Abstract:e13580 Background: Oncology practice is becoming more complex and collaboration between academic and community practices is necessary to optimize care for patients. This qualitative analysis explores perceived gaps between academic and community practices and proposed solutions to a more cohesive oncology ecosystem for optimal quality and care delivery. Methods: A 10-question mixed-methods quantitative and qualitative survey was developed (SurveyMonkey) to identify practice setting, methods of collaboration, perceived barriers, and strategies for improvement. Both open-ended and multiple-choice questions were used. Survey was distributed via social media outreach on Facebook, X, and email. Individual responses were coded by AK, then compared using two sample z-test for proportions. Results: 68 responses were collected, comprising 45.5% community and 44.1% academic physicians. Non-clinician respondents (2.9%) were excluded. Respondents had been in practice: >20 years (16.42%), 10-20 years (40.3%), 5-9 years (19.4%) and < 5 years (23.9%). Collaboration commonly occurred via conferences (73.68%), shared Electronic Medical Records (EMRs, 59.65%) or research (38.6%). The main barriers perceived are summarized (Table). Community oncologists describe lack of appreciation from academics, time constraints, difficulty communicating. Main barriers to collaboration perceived by academic oncologists include delayed referral, time constraints, difficulty communicating, lack of shared EMRs, fear of patient displacement from the primary referring practice, and administrative red tape. Proposed solutions from both groups include engaging community oncologists in academic tumor boards (n=8/68,11.8%) and regional education events (n=2/68, 2.9%). Improved communication. Additionally, both groups were interested in expanding clinical trials in the community. Both groups noted the need for financial incentives and institutional support for successful cooperation (n=3/68, 4.4%). Conclusions: Our data unveiled the nuances of collaboration. Barriers mainly consist of lack of acknowledgement, poor communication, and barriers in EMR utilization. Both groups struggle with time constraints. This data can inform multi-shareholder groups working for systems-based process improvement, expanding collaboration between community and academic oncology. [Table: see text]

Abstract:64 Background: Collaboration between oncology providers and pharmacy teams has improved the safety of oral antineoplastic (OA) prescribing, but opportunities remain for improvements in care delivery. To this end, starting in March 1, 2021, the Dana Farber Cancer Institute Specialty Pharmacy implemented a new workflow - clinical verification of new specialty prescriptions by dedicated OA clinical pharmacists (OAPs) prior to prescription processing. To reduce the impact of initial prescription errors on efficiency and safety, one component of clinical verification was to expand the purview of OAPs to make certain modifications to prescriptions that could be automatically processed without requiring prescriber re-signing. Automatically processed prescription modifications include corrections to quantity or days supply, indication of cycle length or days on/off therapy, and clarification of directions. Here we report on the impact of this portion of the clinical verification program. Methods: OAP prescription modification (OAP-PM) was introduced on November 1, 2022. To assess the impact on efficiency, we conducted a pre-post analysis of the proportion of prescription modifications taking > 60 minutes to complete for 6 months before and after OAP-PM introduction. Using prescription-level data, we analyzed co-variates including pre-/post-OAP-PM, patient age (continuous), site (academic vs. network), communication with someone other than the primary prescriber (y/n), and medication category (anti-neoplastic vs. supportive). We used control charts to explore the contribution of specific medications and sites that might share best practices or be the focus of subsequent, more focused improvement projects. Results: From May 1, 2022 to May 1, 2023, 458 prescription modifications were completed, 180 pre- and 278 post-OAP-PM. The proportion of prescriptions requiring > 60 minutes to complete decreased from 29.4% to 19.8% (p = 0.0184). In multivariable regression, the significant predictors of prescription taking > 60 minutes to complete were pre-OAP-PM (LR 4.9, p = 0.027); network (LR 20.8, p < 0.0001); and communication with someone other than the primary provider (LR 18.6, p < 0.0010). Control charts demonstrated all sites were within 3-sigma limits pre- and post-OAP-PM, but with wide limits at many sites. Individual medications could not be meaningfully analyzed due to the small sample sizes. Conclusions: OAP-PM successfully reduced the proportion of OA prescriptions taking more than an hour to modify. This may be a useful workflow change for other specialty pharmacies to adopt to improve the efficiency of OA care. Additional work is needed to understand whether site-, provider- or medication-specific changes can improve upon this model.

Abstract:Journal of Oncology Pharmacy Practice, Ahead of Print. IntroductionCardiovascular complications can occur in oncology patients secondary to certain cancer therapies. Pharmacists are involved in the care of oncology patients who are at risk of or experiencing cardiotoxicity related to their cancer therapy. Our study aimed to understand how pharmacists in Canada care to these patients and to explore their experiences, perceptions, and challenges.MethodsCanadian pharmacists currently involved in the care of patients receiving cancer treatments and at risk of or experiencing cardiotoxicity were invited to participate in a 30-min telephone interview using an interview guide. A combination of inductive and deductive reasoning was applied using two coders who independently reviewed the transcribed interviews and identified key concepts and themes.ResultsEight pharmacists were interviewed. Perceived benefits included sharing specialized knowledge and conducting safety assessments. Perceived challenges were the lack of role recognition and resources and fractured continuity of care. Proposed future directions were to play a more substantial role in direct medication management, creation of specific guidance and tools to support the clinical decision-making process, and to understand how pharmacists at other sites were providing care through the creation of a community of practice.ConclusionsAs patient-focused medication specialists, pharmacists help guide clinical decision-making, assess cardiac risk factors, and offer individualized education to meet the holistic needs of oncology patients at risk of or experiencing cardiotoxicities. The creation of a cardio-oncology community of practice may allow pharmacists with a common interest to connect, share learnings, and collaborate on how to continue to advance the delivery of care.

Abstract:Oral anticancer medications (OAMs) are high priced with a significant cost-sharing burden to patients, which can lead to catastrophic financial, psychosocial, and clinical repercussions. Cost-conscious prescribing and inclusion of low-cost alternatives can help mitigate this burden, but cost transparency at the point of prescribing remains a major barrier to doing so. Pharmacy assistance programs, including co-payment cards and patient assistance programs administered by manufacturers and foundation-based grants, remain an essential resource for patients facing prohibitive co-payments for OAMs. However, access to these programs is fraught with complexities, including lack of trained financial navigators, limited transparency on eligibility criteria, onerous documentation burdens, and limits in available funding. Despite these drawbacks and the potential for such programs to incentivize manufacturers to keep list prices high, assistance programs have been demonstrated to improve financial well-being for patients with cancer. The increasing development of integrated specialty pharmacies with dedicated, trained pharmacy staff can help improve and standardize access to such programs, but these services are disproportionately available to patients seen at tertiary care centers. Multistakeholder interventions are needed to mitigate the burden of cost sharing for OAMs, including increased clinician knowledge of financial resources and novel assistance mechanisms, investment of institutions in trained financial navigation services and centralized platforms to identify assistance programs, and policies to cap out-of-pocket spending and improve transparency of rates charged by pharmacy benefit managers to a health plan.

Abstract:Journal of Oncology Pharmacy Practice, Ahead of Print. IntroductionThe increasing number of oral anticancer medicines (OAMs) dispensed in community pharmacies and the associated challenges (misuse, management of side effects) give the community pharmacist (CP) a major role in the pharmacotherapeutic management of cancer patients. In France, as a response to these challenges, cancer outpatients can schedule a meeting with their CP to ensure the safe and effective use of OAMs. The objectives of this study were to evaluate the perspectives of these interventions regarding their implementation and the opinion of French CPs.MethodsA declarative survey and semi-structured interviews were conducted with CPs that dispensed at least one OAM between January 2021 and March 2022. The study was conducted between April and August 2022.ResultsEighty-five CPs completed the survey. Of these pharmacists, 21% (n = 18) had already performed OAM interventions and 91% (n = 61) wanted to implement them. Lack of time, knowledge and training were the main barriers to implementation. No correlations were identified between the characteristics of community pharmacies and the likelihood of implementing OAM interventions.ConclusionsConsidering that CPs seem willing to implement them and the favourable context in France, this observational study highlights the potential of OAM interventions to improve the management of cancer patients. Though further studies are required to better evaluate the implementation and the potential effects of these interventions, OAM interventions could be relevant strategies in other healthcare systems to secure the management of cancer patients through the involvement of the CP.

Stakeholders' perceptions of a community-based advanced nurse practitioner-led integrated oncology care model for adults receiving oral anti-cancer medication-A qualitative study

Abstract:Oral anti-cancer medications (OAMs) are increasingly prescribed in oncology, and although administered at home, ongoing monitoring generally requires the patient to attend an acute hospital. With the requirement to provide safe yet convenient care and to increase hospital capacity, the potential exists to transition this cohort of patients to the community to be assessed by oncology health care professionals (HCPs). The onset of COVID-19 facilitated this planned transition. Objective: The primary objective was to understand stakeholders' perceptions of a community-based advanced nurse practitioner (ANP)-led integrated OAM care model for adults. Methods: Qualitative data from interviews and focus groups were obtained from 33 individuals; either service users who attended ANP-led OAM clinics or stakeholders involved in OAM care. Data were subsequently analysed using thematic analysis. Results: Four themes were identified and included reflection on pre-COVID-19 system, role of ANP in current OAM care, importance of robust communication and infrastructural requirements for transition to an integrated OAM care model. Conclusion: This study demonstrated that patients and HCPs perceived the proposal positively. They identified the ANP as the appropriate HCP to care for this cohort and the importance of communication and strategic planning for transitioning this model of care to the community setting. Medication use process and assessment of extemporaneous compounding and alternative routes of administration of oral oncology drugs: Guidance for clinical and oncology pharmacists

Abstract:Cancer treatment is evolving. Oral formulations offer potentially more convenient, effective alternatives to conventional chemotherapy for many cancers. An appealing aspect of oral antineoplastic agents is their relative ease of administration; unfortunately, patients may have difficulty swallowing the tablets or capsules. This shift towards oral administration and the accompanying issues around oral medication adherence highlights the need to assess the safety and feasibility of delivery methods for patients unable to swallow solid dosage forms. A new medication‐use process (MUP) was created to facilitate extemporaneous compounding of oral oncology drugs (OOD). Systematic review consisting of tertiary resources, drug manufacturers, and primary literature was performed to identify information related to extemporaneous compounding or alternative routes of administration for a predetermined list of OODs. A single‐center retrospective review was conducted of patients receiving liquid formulations of OODs to characterize institutional experience. Overall, 116 OODs were reviewed. The majority of OODs had information describing extemporaneous compounding or alternative routes of administration (31% with commercial liquid formulations available or information in tertiary resources; 33% with off‐label manufacturer information or case reports) while 36% had no data. The MUP included three compounding workflows to account for available solid dosage formulations (tablet, hard capsule, gelatin capsule) and creation of an electronic health record order template. Retrospective review identified 41 patients who received compounded liquid formulations of 14 OODs for 13 indications. Liquid OODs were used for a median of 3‐5 days. None of the 41 patients experienced discernable safety or overt effectiveness concerns readily attributed to use of liquid OODs. Implementation of a novel MUP and systematic review of OODs aids the pharmacist in standardizing institutional practice and mitigates risks associated with extemporaneous liquid formulations of OODs facilitating medication delivery of new agents to patients unable to swallow solid dosage forms.

Abstract:Background: With the advent of novel oral oncolytics, the cancer care delivery model for HR+/HER2- breast cancer (BC) is evolving, shifting the responsibility for medication adherence and monitoring and managing of side effects to patients and their caregivers. In this quality improvement (QI) initiative, we aimed to uncover and address gaps in patient and provider education, risk reduction, adverse event management, and shared decision-making in order to improve outcomes for HR+/HER2+ patients on oral oncolytics. Methods: From 9/2022 to 12/2022, 54 HCPs who treat breast cancer and 111 HR+/HER2- BC patients from 4 US community oncology clinics completed surveys assessing attitudes, values, and challenges related to practice patterns, education, and therapy adherence in HR+/HER2- BC. HCPs participated in audit-feedback sessions to reflect on survey results and their own practice, and developed action plans to improve care, which were implemented in subsequent collaborative patient-provider learning sessions. Results: Top reported HCP challenges in HR+/HER2- BC care include providing patient-centered supportive care measures (32%), patient adherence/lack of follow up (22%), and individualizing treatment plans (17%). Additionally, 43% of providers reported that improved collaboration across interprofessional teams would most improve care. Top reported patient challenges include worry about and/or difficulty managing side effects from treatment (26%), difficulty scheduling visits with many different HCPs/following up (24%), and difficulty communicating with healthcare teams (16%). In surveys, 28% of patients reported frequently or very frequently missing/skipping doses of oral breast cancer medication. Top reasons for missing/skipping doses included forgetting or not having it nearby (36%), cost (21%), and side effects (20%). To address the top challenges identified in baseline surveys, HCPs developed and implemented action plans, such as improving the identification of patients at high risk of recurrence, establishing patient-provider educational sessions, increasing the use of patient navigators, incorporating oral adherence toolkits, and educating patients and staff about available supportive care resources. After implementation of patient-provider collaborative education sessions at community clinics, patients reporting high levels of knowledge about BC treatment options and communicating side effects/symptoms increased from 27% to 57%, and 37% to 77%, respectively. At 60-day follow-up, 58% of patients reported taking a more active role in treatment decision-making, and 52% reported setting reminders/alarms/using apps to remember to take medication on time. Conclusions: Patient adherence to oral oncolytic medications remains a challenge in BC treatment. This quality improvement initiative uncovered underlying barriers to effective treatment management of HR+/HER2- breast cancer patients on oral oncolytics. Action plans and educational resources were developed and implemented to address identified barriers and optimize treatment management for HR+/HER2- BC. These methods and findings represent key opportunities for improvement that can be implemented in community oncology practices to improve adherence to oral oncolytics and overall HR+/HER2- breast cancer care. Pharmacologic Management of Persistent Pain in Cancer Survivors

Abstract:Improvements in screening, diagnosis and treatment of cancer has seen cancer mortality substantially diminish in the past three decades. It is estimated there are almost 20 million cancer survivors in the USA alone, but some 40% live with chronic pain after completing treatment. While a broad definition of survivorship that includes all people living with, through and beyond a cancer diagnosis—including those with active cancer—is often used, this narrative review primarily focuses on the management of pain in people who are disease-free after completing primary cancer treatment as adults. Chronic pain in this population needs a different approach to that used for people with a limited prognosis. After describing the common chronic pain syndromes caused by cancer treatment, and the pathophysiologic mechanisms involved, the pharmacologic management of entities such as post-surgical pain, chemotherapy-induced neuropathy, aromatase inhibitor musculoskeletal syndrome and checkpoint inhibitor-related pain are described. The challenges associated with opioid prescribing in this population are given special attention. Expert guidelines on pain management in cancer survivors now recommend a combination of pharmacologic and non-pharmacologic modalities, and these are also briefly covered.

Abstract:The movement to deliver cancer care in resource-limited settings is gaining momentum, with particular emphasis on the creation of cost-effective, rational algorithms utilizing affordable chemotherapeutics to treat curable disease. The delivery of cancer care in resource-replete settings is a concerted effort by a team of multidisciplinary care providers. The oncology pharmacy, which is now considered integral to cancer care in resourced medical practice, developed over the last several decades in an effort to limit healthcare provider exposure to workplace hazards and to limit risk to patients. In developing cancer care services in resource-constrained settings, creation of oncology pharmacies can help to both mitigate the risks to practitioners and patients, and also limit the costs of cancer care and the environmental impact of chemotherapeutics. This article describes the experience and lessons learned in establishing a chemotherapy pharmacy in western Kenya.

Abstract:34 Background: Team-based care improves quality, reduces fragmentation and lowers clinician burnout related to ad hoc care coordination. A key teamwork principle is managing timing and sequence of interdependent care (Vogel JOP 2016). The 4R Oncology model featured by the NCI ASCO Teams initiative fosters effective teamwork by facilitating systematic timing / sequence of interdependent care.1 4R is Right Info / Care / Patient / Time. Using a 4R intervention, a large health system created a high-functioning care team and conducted optimizations of guideline-based interdependent care in breast and lung cancers Oct 2020 - Dec 2021.2 We present results of these optimizations. Methods: We compared data from electronic health record on timing and sequence of care between historical control cohorts of patients who received care pre-4R (breast n=274, lung n=173) to the post-optimization 4R cohorts (breast n=211, lung n=140). Patient and tumor characteristics between control and 4R cohorts in both cancers were similar. Results: Timing and sequence of care for 6 of the 7 breast cancer metrics and all 7 lung cancer metrics was significantly improved between the control and 4R cohorts (Table). Despite significant increase, the rate of timely care for some metrics remained low, such as referrals based on distress screening in breast cancer and palliative care consult in lung cancer. Conclusions: The 4R Oncology model is effective in applying teamwork principles and improving timing and sequence of guideline-based care. Opportunities for further care optimizations exist to expand the 4R benefit to a higher number of patients. A learning system established in our institution will inform iterative optimizations. 1. Abstract:444 Background: CMS approved Chronic (CCM) and Principal Care Management (PCM) to improve patient care. These services are typically non-face-to-face; providers can bill ≥ twenty minutes /month. In oncology, this may help with comorbidity and symptom management to reduce the risk and expense of avoidable hospitalizations and ER visits and pay providers for time spent. In 2021, a system of community cancer centers initiated a program with a partner to manage chronic conditions more effectively at home. Approximately three hundred people were enrolled. A review of the data found that patients did not meet the medical criteria for the program, and care was not billable. An internal program was created to improve the enrollment process and financial sustainability. A clinical needs assessment was completed with our oncologists, outlining revised criteria for patient enrollment. Qualified patients were enrolled by their attending oncologist during the initial face-to-face visit and assigned a dedicated RN. Methods: Three hundred unenrolled, actively treating patients were randomly selected from January 2023 to May 2023 by a group of data analysts not associated with the program to be compared to enrolled patients who have been enrolled on average six months. Patients were compared by age, gender, cancer type, and race. The LACE Index for Readmission, a validated tool using the length of stay, acuity of admission, comorbidities, and ER visits to predict the risk of readmission, was done retrospectively by RNs for all patients. Enrolled patients received a comprehensive clinical needs assessment by RNs, and a provider-approved care plan was created. RNs provided patients with education on lifestyle modification and assisted in setting achievable goals. The care plan was monitored with weekly calls, providing the opportunity to respond to patients’ needs proactively. Symptom and medication management were provided, and psychological and nutritional needs were addressed. This coordination of services was documented in the electronic health record, and superbills were created and sent to billing and coding monthly. Results: See table. Conclusions: The purpose of this study was to compare hospital admissions between the groups. Our findings suggest that CCM and PCM may reduce unplanned admissions, with physician engagement pivotal in driving appropriate enrollment. The current program is labor-intensive and further investigation is required to determine the most cost-effective way of scaling the program with the possible use of artificial intelligence, RPM, or other technology.[Table: see text]

Abstract:Photosynthetic reaction centers are integral membrane complexes. They have potential application as molecular photovoltaic structures and have been used in diverse technological applications. A three-dimensional electrostatic model of the photosystem I reaction center (PSI) embedded in a lipid membrane is presented. The potential is obtained by solving the Poisson-Boltzmann equation with the finite element method (FEM). Simulations showing the potential distribution in a vesicle containing PSI reaction centers under different conditions are presented. Published online Jan 29, 2024. doi: 10.21037/apm-24-23 The November 2023 issues of Annals of Palliative Medicine featured 3 Editorials, 5 Original Articles, 19 Review Articles, 3 Editorial Commentary Articles, and 1 Case Report, along with 3 Letters to the Editor. One of the Editorials (1), commissioned by Editorial Office, critically reexamines the scope of Annals of Palliative Medicine based on a review of the disciplinary development and evolving definition of palliative medicine. While Annals of Palliative Medicine has always focused since its inception on providing up-to-date and cutting-edge information and professional support for health care providers in palliative medicine disciplines to improve the quality of life for patients and their families and caregivers (2), the discipline of palliative medicine has evolved considerable since the first printed issue of the journal in 2012. This Editorial is a highly educational read and discusses the history of palliative medicine and then critically considers the definition of palliative medicine in the contact of varying existing definitions by major medical and other governing bodies. Next, Zhang and colleagues discuss how palliative medicine, palliative care, hospice care, supportive care, and public health palliative care are related. With such a critical assessment and the most up-to-date analysis of the definition of palliative medicine currently available, the authors then reassess the scope of Annals of Palliative Medicine . Recognizing a broad scope of palliative medicine, they defined eight primary focuses of scope for the journal to provide clearer guidance to authors, reviewers, readers, reviewers, and editors as follows (1): Reading this comprehensive Editorial, it is quite clear that palliative medicine has developed into a discipline with a broad scope, encompasses patients from hospice care to those with supportive care needs, and includes palliative care at all stages of various serious and life-threatening diseases and end-of-life situations. Editorial workflow improvements are also discussed, as is a commitment of Annals of Palliative Medicine to continually reassess the scope of palliative medicine and its increasing emphasis on symptom management and supportive care to best serve the needs of its readers and of palliative care and more expanded healthcare providers who rely on its content to inform them on how to provide the most optimal care for their patients (1). Furthermore, the November 2023 journal issue had a preponderance of oncology articles, with 4 of the 5 Original Articles and 16 of the 19 Review Articles focusing on oncology care in the palliative setting. The integration of palliative care with oncology care is a critical element of management in patients with advanced presentations of cancer, and it is a primary focus of this journal, as detailed above. As such, the remainder of this Message from the Editor-in-Chief will focus on one Review Article that discusses this integration between palliative care and oncology and provides commentary on the path ahead and optimizing this integration. Caglayan and colleagues from the United Kingdom detail how both the healthcare environment and the oncology treatment landscape have become progressively more intricate, necessitating providers caring for patients with cancer to become increasingly well versed in the best way to integrate palliative care with cancer care (3). Furthermore, the psychosocial care needs of patients with advanced cancers and their families are often distinct and influence management approaches. The authors describe the key elements of palliative care delivery and focus on early integration of palliative care in the comprehensive management of patients with advanced malignancies. Such early integration of palliative care for these patients has been widely reported in Annals of Palliative Medicine even since the first issues of this journal (4-6) as a means of better improving patient quality of life, symptom control, and patient and provider satisfaction. Such early integration can also reduce aggressive care at the end of life, increase rates of completing advanced directives, reduce hospitalizations, and shorten hospital lengths of stay, all of which can serve to reduce medical costs (7,8). Additionally, in select scenarios, such early integration of palliative care can also significantly improve patient survival (9,10). Caglayan and colleagues discuss the optimal timing of the referral to patient care for patients with advanced malignancies and what the optimal model for service delivery is for these patients. Abstract:As treatment strategies for patients with colorectal cancer advance, there has now become an ever-increasing need for multidisciplinary teams to care for these patients. Recent investigations into the timing and duration of perioperative therapy, as well as, the rise of molecular profiling have led to more systemic chemotherapeutic options. The most efficacious use, in terms of timing and patient selection, of these therapies in the setting of modern operative and radiotherapy techniques requires the generation of care teams discussing cases at multidisciplinary conferences. This review highlights the role of multidisciplinary team conferences, advances in perioperative chemotherapy, current clinical biomarkers, and emerging therapeutic agents for molecular subtypes of metastatic colon cancer. As our understanding of relevant molecular subtypes increases and as data becomes available on treatment response, the treatment of colorectal cancer will become more precise and effective.

Pharmacist-Led Video Consultation to Identify and Mitigate Drug Interactions Among Patients Initiating Oral Anticancer Drugs

Abstract:Purpose: The past decade has seen an increase in oral anticancer drug (OACD) approvals. Polypharmacy and drug-drug interactions (DDIs) likely contribute to OACD toxicity. We assessed a one-time pharmacist-led video consultation to identify DDIs. Methods: We conducted a single-arm telehealth intervention of a one-time 30-minute pharmacist-led video consultation among patients initiating OACDs. The visit focused on identifying polypharmacy and DDIs. Feasibility was defined as ≥50% completion of all study interventions. We determined the prevalence, characteristics, and severity of OACD-related potential DDIs. We also assessed the prevalence of medication list inaccuracies, polypharmacy, patient satisfaction, and patient perception of intervention acceptability, appropriateness, and feasibility. Results: Of 58 eligible patients, 43 (74%) completed the intervention and 33 (57%) completed all evaluations. Median medication per patient was nine (range 4-21), and 98% of patients had at least five prescriptions. The median number of medication list errors was two (range 0-16), with at least one error for 76% and >1 for 52%. Pharmacists identified OACD-related interactions in 18 cases (42%), including change in drug metabolism (eight), elimination (one), and absorption (three). Interactions were classified as Lexicomp categories C (13), D (five), or X (one) requiring close monitoring or a change in treatment. All patients expressed high satisfaction with the intervention and agreed or completely agreed that it was acceptable, appropriate, and feasible. Conclusion: Polypharmacy, medication list errors, and DDIs are prevalent among patients initiating OACDs. A one-time remote pharmacist-led video consultation can address OACD-related DDIs, which may decrease medication complexity and improve adherence.
