ACTH Response to TRH and Somatostatin in Nelson's Syndrome

Abstract:The response of plasma ACTH to TRH reported by Krieger and Luria (1977) in patients with Cushing's disease (CD) and Nelson's syndrome (NS), has been only occasionally confirmed (Pieters, Smals, Benraad and Kloppenborg 1970; Stahnke, Nowakowski and Schroder 1979). However, it was demonstrated that TRH elicited ACTH secretion from human and mouse ACTH-secreting pituitary tumors (Gershengorn, Arevalo, Geras and Rebecchi 1980; Ishibashi and Yamaji 1980; Oki, Nakao, Tanaka, Horii, Nakai, Shimbo, Wata-nabe, Kageyama, Kuwayama and Imura 1981). It has been reported that somatostatin (SRIF) reduces ACTH concentration in some pa tients with CD and NS (Fehm, Voigt, Lang, Beinert, Raptis and Pfeiffer 1976), while conflicting results have been obtained in Ad dison's disease (Fehm et al. 1976; Jara-Albarran, Bayort, Caballero, Eusebio, Garcia-Peris, Toledano and Perez-Modrego 1980). We des cribe a patient with NS, in whom TRH and SRIF paradoxically increased ACTH concentration both in vivo and in vitro.
