators to replicate this intricate system. The Simulator of the Human Intestinal Microbial Ecosystem (SHIME®), a multicompartment dynamic simulator, has emerged as a pivotal in vitro model for studying the interactions and interferences within human gut microbiota. The continuous and real-time monitoring hallmarks, along with the programmatically flexible setup, bestow SHIME® mimicking the entire human intestinal ecosystem with high dynamics and stability, allowing the evaluation of various treatments on bowel microbiota in a con-trolled environment. This review outlines recent developments in SHIME technology, highlighting its applications in food and nutritional science, drug development, and gut health research. Additionally, the prospect of SHIME integrating with other advanced technologies is also discussed.

Abstract:The advancement in biomechanics motivated the development of biomimetic robots. In mastication, there are limited studies on a masticatory robot capable of reproducing the human mandibular trajectory. Available studies have suggested a reliable simulation of the developed masticatory robot based on human mandibular motion. However, there are limited literatures evaluating the robot’s performance and comparing its results to that of a human subject. Benchmarking and evaluating robotic performance with human subject data establishes an important concept which is essential for further development. Therefore, this article presents a methodology to evaluate masticatory performance of the developed masticatory robot and human subject during mastication. This methodology utilizes surface electromyography (SEMG) and Hill’s muscle model to estimate masticatory forces in a human subject during occlusion in mastication, and compares the relevant data to the in vitro simulation results measured from the masticatory robot. With this methodology in place, current limitations of the robot are identified and controls can be refined in further studies to provide more realistic results, aiming at providing more reliable and biologically accurate results.

A Mechanical Simulator of Tongue–Palate Compression to Investigate the Oral Flow of Non-Newtonian Fluids

Abstract:Understanding oral processing is of great interest for the food and drink industry, and for many groups of people who have difficulties in eating and drinking it is of critical importance for their health and quality of life. This paper describes a simulator which bridges a gap in experimental capability between idealized laboratory-based materials-characterization studies and human investigations, which lack detailed objective measurement. This novel in-vitro mechanical oral simulator was constructed to simulate the in-mouth compression of a bolus of food or thick drink between the tongue and hard palate. Tongue compliance was matched to physiological values. A stepper motor-controlled platform reproduces motion of the tongue to compress a fluid bolus against a palate, propelling it posteriorly. The palate surface contains five pressure transducers. Internal bolus flow is imaged using a laser-lit particle image velocimetry system at 1000 frames per second. Each component of the simulator was calibrated. Example results are included from a case study demonstrating differences in pressure-flow characteristics between two model food materials created with starch and gum. Consistent with the in-vivo studies, pressure was higher anteriorly (bolus tail) than posteriorly near ejection into the oropharyngeal space. Bolus transport, with intrabolus shear rates 040 s1, required relatively low tongue pressure: 0.51.4 kPa for both the boluses. However, clearing the bolus ($ < {\text{0.5}}\,{\text{mm}}$ residual coating) required much higher pressure (5.4 versus 1.7kPa), for example, starch-based material versus the gum-based sample despite both having equal apparent viscosity of 0.47Pa.s at 50 s1. The simulator has, thus, demonstrated the ability to quantify how rheologically different materials may behave differently within the mouth.

Improved single- and multi-contact life-time testing of dental restorative materials using key characteristics of the human masticatory system and a force/position-controlled robotic dental wear simulator

Abstract:This paper presents a new in vitro wear simulator based on spatial parallel kinematics and a biologically inspired implicit force/position hybrid controller to replicate chewing movements and dental wear formations on dental components, such as crowns, bridges or a full set of teeth. The human mandible, guided by passive structures such as posterior teeth and the two temporomandibular joints, moves with up to 6 degrees of freedom (DOF) in Cartesian space. The currently available wear simulators lack the ability to perform these chewing movements. In many cases, their lack of sufficient DOF enables them only to replicate the sliding motion of a single occlusal contact point by neglecting rotational movements and the motion along one Cartesian axis. The motion and forces of more than one occlusal contact points cannot accurately be replicated by these instruments. Furthermore, the majority of wear simulators are unable to control simultaneously the main wear-affecting parameters, considering abrasive mechanical wear, which are the occlusal sliding motion and bite forces in the constraint contact phase of the human chewing cycle. It has been shown that such discrepancies between the true in vivo and the simulated in vitro condition influence the outcome and the quality of wear studies. This can be improved by implementing biological features of the human masticatory system such as tooth compliance realized through the passive action of the periodontal ligament and active bite force control realized though the central nervous system using feedback from periodontal preceptors. The simulator described in this paper can be used for single- and multi-occlusal contact testing due to its kinematics and ability to exactly replicate human translational and rotational mandibular movements with up to 6 DOF without neglecting movements along or around the three Cartesian axes. Recorded human mandibular motion and occlusal force data are the reference inputs of the simulator. Experimental studies of wear using this simulator demonstrate that integrating the biological feature of combined force/position hybrid control in dental material testing improves the linearity and reduces the variability of results. In addition, it has been shown that present biaxially operated dental wear simulators are likely to provide misleading results in comparative in vitro/in vivo one-contact studies due to neglecting the occlusal sliding motion in one plane which could introduce an error of up to 49% since occlusal sliding motion D and volumetric wear loss V(loss) are proportional.

Abstract:Biomimetic materials are able to mimic the structure and functional properties of native tissues especially natural oral tissues. They have attracted growing attention for their potential to achieve configurable and functional reconstruction in oral medicine. Though tremendous progress has been made regarding biomimetic materials, significant challenges still remain in terms of controversy on the mechanism of tooth tissue regeneration, lack of options for manufacturing such materials and insufficiency of in vivo experimental tests in related fields. In this review, the biomimetic materials used in oral medicine are summarized systematically, including tooth defect, tooth loss, periodontal diseases and maxillofacial bone defect. Various theoretical foundations of biomimetic materials research are reviewed, introducing the current and pertinent results. The benefits and limitations of these materials are summed up at the same time. Finally, challenges and potential of this field are discussed. This review provides the framework and support for further research in addition to giving a generally novel and fundamental basis for the utilization of biomimetic materials in the future.

In vitro evaluation of a mechanical testing chewing simulator

Abstract:OBJECTIVES: In this in vitro study, the force-profile of a commonly used chewing simulator (Williytec, Munich, Germany) utilizing fixed weights for loading was evaluated.METHODS: Using piezoelectric force sensors the maximum and mean values of the applied forces were recorded in three different load configurations in order to determine the repetition accuracy in one test chamber. The variation in resulting forces of the eight test loading-chambers when using three different loads and descending speeds was explored.RESULTS: The simulator showed high load repetition accuracy between the different cycle rates. Significant differences (up to 38.2+/-0.4N) were observed between the different specimen chambers. In addition, the recorded loads were generally both higher (up to 137.5+/-0.4N) and lower than the nominal loads defined by the static weights. The extent of load variation at contact was highly dependent on the descending cross-speed and selected weight. Finally there were also ringing in the load profiles attributed to vibrations of the mechanical setup.SIGNIFICANCE: Studies using weight-controlled chewing simulators should consider these effects when reporting results. In addition, calibrations should be performed to check uniformity of tests conditions for each test chamber.

Usefulness of Fungiflora Y to detect fungus in a frozen section of allergic mucin

Abstract:Allergic fungal sinusitis is a non‐invasive disease characterized by pansinusitis with nasal polyps, with most causative agents being members of the dematiaceous fungi. Reported herein is the ninth Japanese case of allergic fungal sinusitis; an Alternaria species was culturally identified as the etiological agent. The present patient, a 32‐year‐old man, presented with multiple sinusitis and nasal polyps. Allergic mucin, which is a histological hallmark of this disease, was examined intraoperatively on frozen section. Although characteristic histology, including laminated masses of mucin and aggregates of eosinophils, was seen in HE‐stained sections, fungal hyphae were scarce and recognized merely as clear spaces. Because fluorescent stilbene derivatives seem useful for the rapid detection of fungi, Fungiflora Y, a commercially available staining kit that includes a fluorescent stilbene derivative, was applied to the sections, and hyphae were successfully visualized using a fluorescence microscope. This stain takes only several minutes to perform. It is concluded that Fungiflora Y should be used when fungal elements are hard to find on HE stain, especially in cases in which rapid detection of fungus is requested.

Abstract:Mastication is a vital human function and uses an intricate coordination of muscle activation to break down food. Collection of detailed muscle activation patterns is complex and commonly only masseter and anterior temporalis muscle activation are recorded. Chewing is the orofacial task with the highest muscle forces, potentially leading to high temporomandibular joint (TMJ) loading. Increased TMJ loading is often associated with the onset and progression of temporomandibular disorders (TMD). Hence, studying TMJ mechanical stress during mastication is a central task. Current TMD self-management guidelines suggest eating small and soft pieces of food, but patient safety concerns inhibit in vivo investigations of TMJ biomechanics and currently no in silico model of muscle recruitment and TMJ biomechanics during chewing exists. For this purpose, we have developed a state-of-the-art in silico model, combining rigid body bones, finite element TMJ discs and line actuator muscles. To solve the problems regarding muscle activation measurement, we used a forward dynamics tracking approach, optimizing muscle activations driven by mandibular motion. We include a total of 256 different combinations of food bolus size, stiffness and position in our study and report kinematics, muscle activation patterns and TMJ disc von Mises stress. Computed mandibular kinematics agree well with previous measurements. The computed muscle activation pattern stayed stable over all simulations, with changes to the magnitude relative to stiffness and size of the bolus. Our biomedical simulation results agree with the clinical guidelines regarding bolus modifications as smaller and softer food boluses lead to less TMJ loading. The computed mechanical stress results help to strengthen the confidence in TMD self-management recommendations of eating soft and small pieces of food to reduce TMJ pain.

Abstract:The prevalence of prosthodontic treatment has been well recognized, and the need is continuously increasing with the ageing population. While the oral mucosa plays a critical role in the treatment outcome, the associated biomechanics is not yet fully understood. Using the literature available, this paper provides a critical review on four aspects of mucosal biomechanics, including static, dynamic, volumetric and interactive responses, which are interpreted by its elasticity, viscosity/permeability, apparent Poisson's ratio and friction coefficient, respectively. Both empirical studies and numerical models are analysed and compared to gain anatomical and physiological insights. Furthermore, the clinical applications of such biomechanical knowledge on the mucosa are explored to address some critical concerns, including stimuli for tissue remodelling (interstitial hydrostatic pressure), pressure–pain thresholds, tissue displaceability and residual bone resorption. Through this review, the state of the art in mucosal biomechanics and their clinical implications are discussed for future research interests, including clinical applications, computational modelling, design optimization and prosthetic fabrication.

Abstract:Soft gastric simulators are in vitro biomimetic modules that can reproduce the antral contraction waves (ACWs). Along with providing information concerning stomach contents, stomach simulators enable experts to evaluate the digestion process of foods and drugs. Traditionally, open-loop control approaches were implemented on stomach simulators to produce ACWs. Constructing a closed-loop control system is essential to improve the simulator's ability to imitate ACWs in additional scenarios and avoid constant tuning. Closed-loop control can enhance stomach simulators in accuracy, responding to various food and drug contents, timing, and unknown disturbances. In this article, a new generation of anatomically realistic soft pneumatic gastric simulators is designed and fabricated. The presented simulator represents the antrum, the lower portion of the stomach where ACWs occur. It is equipped with a real-time feedback system to implement diverse closed-loop controllers on demand. All the details of the physical design, fabrication, and assembly process are discussed. Also, the measures taken for the mechatronics design and sensory system are highlighted in this article. Through several implementation algorithms and techniques, three closed-loop controllers, including model-based and model-free schemes are designed and successfully applied on the presented simulator to imitate ACWs. All the experimental outcomes are carefully analyzed and compared against the biological counterparts. It is demonstrated that the presented simulator can serve as a reliable tool and method to scrutinize digestion and promote novel technologies around the human stomach and the digestion process. This research methodology can also be utilized to develop other biomimetic and bioinspired applications.

Dynamic finite element modelling of the macaque mandible during a complete mastication gape cycle

Abstract:Three-dimensional finite element models (FEMs) are powerful tools for studying the mechanical behaviour of the feeding system. Using validated, static FEMs we have previously shown that in rhesus macaques the largest food-related differences in strain magnitudes during unilateral postcanine chewing extend from the lingual symphysis to the endocondylar ridge of the balancing-side ramus. However, static FEMs only model a single time point during the gape cycle and probably do not fully capture the mechanical behaviour of the jaw during mastication. Bone strain patterns and moments applied to the mandible are known to vary during the gape cycle owing to variation in the activation peaks of the jaw-elevator muscles, suggesting that dynamic models are superior to static ones in studying feeding biomechanics. To test this hypothesis, we built dynamic FEMs of a complete gape cycle using muscle force data from in vivo experiments to elucidate the impact of relative timing of muscle force on mandible biomechanics. Results show that loading and strain regimes vary across the chewing cycle in subtly different ways for different foods, something which was not apparent in static FEMs. These results indicate that dynamic three-dimensional FEMs are more informative than static three-dimensional FEMs in capturing the mechanical behaviour of the jaw during feeding by reflecting the asymmetry in jaw-adductor muscle activations during a gape cycle. Abstract:Performance evaluation is indispensable for a surgical simulator to become acceptable. A haptics-based dental simulator (iDental) has been developed and preliminary user evaluation on its first-generation prototype has been carried out to gain the knowledge. Based on detailed requirement analysis of Periodontics procedures, a combined evaluation method including qualitative and quantitative analysis was designed. Construct validity was used to compare the performance difference between two groups of participants (faculty members and dental graduate students). These participants were required to perform three periodontal examination and treatment procedures including periodontal pocket probing, calculus detection, and removal. From the evaluation results, we found that penetration between tool and teeth or cheek will greatly decrease the fidelity of the simulation, therefore, it is necessary to utilize 6-DOF haptic device with both force and torque feedback in dental simulator, and accordingly it is needed to extend point-based rendering to 6-DOF haptic rendering of multiregion contacts. Furthermore, several other key research topics that will enable haptic technology to be effective in a practical dental simulator were identified, including simulation of deformable body such as tongue and gingival, and simulation of occlusion of tongue and cheek on teeth, etc.
An investigation into structural behaviors of skulls chewing food in different occlusal relationships using FEM

Abstract:Objectives: This study aims to investigate the effect of different occlusal relationships on skull structural and mechanical behaviors through simulation of chewing food. Methods: Finite element (FE) skull models of occlusion for Class I, end-on Class II, and full-cusp Class II were generated. End-on Class II and full-cusp Class II were chosen as mild and severe Class II occlusions, respectively. A simplified food bolus was introduced between the upper and lower dentition of the right molars. Chewing food was simulated in the skulls by moving the mandible. An experiment was conducted to measure strains at selective locations and compared them to the analytical results for validation. Results: In the early stages of mandibular movement, masticatory forces predicted from the skull models without food were lower than the skull models with food but increased drastically after occluding teeth full enough. As a result, the relationship between masticatory force and mandible movement shows that there is no significant difference between the skull models with food and without food in the range of human masticatory force, approximately 250 N. In all the cases of skulls including a food bolus, stress was similarly propagated from the mandible to the maxilla and concentrated in the same regions, including the mandibular notch and alveolar bone around the lower molars. Conclusion: It is predicted that there is no significant difference of bite force-mandible movement relationships and stress distributions of skull and teeth, between end-on Class II and full-cusp Class II models. When simulating chewing activities on candy and carrot, it is also found that there is no difference of masticatory performance between Class II occlusions, from structural as well as mechanical perspectives.
Prototyping a robotic dental testing simulator

Abstract:A parallel robot based on the Stewart platform is being developed to simulate jaw motion and investigate its effect on jaw function to test the wearing away of dental components such as individual teeth, crowns, bridges, full set of dentures, and implant-supported overdentures by controlling chewing motion. The current paper only describes the comparison between an alternative configuration proposed by Xu and the Stewart platform configuration. The Stewart platform was chosen as an ideal structure for simulating human mastication as it is easily assembled, has high rigidity, high load-carrying capacity, and accurate positioning capability. The kinematics and singularities of the Stewart platform have been analysed and software developed to (a) test the control algorithms/strategy of muscle movement for the six degree of freedom of mastication cycle and (b) simulate and observe various design options to be able to make the best judgement in product development. The human replica skull has been analysed and reverse engineered with further simplification before integration with the Stewart platform computer-aided design (CAD) to develop the robotic dental testing simulator. Assembly modelling of the reproduced skull was critically analysed for good occlusion in CAD environment. A pulse-width modulation (PWM) circuit plus interface was built to control position and speed of the chosen actuators. A computer numerical control (CNC) machine and wire-electro-discharge machining (wire EDM) were used to manufacture the critical parts such as lower mandible, upper maxilla, and universal joints.
Three-dimensional modeling of an individualized functional masticatory system and bite force analysis with an orthodontic bite plate

Abstract:Purpose Orthodontic tooth movement is affected by bite forces generated from the masticatory system. This study aims to study three-dimensional (3D) modeling of the individualized functional masticatory system and explore its application in orthodontics. Methods An individualized masticatory system model containing the craniomaxilla, mandible, 4 pairs of primary masticatory muscles and complete dentition, including roots and precise dental crowns, was developed using 3D images from spiral computed tomography and digital casts. By registering global coordinates and using data transformation, individual movement data for mandibular opening, lateral excursion and protrusion were recorded with an Arcus Digma system and applied to this model to simulate the functional movements of the mandible. Using the finite element method, deformations and displacement of the masticatory muscles were simulated along with the mandibular movements. Under individualized muscle loading, the bite forces of the lower incisors with the orthodontic bite plate were analyzed. Results Individualized mandibular movements were simulated, and the performance of the masticatory muscles along with the mandibular movements was measured. The bite force generated on the lower incisors with different thicknesses and the orientations of the orthodontic bite plate were acquired. Conclusion An individualized 3D masticatory system model was constructed using advanced 3D data processing software that integrated 3D images from different sources. Individualized mandibular movement and masticatory muscle performance were simulated using this model. The analysis of the bite force generated on the lower incisors with the orthodontic bite plate suggested that a thickness of 3 mm may be appropriate for clinical use.
