Abstract:Objectives To assess 52-week safety and efficacy of bimekizumab in patients with active psoriatic arthritis (PsA) and prior inadequate response/intolerance to tumour necrosis factor inhibitors. Methods Patients completing the 16-week phase III double-blind, placebo-controlled BE COMPLETE (NCT03896581) study entered the open-label extension, BE VITAL (NCT04009499). All patients in BE VITAL received 160 mg bimekizumab every 4 weeks. Safety and efficacy are reported to week 52. Results A total of 347/400 (86.8%) patients completed week 52. To week 52, the exposure-adjusted incidence rate/100 patient-years for ≥1 treatment-emergent adverse event (TEAE) was 126.0, and was 7.0 for serious TEAEs. The most frequent TEAEs were SARS-CoV-2 (COVID-19), oral candidiasis, nasopharyngitis and urinary tract infection. All fungal infections were mild or moderate in severity and localised; two patients discontinued the study due to oral candidiasis. No cases of active tuberculosis, uveitis or inflammatory bowel disease were reported. One sudden death occurred. Sustained efficacy was observed with bimekizumab from week 16 to ‍52 across clinical and patient-reported outcomes. At week 52, 51.7% bimekizumab-randomised and 40.6% placebo/bimekizumab patients (receiving bimekizumab from week 16 to 52) had ≥50% improvement in the American College of Rheumatology criteria. Complete skin clearance (Psoriasis Area and Severity Index 100) was achieved by 65.9% bimekizumab and 60.2% placebo/bimekizumab patients at week 52. Minimal disease activity was achieved by 47.2% bimekizumab and 33.1% placebo/bimekizumab patients at week 52. Conclusions Bimekizumab demonstrated a safety profile consistent with previous reports; no new safety signals were identified. Sustained efficacy was observed from week 16 to 52.

Abstract:Therapeutic Advances in Musculoskeletal Disease, Volume 16, Issue , January-December 2024. Background:Psoriatic arthritis (PsA) is a chronic inflammatory disease that causes pain and fatigue, reduces physical function, and negatively impacts health-related quality of life (HRQoL). In the phase III BE OPTIMAL and BE COMPLETE studies, bimekizumab demonstrated clinical efficacy and meaningful improvements in patient-reported outcome (PRO) measures in biologic disease-modifying antirheumatic drug (bDMARD)-naïve patients, and those who had prior inadequate response/intolerance to tumor necrosis factor inhibitors (TNFi-IR).Objectives:To examine the association between achieving increasingly stringent clinical disease control criteria and improvements in PRO measures in patients with active PsA receiving bimekizumab.Design:Post hoc analysis of two phase III studies.Methods:BE OPTIMAL and BE COMPLETE assessed subcutaneous bimekizumab 160 mg every 4 weeks in bDMARD-naïve and TNFi-IR patients with active PsA. Disease control was assessed using American College of Rheumatology (ACR) response criteria, Minimal Disease Activity, Disease Activity Index for Psoriatic Arthritis, and the composite outcome of ACR50 and 100% improvement in Psoriasis Area and Severity Index. Associations between clinical disease control criteria and PRO measures of pain, fatigue, physical function, and HRQoL were assessed at week 16 and week 52/40 (BE OPTIMAL/BE COMPLETE).Results:Achievement of increasingly stringent clinical disease control criteria was generally associated with sequentially greater improvements in all PRO measures, including pain. At week 52/40, 94.7% of bDMARD-naïve and 97.6% of TNFi-IR patients achieving ACR70 reported ⩾50% improvements in pain from baseline, and the greatest numerical improvements (−48.5 bDMARD-naïve; −54.7 TNFi-IR). This pattern was evident as early as week 16 and sustained when assessed at week 52/40 across the majority of clinical disease control criteria and PRO measures reported.Conclusion:The achievement of increasingly stringent thresholds of disease control was associated with corresponding greater improvements in PROs, for patients receiving bimekizumab treatment, irrespective of prior TNFi use.Trial registration ClinicalTrials.gov:NCT03895203, NCT03896581, and NCT04009499.

Abstract:Abstract Background/Aims Bimekizumab (BKZ), a humanised monoclonal IgG1 antibody that inhibits IL-17A and IL-17F, has demonstrated rapid efficacy in joint and skin outcomes for patients with active psoriatic arthritis (PsA) who are bDMARD-naïve (BE OPTIMAL; NCT03895203) and TNFi-inadequate responders (TNFi-IR) (BE COMPLETE; NCT03896581). Here we report longer-term PsA response criteria (PsARC) response, along with health-related quality of life (HRQoL) outcomes in BE OPTIMAL and BE COMPLETE. Methods BE OPTIMAL and BE COMPLETE were randomised multicentre, double-blind, placebo (PBO)-controlled, parallel-group, phase 3 trials. BE OPTIMAL included a 16-week (Wk) double-blind, PBO-controlled period and a 36-Wk active-treatment period. Patients were randomised (3:2) to receive BKZ 160mg Q4W, or PBO. At Wk16, PBO patients switched to BKZ. In BE COMPLETE, patients with inadequate response or intolerance to TNFis were randomised (2:1) to receive either BKZ 160mg Q4W or PBO for 16Wks after which they were eligible for entry into BE VITAL (NCT04009499; open-label extension). We report PsARC response rates at Wk52 (BE OPTIMAL) and Wk40 (BE COMPLETE). Additionally, the association of PsARC responders with HRQoL-related outcomes (HAQ-DI, FACIT-Fatigue and SF-36) for the randomised set are also reported. Data are reported as non-responder imputation (NRI) and observed cases (OC). Results In BE OPTIMAL, 821/852 (96.4%) patients completed Wk16 and 761 (89.3%) completed Wk52. In BE COMPLETE, 388/400 (97%) patients completed Wk16, 377 (94.3%) entered BE VITAL and 360 (90.0%) completed Wk40. Baseline characteristics were comparable between groups within both trials. In BE OPTIMAL, PsARC response (NRI) was achieved by 80.3% BKZ and 40.2% PBO patients at Wk16 and was sustained or increased at Wk52 to 79.1% BKZ and 79.7% PBO/BKZ patients. In BE COMPLETE, PsARC response (NRI) was achieved by 85.4% BKZ and 30.8% PBO at Wk16 and was sustained or increased at Wk40 to 77.9% BKZ and 75.2% PBO/BKZ patients. Clinically meaningful improvements in HAQ-DI and SF 36-PCS scores, and FACIT-Fatigue were achieved from baseline among PsARC responders compared to non-responders (Table). Conclusion Improvements in PsARC were sustained up to 1 year in BKZ-treated patients and associated with improvement in HRQoL and fatigue, regardless of whether patients were bDMARD-naïve (BE OPTIMAL) or TNFi-IR (BE COMPLETE). Bimekizumab for the Treatment of Moderate-to-severe Plaque Psoriasis: a Meta-Analysis of Randomized Clinical Trials

Abstract:Background: Bimekizumab is a monoclonal IgG1 antibody that selectively inhibits both interleukin (IL)-17A and IL-17F, and is a promising drug for patients with moderate-to-severe plaque psoriasis. Objectives: This study aimed to assess the efficacy and safety of bimekizumab in treating patients with psoriasis and to determine the optimal maintenance dosing schedules of bimekizumab. Methods and design: Eligible trials were identified from PubMed, Cochrane Controlled Register of Trials, Embase, ClinicalTrials.gov, and Chinese medical databases. Only double-blind, randomized, active comparator, or placebo-controlled trials of bimekizumab treatment on patients with psoriasis were included in this study. Results: Five studies were identified, which included 2473 patients with moderate-to-severe plaque psoriasis. The results indicated that bimekizumab had better efficacy than placebo or active comparator for Psoriasis Area and Severity Index (PASI) 90 [risk ratio (RR) = 29.29, 1.52; 95% confidence interval (CI) = 10.30–83.30, 1.06–2.19], PASI 100 (RR = 59.87, 2.06; 95% CI = 15.06–237.99, 1.12–3.79), and Investigator’s Global Assessment scores of 0 or 1 (IGA 0/1) (RR = 21.55, 1.36; 95% CI = 9.25–50.19, 1.02–1.81). Faster onset of clinically meaningful responses was observed with bimekizumab compared with both active comparators (RR = 2.59; 95% CI = 1.32–5.10) and placebo (RR = 40.46; 95% CI = 13.19–124.13), with PASI 75 response observed at week 4 after one dose. Subgroup analysis showed no significant difference in the reduction of PASI scores between 320 mg q4w dosage and q8w dosage (RR = 1.00; 95% CI = 0.96–1.03). Rates of patients with adverse events (AEs) were comparable in the bimekizumab and active comparator groups (RR = 1.13; 95% CI = 1.01–1.26), and oral candidiasis was one of the most common treatment-emergent AEs. Conclusion: The results of this meta-analysis suggest that bimekizumab is more efficacious and has a rapid onset of action than active comparators and placebo in the treatment of moderate-to-severe plaque psoriasis. After 16 weeks of initial maintenance treatment, both bimekizumab maintenance dosing schedules (320 mg every 4 and 8 weeks) had similar efficacy.
Effectiveness of long‐term bimekizumab treatment and predictive factors for responders in moderate‐to‐severe psoriasis: A 52‐week real‐world study

Abstract:Psoriasis is a chronic, inflammatory skin disease in which the interleukin (IL)‐23/IL‐17 axis plays a central role. Bimekizumab is a novel antibody that targets both IL‐17A and IL‐17F. This retrospective study aimed to assess the long‐term effectiveness and safety of 52‐week treatment with bimekizumab, and to identify predictive factors for short‐ (16 weeks) and long‐term (52 weeks) responders (i.e., achievers of a Psoriasis Area and Severity Index (PASI) score of 100) to bimekizumab in Japanese patients with psoriasis. The study was conducted on 56 Japanese patients (aged ≥ 15 years) with moderate‐to‐severe psoriasis treated with bimekizumab from May 2022 to March 2024. The therapeutic effectiveness was evaluated by the transition of PASI scores during treatment. Baseline characteristics and clinical and laboratory indexes were compared between responders and poor responders. Treatment‐emergent adverse events (TEAEs) were recorded to assess the safety of the treatment. At week 52, the achievement of PASI 100, static Physician's Global Assessment 0/1, and the Dermatology Life Quality Index 0/1 were 72.4%, 94.7%, and 93.3%, respectively. Short‐term responders showed lower baseline values of neutrophil‐to‐lymphocyte ratio, monocyte‐to‐lymphocyte ratio (MLR), and systemic inflammatory response index compared to poor responders. Long‐term responders showed younger age and lower MLR compared to poor responders. TEAEs were mild or moderate, without serious adverse events. Long‐term treatment with bimekizumab is effective and safe for psoriasis patients. Lower MLR and younger age might predict long‐term response to treatment with bimekizumab, aiding in personalized treatment strategies.

Abstract:Objective To assess the impact of bimekizumab on physical functioning, sleep, work productivity and overall health-related quality of life (HRQoL) in patients with non-radiographic (nr-) and radiographic (r-) axial spondyloarthritis (axSpA) in the phase 3 studies BE MOBILE 1 and 2. Methods Patients were randomised to subcutaneous bimekizumab 160 mg or placebo every 4 weeks; from Week 16, all patients received bimekizumab 160 mg every 4 weeks. We report the following outcomes to Week 52: Bath Ankylosing Spondylitis Functional Index (BASFI), Medical Outcomes Study Sleep Scale Revised (MOS-Sleep-R) Index II, Work Productivity and Activity Impairment: axSpA (WPAI:axSpA), Short Form-36 Physical and Mental Component Summary (SF-36 PCS/MCS) and Ankylosing Spondylitis Quality of Life (ASQoL). Results At Week 16, bimekizumab-randomised patients demonstrated significantly greater improvement from baseline versus placebo in BASFI, SF-36 PCS and ASQoL (p<0.001), and numerically greater improvements in MOS-Sleep-R Index II and WPAI:axSpA scores. Higher proportions of bimekizumab-randomised versus placebo-randomised patients at Week 16 achieved increasingly stringent thresholds for improvements in BASFI (0 to ≤4), and thresholds for meaningful improvements in SF-36 PCS (≥5-point increase from baseline) and ASQoL (≥4-point decrease from baseline). Responses were sustained or further improved to Week 52, where 60%–70% of bimekizumab-treated patients achieved BASFI ≤4 and meaningful improvements in SF-36 PCS and ASQoL, regardless of whether originally randomised to bimekizumab or placebo. Conclusion Bimekizumab treatment led to early improvements in physical function, sleep, work productivity and overall HRQoL at Week 16 in patients across the full axSpA disease spectrum. Abstract:Abstract Psoriasis is a chronic, systemic, immune-mediated disease that is associated with severe comorbidities including depression, psoriatic arthritis and cardiovascular disease. As a result, psoriasis impacts on patient wellbeing and quality of life. Biologic therapies are highly effective for psoriasis; however, 11–35% of patients do not respond to their first treatment within the first year. Bimekizumab is a humanized monoclonal antibody that neutralizes the activity of the proinflammatory cytokines interleukin-17A and interleukin-17F, which are pivotal in developing psoriasis. This study assessed changes in disease severity and health-related quality of life in patients with psoriasis who were treated with bimekizumab in routine clinical practice. This cross-sectional study included adult patients with psoriasis who were attending routine clinical practice visits at five hospitals in the UK. Patients may have received prior therapy for psoriasis, and all were subsequently treated with bimekizumab. In total, 66 patients were included in the study; 35 (53%) were women, the mean age was 47.1 years (SD 15.5), the mean duration of disease was 19.6 years (SD 9.9) and the mean body mass index was 31.2 kg m−2 (SD 6.4). At baseline, the majority of patients (38 of 62, 61%) had extensive or widespread psoriasis, and 23 of 58 (40%) had psoriatic arthritis. Seven patients (11%) were treatment naive to biologic therapy, 55 of 65 (85%) had not responded to previous biologic therapy, and 43 of 65 (66%) had received adalimumab. After initiation of bimekizumab, there was a substantial improvement in disease severity at week 16. The mean Psoriasis Area and Severity Index (PASI) at baseline was 17.1, and no patients had a score of 0; the mean PASI at week 16 was 1.7, and 20 of 52 patients (38%) had a score of 0. This improvement in PASI was accompanied by an improvement in quality of life (Table). Qualitative data reported by patients indicated that treatment with bimekizumab improved their daily lives and decreased psychological stresses. In this study, bimekizumab reduced the severity of psoriasis and improved patients’ quality of life and overall wellbeing. These results indicate that bimekizumab should be considered for the treatment of patients with severe psoriasis where rapid and significant clearance of psoriasis is clinically appropriate, including treatment-naive patients and those resistant to previous biologic therapy. Comparative efficacy and safety of bimekizumab in psoriatic arthritis: a systematic literature review and network meta-analysis

Abstract:Abstract Objectives To understand the relative efficacy and safety of bimekizumab, a selective inhibitor of IL-17F in addition to IL-17A, vs other biologic and targeted synthetic DMARDs (b/tsDMARDs) for PsA using network meta-analysis (NMA). Methods A systematic literature review (most recent update conducted on 1 January 2023) identified randomized controlled trials (RCTs) of b/tsDMARDs in PsA. Bayesian NMAs were conducted for efficacy outcomes at Weeks 12–24 for b/tsDMARD-naïve and TNF inhibitor (TNFi)-experienced patients. Safety at Weeks 12–24 was analysed in a mixed population. Odds ratios (ORs) and differences of mean change with the associated 95% credible interval (CrI) were calculated for the best-fitting models, and the surface under the cumulative ranking curve (SUCRA) values were calculated to determine relative rank. Results The NMA included 41 RCTs for 22 b/tsDMARDs. For minimal disease activity (MDA), bimekizumab ranked 1st in b/tsDMARD-naïve patients and 2nd in TNFi-experienced patients. In b/tsDMARD-naïve patients, bimekizumab ranked 6th, 5th and 3rd for ACR response ACR20/50/70, respectively. In TNFi-experienced patients, bimekizumab ranked 1st, 2nd and 1st for ACR20/50/70, respectively. For Psoriasis Area and Severity Index 90/100, bimekizumab ranked 2nd and 1st in b/tsDMARD-naïve patients, respectively, and 1st and 2nd in TNFi-experienced patients, respectively. Bimekizumab was comparable to b/tsDMARDs for serious adverse events. Conclusion Bimekizumab ranked favourably among b/tsDMARDs for efficacy on joint, skin and MDA outcomes, and showed comparable safety, suggesting it may be a beneficial treatment option for patients with PsA.

Abstract:(200/200) Background Patients with psoriasis are at increased risk of liver function abnormalities. Objective Explore rates of hepatic treatment-emergent adverse events (TEAEs) and changes in liver parameters in bimekizumab-treated patients with psoriasis. Methods Data are reported from five phase 3/3b trials over 2 years. Hepatic TEAEs, laboratory elevations in alanine aminotransferase (ALT) or aspartate aminotransferase (AST), and changes in clinical markers of liver fibrosis (Fibrosis-4 [FIB-4] Index and AST to Platelet Ratio Index [APRI]) are reported. TEAEs are presented using exposure-adjusted incidence rates (EAIRs) per 100 patient-years (PY). Results 2,186 patients received ≥1 bimekizumab dose. Over 2 years, the EAIR of hepatic TEAEs was 3.5/100 PY and did not increase from first to second year. 2-year EAIRs of ALT/AST elevations >3x and >5x ULN were 2.3 and 0.6/100 PY; rates were similar to placebo, adalimumab, secukinumab, and ustekinumab during controlled study periods. FIB-4 and APRI scores did not increase through 2 years, regardless of fibrosis risk at baseline. Limitations Obesity, diabetes, dyslipidemia, chronic alcohol consumption, and medication changes are confounding factors for hepatic dysfunction. Conclusion Rates of hepatic adverse events with bimekizumab were consistent through 2 years; incidences of transaminase elevations were similar to comparators during phase 3/3b controlled study periods.

Abstract:Although several biological disease-modifying antirheumatic drugs (bDMARDs), including interleukin (IL)-17A inhibitors, are approved for psoriatic arthritis, the treatment of this disease remains suboptimal. Bimekizumab (Bimzelx ® ), a dual IL-17A and IL-17F inhibitor, is approved in the EU, Great Britain and Japan for the treatment of psoriatic arthritis. In pivotal phase 3 clinical trials in patients who were bDMARD-naïve or previously had an inadequate response or intolerance to tumour necrosis factor (TNF) α inhibitors, bimekizumab improved the signs and symptoms of psoriatic arthritis across a range of joint, skin, radiographic and patient-reported outcomes compared with placebo, including the proportion of patients achieving a ≥ 50% response in the American College of Rheumatology criteria. Phase 2 clinical trial data have shown that responses are maintained up to 3 years. Bimekizumab was generally well tolerated in patients with psoriatic arthritis, with a safety profile consistent with that in other approved indications. The most common adverse events included nasopharyngitis, upper respiratory tract infection, oral candidiasis, headache and diarrhoea. In conclusion, bimekizumab extends the treatment options available to patients with psoriatic arthritis.

Abstract:IntroductionMatching-adjusted indirect comparisons (MAIC) were used to assess the relative efficacy of bimekizumab 160 mg every 4 weeks (Q4W) compared to guselkumab 100 mg Q4W or every 8 weeks (Q8W) at 48/52 weeks in patients with psoriatic arthritis (PsA) who were biologic disease-modifying antirheumatic drug-naïve (bDMARD-naïve) or with previous inadequate response or intolerance to tumor necrosis factor inhibitors (TNFi-IR).MethodsRelevant trials were identified as part of a systematic literature review. For patients who were bDMARD-naïve, individual patient data (IPD) from BE OPTIMAL (N = 431) was matched to summary data from DISCOVER-2 (Q4W, n = 245; Q8W, n = 248). For patients who were TNFi-IR, IPD from BE COMPLETE (n = 267) and summary data from COSMOS (Q8W, N = 189). Trial populations were re-weighted using propensity scores. Unanchored comparisons of recalculated bimekizumab and guselkumab 48- or 52-week non-responder imputation outcomes for 20/50/70% improvement in American College of Rheumatology score (ACR20/50/70) and minimal disease activity (MDA) index were analyzed.ResultsIn patients who were bDMARD-naïve, bimekizumab was associated with a greater likelihood of ACR50 (odds ratio [95% confidence interval] 1.62 [1.07, 2.44]; p = 0.021), ACR70 (2.20 [1.43, 3.38]; p < 0.001), and MDA (1.82 [1.20, 2.76]; p = 0.005) compared to guselkumab Q4W at week 52. Bimekizumab also had a greater likelihood of ACR70 response (2.08 [1.34, 3.22]; p = 0.001) and MDA (2.07 [1.35, 3.17]; p < 0.001) compared to guselkumab Q8W at week 52. In patients who were TNFi-IR, bimekizumab had a greater likelihood in achieving all evaluated outcomes compared to guselkumab Q8W at week 48/52 (ACR20, 1.77 [1.15, 2.72]; p = 0.010; ACR50, 1.56 [1.03, 2.36]; p = 0.037; ACR70, 1.66 [1.05, 2.61]; p = 0.028; and MDA, 1.95 [1.27, 3.02]; p = 0.003).ConclusionsAccording to MAICs, bimekizumab demonstrated greater or comparable efficacy on ACR50/70 and MDA outcomes than guselkumab in patients with PsA who were bDMARD-naïve and TNFi-IR at week 48/52. Abstract:Abstract Background/Aims Given the chronic nature of psoriatic arthritis (PsA), sustaining high levels of disease control with treatment is important. Assessing maintenance of response in patients that achieve treatment targets is of interest as loss of response can occur with long-term therapy. Bimekizumab (BKZ), a monoclonal IgG1 antibody that selectively inhibits interleukin (IL)-17F in addition to IL-17A, has demonstrated rapid, clinically meaningful joint and skin efficacy responses at Week (Wk) 16 versus placebo (PBO) in patients with PsA; responses were sustained to Wk52. We report the maintenance of response in joint and skin efficacy outcomes to 52 wks in BKZ-treated patients with PsA who responded at Wk16. Methods BE OPTIMAL (NCT03895203), which included a 16-wk double-blind, PBO-controlled period and a 36-wk active treatment-blind period, assessed BKZ in biologic disease-modifying antirheumatic drug (DMARD)-naïve patients with active PsA. Patients were randomised 3:2:1 to subcutaneous BKZ 160mg every 4 wks (Q4W), PBO or reference (adalimumab 40mg Q2W). At Wk16, PBO patients switched to BKZ 160mg Q4W. Maintenance of response is reported as the percentage of BKZ-treated patients who achieved response at Wk16 and maintained at Wk52. Data are reported for patients randomised to BKZ 160mg Q4W at baseline. Endpoints include American College of Rheumatology (ACR)20/50/70, Psoriasis Area and Severity Index (PASI)75/90/100, minimal/very low disease activity (MDA/VLDA) and Disease Activity Index for PSoriatic Arthritis (DAPSA) remission or low disease activity (REM+LDA; ≤14) and remission (REM; ≤4) responses. Wk16 responders are reported using nonresponder imputation (NRI); maintenance of response to Wk52 is reported using NRI and observed case (OC). Treatment-emergent adverse events (TEAEs) to Wk52 are reported for patients who received ≥1 dose of BKZ, including patients randomised to PBO at baseline. Results At baseline, 431 patients were randomised to BKZ 160mg Q4W; 217/431 (50.3%) had psoriasis affecting ≥3% body surface area (BSA). 414/431 (96.1%) completed Wk16; 388 (90.0%) completed Wk52. Most patients who achieved response at Wk16 maintained at Wk52 across a range of joint and skin outcomes. At Wk16, 268 (62.2%)/189 (43.9%)/105 (24.4%) patients achieved ACR20/50/70; responses were maintained at Wk52 by 88.4%/86.8%/82.9% (NRI); 92.9%/91.1%/87.9% (OC). Of 217 patients with psoriasis affecting ≥3% BSA at baseline, 168 (77.4%)/133 (61.3%)/103 (47.5%) achieved PASI75/90/100 at Wk16. Most patients maintained response at Wk52: 88.1%/82.7%/79.6% (NRI); 98.7%/94.0%/89.1% (OC). The same pattern was observed for composite measures of efficacy. 194(45.0%) patients achieved MDA at Wk16; 85.6% (NRI)/90.7% (OC) maintained response at Wk52. At Wk16, 205 (47.6%)/47 (10.9%) achieved DAPSA REM+LDA/DAPSA REM; 85.9%/68.1% maintained response at Wk52. To Wk52, 555/702 (79.1%) BKZ-treated patients reported ≥1 TEAE; 46 (6.6%) reported serious TEAEs. Conclusion With BKZ treatment, most Wk16 responders maintained robust efficacy responses to Wk52 across joint, skin and composite efficacy outcomes. Abstract:Abstract Background/Aims Psoriatic arthritis (PsA) is a chronic disease affecting multiple domains; however, patients can experience loss of response with long-term therapy. Therefore, maintaining long-term treatment responses is important. Bimekizumab (BKZ), a monoclonal IgG1 antibody that selectively inhibits IL-17F in addition to IL-17A, demonstrated rapid and clinically meaningful improvements in joint and skin efficacy outcomes to Week 16, vs placebo (PBO), that were sustained to Week 52. The objective of this analysis was to report maintenance of response in joint, skin and composite efficacy outcomes to 52 weeks in BKZ-treated patients with PsA who were Week 16 responders. Methods BE COMPLETE (NCT03896581), a 16-week double-blind phase 3 study, included patients with active PsA who had inadequate response or intolerance to 1-2 TNF inhibitors (TNFi-IR). Patients were randomised 2:1 to subcutaneous BKZ 160 mg every 4 weeks (Q4W) or PBO. Patients completing Week 16 were eligible to enter an open-label extension, BE VITAL (NCT04009499). Maintenance of response is reported as the percentage of BKZ-treated patients who achieved a response at Week 16 and maintained response at Week 52. ACR20/50/70, Psoriasis Area and Severity Index (PASI)75/90/100, minimal/very low disease activity (MDA/VLDA) and Disease Activity Index for PsA (DAPSA) remission/low disease activity (REM+LDA; ≤14) and remission (REM; ≤4) responses are presented. Data are reported as observed case (OC) and using non-responder imputation (NRI) or multiple imputation (MI). Treatment-emergent adverse events (TEAEs) to Week 52 are reported for patients who received ≥1 dose of BKZ, including those who entered BE VITAL (PBO/BKZ). Results Overall, 267 pts were randomised to BKZ 160 mg Q4W; 263 (98.5%) and 236 (88.4%) completed Weeks 16 and 52, respectively. Of BKZ-treated pts who were Week 16 responders, ≥80% maintained response across all joint and skin outcomes, as well as MDA and DAPSA REM+LDA. At Week 16, 179 (67.0%), 116 (43.4%), and 71 (26.6%) patients achieved ACR20/50/70, respectively. Over 80% of responders maintained ACR20/50/70 response at Week 52: 81.6%, 80.2%, 83.1% (NRI); 89.6%, 86.1%, 85.5% (OC). Of 176 patients with psoriasis affecting ≥3% body surface area (BSA) at baseline, 145 (82.4%), 121 (68.8%) and 103 (58.5%) achieved PASI75/90/100 at Week 16; 88.3%, 88.4%, 84.5% (NRI); 97.0%, 96.4%, 91.6% (OC) maintained a PASI 75/90/100 response at Week 52. Similar results were observed for composite measures; 118 (44.2%) patients achieved MDA at Week 16; 81.4%/87.3% (NRI/OC) maintained their response at Week 52. High proportions of Week 16 VLDA/DAPSA REM+LDA/DAPSA REM responders also maintained response at Week 52. To Week 52, 243/388 (62.6%) BKZtreated patients reported ≥1 TEAE; 23 (5.9%) reported serious TEAEs. Conclusion BKZ demonstrated robust maintenance of response at Week 52 in TNFi-IR patients with PsA who responded to BKZ treatment at Week 16. The safety profile was consistent with previous reports. Abstract:IntroductionMatching-adjusted indirect comparisons (MAICs) were used to compare the efficacy of bimekizumab and secukinumab 150 mg and 300 mg at 52 weeks for the treatment of psoriatic arthritis (PsA) in patients who were biologic disease-modifying anti-rheumatic drug-naïve (bDMARD-naïve) or with previous inadequate response or intolerance to tumor necrosis factor inhibitors (TNFi-IR).MethodsRelevant trials were systematically identified. Individual patient data from bimekizumab randomized controlled trials, BE OPTIMAL (N = 431) and BE COMPLETE (N = 267), were matched to aggregate data from bDMARD-naïve and TNFi-IR patient subgroups from FUTURE 2 using secukinumab 150 mg and 300 mg doses (bDMARD-naïve: N = 63/37; TNFi-IR: N = 67/33). To adjust for cross-trial differences, patients from the bimekizumab trials were re-weighted using propensity scores to match the baseline characteristics of patients in the secukinumab trials. Unanchored comparisons of recalculated bimekizumab and secukinumab 52-week non-responder imputation outcomes for 20/50/70% improvement in American College of Rheumatology score (ACR20/50/70) and minimal disease activity (MDA) index were analyzed.ResultsIn patients who were bDMARD-naïve, bimekizumab had a greater likelihood of ACR70 response than secukinumab 150 mg (odds ratio [95% confidence interval] 2.39 [1.26, 4.53]; p = 0.008) and secukinumab 300 mg (2.03 [1.11, 3.72]; p = 0.021) at 52 weeks. In patients who were TNFi-IR, bimekizumab had a greater likelihood of response compared to secukinumab 150 mg for ACR20 (3.50 [1.64–7.49]; p = 0.001), ACR50 (3.32 [1.41, 7.80]; p = 0.006), ACR70 (2.95 [1.08, 8.07]; p = 0.035) and MDA (3.52 [1.38, 8.99]; p = 0.009), and a greater likelihood of response compared to secukinumab 300 mg for ACR50 (2.44 [1.06, 5.65]; p = 0.037) and MDA (2.92 [1.20, 7.09]; p = 0.018) at 52 weeks.ConclusionIn this MAIC analysis, the efficacy of bimekizumab, as demonstrated by the likelihood of ACR20/50/70 and MDA response at 52 weeks, was greater or comparable to secukinumab 150 mg and 300 mg for patients with PsA who were bDMARD-naïve and TNFi-IR.Trial Registration NumbersNCT03895203, NCT03896581, NCT04009499, NCT01752634, NCT01989468, NCT02294227, NCT02404350.

Abstract:Abstract Background/Aims The Group for Research and Assessment of Psoriasis and Psoriatic Arthritis (PsA; GRAPPA) domainbased treatment recommendations for PsA focus on six domains: peripheral arthritis, axial disease, enthesitis, dactylitis, skin psoriasis, and nail psoriasis, and PsA-related conditions: uveitis and IBD. Bimekizumab (BKZ), a monoclonal IgG1 antibody that selectively inhibits IL17F in addition to IL17A, has superior clinical efficacy vs placebo (PBO) to Week (Wk)16 in phase 3 clinical trials of PsA patients. In patients with psoriasis, superior skin domain efficacy was demonstrated versus secukinumab (IL-17A inhibitor), ustekinumab (IL-12/23 inhibitor), and adalimumab (TNF inhibitor [TNFi]). Methods Included patients received subcutaneous BKZ 160 mg or PBO every 4-wks (Q4W) in BE OPTIMAL (NCT03895203; biologic disease-modifying antirheumatic drugs [bDMARD]naïve patients with PsA), BE COMPLETE (NCT03896581; patients with PsA who were TNFiinadequate responders [TNFiIR]), BE MOBILE 1 (NCT03928704; nonradiographic axial spondyloarthritis [nr-axSpA]) and BE MOBILE 2 (NCT03928743; radiographic [r-]axSpA [i.e. ankylosing spondylitis]). BE OPTIMAL included a reference arm (adalimumab 40 mg Q2W) to Wk52; data not shown. From Wk16, all PBO-randomised patients received BKZ 160 mg Q4W to Wk52 (PBO/BKZ). Wk16 completers from BE COMPLETE entered BE VITAL (NCT04009499; openlabel extension). Outcomes are reported by GRAPPA domain as change from baseline and/or responder rate. Missing data were imputed using non-responder (binary) or multiple (continuous) imputation. Results Wk52 completion was high (BE OPTIMAL: 770/852 [90.4%], BE COMPLETE: 347/400 [86.8%], BE MOBILE 1: 220/254 [86.6%], BE MOBILE 2: 298/332 [89.8%]). Baseline demographics and disease characteristics were previously reported. Across all GRAPPA domains, improvements from Wk16 were sustained at Wk52 in BKZ-treated patients across all studies. Individual domain responses were generally consistent between bDMARD naïve and TNFi-IR patients; results from BE MOBILE 1 and 2 demonstrated BKZ efficacy in patients with axSpA (Table). Responses were generally consistent between BKZ and PBO/BKZ patients at Wk52. To Wk52, there were low rates of adjudicated IBD (2 [0.2%] in BE OPTIMAL) and no instances of uveitis. Conclusion Treatment with BKZ resulted in robust and sustained improvements across GRAPPA domains to Wk52 for both bDMARD naïve and TNFi-IR patients with PsA; results from patients with axSpA support efficacy in the axial domain. Abstract:Bimekizumab, which suppresses both interleukin (IL)‐17A and IL‐17F, has recently been approved as a biologic for psoriasis. We aimed to evaluate the real‐world effectiveness and safety of bimekizumab for psoriasis and to identify predictive factors for its treatment responsiveness. We analyzed 36 Japanese patients with psoriasis (19 with psoriasis vulgaris and 17 with psoriatic arthritis) from May 2022 to September 2023. All patients received bimekizumab (320 mg every 4 weeks) until week 16. Seventeen patients (43.2%) had experienced bio‐switch. The median (interquartile range) baseline total psoriasis area and severity index (PASI) was 6 (3.2–20.0). Total PASI rapidly and significantly decreased at week 4 by a median 79.8% from baseline, and gradually decreased thereafter. The PASI on the trunk, and upper and lower limbs rapidly and significantly decreased at week 4 compared to baseline and plateaued thereafter. The neutrophil‐to‐lymphocyte ratio and neutrophil number significantly decreased at week 16 compared to baseline. At weeks 4, 8, 12, and 16, the achievement rate of absolute PASI ≤2 was 72.2%, 80.6%, 92.9%, and 96.4%, respectively; that of absolute PASI ≤1 was 41.7%, 61.3%, 85.7%, and 82.1%; that of PASI 75 was 55.5%, 52.9%, 69.7%, and 75.8%; that of PASI 90 was 36.1%, 50.0%, 57.6%, and 62.9%; and that of PASI 100 was 19.4%, 38.2%, 51.5%, or 57.6%, respectively. Linear multivariate regression analysis revealed that younger age was associated with a higher percentage reduction of total PASI at weeks 4 and 8. There were no serious or fatal adverse events during treatment. In conclusion, bimekizumab rapidly and remarkably reduced the total PASI together with high achievement rates of absolute PASI ≤1 and ≤2, and with favorable safety in real‐world clinical practice. Younger age may be a predictive factor for a good treatment response to bimekizumab.

Abstract:Background Patients with psoriasis have increased risk of suicidal ideation/behavior (SIB) and depression. Bimekizumab, a biologic that inhibits interleukin (IL)-17A and IL-17F, received FDA approval in 2023 for moderate to severe plaque psoriasis, following 2021 EMA approval. Objective To report SIB and depression in patients with moderate to severe psoriasis treated in bimekizumab clinical trials. Methods Mental health changes, including neuropsychiatric events, were actively monitored across nine bimekizumab in psoriasis phase 2/3 trials. The patient-reported electronic Columbia-Suicide Severity Rating Scale (eC-SSRS; measuring SIB) and Patient Health Questionnaire-9 (PHQ-9; measuring depression) were administered, monitored by an independent Neuropsychiatric Adjudication Committee. Results Throughout 7,166 patient-years (PY) of bimekizumab exposure, the adjudicated SIB rate was 0.13/100PY; SIB ranges for the general psoriasis population and patients receiving anti-IL-17A/anti-IL-23 therapies are 0.09-0.54/100PY and 0.09-0.19/100PY, respectively. At Week 16, 92.9% vs. 81.1% of bimekizumab- vs. placebo-treated patients had no/minimal depression. New-onset positive eC-SSRS responses and mean PHQ-9 scores were low for bimekizumab-treated patients. Limitations Patient exclusion for significant/severe pre-specified SIB/depression history. Conclusion The long-term adjudicated SIB rate with bimekizumab was low and within ranges reported in the general psoriasis patient population and psoriasis patients treated with anti-IL-17A/anti-IL-23 biologics. Screening/monitoring questionnaires reported low SIB and depression levels.
