Abstract:Background: Primary cardiac tumors are rare. Myxoma is the most common cardiac tumor and presents in the left atrium. Improvements tend to occur with early diagnosis, and with the help of high-resolution imaging technology, such as transesophageal echocardiography (TEE). Patients typically present with tightness of the chest, fatigue and lassitude. There are also cases of misdiagnosis and missed diagnosis. Case presentation: A 59-year-old Chinese woman complained of tightness in her chest, fatigue and lassitude for eight years. She had been misdiagnosed with schizophrenia by the local rural hospital. Computed tomography (CT) confirmed a giant mass in the left atrium, and transthoracic echocardiography (TTE) revealed left atrial enlargement with an occupying lesion. For this patient, we performed tumor resection surgery with a cardiopulmonary bypass. The giant tumor was partially detached during the operation, and the thrombus was removed successfully. After the operation, the mental health department of our hospital thought that the patient's diagnosis of schizophrenia was misdiagnosed. The patient recovered well and appeared rejuvenated after the operation. A two-year follow-up found no adverse events since the operation. Conclusions: When making a diagnosis for a patient who presented with tightness of the chest, we need to administer a cardiac physical examination and echocardiography. The physician diagnosing schizophrenia needs to first rule out any organic diseases. A careful and complete resection of the cardiac myxoma is a vital operation, due to the risk of thromboembolism.

Abstract:A 24-year-old woman admitted with mild chest distress associated with activity without chest complaint for twenty days. Two orifices were visible at the level of the mitral valve with a transthoracic short-axis view of the two-dimensional and three-dimensional echocardiography. The left ventricle was mildly dilatated and the left ventricular wall was thickened, especially at the apex and anterolateral wall, and appeared sponge-like. There were numerous, excessively prominent trabeculations associated with intertrabecular recesses. Although the coexistence of NVM and DOMV could be a coincidence, we believe that both defects were probably caused by a developmental arrest of the left ventricular myocardium in the present case.

Abstract:Objective To evaluate the diagnostic value of intermediate type of atrioventricular septal defect using color Doppler echocardiography.Methods Fifteen patients with intermediate type of atrioventricular septal defect were studied by color Doppler echocardiography and the echocardiographic characteristics were analysed.Results All 15 cases were accurately diagnosed by color Doppler echocardiography. The most important echocardiographic characteristics of intermediate type of atrioventricular septal defect included:A large atrioventricular defect consisted of a premium atrial septal defect and an inlet ventricular septal defect.②Two separate atrioventricular valves were at the same level. Nearly half of the patients had cleft mitral or/and tricuspid valves.③CDFI demonstrated all patients had significant atrioventricular valvular regurgitation.Conclusion Color Doppler echocardiography has important value in diagnosis of intermediate type of atrioventricular septal defect.
[Double-chambered Right Ventricle with Difficult Preoperative Diagnosis of Ventricular Septal Defect in Adulthood]

Abstract:Case 1 is a 65-year-old woman with right ventricular outflow tract stenosis due to dyspnea on exertion. Preoperative echocardiography revealed no shunt between the right and left ventricles. After resecting the right ventricular muscle bundle and weaning from cardiopulmonary bypass, transesophageal echocardiography confirmed a shunt between both ventricles. Right ventriculotomy under second cardiopulmonary bypass revealed a ventricular septal defect (VSD) that was closed with an expanded polytetrafluoroethylene patch. Case 2 is a 58-year-old woman with a double-chambered right ventricle due to dyspnea on exertion. Preoperative echocardiography revealed a perimembranous VSD that coexisted;however, the VSD was completely covered by a membranous septal aneurysm, and no shunt appeared between both ventricles perioperatively. If right ventricular outflow tract stenosis/double-chambered right ventricle has been diagnosed in adulthood, the echocardiographic findings could have differed preoperatively and postoperatively. Combined use with other imaging modalities should improve the diagnostic accuracy of congenital heart disease.

Abstract:A 55-year-old man was admitted to our hospital because of chest distress, associated with activity. Two-dimensional echocardiography (2DE) demonstrated a suspected trabeculation versus false tendon of the left ventricular apex cordis but not meeting the diagnostic criteria of noncompaction of the ventricular myocardium (NVM). Threedimensional echocardiography (3DE) revealed more prominent trabeculations and deeper intertrabecular recesses of the left ventricular apex, which were consistent with the diagnostic criteria of NVM. In contrast to 2DE, 3DE provides wide, pyramid-shaped datasets that encompass the entire left ventricle. (Neth Heart J 2009;17:208-10.).

Abstract:Introduction: Double outlet left ventricle (DOLV) is a rare congenital heart anomaly, and cases of DOLV with an intact ventricular septum are uncommon. To date, only four such cases have been reported in the medical literature. Case presentation: This report presents a case of prenatally diagnosed DOLV. A fetal echocardiogram at 21 weeks of gestation demonstrated both great arteries, aorta and pulmonary artery, arising from the left ventricle with severely dysplastic tricuspid valve and severe hypoplasia of the right ventricle. Subsequent echocardiograms demonstrated no ventricular septal defect. The patient required balloon atrial septostomy in the first week of life, underwent pulmonary artery banding at 5 weeks of life, and is currently status post-bidirectional Glenn, and is awaiting final Fontan palliation. Conclusion: Prenatal diagnosis aided in predicting and guiding postnatal management.

Abstract:Double-chambered right ventricle (DCRV) is a rare congenital heart defect in adults, manifesting with progressive right ventricular outflow tract obstruction. We describe the first case of DCRV coexisting with hypertrophic cardiomyopathy, which is complicated by atrial flutter. A middle-aged woman with recurrent symptomatic atrial flutter who had previously been diagnosed with biventricular hypertrophic cardiomyopathy was admitted to our department. Echocardiography and cardiac magnetic resonance revealed asymmetrical interventricular septal hypertrophy, and abnormal muscle bundles within the right ventricle, generating an obstructive gradient. Genetic testing detected a hypertrophic cardiomyopathy-associated mutation: MYH7 , c.4135G > A, p. Ala1379Thr. A diagnosis of DCRV complicated by hypertrophic cardiomyopathy and atrial flutter was made. Surgical intervention was performed, which included radiofrequency ablation, removal of abnormal muscle bundles, and ventricular septal defect repair. Intraoperative transesophageal echocardiography demonstrated the well-corrected right ventricular outflow tract. Free of early postoperative complications, the patient was discharged in sinus rhythm on the 11th day after the surgery. Unfortunately, the patient died from a sudden death 38 days following the surgery. In conclusion, the coexistence of DCRV with hypertrophic cardiomyopathy in patients is an uncommon condition. The present case highlights the importance of diagnostic imaging in the management of this disorder.

Abstract:BACKGROUND Double-chambered left ventricle (DCLV) is an extremely rare congenital disease in which the left ventricle (LV) is divided by abnormal muscle tissue. Due to its rarity, there is a lack of data on the disease, including its diagnosis, treatment, and prognosis. Accordingly, we report a case in which DCLV was diagnosed and followed up. CASE SUMMARY A 45-year-old man presented to our hospital due to abnormal findings on an electrocardiogram recorded during a health check. He had no specific cardiac symptoms, comorbidities or relevant past medical history. Echocardiography revealed that the LV was divided into two by muscle fibers. There were no findings of ischemia on coronary angiography and coronary computed tomography angiography performed to exclude differential diagnoses. After comprehensive analysis of the images, DCLV was diagnosed. As it seemed to be asymptomatic DCLV, we decided the patient was to be observed without administering any medication. However, follow-up echocardiography revealed a thrombus in the accessory chamber (AC). Anticoagulant medication was initiated, the thrombus resolved, and the patient is currently undergoing follow-up without any specific symptoms. CONCLUSION Asymptomatic, uncomplicated DCLV was diagnosed through multimodal imaging; however, a thrombus in the AC occurred during the follow-up. The findings highlight that multimodal imaging is essential in diagnosing DCLV, and that anticoagulation is important in its management.

Abstract:Double chambered left ventricle (DCLV) is an uncommon congenital heart condition typically identified incidentally, with the majority of patients showing no symptoms and experiencing a benign course. It is crucial to differentiate DCLV from other abnormalities like diverticulum or aneurysm, which can have significant clinical implications. Due to the limited available data, our understanding of the natural progression, prognosis, complications, and treatment options for this rare condition is poorly defined. A review of the medical literature reveals the use of various overlapping terms when describing DCLV. In our case report, we present the evaluation of a young male who sought medical attention for palpitations. Initially, DCLV was diagnosed through 2D echocardiography. However, subsequent cardiac magnetic resonance imaging (CMR) did not confirm the presence of two distinct chambers but instead revealed an anomalous apical basal muscle bundle (ABMB) and atypical left ventricular (LV) trabecularization that resembled DCLV.

Abstract:Objective:To evaluate the value of echocardiography in the diagnosis of double outlet right ventricle (DORV).Methods:The clinical data of 106 cases with DORV undergoing correction surgery were retrospectively analyzed.The results were compared with the results of preoperative and transthoracic echocardiography.Results:104 cases of the 106 cases were correctly diagnosed by echocardiography,the diagnostic rate was 98 ％,and 2 cases were misdiagnosed as complete transposition of the great arteries.106 cases of patients with puhnonary artery stenosis,pulmonary hypertension accounted for 54.7％,accounting for 43.4％;in the subaortic VSD accounted for 46.2％,from the two artery accounted for 25.5％;ventricular septal defect pulmonary valve located under accounted for 20.8％,accounting for 7.5％ of the aortic valve in double.There were 100 cases of atrioventricular connection and 6 cases of atrioventricular junction discordance in the relation of atrioventricular connection and the location of great arteries.In 93 cases of right transposition of the great arteries,13 cases were left transposition of the great arteries.Conclusion:The diagnostic accuracy of echocardiography in DORV is high.It can be indicated that the classification is of great significance for the selection of surgical procedures.

Abstract:In this case report, we present the delayed diagnosis of an atypical partial anomalous pulmonary venous connection, which was initially misdiagnosed as primary pulmonary arterial hypertension. It was difficult to make a conclusive diagnosis using transthoracic echocardiography (TTE), because the two high right superior pulmonary veins drained into the superior vena cava and limited the shunt of patent foramen ovale. Preoperative TTE only showed right heart volume overload, pulmonary arterial hypertension, and severe tricuspid valve insufficiency. A chest CT-angiography (angio-CT) finally found the anomalous right superior pulmonary venous connection, which was further confirmed by surgery. The postoperative course was uneventful.

Abstract:We treated a female patient known to have a double-chambered right ventricle (DCRV) who presented with symptoms of an acute myocardial infarction (AMI). Emergent coronary artery catheterization revealed acute right coronary artery (RCA) occlusion and proximal left anterior descending (LAD) stenosis. We performed percutaneous coronary intervention (PCI) for the RCA occlusion. Right heart catheterization revealed a pressure gradient across the mid-RV of 58 mmHg. Computed tomography and magnetic resonance imaging revealed no other congenital cardiac abnormalities. She underwent surgical repair of the RV stenosis and coronary artery bypass surgery for LAD stenosis.

Abstract:Background: Right ventricular volume overload is the key finding in a patient with previously undiagnosed atrial septal defect (ASD). Case summary: A 68-year-old female was referred to our hospital due to progressive pulmonary artery dilatation observed on her chest X-ray. Echocardiography revealed a secundum ASD with right ventricular dilatation. She had undergone aortic root replacement and aortic valve replacement for annuloaortic ectasia and aortic insufficiency 12 years prior to the diagnosis. She was also diagnosed with Marfan syndrome, which was supported by family histories. Computed tomography did not show a secundum ASD before the surgery. We finally closed the secundum ASD with catheter closure device. Discussion: Ascending aneurysm might mask the presence of secundum ASD. Monitoring the change in pulmonary artery dilatation overtime is useful for the diagnosing secundum ASD.

Abstract:Background: Pulmonary sequestration is an uncommon pulmonary disorder. We presented an adult case with recurrent pulmonary infection firstly misdiagnosed as pneumonia, which proved as pulmonary sequestration by enhanced CT scan and CT angiography. Methods: Appropriate laboratory tests, chest CT scan, bronchoscopy, and CT angiography were performed for diagnosis. Results: The white blood cells detected by routine blood test were 11.8 x 10(9)/L, the plain chest CT scan showed the volume of the lower lobe of the left lung decreased and the density increased. Enhanced CT and maximum intensity projection (MIP) algorithms were used for three-dimensional (3D) reconstruction of the images: no abnormally enhanced shadows were seen in the reduced lower lobe of the left lung, and tortuous vascular shadows were seen in the mediastinum. Bronchoscopy showed a narrowing of the opening in the dorsal segment of the lower lobe of the left lung. Thoracic aortography revealed an abnormal arterial supply to the lower left lung, the pathological results of thoracoscopic resection of the lower left lung were pulmonary sequestration. Conclusions: Pulmonary consolidation may be more than a simple pulmonary infection. Physicians should consider the possibility of pulmonary sequestration in patients with recurrent or refractory pneumonia. Enhanced CT findings of abnormal blood vessel supply are helpful for pulmonary sequestration diagnosis, and CT angiography is the gold standard for diagnosis.

Abstract:Double-outlet right atrium is an extremely rare congenital cardiac defect, in which the right atrium drains into both ventricles. Here in, we present a peculiar case involving an 8-year-old female with double-outlet right atrium characterized by three distinct atrioventricular valves and a membranous ventricular septal defect. This is associated with right atrium to left ventricular flow through the accessory atrioventricular valve while maintaining adequate size of the right ventricle. Additionally, we include a comprehensive review of the literature to underscore the variability in clinical presentation and management strategies.

Abstract:Double chambered right ventricle is a rare congenital heart disease that is characterised by the presence of an anomalous muscle bundle that divides the right ventricle into a low pressure superior (distal) chamber and a high pressure inferior (proximal) chamber. It is found in association with a ventricular septal defect in 90% cases with other associations being tetralogy of Fallot, transposition of great vessels, atrial septal defect and Ebstein's anomaly. On the other hand, subaortic membrane is a form of discrete subaortic stenosis that is characterised by a membranous diaphragm in the subvalvular location of the left ventricular outflow tract. Both of these entities are responsible for causing subvalvular outflow tract obstruction. The occurrence of double chambered right ventricle in association with subaortic membrane is an extremely rare entity with only a few case reports available in the literature.

Abstract:Objective To assess the diagnosic value of echocardiography in congenital left coronary artery atresia (LCAA).Methods The echocardiographic characteristics of 3 hospitalized cases with LCAA were analyzed retrospectively and compared with angiographic results and surgical findings.Results Two cases were reminded correctly, the other case was misdiagnosed as severe mitral regurgitation.The characteristics of the echocardiography in 3 cases were absence of the left coronary artery ostium from the left coronary sinus, dialation of right coronary artery, moderate to severe mitral regurgitation, increasing echogenicity of the papillary muscles and collateral vessels in septum.Compared with the elder patient, 2 infant patients had poorer left cardiac function and less intercoronary collateral circulation.Conclusion Echocardiography, as a noninvasive method, could reveal congenital atresia of the left coronary artery, and provides more evidence for the further investigation and treatment.

Abstract:Background: Lipomatous atrial septal hypertrophy (LASH) with atrial septal defect (ASD) is a rare congenital anomaly. Although LASH is a histologically benign cardiac lesion characterized by excessive fat deposition in the interatrial septum that spares the fossa ovale, it has been associated with supraventricular arrhythmias or sick sinus syndrome. Application of multimodal imaging is crucial for accurate diagnosis, appropriate treatment of LASH with ASD, and follow-up. Case summary: A 68-year-old female patient presented with recurrent chest tightness and palpitation. Multimodal imaging revealed the characterizations of LASH and ASD. Two-dimensional transesophageal echocardiography showed a "dumbbell"-shaped involvement of the cephalad and caudal regions with sparing of a single secundum ASD. The septum with a brightness feature is an uncommon condition characterized by the deposition of unencapsulated fat cells in the atrial septum. Real-time four-dimensional transesophageal echocardiography reflected the lipomatous hypertrophy of the atrial septum and an oval-shaped ASD. Cardiac computer tomography angiography later confirmed this finding. The patient achieved a good clinical response with an ASD percutaneous occlusion guided by intracardiac echocardiography (ICE). Conclusion: This case demonstrates a LASH combined with ASD. Multimodality imaging can provide an accurate diagnosis and may guide the procedure for precise occlusion.

Abstract:We describe the echocardiographic features of 4 new cases of a rare atrial septal malformation consisting a double atrial septum with a midline chamber between the left and right atrium. Half of the cases had major left-sided obstructive lesions. Transthoracic, transesophageal, intracardiac, and fetal echocardiographic features of this anomaly are demonstrated. Previous descriptions, embryologic speculations, and clinical considerations are discussed.
