Abstract:Introduction: It is challenging at baseline to predict when and which individuals who meet criteria for mild cognitive impairment (MCI) will ultimately progress to Alzheimer's disease (AD) dementia. Methods: A deep learning method is developed and validated based on MRI scans of 2146 subjects (803 for training and 1343 for validation) to predict MCI subjects' progression to AD dementia in a time-to-event analysis setting. Results: The deep learning time-to-event model predicted individual subjects' progression to AD dementia with a concordance index (C-index) of 0.762 on 439 ADNI testing MCI subjects with follow-up duration from 6 to 78 months (quartiles: [24, 42, 54]) and a C-index of 0.781 on 40 AIBL testing MCI subjects with follow-up duration from 18-54 months (quartiles: [18, 36,54]). The predicted progression risk also clustered individual subjects into subgroups with significant differences in their progression time to AD dementia (p<0.0002). Improved performance for predicting progression to AD dementia (C-index=0.864) was obtained when the deep learning based progression risk was combined with baseline clinical measures. Conclusion: Our method provides a cost effective and accurate means for prognosis and potentially to facilitate enrollment in clinical trials with individuals likely to progress within a specific temporal period.

Abstract:Mild cognitive impairment (MCI) is an intermediate stage between healthy aging and Alzheimer's disease (AD), and AD is a progressive neurodegenerative disorder that affects around 50 million people worldwide. As new AD treatments begin to be developed, one key goal of AD research is to predict which individuals with MCI are most likely to progress to AD over a given interval (such as 2 years); if successful, these individuals could be preferentially enrolled in drug trials that aim to slow AD progression. Here we benchmarked a range of MCI-to-AD predictive models including linear regressions, support vector machines, and random forests, using predictors from anatomical and diffusion-weighted brain MRI, age, sex, APOE genotype and standardized clinical scores. In evaluations on 2,448 subjects (1,132 MCI, 883 healthy controls, 433 with dementia) from the ADNI study, models including PCA-compacted features achieved a balanced accuracy of 75.3% (using cortical features) and 89.7% using diffusion MRI measures on test set, suggesting the added prognostic value of microstructural metrics obtainable with diffusion MRI.

Abstract:The number of service visits of Alzheimer's disease (AD) patients is different from each other and their visit time intervals are non-uniform. Although the literature has revealed many approaches in disease progression modeling, they fail to leverage these time-relevant part of patients' medical records in predicting disease's future status. This paper investigates how to predict the AD progression for a patient's next medical visit through leveraging heterogeneous medical data. Data provided by the National Alzheimer's Coordinating Center includes 5432 patients with probable AD from August 31, 2005 to May 25, 2017. Long short-term memory recurrent neural networks (RNN) are adopted. The approach relies on an enhanced "many-to-one" RNN architecture to support the shift of time steps. Hence, the approach can deal with patients' various numbers of visits and uneven time intervals. The results show that the proposed approach can be utilized to predict patients' AD progressions on their next visits with over 99% accuracy, significantly outperforming classic baseline methods. This study confirms that RNN can effectively solve the AD progression prediction problem by fully leveraging the inherent temporal and medical patterns derived from patients' historical visits. More promisingly, the approach can be customarily applied to other chronic disease progression problems.

Abstract:Aims: To investigate the hypothesis that commonly occurring bacterial toxins cause sudden infant death syndrome (SIDS) by (1), determining in which tissues bacterial toxins are concentrated after intravenous injection in rats; and (2), seeing if the same tissues contain detectable toxins in cases of SIDS. Methods: The tissue distribution of intravenously injected staphylococcal enterotoxin A (SEA), enterotoxin B (SEB), enterotoxin C (SEC), enterotoxin D (SED), toxic shock syndrome toxin (TSST-1), and a-haemolysin was studied in rats using immunohistology and polyacrylamide gel electrophoresis with immunoblotting. Immunostaining was also carried out on formalin fixed kidneys from cases of SIDS and a comparison series of necropsy cases using anti-SEA, anti-SEB, anti-SEC2 and anti-SED. Results: Immunohistology showed that SEB, SEC, SED and TSST-1 were all concentrated in the proximal convoluted tubular cells ofthe kidney. The presence of these toxins was confirmed in kidney homogenates using electrophoresis and immunoblotting. There was positive granular staining in the proximal convoluted tubular cells of the kidney in 36% of SIDS cases and 12% of the comparison series with anti SEC2 (X2 = 6; p < 0.025). Conclusion: SEC, or a bacterial toxin with epitopes in common, could have a pathogenic role in SIDS. Deep Recurrent Model for Individualized Prediction of Alzheimer's Disease Progression

Abstract:Alzheimer's disease (AD) is known as one of the major causes of dementia and is characterized by slow progression over several years, with no treatments or available medicines. In this regard, there have been efforts to identify the risk of developing AD in its earliest time. While many of the previous works considered cross-sectional analysis, more recent studies have focused on the diagnosis and prognosis of AD with longitudinal or time series data in a way of disease progression modeling (DPM). Under the same problem settings, in this work, we propose a novel computational framework that can predict the phenotypic measurements of MRI biomarkers and trajectories of clinical status along with cognitive scores at multiple future time points. However, in handling time series data, it generally faces with many unexpected missing observations. In regard to such an unfavorable situation, we define a secondary problem of estimating those missing values and tackle it in a systematic way by taking account of temporal and multivariate relations inherent in time series data. Concretely, we propose a deep recurrent network that jointly tackles the four problems of (i) missing value imputation, (ii) phenotypic measurements forecasting, (iii) trajectory estimation of the cognitive score, and (iv) clinical status prediction of a subject based on his/her longitudinal imaging biomarkers. Notably, the learnable model parameters of our network are trained in an end-to-end manner with our circumspectly defined loss function. In our experiments over TADPOLE challenge cohort, we measured performance for various metrics and compared our method to competing methods in the literature. Exhaustive analyses and ablation studies were also conducted to better confirm the effectiveness of our method.

Forecasting the Progression of Alzheimer's Disease Using Neural Networks and a Novel Pre-Processing Algorithm

Abstract:Alzheimer's disease (AD) is the most common neurodegenerative disease in older people. Despite considerable efforts to find a cure for AD, there is a 99.6% failure rate of clinical trials for AD drugs, likely because AD patients cannot easily be identified at early stages. This project investigated machine learning approaches to predict the clinical state of patients in future years to benefit AD research. Clinical data from 1737 patients was obtained from the Alzheimer's Disease Neuroimaging Initiative (ADNI) database and was processed using the "All-Pairs" technique, a novel methodology created for this project involving the comparison of all possible pairs of temporal data points for each patient. This data was then used to train various machine learning models. Models were evaluated using 7-fold cross-validation on the training dataset and confirmed using data from a separate testing dataset (110 patients). A neural network model was effective (mAUC = 0.866) at predicting the progression of AD on a month-by-month basis, both in patients who were initially cognitively normal and in patients suffering from mild cognitive impairment. Such a model could be used to identify patients at early stages of AD and who are therefore good candidates for clinical trials for AD therapeutics.

Abstract:Alzheimer's disease (AD) is a neurological illness that causes cognitive impairment and has no known treatment. The premise for delivering timely therapy is the early diagnosis of AD before clinical symptoms appear. Mild cognitive impairment is an intermediate stage in which cognitively normal patients can be distinguished from those with AD. In this study, we propose a hybrid multimodal deep-learning framework consisting of a 3D convolutional neural network (3D CNN) followed by a bidirectional recurrent neural network (BRNN). The proposed 3D CNN captures intra-slice features from each 3D magnetic resonance imaging (MRI) volume, whereas the BRNN module identifies the inter-sequence patterns that lead to AD. This study is conducted based on longitudinal 3D MRI volumes collected over a six-months time span. We further investigate the effect of fusing MRI with cross-sectional biomarkers, such as patients' demographic and cognitive scores from their baseline visit. In addition, we present a novel explainability approach that helps domain experts and practitioners to understand the end output of the proposed multimodal. Extensive experiments reveal that the accuracy, precision, recall, and area under the receiver operating characteristic curve of the proposed framework are 96%, 99%, 92%, and 96%, respectively. These results are based on the fusion of MRI and demographic features and indicate that the proposed framework becomes more stable when exposed to a more complete set of longitudinal data. Moreover, the explainability module provides extra support for the progression claim by more accurately identifying the brain regions that domain experts commonly report during diagnoses.

Abstract:Clinical trials are increasingly focused on pre-manifest and early Alzheimer’s disease (AD). Accurately predicting clinical progressions from normal to MCI or from MCI to dementia/AD versus non-progression is challenging. Accurate identification of symptomatic progressors is important to avoid unnecessary treatment and improve trial efficiency. Due to large inter-individual variability, biomarker positivity and comorbidity information are often insufficient to identify those destined to have symptomatic progressions. Using only clinical variables, we aimed to predict clinical progressions, estimating probabilities of progressions with a small set of variables selected by machine learning approaches. This work updates our previous work that was applied to the National Alzheimer’s Coordinating Center (NACC) Uniform Data Set Version 2 (V2), by using the most recent version (V3) with additional analyses. We generated a user-friendly conversion probability calculator which can be used for effectively pre-screening trial participants.

Abstract:Objectives: Accurately predicting when patients with mild cognitive impairment (MCI) will progress to dementia is a formidable challenge. This work aims to develop a predictive deep learning model to accurately predict future cognitive decline and magnetic resonance imaging (MRI) marker changes over time at the individual level for patients with MCI. Methods: We recruited 657 amnestic patients with MCI from the Samsung Medical Center who underwent cognitive tests, brain MRI scans, and amyloid-β (Aβ) positron emission tomography (PET) scans. We devised a novel deep learning architecture by leveraging an attention mechanism in a recurrent neural network. We trained a predictive model by inputting age, gender, education, apolipoprotein E genotype, neuropsychological test scores, and brain MRI and amyloid PET features. Cognitive outcomes and MRI features of an MCI subject were predicted using the proposed network. Results: The proposed predictive model demonstrated good prediction performance (AUC = 0.814 ± 0.035) in five-fold cross-validation, along with reliable prediction in cognitive decline and MRI markers over time. Faster cognitive decline and brain atrophy in larger regions were forecasted in patients with Aβ (+) than with Aβ (−). Conclusion: The proposed method provides effective and accurate means for predicting the progression of individuals within a specific period. This model could assist clinicians in identifying subjects at a higher risk of rapid cognitive decline by predicting future cognitive decline and MRI marker changes over time for patients with MCI. Future studies should validate and refine the proposed predictive model further to improve clinical decision-making.

Abstract:Alzheimer's disease (AD) is a severe neurodegenerative disorder that usually starts slowly and progressively worsens. Predicting the progression of Alzheimer's disease with longitudinal analysis on the time series data has recently received increasing attention. However, training an accurate progression model for brain network faces two major challenges: missing features, and the small sample size during the follow-up study. According to our analysis on the AD progression task, we thoroughly analyze the correlation among the multiple predictive tasks of AD progression at multiple time points. Thus, we propose a multi-task learning framework that can adaptively impute missing values and predict future progression over time from a subject's historical measurements. Progression is measured in terms of MRI volumetric measurements, trajectories of a cognitive score and clinical status. To this end, we propose a new perspective of predicting the AD progression with a multi-task learning paradigm. In our multi-task learning paradigm, we hypothesize that the inherent correlations exist among: (i). the prediction tasks of clinical diagnosis, cognition and ventricular volume at each time point; (ii). the tasks of imputation and prediction; and (iii). the prediction tasks at multiple future time points. According to our findings of the task correlation, we develop an end-to-end deep multi-task learning method to jointly improve the performance of assigning missing value and prediction. We design a balanced multi-task dynamic weight optimization. With in-depth analysis and empirical evidence on Alzheimer's Disease Neuroimaging Initiative (ADNI), we show the benefits and flexibility of the proposed multi-task learning model, especially for the prediction at the M60 time point. The proposed approach achieves 5.6%, 5.7%, 4.0% and 11.8% improvement with respect to mAUC, BCA and MAE (ADAS-Cog13 and Ventricles), respectively.

Abstract:Alzheimer's disease (AD) is the most common age-related dementia. Mild cognitive impairment (MCI) is the early stage of cognitive decline before AD. It is crucial to predict the MCI-to-AD conversion for precise management, which remains challenging due to the diversity of patients. Previous evidence shows that the brain network generated from diffusion MRI promises to classify dementia using deep learning. However, the limited availability of diffusion MRI challenges the model training. In this study, we develop a self-supervised contrastive learning approach to generate structural brain networks from routine anatomical MRI under the guidance of diffusion MRI. The generated brain networks are applied to train a learning framework for predicting the MCI-to-AD conversion. Instead of directly modelling the AD brain networks, we train a graph encoder and a variational autoencoder to model the healthy ageing trajectories from brain networks of healthy controls. To predict the MCI-to-AD conversion, we further design a recurrent neural networks based approach to model the longitudinal deviation of patients' brain networks from the healthy ageing trajectory. Numerical results show that the proposed methods outperform the benchmarks in the prediction task. We also visualize the model interpretation to explain the prediction and identify abnormal changes of white matter tracts.

Abstract:Background: In recent years, researchers have focused on developing precise models for the progression of Alzheimer's disease (AD) using deep neural networks. Forecasting the progression of AD through the analysis of time series data represents a promising approach. Objective: The primary objective of this research is to formulate an effective methodology for forecasting the progression of AD through the integration of multi-task learning techniques and the analysis of pertinent medical data. Methods: This study primarily utilized volumetric measurements obtained through magnetic resonance imaging (MRI), trajectories of cognitive assessments, and clinical status indicators. The research encompassed 150 patients diagnosed with AD who underwent examination between 2020 and 2022 in Beijing, China. A multi-task learning approach was employed to train forecasting models using MRI data, trajectories of cognitive assessments, and clinical status. Correlation analysis was conducted at various time points. Results: At the baseline, a robust correlation was observed among the forecasting tasks: 0.75 for volumetric MRI measurements, 0.62 for trajectories of cognitive assessment, and 0.48 for clinical status. The implementation of a multi-task learning framework enhanced performance by 12.7% for imputing missing values and 14.8% for prediction accuracy. Conclusions: The findings of our study, indicate that multi-task learning can effectively predict the progression of AD. However, it is important to note that the study's generalizability may be limited due to the restricted dataset and the specific population under examination. These conclusions represent a significant stride toward more precise diagnosis and treatment of this neurological disorder.

Predicting Progression From Mild Cognitive Impairment to Alzheimer's Dementia With Adversarial Attacks

Abstract:Early diagnosis of Alzheimer's disease plays a crucial role in treatment planning that might slow down the disease's progression. This problem is commonly posed as a classification task performed by machine learning and deep learning techniques. Although data-driven techniques set the state-of-the-art in many domains, the scale of the available datasets in Alzheimer's research is not sufficient to learn complex models from patient data. This study proposes a simple yet promising framework to predict the conversion from Mild Cognitive Impairment (MCI) to Alzheimer's Disease (AD). The proposed framework comprises a shallow neural network for binary classification and a single-step gradient-based adversarial attack to find an adversarial progression direction in the input space. The step size required for the adversarial attack to change a patient's diagnosis from MCI to AD indicates the distance to the decision boundary. The patient's diagnosis at the next visit is predicted by employing this notion of distance to the decision boundary. We also present a potential application of the proposed framework to patient subtyping. Experiments with two publicly available datasets for Alzheimer's disease research imply that the proposed framework can predict MCI-to-AD conversions and assist in subtyping by only training a shallow neural network.
Estimating Dementia Onset: AT(N) Profiles and Predictive Modeling in Mild Cognitive Impairment Patients

Abstract:Background: Mild Cognitive Impairment (MCI) usually precedes the symptomatic phase of dementia and constitutes a window of opportunities for preventive therapies. Objectives: The objective of this study was to predict the time an MCI patient has left to reach dementia and obtain the most likely natural history in the progression of MCI towards dementia. Methods: This study was conducted on 633 MCI patients and 145 subjects with dementia through 4726 visits over 15 years from Alzheimer Disease Neuroimaging Initiative (ADNI) cohort. A combination of data from AT(N) profiles at baseline and longitudinal predictive modeling was applied. A data-driven approach was proposed for categorical diagnosis prediction and timeline estimation of cognitive decline progression, which combined supervised and unsupervised learning techniques. Results: A reduced vector of only neuropsychological measures was selected for training the models. At baseline, this approach had high performance in detecting subjects at high risk of converting from MCI to dementia in the coming years. Furthermore, a Disease Progression Model (DPM) was built and also verified using three metrics. As a result of the DPM focused on the studied population, it was inferred that amyloid pathology (A+) appears about 7 years before dementia, and tau pathology (T+) and neurodegeneration (N+) occur almost simultaneously, between 3 and 4 years before dementia. In addition, MCI-A+ subjects were shown to progress more rapidly to dementia compared to MCI-A- subjects. Conclusion: Based on proposed natural histories and cross-sectional and longitudinal analysis of AD markers, the results indicated that only a single cerebrospinal fluid sample is necessary during the prodromal phase of AD. Prediction from MCI into dementia and its timeline can be achieved exclusively through neuropsychological measures.

Abstract:Alzheimer's disease (AD) is a neurodegenerative disease and the most common form of dementia, affecting many millions around the world. Accurate prediction of AD is crucial for effective intervention. We develop a longitudinal data prediction framework based on functional data analysis to identify when an early prediction can reasonably be made. As the regional brain atrophy is related to AD progression, we fit our model to the longitudinal volumetric changes of five regions of interest (ROIs) quantified with MRIs: hippocampus (H), entorhinal cortex (EC), middle temporal cortex (MTC), fusiform gyrus (FG) and whole brain (WB). To evaluate the AD prediction based on each ROI and the combinations of some of them, we compare different choices by their accuracy, sensitivity, specificity and area under the curve (AUC) through training and testing procedures. The results show that these ROI volumes have prediction power as early as 3 years in advance. Among all the models, the overall sensitivity is around <span class="InlineEquation">\(80\%\)</span>, specificity is above <span class="InlineEquation">\(70\%\)</span>, accuracy is around <span class="InlineEquation">\(75\%\)</span> and AUC above <span class="InlineEquation">\(80\%\)</span>. Among all the ROIs, EC is the best predictor (with the AUCs above 0.83 for 1-year and 2-year advanced prediction), followed by MTC and hippocampus. We also find that the combination of H + EC + MTC is the best combination (with AUCs of 0.86 for 1-year, 0.85 for 2-year, and 0.82 for 3-year advanced prediction). The key finding is that the AUC of 1-year prediction is not much different from that of 3-year prediction. In other words, we can use 3-year advanced prediction.
Prediction of Progressive Mild Cognitive Impairment by Multi-Modal Neuroimaging Biomarkers

Abstract:For patients with mild cognitive impairment (MCI), the likelihood of progression to probable Alzheimer's disease (AD) is important not only for individual patient care, but also for the identification of participants in clinical trial, so as to provide early interventions. Biomarkers based on various neuroimaging modalities could offer complementary information regarding different aspects of disease progression. The current study adopted a weighted multi-modality sparse representation-based classification method to combine data from the Alzheimer's Disease Neuroimaging Initiative (ADNI) database, from three imaging modalities: Volumetric magnetic resonance imaging (MRI), fluorodeoxyglucose (FDG) positron emission tomography (PET), and florbetapir PET. We included 117 normal controls (NC) and 110 MCI patients, 27 of whom progressed to AD within 36 months (pMCI), while the remaining 83 remained stable (sMCI) over the same time period. Modality-specific biomarkers were identified to distinguish MCI from NC and to predict pMCI among MCI. These included the hippocampus, amygdala, middle temporal and inferior temporal regions for MRI, the posterior cingulum, precentral, and postcentral regions for FDG-PET, and the hippocampus, amygdala, and putamen for florbetapir PET. Results indicated that FDG-PET may be a more effective modality in discriminating MCI from NC and in predicting pMCI than florbetapir PET and MRI. Combining modality-specific sensitive biomarkers from the three modalities boosted the discrimination accuracy of MCI from NC (76.7%) and the prediction accuracy of pMCI (82.5%) when compared with the best single-modality results (73.6% for MCI and 75.6% for pMCI with FDG-PET).

Abstract:Disease progression modeling (DPM) using longitudinal data is a challenging machine learning task. Existing DPM algorithms neglect temporal dependencies among measurements, make parametric assumptions about biomarker trajectories, do not model multiple biomarkers jointly, and need an alignment of subjects' trajectories. In this paper, recurrent neural networks (RNNs) are utilized to address these issues. However, in many cases, longitudinal cohorts contain incomplete data, which hinders the application of standard RNNs and requires a pre-processing step such as imputation of the missing values. Instead, we propose a generalized training rule for the most widely used RNN architecture, long short-term memory (LSTM) networks, that can handle both missing predictor and target values. The proposed LSTM algorithm is applied to model the progression of Alzheimer's disease (AD) using six volumetric magnetic resonance imaging (MRI) biomarkers, i.e., volumes of ventricles, hippocampus, whole brain, fusiform, middle temporal gyrus, and entorhinal cortex, and it is compared to standard LSTM networks with data imputation and a parametric, regression-based DPM method. The results show that the proposed algorithm achieves a significantly lower mean absolute error (MAE) than the alternatives with p < 0.05 using Wilcoxon signed rank test in predicting values of almost all of the MRI biomarkers. Moreover, a linear discriminant analysis (LDA) classifier applied to the predicted biomarker values produces a significantly larger AUC of 0.90 vs. at most 0.84 with p < 0.001 using McNemar's test for clinical diagnosis of AD. Inspection of MAE curves as a function of the amount of missing data reveals that the proposed LSTM algorithm achieves the best performance up until more than 74% missing values. Finally, it is illustrated how the method can successfully be applied to data with varying time intervals.
Prediction of transition from mild cognitive impairment to Alzheimer's disease based on a logistic regression–artificial neural network–decision tree model

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Aim</h3><p>To develop a logistic regression model, artificial neural network (ANN) model and decision tree (DT) model for the progression of mild cognitive impairment (MCI) to Alzheimer's disease (AD) to compare the performance of the three models.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>A total of 425 patients with MCI were screened from the original cohort. The actual follow up included 361 patients, with AD as the outcome variable. Three kinds of prediction models were developed: a logistic regression model, ANN model and DT model. The performance of all three models was measured with accuracy, sensitivity, positive predictive value and area under the receiver operating characteristic curve.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>A total of 121 patients with MCI developed AD, and the average conversion rate was 9.49% per year. The ANN model had higher accuracy (89.52 ± 0.36%), area under the receiver operating characteristic curve (92.08 ± 0.12), sensitivity (82.11 ± 0.42%) and positive predictive value (75.26 ± 0.86%) than the other two models. The first five important predictors of the ANN model were, in order, ADL score, age, urine AD‐associated neuronal thread protein, alcohol consumption and smoking. For the DT model, they were age, activities of daily living score, family history of dementia, urine AD‐associated neuronal thread protein and alcohol consumption. For the logistic regression model, they were age, sex, activities of daily living score, alcohol consumption and smoking.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>The logistic regression, ANN and DT models performed well at predicting the transition from MCI to AD with ideal stability. However, the ANN model had the best predictive value. Abstract:This study aimed to address the issue of larger prediction errors existing in intelligent predictive tasks related to Alzheimer's disease (AD). A cohort of 487 enrolled participants was categorized into three groups: normal control (138 individuals), mild cognitive impairment (238 patients), and AD (111 patients) in this study. An improved multifeature squeeze-and-excitation-dilated residual network (MFSE-DRN) was proposed for two important AD predictions: clinical scores and conversion probability. The model was characterized as three modules: squeeze-and-excitation-dilated residual block (SE-DRB), multifusion pooling (MF-Pool), and multimodal feature fusion. To assess its performance, the proposed model was compared with two other novel models: ranking convolutional neural network (RCNN) and 3D vision geometrical group network (3D-VGGNet). Our method showed the best performance in the two AD predicted tasks. For the clinical scores prediction, the root-mean-square errors (RMSEs) and mean absolute errors (MAEs) of mini-mental state examination (MMSE) and AD assessment scale–cognitive 11-item (ADAS-11) were 1.97, 1.46 and 4.20, 3.19 within 6 months; 2.48, 1.69 and 4.81, 3.44 within 12 months; 2.67, 1.86 and 5.81, 3.83 within 24 months; 3.02, 2.03 and 5.09, 3.43 within 36 months, respectively. At the AD conversion probability prediction, the prediction accuracies within 12, 24, and 36 months reached to 88.0, 85.5, and 88.4%, respectively. The AD predication would play a great role in clinical applications.

A Novel Artificial Intelligence (AI) Method to Classify and Predict the Progression of Alzheimers Disease

Abstract:Purpose: The objective of this study was to develop a novel AI-ensembled network based on the most important features and affected brain regions to accurately classify and exhibit the pattern of progression of the stages of Cognitive Impairment (CI). Methods: We proposed a novel ensembled architecture, 3D ResNet-18 - RF (Random Forest), and used this network to categorize the stages of Alzheimers disease (AD). The residual unit (blocks of ResNet) was introduced to the 3D Convolutional Neural network (CNN) to solve the degradation problem. It was considered an innovative strategy since the combination with fine-tuning resulted in higher accuracy. This network was trained on selected features and affected brain regions. The structured magnetic resonance images (MRI) were collected from the Alzheimer's Disease Neuroimaging Initiative (ADNI) dataset, and the random forest was used for determining the importance of the features and affected regions from the parcellated 170 regions of interest (ROIs) using Atlas, automated anatomical labeling 3(AAL-3). This framework classified five categories of AD and detected the progression pattern. Results: The proposed network showed promising results with a 66% F-1 score, 76% sensitivity, and 93.5% specificity, which outperformed the performance of conventional methods for categorizing five categories. Ventral Posterolateral and Pulvinar lateral regions were the regions most affected, indicating the progression from early MCI to AD. The five-fold validation accuracy for the developed model was 60.02%. Conclusion: The results showed that the gray matter to white matter ratio was the most significant feature, which also accurately predicted the progression pattern. The performance metrics fluctuated with different hyperparameters, but they never exceeded 0.05% of the estimated results, indicating the validity and originality of the suggested methodology.
