Abstract:OBJECTIVE:To analyze the clinical characteristics, diagnosis and treatment of pseudohypoparathyroidism (PHP).METHODS:The clinical data of 15 patients with pseudohypoparathyroidism (including 9 male and 6 female patients) admitted in our hospital between January, 1990 and July, 2011 were reviewed.RESULTS:The disease course of the patients ranged from 3 days to 21 years, and such symptoms of tetany and fatigue were found in all the patients. Most of the patients had a history of seizures. Laboratory tests suggested commonly low serum calcium, hyperphosphatemia, and parathyroid hormone (PTH) elevation. Head CT indicated multiple intracranial calcifications in 9 cases, and abnormal thyroid function was found in 4 cases. No specific treatment was available for this disease, and life-long calcium and vitamin D supplementation was advised to prevent acute attacks and disease progression.CONCLUSION:PHP is a rare genetic disease with a high rate of misdiagnosis in initial diagnosis. For repeated tetany and epileptic attacks and children with congenital developmental defects, examinations of blood calcium, phosphorus, and PTH and brain CT should be ordered as soon as possible. Long-term calcium and vitamin D supplementation is suggested for the treatment, and the presence of concomitant thyroid dysfunction or hypogonadism necessitates corresponding treatments.

Abstract:ObjectiveTo improve the clinical diagnosis of pseudohypoparathyroidism(PHP).MethodsClinical data obtained from 10 PHP patients admitted to our hospital from January 1990 to August 2008 were retrospectively reviewed.ResultsThe hospital stay time of ten cases was from 3 days to 9 years.The disease occurred at a younger age.The patients had a history of tetany and epileptic attacks.Laboratory test suggested hypocalcemia,hyperphosphatemia,elevated parathyroid hormone(PTH)level.CT scan of head showed cranial calcifications in 9 out of the 10 patients.Plain film of skeleton showed abnormalities in 6 patients.Only two patients had Albright's hereditary osteodystropliy(AHO)deformity,a major feature of PHP.Four patients had abnormal thyroid function.Two patients had persistent hypokalcemia.At present,no specific therapy is available for PHP.Treatment with calcium and active form of vitamin D can effectively prevent active attack of PHP and limit the irreversible changes.Thyroxine substitufive therapy is needed when hypothyroidism occurs with PHP.ConclusionCalcium,phosphorus,and PTH should be detected in patients with reoccurrence of tetany,epileptic attacks and AHO deformity.Brain CT should be performed.Long-term use of calcium and vitamin D can control the symptoms of PHP.It is necessary to detect the thyroid function of patients with PHP for the early treatment of PHP.
Multiple fractures due to hungry bone syndrome following parathyroidectomy: a clinical case report and review of literature

Abstract:Background: Hungry bone syndrome (HBS) is defined as prolonged hypocalcemia caused by a sudden decrease in parathyroid hormone (PTH) levels after parathyroidectomy (PTX). Multiple fractures after PTX due to HBS in an end-stage renal disease (ESRD) patient on chronic hemodialysis (HD) are challenging and rare medical conditions presented in this study. Case presentation: A 42-year-old ESRD patient on HD 3 times a week presented to Shariati Hospital, Tehran, Iran, complaining of worsening bone pain and loss of appetite. Laboratory data revealed an intact parathyroid hormone (iPTH) concentration of 2500 pg/mL, an alkaline phosphatase (Alp) level of 4340 IU/L, a phosphorus (P) level of 9 mg/dL, and a calcium (Ca) concentration of 7.2 mg/dL. Sestamibi scintigraphy revealed parathyroid adenoma. The findings suggested tertiary hyperparathyroidism (HPT-III), and the patient was scheduled for total PTX. Approximately one month after surgery, the patient was referred due to convulsions, leg mobility problems, and worsening bone pain. There was bilateral femoral ecchymosis. The Ca concentration was 5.8 mg/dL, and radiological evaluations revealed multiple skeletal fractures. HBS after PTX was suggested for this patient. After several days of hospitalization, he suffered subcutaneous emphysema followed by rib fractures and passed away. Conclusions: Multiple fractures after PTX due to HBS following HPT-III in ESRD patients are rare and demanding, highlighting the necessity of timely diagnosis and management of patients with HPT-III. Severe hypocalcemia following PTX can cause skeletal disorders. However, the surgical treatment of parathyroid adenomas may be more important than the risk of complications associated with bone health.

The molecular genetic mechanism of PHP is related to an inactivation mutation or an epigenetic modification of the GNAS gene, which encodes stimulating G protein α subunit (Gsα). 1 The change in the GNAS gene leads to a decrease in Gsα activity, so it cannot activate the downstream adenylate cyclase system to produce the second messenger cyclic adenosine monophosphate (cAMP). Finally, the inhibition of the Gsα/cAMP/protein kinase A (PKA) signaling pathway produces a series of pathological reactions. 2 The first symptoms are usually those of tetany due to low calcium, and some patients have epileptic seizures. In other cases, the first symptoms are skin heterotopic ossification, congenital hypothyroidism, or dwarfism. The main feature of the disease is the resistance of target organs (bone and kidney) to parathyroid hormone (PTH), which results in hypocalcemia, hyperphosphatemia, and elevated serum PTH concentration. Other clinical manifestations include ectopic calcification (especially in the basal ganglia and lens), ectopic ossification (most often in the dermis and subcutaneous fat), and Albright hereditary osteodystrophy (AHO). In some cases, resistance to other hormones such as thyroid-stimulating hormone (TSH), growth hormone–releasing hormone (GHRH), and follicle-stimulating hormone (FSH)/luteinizing hormone (LH) may also occur. 3,4 A 32-year-old woman was admitted to the neurology outpatient clinic of our hospital for "dizziness and headache for 2 weeks." She did not experience nausea and vomiting. Head computed tomography (CT) showed bilateral symmetrical high-density shadows in the basal ganglia that resembled those in metabolic lesions (Figure 1). The concentrations of biochemical parameters were as follows: total calcium, 2.18 (normal range, 2.11–2.52) mmol/L; inorganic phosphorus, 1.40 (normal range, 0.85–1.51) mmol/L; PTH, 231 (normal range, 12–88) pg/mL. She was then transferred to the endocrinology outpatient clinic and admitted to the hospital. Her medical history revealed that she had seen a doctor in another hospital 4 years ago because of facial and limb convulsions. At that time, the following parameters were measured: calcium, 1.43 (2.11–2.52) mmol/L; PTH, 439.70 (12–88) pg/mL; thyroxine, 84.70 (55.34–160.88) nmol/L; TSH, 5.96 (0.40–4.34) mIU/L; on that basis, the patient was diagnosed with "hypocalcemia" and "subclinical hypothyroidism." She was discharged with symptomatic treatment, including supplementation of calcium, vitamin D, and thyroid hormone. She usually took calcium tablets (details unknown) and did not undergo follow-up examinations. She denied any history of chronic disease and surgery but reported that her brother had a history of hypocalcemia. There was no genetic history in the family. Physical examination revealed the following: blood pressure, 100/75 mm Hg; height, 160 cm; weight, 55.5 kg; BMI, 21.7 kg/m2; clear mind, normal development, good nutrition; no abnormalities of the heart, lungs, and abdomen; no obvious abnormalities of the spine and limbs; Chvostek sign, Trousseau sign, and peroneal nerve reflex were all negative. Abstract:Background Pseudohypoparathyroidism (PHP) encompasses a highly heterogenous group of disorders, characterized by parathyroid hormone (PTH) resistance caused by mutations in the GNAS gene or other upstream targets. Here, we investigate the characteristics of a female patient diagnosed with PHP complicated with hypokalemia, and her family members. Case presentation and gene analysis A 27-year-old female patient occasionally exhibited asymptomatic hypocalcemia and hypokalemia during her pregnancy 1 year ago. Seven months after delivery, she experienced tetany and dysphonia with diarrhea. Tetany symptoms were relieved after intravenous calcium gluconate supplementation and she was then transferred to our Hospital. Laboratory assessments of the patient revealed hypokalemia, hypocalcemia and hyperphosphatemia despite elevated PTH levels. CT scanning of the brain revealed globus pallidus calcification. Possible mutations in GNAS and hypokalemia related genes were identified using WES, exon copies of STX16 were analized by MLPA and the methylation status of GNAS in three differential methylated regions (DMRs) was analyzed by methylation-specific polymerase chain reaction, followed by confirmation with gene sequencing. The patient was clinically diagnosed with PHP-1b. Loss of methylation in the A/B region and hypermethylation in the NESP55 region were detected. No other mutations in GNAS or hypokalemia related genes and no deletions of STX16 exons were detected. A negative family history and abnormal DMRs in GNAS led to a diagnosis of sporadic PHP-1b of the patient. Conclusions Hypokalemia is a rare disorder associated with PHP-1b. Analysis of genetic and epigenetic mutations can aid in the diagnosis and accurate subtyping of PHP.
[The role and importance of CT-guided stereotaxic brain biopsy in neurosurgery. Experiences with 523 cases].

Abstract:Between 1989 and 1996, 523 stereotactic biopsies of different intracranial lesions were performed at our institution. In 96.3% of the cases accurate histological diagnosis was made. In 59 cases the drainage of the abscess or cyst was carried out. In 48 cases the lesion was axial, in 27 parasellar, 7 pineal and 37 infratentorial. In the rest of the cases the lesion was in the supratentorial hemispheres. Transient neurological deficits were observed in 3.4% of the cases and craniotomy with haematoma evacuation had to be carried out in one case following the biopsy. There was no mortality associated with the interventions in our material. Our experience supports that CT guided biopsy is a safe and efficient method for obtaining histological diagnosis in different intracranial lesions and showed to be very useful in planning te appropriate treatment for each patient.

Abstract:Introduction Pseudohypoparathyroidism (PHP) is characterized by renal resistance to the effects of parathyroid hormone (PTH) resulting in hypocalcemia, hyperphosphatemia and elevated parathyroid hormone (PTH). Hypercalcemia in PHP has previously been reported to occur due to the development of tertiary hyperparathyroidism or due to over-replacement of calcitriol. This is the first reported case of parathyroid carcinoma in a patient with PHP. The presenting features were hypercalcemia and marked elevation of PTH over the course of 3 years. Clinical case A 47 year old woman with PHP type 1B presented for evaluation of hypercalcemia for the past 3 years. She was diagnosed with PHP at the age of 5 after a hypocalcemia seizure. At that time, she had hypocalcemia (5.5-6 mg/dl), hyperphosphatemia (9.3-9.7 mg/dl) and elevated PTH (2442 pg/ml, normal 160-360 pg/ml). Renal function was normal. Tubular reabsorption of phosphate was unchanged at 98% after an injection of PTH leading to the diagnosis of PHP. She had a long history of non-adherence to medication and multiple hypocalcemic seizures. She developed hypercalcemia (10.9 mg/dl), elevated PTH (1064.7 pg/ml, normal 12-88 pg/ml) and normalization of serum phosphate (2.8 mg/dl) at the age of 44 while on calcitriol 0.25 mcg daily. She remained hypercalcemic when calcitriol was stopped. Neck ultrasound revealed a 2.2 x 1.6 x 1.3 cm exophytic, hypoechoic nodule at the inferior pole of the left thyroid lobe with increased vascularity. It was irregularly marginated along the thyroid and had no clear border anteriorly along the strap muscles. Parathyroid exploration and total thyroidectomy was performed. Pathology revealed parathyroid carcinoma with multiple foci of vascular invasion and one focus of soft tissue extension. Post-operative PTH declined to 2.6 pg/ml and patient had recurrence of hypocalcemia (total calcium 6.5 mg/dl) due to which calcitriol was restarted. She developed hungry bone syndrome necessitating IV calcium for 2 days. Conclusion The initial presentation with spontaneous hypercalcemia and history of non-adherence with medication prompted concern for tertiary hyperparathyroidism but the PTH elevation was higher than in prior cases of tertiary hyperparathyroidism and had doubled over 3 years despite medical therapy with calcitriol raising concern for a neoplastic process. Rapid development of hypercalcemia in a patient with PHP should raise the suspicion for parathyroid carcinoma. PHP type 1B is associated with defects in the regulatory elements of GNAS1 whereas PHP type 1A is due to loss of function mutations in GNAS1. All cases of hypercalcemic hyperparathyroidism in PHP have been noted in PHP type 1B which indicates that the resistance to PTH in PHP type 1B is only relative and can be overcome when the PTH becomes markedly elevated. The PTH elevation was sufficient to excrete urinary phosphate and normalize the serum phosphate level.
Brown Tumors: The Hidden Face of Primary and Renal Hyperparathyroidism Amid Real-Life Settings

Abstract:Brown tumors, an exceptional bone complication of severe primary (PHP) or renal (secondary) hyperparathyroidism (RHP), are caused by long-standing, elevated parathormone (PTH)-induced osteoclast activation causing multinucleated giant cell conglomerates with hemosiderin deposits in addition to the local production of cytokines and growth factors. We aim to present an adult case series including two females displaying this complication as part of a multidisciplinary complex panel in high PTH-related ailments. The approach was different since they had distinct medical backgrounds and posed a wide area of challenges amid real-life settings, namely, a 38-year-old lady with PHP and long-term uncontrolled hypercalcemia (with a history of pregnancy-associated PHP, the removal of a cystic jaw tumor, as well as a family and personal positive diagnosis of polycystic kidney disease, probably a PHP-jaw tumor syndrome), as well as, a 26-year-old woman with congenital single kidney and chronic renal disease-associated RHP who was poorly controlled under dialysis and developed severe anemia and episodes of metabolic acidosis (including one presentation that required emergency hemodialysis and was complicated with convulsive seizures, followed by resuscitated respiratory arrest). Both subjects displayed a severe picture of PHP/RHP with PTH levels of >1000 pg/mL and >2000 pg/mL and elevated serum bone turnover markers. Additionally, they had multiple brown tumors at the level of the ribs and pelvis (asymptomatically) and the spine, skull, and pelvis (complicated with a spontaneous cervical fracture). As an endocrine approach, the control of the underlying parathyroid disease was provided via surgery in PHP (for the postparathyroidectomy hungry bone syndrome) via medical intervention (with vitamin D analogs) in RHP. Additionally, in this case, since the diagnosis was not clear, a multidisciplinary decision to perform a biopsy was taken (which proved inconclusive), and the resection of the skull tumor to confirm the histological traits. This series highlights the importance of addressing the entire multidisciplinary panel of co-morbidities for a better outcome in patients with PHP/RHP-related brown tumors. However, in the instance of real-life medicine, poor compliance and reduced adherence to recommendations might impair the overall health status. Thus, sometimes, a direct approach at the level of cystic lesion is taken into consideration; this stands for a narrow frame of decision, and it is a matter of personalized decision. As seen here, brown tumors represent the hidden face of PHP/RHP, primarily the complex and severe forms, and awareness is essential even in the modern era.
Case report: Familial hypoparathyroidism with elevated parathyroid hormone due to an inactivating PTH mutation

Abstract:Introduction: So far, only 11 PTH mutations have been described as causes of familial isolated hypoparathyroidism (FIH). In this report, we describe a family with FIH but with significant elevation of functionally inactive PTH due to a PTH mutation. We also show a positive therapeutic outcome of recombinant human PTH (teriparatide) therapy in one of the siblings who was not well controlled on large doses of calcitriol and calcium replacement therapy. Case description: The proband is a 34-year-old woman who has a history of chronic severe hypocalcemia (HypoCa) since birth. She and her three brothers (33-year-old male twins, and a 21-year-old male) were diagnosed with pseudohypoparathyroidism type 1b (PHPT 1b) based on the presence of chronic HypoCa (serum Ca 1.6-1.85 mmol/l) since birth associated with significantly elevated plasma PTH levels in the range of 310-564 pg/dl (normal range 10-65) and absence of signs of Albright hereditary osteodystrophy. Molecular studies: WES showed no pathogenic, likely pathogenic or variants of unknown significance in any known calcium-associated genetic disorder but a bi-allelic variant in the PTH itself ((NM_000315.4:c.128G>A, p.Gly43Glu). This was confirmed by Sanger sequencing in the patient and her affected brothers. Management: Because the patient's HypoCa was not controlled on large doses of calcitriol and calcium carbonate, a trial of teriparatide 20 mcg SC daily was started and resulted in normalization of calcium, decline in PTH levels and significant improvement in her general wellbeing. Conclusion: High PTH in the presence of congenital hypocalcemia is not always due to receptor or post-receptor defect and can be due to a biologically inactive mutated PTH. In such cases, treatment with teriparatide may result in stabilization of biochemical profile and improvement in quality of life.

Abstract:Introduction: Pseudohypoparathyroidism (PHP) indicates a group of rare disorders characterized by end-organ resistance to various hormones, primarily parathyroid hormone (PTH). One of its most common type is PHP-Ia, which is caused by maternally inherited inactivating mutations inGNAS.In this report, we present a Chinese girl with typical features of PHP-Ia and a novel mutation of theGNASgene. Patient concerns: A 9-year-old Chinese girl presented with recurrent epileptic seizure. Diagnosis: Biochemical and imaging findings were consistent with PHP-Ia, including typical Albright hereditary osteodystrophy phenotype (short stature, round face, brachydactyly, and mild mental retardation), PTH resistance (hypocalcemia, hyperphosphatemia, elevated serum PTH, and multiple intracranial calcification) and thyroid stimulating hormone resistance (elevated serum thyroid stimulating hormone). Interventions: The patient was given 1 alpha-hydroxylated vitamin D (calcitriol, 0.5 ug/d), calcium carbonate and vitamin D(3)tablets (1.5 g/d, including 600 mg calcium and 125 IU vitamin D-3). DNA analysis of theGNASgene was performed for the whole family. Outcomes: Investigation of theGNASgene revealed a novel mutation c.313delG (p.Glu105Lysfs*7) in the patient, as well as her mother. So the diagnosis of PHP-Ia was confirmed. Conclusion: The study further expands the spectrum of knownGNASmutations associated with PHP and lay emphasis on the genetic analysis ofGNASgene for identifying genetic abnormalities as well as making diagnosis and differentiation of various subtypes of PHP.
Unravelling a hidden pathology of a vertebral fracture in a teenage girl

Abstract:Background: Although the skeleton remains a common target of primary hyperparathyroidism, the classic bone disease "osteitis fibrosa cystica" is currently rare due to early diagnosis. This case represents severe classic bone manifestations of primary hyperparathyroidism due to delayed diagnosis and delayed medical attention. Case presentation: A 19-year-old young female was symptomatically managed for chronic back pain and nonspecific bone pain in the small joints of both hands over 2 months by a general practitioner. The patient had delayed seeking for treatment for 3 months. Later, she was evaluated for tuberculosis, hematological malignancies and rheumatic disorders following a fractured T12 vertebra and underwent pedicle screw fixation. However, clinical examination and investigations, including biochemistry, imaging and histology, ruled out the above conditions. Unfortunately, serum calcium level was not performed at the initial presentation. Later, primary hyperparathyroidism was diagnosed on the basis of moderate hypercalcaemia and elevated intact PTH levels (2064 pg/ml). She had sufficient vitamin D levels and normal kidney function. Her DXA scan revealed severe secondary osteoporosis with the lowest Z score of -8 at the total lumbar spine. Ultrasonography of the thyroid revealed a hypo echoic mass in the left lower neck, and localization studies with technetium-99 m sestamibi and 4D-CT revealed a left inferior parathyroid adenoma (1.6 × 1.5 × 1.6 cm). CT scan also revealed brown tumors in the mandible and vertebrae and diffuse bony changes in the skull, sternum, humerus and vertebrae. Her radiographs revealed subperiosteal bone resorption on the radial aspects of the middle and distal phalanges and brown tumors in both the ulna and fibula. We excluded MEN and other hereditary syndromes in our patient with a personal and family history and with a normal pituitary hormone profile because of poor resources for genetic testing. She underwent parathyroid adenoma excision, and the postoperative period was complicated with hungry bone syndrome, requiring high doses of calcium and active vitamin D supplements. These supplements were gradually weaned off over 6 months, and she recovered with normal biochemical investigations. Histology revealed parathyroid adenoma without malignant features. Conclusion: In developing countries where routine calcium screening is not available, clinicians should be aware of various manifestations of primary hyperparathyroidism to allow diagnosis as soon as possible without delay to prevent further progression, as it is a treatable condition.

Abstract:Objectives Pseudohypoparathyroidism type 1A (PHP1A) encompasses the association of resistance to multiple hormones, features of Albright hereditary osteodystrophy and decreased Gsα activity. Little is known about the early signs of PHP1A, with a delay in diagnosis. We report two PHP1A cases and their clinical and biochemical findings during a 20-year follow-up. Case presentation Clinical suspicion was based on obesity, TSH resistance and ectopic ossifications which appeared several months before PTH resistance, at almost 3 years of age. Treatment with levothyroxine, calcitriol and calcium was required in both patients. DNA sequencing of GNAS gene detected a heterozygous pathogenic variant within exon 7 (c.569_570delAT) in patient one and a deletion from XLAS to GNAS-exon 5 on the maternal allele in patient 2. In patient 1, ectopic ossifications that required surgical excision were found. Noticeably, patient 2 displayed adult short stature, intracranial calcifications and psychomotor delay. In terms of weight, despite early diagnosis of obesity, dietary measures were established successfully in both cases. Conclusions GNAS mutations should be considered in patients with obesity, ectopic ossifications and TSH resistance presented in early infancy. These cases emphasize the highly heterogeneous clinical picture PHP1A patients may present, especially in terms of final height and cognitive impairment.

Venous Thrombosis in a Pseudohypoparathyroidism Patient with a Novel GNAS Frameshift Mutation and Complete Resolution of Vascular Calcifications with Acetazolamide Treatment

Abstract:Introduction: Pseudohypoparathyroidism type IA (PHP1A) is characterized by end-organ resistance to multiple hormones and Albright's hereditary osteodystrophy (AHO). PHP1A is caused by inactivating mutations of the GNAS gene encoding the α-subunit of the stimulatory G protein (Gsα). In line with the underlying genetic defect, impaired inhibition of platelet aggregation has been demonstrated in some patients. However, no PHP1A case with thrombotic events has been described. Also, PHP1A cases typically have subcutaneous ossifications, but soft tissue calcifications are another common finding. Treatment options for those and other nonhormonal features of PHP1A are limited. Case presentation: A female patient presented with short stature, fatigue, and exercise-induced carpopedal spasms at age 117/12 years. Diagnosis of PHP1A was made based on hypocalcemia, hyperphosphatemia, elevated serum parathyroid hormone, and AHO features, including short stature and brachydactyly. A novel frameshift variant was detected in the last exon of GNAS (c.1065_1068delGCGT, p.R356Tfs*47), showing complete loss of baseline and receptor-stimulated activity in transfected cells. The patient developed venous thrombosis and vascular and subcutaneous calcifications on both forearms after venous puncture on the right and extravasation of calcium gluconate during treatment on the left. The thrombosis and calcifications completely resolved following treatment with low-molecular-weight heparin and acetazolamide for 5 and 8 months, respectively. Conclusions: This case represents the first PHP1A patient displaying thrombosis and the first successful use of acetazolamide for PHP1A-associated soft tissue calcifications, thus providing new insights into the treatment of non-endocrinological features in this disease.
Rare Presentation of Atypical Fractures in a Patient of Autoimmune Polyglandular Syndrome - A Case Report

Abstract:Introduction: An atypical bone fracture is a complete or partial discontinuity of bone which occurs without sufficient trauma. Autoimmune polyglandular syndromes are complex conditions characterized by the simultaneous presence of at least two autoimmune-mediated endocrinopathies. These syndromes exhibit considerable heterogeneity in their manifestations and typically unfold sequentially, with a significant time gap between the onset of the initial and subsequent glandular autoimmune diseases. In this article, we report a case of bilateral subtrochanteric atypical fractures in a patient with autoimmune polyglandular syndrome. This case scenario has not been reported in the literature so far to the best of our knowledge. Case report: A 37-year-old female patient presented at a tertiary care center in a metropolitan city with complaints of pain in both hips and the upper thigh. The patient faced difficulty walking and was unable to do so due to severe pain. Notably, there was no history of a fall or trauma. A radiographic examination revealed bilateral subtrochanteric atypical fractures. Upon further investigation, the patient was also found to be having hypothyroidism, hypoparathyroidism, pancytopenia, and an autoimmune disorder. The patient received a diagnosis of Autoimmune polyglandular syndrome with bilateral subtrochanteric atypical fractures. Treatment involved a comprehensive approach, including thyroxine for hypothyroidism, low-dose steroids, immunosuppressive agents, calcium supplementation, parathyroid hormone, and transfusion of blood and blood products. Due to the fractures being extra-articular, incomplete, and undisplaced, a conservative management approach was adopted. Conclusion: The occurrence of atypical and pathological fractures, particularly when bilateral or recurrent, necessitates a comprehensive evaluation to determine the root cause. It is imperative to address the underlying etiology thoroughly in every patient. Neglecting to do so may lead to a recurrence or insufficient resolution of the pathology. In instances where two or more endocrine organs are involved, consideration should be given to autoimmune polyglandular syndrome as a potential diagnosis.

Abstract:This case report describes a 52-year-old patient presenting with recurrent episodes of pancreatitis and renal stones. Further investigation revealed hypocalcemia and elevated parathyroid hormone (PTH) levels, leading to diagnosis of a parathyroid adenoma. This case highlights the importance of considering primary hyperparathyroidism in patients with recurrent pancreatitis and renal stones, as early diagnosis and surgical intervention can prevent recurrence and reduce morbidity. Abstract: Primary Hyperparathyroidism secondary to Parathyroid adenoma, rarely presents as acute pancreatitis. A 38-year-young male with a history of recurrent renal stones referred from a local center, presented to the emergency services, with a diagnosis of acute pancreatitis and bilateral renal stones. Laboratory evaluation showed an elevated calcium level, elevated PTH levels, low vitamin D, and low phosphorus levels. CT scan done outside was suggestive of acute pancreatitis along with bilateral renal calculi. USG neck and MIBI scan done as a part of hypercalcemia evaluation showed presence of a right parathyroid adenoma. Parathyroid adenoma was later removed, and calcium and parathyroid levels were normal on subsequent follow ups.

Abstract:Abstract Zoledronic acid is a very effective (IV) amino bisphosphonate which is indicated in osteoporosis, hypercalcemia of malignancy, multiple myeloma, Paget’s disease, and bone metastases from solid tumors. Bisphosphonate inhibits bone resorption through actions on osteoclast activity resulting in increasing bone density. Unfortunately, there are side effects associated with zoledronic acid one of those is mild to moderate hypocalcemia. Hungry Bone Syndrome (HBS) is defined as a severe drop in calcium levels less than 2.1 mmol/L and/or prolonged hypocalcemia for more than 4 days post parathyroidectomy. Most seen in patients’ who have secondary hyperparathyroidism compared to primary hyperparathyroidism. The sudden drop of parathyroid (PTH) levels post parathyroid resection after a prolonged duration of high PTH levels causes net calcium to move into the bone, this is the most proposed hypothesis. The authors report a case of a 32 years old female known case of severe osteoporosis presented to the emergency department complaining of left hip pain after a short height fall admitted as a case of pathological fracture. Furthermore, upon admission was vitally stable although on examination the patient had left side hip pain and tenderness otherwise unremarkable. Initial lab investigation showed a calcium level of 2.57 mmol/L and PTH level 37 pmol/L otherwise electrolytes and full blood count were unremarkable. Radiological investigations were done, and an X-ray has shown a left hip fracture where the patient underwent an operating room for fixation. In addition, a 99mTc-HDP bone scan revealed hyperparathyroidism and brown tumor. 99m Tc SESTA-MIBI whole body scan suggested metabolic bone disease. Furthermore, the parathyroid showed suggested typical parathyroid adenoma below and lateral to the lower pole of the left thyroid lobe. The ultrasound of the thyroid showing a well-defined hypoechoic nodule 2.5x1.15 cm with significant hypervascularity is seen, which is most likely to be enlarged left parathyroid gland. Thus, diagnosis of primary hyperparathyroidism has been reached with parathyroid adenoma which was resected during a hospital stay. The patient received pre-op zoledronic acid as a treatment for osteoporosis prior to the hip operation. After the parathyroid adenoma resection, the patient had a sudden drop of PTH and developed hypocalcemia which was 1.1 mmol/L. Thus, the complication of post operated parathyroid adenoma of HBS diagnosis has been made. Interestingly the patient needed to increase the length of stay of the hospital due to refractory hypocalcemia where the patient has been receiving IV calcium and vitamin D supplementation which hadn’t improved until a month of IV calcium gluconate. Although, the patient didn’t develop any signs or symptoms of hypocalcemia.

Abstract:Background: Primary hyperparathyroidism, while increasing the susceptibility to osteoporosis, also amplifies the potential for fractures in vulnerable areas such as the femoral neck. It can also serve as an infrequent etiological factor behind vertebral compression fractures. Case report: This report discusses a case of multiple acute non-traumatic vertebral compression fractures in a patient diagnosed with primary hyperparathyroidism. The patient, a 79-year-old female with osteopenia (T Score -2.0, medically treated), had a history of left breast cancer treated with a partial mastectomy and radiation therapy. She presented with midline back pain resulting from T12 and L2 compression fractures and underwent balloon kyphoplasty. A week later, she reported severe low back pain, despite the absence of any new traumatic event. Repeat imaging showed multiple new, acute compression fractures at T10, T11, L1, and L3. Further workup revealed elevated parathyroid hormone levels and hypercalcemia, leading to a diagnosis of hyperparathyroidism. Conclusion: Multiple acute non-traumatic vertebral body compression fractures due to hyperparathyroidism is an uncommon clinical manifestation. This case emphasizes the need for an extended work-up of secondary osteoporosis in patients who experience multiple vertebral compression fractures.
