Abstract:Purpose: Perceptual learning (PL) has shown promising performance in restoring visual function in adolescent amblyopes. We retrospectively compared the effect of a well-accepted PL paradigm on patients with anisometropic amblyopia with or without a patching therapy history (patching therapy [PT] group versus no patching therapy [NPT] group).Methods: Eighteen PT and 13 NPT patients with anisometropic amblyopia underwent monocular PL for 3 months. During training, patients practiced a Gabor detection task following the lateral masking paradigm by applying a temporal two-alternative forced choice procedure with the amblyopic eye. Monocular contrast sensitivity functions (CSF), visual acuity, interocular differences in visual function metrics, and stereoacuity were compared before and after training.Results: PL improved the visual acuity of the amblyopia eyes by 0.5 lines on average in the PT group and 1.5 lines in the NPT group. A significant reduction in the interocular difference in visual acuity was observed in the NPT group (P < 0.01) but not in the PT group (P = 0.05). Regarding CSF metrics, the area under the log CSF and cutoff in the amblyopic eyes of the NPT groups increased after training (P < 0.05). In addition, the interocular differences of the CSF metrics (P < 0.05) in the NPT group were significantly reduced. However, in the PT group, all the CSF metrics were unchanged after training. A total of 27 of 31 patients in both groups had no measurable stereopsis pretraining, and recovery after training was not significant.Conclusions: PL based on a lateral masking training paradigm improved visual function in anisometropic amblyopia. Patients without a patching history achieved greater benefits.Translational Relevance: PL based on a lateral masking training paradigm could be a new treatment for amblyopia.

Enhanced neural function in highly aberrated eyes following perceptual learning with adaptive optics

Abstract:Highly aberrated keratoconic (KC) eyes do not elicit the expected visual advantage from customized optical corrections. This is attributed to the neural insensitivity arising from chronic visual experience with poor retinal image quality, dominated by low spatial frequencies. The goal of this study was to investigate if targeted perceptual learning with adaptive optics (AO) can stimulate neural plasticity in these highly aberrated eyes. The worse eye of 2 KC subjects was trained in a contrast threshold test under AO correction. Prior to training, tumbling 'E' visual acuity and contrast sensitivity at 4, 8, 12, 16, 20, 24 and 28 c/deg were measured in both the trained and untrained eyes of each subject with their routine prescription and with AO correction for a 6mm pupil. The high spatial frequency requiring 50% contrast for detection with AO correction was picked as the training frequency. Subjects were required to train on a contrast detection test with AO correction for 1h for 5 consecutive days. During each training session, threshold contrast measurement at the training frequency with AO was conducted. Pre-training measures were repeated after the 5 training sessions in both eyes (i.e., post-training). After training, contrast sensitivity under AO correction improved on average across spatial frequency by a factor of 1.91 (range: 1.77-2.04) and 1.75 (1.22-2.34) for the two subjects. This improvement in contrast sensitivity transferred to visual acuity with the two subjects improving by 1.5 and 1.3 lines respectively with AO following training. One of the two subjects denoted an interocular transfer of training and an improvement in performance with their routine prescription post-training. This training-induced visual benefit demonstrates the potential of AO as a tool for neural rehabilitation in patients with abnormal corneas. Moreover, it reveals a sufficient degree of neural plasticity in normally developed adults who have a long history of abnormal visual experience due to optical imperfections.
Factors Affecting Long-Term Compliance with Rigid Gas-Permeable Contact Lens Wear in Patients with Keratoconus

Abstract:The purpose of the study was to investigate the factors affecting long-term compliance with rigid gas-permeable contact lens (RGPCL) wear in patients with keratoconus (KC). A total of 189 patients with KC (374 eyes) were included in the study, and were divided into two groups: the compliant group and the non-compliant group. Corneal topographic measurements, refractive results, and RGPCL parameters were compared between the two groups. A vision-related quality of life questionnaire was completed by all of the patients. The results demonstrated that patients diagnosed with bilateral KC were more compliant with RGPCL wear than patients diagnosed with unilateral KC (p = 0.0167). There were no significant differences between the compliant and non-compliant groups in terms of their corneal topographic measurements, refractive results, RGPCL parameters, or corneal cross-linking surgery history (all p > 0.05). In contrast, KC patients’ subjective experience with RGPCL wear—including visual acuity (p = 0.006), overall satisfaction (p < 0.001), quality of life (p < 0.001), and good adaptation during the short-term (p < 0.001)—had a significant effect on the long-term compliance with RGPCL wear. In conclusion, patients’ subjective experiences, rather than their ocular biometrics, significantly influence their long-term compliance with RGPCL wear.

Abstract:Proinflammatory cytokines may be important in the pathogenesis of human immunodeficiency virus type 1 (HIV-1) disease. Tenidap decreases interleukin (IL)-6, IL-1, and tumor necrosis factor (TNF) production by peripheral blood mononuclear cells and decreases IL-6 plasma levels in rheumatoid arthritis patients. In this randomized double-blind study, 43 HIV-1-infected patients received tenidap (120 mg) or placebo daily for 6 weeks and then crossed over to the alternative therapy for an additional 6 weeks. Mean entry CD4 cell count was 140/microL. Analyses were performed on cytokines, acute-phase proteins, virus load, and CD4 cell counts. With the exception of small differences in plasma TNF levels, tenidap had no significant effect on these indices. Significant correlations of plasma IL-6 and TNF levels with HIV-1 RNA were noted. Six patients discontinued tenidap due to rash. The effects of tenidap in HIV-1 infection contrast to results in arthritis patients, in whom tenidap decreased plasma levels of IL-6 and acute-phase proteins.
Retardation of Myopia by Multifocal Soft Contact Lens and Orthokeratology: A 1-Year Randomized Clinical Trial

Abstract:Supplemental Digital Content is Available in the Text. Objectives: This randomized, single-blind, clinical trial compared the effectiveness of multifocal soft contact lenses (MFSCLs), orthokeratology contact lenses (Ortho-kCLs), and single vision spectacles (SVSs) for myopia control. Methods: Sixty-six eligible Chinese subjects, aged 7 to 15 years old with cycloplegic refraction measurements between −1.00 and −8.00 diopters (D), astigmatism not more than 1.00 D, and no history of myopia control treatment, were randomly assigned to wear MFSCLs, Ortho-kCLs, or SVSs for 1 year. For all three groups, baseline measurements of cycloplegic refraction, axial length (AL), and corneal endothelial cell density (CECD) were made. At the 6- and 12-month follow-up visits, changes in cycloplegic refraction, AL, and CECD were measured in the MFSCL and SVS groups. For the Ortho-kCL group, only changes in the AL were measured at 6 and 12 months, and CECD was measured at the 12-month follow-up visit. Results: After 1 year of lens wear, myopia progression of the SVS group, −0.938±0.117 D, was greater than that of the MFSCLs group, −0.591±0.106 D (P=0.032). Thus, MFSCLs reduced the rate of myopia progression by 37.0% compared with the SVSs. The AL elongations after 1 year were 0.30±0.03 mm for MFSCLs (P=0.027 vs SVSs), 0.31±0.04 mm for Ortho-kCLs (P=0.049 vs SVSs), and 0.41±0.04 mm for SVSs. Compared with the SVS group, the reduction in AL elongation was 26.8% and 24.4% in the MFSCL and Ortho-kCL groups, respectively. There were no significant differences in CECD among the three groups (P>0.05). Conclusions: Compared with SVSs, wearing MFSCLs and Ortho-kCLs significantly delayed myopia progression. MFSCLs and Ortho-kCLs are safe and promising methods of myopia control (chictr.org number, ChiCTR2100048452).

Abstract:OBJECTIVE:To investigate the effect of conductive keratoplasty (CK) for presbyopia and 2 years follow-up. METHODS:This study is prospective clinical trial. CK was performed on 34 patients for presbyopia, in which 26 hyperopic patients underwent binocular operations and 8 emmetropic patients underwent monocular operation. The following-up time was 24 months. RESULTS:At 24 months postoperatively, for the hyperopia group, binocular uncorrected near visual acuity (33 cm) (5-logMAR) (4.63 ± 0.12) was increased significantly (t = 9.237, P < 0.001) compared pre-operatively (4.06 ± 0.15); binocular uncorrected distance visual acuity (4.99 ± 0.02) was significantly increased (t = 6.718, P < 0.05) compared pre-operatively (4.82 ± 0.21); for the emmetropia group, binocular uncorrected near visual acuity (33 cm) (5-logMAR) (4.68 ± 0.16) was increased significantly (t = 10.413, P < 0.001) compared pre-operatively (4.13 ± 0.18); binocular uncorrected distance visual acuity was same as pre-operative one; compared pre-operatively (+0.97 ± 0.63D), manifest refractive spherical equivalent was decreased significantly (P < 0.001) to peak value (-1.21 ± 1.00) D at 1 week, and then regressed to a relative plateau (-0.40 ± 0.70) D at 24 months; the regressive rate was decreased from (+0.35 ± 0.44) D/month at 1 month postoperatively to (+0.01 ± 0.01) D/months at 24 months postoperatively. Contrast sensitivity and glare sensitivity, intraocular pressure, tear break-up time, endothelial cell count, central corneal thickness, stereopsis function and best corrected visual acuity were not significantly changed. CONCLUSIONS:For treatment of presbyopia, CK appeared to be safe, effective, refractive-predictable and controllable, and relatively stable at 24 months post-operatively. More long-time follow-up is necessary for further evaluation.

Asynchronous Dichoptic Visual Perceptual Training for Patients with Primary Open-Angle Glaucoma: A Pilot Study

Abstract:Abstract Purpose We aimed to observe changes in the visual function of patients with primary open-angle glaucoma (POAG) after undergoing binocular asynchronous visual training. Methods Seven patients with POAG with binocular visual field defects underwent binocular asynchronous dichoptic virtual reality (VR)-based visual perceptual training for 20 days (45 min/session/day, 5 sessions/week, for 4 weeks). Perimetry, contrast sensitivity (CS), and vision-related quality of life assessments were performed for all patients. Results Six months after completing training, nine of 14 eyes showed better performance in the perimetry (the mean deviation [MD] of perimetry was improved as compared to baseline), including six severe eyes and three mild POAG eyes. Moreover, the MD values of four of the nine eyes showed significant improvement (more than 1-dB increase as compared to baseline), including three severe eyes and one mild POAG eye. However, the MD values did not differ significantly between baseline and post-training. Contrast sensitivity tests, performed at three spatial frequencies (3, 6, and 18 cycles/degree) were significantly enhanced after asynchronous dichoptic training ( p = 0.021, 0.026, and 0.020, respectively). Conclusion Patients with POAG, particularly those with severe POAG, performed significantly better in perimetry after training. All patients showed significantly improved performance on the CS tests. Improvements in visual function were sustained for at least 6 months. These results suggest that visual rehabilitation in patients with POAG can be achieved through asynchronous VR-based dichoptic visual perceptual training. A larger randomized clinical trial is required to confirm these effects. Trial registration: The trial registration number is #ChiCTR2100054625.
Long-term application of computer-based pleoptics in home therapy: selected results of a prospective multicenter study

Abstract:Introduction: The paper presents selected results of a prospective multicenter study. The reported study was aimed at the evaluation of a software-based stimulation method of computer training applied in addition to occlusion as a complementary treatment for therapy-resistant cases of amblyopia. The stimulus was a drifting sinusoidal grating of a spatial frequency of 0.3 cyc/deg and a temporal frequency of 1 cyc/sec, reciprocally coordinated with each other to a drift of 0.33 deg/sec. This pattern was implemented as a background stimulus into simple computer games to bind attention by sensory-motor coordination tasks. According to an earlier proposed hypothesis, the stimulation aims at the provocation of stimulus-induced phase-coupling in order to contribute to the refreshment of synchronization and coordination processes in the visual transmission channels. Materials and methods: To assess the outcome of the therapy, we studied the development of the visual acuity during a period of 6 months. Our cooperating partners of this prospective multicenter study were strabologic departments in ophthalmic clinics and private practices as well. For the issue of therapy control, a partial sample of 55 patients from an overall sample of 198 patients was selected, according to the criterion of strong therapy resistance. Results: The visual acuity was increased about two logarithmic steps by an occlusion combined with computer training in addition to the earlier obtained gain of the same amount by occlusion alone. Recalculated relatively to the duration of the therapy periods, the computer training combined with occlusion was found to be about twice as effective as the preceding occlusion alone. Discussion: The results of combined computer training and occlusion show an additional increase of the same amount as the preceding occlusion alone, which yielded at its end no further advantage to the development of visual acuity in the selected sample of our 55 therapy-resistant patients. In a concluding theoretical note, a preliminary hypothesis about the neuronal mechanisms of the stimulus-induced treatment effect is discussed.

However, there have been few studies made to establish this assumption as fact. Radioactive iodine has now been widely used for about ten years in the study of thyroid physiology and iodine metabolism. In order to interpret correctly the results of these studies, knowledge of any possible radiation effects of radioactive iodine tracer doses is necessary. Abstract:Advancements in diagnostic methods and surgical techniques for keratoconus (KC) have increased non-invasive treatment options. Successful surgical planning for KC involves a combination of clinical science, empirical evidence, and surgical expertise. Assessment of disease progression is crucial, and halting the progression should be the focus if it is progressive. While surgeons used to rely on experience alone to decide the surgical method, comparing the network of primary factors, such as visual acuity, across studies can help them choose the most appropriate treatments for each patient and achieve optimal outcomes. Meticulous tabulation methods facilitate interpretation, highlighting the importance of selecting the correct surgical and rehabilitation approach based on each patient's condition and stage of the disease. We detail the outcomes of a comprehensive network meta-analysis comparing the effectiveness of various combined therapeutic refractive treatments for KC at identical stages of the disease, spanning four distinct follow-up intervals. Additionally, the comprehensive analysis suggests that for corneas with optimal best corrected visual acuity (BCVA) preoperatively (classified as regular), combining phakic intraocular lenses with intracorneal ring segments (ICRS) and corneal cross-linking (CXL) could offer the best therapeutic approach provided the disease stage does not exceed stage 3. For irregular corneas, although initial follow-ups show a significant difference in BCVA with surface ablation, longer-term follow-ups recommend combining surface ablation with ICRS and CXL, especially at higher stages.

Abstract:AIM: To evaluate the visual outcomes of standard amblyopic treatment add-on training via perceptual learning in refractive amblyopic children and to identify the risk factors for treatment failure. METHODS: Retrospective charts were reviewed in children with refractive amblyopia who received standard treatment and add-on Cambridge Visual Stimulator (CAM) training. The add-on CAM group that was enrolled had worn full-corrected glasses for at least 2mo before training. A control group received only the standard treatment. Treatment success was defined as best-corrected visual acuity (BCVA) ≥20/25. The age, sex, initial BCVA, refractive errors, sessions and duration of training, and final BCVA were recorded. RESULTS: A total of 209 children (129 children in add-on CAM group and 80 children in control group) were enrolled. Seventy-six percent of unilateral and 87% of bilateral amblyopic children achieved treatment success. In children with unilateral or bilateral moderate amblyopia, the duration to reach BCVA ≥20/25 was significantly shorter in add-on CAM group than in control group. Poor initial BCVA (P<0.001) and high astigmatism (P=0.007) were risk factors for treatment failure after add-on CAM training. Age, sex, and types of refractive error were not associated with treatment success. CONCLUSION: Add-on CAM training is an effective strategy for visual improvement and can shorten the treatment course when the effect of standard treatment is limited in amblyopic children.

Abstract:Purpose To investigate the potential improvement in visual acuity and subjective perception of image quality in patients with keratoconus using non‐orthogonal correction (NOC) cylinder trial lenses where the steep and flat power meridians are set at angles less or greater than 90°. Methods A set of NOC plano/cylindrical trial lenses, where the axes between the power meridians were set at a range of non‐orthogonal angles, were used to refract 18 participants with keratoconus in whom 23 eyes were used for testing. Corneal elevation data were processed by bespoke MATLAB code from Pentacam Scheimpflug tomographer scans. Each participant first underwent subjective refraction using standard orthogonal cylinder trial lenses, and the monocular best‐corrected visual acuity (BCVA, logMAR) was recorded for each eye. They then underwent a second subjective refraction using NOC cylinder trial lenses created for the study and completed a questionnaire to elicit their subjective appraisal of letter clarity and ghosting. Results Fourteen (61%) eyes demonstrated an increase in objective BCVA with the NOC versus the orthogonal correction; seven (30%) eyes showed no change and in two (9%) eyes, the BCVA was slightly worse. Further, 87% and 79% experienced an increase in letter clarity and a reduction in ghosting, respectively, independent of changes in BCVA. The majority of non‐orthogonal angles were in the range of 80°–85°, and it was possible to refine the cylinder and axis of the NOC further compared with the orthogonal correction. All but one of the participants said they would be interested in trying non‐orthogonal spectacles if the opportunity arose. Conclusions Correcting irregular astigmatism in keratoconic individuals with non‐orthogonal spectacle correction may provide benefit in terms of increased visual acuity, improvements in letter clarity and a reduction of ghosting effects. This type of correction has the potential to improve the overall quality of life for patients with keratoconus.

Abstract:Purpose: To investigate the safety and efficacy of topography-guided transepithelial accelerated corneal collagen cross-linking for low refractive error correction in patients with keratoconus. Methods: This was a prospective self-controlled study. Eighteen patients (18 eyes) were enrolled and assessed at 6 visits (pre-operation, 1 w, 1 month, 3 months, 6 months, and 1 year postoperatively). The examination at every visit included analysis of uncorrected visual acuity (UCVA), best-corrected visual acuity (BCVA), corneal topography, and corneal endothelial cell counts. Data are expressed as mean ± standard deviation (SD). The p-value was determined using repeated-measures analysis of variance. Results: No complications occurred in any eye during 1 year follow-up period. At each visit after the operation, the corneal K values and spherical equivalent (SE) were reduced, while the visual acuity values were increased compared with those preoperatively, although these results were not statistically significant (p > 0.05). UCVA of nearly 1/3 of the patients was enhanced by at least 3 lines at each follow-up visit. During the whole follow-up, corneal endothelial cell counts were stable (p > 0.05). Regarding topography, part of the corneal cone was flattened after the operation. Conclusion: Topography-guided transepithelial-accelerated corneal collagen cross-linking is safe and may correct low refractive error in keratoconus treatment. Further studies and improvements are required in this regard.
Internet-based perceptual learning in treating amblyopia

Abstract:Introduction: Amblyopia is a common childhood condition, which affects 2%-3% of the population. The efficacy of conventional treatment in amblyopia seems not to be high and recently perceptual learning has been used for treating amblyopia. The aim of this study was to address the efficacy of Internet-based perceptual learning in treating amblyopia. Methods: A total of 530 eyes of 341 patients with amblyopia presenting to the outpatient department of West China Hospital of Sichuan University between February 2011 and December 2011 were reviewed. A retrospective cohort study was conducted to compare the efficacy of Internet-based perceptual learning and conventional treatment in amblyopia. The efficacy was evaluated by the change in visual acuity between pretreatment and posttreatment. Results: The change in visual acuity between pretreatment and posttreatment by Internet-based perceptual learning was larger than that by conventional treatment in ametropic and strabismic amblyopia (p<0.05), but smaller than that in anisometropic and other types of amblyopia (p<0.05). The improvement in visual acuity by Internet-based perceptual learning was larger for patients with amblyopia not younger than 7 years (p<0.05). The mean cure time of patients with amblyopia by Internet-based perceptual learning was 3.06 ± 1.42 months, while conventional treatment required 3.52 ± 1.67 months to reach the same improvement (p<0.05). Conclusions: Internet-based perceptual learning can be considered as an alternative to conventional treatment. It is especially suitable for ametropic and strabismic patients with amblyopia who are older than 7 years and can shorten the cure time of amblyopia.
Perceptual Learning Improves Adult Amblyopic Vision Through Rule-Based Cognitive Compensation

Abstract:PURPOSE. We investigated whether perceptual learning in adults with amblyopia could be enabled to transfer completely to an orthogonal orientation, which would suggest that amblyopic perceptual learning results mainly from high-level cognitive compensation, rather than plasticity in the amblyopic early visual brain.METHODS. Nineteen adults (mean age = 22.5 years) with anisometropic and/or strabismic amblyopia were trained following a training-plus-exposure (TPE) protocol. The amblyopic eyes practiced contrast, orientation, or Vernier discrimination at one orientation for six to eight sessions. Then the amblyopic or nonamblyopic eyes were exposed to an orthogonal orientation via practicing an irrelevant task. Training was first performed at a lower spatial frequency (SF), then at a higher SF near the cutoff frequency of the amblyopic eye.RESULTS. Perceptual learning was initially orientation specific. However, after exposure to the orthogonal orientation, learning transferred to an orthogonal orientation completely. Reversing the exposure and training order failed to produce transfer. Initial lower SF training led to broad improvement of contrast sensitivity, and later higher SF training led to more specific improvement at high SFs. Training improved visual acuity by 1.5 to 1.6 lines (P < 0.001) in the amblyopic eyes with computerized tests and a clinical E acuity chart. It also improved stereoacuity by 53% (P < 0.001).CONCLUSIONS. The complete transfer of learning suggests that perceptual learning in amblyopia may reflect high-level learning of rules for performing a visual discrimination task. These rules are applicable to new orientations to enable learning transfer. Therefore, perceptual learning may improve amblyopic vision mainly through rule-based cognitive compensation.
The Assistance of Electronic Visual Aids with Perceptual Learning for the Improvement in Visual Acuity in Visually Impaired Children

Abstract:To investigate the effect of electronic visual aids (EVA) combined with perceptual learning (PL) for the improvement in visual acuity for moderate to severe visually impaired and blind children. Twenty-eight 6- to 14-year-old visually impaired children (19 boys and 9 girls) were divided into two groups: PL under the assistance of EVA (Group A, 14 children) and simple PL without EVA (Group B, 14 children). The content of PL was to search the inversed “E” in the crowding strings and connected with lines, 30 minutes a day for 6 months. EVA can provide 5–10 times magnification on the 4.3-inch screen. Uncorrected distance visual acuity (UCVA), best-corrected visual acuity (BCVA), near visual acuity (NVA) and refractive error were evaluated before and 3 and 6 months after training. Baseline UCVA, BCVA or NVA was comparable between the two groups. Three months after training, UCVA, BCVA and NVA improved significantly in Group A (p < 0.05). At the end of training, UCVA and BCVA continued progressing in Group A compared to 3 months (p = 0.01, 0.02), but visual acuity did not show significant improvement in Group B during the same time, except that UCVA improved at the first follow-up. Refractive error had no significant change post-training in both groups. Visually impaired children can benefit more from the combination of PL with EVA than simple PL, and the improvement in visual acuity accompanied no significant myopic shift. It may provide a new method of treatment and rehabilitation in visually impaired children.
Predicting Short-Term Subjective Vision Performance of Contact Lenses Used in Myopia Control

Abstract:Objective: To investigate whether initial assessment of contact lenses prescribed for myopia control (MC) predicts short-term visual performance. Method: Retrospective analysis of 43 participants in a double-masked, randomized, cross-over trial wearing at least one lens: single-vision (SV) lens (1-DAY ACUVUE MOIST) or MC lenses (MiSight or Proclear Multifocal-Distance +2.00D). Participants completed questionnaires at the fitting visit, a take-home questionnaire (THQ) 3 days after fitting and finally at the assessment visit (≥5 days after fitting). Questions comprised vision clarity and lack of ghosting (distance, intermediate, near at day/night time); vision stability; driving vision; overall vision satisfaction and comfort (1–10 scale, 1-point steps); and willingness to purchase based on vision and MC benefit of lens (yes/no response). Visual acuity was measured at fitting and assessment visits. Results: Vision clarity (intermediate and near) was significantly worse at assessment compared with fitting while wearing MC lenses (P<0.001), as was overall vision satisfaction (P<0.001), comfort (P<0.001), and vision stability (P=0.001) while wearing either SV or MC lenses. Participants willing to purchase at assessment visit was 84% with SV and 36% with MC lenses, increasing to 88% (SV, P=1.00) and 61% (MC, P<0.001) if the lenses slowed myopia progression. Visual acuity was no different with either MC or SV lenses at fitting or assessment (P≥0.251). Conclusion: Initial performance at fitting did not predict short-term performance for SV or MC lenses. A significant increase in willingness to purchase if lenses slowed myopia progression was observed while wearing MC lenses. Educating patients on the benefits might increase acceptability of MC lenses.

A New Perceptual Training Strategy to Improve Vision Impaired by Central Vision Loss.

Abstract:Patients with central vision loss depend on peripheral vision for everyday functions. A preferred retinal locus (PRL) on the intact retina is commonly trained as a new "fovea" to help. However, reprogramming the fovea-centered oculomotor control is difficult, so saccades often bring the defunct fovea to block the target. Aligning PRL with distant targets also requires multiple saccades and sometimes head movements. To overcome these problems, we attempted to train normal-sighted observers to form a preferred retinal annulus (PRA) around a simulated scotoma, so that they could rely on the same fovea-centered oculomotor system and make short saccades to align PRA with the target. Observers with an invisible simulated central scotoma (5° radius) practiced making saccades to see a tumbling-E target at 10° eccentricity. The otherwise blurred E target became clear when saccades brought a scotoma-abutting clear window (2° radius) to it. The location of the clear window was either fixed for PRL training, or changing among 12 locations for PRA training. Various cues aided the saccades through training. Practice quickly established a PRL or PRA. Comparing to PRL-trained observers whose first saccade persistently blocked the target with scotoma, PRA-trained observers produced more accurate first saccade. The benefits of more accurate PRA-based saccades also outweighed the costs of slower latency. PRA training may provide an efficient strategy to cope with central vision loss, especially for aging patients who have major difficulties adapting to a PRL.
Customized Visual Discrimination Digital Therapy According to Visual Field Defects in Chronic Stroke Patients

Abstract:Background and purpose: Visual perceptual learning (VPL) may improve visual field defects (VFDs) after chronic stroke, but the optimal training duration and location remain unknown. This prospective study aimed to determine the efficacy of 8 weeks of VFD-customized visual discrimination training in improving poststroke VFDs. Methods: Prospectively enrolled patients with poststroke VFDs initially received no training for 8 weeks (no-training phase). They subsequently underwent our customized VPL program that included orientation-discrimination tasks in individualized blind fields and central letter-discrimination tasks three times per week for 8 weeks (training phase). We analyzed the luminance detection sensitivity and deviation as measured using Humphrey visual field tests before and after the no-training and training phases. The vision-related quality of life was assessed at baseline and at a 16-week follow-up using the National Eye Institute Visual Function Questionnaire-25 (NEI-VFQ-25). Results: Changes in mean total deviation (MTD) scores were greater during the training phase than during the no-training phase (defective hemifield, p=0.002; whole field, p=0.004). The MTD scores improved during the training phase (defective hemifield, p=0.004; whole field, p=0.016), but not during the no-training phase (defective hemifield, p=0.178; whole field, p=0.178). The difference between the improved and worsened areas (≥6 dB changes in luminance detection sensitivity) was greater during the training phase than during the no-training phase (p=0.009). The vision-specific social functioning subscore of the NEI-VFQ-25 improved after the 16-week study period (p=0.040). Conclusions: Our 8-week VFD-customized visual discrimination training protocol may effectively improve VFDs and vision-specific social functioning in chronic stroke patients.
