Balance training for neuromuscular control and performance enhancement: a systematic review

Abstract:Objective: As a result of inconsistencies in reported findings, controversy exists regarding the effectiveness of balance training for improving functional performance and neuromuscular control. Thus, its practical benefit in athletic training remains inconclusive. Our objective was to evaluate the effectiveness of training interventions in enhancing neuromuscular control and functional performance. Data sources: Two independent reviewers performed a literature search in Cochrane Bone, Joint and Muscle Trauma Group Register and Cochrane Controlled Trials Register, MEDLINE, EMBASE, PEDro (Physiotherapy Evidence Database), and SCOPUS. Study selection: Randomized controlled trials and controlled trials without randomization with healthy and physically active participants aged up to 40 years old were considered for inclusion. Outcomes of interest were postural control, muscle strength, agility, jump performance, sprint performance, muscle reflex activity, rate of force development, reaction time, and electromyography. Data extraction: Data of interest were methodologic assessment, training intervention, outcome, timing of the outcome assessment, and results. Standardized mean differences and 95% confidence intervals were calculated when data were sufficient. Data synthesis: In total, 20 randomized clinical trials met the inclusion criteria. Balance training was effective in improving postural sway and functional balance when compared with untrained control participants. Larger effect sizes were shown for training programs of longer duration. Although controversial findings were reported for jumping performance, agility, and neuromuscular control, there are indications for the effectiveness of balance training in these outcomes. When compared with plyometric or strength training, conflicting results or no effects of balance training were reported for strength improvements and changes in sprint performance. Conclusions: We conclude that balance training can be effective for postural and neuromuscular control improvements. However, as a result of the low methodologic quality and training differences, further research is strongly recommended.

Abstract:Objective: This review study aimed to assess the impact of neuromuscular training (NT) on athletes' physical fitness in sports. Methods: Three independent reviewers conducted a literature search in various databases: EBSCOHOST, PubMed, WOS, Mendeley, Scopus, ProQuest, Science Direct, additional references, and Google Scholar. The methodological quality was examined using Lubans' predetermined methods, and data that included trials were excluded. Results: This review included 18 well-conducted systematic studies from 144 relevant publications. These studies were reviewed and have been given a score of 6. Medium-risk studies were scored 3 or 4, while low-risk studies were scored 5 or 6. None of the studies had a high-risk bias. The NT intervention revealed that balance ( n = 10) was the main characteristic of physical fitness that was evaluated, followed by agility ( n = 6), muscular strength ( n = 4), speed ( n = 5), endurance and muscular power ( n = 2). Subsequently, most studies used an intervention such as plyometric and strength training exercises to improve agility, balance, and muscular strength among athletes. Conclusion: This review implicated that (NT) focuses on exercises that enhance motor skills which aid athletes in moving their bodies according to their situational needs. The athletes' slower and faster directions influence their agility, muscular strength, and balance, essential for player performance. It is recommended for future research to investigate the effects of neuromuscular training (length of 12-weeks, frequency of 3 days per week and 90-min duration) on physical fitness components (coordination, reaction-time, flexibility, cardiovascular fitness, cardiorespiratory fitness and body composition) that are essential for all ages of male and female athletes in all sports.

Abstract:Neurorehabilitation and Neural Repair, Ahead of Print. BackgroundBalance training covers a range of different modalities and complexity levels for people with multiple sclerosis (MS). When evaluating the effects of balance training across different kinds of interventions, determination of the specific intervention content that predict effects are needed.ObjectiveTo investigate the effects of balance training on gait and dynamic balance outcomes.MethodsFour databases were systematically searched. Randomized controlled trials involving people with MS (Expanded Disability Status Scale [EDSS] score ≤7.5) where at least 50% of the intervention targeted balance control were included. Interventions were categorized based on training types. Risk-of-bias was assessed using the Tool for the Assessment of Study Quality and Reporting in Exercise (TESTEX).ResultsA total of 18 included studies involved 902 people with MS (EDSS range from 0 to 7.5). Interventions evaluated with a balance composite score or a mobility test showed a moderate effect size (ES = 0.46 [95% confidence interval (CI) = 0.18 to 0.74]; p < .01) and a small overall ES (ES = 0.19 [95% CI = 0.01–0.36]; p = .04), respectively, across different training types. Stepping and gait speed outcomes showed no effect. Cognitive dual-task training showed a significant effect (ES = 0.81 [95% CI = 0.24 to 1.37]) on subgroup level, when evaluated with a mobility outcome measure. The median TESTEX score on study quality and reporting was 11 (maximum score = 15).ConclusionsImprovements of balance were found across interventions when measured by balance composite scores and mobility tests, but not when measured by stepping or gait speed outcomes. Large training volume was positively associated with effect on balance. A definition of intensity in balance training is needed for evaluation of its impact on the effect of balance interventions.

Abstract:Abstract Background This study aims to investigate the impact of neuromuscular training (NMT) on static and dynamic postural balance (PB) among high‐level male runners with intellectual disability. Method Twenty‐seven runners were randomly assigned to a NMT group and a control group who maintained their conventional training. Static and dynamic PB were assessed using the centre of pressure (CoP) excursions (in bipedal and unipedal stances under open eyes (OE) and closed eyes (CE) conditions) and the star excursion balance test (SEBT), respectively, at pre‐training and post‐training. Results The NMT group showed significantly (p < 0.05) decreased CoP values and increased SEBT scores at post‐training compared to pre‐training. The switch from OE to CE did not affect static PB in the bipedal stance, only in the NMT group. Conclusions The NMT was effective in improving static and dynamic PB in runners with intellectual disability. The NMT could reduce visual dependency.

Abstract:Background: Stroke, which is a common clinical cerebrovascular disease, causes approximately 83% of survivors to suffer from balance impairments. Balance and gait training (BGT) is widely used to restore balance in patients with stroke. However, its wide variety presents clinicians with a dilemma when selecting interventions. This study aimed to compare and rank BGT interventions by quantifying information based on randomized controlled trials (RCTs). Methods: We conducted a network meta-analysis (NMA) of non-gait-trained controls and head-to-head RCTs and compared the effects of 12 BGT interventions. A total of nine literature databases, including Medline, Embase, Cochrane Library, Web of Science, Scopus, SPORTDiscus, ClinicalTrials.gov, CNKI, and Chinese biomedical literature databases, were searched from their database inception to August 2023. Two authors independently selected studies and extracted data. The difference in outcomes, which were expressed as standardized mean differences and confidence intervals (CIs) of 95%, were explored in this meta-analysis. Results: A total of 66 studies with 1,933 participants were included. Effect size estimates showed that not all BGT interventions were more effective than controls, with treadmill training as the least effective for balance test batteries (SMD = −0.41, 95% CI [−1.09, 0.27]) and proactive balance (SMD = −0.50, 95% CI [−1.14, 0.14]). Body-weight-supported treadmill training with external stimulation was most effective for proactive balance and dynamic steady-state balance (SMD = 1.57, 95% CI [−0.03, 3.16]); SMD = 1.18, 95% CI [0.67, 1.68]. Virtual reality gait training (SMD = 1.37, 95% CI [0.62, 2.11]) had the best effect on improving balance test batteries, while dual-task BGT (SMD = 1.64, 95% CI [0.50, 2.78]) had the best effect on static steady-state balance. After analyses for possible impact covariates, the findings through the outcomes did not change substantially. Confidence in the evidence was generally low or very low. Conclusion: This NMA suggested that virtual reality gait training was the most effective BGT modality for improving balance test batteries. Body-weight support treadmill training with external stimulation was the most effective for improving active and dynamic balance. In addition, dual-task BGT was the best choice for improving static balance. However, balance is a multidimensional concept, and patients' different needs should be considered when selecting BGT. Comparative efficacy of gait training for balance outcomes in patients with stroke: A systematic review and network meta-analysis

Abstract:Background: Growing evidence suggests that gait training can improve stroke patients' balance outcomes. However, it remains unclear which type of gait training is more effective in improving certain types of balance outcomes in patients with stroke. Thus, this network meta-analysis (NMA) included six types of gait training (treadmill, body-weight-supported treadmill, virtual reality gait training, robotic-assisted gait training, overground walking training, and conventional gait training) and four types of balance outcomes (static steady-state balance, dynamic steady-state balance, proactive balance, and balance test batteries), aiming to compare the efficacy of different gait training on specific types of balance outcomes in stroke patients and determine the most effective gait training. Method: We searched PubMed, Embase, Medline, Web of Science, and Cochrane Library databases from inception until 25 April 2022. Randomized controlled trials (RCTs) of gait training for the treatment of balance outcomes after stroke were included. RoB2 was used to assess the risk of bias in the included studies. Frequentist random-effects network meta-analysis (NMA) was used to evaluate the effect of gait training on four categories of balance outcomes. Result: A total of 61 RCTs from 2,551 citations, encompassing 2,328 stroke patients, were included in this study. Pooled results showed that body-weight-support treadmill (SMD = 0.30, 95% CI [0.01, 0.58]) and treadmill (SMD = 0.25, 95% CI [0.00, 0.49]) could improve the dynamic steady-state balance. Virtual reality gait training (SMD = 0.41, 95% CI [0.10, 0.71]) and body-weight-supported treadmill (SMD = 0.41, 95% CI [0.02, 0.80]) demonstrated better effects in improving balance test batteries. However, none of included gait training showed a significant effect on static steady-state balance and proactive balance. Conclusion: Gait training is an effective treatment for improving stroke patients' dynamic steady-state balance and balance test batteries. However, gait training had no significant effect on static steady-state balance and proactive balance. To achieve maximum efficacy, clinicians should consider this evidence when recommending rehabilitation training to stroke patients. Considering body-weight-supported treadmill is not common for chronic stroke patients in clinical practice, the treadmill is recommended for those who want to improve dynamic steady-state balance, and virtual reality gait training is recommended for those who want to improve balance test batteries. Limitation: Missing evidence in relation to some types of gait training is supposed to be taken into consideration. Moreover, we fail to assess reactive balance in this NMA since few included trials reported this outcome. Systematic Review Registration: PROSPERO, identifier CRD42022349965.

Abstract:Background: Postural balance represents a fundamental movement skill for the successful performance of everyday and sport-related activities. There is ample evidence on the effectiveness of balance training on balance performance in athletic and non-athletic population. However, less is known on potential transfer effects of other training types, such as plyometric jump training (PJT) on measures of balance. Given that PJT is a highly dynamic exercise mode with various forms of jump-landing tasks, high levels of postural control are needed to successfully perform PJT exercises. Accordingly, PJT has the potential to not only improve measures of muscle strength and power but also balance. Objective: To systematically review and synthetize evidence from randomized and non-randomized controlled trials regarding the effects of PJT on measures of balance in apparently healthy participants. Methods: Systematic literature searches were performed in the electronic databases PubMed, Web of Science, and SCOPUS. A PICOS approach was applied to define inclusion criteria, (i) apparently healthy participants, with no restrictions on their fitness level, sex, or age, (ii) a PJT program, (iii) active controls (any sport-related activity) or specific active controls (a specific exercise type such as balance training), (iv) assessment of dynamic, static balance pre- and post-PJT, (v) randomized controlled trials and controlled trials. The methodological quality of studies was assessed using the Physiotherapy Evidence Database (PEDro) scale. This meta-analysis was computed using the inverse variance random-effects model. The significance level was set at p <0.05. Results: The initial search retrieved 8,251 plus 23 records identified through other sources. Forty-two articles met our inclusion criteria for qualitative and 38 for quantitative analysis (1,806 participants [990 males, 816 females], age range 9–63 years). PJT interventions lasted between 4 and 36 weeks. The median PEDro score was 6 and no study had low methodological quality (≤3). The analysis revealed significant small effects of PJT on overall (dynamic and static) balance ( ES = 0.46; 95% CI = 0.32–0.61; p < 0.001), dynamic (e.g., Y-balance test) balance ( ES = 0.50; 95% CI = 0.30–0.71; p < 0.001), and static (e.g., flamingo balance test) balance ( ES = 0.49; 95% CI = 0.31–0.67; p < 0.001). The moderator analyses revealed that sex and/or age did not moderate balance performance outcomes. When PJT was compared to specific active controls (i.e., participants undergoing balance training, whole body vibration training, resistance training), both PJT and alternative training methods showed similar effects on overall (dynamic and static) balance ( p = 0.534). Specifically, when PJT was compared to balance training, both training types showed similar effects on overall (dynamic and static) balance ( p = 0.514). Conclusion: Compared to active controls, PJT showed small effects on overall balance, dynamic and static balance. Additionally, PJT produced similar balance improvements compared to other training types (i.e., balance training). Although PJT is widely used in athletic and recreational sport settings to improve athletes' physical fitness (e.g., jumping; sprinting), our systematic review with meta-analysis is novel in as much as it indicates that PJT also improves balance performance. The observed PJT-related balance enhancements were irrespective of sex and participants' age. Therefore, PJT appears to be an adequate training regime to improve balance in both, athletic and recreational settings.

Elite Female Basketball Players' Body-Weight Neuromuscular Training and Performance on the Y-Balance Test

Abstract:Context: Neuromuscular training enhances unconscious motor responses by stimulating both the afferent signals and central mechanisms responsible for dynamic joint control. Dynamic joint-control training is a vital component of injury-prevention programs. Objective: To investigate the effects of body-weight neuromuscular training on Y-Balance Test (YBT) performance and postural control in female basketball players. Design: Randomized controlled clinical trial. Setting: Basketball practice sessions. Patients or other participants: A total of 28 healthy elite female basketball players were randomly assigned to an experimental (n = 14) or a control group (n = 14). Intervention(s): Before their regular practice sessions, the experimental group warmed up with body-weight neuromuscular exercises and the control group with standard tactical-technical exercises twice weekly for 8 weeks. Main outcome measure(s): Anterior-, posteromedial-, and posterolateral-reach and composite YBT scores were measured before and after 8 weeks of training. Results: Improvement over baseline scores was noted in the posteromedial (right = 86.5 ± 4.5 cm versus 89.6 ± 2.2 cm, +3.5%, P = .049; left = 85.5 ± 4.3 cm versus 90.2 ± 2.7 cm, +5.5%, P = .038)- and posterolateral (right = 90.7 ± 3.6 cm versus 94.0 ± 2.7 cm, +3.6%, P = .016; left = 90.9 ± 3.5 cm versus 94.2 ± 2.6 cm, +3.6%, P = .011)-reach directions and in the composite YBT scores (right = 88.6% ± 3.2% versus 94.0% ± 1.8%, +5.4%, P = .0004; left = 89.2% ± 3.2% versus 94.5% ± 3.0%, +5.8%, P = .001) of the experimental group. No differences in anterior reach were detected in either group. Differences were noted in postintervention scores for posteromedial reach (right = 89.6 ± 2.2 cm versus 84.3 ± 4.4 cm, +4.1%, P = .005; left = 94.2 ± 2.6 cm versus 84.8 ± 4.4 cm, +10%, P = .003) and composite scores (right = 94.0% ± 1.8% versus 87.3% ± 2.0%, +7.1%, P = .003; left = 94.8% ± 3.0% versus 87.9% ± 3.4%, +7.3%, P < .0001) between the experimental and control groups. Conclusions: Body-weight neuromuscular training improved postural control and lower limb stability in female basketball players as assessed with the YBT. Incorporating neuromuscular training into the workout routines for basketball players may enhance joint awareness and reduce the risk of lower extremity injury.
Comparison the Effect of Neuromuscular Exercises With Other Training Modalities on Balance and Motor Function in People With Chronic Ankle Instability: A Systematic Review and Meta-Analysis

Abstract:Objective Identifying the factors affecting the reduction of ankle sprain complications, rapid recovery, and prevention of re-injury after an ankle sprain are essential. Therefore, the present meta-analysis investigates which training modalities have a better effect on the balance and motor function of people with chronic ankle instability. Materials & Methods The relevant articles were identified by searching 8 international databases of MEDLINE/PubMed, Embase/Scopus, LILACS, CINAHL, Central (Cochrane Central Register of Controlled Trials), Web of Science, PEDro, and Google Scholar. This work was also done in Persian databases Magiran, Irandoc, Iranmedex, MedLib, and SID, equivalent to English keywords searched from 8 databases. After collecting the search results, first, the title and then the abstracts of the articles were read. If the articles met the inclusion and exclusion criteria, their results would be used in the review study, otherwise discarded. The search period comprised from the beginning to December 2021. In addition, a manual search and full review of article sources was performed. The exclusion criteria were articles whose statistical population was people other than people with chronic ankle instability, published articles other than English and Persian, and articles that examined the effect of exercise programs on factors other than balance and motor function had paid. Hedges’ effect size (ratio of the mean difference between the two groups to the mixed weight standard deviation) was used for meta-analysis. Heterogeneity between studies was assessed using the I2 index. If the heterogeneity is above 50%, the random effects model method is used, and if it is below 50%, the fixed-effects model method is used. The bias of published studies was assessed using Egger’s test. Comprehensive meta-analysis v.2.0 software was used to analyze the data. Results After reviewing the full text of the found articles, 21 articles using 24 training protocols were selected. The final articles were divided into two categories for analysis: articles comparing neuromuscular exercises with other exercises and articles comparing new and combined balance exercises with other exercises with traditional balance exercises. By a superficial qualitative review of articles with the PEDro tool, the average scores that articles received from this scale were higher than 7, which shows that the articles used for the meta-analysis of this study are of high quality and the results of these studies are reliable. Conclusion The results of studies showed that neuromuscular training has a better effect on static and dynamic balance in people with chronic ankle instability than other training protocols. However, there was no significant difference between the two types of training groups in motor performance. The results of studies also showed that balance-combination exercises, as well as new methods of balance exercises compared to traditional balance exercises, have a more significant effect on static balance and motor function of people with chronic ankle instability. However, no significant difference between the two types of training groups was observed in the dynamic balance. Also, the effect of new combined-balance exercises on dynamic balance was better than traditional balance exercises. It seems that the usefulness of balance-combination exercises and performing balance exercises with new methods is more useful than other training protocols on balance and postural control of people with chronic ankle instability.

The effect of adding real-time postural feedback in balance and mobility training in older adults: A systematic review and meta-analysis

Abstract:Objectives This study aimed to systematically review the additional value of providing real-time postural feedback during balance and mobility training in older people. Methods PubMed, Embase, CINAHL, and Web-of-Science were searched from inception to August 2023. Studies comparing the effectiveness of feedback-based versus non-feedback-based postural balance or mobility training on balance or mobility outcomes were selected. Similar outcomes were pooled in meta-analyses using a random-effect model. The quality of evidence for available outcomes was rated by Grading of Recommendations Assessment, Development and Evaluation (GRADE). Results Eight studies were identified with 203 subjects. Two studies showed that providing postural feedback immediately improved stability in static balance and gait. For the post-training effect, however, no significant change was found in trunk movement during single-leg standing (i.e., pitch angle, MD=0.65, 95 %CI=-0.77 to 2.07, low-quality; roll angle, MD=0.96, 95 %CI=-0.87 to 2.80, moderate-quality), in the Mini-BESTest (MD=1.88, 95 %CI=-0.05 to 3.80, moderate-quality), and in balance confidence (MD=0.29, 95 %CI=-3.43 to 4.2, moderate-quality). A worsened functional reach distance was associated with providing feedback during balance training (MD=-3.26, 95 %CI=-6.31 to -0.21, high-quality). Meta-analyses on mobility outcomes were mostly insignificant, except for the trunk-roll angle of walking (MD=0.87, 95 %CI=0.05 to 1.70, low-quality) and trunk-pitch angle of walking with head-turning (MD=1.87, 95 %CI=0.95 to 2.79, moderate-quality). Conclusion Adding real-time postural feedback to balance and mobility training might immediately improve stability in balance and mobility in older people. However, mixed results were reported for its post-training effect.

Neuromuscular and balance adaptations following acute stretching exercise: a randomized control trial

Abstract:Purpose: This study aimed to examine the acute effects of static stretching (SS) and dynamic stretching (DS) on neuromuscular function and balance in recreationally active men. Method: Sixty participants were randomly assigned to SS, DS, or no stretching (NS) groups. Before and after their respective stretching protocols, participants were assessed using the stork balance test (SBT), Y-balance test (YBT), T-change of direction test (CoD T-test), countermovement jump test (CMJT), squat jump test (SJT), and five-time jump test (FJT). Results: Significant main effects of time were observed for the SBT, YBT, and CoD T-test. Significant interactions between time and group were found for the SBT, YBT, CoD T-test, and CMJT (P < 0.05). Compared to the NS group, the SS group showed significant improvement in the SBT (P < 0.05), while the DS group demonstrated significant improvements in the SBT, YBT (all directions), CoD T-test, CMJT, and SJT (P < 0.05). Post-training, the DS group showed greater improvements than the SS group in the YBT, CoD T-test, CMJT, and SJT (P < 0.05), with no significant differences in the SBT. Conclusion: SS acutely improves static balance, while DS has a broader impact, enhancing both neuromuscular function and balance.

Abstract:This paper presents a brief overview of the Electra laser system and report on the most recent results. The laser system consists of an electron beam pumped main amplifier with an aperture of 30times30 cm2, an e-beam pumped 10times10 cm2 pre-amplifier, and a commercial discharge laser serving as the seed oscillator. The main amplifier, currently operated as an oscillator, has demonstrated single shot and rep-rate laser energies exceeding 700 J, with a pulse width of 100 ns at 248 nm. Continuous operation of this laser in an oscillator mode has lasted for more than 2.5 hours without failure at 1 and 2.5 Hz. Tests at higher repetition rates and longer runs are ongoing. The pre-amplifier uses a fast gas Marx, pulse forming lines, a single stage magnetic switch, and transit time isolators and operates with a rep-rate of up to 5 Hz.

Abstract:Background: Lower limb injuries in sports affect postural stability and balance. Muscular imbalance, postural malalignments, and altered biomechanics occurred in athletes due to injuries. Objective: To determine the effects of the 6-week neuromuscular training program on single-limb stability in young female athletes. Methodology: A single-subject study design was used in which n=20 female athletes aged between 18 to 25 years with previous lower limb injuries that lead to impaired balance and postural instability participated in this study included by the convenience sampling from the Faisalabad sports complex.. Athletes completed 6-week neuromuscular training program, with every session of 90 min and 3 times a week. Assessment of athletes was performed before and after the completion of the neuromuscular training program at 6 weeks, with a single-limb stance test, Stork balance stand test, and star excursion balance test. Results: The mean age of participants was 20.6±1.82 years. Single limb stance test score with eyes open pre 27.70±13.85 and post-test treatment 40.75±16.45 showed significant results with (p <0.01). Stork balance test scores pre 28.40±17.25 and post-treatment 38.85±14.543 also showed significant results with (p <0.01). Star excursion balance test right and left anterior, left medial, right and left lateral also showed significant results with (p <0.01). Star excursion balance test right medial pre and post-treatment showed insignificant results with (p-value =0.234). Conclusion: Although 6-week neuromuscular training program showed a significant improvement on single-limb stability in young female athletes. But without control or comparative group we cannot establish its gender specific effectiveness. J Strength Cond Res 32(4): 911–920, 2018—The aim of the present randomized controlled trial was to evaluate the effects of an 8-week neuromuscular training program focused on core stability, plyometric, and body-weight strengthening exercises on dynamic postural control and vertical jump performance in elite junior skiers. Twenty-four Italian elite junior male skiers were recruited and randomized to either an experimental group (EG), performing neuromuscular warm-up exercises, (EG; n = 12; age 18 ± 1 years; body mass 66 ± 21 kg; height 1.70 ± 0.1 m) or a control group (CG) involved in a standard warm-up (CG; n = 12; age 18 ± 1 years; body mass 62 ± 14 kg; height 1.73 ± 0.1 m). lower quarter Y-Balance Test (YBT), countermovement jump (CMJ), and drop jump (DJ) at baseline (PRE) and at the end (POST) of the experimental procedures were performed. No significant differences between EG and CG were observed at baseline. Results showed that EG achieved positive effects from PRE to POST measures in the anterior, posteromedial, posterolateral directions, and composite score of YBT for both lower limbs, whereas no significant differences were detected for CG. Furthermore, 2-way analysis of variance with Bonferroni's multiple comparisons test did not reveal any significant differences in CMJ and DJ for both EG and CG. The inclusion of an 8-week neuromuscular warm-up program led to positive effects in dynamic balance ability but not in vertical jump performance in elite junior skiers. Neuromuscular training may be an effective intervention to specifically increase lower limb joint awareness and postural control.

Abstract:Introductin: Improper gait patterns, impaired balance and foot drop consistently plague stroke survivors, preventing them from walking independently and safely. Neuromuscular electrical stimulation (NMES) technology can help patients reactivate their muscles and regain motor coordination. This study aims to systematically review and summarize the evidence for the potential benefits of NMES on the improvement of gait patterns after stroke. Evidence acquisition: PubMed, Cochrane Library, Embase, Science Direct and Web of Science were systematically searched until April 2024, to identify randomized controlled trials with the following criteria: stroke survivors as participants; NMES as intervention; conventional rehabilitation as a comparator; and gait assessment, through scales or quantitative parameters, as outcome measures. Evidence synthesis: 29 publications involving 1711 patients met the inclusion criteria. Meta-analysis showed no significant differences in Ten-meter walk test, Fugl-Meyer assessment lower extremity, Modified Ashworth Assessment and asymmetry between the NMES group and the control group. Besides, NMES was associated with changes in outcome indicators such as quantitative gait analysis speed [SMD = 0.53, 95% CI (0.20, 0.85), P = 0.001], cadence [SMD = 0.76, 95% CI (0.32, 1.20), P = 0.0008], affected side step length [SMD = 0.73, 95% CI (0.16, 1.31), P = 0.01], angle of ankle dorsiflexion [WMD = 1.57, 95% CI (0.80, 2.33), P < 0.0001], Six-Minute Walk Test [WMD = 14.83, 95% CI (13.55, 16.11), P<0.00001]. According to the PEDro scale, 21 (72.4%) studies were of high quality and 8 were of moderate quality (27.6%). Conclusions: Taken together, the review synthesis indicated that NMES might play a potential role in stroke-induced walking dysfunction. And NMES may be superior for survivors in the chronic phase than the acute and subacute phases, and the efficacy of short sessions received by patients was greater than that of those who participated in a longer session. Additionally, further comparisons of the effects of NMES with different types or stimulation frequencies may provide unexpected benefits.

Abstract:[Purpose] The purpose of this study is to examine the effects of dynamic exercise utilizing the proprioceptive neuromuscular facilitation patterns accompanied by the abdomen drawing-in on balance of healthy adults. [Participants and Methods] The total number of participants was 30, and 15 were randomly placed in the training group (TG) and 15 in the control group (CG). The participants in the TG have conducted the 3 sets to 5 sets of dynamic exercise utilizing the proprioceptive neuromuscular facilitation patterns per day, 3 times a week for 6 weeks. The balance was measured by the Terax, a balance-measuring device with force plates. [Results] Comparing the TG's balance pre-test and post-test, there was a statistical significance in stability score (SS), weight distribution index (WDI), CD force plates and AC force plates (A: left rearfoot, B: left forefoot, C: right rearfoot, D: right forefoot). [Conclusion] Dynamic trunk stabilization exercise utilizing the proprioceptive neuromuscular facilitation patterns seem to help increase the balance of healthy adults.

Abstract:Objective To assess the effects of dual-task training on gait and balance in stroke patients. Data sources: A systematic review of PubMed, Web of Science, Embase and Cochrane Library from their inception through 20 August 2021. Review methods The bibliography was screened to identify randomized controlled trials that applied dual-task training to rehabilitation function training in stroke patients. Two reviewers independently screened references, selected relevant studies, extracted data and assessed risk of bias using the Cochrane tool of bias. The primary outcome was the gait and balance parameters. Results A total of 1992 studies were identified and 15 randomized controlled trials were finally included (512 individuals) were analyzed. A meta-analysis was performed and a beneficial effect on rehabilitation training was found. Compared to patients who received conventional rehabilitation therapy, those who received dual-task training showed greater improvement in step length (MD = 3.46, 95% CI [1.01, 5.92], P = 0.006), cadence (MD = 4.92, 95% CI [3.10, 6.74], P < 0.001) and berg balance scale score (MD = 3.10, 95% CI [0.11, 6.09], P = 0.040). There were no differences in the improvements in gait speed (MD = 2.89, 95% CI [ − 2.02, 7.80], P = 0.250) and timed up and go test (MD = −2.62, 95% CI [ − 7.94, 2.71], P = 0.340) between dual-task and control groups. Conclusion Dual-task training is an effective training for rehabilitation of stroke patients in step length and cadence, however, the superiority of dual-task training for improving balance function needs further discussion.

Abstract:Balance training, defined here as training of postural equilibrium, improves postural control and reduces the rate of falls especially in older adults. This systematic review aimed to determine the neuroplasticity induced by such training in younger (18–30 years old) and older adults (≥65 years old). We focused on spinal and corticospinal pathways, as studied with electrophysiology, in people without neurological or other systemic disorders. We were specifically interested in the change in the excitability of these pathways before and after training. Searches were conducted in four databases: MEDLINE, CINAHL, Scopus, and Embase. A total of 1,172 abstracts were screened, and 14 articles were included. Quality of the studies was evaluated with the Downs and Black checklist. Twelve of the studies measured spinal reflexes, with ten measuring the soleus H-reflex. The H-reflex amplitude was consistently reduced in younger adults after balance training, while mixed results were found in older adults, with many showing an increase in the H-reflex after training. The differences in results between studies of younger vs. older adults may be related to the differences in their H-reflexes at baseline, with older adults showing much smaller H-reflexes than younger adults. Five studies measured corticospinal and intracortical excitability using transcranial magnetic stimulation. Younger adults showed reduced corticospinal excitability and enhanced intracortical inhibition after balance training. Two studies on older adults reported mixed results after training. No conclusions could be drawn for corticospinal and intracortical plasticity given the small number of studies. Overall, balance training induced measurable change in spinal excitability, with different changes seen in younger compared to older adults.

Abstract:The present study aims to determine the potential benefits of PNF on balance and gait function in patients with chronic stroke by using a systematic review and meta-analysis. Systematic review in the following databases: MEDLINE/PubMed, Physiotherapy Evidence Database (PEDro), Cochrane Library and Google Scholar. Studies up to September 2020 are included. A systematic database search was conducted for randomized control trials (RCTs) that investigated the effects of PNF intervention in patients with chronic stroke using balance and gait parameters as outcome measures. The primary outcomes of interest were Berg Balance Scale (BBS), Functional Reach Test (FRT), Timed Up and Go Test (TUG) and 10-Meter Walking Test (10MWT). Nineteen studies with 532 participants were included, of which twelve studies with 327 participants were included for meta-analysis. When the data were pooled, PNF made statistically significant improvements in balance with BBS, FRT and TUG (p < 0.05) or gait velocity with 10MWT (p < 0.001) when compared to the control. This review indicates that PNF is a potential treatment strategy in chronic stroke rehabilitation on balance and gait speed. Further high-quality research is required for concluding a consensus of intervention and research on PNF.
