Abstract:BACKGROUND AND OBJECTIVE: The issue of racial and ethnic disparities in healthcare has been a significant concern for many years. It encompasses various aspects, including disease prevention, diagnosis, management, and end-of-life (EOL) care. Research has found that timely intervention with palliative care can result in better EOL care and reduced healthcare costs. This review aims to detail the role of healthcare disparities impacting palliative care, hospice enrollment, and EOL care in patients with serious illnesses who are facing EOL. It addresses the factors that play a role in creating these disparities and describes specific interventions that may reduce disparities in the provision of EOL care.METHODS: Authors searched, PubMed Central, Medline, and PubMed databases using Racial Disparity and End-of-Life/Palliative Care combinations. A total of 57 studies were identified. All articles were reviewed, and the available evidence was synthesized and to identify key domains in EOL care impacted by racial disparities and the factors contributing to them.KEY CONTENT AND FINDINGS: Several patient, provider, and institutional level factors may be responsible for disparities seen in EOL care, including health literacy, access to care, mistrust of the healthcare system, social determinants of health (SDH), medical racism, cultural and religious customs, and communication at EOL. Disparities in EOL care experienced by minority patients is an extension of the systemic and institutionalized racism rampant in the healthcare system. Providers must work on multiple fronts to address this inequity and injustice, the first of which is recognition and conversation regarding disparities in EOL care.CONCLUSIONS: Disparities in communication, palliative and hospice care utilization, and symptom management must be eradicated. Palliative care and hospice should be made accessible for all patients and families experiencing severe illness regardless of their racial or ethnic background.

Abstract:Background: While disparities in end-of-life care have been well-documented, explanations for the persistence of disparities are less clear. This study sought to examine diverse perceptions of end-of-life care, especially regarding how medical professionals can better serve all populations. Objective: To investigate similarities and differences in end-of-life care preferences, across racial and ethnic groups. Design: This work consists of a qualitative study utilizing in-depth focus group discussions. Setting/Participants: Six community-based focus groups were conducted with a total of 39 participants. Two groups were composed of African American participants, 2 had Latino participants, and 2 groups had white participants. Results: Analysis produced 3 major themes: (1) clear, comprehensive, and culturally relevant provider–patient communication regarding serious illness; (2) provider characteristics and competency; and (3) health system supports and barriers. Although all groups had individuals who expressed a strong preference for direct communication, individuals varied within groups. All groups discussed concerns that the costs of care are high and that financial considerations are given more importance than high-quality care. Groups diverged in their focus on provider characteristics and feelings of marginalization. African American and Latino groups emphasized a desire to match characteristics with providers, and African American groups discussed that their marginalization in the health-care system requires hypervigilance to receive high-quality care. Conclusions: Improvements in care would come from acknowledging diversity within groups, provider demonstration of comfort and competence, more effective care coordination, and recruitment of providers who share similar characteristics with the communities they serve.
Racial and Ethnic Disparities in Intensity of Care at the End of Life for Patients With Lung Cancer: A 13-Year Population-Based Study

Abstract:PURPOSE Lung cancer is the leading cause of cancer death in the United States. Disparities in lung cancer mortality among racial and ethnic minorities are well documented. Less is known as to whether racial and ethnic minority patients with lung cancer experience higher rates of intensity of care at the end of life (EOL) compared with non-Hispanic White (NHW) patients. METHODS We conducted a population-based analysis of patients 18 years and older with a lung cancer diagnosis who died between 2005 and 2018 using the California Cancer Registry linked to patient discharge data abstracts. Our primary outcome was intensity of care in the last 14 days before death (defined as any hospital admission or emergency department [ED] visit, intensive care unit [ICU] admission, intubation, cardiopulmonary resuscitation [CPR], hemodialysis, and death in an acute care setting). We used multivariable logistic regression models to evaluate associations between race and ethnicity and intensity of EOL care. RESULTS Among 207,429 patients with lung cancer who died from 2005 to 2018, the median age was 74 years (range, 18-107) and 106,821 (51%) were male, 146,872 (70.8%) were NHW, 1,045 (0.5%) were American Indian, 21,697 (10.5%) were Asian Pacific Islander (API), 15,490 (7.5%) were Black, and 22,325 (10.8%) were Hispanic. Compared with NHW patients, in the last 14 days before death, API, Black, and Hispanic patients had greater odds of a hospital admission, an ICU admission, intubation, CPR, and hemodialysis and greater odds of a hospital or ED death. CONCLUSION Compared with NHW patients, API, Black, and Hispanic patients who died with lung cancer experienced higher intensity of EOL care. Future studies should develop approaches to eliminate such racial and ethnic disparities in care delivery at the EOL.

Abstract:Importance Shared decision-making is the preferred method for evaluating complex tradeoffs in the care of patients with critical illness. However, it remains unknown whether critical care clinicians engage diverse patients and caregivers equitably in shared decision-making. Objective To compare critical care clinicians' approaches to shared decision-making in recorded conversations with Black and White caregivers of patients with critical illness. Design, Setting, and Participants This thematic analysis consisted of unstructured clinician-caregiver meetings audio-recorded during a randomized clinical trial of a decision aid about prolonged mechanical ventilation at 13 intensive care units in the US. Participants in meetings included critical care clinicians and Black or White caregivers of patients who underwent mechanical ventilation. The codebook included components of shared decision-making and known mechanisms of racial disparities in clinical communication. Analysts were blinded to caregiver race during coding. Patterns within and across racial groups were evaluated to identify themes. Data analysis was conducted between August 2021 and April 2023. Main Outcomes and Measures The main outcomes were themes describing clinician behaviors varying by self-reported race of the caregivers. Results The overall sample comprised 20 Black and 19 White caregivers for a total of 39 audio-recorded meetings with clinicians. The duration of meetings was similar for both Black and White caregivers (mean [SD], 23.9 [13.7] minutes vs 22.1 [11.2] minutes, respectively). Both Black and White caregivers were generally middle-aged (mean [SD] age, 47.6 [9.9] years vs 51.9 [8.8] years, respectively), female (15 [75.0%] vs 14 [73.7%], respectively), and possessed a high level of self-assessed health literacy, which was scored from 3 to 15 with lower scores indicating increasing health literacy (mean [SD], 5.8 [2.3] vs 5.3 [2.0], respectively). Clinicians conducting meetings with Black and White caregivers were generally young (mean [SD] age, 38.8 [6.6] years vs 37.9 [8.2] years, respectively), male (13 [72.2%] vs 12 [70.6%], respectively), and White (14 [77.8%] vs 17 [100%], respectively). Four variations in clinicians' shared decision-making behaviors by caregiver race were identified: (1) providing limited emotional support for Black caregivers, (2) failing to acknowledge trust and gratitude expressed by Black caregivers, (3) sharing limited medical information with Black caregivers, and (4) challenging Black caregivers' preferences for restorative care. These themes encompass both relational and informational aspects of shared decision-making. Conclusions and Relevance The results of this thematic analysis showed that critical care clinicians missed opportunities to acknowledge emotions and value the knowledge of Black caregivers compared with White caregivers. These findings may inform future clinician-level interventions aimed at promoting equitable shared decision-making.

Abstract:Black Americans are less likely than White Americans to have advance directives, die while receiving hospice services, or have their end-of life wishes honored. The root causes of disparities include imbalance of resources, lack of trust in health care institutions, lack of adequate education regarding end-of-life options, communication differences of health care providers with black vs white patients, variable access to hospice services in different communities, and poorer pain management for Black patients compared to White patients. Because root causes are numerous, comprehensive solutions are required. When advance care planning is in place, people are more likely to choose care focused on priorities and comfort than on seeking aggressive, sometimes futile, interventions in the last weeks of life. One important component of the solution should include listening to narrative stories of Black people as they encounter life-limiting diagnoses. Gathering the stories about life events and how strength was found through adversities can be a tool for growing trusting relationships and engaging in shared decision-making. Health care professionals should invite Black patients with serious illnesses to explore the sources of their strengths and identify their core values to work toward developing directives for the nature and place of their end-of-life and help to mitigate disparities in high quality end-of-life care.

Abstract:Patients with advanced colorectal cancer nearing the end of life require a multidisciplinary approach to address the unique challenges they face. Using a case vignette, we outline the various stages of a patient's journey with advanced rectal cancer and the common obstacles to their care as they interface with the medical system. We highlight how Black persons might be vulnerable to differences in screening, treatment, procedural interventions, end-of-life care, and health care decision-making. The article also addresses how health inequities and barriers relating to concordant treatment goals can impact the relationship between patient and surgeon. This vignette illustrates the importance of ensuring a team approach to treatment and the value of early involvement of palliative medicine for patients with advanced colorectal cancer. Abstract:Acquired resistance to Schistosoma mansoni infection was measured in S. mansoni-infected or irradiated cercaria-immunized mice, and in normal mice to which the former had been surgically joined. Such parabiotic partners were shown to freely exchange humoral and cellular blood constituents. There was no detectable transfer of resistance from mice infected for 8, 12, or 28 weeks to their uninfected partners, even if parabiosis was established before the initial infection and maintained to autopsy. In comparison with parabiosed controls, the number of adult worms surviving from a challenge infection was reduced by 51-96% in the previously infected mice but was not significantly reduced in their uninfected partners. In contrast, mice immunized with irradiated cercariae and their nonimmunized parabiotic partners showed similar levels of resistance. These data indicate that the resistance induced in mice by irradiated cercariae can be transferred, confirm that at least under some experimental conditions the resistance induced in mice by a previous S. mansoni infection is not readily transferred, and provide additional evidence that the resistance induced by normal infection and irradiated cercarial immunization differ in some fundamental way.

Abstract:Esophageal cancer is an aggressive malignancy with rising incidence globally. Despite significant therapeutic advances, overall outcomes remain sobering among patients with advanced disease. Patients with advanced esophageal cancer suffer from a heavy burden of a wide array of symptoms including dysphagia and cachexia/anorexia, necessitating the need for focused symptom management and palliative care. Studies focused on other cancer types have shown that Black patients are more likely to receive aggressive care at end-of-life (EOL) and suffer from a decline in quality of life. We sought to investigate racial differences in EOL care among patients with advanced esophageal cancer in the US.

Abstract:Racial and ethnic disparities exist in the delivery of regional anesthesia in the United States. Anesthesiologists have ethical and economic obligations to address existing disparities in regional anesthesia care.Current evidence of racial and ethnic disparities in regional anesthesia utilization in adult patients in the United States is presented. Potential contributors and solutions to racial disparities are also discussed.Literature search was performed for studies examining racial and ethnic disparities in utilization of regional anesthesia, including neuraxial anesthesia and/or peripheral nerve blocks.While minoritized patients are generally less likely to receive regional anesthesia than white patients, the pattern of disparities for different racial/ethnic groups and for types of regional anesthetics can be complex and varied. Contributors to racial/ethnic disparities in regional anesthesia span hospital, provider, and patient-level factors. Potential solutions include standardization of regional anesthetic practices via Enhanced Recovery After Surgery (ERAS) pathways, increasing patient education, health literacy, language translation services, and improving diversity and cultural competency in the anesthesiology workforce.Racial and ethnic disparities in regional anesthesia exist. Contributors and solutions to these disparities are multifaceted. Much work remains within the subspecialty of regional anesthesia to identify and address such disparities.

Abstract:Objectives: To understand frontline ICU clinician’s perceptions of end-of-life care delivery in the ICU. Design: Qualitative observational cross-sectional study. Setting: Seven ICUs across three hospitals in an integrated academic health system. SUBJECTS: ICU clinicians (physicians [critical care, palliative care], advanced practice providers, nurses, social workers, chaplains). Interventions: None. Measurements and Main Results: In total, 27 semi-structured interviews were conducted, recorded, and transcribed. The research team reviewed all transcripts inductively to develop a codebook. Thematic analysis was conducted through coding, category formulation, and sorting for data reduction to identify central themes. Deductive reasoning facilitated data category formulation and thematic structuring anchored on the Systems Engineering Initiative for Patient Safety model identified that work systems (people, environment, tools, tasks) lead to processes and outcomes. Four themes were barriers or facilitators to end-of-life care. First, work system barriers delayed end-of-life care communication among clinicians as well as between clinicians and families. For example, over-reliance on palliative care people in handling end-of-life discussions prevented timely end-of-life care discussions with families. Second, clinician-level variability existed in end-of-life communication tasks. For example, end-of-life care discussions varied greatly in process and outcomes depending on the clinician leading the conversation. Third, clinician-family-patient priorities or treatment goals were misaligned. Conversely, regular discussion and joint decisions facilitated higher familial confidence in end-of-life care delivery process. These detailed discussions between care teams aligned priorities and led to fewer situations where patients/families received conflicting information. Fourth, clinician moral distress occurred from providing nonbeneficial care. Interviewees reported standardized end-of-life care discussion process incorporated by the people in the work system including patient, family, and clinicians were foundational to delivering end-of-life care that reduced both patient and family suffering, as well as clinician moral distress. Conclusions: Standardized work system communication tasks may improve end-of life discussion processes between clinicians and families.

Abstract:Objective Protocol to explore what is known about communication between critical care providers and patients and families from culturally and linguistically diverse backgrounds (defined as people who are either from minority ethnic groups, non-English-speaking backgrounds who may have diverse cultural, linguistic, spiritual and religious affiliations and opinions) about death, dying, end-of-life care and organ donation in the intensive care unit (ICU). Introduction Patients from culturally and linguistically diverse backgrounds experience barriers to optimised care when admitted to the ICU. These barriers appear to derive from differences in language, cultural, societal and ethical expectations between patients, their families and healthcare professionals. These barriers may significantly impact the delivery of end-of-life care to patients from culturally and linguistically diverse backgrounds. Therefore, this has the potential for inadequate management of medical, psychological and existential distress. Inclusion criteria Studies of all designs reporting for adult (age ≥18 years) patients and family members from culturally and linguistically diverse backgrounds at end-of-life in the ICU setting will be included. Studies that report results for patients aged <18 years or that are based outside the ICU will be excluded. Methods Relevant sources will be retrieved, and their citation details will be imported into the Joanna Briggs Institute (JBI) System for the Unified Management, Assessment and Review of Information. This scoping review was guided by the JBI methodology for scoping reviews and reported in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews guidelines. A systematic search was conducted in EBSCOhost, Web of Science, PubMed Central and SciELO, OVID Medline, CINAHL, and Scopus, limited to English-language publications, without date limitation. Key study characteristics and findings will be extracted using a data extraction tool developed by the reviewers. Anticipating heterogeneous study designs, findings will be presented as a thematic synthesis. Ethics and dissemination This is a protocol for a scoping review, formal ethics approval from the Human Research Ethics Committee (HREC) of the Local Health Network will be obtained for research projects that could potentially stem from this review and will then be subsequently disseminated through proper channels.

Barriers to Hospice Care in Trauma Patients: The Disparities in End-of-Life Care

Abstract:Introduction: End-of-life and palliative care are important aspects of trauma care and are not well defined. This analysis evaluates the racial and socioeconomic disparities in terms of utilization of hospice services for critically ill trauma patients. Methods: Trauma patients ≥15 years old from 2012 to 2015 were queried from the National Trauma Databank. Chi-square and multivariate logistic regression analyses for disposition to hospice were performed after controlling for age, gender, comorbidities, injury severity, insurance, race, and ethnicity. Negative binomial regression analysis with margins for length of stay (LOS) was calculated for all patients discharged to hospice. Results: Chi-square analysis of 2 966 444 patient’s transition to hospice found patients with cardiac disease, bleeding and psychiatric disorders, chemotherapy, cancer, diabetes, cirrhosis, respiratory disease, renal failure, cirrhosis, and cerebrovascular accident (CVA) affected transfer ( P < .0001). Logistic regression analysis after controlling for covariates showed uninsured patients were discharged to hospice significantly less than insured patients (odds ratio [OR]: 0.71; P < .0001). Asian, African American, and Hispanic patients all received less hospice care than Caucasian patients (OR: 0.65, 0.60, 0.73; P < .0001). Negative binomial regression analysis with margins for LOS showed Medicare patients were transferred to hospice 1.2 days sooner than insured patients while uninsured patients remained in the hospital 1.6 days longer ( P < .001). When compare to Caucasians, African Americans patients stayed 3.7 days longer in the hospital and Hispanics 2.4 days longer prior to transfer to hospice ( P < .0001). In all patients with polytrauma, African Americans stayed 4.9 days longer and Hispanics 2.3 days longer as compared to Caucasians ( P < .0001). Conclusions: Race and ethnicity are independent predictors of a trauma patient’s transition to hospice care and significantly affect LOS. Our data demonstrate prominent racial and socioeconomic disparities exist, with uninsured and minority patients being less likely to receive hospice services and having a delay in transition to hospice care when compared to their insured Caucasian counterparts.

Abstract:OBJECTIVE: To examine whether regional practice patterns impact racial/ethnic differences in intensity of end-of-life care for cancer decedents.DATA SOURCES: The linked Surveillance, Epidemiology, and End Results (SEER)-Medicare database.STUDY DESIGN: We classified hospital referral regions (HRRs) based on mean 6-month end-of-life care expenditures, which represented regional practice patterns. Using hierarchical generalized linear models, we examined racial/ethnic differences in the intensity of end-of-life care across levels of HRR expenditures.PRINCIPAL FINDINGS: There was greater variation in intensity of end-of-life care among Hispanics, Asians, and whites in high-expenditure HRRs than in low-expenditure HRRs.CONCLUSIONS: Local practice patterns may influence racial/ethnic differences in end-of-life care.

Understand limitations of current tools for measuring mood disorder symptoms, caregiver burden, and communication perceptions Importance Caring for a person at the end of life presents many challenges for caregivers and requires support, but racial disparities in family caregiver resources and supports persist. It is unknown whether disparities in anxiety, depression, quality of life, or hospice communication exist between Black and White hospice family caregivers. Objective(s) To describe Black and White hospice family caregivers and compare anxiety, depression, quality of life, caregiver burden, and perceptions of caregiver-centered hospice team communication between Black and White hospice family caregivers. Method(s) In this secondary analysis of baseline data from two randomized clinical trials, we analyzed data from 722 Black and White hospice family caregivers ages 18+ from the Midwestern and Northeastern United States. Outcomes included anxiety, depression, quality of life, caregiver burden, and caregiver-rated perceptions of caregiver-centered hospice communication. Results Greater proportions of Black caregivers were unmarried, had less formal education, had lower household income, and endured employment-related changes due to caregiving. Despite fewer economic supports yet similar caregiving responsibilities, in adjusted analyses Black caregivers were not more likely to experience lower quality of life (including in the financial domain) or greater anxiety, depression, or caregiver burden compared to White caregivers. 32% of hospice family caregivers had moderate to severe anxiety, and 32% had moderate to severe depression. Black and White hospice family caregivers similarly perceived caregiver-centered communication by hospice teams. Conclusion(s) Anxiety and depression are highly prevalent among hospice family caregivers. Despite having fewer socioeconomic resources than White hospice family caregivers, Black hospice family caregivers were not more likely to experience anxiety, depression, or caregiver burden, as measured by standard instruments. Hospice team communication was perceived similarly across races. Impact Interventions that support hospice family caregivers across races and culturally concordant tools that identify mood disorders and burdens in non-White caregiver populations are needed. Research is needed to understand how Black culture and perceptions of caregiving help mediate the effects of socioeconomic stressors and social determinants of caregiver health on mental health outcomes.

Disparities in Palliative and Hospice Care and Completion of Advance Care Planning and Directives Among Non-Hispanic Blacks: A Scoping Review of Recent Literature

Abstract:Objectives: Published research in disparities in advance care planning, palliative, and end-of-life care is limited. However, available data points to significant barriers to palliative and end-of-life care among minority adults. The main objective of this scoping review was to summarize the current published research and literature on disparities in palliative and hospice care and completion of advance care planning and directives among non-Hispanc Blacks. Methods: The scoping review method was used because currently published research in disparities in palliative and hospice cares as well as advance care planning are limited. Nine electronic databases and websites were searched to identify English-language peer-reviewed publications published within last 20 years. A total of 147 studies that addressed palliative care, hospice care, and advance care planning and included non-Hispanic Blacks were incorporated in this study. The literature review include manuscripts that discuss the intersection of social determinants of health and end-of-life care for non-Hispanic Blacks. We examined the potential role and impact of several factors, including knowledge regarding palliative and hospice care; healthcare literacy; communication with providers and family; perceived or experienced discrimination with healthcare systems; mistrust in healthcare providers; health care coverage, religious-related activities and beliefs on palliative and hospice care utilization and completion of advance directives among non-Hispanic Blacks. Discussion: Cross-sectional and longitudinal national surveys, as well as local community- and clinic-based data, unequivocally point to major disparities in palliative and hospice care in the United States. Results suggest that national and community-based, multi-faceted, multi-disciplinary, theoretical-based, resourceful, culturally-sensitive interventions are urgently needed. A number of practical investigational interventions are offered. Additionally, we identify several research questions which need to be addressed in future research.
Exploring Pediatric Code Status, Advance Care Planning, and Mode of Death Disparities at End of Life

Abstract:Context Children from marginalized groups are at risk for worse medical outcomes, yet little is known about their end of life experiences. Objective We examine the characteristics of deceased children with a focus on race, ethnicity, and preferred language. Methods We conducted a cross-sectional study of patients who died at Texas Children's Hospital from 2018-2019. Demographics, date of death, and final code status were extracted. Medical complexity prior to admission and mode of death were obtained from chart review. Results We included 433 patients. Over one-third were Hispanic (39.3%) with 42.3% preferring Spanish. The majority were White (61.4%) or Black (21.0%). Most patients had significant medical complexity (52.0%) or were infants in their birth admission (29.4%). Half (52.4%) received palliative care; patients with increased medical complexity were more likely to have palliative care involved (p<0.001). There were no differences in palliative care, code status, or mode of death by race. Hispanic patients were less likely to have a full code status (OR 0.42, 95% CI 0.25-0.73). Spanish-speaking patients were more likely to have palliative care involvement (OR 2.05, 95% CI 1.21-3.46) and less likely full code orders (OR 0.24, 95% CI 0.1-0.63). Conclusion Palliative care services are engaged with most children at end of life and is accessible to marginalized patient groups. Spanish-speaking patients have different code status orders and modes of death at end of life. Further studies are needed to elucidate explanatory factors for differences revealed and multi-center studies are needed to characterize more widespread experiences.

Abstract:‘Prevalence of Tourette syndrome’ SIR–I wish to comment on Professor Traverse’s recent correspondence regarding the prevalence of Tourette syndrome (TS), published in Developmental Medicine and Child Neurology 40: 847. In agreement with the views of Professor Traverse, I have proposed a parallel between the increased usage of neurostimulants, especially methylphenidate (MPH) and the increased prevalence of TS in the US since the 1970s1. Furthermore, the remarkably large percentage of patients with TS and comorbid attention-deficit–hyperactivity disorder (ADHD) suggests that MPH is a major, probably the most common, precipitating cause of TS and tics in the US1. In a recent letter to the Journal of the Royal Society of Medicine, submitted in November 1998, I indicated that the increase in the prevalence of TS noted in the US would not be expected in the UK, because MPH was not generally prescribed2. At a meeting in Tunbridge Wells, England, in the 1970s, as a speaker and a privileged guest of the late Dr Ronald Mac Keith, I was criticized, even gently scolded, for my interest and research in ADHD, then called minimal brain dysfunction. Dr Mac Keith expressed the opinion that the entity was overemphasized in the US and did not exist in the UK at that time. In recent years the diagnosis of ADHD has received limited attention in the UK, and my British colleagues have begun to treat it with MPH3. The relation between MPH and TS is controversial. A review article from the National Institute of Mental health (NIMH), Bethesda, US, tends to minimize the risk of neurostimulant medication in patients with tics and TS4. The authors condone the use of neurostimulants but caution that parental informed consent should be obtained, a practice that could be questioned on ethical grounds. An estimated 50 to 80% of patients with TS have ADHD, and the use of MPH is cited as the cause of tics in more than 25% of children with ADHD5. Contrary to the NIMH review, evidence favors avoidance of the use of MPH in patients with a personal or family history of TS1, and dosage in treatment of ADHD should be conservative.
Unraveling Racial Disparities in Supportive Care Medication Use among End-of-Life Pancreatic Cancer Patients: Focus on Pain Management and Psychiatric Therapies.

Abstract:AbstractBackground:Supportive care medication use differences may contribute to racial disparities observed in health-related quality of life in patients with pancreatic cancer.Methods:In this observation study using the Surveillance, Epidemiology, and End Results-Medicare linked database, we sought to examine supportive care medication use disparities in patients with pancreatic cancer from 2005 to 2017 by race and ethnicity.Results:Among 74,309 patients included in the final analysis, racial and ethnic disparities in the use of supportive care medications were identified. After adjustment for confounding factors and compared with non-Hispanic Whites, minorities had significantly less use of opioids [Black: adjusted OR (aOR), 0.84; 95% confidence interval (CI), 0.79–0.88; Asian: aOR, 0.84; 95% CI, 0.79–0.90), and skeletomuscular relaxants (Black: aOR, 0.90; 95% CI, 0.82–0.99; Hispanic: aOR, 0.82; 95% CI, 0.74–0.91; Asian: aOR, 0.59; 95% CI, 0.51–0.68), and increased use of non-opioid analgesics (Hispanic: aOR, 1.16; 95% CI, 1.01–1.14; Asian: aOR, 1.37; 95% CI, 1.26–1.49). Racial and ethnic minorities had less use of antidepressants (Black: aOR, 0.56; 95% CI, 0.53–0.59; Hispanic: aOR, 0.77; 95% CI, 0.73–0.82; Asian: aOR, 0.47; 95% CI, 0.44–0.51), anxiolytics (Black: aOR, 0.78; 95% CI, 0.74–0.82; Hispanic: aOR, 0.66; 95% CI, 0.62–0.71; Asian: aOR, 0.52; 95% CI, 0.48–0.57), and antipsychotics (Hispanic: aOR, 0.90; 95% CI, 0.82–0.99; Asian: aOR, 0.84; 95% CI, 0.74–0.95).Conclusions:Racial and ethnic disparities in the use of supportive care medications among patients with pancreatic cancer were observed, with the differences unexplained by sociodemographic factors.Impact:Future studies should identify strategies to promote equitable use of supportive care medications among racial minorities and explore factors that may influence their use in these populations.
Racial Differences in Palliative Care Use in Heart Failure Decedents

Abstract:Background Minoritized individuals experience greater heart failure (HF) incidence and mortality, yet racial disparities in palliative care (PC) in HF are unknown. Methods This retrospective study used electronic medical records to identify adults who were hospitalized at an academic health system and died from HF between 2012-2018. Using multivariable logistic regression, we examined associations between decedent characteristics and PC consultations (PCC). Results Of 1,987 decedents, 45.8% (n=911) received PCCs. Black decedents had 60% greater odds of receiving PCC (OR = 1.60; 95% CI = 1.21-2.11) than Whites. Median time from PCC to death was shorter among White than Black decedents (31.2 vs 51.5 days, p=.001). Mean age at death was younger among Black versus White decedents (71.3 (14.8) vs 81.8 (12.3), p<.001) and decedents of "other" race (71.3 (14.8) vs. 80.3 (10.4), p=.001). Black decedents were more likely than Whites to receive inotropes (54.4% vs. 42.3%, p<.001), and to be admitted to the hospital (39.5% vs. 29.7%, p<.001) and ICU in their last month (30.3% vs. 18.3%, p<.001). Conclusions Findings suggest greater recognition of palliative needs among Black individuals with HF; however, most referrals to PC occur late in the disease trajectory.
