Abstract:Objective To analyze dynamic changes of ocular axial length in adolescent myopia (low myopia group and moderate myopia group,low age group and high age group) after orthokeratology.Methods This study elected 63 adolescents of wearing ortho-k in Jinhua Eye Hospital from January 2009 to December 2012,male 28,female 35,a total of 121 eyes,ranging in age from 8 to 14 years old,diopter degree ≤-6.00D,with-the-rule astigmatism ≤-l.50D,against-the-rule astigmatism ≤-0.75D.The changes of ocular axis after 1 week,1 month,3 months,6 months,12 months,18 months,24 months of wearing Ortho-K contact lens through ovemight wear were observed.The volunteers were divided into 4 groups:low myopia group (the Diopter degree ≤-3.00 D) and moderate myopia group (-3.25 ～-6.00 D);low age group (8 to 10 years old) and aged group (11～14 years old).This grouping was based on the degree of myopia and age.Results The growth of ocular axis after six months of wearing Ortho-K was statistically significant compared with unwearing (t =4.68,P ＜0.05).The difference between ocular axial growth of the moderate myopia group and the low myopia group after wearing a month later was statistically significant (t =3.4,P ＜0.05),the axial growth of moderate myopia group was slow,the axial growth between the low age group and the high age group had no significant difference (P ＞0.05).Conclusions The effect of wearing ortho-k contact lens of controlling myopia is better for moderate myopia than low myopia,while the factor of age has no effect on controlling myopia.
The Effect of Treatment Zone Decentration on Myopic Progression during Or-thokeratology

Abstract:Purpose: To evaluate the relationship between magnitude of orthokeratology (OrthoK) treatment zone decentration and 2-year axial length (AL) elongation in myopic children. Methods: One-hundred and one Chinese children who wore OrthoK contact lenses for 2 years. The magnitude and direction of the OrthoK treatment zone center from the entrance pupil center were recorded after 3 and 24 months of lens wear along with AL measurement. Stepwise multiple linear regression analysis was performed to assess which factors significantly affected an increase in AL. Results: After 3 and 24 months of OrthoK treatment, the mean (+/- standard deviation [SD]) magnitude of the OrthoK treatment zone decentration was 0.64 +/- 0.38 mm and 0.68 +/- 0.32 mm, respectively. There were no significant differences between the two time points (P > .05). After 2 years of OrthoK contact lenses wear, the mean (+/- SD) AL growth was 0.36 +/- 0.34 mm. The axial elongation was slightly correlated with baseline age of subjects (r = -0.073, P < .001), baseline spherical equivalent refractive error (r = -0.088, P < .001) and magnitude decentration of treatment zone (r = -0.190, P = .027). Conclusions: The decentration of OrthoK treatment zone stabilizes after 3 months of lens wear and slightly decreases AL growth.
Treatment zone decentration promotes retinal reshaping in Chinese myopic children wearing orthokeratology lenses

Abstract:Purpose: To investigate whether the treatment zone (TZ) decentration in orthokeratology (OK) lenses affects retinal expansion in Chinese children with myopia. Methods: Children aged 8 to 13 years (n = 30) were assessed over 13 months comprising 12 months of OK lens wear followed by discontinuation of lens wear for 1 month. Corneal topography was measured at 0, 1, 3, 6, 9, 12 and 13 months. TZ decentration of the OK lens was calculated, and subjects were subdivided into a small decentration group (group S) and a large decentration group (group L) based on the median value of the weighted average decentration (dave ). Central axial length (AL) and peripheral eye lengths (PELs) at the central retina, as well as 10°, 20° and 30° nasally and temporally were measured at 0 and 13 months under cycloplegia. Second-order polynomial (y = ax2 + bx + c) and linear fits (y = Kx + B) were applied to the peripheral relative eye length (PREL), and the coefficients 'a' and 'K' were used to describe the shape of the eye. Results: Mean AL growth for one year was 0.28 ± 0.17 mm. In a multiple linear regression model, AL elongation was related to the baseline age (β = -0.41, p = 0.01) and the dave (β = -0.37, p = 0.03) (R2 = 0.34, p = 0.002). When compared with smaller dave (0.45 ± 0.15 mm), a larger dave (0.89 ± 0.17 mm) was associated with slower ocular growth (central: 0.20 ± 0.13 mm vs. 0.35 ± 0.17 mm, p = 0.009; 10° nasal: 0.26 ± 0.18 mm vs. 0.45 ± 0.21 mm, p = 0.02; 10° temporal: 0.17 ± 0.14 mm vs. 0.32 ± 0.19 mm, p = 0.02) and more oblate retina shape ('a': -0.13 ± 0.02 vs. -0.14 ± 0.02, p = 0.02; Knasal : 0.35 ± 0.11 vs. 0.39 ± 0.09, p = 0.02; Ktemporal : -0.42 ± 0.08 vs. -0.46 ± 0.08, p = 0.004). Conclusions: Greater TZ decentration with the use of OK lenses was associated with slower axial growth and a more oblate retinal shape. TZ decentration caused local defocusing changes, which may inhibit myopic progression. These findings may have important implications for improving optical designs for myopia control.
Effects of orthokeratology on corneal reshaping and the delaying of axial eye growth in children

Abstract:Purpose: To investigate the inhibition of myopia progression and axial elongation in children wearing orthokeratology (OK) lenses, as well as to evaluate the status of corneal reshaping, this study explores the relationship between changes in central corneal curvature (K-value) and e-value induced by OK lenses and axial elongation. Methods: In this study, it is planned to select children aged 8-15 who wear orthokeratology lenses at the Pediatric Ophthalmology and Strabismus Clinic of the Second Affiliated Hospital of Dalian Medical University. All children will undergo slit lamp examination, visual acuity assessment, computerized refraction, intraocular pressure measurement, biometry, and corneal topography examination before lens wear and at 1 month, 3 months, and 6 months after lens wear in the pediatric ophthalmology clinic. Based on age (lower age group (8 < age ≤12 years); higher age group (12 < age ≤15 years)) and baseline equivalent spherical (SE) value (mild myopia group (-1.00 D < SE ≤ -3.25D); moderate myopia group (-3.25 D < SE ≤ -6.00 D)), four groups will be formed by pairing these factors. Suitable data will be selected according to inclusion and exclusion criteria, and different groups will be included. Data will be organized, and statistical analysis will be performed using SPSS software to obtain the results. The expected results will be discussed and analyzed. Results: After wearing OK lenses, all four groups achieved good visual acuity at follow-up. At 6 months, there were no significant differences in visual acuity among the four groups (P = 0.149, >0.05). There were no significant differences in refractive error among the four groups (P = 0.066, >0.05). Baseline axial length differed significantly among the four groups (P = 0.000, <0.001), with the LM group having longer axial length than the LL group (P < 0.001, paired samples t-test), and the HM group having longer axial length than the HL group (P < 0.001, paired samples t-test). However, there were no significant differences in axial length change compared to baseline among the groups at 1 month, 3 months, and 6 months (P 1 = 0.053; P 3 = 0.557; P 6 = 0.329, >0.05). Significant differences were observed in corneal flat K-value change compared to baseline among the four groups at 1 month, 3 months, and 6 months (P 1 = 0.001, P 3 = 0.001, P 6 = 0.004, <0.05). There were no significant differences in e-value change among the groups at 1 and 3 months (P 1 = 0.205, P 3 = 0.252, >0.05), but significant differences were found in e-value change compared to baseline at 6 months (P 6 = 0.010, <0.05). Multiple regression analysis with changes in central corneal flat K-value and e-value as independent variables and axial elongation as the dependent variable showed a correlation between e-value change at 6 months and axial elongation (P = 0.004, <0.05), indicating a negative correlation. Conclusion: Orthokeratology (OK) lenses effectively improve myopic children's vision by reshaping the cornea, leading to reduced central corneal curvature and flattening of its anterior surface. The effectiveness of OK lenses is not significantly affected by age or initial myopia severity. Children of varying ages and myopia levels experience similar levels of axial length control with OK lens wear. Changes in corneal shape due to OK lenses affect axial elongation, with greater changes in corneal morphology associated with smaller increases in axial length.

Abstract:Measurements of pulsed nuclear magnetic resonance relaxation times T1 and T2 were made at 2.7 MHz and 15 MHz on water protons in liver, kidney, spleen, muscle, and brain tissue from normal A.SW mice, and in the same tissues and tumors from A.SW mice developing MSWBS tumors (an ascites sarcoma) following dorsal sc implantation of tumor fragments. The measurement precision obtained from improved spectrometer design made it possible to show that T1 and T2 in all tissues except brain were increased by the presence of the tumor in the animal. The responses exhibited by T1 and T2 in liver and kidney were proportional to the size of the tumor. The smaller responses shown by T1 in spleen (15 MHz) and T1 and T2 in muscle (2.7 MHz) also showed a significant correlation with tumor size. The relaxation times for tumor (T1 at 2.7 MHz, T2 at 2.7 and 15 MHz) showed a significant negative correlation with tumor size: The times decreased as tumor size increased. The results were analyzed by use of the two-phase fast exchange model and were consistent with the effects expected if tissue water content increased and tumor water content decreased as tumor size increased. The analysis indicated that the effects arose primarily through changes in b, the fraction of water bound to fast exchange sites on the protein, with important modifications from changes in the correlation times Tc and Tm;Tr controlled the frequency that must be chosen for specific diagnostic applications.
One-year results for myopia control of orthokeratology with different back optic zone diameters: a randomized trial using a novel multispectral-based topographer

Abstract:AIM: To present the 1-year results of a prospective cohort study investigating the efficacy, potential mechanism, and safety of orthokeratology (ortho-k) with different back optic zone diameters (BOZD) for myopia control in children. METHODS: This randomized clinical study was performed between Dec. 2020 and Dec. 2021. Participants were randomly assigned to three groups wearing ortho-k: 5 mm BOZD (5-MM group), 5.5 mm BOZD (5.5-MM group), and 6 mm BOZD (6-MM group). The 1-year data were recorded, including axial length, relative peripheral refraction (RPR, measured by multispectral refractive topography, MRT), and visual quality. The contrast sensitivity (CS) was evaluated by CSV-1000 instrument with spatial frequencies of 3, 6, 12, and 18 cycles/degree (c/d); the corneal higher-order aberrations (HOAs) were measured by iTrace aberration analyzer. The one-way ANOVA was performed to assess the differences between the three groups. The correlation between the change in AL and RPR was calculated by Pearson’s correlation coefficient. RESULTS: The 1-year results of 20, 21, and 21 subjects in the 5-MM, 5.5-MM, and 6-MM groups, respectively, were presented. There were no statistical differences in baseline age, sex, or ocular parameters between the three groups (all P>0.05). At the 1-year visit, the 5-MM group had lower axial elongation than the 6-MM group (0.07±0.09 vs 0.18±0.11 mm, P=0.001). The 5-MM group had more myopic total RPR (TRPR, P=0.014), with RPR in the 15°–30° (RPR 15–30, P=0.015), 30°–45° (RPR 30–45, P=0.011), temporal (RPR-T, P=0.008), and nasal area (RPR-N, P<0.001) than the 6-MM group. RPR 15–30 in the 5.5-MM group was more myopic than that in the 6-MM group (P=0.002), and RPR-N in the 5-MM group was more myopic than that in the 5.5-MM group (P<0.001). There were positive correlations between the axial elongation and the change in TRPR (r=0.756, P<0.001), RPR 15–30 (r=0.364, P=0.004), RPR 30–45 (r=0.306, P=0.016), and RPR-N (r=0.253, P=0.047). The CS decreased at 3 c/d (P<0.001), and the corneal HOAs increased in the 5-MM group (P=0.030). CONCLUSION: Ortho-k with 5 mm BOZD can control myopia progression more effectively. The mechanism may be associated with greater myopic shifts in RPR.

Abstract:Objective: To study the effect of treatment zone (TZ) decentration on axial length growth (ALG) in adolescents after wearing the orthokeratology lenses (OK lenses). Materials and methods: This retrospective clinical study selected 251 adolescents who were fitted OK lenses at the Clinical College of Ophthalmology, Tianjin Medical University (Tianjin, China) from January 2018–December 2018 and wore them continuously for >12 months. The age of the subjects was 8–15 years, spherical equivalent (SE): −1.00 to −5.00 diopter (D), and astigmatism ≤ 1.50 D. The corneal topography were recorded at baseline and 1-, 6-, and 12-month visits, and the axial length (AL) were recorded at baseline and 6-, 12-month visits. The data of the right eye were collected for statistical analysis. Results: The subjects were divided into three groups according to the decentration distance of the TZ after wearing lenses for 1 month: 56 cases in the mild ( 1.0 mm). A significant difference was detected in the ALG between the three groups after wearing lenses for 6 and 12 months ( F = 10.223, P < 0.001; F = 13.380, P < 0.001, respectively). Among these, the 6- and 12-month ALG of the mild decentration group was significantly higher than that of the other two groups. Multivariable linear regression analysis showed that age, baseline SE, and 1-month decentration distance associated with the 12-month ALG ( P < 0.001, P < 0.001, and P = 0.001, respectively). Conclusion: The decentration of the TZ of the OK lens affected the growth of the AL in adolescents, i.e., the greater the decentration, the slower the ALG.

Abstract:PURPOSE: To investigate the effects of modifications in back optical zone diameter (BOZD), reverse curve width (RCW), and compression factor (CF) on refractive error changes and axial elongation in myopic children undergoing orthokeratology (ortho-k) over a 12-month period.METHOD: In this retrospective study, data from 126 myopic children undergoing ortho-k fitting were analyzed. Subjects were categorized into four distinct groups based on lens design parameters: Group A (BOZD 6.0 mm, RCW 0.6 mm, CF 0.75 D); Group B (BOZD 6.0 mm, RCW 0.6 mm, CF 1.25 D); Group C (BOZD 5.4 mm, RCW 0.9 mm, CF 1.25 D); and Group D (BOZD 5.0 mm, RCW 1.1 mm, CF 1.25 D). The study evaluated uncorrected visual acuity (UCVA), corneal topography, and axial length (AL) at intervals, using Linear Mixed Models (LMMs) for time-based changes, and ANOVA or Kruskal-Wallis tests for group differences in AL elongation. A multivariable regression analysis identified factors independently associated with AL elongation.RESULTS: Within the first day and week, all four groups displayed significant improvements in UCVA and alterations in corneal curvature, which subsequently stabilized. Although UCVA variations between groups were subtle, Group D had less corneal curvature change than Groups A and B initially and exhibited significantly less AL elongation after one year. No significant difference in corneal curvature change or AL elongation was observed between Group C and the other groups. Multiple regression analysis indicated that older baseline age, greater baseline spherical equivalent refractive error, and smaller BOZD were associated with less AL elongation.CONCLUSION: The study reveals a positive correlation between BOZD and axial length growth over the 12-month period. A pure 0.5 D CF increment demonstrates a nonsignificant impact. This study provides new ideas into optimizing the parameters of ortho-k lenses.

Long-term Follow-Up of Changes in Ocular Biometric Parameters in Orthokeratology Lens Wearers with Relatively Large-Scale Axial Length Reduction

Abstract:Abstract Background To investigate ocular biological characteristics for myopic children with axial length (AL) reduction during orthokeratology (Ortho-K) treatment and provide clinical clues for better myopia control effects. Methods Changes in ocular parameters and treatment zone (TZ) in 75 subjects who completed one-year Ortho-K treatment were retrospectively reviewed. The subjects were divided into two groups according to one-year AL change: the AL reduction group (n = 37) and the AL elongation group (n = 38). Univariate and multivariate regression analyses were performed to determine the association between TZ, ocular parameters, and AL change. Results There was no significant difference in baseline between the two groups (all P > 0.05). After one year of Ortho-K treatment, compared with those in the AL elongation group, children in the AL reduction group had a decreased anterior chamber depth (ACD) (P < 0.001), thickened crystalline lens thickness (CLT) (P = 0.002), thinned vitreous chamber depth (VCD) (P < 0.001) and smaller TZ (P = 0.03), but no difference in central corneal thickness (CCT) and pupil diameter (PD). In the multivariable analyses, AL reduction was negatively associated with baseline age (beta: − 0.048; 95% CI: − 0.083 to − 0.013; P = 0.009) and positively associated with the TZ (beta: 0.024; 95% CI: 0.009 to 0.040; P = 0.003). Conclusions In AL reduction eyes, thickened CLT, decreased ACD and thinned VCD were observed during Ortho-K treatment, which could be suggested as indicators for better myopia control effects in the clinic. Older baseline age and smaller TZ wearing Ortho-K were also associated with AL change. Thickened CLT may be a result of compensation for AL-reduction eyes.
Efficacy of Small Back Optic Zone Design on Myopia Control for Corneal Refractive Therapy (CRT): a One-Year Prospective Cohort Study.

Abstract:Background To investigate the control effect on the axial length elongation using corneal refractive therapy (CRT) with different optical zone diameters (BOZDs). We also sought to compare the difference in higher-order aberrations (HOAs), treatment zone (TZ) size and Zernike defocus coefficient with different BOZDs and seek the optimal parameter for predicting axial elongation. Methods This prospective cohort study included 7- to 14-year-olds fitted with orthokeratology (ortho-K) lenses of 5-mm (5-mm group) or 6-mm BOZD (6-mm group). Axial length (AL), corneal topography, HOAs and Zernike defocus coefficient were obtained at baseline, and 1, 3, 6, 9 and 12 months follow-up visits. Multivariate regression analyses were used to explore the association between AL change and ocular biometric parameters. Receiver operating characteristic (ROC) curve analysis was used to determine the best diagnostic value for AL change in ocular biometric parameters. Results In total, 301 participants completed the one-year follow-up. The mean AL change in the 5-mm group (0.13 ± 0.18 mm) was less than that in the 6-mm group (0.27 ± 0.15 mm) at the 12 months visit. The TZ size and decentration were smaller, while the Zernike defocus coefficient and HOAs were higher in the 5-mm group (all P < 0.05). Older age and smaller TZ size were protective factors against AL elongation in multiple regression. In predicting AL elongation, TZ diameter yielded an area under the ROC curve of 0.684 with a cut-off value of 3.82 mm. Conclusion The 5-mm group showed 0.14 mm (51.8%) less axial elongation than the 6-mm group. The 5-mm BOZD produced a smaller TZ size, higher Zernike defocus coefficient and higher HOA after reshaping of the cornea. TZ size was the best predictor of AL elongation. TZ diameter less than 3.82 mm may lead to AL elongation less than 0.2 mm in one year.

Abstract:Purpose: This study was conducted to determine whether orthokeratology (OK) lenses with a smaller back optic zone diameter (BOZD) could exhibit stronger myopia control effects. Method: A meta-analysis was registered in PROSPERO (CRD42023408184). A comprehensive systematic database search was conducted, encompassing PubMed, Cochrane Library, EMBASE, MEDLINE, Web of Science, Ovid, CNKI and CBM, to identify relevant studies up to 25 March 2023. The primary inclusion criteria for this meta-analysis were studies that investigated the myopia control effect of OK lenses with a small optical treatment area (≤5 mm). To assess the quality of the retrieved articles, two researchers evaluated them using the Cochrane bias risk assessment criteria. The primary outcome measures were the changes in axial length (AL) and refractive error, using the weighted mean differences (WMD) and 95% confidence intervals (CI) to assess differences between small and traditional back optical treatment zone groups in terms of these outcomes. Results: The analysis encompassed five eligible studies, with a 1 year duration. The average difference in AL between the groups was 0.12 mm (WMD = -0.12, 95% CI [-0.16, -0.09], p < 0.00001). Likewise, the average difference in refractive error between the two groups was 0.44 D (WMD = 0.44, 95% CI [0.30, 0.57], p < 0.00001). None of the studies reported severe adverse events. Conclusions: Current evidence suggests that OK lenses with smaller back optical treatment zone are more effective in preventing myopia progression than traditional lenses. However, a longer-term evaluation is warranted.
Long-Term Efficacy of Orthokeratology to Control Myopia Progression.

Abstract:OBJECTIVES:To assess the efficacy of orthokeratology in controlling the rate of myopia progression in children and investigate the factors associated with axial length (AL) growth rate with an average of 48 months of orthokeratology lens wear.METHODS:As a retrospective study, 84 subjects underwent relatively complete ophthalmologic examinations. After initial lens wear, AL was measured on average every 12 months. The linear mixed-effects model (LMM) was used to compare the differences in AL growth rates at each time interval. The contribution of the independent variables to AL change was assessed using multiple linear regression.RESULTS:In the LMM, there was a significant difference in the AL growth rate ( P <0.001) at each follow-up. The growth rate of AL was associated with initial AL, spherical equivalent refractive errors (SERs) and diameter of lens ( P =0.045, 0.003 and 0.037, respectively). When the baseline age was included as a factor, the influence of initial AL and SER became insignificant in the analysis, whereas age and diameter of lens were significantly correlated with the growth rate of AL ( P< 0.001 and P< 0.001, respectively). There were significant differences in growth rates among different age groups.CONCLUSIONS:Results of the study demonstrated that the factors associated with lower growth rate in AL were older age and longer diameter of lens.
Effects of Orthokeratology on Axial Length Growth in Myopic Anisometropes.

In Part One, 25 anisometropic participants (mean age, 11.2 ± 1.9 years; 11 females and 14 males) were fitted with ortho-k lenses in the more myopic eye only, and the rate of AL growth was compared between the ortho-k lens wearing eye and the untreated contralateral eye over an average period of 23.1 ± 8.3 months. In Part Two, 8 participants who developed myopia in the contralateral eye received ortho-k treatment in both eyes for an average of 12 months; the rate of AL growth before and after ortho-k treatment in the newly developed myopic eye was compared.RESULTS:In Part One, the rate of AL elongation in the ortho-k treated eye (0.08 ± 0.15 mm/year) was significantly slower than in the contralateral eye (0.39 ± 0.32 mm/year) (P < 0.001). At the completion of Part One, 16 out of 25 participants (64%) developed myopia in the initially non-myopic eye. In Part Two, the rate of AL elongation after ortho-k treatment in the newly developed myopic eye (0.20 mm/year) was significantly slower than that before ortho-k treatment in the same eye (0.49 mm/year) (P = 0.012).CONCLUSION:Ortho-k treatment slows AL growth in the more myopic eye of anisometropic patients; should the contralateral eye develop myopia in the future, ortho-k is capable of slowing down AL growth in that eye as well.

Abstract:AIM To explore the efficacy of the orthokeratology lens for anisometropic myopia progression. METHODS A retrospective study was performed. Cycloplegic refraction and axial length (AL) were collected from 50 children (10.52±1.72y) who visited Peking University Third Hospital from July 2015 to August 2020. These children's one eyes (Group A) received monocular orthokeratology lenses at first, after different durations (12.20±6.94mo), their contralateral eyes (Group B) developed myopia and receive orthokeratology as well. The data in 1-year of binocular period were recorded. AL growth rate (difference of follow-up and baseline per month) were compared between two groups by paired t test. Interocular differences of AL were compared by Wilcoxon test. RESULTS During monocular period, the AL growth rate of the Group A (0.008±0.022 mm/mo) was significantly slower than that of the Group B (0.038±0.018 mm/mo; P<0.0001). However, during binocular period, the AL growth rate of the Group A (0.026±0.014 mm/mo) was significantly faster than that of the Group B (0.016±0.015 mm/mo; P<0.0001). The AL difference between both eyes was 0.6 (0.46) mm, then significantly decreased to 0.22 (0.39) mm when started binocular treatment (P<0.0001). However, it was significantly increased to 0.30 (0.32) mm after a year (P<0.0001), but still significantly lower than baseline (P<0.0001). CONCLUSION The orthokeratology lens is efficient for control the AL elongation of monocular myopia eyes and reduce anisometropia. For the condition that the contralateral eyes develop myopia and receive orthokeratology lens later, there is no efficiency observed on control interocular difference of AL during binocular treatment.

Abstract:OBJECTIVES:To analyze the changes in visual performance and optical quality after orthokeratology (ortho-k) treatment and its correlation with the magnitude of lens decentration and the diameter of treatment zone (TxZ). METHODS:A prospective study was conducted in 27 eyes of 27 patients wearing ortho-k contact lens. Area under log contrast sensitivity function (AULCSF), modulation transfer function cutoff values (MTFcutoff), Strehl ratio (SR), and objective scattering index (OSI) were measured and compared before and 1 month after ortho-k lens treatment. Refractive sphere, astigmatism, and diameter and decentration of TxZ were tested against the change in AULCSF, MTFcutoff, SR, and OSI after ortho-k treatment in stepwise multiple linear regression models. RESULTS:Objective scattering index significantly increased and AULCSF, MTFcutoff, and SR significantly decreased after ortho-k lens wear (all P<0.001). The mean TxZ diameter was 3.61±0.26 mm, and the mean magnitude of TxZ decentration was 0.60±0.16 mm. The amount of lens decentration significantly correlated with the change in AULCSF and MTFcutoff (P=0.027 and P=0.005, respectively). The diameter of TxZ significantly correlated with the change in MTFcutoff and SR (P=0.005 and P=0.024, respectively). CONCLUSIONS:Visual and optical quality decreases after ortho-k treatment, which can be alleviated by a larger TxZ diameter and a smaller lens decentration.

Abstract:Purpose To evaluate the effect of 0.01% atropine combined with orthokeratology (OK) lens on axial elongation in schoolchildren with myopia. Methods Sixty children aged 8–12 years with spherical equivalent refraction (SER) from -1.00D to -4.00D in both eyes were enrolled in this randomized, double-masked, placebo-controlled, cross-over trial. Children who had been wearing OK lenses for 2 months were randomly assigned into combination group (combination of OK lens and 0.01% atropine) for 1 year followed by control group (combination of OK lens and placebo) for another 1 year or vice versa. This trial was registered in the Chinese Clinical Trial Registry (Number: ChiCTR2000033904, 16/06/2020). The primary outcome was changes in axial length (AL). Data of right eyes were analyzed. Results There were statistically significant differences in the changes in AL between combination and control groups after generalized estimating equation model adjusting for age and baseline SER ( p = 0.001). The mean axial elongation difference between combination and control groups was 0.10 mm in the first year (0.10 ± 0.13 mm vs. 0.20 ±0.15 mm; p = 0.01), and 0.09 mm in the second year (0.22 ± 0.10 mm vs. 0.13 ± 0.14 mm; p = 0.01), respectively. The mean axial elongation difference of two groups in the first year was similar to that in the second year during the cross-over treatment. Conclusion In central Mainland China in myopic children, the treatment of combination therapy is more effective than single OK lens in controlling axial elongation.

The Impact of Back Optic Zone Design in Orthokeratology on Visual Performance

Abstract:Purpose: To evaluate the visual performance in adolescents undergoing orthokeratology (OrthoK) treatment with two different optical zone diameters (OZDs). Methods: This randomized, double-masked, self-controlled prospective study was conducted at Tianjin Eye Hospital (Tianjin, China) in June 2022. Thirty-six eligible schoolchildren were enrolled and fitted with corneal refractive therapy lenses with two sizes of OZDs (5 mm [5OZ] and 6 mm [6OZ]). Each participant was randomized to wear the 5OZ in one eye and the 6OZ in the contralateral eye. Subjective visual quality was assessed using visual acuity, refraction, contrast sensitivity function, and visual symptoms, and the objective optical quality was assessed using ocular higher order aberrations (HOAs) and modulation transfer function (MTF). Results: Thirty-five myopic children completed a 1-month follow-up visit. The 5OZ lens induced significantly smaller treatment zone diameters than the 6OZ lens (P < 0.001). Subjective visual quality did not differ significantly between the two groups. Compared to baseline, aberrations of Z40, coma-like, spherical-like, and total HOAs in both groups increased significantly (P < 0.05). For the 3-mm pupils, spherical aberration in the 5OZ group was significantly higher than that in the 6OZ group (P < 0.05). The MTF value of the 6OZ group was significantly higher than that of 5OZ group for 0.3 and 1.5 cycles per degree for the 3-mm pupils (P = 0.006 and P = 0.026, respectively). However, HOAs or MTF did not differ significantly between the two groups for the 5-mm pupils. Conclusions: The difference induced by varying OZD was significant only in the smaller pupil condition. The selection of OZD in OrthoK designs in real-world patient management should be done while considering individual pupil size. Translational relevance: This study revealed that the objective visual quality of small OZD lenses was only slightly affected for the small pupil size.
Long-term effect of orthokeratology on choroidal thickness and choroidal contour in myopic children

Abstract:Purpose To investigate the long-term effect of orthokeratology (ortho-k) on the choroidal thickness and choroidal contour in myopic children. Methods Subjects were from a conducted 2-year randomised clinical trial. Children (n=80) aged 8–12 years with spherical equivalent refraction of −1.00 to −6.00D were randomly assigned to the control group (n=40) and ortho-k group (n=40). Optical coherence tomography images were collected at the baseline, 1-month, 6-month, 12-month, 18-month and 24-month visits, then the choroidal thickness and choroid contour were calculated. Axial length (AL) and other ocular biometrics were also measured. Results During 2 years, in the control group, the choroidal thickness became thinning and the choroidal contour became prolate with time at all visits (all p < 0.001). Ortho-k can improve the choroidal thickness (all p < 0.001) and maintain the choroidal contour at all visits (all p < 0.05). In the ortho-k group, the choroidal contour was less changed in the temporal than nasal (p = 0.008), and the choroidal thickness was more thickening in the temporal 3 mm (p < 0.001). Two-year change in choroidal thickness was significantly associated with the 2-year AL change in the control group (r=−0.52, p < 0.001), however, this trend was broken by ortho-k (r=−0.05, p = 0.342). After being adjusted by other variables in the multivariable regression model, the effect of ortho-k on choroidal thickness was stable. Conclusions In the current 2-year prospective study, ortho-k can improve the choroidal thickness and maintain the choroidal contour, but this effect diminished in a long term. Further study with larger sample size and longer follow-up is warranted to refine this issue.

Retardation of Myopia by Multifocal Soft Contact Lens and Orthokeratology: A 1-Year Randomized Clinical Trial

Objectives: This randomized, single-blind, clinical trial compared the effectiveness of multifocal soft contact lenses (MFSCLs), orthokeratology contact lenses (Ortho-kCLs), and single vision spectacles (SVSs) for myopia control. Methods: Sixty-six eligible Chinese subjects, aged 7 to 15 years old with cycloplegic refraction measurements between −1.00 and −8.00 diopters (D), astigmatism not more than 1.00 D, and no history of myopia control treatment, were randomly assigned to wear MFSCLs, Ortho-kCLs, or SVSs for 1 year. For all three groups, baseline measurements of cycloplegic refraction, axial length (AL), and corneal endothelial cell density (CECD) were made. At the 6- and 12-month follow-up visits, changes in cycloplegic refraction, AL, and CECD were measured in the MFSCL and SVS groups. For the Ortho-kCL group, only changes in the AL were measured at 6 and 12 months, and CECD was measured at the 12-month follow-up visit. Results: After 1 year of lens wear, myopia progression of the SVS group, −0.938±0.117 D, was greater than that of the MFSCLs group, −0.591±0.106 D (P=0.032). Thus, MFSCLs reduced the rate of myopia progression by 37.0% compared with the SVSs. The AL elongations after 1 year were 0.30±0.03 mm for MFSCLs (P=0.027 vs SVSs), 0.31±0.04 mm for Ortho-kCLs (P=0.049 vs SVSs), and 0.41±0.04 mm for SVSs. Compared with the SVS group, the reduction in AL elongation was 26.8% and 24.4% in the MFSCL and Ortho-kCL groups, respectively. There were no significant differences in CECD among the three groups (P>0.05). Conclusions: Compared with SVSs, wearing MFSCLs and Ortho-kCLs significantly delayed myopia progression. MFSCLs and Ortho-kCLs are safe and promising methods of myopia control (chictr.org number, ChiCTR2100048452).

Abstract:Purpose: To investigate the effects of overnight orthokeratology (OK) lenses wear on the interocular axial length (AL) difference in anisomyopic children.Methods: Sixty anisomyopic children (8-14 years old) were enrolled in this one-year prospective, randomised controlled study. Subjects were randomly assigned to wear OK lenses (OK group) or single-vision spectacles (control group). AL was monitored at baseline and every six months using an IOL Master biometer, and axial elongation and interocular AL differences were compared between the groups. Results: In the control group, the more myopic eyes had similar axial elongation (0.36 +/- 0.17 mm) to the less myopic eyes (0.37 +/- 0.17 mm) at the one-year follow-up (P > 0.05). In the OK group, the less myopic eyes exhibited significantly greater axial elongation (0.24 +/- 0.17 mm) than the more myopic eyes (0.13 +/- 0.13 mm) at the one-year follow-up (P < 0.05). The mean interocular AL difference significantly decreased in the OK group over one year, from 0.47 +/- 0.24 mm to 0.35 +/- 0.22 mm (P < 0.05). However, the mean interocular AL difference decreased only slightly in the control group, from 0.56 +/- 0.28 mm to 0.55 +/- 0.28 mm (P > 0.05).Conclusion: This randomised, controlled study demonstrated that OK reduces the interocular AL difference in anisomyopic children due to stronger myopic control of the more myopic eye.
