Abstract:Purpose: The purpose of the study was to discover long-term effects of Problem-based learning (PBL) and Simulation Problem-based learning (S-PBL) on critical thinking, problem solving abilities, learning attitude, motivation, and learning satisfaction among nursing students at Cheju Halla College. These students were taking problem based learning and simulation as a problem based learning method with an integrated curriculum. Methods: This study used a pretest-posttest with repeated measure design. Data was collected using convenience sampling from the beginning of the 1st semester to the end of the 2nd year when the PBL and S-PBL were completed by those who were enrolled in the integrated nursing curriculum. One-hundred eighty-three surveys were collected and analyzed during the repeat data collection. Results: There we restatistically significant differences of critical thinking, problem solving abilities, learning attitude, motivation and satisfaction post PBL and S-PBL. Conclusion: This study contributes to our understanding of outcomes from the PBL and S-PBL approach. The students undertaking PBL and S-PBL demonstrated that they developed a more positive attitude about their educational experience. In addition, students' tendency to think critically and problem solve improved through the use of the PBL and S-PBL approach.

Abstract:BackgroundAcute care and critical care are among the most challenging tasks in nursing, which requires information, knowledge, and skills across multiple areas. Scenario simulations can teach nursing students how to respond to these challenges in a safe environment, which can also reduce the stress of acute and critical care prior to exposure to a clinical setting. However, few studies have examined whether scenario simulations of acute and critical care can improve the abilities of nursing students.ObjectivesTo examine the effects of acute and critical care scenario simulations for nursing students.DesignA quasi-experimental design.SettingA department of nursing at a university.ParticipantsA total of 88 senior nursing students enrolled in a course in acute and critical care nursing volunteered to participate.MethodsThe experience provided by scenario simulations was guided by the best practice standards of the International Nursing Association for Clinical Simulation and Learning, which recommends outcome measures include a change in knowledge, skills, and attitudes. Students completed three self-assessment instruments before and after completion of the course: simulation learning effectiveness, self-reflection and insight, and satisfaction with the simulation format. Comparisons of pre-test and post-test scores on the self-assessment instruments evaluated the effects of the simulation learning.ResultsPost-test scores for subscale of self-regulation for simulation learning effectiveness and insight were significantly higher compared with pre-test scores (t = −2.85, p < 0.01 and t = −5.23, p < 0.001, respectively). There was also a significant increase for learning satisfaction in post-test, compared with pre-test (t = −3.70, p < 0.001).ConclusionThe use of scenario simulations for teaching acute and critical care nursing improved self-regulation, insight and learning satisfaction for undergraduate nursing students.

Effects of Blended Simulation on Nursing Students’ Critical Thinking Skills: A Quantitative Study

Abstract:Introduction Critical thinking is regarded as imperative to healthcare quality and patient outcomes; therefore, effective strategies in nursing education are required to promote students’ critical thinking abilities, leading to their success in clinical work. Accordingly, simulation-based education has been suggested as a measure for achieving this goal. Objective The aim of this study was to explore whether a nursing education course with blended simulation activities (hands-on simulations with high-fidelity manikins and a web-based interactive simulation program) could increase nursing students’ critical thinking skills. Method A quasiexperimental, one-group pretest and post-test design was utilized. Data were collected through premeasurement and postmeasurement using a critical thinking questionnaire and were analyzed using paired sample t-tests, independent sample t-tests, and the nonparametric Wilcoxon signed-rank test. The effect size was calculated using Cohen's d formula. Results Sixty-one nursing students (57 women and four men, mean age 30 years) participated in the study. Findings of the paired sample t-test showed a significantly higher mean score for posteducation than pre-education, indicating a significant change in nurses’ critical thinking capabilities ( p < .001). The results for Cohen's d formula ( − 0.87) of the mean scores between pre-education and posteducation indicated a large effect size. The Wilcoxon signed-rank test also showed a statistically significant increase in the students’ critical thinking abilities between pre-education and posteducation measures ( p < .001). No statistically significant differences were found in the mean score according to age or sex. Conclusion This study concluded that blended simulation-based education can increase nursing students’ critical thinking capabilities. As a result, this study builds on the use of simulation as a measure for developing and promoting critical thinking abilities during nursing education.

Abstract:AimsTo examine the effects of a simulation-based nursing process educational program on nursing students' confidence in communication and foundational understanding of the nursing process.BackgroundThe nursing process is a core component of patient-centered nursing care that is underpinned by effective communication. However, learning how to engage patients and carry out the distinct steps of the nursing process can be challenging if students are not able to apply knowledge to practice. Simulation-based learning programs can provide an immersive opportunity for students to enhance their understanding of the nursing process as well as improve their communication skills.DesignA mixed methods randomized controlled trialMethodsTwo Fundamentals of Nursing classes were recruited from an university in northern Taiwan. The experimental group (n=58) engaged with the simulation-based nursing process educational program, which consisted of an online interactive animation and a standardized patient simulation. The control group (n=49) was assigned a case-study. Confidence in Communication surveys, assignment and examination scores, satisfaction questionnaires and post-simulation reflection logs were used for data collection. Data were analyzed using SPSS 22.0 and qualitative content analysis.ResultsBoth groups showed statistically significant improvement in Confidence in Communication (p<.001). The experimental group performed better on the assignment than the control group (p<.001). No significant difference was seen between either groups' examination performances. The animation's high usage rate (mean 5.91 interactions per-user) and the results of the satisfaction questionnaires indicate the program's positive reception from the experimental group. The following themes emerged: effective communication skills, problem solving, confidence, feeling prepared and novel learning experience.ConclusionThe results demonstrate that the simulation-based nursing process educational program increased students' self-confidence in communication and non-verbal communication skills as well as foundational understanding of how to carry out the nursing process. The use of interactive simulation-based learning experiences is recommended to improve nursing students' understanding of how to apply abstract concepts into practice.

Nurses need extensive knowledge, skills, and abilities. Problem-Based Learning (PBL) and Simulation-Based Learning (SBL) are effective student-centered methods. Which method is better for surgical nurse training? More research is needed to determine the best approach for undergraduate surgical nurse education. Purpose: To compare the impact of PBL and SBL on undergraduate nursing students' performance and improve learning outcomes in surgical nursing education. Methods: We used a pretest/post-test design with 318 nursing undergraduates randomly assigned to two groups. Participants completed three progressive scenarios focused on surgical nursing cases. Experts blindly reviewed video recordings using the 70-item Korean Nurses' Core Competence Scale (KNCCS) to assess performance. The 13-item Satisfaction and Self-confidence in learning Scale (SSS) measured learning confidence and satisfaction. SBL participants also completed the 16-item Educational Practices in Simulation Scale (EPSS) and 20-item Simulation Design Scale (SDS). Results: The study found significant positive effects on both groups, with noticeable improvements in post-test, retention, and follow-up test results ( P < 0.001). The SBL group showed higher competency levels in nurses ( P < 0.001). The Cohen's d and effect size (r) for various skills were as follows: clinical performance (0.84767 and 6.39023), critical thinking (0.31017 and 0.15325), professional attitude (0.85868 and 0.39452), and communication skills (1.55149 and 0.61294). The satisfaction and self-confidence of nurses were higher in the SBL group (4.53± 0.596; 4.47± 0.611) compared to the PBL group (4.32± 0.689; 4.25± 0.632) in all dimensions of SSS (all P < 0.05). The SBL group also scored high in simulation design and EPSS. However, improvements are needed in fidelity, objectives, information, and students' expectations. Conclusion: SBL and PBL improve nurses' core competence, satisfaction, and self-confidence. SBL is superior. This study promotes student-centered education, enhancing surgical nursing professionals' quality and ensuring future patient safety. Impact of simulation-based education on the performance assessment, knowledge retention and mentality of nursing students: A systematic review and meta-analysis (Preprint)

Abstract:BACKGROUND Simulation-based education is a new type of teaching method that is suitable for clinic-related majors. OBJECTIVE To determine its impact on clinical skill assessment, performance maintenance, and mentality of nursing students. METHODS After searching PubMed, Embase, Web of Science, The Cochrane Library, MEDLINE, and EBSCO database, we conducted a systematic collection of randomized controlled trials (RCTs) on the impact of simulation-based education on performance evaluation and mentality of nursing students. The retrieval time limit was from the establishment of the database to March 12, 2021. Meta-analysis was performed using RevMan5.4 software, and two researchers used ROB2.0 software to evaluate the risk of bias in the included literature. The quality evaluation of the outcome indicators was performed using GRADEpro. A standardized mean difference (SMD) with a 95% confidence interval (CI) was used in estimating the pooled effects of RCTs. RESULTS A total of 21 RCTs were performed, including 1683 nursing students. The results of meta-analysis showed that simulation-based education significantly improved the assessment scores of nursing students (SMD = 1.46, 95% CI = 1.02 to 1.90, P < 0.00001), self-confidence (SMD = 1.19, 95% CI = 0.48 to 1.90, P = 0.001), satisfaction (SMD = 0.86, 95% CI = 0.13 to 1.60, P = 0.02), and knowledge retention (SMD = 1.89, 95% CI = 0.76 to 2.87, P = 0.0008), and the difference was statistically significant. The results of subgroup analysis showed that long-term intervention (MD = 1.44, 95% CI = 0.62, 2.26, P = 0.0006) and short-term intervention (MD = 1.46, 95% CI = 0.95, 1.97, P < 0.00001) improved the performance assessment of nursing students, and the difference was statistically significant. CONCLUSIONS Simulation-based education can significantly improve the assessment scores, self-confidence, and satisfaction of the nursing students, and the scores can be effectively retained for a period of time. Limited by the quality of the number of included studies, the above conclusions need to be verified by high-quality studies.

Abstract:Aim: This study aimed to evaluate the effects of problem-based learning integrated with the nursing process on clinical reasoning, communication skills attitude and nursing process competency of nursing students. Background: Systematic and integrated application of nursing process in various nursing scenarios are necessary as an alternative to limited learning experiences during conventional lectured-based learning. Thus, an innovative nursing curriculum should be developed by adopting problem-based learning integrated with the nursing process, the effectiveness of this process should be tested to improve the quality of nursing education and explore effective educational strategies. Design: A quasi-experimental non-equivalent control group pre-test and post-test design. Methods: The study was conducted from September to October, 2023. Third-year nursing students who were selected by intentional sampling were allocated to the control (n=69) and experimental (n=87) groups. The control group participated in conventional lectures, whereas the experimental group participated in the problem-based learning integrated with the nursing process (six sessions) regarding various high-risk obstetrics and gynecology nursing. Thereafter, the students completed structured questionnaire on Google surveys including items on clinical reasoning, communication skills attitude and nursing process competency. Results: Clinical reasoning (t=-4.599, p=<.001), communication skills attitude (t=-1.848, p=.033) and nursing process competency (t=-3.795, p<.001) increased significantly more in the post-test for the experimental than the control group. Conclusions: This study shed light on the learning effects that enable effective communication and enhance confidence in nursing students for applying clinical reasoning and the nursing process based on various high-risk obstetrics and gynecology nursing modules. Moreover, learners are expected to experience knowledge expansion and transfer through reasoning and application. Therefore, modules of high-prevalence diseases should be developed for various nursing major courses and problem-based learning integrated with the nursing process linked to these modules should be designed for use in nursing classes.
Effectiveness of ethics case based on blended learning approaches on medical students' learning: A quasi-experimental study

Abstract:Background: The use of active teaching methods in ethics education, instead of being merely teacher centred, can lead students to problem-solving in a practical way. Therefore, this study was conducted with the aim of comprehensive medical ethics education based on educational approaches and the effect of this approach on students' learning and moral reasoning skills. Materials and methods: This quasi-experimental intervention study was performed on seventy physiopathology medical students who had taken the medical ethics course. The students' educational program included teaching theoretical concept of medical ethics, discussing ethical cases and clinical reasoning in class, and then discussing in forum environment and individual assignments in the learning management system (LMS) system and completing the portfolio. Student evaluation was associated students' knowledge, attitu de, and moral reasoning skills and learning. A comprehensive program including forum participation, individual assignments in LMS, theory and objective structured clinical examination (OSCE) tests, and portfolio completion was used to evaluation of students. The students' learning was compared to students of the previous year, which was conducted through a traditional teacher-centered method. Data analysis was conducted using IBM® SPSS® Statistics 21 at two levels of descriptive statistics and analytic statistics. Results: The results showed that there was a significant difference between the mean learning of students compared with traditional class (P < 0.05). Furthermore, comparison of students' practical scores through the OSCE test showed its impact on changing students' knowledge, attitude, and performance in moral reasoning. Conclusion: According to research findings, it seems that the mentioned model is one of the important and new issues as a suitable method of teaching medical ethics. Furthermore, it can be suitable model in other medical courses. The more research in multicentral may be need to assess other effects on students' learning indicators.

Abstract:Background This study investigated how final-year undergraduate nursing students’ critical thinking skills were impacted after attending a purposefully designed 15 hour program with nine hours of simulation-based learning activities aligned with a theoretical framework of clinical reasoning comprising the Clinical Reasoning Cycle and Debriefing for Meaningful Learning. Method A one-group pre-test post-test design was used. Fifty-six final semester undergraduate nursing students from five campuses of one Australian university participated in this study. Data were collected before and after the simulation program using the Health Sciences Reasoning Test. Results Analysis of data using paired sample t-tests demonstrated a positive change in critical thinking scores after completing the program (95% confidence interval 0.206-2.079). A total of 62.5% of participants achieved two scores higher after completing the simulation program. Conclusions Implementing a simulation program designed according to the Clinical Reasoning Cycle and using the Debriefing for Meaningful Learning model is associated with enhancing the critical thinking skills of nursing students.

Abstract:Introduction Utilizing simulation in nursing education is increasingly becoming a popular teaching pedagogy. Simulation provides opportunity to the nursing students to practice the clinical skills through various real life like situational experiences. Simulation improves the clinical decision making skills of the nursing students. Objective A descriptive, cross sectional, correlational study was conducted to evaluate the efficacy of simulation using NLN/Jeffries Nursing Education Simulation Framework on satisfaction and self-confidence of Undergraduate nursing students. Methods 370 undergraduate nursing students participated in the study. Educational practices, Simulation design, Student satisfaction and Self-confidence questionnaires were used. Results Majority of the nursing students perceived agreement on various domains of student satisfaction, self-confidence, educational practices and simulation design used in the curriculum. The students were self-confident to care for similar patients in the clinical area and satisfied with this instructional method. Satisfaction and self-confidence were correlated with educational practices and simulation design. Conclusion Developing simulations using NLN/Jeffries Nursing Education Simulation framework will provide meaningful, engaging, and pedagogically sound experiences for the students. Pre-briefing and de-briefing are meaningful strategies to enhance the learning of students.
Effects of using standardized patients on nursing students’ moral skills

Abstract:Background: Nurses and nursing students increasingly confront ethical problems in clinical practice. Moral sensitivity, moral reasoning, and ethical decision-making are therefore important skills throughout the nursing profession. Innovative teaching methods as part of the ethics training of nursing students help them acquire these fundamental skills. Aim: This study investigated the effects and potential benefits of using standardized patients in ethics education on nursing baccalaureate students’ moral sensitivity, moral reasoning, and ethical decision-making by comparing this method with in-class case analyses. Research design: This is a quasi-experimental study. Participants and research context: The sample comprised 89 students in Hacettepe University’s Faculty of Nursing. Following lectures describing the theoretical components of ethics, students were randomly assigned to two working groups, one using standardized patients and the other using in-class case analyses. Data were collected using the Moral Sensitivity Questionnaire, Rest’s Defining Issues Test, and the Nursing Dilemma Test. All data were analysed using IBM SPSS Statistics Version 23. Ethical considerations: Ethical approval and official permission were obtained. All participating students completed informed consent forms. Findings: According to the results, the moral sensitivity of students in the standardized patient group significantly improved over time compared to those in the case analysis group, while the mean scores of students in both groups for moral reasoning and ethical decision-making were not statistically significant. Conclusion: Based on our results, we recommend the use of both standardized patients and case analysis as appropriate teaching methods in ethics education.

Abstract:Background: Simulation-based training is a new strategy in teaching that gives the students good opportunities to learn and apply what they learn in nursing care safely. Aim: This study conducted to evaluate the effects of simulation-based training on nursing students’ communication skill, self-efficacy and clinical competence in practice. Subjects and Methods: Quiz-experimental design was used in this study (pre-posttest intervention), it was carried out on 100 nursing students first semester in 2019 using low and high-fidelity simulators. This study was carried out at College of Applied Medical Sciences-Bshia University. Data Collection: demographic data, communication skill, self-efficacy and clinical competence questionnaires. Analysis is done by SPSS version 20 software. Results: Participants who received the simulation-based training, showed statistical significant improvement in communication skill, self-efficacy, and clinical competence scores after participation in the simulation program (t = −32.64, p = 0.001; t = −19.9, p = 0.001; 16.4, p = 0.001). Also, there are significant relation between gender and clinical competency (t = 2.768, p < 0.05). Conclusion: Simulation-based training in medical courses is effective in enhancing communication skill, self-efficacy and clinical competence. Multiple-patient simulations as a teaching-learning strategy in the nursing curriculum are highly recommended.

Abstract:BACKGROUND: The nursing module teaches basic nursing principles and procedures to undergraduate nursing students. This course is important for the acquisition of procedural and psychomotor skills. The aim of the present study is to evaluate the influence of blended training on the acquisition of practical skills related to gastric insertion tubes. MATERIALS AND METHODS: A quasi-experimental study was undertaken, containing three groups: The blended group benefited from e-learning, followed by simulation), while the two groups benefited from theoretical teaching and procedural simulation. Then, the degrees of acquisition of declarative and procedural knowledge and the time of completion were measured. RESULTS: The declarative knowledge grade was higher in the Simulation (16.07) and Blended (15.21) groups than in the Traditional Group (11.66), with a statistically significant difference (P < 0.001). The results also showed a statistically significant difference between the procedural knowledge grades of the three groups (P < 0.001). A statistically significant correlation was detected between declarative and procedural knowledge scores (r = 0.58, P < 0.001) and a negative correlation between procedural knowledge and the time of completion (r = -0.422, P < 0.001). CONCLUSIONS: The results of this experimentation confirm the positive effect of the blended learning approach on the acquisition of declarative and procedural knowledge as well as the time management allocated to teaching.
Effects of simulation with problem-based learning (S-PBL) on nursing students' clinical reasoning ability: based on Tanner's clinical judgment model

Abstract:Background: Clinical reasoning ability, a complex cognitive and metacognitive process, is a crucial core competency required in nursing practice. Therefore, undergraduate nursing students should be provided with nursing education to strengthen their clinical reasoning ability based on real-life nursing scenarios. Methods: This study was conducted using a quasi-experimental single-group pretest-posttest design. Three sessions (lasting three hours each) of Simulation with Problem-Based Learning (S-PBL) using high-risk obstetrics-gynecology scenarios were provided to 71 third-year nursing students of a university. The sessions were conducted from September to December 2022, and they aimed to strengthen their clinical reasoning ability. For data collection, an online survey was conducted using Rubric for Clinical Reasoning and learning satisfaction evaluation tool. Data were analyzed using descriptive statistics and repeated measures analysis of variance in SPSS. Results: The mean score of clinical reasoning ability significantly increased from 29.42 (standard deviation: 4.62) out of 40 points in the pre-test to 32.28 (4.36), 33.44 (5.35), and 33.80 (5.91) after the first, second, and third S-PBL sessions, respectively (F = 61.668, p < .001). The learning satisfaction score was as high as 107.04 (12.66) out of 120 points. Conclusion: This S-PBL program is an effective nursing education strategy to strengthen nursing students' clinical reasoning ability. Future studies must examine learner variables and standardize the S-PBL design and operation process by comparison to a traditional teaching approach and a higher range of clincal reasoning ability.

Abstract:BackgroundIt is crucial to diversify the methods for clinical nurses to continue learning nursing ethics in that ethical decision-making ability in nursing is regarded as one of the core competencies in nursing practice.ObjectiveIn response to nurses' learning needs for nursing ethics, this study built an online learning platform for the pertinent topic, and tested its effect on improving nurses' ethical decision-making ability and critical thinking ability.DesignA qualitative method was adopted to develop interventions, which were designed as a teaching practice research consisting of a study group and a control group.ParticipantsA total of 93 nurses, including 20 interviewees and 30 in the study group and 43 in the control group.MethodQualitative methods were employed to understand the learning needs of nurses. The judgment about nursing decisions and the critical thinking disposition inventory, learning effectiveness questionnaire and learning software quality evaluation scale were used as research tools in this teaching practice research. The SPSS 25.0 was adopted to analyze data by paired sample t-test and independent sample t-test.ResultsIn the measurement of the judgment about nursing decisions scale, the study group scored higher than the control group. The critical thinking disposition inventory scale identified a similar result in the total score incorporating the four dimensions, including analyticity, systematicity, critical thinking self-confidence, inquisitiveness. There is a correlation between learning software quality and learning effect, with a correlation coefficient of 0.640.ConclusionThe online learning platform of nursing ethics built in this study has positive learning effects, and it demonstrates effectiveness to improve nurses' abilities in nursing ethics, decision-making and critical thinking. It is expected to be a viable way to improve the continuity of nurses' study of ethics.

Abstract:Objective To evaluate the effect of problem-based learning(PBL) strategy in classroom teaching of Medical Nursing course.Methods Totally 30 undergraduate nursing students received the PBL method in classroom teaching of nursing care of stroke patients.The students were divided into 4 groups and there were 2 tutors in each group.Questionnaire survey was used to evaluate the teaching effects in aspects of the tutors'evaluation on the students'performance in PBL,the students'evaluation on the PBL course and the students'evaluation on the tutors'performance in PBL.Results The tutors'evaluation on the students'performance in PBL improved(P=0.027).The students thought that the moderator of each group had outstanding performance,PBL contributed to the students'development in exploring and utilizing literature,critical thinking and communicative language competence.But the students thought that they lacked sufficient time and energy for adequate preparation.The scores of tutors'performance were satisfied in several aspects such as preparation and attitudes,communication and feedback,but relatively low in controlling the process.Conclusions PBL method is beneficial for nursing students to improve their comprehensive abilities.Nursing tutors need to further enhance their ability in controlling the PBL process.
