Abstract:ObjectiveThis article reviews the causes of aortoenteric fistulas, diagnostic options, and important CT findings.ConclusionAortoenteric fistula, a rare but potentially fatal entity, presents a significant challenge to radiologists in diagnosis, largely because of its subtle and nonspecific imaging findings. These fistulas can be divided into primary and secondary forms, depending on the presence or absence of prior aortic reconstructive surgery, but the secondary form is more common. Typical CT findings, which can overlap with those seen in perigraft infection, aortitis, infected/mycotic aneurysms, perianeurysmal fibrosis, and the immediate post-operative period after placement of a graft, include: Effacement of the fat planes around the aorta, perigraft fluid/soft tissue thickening, ectopic gas, tethering of adjacent thickened bowel loops towards the aortic graft, and in rare cases, extravasation of contrast from the aorta into the involved segment of bowel.

Abstract:A 61-year-old man who underwent total gastrectomy and splenectomy for gastric cancer was placed a Port-A-Cath through the anterior abdominal wall just lateral to the rectus muscle in the right upper quadrant for intraperitoneal chemotherapy 14 years ago. He recovered well after the surgery and chemotherapy. The Port-A-Cath was left in situ for 14 years. The patient’s course was uncomplicated until approximately one year ago, when he presented with recurrent redness, swelling and pain around the subcutaneous port. The symptoms were alleviated after antibiotic treatment and the Port-A-Cath was removed in the local hospital. Three days later, rigor and fever with recordings of a 40 degrees Celsius body temperature occurred. The patient was thus admitted to the local hospital. Two days later, he developed abdominal distension and a cankerous sore at the site of the operative incision. The wound healed after local application of gentamycin and frequent wound dressing changes, yet 7 days later, the patient experienced the same symptoms of rigor and fever with a temperature of 40 degrees Celsius. After the administration of several antibiotics and glucocorticoids, his body temperature hovered between 37.5 and 38.0 degrees Celsius prompting admittance to our university hospital. Laboratory tests revealed leukocytosis (11.0×10/L), with a neutrophilic percentage of 78.7% and an elevated C-reactive protein (CRP) of 69 mg/L. The thoracic computed tomography (CT) showed slight bilateral pleural effusion, pericardial effusion and mild mediasternal pneumatosis while ultrasound tests confirmed the pleural effusion and revealed a small quantity of ascites. With time, antibiotics ceased to have an effect on the patient’s symptoms and he gradually developed dyspnea, palpitation and orthopnea with a body temperature of 39 degrees Celsius. Abstract:Background Spinal arachnoid cysts are rare and have varied clinical manifestations depending on the affected spinal region and nerve roots. A complete cyst excision with fistula closure is the first choice of treatment. However, it might be difficult to localize the specific position of the fistula because previous images have no enhancements or the fistula is too tiny to be detected. Case presentation This case is a giant lumbar extradural arachnoid cyst. We administered a lumbar injection with contrast medium into subarachnoid space under digital subtraction angiography (DSA) and disclosed the fistula. Confirming the location of fistula enabled us to perform minimally invasive surgery to ligate the fistula. Surgical intervention for a spinal arachnoid cyst might encounter the problem of the formation of a postoperative cerebrospinal fluid (CSF) fistula. We propose the option of detecting the fistula preoperatively for minimal invasive surgery. Recurrence depends on the long-term follow-up, and more cases are needed to further evaluate our technique. Conclusions The real-time contrast medium technique for spinal arachnoid cysts contributes to the complete ligation with minimally invasive surgery.

Abstract:Gastrointestinal bleeding due to aortoenteric fistula is extremely rare. Aortoenteric fistula is difficult to be diagnosed timely and entails a significant morbidity and mortality. Herein, we present an uncommon case of gastrointestinal bleeding caused by aortoduodenal fistula, which was a complication of a successful aortic reconstruction 4 months ago for an aortic pseudoaneurysm resulted from a stab wound 12 years ago. An urgent laparotomy confirmed an aortoduodenal fistula and repaired the defects in aorta and duodenum, but a prolonged shock led to the patient’s death. In summary, early diagnosis and surgical intervention for aortoenteric fistula are vital for survival.

Abstract:A 69-year-old male, three years post-endovascular exclusion for an abdominal aortic aneurysm, presented with asthenia and fever. An abdominal CT scan showed no gastrointestinal tract communications, abscess, or contrast extravasation. Tc-99m-HMPAO-labeled leukocytes scintigraphy with SPECT/CT revealed increased uptake on the posterior surface of the aortic graft, along with air bubbles in its right iliac limb. Upper gastrointestinal endoscopy was performed, revealing a duodenal ulcer in the transition between the second and third portions. The ulcer exhibited yellow graft tissue at its center. The patient underwent in situ reconstruction, involving the replacement of the infected prosthetic graft, and the duodenal defect was addressed through segmental resection and duodenojejunal anastomosis. Secondary aorto-duodenal fistula (SADF), a rare complication of vascular surgery, may arise from factors such as local infection or graft-bowel contact. SADF, often located in the duodenum, poses a high mortality risk, necessitating early diagnosis. Clinical presentation varies from significant upper gastrointestinal bleeding to obscured bleeding.

Abstract:Aorto-atrial fistula (AAF) and aorto-ventricular fistula (AVF) are a rare, but potentially life-threatening condition, where an anomalous connection forms between the aortic structures and the cardiac atria or ventricles. The exact incidence of this condition is unknown, as are the major precipitating factors and best treatment options. It may be congenital, secondary to conditions such as aortic dissection, infective endocarditis, or valve replacement, or iatrogenic in nature. In the case reported here a 72-year-old male who was found to have an infective endocarditis complicated by right AAF and AVF. This case illustrates the important role of transesophageal echocardiography in the evaluation of a patient with subacute infective endocarditis with a very rare complication. Learning points: Aortic fistulas are a rare, potentially fatal, complications of subacute infective endocarditis.Early recognition is greatly assisted by transesophageal echocardiography.

Abstract:Vascular and Endovascular Surgery, Ahead of Print. Background: Primary aortocaval fistulas (ACF) are a rare complication of abdominal aortic aneurysm (AAA), for which treatment options encompass both endovascular and open surgical intervention. Purpose: To report a rare presentation of primary aortocaval fistula. Research Design: Case Report. Study Sample: Single Patient Case. Data Collection and/or Analysis: Single case report. Results: We present a 54-year-old male with a ruptured AAA and associated ACF uniquely located near the caval confluence which was managed through primary endovascular exclusion. Notably, we report significant migration of our graft upon deployment, due to high flow through the ACF. Following a literature review on ACFs, we remark on the anatomic challenges regarding management of an ACF. Conclusions: Awareness of the clinical signs and symptoms remains imperative in proper ACF management given its high mortality.

Treatments consist of aortic resection, thoracic endovascular aortic repair, esophagectomy with diversion, and primary esophageal repair. The appropriate treatment is dependent on patient hemodynamics and fitness and familiarity with operative approaches by the treating team.
A complicated case of primary aortojejunal fistula: A case report

Abstract:Background: Aortoenteric fistulas are rare and life-threatening pathology characterized by an abnormal connection between the aorta and the gastrointestinal tract. Case presentation: The patient is a 61-year-old male who initially presented with hypogastric pain, hematemesis, and melena. Computed tomography angiography (CTA) revealed an abdominal aorta aneurysm but not a fistula. Imaging modalities were inconclusive in the diagnosis. The patient became unstable hemodynamically and was transferred to the operation room. The definitive diagnosis of aortoenteric fistula was confirmed during surgical exploration. Urgent surgery was performed; however, the patient experienced a cascade of complications, including rebleeding, intestinal leakage, and hemodynamic instability due to aortic bleeding. Despite rigorous interventions, the patient expired due to multiple organ failure 53 days after the first repair surgery. Clinical discussion: There is no definite imaging method due to the lack of guidelines, and the absence of exact findings has led to intraoperative diagnosis in up to 50 % of cases. This is one of the modalities of choice to examine suspected aortoenteric fistulas. Many authors prefer Computed tomography (CT) with intravenous contrast for suspected AEF despite its limitations in clarity. Others recommend CT angiography as the preferred modality. It is worth noting that, as reported in a comprehensive retrospective review, the mortality rate is approximately 46 % within 60 days after AEF repair surgery. Conclusion: This report adds to the limited data about primary aortojejunal fistulas, an extremely rare type of aortoenteric fistulas which has been reported in only a few cases. Understanding the importance of promptly suspecting, diagnosing, and intervening is crucial, emphasizing the importance of sharing such cases for medical guidance and better patient outcomes.

Abstract:Aortoenteric fistulas are an uncommon cause of gastrointestinal bleeding, and iliac-appendiceal fistulas are an even rarer cause. We describe a case of an iliac-appendiceal fistula in a patient who presented several months after aortic reconstruction with gastrointestinal bleeding. An extensive workup revealed that the source of bleeding was localized to the appendiceal orifice. The patient underwent an appendectomy with a two-stage procedure involving the iliac graft for definitive repair and ultimately recovered well. Despite the rarity of aortoenteric and iliac-appendiceal fistulas causing gastrointestinal bleeding, keeping a high index of suspicion in patients with a prior vascular repair can prevent death.

Abstract:Celiac trunk aneurysms (CTAs) are rare and usually asymptomatic. Although most of these aneurysms can be treated with percutaneous embolization, some uncommon locations of the aneurysm may make this approach impossible. We report a patient with a celiac trunk aneurysm (CTA) and a proximal splenic artery aneurysm (SAA). Due to the size and location of these two aneurysms, after multidisciplinary discussion, endovascular management was considered inappropriate and they were treated by laparoscopic ligation of the two aneurysms and revascularization. This procedure offers good postoperative recovery with good preservation of the visceral function. Some collateral vessels in the viscera were obvious on postoperative day 7.

Abstract:Background: Aortoesophageal fistula (AEF) caused by the ingestion of foreignbodiesis rare and often life threatening. We aim to summarize effective measures to improve the curative effect and survival rate. Case Reports: Between January 2010 and September 2015, three patients with AEF received endovascular stent-graft treatment in our hospital. Through the effective prevention of hemorrhage by stent-grafting, surgical repair of the esophageal rupture was performed successfully. All three cases were treated successfully and survived up to the day that this article was written. Comments: It may be safer to first perform an endovascular stent-graft to prevent hemorrhage in AEF.

Abstract:Aorto-duodenal fistula (ADF) is a rare cause of upper gastrointestinal bleeding, but it is associated with high mortality. It usually occurs in patients with prior aortic surgery or who have undergone aortic graft placement. Abdominal aortic aneurysm (AAA) might be a cause of primary ADF, which could develop into sudden shock. Because ADF is difficult to diagnose, surgery to correct it has a poor outcome. Abstract:The rupture of an internal iliac artery aneurysm in the colon is a rare but potentially fatal complication. We report a rectal fistula of an asymptomatic internal iliac artery aneurysm that was discovered incidentally during a medical examination. A 77-year-old man presented at a local hospital for a general medical examination. Although the blood reports revealed severe anemia, the patient did not complain of any associated symptoms including dizziness and hematochezia. Moreover, there was no palpable mass in the patient's abdomen, and there was no evidence of hematochezia, as the patient had been using a bidet. Interestingly, computed tomography (CT) revealed a large right internal iliac artery aneurysm. There was a suspicious finding of a fistula within the colon in the CT, but it was undetected in the preoperative sigmoidoscopy. Furthermore, operative findings showed a protruding retroperitoneal mass adhering to the mesentery of the sigmoid colon. During aneurysm resection, the presence of a fistula was unclear. However, a fistula tract, devoid of any infectious bacteria such as tuberculosis, was found in the specimen after colon resection. After a recovery period of approximately one week, the patient was discharged from the hospital without any unusual findings on the post-operative CT. Sigmoid colonic fistulas arising from iliac artery aneurysms are rare. Also, diagnosis may be delayed in special circumstances wherein a patient routinely uses a bidet.
Atypical esophageal submucosal tumor lesion with aortoesophageal fistula after thoracic endovascular aortic repair

Abstract:Aortoesophageal fistula (AEF) is a rare but lethal entity that is difficult to diagnose [1]. Despite the promising efficacy of thoracic endovascular aortic repair (TEVAR), which promotes the clinical use of this procedure, the incidence of AEF after TEVAR (post-TEVAR AEF) has increased, making it a major complication [2]. A 77-year-old man who had undergone TEVAR 2 years previously was hospitalized for an iliopsoas abscess. He also had intermittent tarry stools and progressive anemia. Upper gastrointestinal endoscopy (UGE) revealed a submucosal tumor (SMT)-like protrusion that included ulcerative lesions in the upper esophagus ([Fig. 1] a . Contrast-enhanced computed tomography (CT) imaging revealed extravasation of contrast outside the aortic lumen ([Fig. 1] b ). The man's symptoms were due to the presence of a post-TEVAR AEF accompanied by a stent graft infection; subsequently, a second TEVAR procedure was performed. Seven days postoperatively, UGE revealed an ulcerative lesion without debris ([Fig. 1] c ). Two months postoperatively, the contrast-enhanced CT image showed contrast agent in the aortic lumen with no evidence of leakage ([Fig. 1] d ). Also at 2 months postoperatively, UGE revealed a recess with an ulcer scar replacing the initial SMT-like lesion (SMTL) ([Fig. 2] a ). Two centimeters from the initially detected AEF lesion on the anal side, another SMTL was identified, which had not been found at the first post-TEVAR AEF detection on UGE. The SMTL protruded into and withdrew out of the esophagus in synchronization with breathing ([Fig. 2] b ). White-light and narrow-band endoscopic imaging showed that the normal mucosa was elongated with normal vessels near the SMTL ([Video 1]). We suspected that the secondary SMTL originated as a granular mass lesion due to mediastinal infection from the post-TEVAR AEF onto a fragile localized muscular defect [3] [4] [5]. Seven months later, a similar SMTL was identified at the AEF scar ([Fig. 2] c, d ). In this article we have described a rare endoscopic finding obtained during the long-term follow-up of a post-TEVAR AEF with SMTL showing anomalous movement. The incidence ranges from 0.36% to 1.6%, and a high clinical index of suspicion should be maintained for causes of UGIB in patients with a history of aneurysmal disease or aortic surgery. Computed tomography angiography (CTA) is recommended as the initial diagnostic test of choice. Early diagnosis of AEF is crucial due to the high fatality rate without intervention. We present a case of a man with a history of aortic surgery who presented with syncope and an isolated episode of melena, found to have an AEF on endoscopy.
