Abstract:In this work,in order to restore the movements of lower limbs for paralyzed patients,a functional electrical stimulation(FES) rehabilitation system based on brain-computer interface(BCI) was developed.BCI adopts the technology of steady-state visual evoked potential(SSVEP) based EEG.Linear discriminated analysis was used as the classifier to process frequency-domain features of EEG.Five types of motion intention can be recognized,i.e.start,fast,slow,stop and idle states.The intention recognized was translated as a command to trigger FES system,and stimulated the concerned muscles of lower limbs to generate movements,meanwhile joint angles were measured.Six healthy subjects took part in the pure BCI experiments,and two of the subjects took part in experiments of swinging and walking movements controlled by BCI-FES separately.During swinging experiment,rectus femoris was stimulated.During walking experiment,iliopsoas,glutaeus maximus,rectus femoris,and hamstrings of two legs were stimulated.FES artifacts on EEG were analyzed during experiments.The results showed that the motion intention was well recognized(average recognition accuracy rate was over 85%),and the subjects can realize the expected lower limb movements via FES accordingly.

Abstract:Abstract Rehabilitation medicine has developed rapidly in recent years. Brain computer interface and functional electrical stimulation are very cutting-edge technologies in this field. Because brain computer interface provides a real-time operation platform for patients to operate their limbs according to their intention for functional electrical stimulation, the research on BCI based FES has gradually increased in recent years. This paper discusses the current development status and technical application of FES and BCI. The research status of BCI based FES is discussed, and the existing problems of various research are summarized. According to the research findings, this field is a new technology with great application prospects in the field of modern rehabilitation engineering.

Abstract:Abstract Delivering short trains of electric pulses to the muscles and nerves can elicit action potentials resulting in muscle contractions. When the stimulations are sequenced to generate functional movements, such as grasping or walking, the application is referred to as functional electrical stimulation (FES). Implications of the motor and sensory recruitment of muscles using FES go beyond simple contraction of muscles. Evidence suggests that FES can induce short- and long-term neurophysiological changes in the central nervous system by varying the stimulation parameters and delivery methods. By taking advantage of this, FES has been used to restore voluntary movement in individuals with neurological injuries with a technique called FES therapy (FEST). However, long-lasting cortical re-organization (neuroplasticity) depends on the ability to synchronize the descending (voluntary) commands and the successful execution of the intended task using a FES. Brain-computer interface (BCI) technologies offer a way to synchronize cortical commands and movements generated by FES, which can be advantageous for inducing neuroplasticity. Therefore, the aim of this review paper is to discuss the neurophysiological mechanisms of electrical stimulation of muscles and nerves and how BCI-controlled FES can be used in rehabilitation to improve motor function.

Abstract:Brain–computer interface (BCI) controlled functional electrical stimulation (FES) is a novel technique for volitional control of FES using a signal derived from the brain. The intuitive nature of BCI-FES has promoted its use to control FES-based assistive device to augment an imagined movement. Researchers are now exploring extensively the use of the BCI-FES as a neurorehabilitation system based on the fact that it necessitates active participation and establishes temporal association between efferent information, from cortical activation due to attempted movement and afferent information due to peripheral stimulation. The research literature mainly focuses on people with stroke for BCI-FES rehabilitation whilst people with spinal cord injury (SCI) have been considered good candidates for BCI-FES as an assistive device. This chapter discusses application of BCI-FES as neurorehabilitative system for the hand function in people with SCI, and highlights issues related to altered sensorimotor cortical activity as a result of the injury. Based on experimental results this chapter also introduces a concept of researcher-therapist-caregiver knowledge transfer and introduces a concept of caregiver and patient self-managed BCI-FES for home-based rehabilitation.
The effect of brain-computer interface controlled functional electrical stimulation training on rehabilitation of upper limb after stroke: a systematic review and meta-analysis

Abstract:Introduction: Several clinical studies have demonstrated that brain-computer interfaces (BCIs) controlled functional electrical stimulation (FES) facilitate neurological recovery in patients with stroke. This review aims to evaluate the effectiveness of BCI-FES training on upper limb functional recovery in stroke patients. Methods: PubMed, Embase, Cochrane Library, Science Direct and Web of Science were systematically searched from inception to October 2023. Randomized controlled trials (RCTs) employing BCI-FES training were included. The methodological quality of the RCTs was assessed using the PEDro scale. Meta-analysis was conducted using RevMan 5.4.1 and STATA 18. Results: The meta-analysis comprised 290 patients from 10 RCTs. Results showed a moderate effect size in upper limb function recovery through BCI-FES training (SMD = 0.50, 95% CI: 0.26–0.73, I 2 = 0%, p < 0.0001). Subgroup analysis revealed that BCI-FES training significantly enhanced upper limb motor function in BCI-FES vs. FES group (SMD = 0.37, 95% CI: 0.00–0.74, I 2 = 21%, p = 0.05), and the BCI-FES + CR vs. CR group (SMD = 0.61, 95% CI: 0.28–0.95, I 2 = 0%, p = 0.0003). Moreover, BCI-FES training demonstrated effectiveness in both subacute (SMD = 0.56, 95% CI: 0.25–0.87, I 2 = 0%, p = 0.0004) and chronic groups (SMD = 0.42, 95% CI: 0.05–0.78, I 2 = 45%, p = 0.02). Subgroup analysis showed that both adjusting (SMD = 0.55, 95% CI: 0.24–0.87, I 2 = 0%, p = 0.0006) and fixing (SMD = 0.43, 95% CI: 0.07–0.78, I 2 = 46%, p = 0.02). BCI thresholds before training significantly improved motor function in stroke patients. Both motor imagery (MI) (SMD = 0.41 95% CI: 0.12–0.71, I 2 = 13%, p = 0.006) and action observation (AO) (SMD = 0.73, 95% CI: 0.26–1.20, I 2 = 0%, p = 0.002) as mental tasks significantly improved upper limb function in stroke patients. Discussion: BCI-FES has significant immediate effects on upper limb function in subacute and chronic stroke patients, but evidence for its long-term impact remains limited. Using AO as the mental task may be a more effective BCI-FES training strategy. Abstract:Brain–computer interface (BCI) is a novel communication method between brain and machine. It enables signals from the human brain to influence or control external devices. Currently, much research interest is focused on the BCI-based neural rehabilitation of patients with motor and cognitive diseases. Over the decades, BCI has become an alternative treatment for motor and cognitive rehabilitation. Previous studies demonstrated the usefulness of BCI intervention in restoring motor function and recovery of the damaged brain. Electroencephalogram (EEG)-based BCI intervention could cast light on the mechanisms underlying neuroplasticity during upper limb recovery by providing feedback to the damaged brain. BCI could act as a useful tool to aid patients with daily communication and basic movement in severe motor loss cases like amyotrophic lateral sclerosis (ALS). Furthermore, recent findings have reported the therapeutic efficacy of BCI in people suffering from other diseases with different levels of motor impairment such as spastic cerebral palsy, neuropathic pain, etc. Besides motor functional recovery, BCI also plays its role in improving the behavior of patients with cognitive diseases like attention-deficit/hyperactivity disorder (ADHD). The BCI-based neurofeedback training is focused on either reducing the ratio of theta and beta rhythm, or enabling the patients to regulate their own slow cortical potentials, and both have made progress in increasing attention and alertness. With summary of several clinical studies with strong evidence, we present cutting edge results from the clinical application of BCI in motor and cognitive diseases, including stroke, spinal cord injury, ALS, and ADHD.

Using SSVEP based brain-computer interface to control functional electrical stimulation training system

Abstract:In this work, a functional electrical stimulation (FES) training system using steady-state visual evoked potential (SSVEP) based brain-computer interface (BCI) was designed to realize the control of upper limb movements. Subjects were required to initiatively focus on one of five flickering lights with different frequencies on the computer screen, while the electroencephalogram (EEG) signal was acquired from the channels at the visual cortex region. The five primary flickering frequencies and their harmonic components were extracted as classification features from the EEG channels at the visual cortex region, and then linear discriminant analysis (LDA) classifier in pairwise strategy was used to decode the subject's intention corresponding to the flickering light that the subject was focusing on. Thereafter the user's intention was transformed into a command to trigger the FES system to generate the desired stimulation pattern. The experimental results showed that the feature extraction and classification methods were efficient in on-line classification. Moreover an energy bar was applied to the human-machine interaction interface to enhance the performance of the system as a dynamic feedback to the user. The results indicated that the subjects could control the FES training system to realize the predefined action sequences with their own intention. © 2011 IEEE.

Abstract:Brain computer interfaces (BCIs) have been demonstrated to have the potential to enhance motor recovery after stroke. However, some stroke patients with severe paralysis have difficulty achieving the BCI performance required for participating in BCI-based rehabilitative interventions, limiting their clinical benefits. To address this issue, we presented a BCI intervention approach that can adapt to patients' BCI performance and reported that adaptive BCI-based functional electrical stimulation (FES) treatment induced clinically significant, long-term improvements in upper extremity motor function after stroke more effectively than FES treatment without BCI intervention. These improvements were accompanied by a more optimized brain functional reorganization. Further comparative analysis revealed that stroke patients with low BCI performance (LBP) had no significant difference from patients with high BCI performance in rehabilitation efficacy improvement. Our findings suggested that the current intervention may be an effective way for LBP patients to engage in BCI-based rehabilitation treatment and may promote lasting motor recovery, thus contributing to expanding the applicability of BCI-based rehabilitation treatments to pave the way for more effective rehabilitation treatments.

Abstract:The neural pathways of patients with hand dysfunction are impaired between the brain and the hand or prosthetic. The brain-computer interface (BCI) as a novel technology can bypass the impaired bidirectional neural pathways, which can help to restore hand function. In this review, we focus on BCI used for restoring the hand function. Non-invasive methods like EEG perform well in extracting motor intention and somatic sensation. But EEG experiments need a lot of time to set up. The invasive BCI (iBCI) is characterized by fast construction time, high temporal and spatial resolution, and high classification accuracy. But iBCI requires an electrode array (EA) to be implanted in the patient's brain. The surgical procedure may cause secondary harm to the patients, and postoperative risks such as infection and displacement exist. Functional electrical stimulation (FES) based on BCI also plays an important role in hand function restoration. In this review, we will focus on BCI used for restoring the hand function.
Application of a Brain–Computer Interface System with Visual and Motor Feedback in Limb and Brain Functional Rehabilitation after Stroke: Case Report

Abstract:(1) Objective: To investigate the feasibility, safety, and effectiveness of a brain–computer interface (BCI) system with visual and motor feedback in limb and brain function rehabilitation after stroke. (2) Methods: First, we recruited three hemiplegic stroke patients to perform rehabilitation training using a BCI system with visual and motor feedback for two consecutive days (four sessions) to verify the feasibility and safety of the system. Then, we recruited five other hemiplegic stroke patients for rehabilitation training (6 days a week, lasting for 12–14 days) using the same BCI system to verify the effectiveness. The mean and Cohen’s w were used to compare the changes in limb motor and brain functions before and after training. (3) Results: In the feasibility verification, the continuous motor state switching time (CMSST) of the three patients was 17.8 ± 21.0s, and the motor state percentages (MSPs) in the upper and lower limb training were 52.6 ± 25.7% and 72.4 ± 24.0%, respectively. The effective training revolutions (ETRs) per minute were 25.8 ± 13.0 for upper limb and 24.8 ± 6.4 for lower limb. There were no adverse events during the training process. Compared with the baseline, the motor function indices of the five patients were improved, including sitting balance ability, upper limb Fugel–Meyer assessment (FMA), lower limb FMA, 6 min walking distance, modified Barthel index, and root mean square (RMS) value of triceps surae, which increased by 0.4, 8.0, 5.4, 11.4, 7.0, and 0.9, respectively, and all had large effect sizes (Cohen’s w ≥ 0.5). The brain function indices of the five patients, including the amplitudes of the motor evoked potentials (MEP) on the non-lesion side and lesion side, increased by 3.6 and 3.7, respectively; the latency of MEP on the non-lesion side was shortened by 2.6 ms, and all had large effect sizes (Cohen’s w ≥ 0.5). (4) Conclusions: The BCI system with visual and motor feedback is applicable in active rehabilitation training of stroke patients with hemiplegia, and the pilot results show potential multidimensional benefits after a short course of treatment.

Abstract:Rehabilitation of motor impairment after stroke has an important medical value. However, the present stroke rehabilitation mainly are passive, its efficacy is limited. The BCI-FES system focused on the limit of passive stroke rehabilitation, combined with motor imagery (MI) and functional electrical stimulation (FES), through the pattern recognition of motor imagery mode of patients with EEG signal, generated FES signal. The BCI-FES system achieved the active rehabilitation of patients with mind-control, stimulated brain plasticity and improved rehabilitation efficacy. In this study, the BCI-FES system and clinical rehabilitation evaluation of post-stroke hemiplegia patients was investigated, and the rehabilitation efficacy showed, affected motor related cortex of patient subject was activated significantly, and motor function was further enhanced, by brain plasticity guidance of BCI-FES.

Abstract:Background: Stroke is one of the most common neurological conditions that often leads to upper limb motor impairments, significantly affecting individuals' quality of life. Rehabilitation strategies are crucial in facilitating post-stroke recovery and improving functional independence. Functional Electrical Stimulation (FES) systems have emerged as promising upper limb rehabilitation tools, offering innovative neuromuscular reeducation approaches. Objective: The main objective of this paper is to provide a comprehensive systematic review of the start-of-the-art functional electrical stimulation (FES) systems for upper limb neurorehabilitation in post-stroke therapy. More specifically, this paper aims to review different types of FES systems, their feasibility testing, or randomized control trials (RCT) studies. Methods: The FES systems classification is based on the involvement of patient feedback within the FES control, which mainly includes "Open-Loop FES Systems" (manually controlled) and "Closed-Loop FES Systems" (brain-computer interface-BCI and electromyography-EMG controlled). Thus, valuable insights are presented into the technological advantages and effectiveness of Manual FES, EEG-FES, and EMG-FES systems. Results and discussion: The review analyzed 25 studies and found that the use of FES-based rehabilitation systems resulted in favorable outcomes for the stroke recovery of upper limb functional movements, as measured by the FMA (Fugl-Meyer Assessment) (Manually controlled FES: mean difference = 5.6, 95% CI (3.77, 7.5), P < 0.001; BCI-controlled FES: mean difference = 5.37, 95% CI (4.2, 6.6), P < 0.001; EMG-controlled FES: mean difference = 14.14, 95% CI (11.72, 16.6), P < 0.001) and ARAT (Action Research Arm Test) (EMG-controlled FES: mean difference = 11.9, 95% CI (8.8, 14.9), P < 0.001) scores. Furthermore, the shortcomings, clinical considerations, comparison to non-FES systems, design improvements, and possible future implications are also discussed for improving stroke rehabilitation systems and advancing post-stroke recovery. Thus, summarizing the existing literature, this review paper can help researchers identify areas for further investigation. This can lead to formulating research questions and developing new studies aimed at improving FES systems and their outcomes in upper limb rehabilitation.
Transformation of primary hamster embryo fibroblasts by type 2 simplex virus: evidence for a "hit and run" mechanism.

Abstract:The phenomenon of cell transformation by type 2 herpes simplex virus has been investigated. Primary hamster embryo fibroblasts were exposed to type 2 herpes virus under conditions which would restrict or inhibit the lytic events of virus-cell interaction. Cell lines were established by single-cell cloning. There was evidence of altered cell morphology with altered biological activity in terms of longevity and oncogenicity; there was, however, no evidence of virus specific antigen or incorporation of viral nucleic acid into the host cell genome. Virus specific antigen was only detected in the early passages of an uncloned transformed cell line. We are thus unable to confirm previous studies (vide supra) and are obliged to propose a "hit and run" model for in vitro cell transformation by type 2 herpes simplex virus.
A New Technique of Rehabilitation Training Based on Motor Imagine Using Brain Computer Interface-Functional Electric Stimulation System and It's Effect on Plasticity of Brain of a Stroke Patient

Abstract:Objective: To study the feasibility of brain computer interface combined with functional electric stimulation (BCI-FES) applied to a patient with stroke as a new rehabilitation training system and to explore its mechanism. Method: The participant was a 40 years old man with severe left-hand paralysis after stroke. He received 4 weeks training with BCI-FES system based on motor imagine. The motor function of upper limb was assessed and fMRI examination was conducted pre- and after training. Result: Maximum grasp-relax speed of affected hand increased by 24.7% after training. When motion task was executed the activations in primary motor area(M1) and supplement motor area(SMA) of ipsilateral hemisphere were observed with fMRI after training, and contralateral activations in M1 and premotor cortex(PMC) decreased in addition. When motor imagine task was executed, the activations transferred to bilateral SMA and ipsilateral posterior parietal after BCI training. Conclusion: BCI-FES was a kind of feasible rehabilitation therapy for stroke survivor. The mechanism of functional recovery by using BCI-FES was to promote the plasticity of central nervous system.

Abstract:Each year, 795,000 stroke patients suffer a new or recurrent stroke and 235,000 severe traumatic brain injuries (TBIs) occur in the US. These patients are susceptible to a combination of significant motor, sensory, and cognitive deficits, and it becomes difficult or impossible for them to perform activities of daily living due to residual functional impairments. Recently, sensorimotor rhythm (SMR)-based brain–computer interface (BCI)-controlled functional electrical stimulation (FES) has been studied for restoration and rehabilitation of motor deficits. To provide future neuroergonomists with the limitations of current BCI-controlled FES research, this chapter presents the state-of-the-art SMR-based BCI-controlled FES technologies, such as current motor imagery (MI) training procedures and guidelines, an EEG-channel montage used to decode MI features, and brain features evoked by MI.

Abstract:We had reviewed the researches on brain computer interface (BCI) technology in the treatment of motor dysfunction after stroke by scholars from domestic and foreign, to explain in four aspects, such as definition and classification, working principle and theoretical basis in clinical treatment, specific clinical application and the existing problems. BCI is a communication system technology that combines hardware and software, which enables human beings to interact with the surrounding environment without being affected by peripheral nerves and muscles by using the control signals generated by EEG activities. It will help patients achieve certain functions and improve the quality of life by controlling the external devices. BCI has the characteristics of high safety, convenient treatment and good effect. According to the location of signal acquisition, BCI is divided into non-invasive BCI and invasive BCI, while the non-invasive BCI is used more commonly in clinic. The work flow of BCI in treating stroke patients could be roughly divided into five steps: signal acquisition, preprocessing or signal enhancement, feature extraction, classification and control interface. At present, BCI technology has been applied to treat stroke patients in the upper limbs, hands, lower limbs or walking dysfunction. It is mainly through the methods of recovery and replacement to promote the brain plasticity of stroke patients, so as to help the motor function of stroke patients in rehabilitation. However, there are still some deficiencies of BCI technology in clinical, such as the quality of BCI signal acquisition, the parameter setting of BCI treatment exercise prescription, and the selection difficulties of BCI treatment mode. In conclusion, BCI has a good effect in the treatment of post-stroke motor dysfunction, which provided a new and effective reference to improve the clinical effect of BCI, but the next step is to further study the existing problems and the specific mechanism in order to improve the clinical effect of BCI.

Abstract:Advances in brain science and computer technology in the past decade have led to exciting developments in brain-computer interface (BCI), thereby making BCI a top research area in applied science. The renaissance of BCI opens new methods of neurorehabilitation for physically disabled people (e.g. paralyzed patients and amputees) and patients with brain injuries (e.g. stroke patients). Recent technological advances such as wireless recording, machine learning analysis, and real-time temporal resolution have increased interest in electroencephalographic (EEG) based BCI approaches. Many BCI studies have focused on decoding EEG signals associated with whole-body kinematics/kinetics, motor imagery, and various senses. Thus, there is a need to understand the various experimental paradigms used in EEG-based BCI systems. Moreover, given that there are many available options, it is essential to choose the most appropriate BCI application to properly manipulate a neuroprosthetic or neurorehabilitation device. The current review evaluates EEG-based BCI paradigms regarding their advantages and disadvantages from a variety of perspectives. For each paradigm, various EEG decoding algorithms and classification methods are evaluated. The applications of these paradigms with targeted patients are summarized. Finally, potential problems with EEG-based BCI systems are discussed, and possible solutions are proposed.

Abstract:Patients with severe CNS injuries struggle primarily with their sensorimotor function and communication with the outside world. There is an urgent need for advanced neural rehabilitation and intelligent interaction technology to provide help for patients with nerve injuries. Recent studies have established the brain-computer interface (BCI) in order to provide patients with appropriate interaction methods or more intelligent rehabilitation training. This paper reviews the most recent research on brain-computer-interface-based non-invasive rehabilitation systems. Various endogenous and exogenous methods, advantages, limitations, and challenges are discussed and proposed. In addition, the paper discusses the communication between the various brain-computer interface modes used between severely paralyzed and locked patients and the surrounding environment, particularly the brain-computer interaction system utilizing exogenous (induced) EEG signals (such as P300 and SSVEP). This discussion reveals with an examination of the interface for collecting EEG signals, EEG components, and signal postprocessing. Furthermore, the paper describes the development of natural interaction strategies, with a focus on signal acquisition, data processing, pattern recognition algorithms, and control techniques.
