Abstract:Objective To study the outcome of the total hip arthroplasty ( THA) for the traumatic arthritis after acetabular fracture .Methods 32 hips in 32 patients who had the traumatic arthritis after acetabular fracture were followed up , including 20 males and 12 females, mean age 45.3 years old.All the patients recevied THA .All the implants were bio-prostheses .Harris score and UCLA score were used to evaluate the hip function before and after the surgery; the standard anteroposterior X-ray was adopted for radiographic evaluation of the hip prosthesis .Results A follow-up of 26 months on average was carried out.The Harris score improved from 42.5 points to 93.6 points postoperatively , and UCLA score improved from 3.2 points to 8.1 points postoperatively .The changes had statistically significant differences .The radiographic evaluation showed that the prosthesis was in good position , without loosening, dislocation, infection or periprosthetic osteolysis .Conclusions Total hip replacement is an effective treatment for the traumatic arthritis after acetabular fractures;the biological prosthesis should be used as much as possible . It may provide a satisfactory short-term result.
Total Hip Arthroplasty for Posttraumatic Osteoarthritis Secondary to Acetabular Fracture: an Evidence Based on 1,284 Patients from 1970 to 2018

Abstract:Posttraumatic osteoarthritis (PTOA) can be a crippling sequela of acetabular fracture (AF), and total hip arthroplasty (THA) is often necessary to alleviate the clinical progression of symptoms. The purpose of this study was to summarize the existing clinical evidence concerning the surgical management of AF with THA through meta-analyses.Databases were searched for articles published between 1995 and January 2022 that contained the keywords "acetabular," "fracture," "arthroplasty," and "osteoarthritis." Our study was registered in PROSPERO under number CRD42022314997.We screened 3,125 studies and included data from 31 studies with 1,284 patients. The median patient age at the time of THA was 52 years and ranged from 19 to 94 years. The pooled overall survival rate was 88% [86%-90%, 95% confidence interval (CI)] and could reach 83% at ≥15-year follow-up. For the Harris Hip Score, we pooled 22 studies with an overall mean difference of 43.25 (40.40-46.10, 95% CI; P < 0.001), indicating a large clinical effect. The pooled complications (incidence rates) across studies were: heterotopic ossification (22.53%), implant dislocation (4.66%), implant infection (3.44%), and iatrogenic nerve injury (1.07%).THA in patients with PTOA following AF leads to significant improvement in symptoms and function at ≥15-year follow-up. Survival rates of implants free from re-operation or revision after THA decreased with follow-up time and could still reach 83% at ≥15-year follow-up. THA might be an effective therapeutic method for patients with PTOA due to AF.

Abstract:Acetabular fractures can lead to persistent pain and disability. Managing these complex injuries involves several treatment options such as nonoperative methods, percutaneous fixation, open reduction internal fixation (ORIF), and total hip arthroplasty (THA), and various combinations of these techniques. Around 20 to 40% of patients treated with ORIF may develop post-traumatic arthritis requiring THA. To pinpoint the factors contributing to initial treatment failure of acetabular fractures and the subsequent need for THA conversion, we are conducting a systematic review of relevant literature.

Post-traumatic Total Hip Arthroplasty After Acetabular Fractures: Benefits of the Hardinge Approach

Abstract:Post-traumatic arthritis is a common sequelae after undergoing open reduction and internal fixation (ORIF) of acetabular fractures. This often necessitates conversion to total hip arthroplasty (THA) to help alleviate pain and improve function for these patients. Unfortunately, dislocation rates for post-traumatic THA have been alarmingly high especially when the posterior approach has been used. In the setting of prior soft tissue disruption, the theoretical risk of dislocation is even greater. Conversely, the lateral or the abductor-split approach (Hardinge) is associated with decreased dislocation rates. In this retrospective case series, we evaluated the dislocation rate of the Hardinge approach on patients who underwent THA after developing post-traumatic arthritis after acetabulum ORIF. All patients who matched CPT code 27132 (Repair, Revision, and/or Reconstruction Procedures on the Pelvis and Hip Joint), from January 2009 to December 2019, and treated by the senior author, were pulled from the electronic medical record at the University of Pittsburgh Medical Center. Thirty-one of the resultant 110 were treated with THA for post-traumatic arthrosis through a lateral, abductor-splitting Hardinge approach and met the inclusion criteria for further study. Our case series involves 31 patients who underwent post-traumatic THA through a Hardinge approach: the mean age at the time of index acetabular ORIF is 48.5 years, the mean age at the time of THA is 53.5 years, and the mean interval between ORIF and ultimate THA was five years. The mean length of follow-up after THA was 22.4 months. Overall, patients did well with an all-cause revision rate of 9.7%, with no revision performed for loosening of either the acetabular or femoral component. One patient developed an infection. No patient in our group sustained a dislocation, and all implants were stable without evidence of radiographic loosening at the final follow-up. This study found satisfactory results with patients undergoing THA via lateral or abductor split approach (Hardinge) for post-traumatic arthritis after acetabular ORIF. The use of a Hardinge approach for post-traumatic reconstruction of the hip may be protective against dislocation without increasing baseline risks in this difficult patient population.
Management of Intraoperative Acetabular Fractures in Total Hip Arthroplasty: A Current Concept Review

Abstract:Intraoperative acetabular fractures (IAFs), a complication seldomly encountered in total hip arthroplasty, are typically a result of the impact of insertion of a cementless press-fit cup. Factors that contribute to the risk of these types of fractures include poor bone quality, highly sclerotic bone, and the use of a press-fit cup that is excessively large. The approach to management of these fractures is dependent on when they are identified. Immediate stabilization measures should be implemented for management of fractures detected during surgery. When fractures are detected postoperatively, the decision regarding conservative treatment is dependent on the stability of the implant and the specific fracture pattern. In the majority of cases, effective treatment of an acetabular fracture detected intraoperatively can be administered using a multi-hole revision cup along with anchoring screws in the various regions of the acetabulum. Selection of plate osteosynthesis of the posterior column is recommended when there is a large posterior wall fragment or pelvic discontinuity. In cases where anatomical dimensions allow, cup-cage reconstruction may offer a promising alternative to a combined hip procedure. The number of reports addressing the management of IAFs is limited. This review focuses on outlining the strategies that are currently available for management of this seldomly encountered complication.

Abstract:Acetabular fracture is a complex intra-articular injury. With the increasing incidence in recent years, the complications after internal fixation of acetabular fracture are increasing year by year, such as traumatic arthritis of the hip, avascular necrosis of the femoral head, redisplacement of fracture and heterotopic ossification, etc., resulting in joint pain, limited activity, the quality of life of patients is seriously affected. Nowadays, many studies have focused on total hip arthroplasty (THA) in the treatment of postoperative complications of acetabular fractures, and its curative effect is fully affirmed. By studying the efficacy of THA in the treatment of postoperative complications of acetabular fracture, we can have a more comprehensive understanding of the advantages and disadvantages of THA in the diseases, and provide a new perspective for the treatment of the diseases.This paper reviews the efficacy of THA in the treatment of postoperative complications of acetabular fracture in recent years, providing help for follow-up clinical work and research.

Abstract:Logistic regression was adopted to analyze the risk factors of traumatic arthritis after total hip arthroplasty (THA) in the treatment of acetabular fractures, and the clinical treatment was enhanced.A total of 200 patients with acetabular fractures treated in our hospital from February 2019 to April 2021 were enrolled for THA. According to the occurrence of traumatic arthritis after operation, the patients were divided into control group and study group. The control group was patients without traumatic arthritis (n = 165), and the study group was patients with traumatic arthritis after operation (n = 35). The general data were analyzed, the risk factors of traumatic arthritis after THA for acetabular fractures were analyzed by logistic regression, and the clinical treatment methods were promoted.First of all, there exhibited no significant difference in the general data such as sex, age, medical history, and operation site (P > 0.05). There were significant differences in the general data of whether the patients had a job, rehabilitation exercise, and osteoarthritis before operation (P < 0.05). Secondly, we compared the curative effects between two groups. The effective rate in the study group was higher compared to the control group (P < 0.05). After treatment, the Harris hip function score of the study group at discharge, 1 month, 3 months, and 6 months after discharge was higher compared to that of the control group (P < 0.05). The incidence of postoperative complications in the study group was significantly higher compared to that in the control group (P < 0.05). The presence of osteoarthritis, curative effect, poor reduction of fracture, injury of articular cartilage, entry of internal fixation into the joint, avascular necrosis of the femoral head, and infection before operation were significantly correlated with traumatic arthritis after THA in the treatment of acetabular fractures (P < 0.05). Logistic regression analysis indicated that poor reduction, curative effect, articular cartilage injury, entry of internal fixation into the joint, avascular necrosis of the femoral head, infection, and preoperative osteoarthritis were the risk factors of traumatic arthritis after THA in the treatment of acetabular fractures (P < 0.05).Poor fracture reduction, curative effect, articular cartilage injury, internal fixation into the joint, avascular necrosis of the femoral head, infection, and the presence of osteoarthritis before operation are the risk factors of traumatic arthritis after THA in the treatment of acetabular fractures. When performing THA for patients with acetabular fracture, attention should be paid to the presence of osteoarthritis before operation, and for those with poor curative effect, attention should be paid to the occurrence of poor fracture reduction, articular cartilage injury, internal fixation into the joint, avascular necrosis of the femoral head, and combined infection, and timely intervention measures should be taken to reduce the risk of traumatic arthritis after operation.

From these cases, we should consider open reduction with internal fixation as the first treatment option in cases where spot welding of the cup to the host bone is observed.

Abstract:Post-traumatic arthritis (PTA) of the knee is a complex problem, requiring foresight and careful preoperative planning. Each case of PTA is unique, requiring understanding of the altered anatomy, management of any retained hardware, and also the ability to provide the appropriate stability for each affected knee. Successful treatment of PTA with total knee arthroplasty (TKA) requires the creation of stable and well-balanced joint. TKA for PTA provides comparable improvements in pain and function when compared with primary osteoarthritis, however, these patients are at increased risk of post-operative complications, including instability and infection.

Abstract:Aims Acetabular fractures are associated with long-term morbidity. Our prospective cohort study sought to understand the recovery trajectory of this injury over five years. Methods Eligible patients at a level I trauma centre were recruited into a longitudinal registry of surgical acetabular fractures between June 2004 and August 2019. Patient-reported outcome measures (PROMs), including the 36-Item Short Form Health Survey (SF-36) physical component summary (PCS), were recorded at baseline pre-injury recall and six months, one year, two years, and five years postoperatively. Comparative analyses were performed for elementary and associated fracture patterns. The proportion of patients achieving minimal clinically important difference (MCID) was determined. The rate of, and time to, conversion to total hip arthroplasty (THA) was also established. Results We recruited 251 patients (253 fractures), with a 4:1 male to female ratio and mean age of 46.1 years (SD 16.4). Associated fracture patterns accounted for 56.5% of fractures (n = 143). Trajectory analysis showed all timepoints had significant disability versus baseline, including final follow-up (p < 0.001). Elementary fractures had higher SF-36 PCS at six months (p = 0.023) and one year (p = 0.007) compared to associated fractures, but not at two years (p = 0.135) or five years (p = 0.631). The MCID in SF-36 PCS was observed in 37.3% of patients (69/185) between six months and one year, 26.9% of patients (39/145) between one and two years, and 23.3% of patients (20/86) between two and five years, highlighting the long recovery potential of these injuries. A significant proportion of patients failed to attain the MCID after five years (38.1%; 40/105). Conversion to THA occurred in 13.1% of patients (11/110 elementary and 22/143 associated fractures). Approximately two-thirds of THAs (21/33 patients; 63.6%) were performed within two years of index surgery. Conclusion Acetabular fractures significantly impact physical function. Recovery trajectory is often elongated beyond one year, with two-thirds of our patients displaying persistent clinically relevant long-term disability. Total Knee Replacement for Posttraumatic Degenerative Arthritis of the Knee.

Abstract:OBJECTIVE:To evaluate the results of total knee arthroplasty (TKA) in patients with posttraumatic degenerative arthritis due to a previous fracture around the knee.METHODS:We analyzed the results of 15 TKAs, performed from 1997 to 2003, in 15 patients with post-traumatic degenerative arthritis due to a previous fracture around knee. There were 3 women and 12 men with an average age of 58 years (range, 31-76 years). The time from fracture to arthroplasty averaged 8.2 years (range, 2-27 years). Internal fixation had previously been performed in 8 patients resulting in retained hardware. At the time of arthroplasty a femoral fracture malunion was present in two knees. Lateral retinacular release (4 knees), extensor mechanism realignment (1 knee) or medial collateral ligament reconstruction (1 knee) were needed at the time of arthroplasty.RESULTS:Follow-up averaged 35 months (range, 12-73 months). No patient was lost for follow-up. According to the Knee Society Score scale, the mean preoperative knee score was 37 (range, 10-70) and functional score was 41 (range, 0-60). They were improved significantly to a mean of 84 (range, 10-100) and 76 (range, 20-100) points, respectively at the latest follow-up. The mean knee arc of motion were improved from 84 degree preoperation to 94 degree at the latest follow-up. Postoperative manipulation under anesthesia for poor motion was carried out in 4 knees. No knee had aseptic loosening that required subsequent revision. Two knees developed superficial infection and were treated with debridement. It subsequently recovered with the retention of components.CONCLUSIONS:Significant improvement in function and relief of pain has been achieved in patients with previous fractures undergoing subsequent TKA. However, this procedure is technically demanding and patients are at increased risk for restricted motion and need more care following TKA. This study suggests that the outcome of TKA may be improved further by making special efforts to restore limb alignment, to ensure correct component positioning, and to manage soft tissue balance.
Clinical observation on treating traumatic arthritis after acetabulum fracture operation with total hip arthroplasty

Abstract:Objective: To investigate the clinical efficacy of total hip replacement on treating postoperative traumatic arthritis of acetabulum fracture. Methods: From March 2010 to June 2012 in our hospital, 11 cases of traumatic arthritis of the hip, 4 males and 7 females. Age 35-62 years old, an average of 48.5 years. Injured in 4 cases, 4 cases fall, collision injury in 3 cases. 11 cases were unilateral fractures, the right side of the 6 cases, 5 cases on the left side, are all closed fractures. In accordance with the Judet-Letoumel1 fracture classification sub-posterior wall fracture in 3 cases, posterior column fracture in 3 cases, before and after the column fracture l cases, posterior column and posterior wall fractures in 2 cases, anterior fractures in 2 cases.1 patient with femoral neck fracture, 2 patients with femoral head center dislocation. 11 patients underwent THA treatment. Preoperative Harris hip score 25-67 points, an average of 49.6 points.Results: There was no cases of infection, renovated, deep venous thrombosis, the artificial joint dislocation, wound healing Ⅰ. The acetabular defect bone graft prolonging the absence of weight-bearing ambulation time. After 6 months, 1 year and 2 years of regular follow-up, after 6 months can take care of themselves. 4 months after surgery, X-ray examination showed hip function in patients achieve significant improvement. After 2 years, with reference to the Harris score, an average of 94.7 points. Conclusion: Total hip arthroplasty was an effective treatment to rebuild a normal painless hip joint. Because of the lack of patients, the prostecdtive efficacy still needed further observation.

Abstract:Objective: To investigate the clinical effect of total hip replacement (THA) in the treatment of traumatic arthritis secondary to acetabular fracture. Methods: From October 2019 to June 2022, 15 patients with secondary traumatic arthritis of acetabulum fracture were treated with THA. There were 8 males and 7 females, aged from 40 to 76 years old with an average of (59.20±9.46) years old. Prosthesis loosening, dislocation of hip joint, range of motion of hip joint, nerve injury and other conditions were recorded before and after surgery. Harris score, visual analogue scale (VAS) and imaging were used to evaluate hip joint function and surgical effect. Results: Follow-up time ranged 6 to 39 months with an average of (18.33±9.27) months. All the 15 patients successfully completed the operation, no nerve and blood vessel injury during the operation, postoperative wound healing was stageⅠ, no infection, one case of acetabular side prosthesis loosening at half a year after operation, and recovered well after revision surgery, one case of hip dislocation was cured after open reduction treatment, no adverse consequences. Harris score at the last postoperative follow-up was (88.60±4.01) points, compared with the preoperative (47.20±11.77) points, the difference was statistically significant (P<0.05), and VAS at the lateat postoperative follow-up was 1 (1) points, compared with the preoperative 8 (2) points, the difference was statistically significant (P<0.05). At the last follow-up, the pain symptoms were relieved or disappeared, and the joint function was satisfactory. The imaging data of the latest follow-up showed joint was well pseudoradiated, no abnormal ossification occurred, and the prosthesis was not loose. Conclusion: THA is effective in the treatment of traumatic arthritis secondary to acetabular fracture and can effectively improve the quality of life of patients. Preoperative comprehensive evaluation and bone defect evaluation of patients, and intraoperative management of acetabulum, femur, internal fixation and bone defect are key factors for the success of surgery.
Acute Total Hip Arthroplasty Following Acetabular Fracture is Associated with a High Risk of Revision, Dislocation, and Periprosthetic Fracture

Abstract:Background The aim of this study was to compare outcomes between acute, subacute, and delayed arthroplasty for acetabular fractures occurring within one week, from one week to six months, or longer than six months before the index total hip arthroplasty (THA), versus THA without a history of acetabular fracture as a control. Methods We analyzed the records of patients undergoing primary THA who were enrolled in a national database for at least two years before and after the index procedure. Patients who had an initial diagnostic code for acetabular fracture occurring less than one week, from one week to six months, or at least more than six months before the THA were classified as acute THA (aTHA), subacute (saTHA), or delayed (dTHA), respectively. The control group was patients undergoing THA who did not have a history of acetabular fracture. There were 430,349 control primary THAs, 462 aTHAs, 675 saTHAs, and 1,162 dTHAs. Results After adjusting for age, sex, region, and comorbidities, patients who had an aTHA and saTHA experienced statistically significant increased odds of revision, dislocation, and periprosthetic fracture compared to primary THA without a history of acetabular fracture. Similarly, dTHA was associated with increased odds of revision, dislocation, and periprosthetic fractures compared to primary THA. In the multivariate analysis, aTHA had statistically significant higher rates of dislocation when compared to dTHA. Conclusion Patients who had a history of acetabular fractures undergoing aTHA, saTHA, or dTHA have significantly increased rates of revision, periprosthetic fracture, and dislocation compared to primary THA in those who did not have a history of acetabular fractures.

[Periprosthetic acetabular fractures: classifications, treatment and challenges]

Abstract:Periprosthetic acetabular fractures (PPAF) are fractures of the acetabulum with a hip endoprosthesis in place. They are a rare complication, although they are occurring more frequently due to the increase in patients being fitted with total hip arthroplasty (THA) and the long service life of the implants. At present, only a small amount of scientific literature exists, particularly regarding the surgical treatment of these fracture types.The aim of this paper is to provide an overview of the topic of PPAF with a critical review of the current literature and to present the necessary surgical treatment.An evaluation of the current literature on the topic of PPAF with a focus on the surgical treatment of fractures is carried out.Surgical treatment is technically demanding due to the pelvic ring instability with removal of the bone tension for secure acetabular cup fixation and should lead to the recovery of the biomechanical stability of the pelvis and thus the secure anchoring of the acetabular cup. An interdisciplinary approach requiring both trauma surgery and orthopaedic expertise is certainly recommended, as older and comorbid patients with poor bone quality are particularly frequently affected.The treatment of periprosthetic acetabular fractures, especially in older patients, requires not only the individual expertise of surgeon involved in the operation but also a goal-oriented and consistent interdisciplinary approach by the surgeons involved from the fields of orthopaedics and trauma surgery in view of the patients' frequent comorbidities. By working together, the optimal and individualized operation can be performed for the patient.

Abstract:Purpose of the study: The article presents a retrospective evaluation of a group of 119 patients treated for acetabular fractures between 1996 and 2002. Material: In the monitored period 119 patients, (89 men, 30 women) average age 35 years, were hospitalized with an acetabular fracture. Surgically treated were 91 patients (70 men, 21 women), average age 29 years. Prevailing in this group were Type A fractures which accounted for 45%, Type B was represented by 35% and Type C included 19%. Twenty-eight patients were treated conservatively. Method: Indication for surgical treatment was instability, hip incongruence or combination of both. Conservative treatment was chosen in the fractures without displacement, in fractures with secondary congruency and in case of contraindication of surgical treatment. The indication was always based on CT examination with a three-dimensional reconstruction. The surgery was performed within 24 hours in 42%, within 48 hours in 64%, within 72 hours in 87%. Posterior Kocher-Langenbeck approach was used in 54%, anterior ilioinguinal approach in 22% and the combination of both approaches in 24%. Posterior approach was indicated in A1, A2, B1 fractures, ilioinguinal approach in A3, B3, C1 Types, the combination of both approaches in B2, C2, C3 fractures. Osteosynthetic material was chosen according to the fracture type, bone quality and fragment size. Results: The evaluation was based on subjective complaints, radiograph and clinical examination using Harris Hip Score. In the group of 91 patients treated surgically we achieved excellent result in 41 cases (45%), very good result in 28 cases (31%), fair result in 7 cases (9%) and poor result in 13 cases (15%). We recorded 17 peroperative complications--11 times inadequate reduction, 5 times nerve injury, once vascular injury. Early complications included infect--twice and loosening of implant--once. Late complications occurred in 26 cases--paraarticular ossification in 17 cases, aseptic necrosis of the femoral head in 8 cases. Discussion: The treatment of fractures of the acetabulum as any intraarticular fracture is based on a precise anatomical reduction, stable fixation and early mobilization. Difficulties of the treatment of the acetabulum arise from the anatomical relations. Decisive for the indication, the method of treatment and prognosis is the type of the injury of the weight-bearing area of the acetabulum. In order to understand the type of the fracture it is necessary to make a careful preoperative examination based particularly on CT examination with a three-dimensional reconstruction. The indication and treatment strategy--surgical approach, method of reduction, type of fixation--is selected accordingly. Each surgical approach has its limits and also the possibilities of fixation are limited by the type of the fracture. The outcomes depend directly on the quality of the reconstruction of the articular surface and the incidence of complications, caused either by the injury or preoperatively. Conclusion: The treatment of displaced fractures of the acetabulum is on principle surgical. Satisfactory results are produced only by anatomical reconstruction of the weight-bearing area of the acetabulum. Surgical treatment is highly demanding and the long-term results not always correspond to the efforts made.

Abstract:Acetabular fractures in the geriatric population are typically low-energy fractures resulting from a fall from standing height. Compromised bone quality in the elderly, as well as this population’s concomitant medical comorbidities, render the management of such fractures challenging and controversial. Non-operative management remains the mainstay of treatment, although such a choice is associated with numerous and serious complications related to both the hip joint as well as the general condition of the patient. On the other hand, operatively treating acetabular fractures (e.g. , with osteosynthesis or total hip arthroplasty) is gaining popularity. Osteosynthesis can be performed with open reduction and internal fixation or with minimally invasive techniques. Total hip arthroplasty could be performed either in the acute phase combined with osteosynthesis or as a delayed procedure after a period of non-operative management or after failed osteosynthesis of the acetabulum. Regardless of the implemented treatment, orthogeriatric co-management is considered extremely crucial, and it is currently one of the pillars of a successful outcome after an acetabular fracture.
