Abstract:Periosteum as an important component in the construct of bone is mainly responsible for providing nourishment and regulating osteogenic differentiation. When bone defect happens, the functionality of periosteum will also be influenced, furthermore, it will finally hamper the process of bone regeneration. However, fabrication of an artificial periosteum with the capabilities in accelerating angiogenesis and osteogenesis in the defect area is still a challenge for researchers. In this study, we fabricated an organic-inorganic hybrid biomimetic periosteum by electrospinning, which can induce mineralization in situ and control the ions release for long-term in local area. Further, this system exhibited potential capabilities in promoting in vitro, which means the potentiality in accelerating bone regeneration in vivo. Calcium phosphate nanoparticles (CaPs) were fabricated by emulsion method, then CaPs were further incorporated with gelatin-methacryloyl (GelMA) by electrospinning fibers to construct the hybrid hydrogel fibers. The fibers exhibited satisfactory morphology and mechanical properties, additionally, controlled ions release could be observed for over 10 days. Further, significant mineralization was proved on the surface of hybrid fibers after 7 days and 14 days' co-incubation with simulated body fluid (SBF). Then, favorable biocompatibility of the hybrid fibers was approved by co-cultured with MC3T3-E1 cells. Finally, the hybrid fibers exhibited potential capabilities in promoting angiogenesis and osteogenesis by co-culture with HUVECs and MC3T3-E1 cells. This biomimetic organic-inorganic hybrid hydrogel electrospinning periosteum provided a promising strategy to develop periosteum biomaterials with angiogenesis and osteogenesis capabilities.
Electrospun Photocrosslinkable Hydrogel Fibrous Membrane with Metal Ion Trapping Capability As an Artificial Periosteum to Promote Bone Regeneration

Abstract:The periosteum plays a crucial role in both bone development and the healing process of bone fractures. However, few studies have focused on developing artificial periosteum due to the complexity of its construction and the biological risks for clinical practice. To address this issue, we proposed a strategy for the fabrication of a photocrosslinkable natural hydrogel nanofibrous membrane (SFMA-Lap@AcBP) based on methacrylated silk fibroin (SFMA), photoinitiator (Lap), acrylate bisphosphonate (AcBP) through combining electrospinning technology with hydrogel, mimicking the structure and function of periosteum to promote bone regeneration. AcBP were covalently attached to the SFMA hydrogel nanofibrous membrane via conjugation. With the degradation of SFMA-Lap@AcBP hydrogel nanofibrous membrane, the released BP-grafted derivates could promote bone regeneration. Moreover, SFMA-Lap@AcBP hydrogel membranes were found to be able to trap metal ions such as Mg2+ and Ca2+ in vivo, which might play a synergistic role in inducing complete bone regeneration. Furthermore, the hydrogel membranes had excellent cytocompatibility. They could imitate the microenvironment of the extracellular matrix, thereby offering structural and biochemical cues for the adhesion, proliferation, and differentiation of cells, without the need of exogenous cells or inductive growth factors, resulting in a facilitated osteogenesis. After implanting membranes into a cranial periosteal and bone defect in rats, both the amount and quality of new bone in the SFMA-Lap@AcBP group were found to be higher than those in the other groups. Therefore, the hydrogel membrane bionic periosteum was efficient and versatile in inducing bone regeneration, providing a potential strategy to address clinical issues.

Abstract:An artificial periosteum has emerged as an encouraging candidate for bone defect repair. Currently, it remains a great challenge to develop a multifaceted biomimetic periosteum integrating multifunctional features of bioactivities and unique mechanical properties. Here, we successfully fabricated an artificial periosteum (AP) composed of hierarchically assembled Mg-doped mineralized collagen microfibrils with a biomimetically rotated lamellar structure via a "multiscale cascade regulation" strategy combining multiple techniques such as molecular self-assembly, electrospinning, and pressure-driven fusion from molecular to macroscopic levels. The AP has excellent mechanical properties with an ultimate strength and a tensile modulus of 15.9 MPa and 1.1 GPa, respectively. The involvement of Mg-doped nano-hydroxyapatite endowed the AP with good osteogenic and angiogenic activities to promote osteogenic differentiation of bone marrow mesenchymal stem cells and human umbilical vein endothelial cell differentiation into capillary-like structures in vitro. In addition, the results of in vivo evaluations in a rat cranial bone defect model including micro-CT morphology, histological staining, and immunohistochemical analysis showed that Mg-doped mineralized collagen-based AP (MgMC@AP) significantly facilitated cranial bone regeneration and fast vascularization. Our findings suggest that the AP mimicked the composition, lamellar structure, mechanical properties, and biological activities of natural periosteum/lamellae, showing great promise for bone tissue regeneration.

Abstract:The use of biomacromolecules as templates to control the nucleation and growth of hydroxyapatite crystals to prepare bioactive materials is a valuable approach in bone tissue engineering. Here, an artificial periosteum is prepared by biomineralizing Antheraea pernyi fibroin (AF) membrane with prenucleated nanoclusters, which can promote the osteogenic differentiation of mesenchymal stem cells (MSCs) and induce the formation of bone matrix protein in vivo. To achieve this, a biologically inspired prenucleated calcium and phosphorus nanocluster mineralization system is designed to nucleate and generate hydroxyapatite crystals on the surface of the AF membrane. This biomineralization process provides AF membranes with improved elastic modulus and tensile strength. Subsequently, cell viability assay, hemolysis test, and H&E staining show that the mineralized AF (MAF) membranes has good cytocompatibility, hemocompatibility, and histocompatibility in vitro and in vivo. Additionally, the MAF membranes significantly promote osteogenic differentiation of MSCs in the absence of osteogenic inducer in vitro. Experiments in vivo demonstrate that bone‐related matrix proteins are highly expressed in MAF groups with or without MSCs seeded. Therefore, the use of bioinspired prenucleated nanoclusters to prepare artificial periosteum based on biomineralized AF membrane is a promising strategy in the field of bone tissue engineering.

Abstract:The periosteum plays an important role in vascularization and ossification during bone repair. However, in most studies, an artificial periosteum cannot restore both functions of the periosteum concurrently. In this study, a novel nanofiber that can sustain the release of vascular endothelial growth factor (VEGF) and bone morphogenetic protein-2 (BMP-2) was fabricated to enhance the durability of angiogenesis and osteogenesis during bone regeneration. A cell-free tissue engineered periosteum based on an electrospinning poly-l-lactic acid (PLLA) nanofiber was fabricated, on which VEGF and BMP-2 were immobilized through a polydopamine (PDA) coating conveniently and safely (BVP@PLLA membrane). The results indicated a significantly improved loading rate as well as a slow and sustained release of VEGF and BMP-2 with the help of the PDA coating. BMP-2 immobilized on nanofibers successfully induced the osteogenic differentiation of human bone marrow mesenchymal stem cells (BMSCs) in vitro with high expression of runt-related transcription factor 2 (Runx2), osteopontin (OPN), and alkaline phosphatase (ALP). Similarly, angiogenic differentiation of BMSCs with the expression of fetal liver kinase-1 (Flk-1) and vascular endothelial cadherin (VE-cadherin) was observed under the environment of VEGF sustained release. Moreover, an in vivo study revealed that the BVP@PLLA membrane could enhance vascular formation and new bone formation, which accelerates bone regeneration in rat femoral defects along with a massive periosteum defect. Therefore, our study suggests that the novel artificial periosteum with dual growth factor controlled release is a promising system to improve bone regeneration in bone defects along with a massive periosteum defect.

Abstract:Periosteum plays a pivotal role in vascularization, ossification and remodeling during the healing process of bone injury. However, there are few studies focused on the construction of artificial implants with periosteum-mimetic effect. To emulate the primary role of natural periosteum or endosteal tissues in bone regeneration, here we provide a functional biomimetic membrane with micropatterns of site-specific biomineralization. The micropattern is generated by printed hydroxyapatite nanoparticles (HANPs), combined with selective growth of biomineralized apatite and in situ coprecipitation with growth factors (GFs). The biomimetic membrane can sustainably provide a periosteum-mimetic microenvironment, such as long-term topographical guidance for cell recruitment and induced cell differentiation by releasing calcium phosphate and growth factors. We demonstrated that rat mesenchymal stem cells (rMSCs) on such biomimetic membrane exhibited highly aligned organization, leading to enhanced angiogenesis and osteogenesis. In the rat calvarial defect model, the biomimetic membranes with biomineralized pattern could significantly enhance vascularized ossification and accelerate new bone formation. The current work suggests that the functionally biomimetic membranes with specific biomineralized micropatterns can be a promising alternative to periosteal autografts, with great potential for bench-to-bedside translation in orthopedics.

Abstract:Current periosteal grafts have limitations related to low mechanical strength, tissue adhesiveness, and poor osteogenesis and angiogenesis potential. Here, a periosteum mimicking bone aid (PMBA) with similar structure and function to natural periosteum is developed by electrospinning photocrosslinkable methacrylated gelatin (GelMA), l‐arginine‐based unsaturated poly(ester amide) (Arg‐UPEA), and methacrylated hydroxyapatite nanoparticles (nHAMA). Such combination of materials enhances the material mechanical strength, favors the tissue adhesion, and guarantees the sustained activation of nitric oxide–cyclic guanosine monophosphate (NO–cGMP) signaling pathway, with well‐coordinated osteogenic–angiogenic coupling effect for accelerated bone regeneration. This work presents a proof‐of‐concept demonstration of thoroughly considering the progression of implant biomaterials: that is, the initial material components (i.e., GelMA, Arg‐UPEA, and nHAMA) equip the scaffold with suitable structure and function, while its degradation products (i.e., Ca2+ and l‐arginine) are involved in long‐term mediation of physiological activities. It is envisioned that the strategy will inspire the design of high‐performance bioscaffolds toward bone and periosteum tissue engineering.

Abstract:Periosteum, a membrane covering the surface of the bone, plays an essential role in maintaining the function of bone tissue—and especially in providing nourishment and vascularization during the bone regeneration process. Currently, most artificial periostea have relatively weak mechanical strength and a rapid degradation rate, and they lack integrated angiogenesis and osteogenesis functions. In this study, a bi-layer, biomimetic, artificial periosteum composed of a methacrylated gelatin–nano-hydroxyapatite (GelMA-nHA) cambium layer and a poly (N-acryloyl 2-lycine) (PACG) -GelMA-Mg2+ fibrous layer was fabricated via 3D printing. The GelMA-nHA layer is shown to undertake the function of improving osteogenic differentiation of rat bone marrow mesenchymal stem cells with the sustainable release of Ca2+ from nHA nanoparticles. The hydrogen-bonding-strengthened P(ACG-GelMA-L)-Mg2+ hydrogel layer serves to protect the inner defect site and prolong degradation time (60 days) to match new bone regeneration. Furthermore, the released magnesium ion exhibits a prominent effect in regulating the polarization phenotype of macrophage cells into the M2 phenotype and thus promotes the angiogenesis of the human umbilical vein endothelial cells in vitro. This bi-layer artificial periosteum was implanted into a critical-sized cranial bone defect in rats, and the 12-week post-operative outcomes demonstrate optimal new bone regeneration.Graphic abstract

Abstract:BackgroundPeriosteum is a vascularized tissue membrane covering the bone surface and plays a decisive role in bone reconstruction process after fracture. Various artificial periosteum has been developed to assist the allografts or bionic bone scaffolds in accelerating bone healing. Recently, the biomimicking design of artificial periosteum has attracted increasing attention due to the recapitulation of the natural extracellular microenvironment of the periosteum and has presented unique capacity to modulate the cell fates and ultimately enhance the bone formation and improve neovascularization.MethodsA systematic literature search is performed and relevant findings in biomimicking design of artificial periosteum have been reviewed and cited.ResultsWe give a systematical overview of current development of biomimicking design of artificial periosteum. We first summarize the universal strategies for designing biomimicking artificial periosteum including biochemical biomimicry and biophysical biomimicry aspects. We then discuss three types of novel versatile biomimicking artificial periosteum including physical-chemical combined artificial periosteum, heterogeneous structured biomimicking periosteum, and healing phase-targeting biomimicking periosteum. Finally, we comment on the potential implications and prospects in the future design of biomimicking artificial periosteum.ConclusionThis review summarizes the preparation strategies of biomimicking artificial periosteum in recent years with a discussion of material selection, animal model adoption, biophysical and biochemical cues to regulate the cell fates as well as three types of latest developed versatile biomimicking artificial periosteum. In future, integration of innervation, osteochondral regeneration, and osteoimmunomodulation, should be taken into consideration when fabricating multifunctional artificial periosteum.The Translational Potential of this Article: This study provides a holistic view on the design strategy and the therapeutic potential of biomimicking artificial periosteum to promote bone healing. It is hoped to open a new avenue of artificial periosteum design with biomimicking considerations and reposition of the current strategy for accelerated bone healing.

Nano artificial periosteum PLGA/MgO/Quercetin accelerates repair of bone defects through promoting osteogenic − angiogenic coupling effect via Wnt/ β-catenin pathway

Abstract:Bone nonunion or delayed union, caused by stripping or injuring of periosteum, is the most common sequelae of segmental bone defects. The preservation of periosteum, or the use of periosteal grafts, can significantly improve the integration of bone graft, speeding up the process of bone reconstruction. However, in most cases, periosteum cannot be preserved with bioactivity. Thus, it is pivotal to develop artificial periosteum. In this study, artificial periosteum of PLGA/MgO/Quercetin was prepared by electrospinning. PLGA/MgO/Quercetin membranes were shown to have a highly porous surface and microstructure, as observed by scanning electron microscopy. Along with excellent biocompatibility, PLGA/MgO/Quercetin membranes promoted cell proliferation and migration, as well as osteogenic differentiation of BMSCs (Bone marrow mesenchymal stem cells) in a dose-dependent manner through the activation of Wnt/β-Catenin pathway. The PLGA/MgO/Quercetin membranes, with an appropriate concentration of quercetin (<1 wt%), promoted EPCs (Endothelial progenitor cells) angiogenesis. In a subcutaneous implantation model and rat skull defect model, optimal osteogenesis and angiogenesis function were observed for the PLGA/20 wt% MgO/0.1 wt% Quercetin membrane. In conclusion, PLGA/MgO membranes, with an appropriate concentration of quercetin, show a variety of biological activities and are promising materials for the generation of artificial periosteum.

Abstract:An electrospun biomimetic periosteum with an asymmetric structure is prepared through aligned and coaxial electrospinning for programed promotion of bone regeneration. This biomimetic periosteum exhibits controlled release of multiple agents, enabling regulation of the inflammatory, angiogenic, and osteogenic phases that are essential for bone healing. It demonstrates good efficacy in promoting bone regeneration in a rat skull defect model. The effective repair of large bone defects remains a major challenge due to its limited self‐healing capacity. Inspired by the structure and function of the natural periosteum, an electrospun biomimetic periosteum is constructed to programmatically promote bone regeneration using natural bone healing mechanisms. The biomimetic periosteum is composed of a bilayer with an asymmetric structure in which an aligned electrospun poly(ε‐caprolactone)/gelatin/deferoxamine (PCL/GEL/DFO) layer mimics the outer fibrous layer of the periosteum, while a random coaxial electrospun PCL/GEL/aspirin (ASP) shell and PCL/silicon nanoparticles (SiNPs) core layer mimics the inner cambial layer. The bilayer controls the release of ASP, DFO, and SiNPs to precisely regulate the inflammatory, angiogenic, and osteogenic phases of bone repair. The random coaxial inner layer can effectively antioxidize, promoting cell recruitment, proliferation, differentiation, and mineralization, while the aligned outer layer can promote angiogenesis and prevent fibroblast infiltration. In particular, different stages of bone repair are modulated in a rat skull defect model to achieve faster and better bone regeneration. The proposed biomimetic periosteum is expected to be a promising candidate for bone defect healing.

Abstract:It is of great significance to develop osteoinductive artificial scaffold for bone repair and regeneration. We constructed a biomimetic apatite interface on electmspun polycaprolactone fibers by combining layer-by-layer (LbL) nanocoating with mineralization to fabricate an osteoinductive artificial scaffold. After polydopamine modification, cationic type-1 collagen and anionic chondroitin sulfate were sequentially adsorbed on the fiber surface. The fibers coated with the multilayer components served as the precursor matrix to induce apatite deposition. By adjusting the number of the layers and duration of mineralization, the nanoscale morphology of composite fibers was optimized. When ten bilayers of the collagen and chondmitin sulfate were deposited onto the fibers followed by one day-mineralization, the obtained polycaprolactone-apatite composite scaffolds significantly promoted the adhesion, proliferation, and osteogenic differentiation of MC3T3-E1 cells. In a subcutaneous implantation in mice, this composite fiber membrane enhanced in vivo ectopic osteogenesis. Our nanoarchitectural scaffolds were able to mimic the composition and structure of the bone matrix to a certain extent, holding great potential for bone repair and regeneration.
Nano Twin-Fiber Membrane with Osteogenic and Antibacterial Dual Functions As Artificial Periosteum for Long Bone Repairing.

Abstract:Due to the significant role of the periosteum in bone regeneration, we fabricated a novel twin-fiber membrane with antibacterial and osteo-inductive properties as an artificial periosteum by a double spinneret electrospinning method. The antibacterial drug moxifloxacin (MOX) and Chinese medicine icariin (ICA) were introduced into PCL/gelatin and PCL micro/nano fibers respectively to endow the membrane with antibacterial and osteoinduction properties. The twin-fiber structure membrane not only acted as carriers for two different drugs with drastically different solubility in the same fiber membrane, but also achieved stepwise and controlled drug release profiles based on different degradation rates of the two fibers matrix and diffusion rates of the two drugs. Most importantly, only 35% ICA was released out from this dual drug loaded membrane after 1 month and such slow release rate would match with the bone regeneration rate which can hardly be achieved by other drug delivery system. Besides, this dual drug loaded twin-fiber membrane exhibited clear antibacterial effect and excellent in vitro biocompatibility for cells to adhere to and proliferate on. Furthermore, an obvious up-regulation of osteocalcin (OCN), type I collagen (COLI) expression and calcium deposition of MC3T3-E1 cells demonstrated the improved osteogenic differentiation capability. Also, the dual drug loaded twin-fiber membrane displayed fascinating properties contributing to new bone formation in rabbit radius defect model. These results indicate the huge potential of this dual drug loaded twin-fiber membrane as an effective drug release controller and biomimetic multifunctional artificial periosteum to accelerate bone regeneration.

Abstract: The application of microsurgical techniques to gynecology has improved the results of tubal plastic surgery, with the most significant improvement in the reversal of tubal sterilization. Procedures to correct pathologic obstruction of the fallopian tubes have shown similar but less dramatic improvement. At the University of Virginia Medical Center all gynecologic microsurgery procedures are performed by a member of the Endocrine Infertility Division. Preoperative evaluation of patients requesting reversal of sterilization includes a history and physical examination and review of the previous operative note when available. A preoperative hysterosalpingogram is performed to assess the uterine cavity and proximal fallopian tube. Patients have a 1 stage procedure. Laparoscopy is performed to assess the amount of remaining fallopian tube. If conditions are favorable for anastomosis, a laparotomy is then undertaken. The results of the tubal anastomosis procedures at the University of Virginia have been encouraging. Postoperative tubal patency in this group of patients is virtually 100%, and the pregnancy rate is similar to that of Gomel and Winston (Winston reported a 69% pregnancy rate in 1977). A few patients have conceived on the 1st cycle following resumption of intercourse. There have been very few ectopic pregnancies in the University of Virginia series. If cornual anastomosis is not possible because of complete obstruction of the interstitial fallopian tube, a reimplantation is performed. Distal fallopian tube obstruction requires fimbrioplasty or salpingostomy. Cases necessitating extensive surgery due to pelvic adhesions usually result in raw areas on the uterus, broad ligament, or pelvic peritoneum. In this situation a free peritoneal graft is used. Patients undergoing all types of tuboplasty are treated with intraoperative dexamethasone and penicillin followed by a 3-day postoperative course unless there is a history of drug allergy or other contraindication. Results in patients with distal tubal obstruction have correlated closely with the severity of the disease. Patients with minimal tubal dilatation have had the best prognosis, with pregnancy rates in the range of 35-40%. A hysterosalpingogram is performed 2-3 months after surgery to assess tubal patency. As a shrinking number of patients have a history of previous pelvic surgery or appendectomy, some of the principles of microsurgery have been applied to routine pelvic procedures performed on young women whose fertility is a consideration.

Abstract:The design and development of functional biomimetic systems for programmed stem cell response is a field of topical interest. To mimic bone extracellular matrix, we present an innovative strategy for constructing drug-loaded composite nanofibrous scaffolds in this study, which could integrate multiple cues from calcium phosphate mineral, bioactive molecule, and highly ordered fiber topography for the control of stem cell fate. Briefly, inspired by mussel adhesion mechanism, a polydopamine (pDA)-templated nanohydroxyapatite (tHA) was synthesized and then surface-functionalized with bone morphogenetic protein-7-derived peptides via catechol chemistry. Afterward, the resulting peptide-loaded tHA (tHA/pep) particles were blended with polycaprolactone (PCL) solution to fabricate electrospun hybrid nanofibers with random and aligned orientation. Our research demonstrated that the bioactivity of grafted peptides was retained in composite nanofibers. Compared to controls, PCL-tHA/pep composite nanofibers showed improved cytocompatibility. Moreover, the incorporated tHA/pep particles in nanofibers could further facilitate osteogenic differentiation potential of human mesenchymal stem cells (hMSCs). More importantly, the aligned PCL-tHA/pep composite nanofibers showed more osteogenic activity than did randomly oriented counterparts, even under nonosteoinductive conditions, indicating excellent performance of biomimetic design in cell fate decision. After in vivo implantation, the PCL-tHA/pep composite nanofibers with highly ordered structure could significantly promote the regeneration of lamellar-like bones in a rat calvarial critical-sized defect. Accordingly, the presented strategy in our work could be applied for a wide range of potential applications in not only bone regeneration application but also pharmaceutical science.
Seamlessly Adhesive Bionic Periosteum Patches Via Filling Microcracks for Defective Bone Healing

Abstract:A seamlessly adhesive bionic periosteum patch based on the interfacial self‐assembly reaction was designed for repairing bone fractures through filling microcracks. In vitro, the periosteum patch exhibited outstanding self‐healing properties, seamlessly filling bone microcracks. Furthermore, under a mild photothermal stimulation induced by 808 nm NIR, the periosteum patch revealed that less fibrous tissue, and more new bone and blood vessels. Current artificial designs of the periosteum focus on osteogenic or angiogenic properties, while ignoring the filling and integration with bone microcracks, which trigger a prolonged excessive inflammatory reaction and lead to failure of bone regeneration. In this study, seamless adhesive biomimetic periosteum patches (HABP/Sr‐PLA) were prepared to fill microcracks in defective bone via interfacial self‐assembly induced by Sr ions mediated metal‐ligand interactions among pamidronate disodium‐modified hyaluronic acid (HAPD), black phosphorus (BP), and hydrophilic polylactic acid (PLA). In vitro, HABP/Sr‐PLA exhibited excellent self‐healing properties, seamlessly filled bone microcracks, and significantly enhanced osteogenesis and angiogenesis. Furthermore, in a rat cranial defect model, HABP/Sr‐PLA was demonstrated to significantly promote the formation of blood vessels and new bone under mild 808 nm photothermal stimulation (42.8 °C), and the highest protein expression of CD31 and OPN was five times higher than that of the control group and other groups. Therefore, the proposed seamless microcrack‐filled bionic periosteum patch is a promising clinical strategy for promoting bone repair.

Abstract:Existing artificial periostea faces difficulty in supporting the entire bone repair cycle due to the absence of adhesion-centric design and effective cell manipulation, leading to poor inhibition of soft tissue infiltration and induction of osteogenesis and angiogenesis. Here, a Janus periosteum with interior surface adhesion and exterior anatomical patterns to mimic the structure and function of natural periosteum is presented. Photo-crosslinkable polymers are employed to replicate the exterior anisotropic surface structured microgrooves for cell fate manipulation and assemble gecko-inspired fibrillar setae arrays for interior surface adhesion. To further bolster its underwater adhesiveness, mussel-inspired poly (dopamine methacrylamide-co-hydroxyethyl methacrylate) (PDMH) is coated onto the periosteum surfaces. This periosteum presents adhesiveness with strong shear and normal adhesion in both dry and wet conditions due to the coordinated interactions of micro setae arrays and PDMH coating; it also boasts effective cell modulation for enhanced synchronous osteogenesis and angiogenesis, without the aid of growth factors. Moreover, the Janus periosteum is found to enhance bone regeneration in vivo with increased new bone formation and neovascularization. It is envisioned that this Janus periosteum will be able to streamline bone fracture surgical repair as a rapidly adhesive, low-maintenance yet robust bandage to significantly cut down the healing phase.
