Validation of an Updated Algorithm to Identify Patients With Incident Non-Small Cell Lung Cancer in Administrative Claims Databases

Abstract:Purpose: Real-world lung cancer data in administrative claims databases often lack staging information and specific diagnostic codes for lung cancer histology subtypes. This study updates and validates Turner's 2017 treatment-based algorithm using more recent claims and electronic health record (EHR) data. Methods: This study used Optum's deidentified Market Clarity Data of linked medical and pharmacy claims with EHR data. Eligible patients had an incident lung cancer diagnosis (January 2014-December 2020) and ≥one valid histology code for lung cancer 30 days before to 60 days after diagnosis. Histology and stage information from the EHR were used to evaluate the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV). We evaluated the Turner algorithm using cohort 1 patients diagnosed between June 2014 and October 2015 (step 1) and between November 2015 and December 2020 after approval of immunotherapies (step 2). Next, we evaluated cohort 2 patients diagnosed between November 2015 and December 2020 using an updated algorithm incorporating the latest US treatment guidelines (step 3), and compared the results for cohort 2 (Turner algorithm, step 2 patients). Furthermore, an algorithm to determine early NSCLC (eNSCLC; stage I-III) versus metastatic or advanced/metastatic non-small cell lung cancer (stage IV) was evaluated among patients with available histology and stage information. Results: A total of 5,012 patients were included (cohort 1, step 1: n = 406; cohort 1, step 2: n = 2,573; cohort 2, step 3: n = 2,744). The updated algorithm showed improved performance relative to the previous Turner algorithm for sensitivity (0.920-0.932), specificity (0.865-0.923), PPV (0.976-0.988), and NPV (0.640-0.673). The eNSCLC algorithm showed high specificity (0.874) and relatively low sensitivity (0.539). Conclusion: An updated treatment-based algorithm identifying patients with incident NSCLC was validated using EHR data and distinguished lung cancer subtypes in claims databases when EHR data were not available.
Development and Optimization of a Bladder Cancer Algorithm Using SEER-Medicare Claims Data

Abstract:Purpose: Categorizing patients with cancer by their disease stage can be an important tool when conducting administrative claims-based studies. As claims databases frequently do not capture this information, algorithms are increasingly used to define disease stage. To our knowledge, to date, no study has used an algorithm to categorize patients with bladder cancer (BC) by disease stage (non-muscle-invasive BC [NMIBC], muscle-invasive BC [MIBC], or locally advanced/metastatic urothelial carcinoma [la/mUC]) in a US-based health care claims database. Methods: A claims-based algorithm was developed to categorize patients by disease stage on the basis of the administrative claims portion of the SEER-Medicare linked data. The algorithm was validated against a reference SEER registry, and the algorithm's parameters were iteratively modified to improve its performance. Patients were included if they had an initial diagnosis of BC between January 2016 and December 2017 recorded in SEER registry data. Medicare claims data were available for these patients until December 31, 2019. The algorithm was evaluated by assessing percentage agreement, Cohen's kappa (κ), specificity, positive predictive value (PPV), and negative predictive value (NPV) against the SEER categorization. Results: A total of 15,484 patients with SEER-confirmed BC were included: 10,991 (71.0%) with NMIBC, 3,645 (23.5%) with MIBC, and 848 (5.5%) with la/mUC. After multiple rounds of algorithm optimization, the final algorithm had an agreement of 82.5% with SEER, with a κ of 0.58, a PPV of 87.0% for NMIBC, and 76.8% for MIBC and a high NPV for la/mUC of 98.0%. Conclusion: This claims-based algorithm could be a useful approach for researchers conducting claims-based studies categorizing patients with BC at diagnosis.
Molecular Characterization and Prognostic Analysis of Non-Small Cell Lung Cancer: A Single Centric Real-World Study.

Abstract:e21646 Background: Identifying targetable genetic variants may help maximize clinical benefit of non-small cell lung cancer (NSCLC). Here, we retrospectively analyzed the benefit of next-generation sequencing (NGS) for NSCLC in routine clinical practice. Methods: A total of 59 NSCLC patients were enrolled in the study from Jun. 2018 to Dec. 2019. Formalin-fixed paraffin-embedded tissues samples were collected and sequenced by NGS, which detects single-nucleotide variants, small insertions/deletions, rearrangements, and copy-number alterations of either 59 or 1021 tumor related genes. The tumor response was evaluated using RECIST v1.1. Results: The detection rate of targetable variants was 59.32% (35/59), including EGFR mutation 49.15% (29/59), ALK rearrangement 5.08% (3/59), ROS1 rearrangement 1.69% (1/59), BRAF V600E mutation 1.69% (1/59), and ERBB2 mutation 1.69% (1/59). Of 29 patients with EGFR mutations, 5 patients (17.24%) had multiple EGFR mutations, including 2 patients (6.89%) with uncommon mutations. 25patients (86.21%) harbored concurrent mutations that may limit the efficacy of EGFR-TKIs, including EGFR amplification, activation of bypass signaling pathways, TP53 exon8 mutation, BCL2L11 deletion polymorphism, and PI3K-AKT-mTOR and cell cycle gene alterations. Among 25 patients received EGFR-TKIs, TKI effectiveness was evaluated in 18 patients. The disease control rate was 100% and overall response rate was 27.78%. The proportion of patients with TMB-H and/or PD-L1≥1, and targetable variants negative was 33.89% (20/59). Only one patient of this group received immunotherapy with pembrolizumab and achieved SD at the 2 nd month. The proportion of patients with targetable variants negative, TMB-L, and PD-L1 < 1 or unknow patients was 6.78% (4/59), two patients of them left hospital without additional treatment, two patients received chemotherapy and the KRAS mutation present in one of them suggested that he might be resistant to EGFR-TKIs. Conclusions: More than half of NSCLC patients have targetable genetic variants and more than 30% patients have a molecular signature that can be treated with immunotherapy. Compared with immunotherapy, targeted therapy has higher acceptability in real-world. NGS-based genetic testing may have clinical value to predict the effectiveness of targeted therapy and immunotherapy. It can be widely implemented and standardized into clinical use.

Abstract:Sedation is used almost universally in the care of critically ill patients, especially in those who require mechanical ventilatory support or other life-saving invasive procedures. This review will focus on the sedation strategies for critically ill patients and the pharmacology of commonly used sedative agents. The role of neuromuscular blocking agents in the ICU will be examined and the pharmacology of commonly used agents is reviewed. Finally a strategy for rational use of these sedative and neuromuscular blocking agents in critically ill patients will be proposed.

Abstract:Background: Despite the high prevalence of lung cancer, with a five-year survival rate of only 23%, the underlying molecular mechanisms of non-small cell lung cancer (NSCLC) remain unknown. There is a great need to identify reliable candidate biomarker genes for early diagnosis and targeted therapeutic strategies to prevent cancer progression. Methods: In this study, four datasets obtained from the Gene Expression Omnibus were evaluated for NSCLC- associated differentially expressed genes (DEGs) using bioinformatics analysis. About 10 common significant DEGs were shortlisted based on their p-value and FDR ( DOCK4, ID2, SASH1, NPR1, GJA4, TBX2, CD24, HBEGF, GATA3, and DDR1). The expression of significant genes was validated using experimental data obtained from TCGA and the Human Protein Atlas database. The human proteomic data for post- translational modifications was used to interpret the mutations in these genes. Results: Validation of DEGs revealed a significant difference in the expression of hub genes in normal and tumor tissues. Mutation analysis revealed 22.69%, 48.95%, and 47.21% sequence predicted disordered regions of DOCK4, GJA4, and HBEGF, respectively. The gene-gene and drug-gene network analysis revealed important interactions between genes and chemicals suggesting they could act as probable drug targets. The system-level network showed important interactions between these genes, and the drug interaction network showed that these genes are affected by several types of chemicals that could serve as potential drug targets. Conclusions: The study demonstrates the importance of systemic genetics in identifying potential drug- targeted therapies for NSCLC. The integrative system- level approach should contribute to a better understanding of disease etiology and may accelerate drug discovery for many cancer types.

Abstract:Introduction: Across multiple studies, the most common serious adverse event groups that Small Cell Lung Cancer (SCLC) patients experience, whilst undergoing chemotherapy treatment, are: Blood and Lymphatic Disorders, Infections and Infestations together with Metabolism and Nutrition Disorders. The majority of the research that investigates the relationship between adverse events and SCLC patients, focuses on specific adverse events such as neutropenia and thrombocytopenia. Aim: This study aims to utilise machine learning in order to identify those patients who are at risk of developing common serious adverse event groups, as well as their specific adverse event classification grade. Methods: Data from five clinical trial studies were analysed and 12 analysis groups were formed based on the serious adverse event group and grade. Results: The best test runs for each of the models were able to produce an area under the curve (AUC) score of at least 0.714. The best model was the Blood and Lymphatic Disorder group, SAE grade 0 vs. grade 3 (best AUC = 1, sensitivity rate = 0.84, specificity rate = 0.96). Conclusion: The top features that contributed to this prediction were total bilirubin, alkaline phosphatase, and age. Future work should investigate the relationship between these features and common SAE groups.

Abstract:<span> To develop algorithms to identify metastatic castration-sensitive prostate cancer (mCSPC) patients and castration-resistant prostate cancer (mCRPC) patients, using health claims data and laboratory test results. A targeted literature review summarized mCSPC and mCRPC patient selection criteria previously used in real-world retrospective studies. Novel algorithms to identify mCSPC and mCRPC were developed based on diagnosis codes indicating hormone sensitivity/resistance, prostate-specific antigen (PSA) test results, and claims for castration and mCRPC-specific treatments. These algorithms were applied to claims data from Optum Clinformatics Extended DataMart (Date of Death) Databases (commercial insurance/Medicare Advantage [COM/MA]; 01 January 2014–31 July 2019) and Medicare Fee-for-Service (Medicare-FFS; 01 January 2014–31 December 2017). Previous real-world studies identified mCSPC primarily based on metastasis diagnosis codes, and mCRPC based on mCRPC-specific drugs. Using the current study's algorithms, 7034 COM/MA and 19,981 Medicare-FFS patients were identified as having mCSPC, and 2578 COM/MA and 11,554 Medicare-FFS as having mCRPC. Most mCSPC patients were identified based on evidence of being hormone/castration-naive. Patients were identified as having mCRPC most commonly based on rising PSA (COM/MA), or at the metastasis diagnosis date if it occurred after castration (Medicare-FFS). Among patients with mCSPC, 14–17% had evidence of progression to castration resistance during a median 1-year follow-up period, mostly based on use of mCRPC-specific drugs. Comprehensive algorithms based on claims and laboratory data were developed to identify and distinguish patients with mCSPC and mCRPC. This will facilitate appropriate identification of mCSPC and mCRPC patients based on health claims data and better understanding of patient unmet needs in real-world settings.</span>

Abstract:e20581 Background: Small-cell lung cancer (SCLC) is an aggressive disease with rapid progression rate and high metastatic potential that accounts for 15% of all lung cancers. Early-stage SCLC is usually asymptomatic; therefore, the majority of the SCLC patients were diagnosed in advanced stages. Although some radiological features were reported with advanced stages SCLC by CT scans, diagnosis of early stages SCLC from CT images has remained challenging since the radiological features are not distinct. Neuron-specific enolase (NSE) is commonly used in the clinical setting as a biomarker for SCLC, but the sensitivity of the test did not meet the needs. Here, we proposed a study using Circulating Genetically Abnormal Cells (CAC) to improve the diagnosis rate of SCLC in the early stages. Methods: A series of 26 stage I SCLC patients were enrolled from October 2019 to October 2020. All participants were having newly discovered pulmonary nodules ≤ 30mm when enrolled in the study, with no previous cancer history. Before the histopathological examination, 10 ml of peripheral blood were collected from each patient for CAC test, by using a 4 color FISH–based assay to detect the genetic abnormalities of 3p22.1, 3q29, 10q22.3, and CEP10 in the chromosome. Besides, NSE was performed on all participants. Both tests were intended to evaluate the malignant risk of lung nodules. Results: Confirmed by histopathological exam, 69% patients were diagnosed with SCLC located on the central part of the lung, and 31% patients were found to have peripheral-type SCLC. 88% of the patients in the overall cohort were smokers or past smokers, and only 12% participants had a family history of cancer. For Immunohistochemistry tests, the numbers of participants with positive CD56, TTF-1, CgA, CK, Syn and Ki-67 ≥ 60% are 65%, 58%, 12%, 42%, 50% and 69%, respectively. For diagnostic performance, the sensitivity of CAC in the over-all cohort is 73.08% (95%CI: 52.21%-88.43%), compared with 50.00% (95%CI: 29.93%- 70.07%) for NSE. CAC has achieved 77.78% (95%CI: 52.36% - 93.59%) and 62.5% (95%CI：24.49% - 91.48%) sensitivity in the centra and peripheral group. For NSE, the sensitivity of these two groups were 55.56% (95%CI: 30.76% - 78.47%) and 37.5% (95%CI: 8.52% - 75.51%). Conclusions: Compared with NSE, CAC is more sensitive to stage I SCLC, especially for SCLC that arising in central locations. This non-invasive method can be a complementary diagnostic tool with LDCT for smokers aged ≥65 with newly discovered pulmonary nodules. Clinical trial information: ChiCTR1900026233.
Real-World Evidence on Dose-Reduction and Treatment Outcomes in Small Cell Lung Cancer; A Bayesian mixed effects and Competitive Risk Approach

Abstract:Background: Small cell lung cancer (SCLC) is a challenging disease to treat due to rapid progression, development of chemo-resistance, and discrepancies in outcomes between real-world data and clinical trials. Previous studies lack comprehensive analyses of intermediate events and the treatment process, such as treatment decisions, progression of disease, and the occurrence of adverse events (AEs) over time. The aim of this study was to apply advanced statistical methods on a longitudinal SCLC data set in order to identify factors of importance for the risk of AEs and for survival. Methods: Information was collected on the treatment pathways of 421 SCLC patients treated at the Karolinska University Hospital between 2016-2022 (Stockholm, Sweden). Analysis focused on the impact of dose-adjustment on adverse events (AEs), including neutropenia, by estimating odds ratios (OR) using Bayesian mixed-effects modelling. Covariate's effects on ECOG performance status (PS) deterioration and early discontinuation of chemotherapy with cause-specific hazard ratios (csHR) were explored using competitive risk models. This approach was applied to cohorts of patients receiving first line platinum etoposide, and second line platinum etoposide or platinum irinotecan. Results: At the end of the first line treatment, most patients exhibited tumour regression (n=167). Patients with neutropenia had longer overall survival (HR: 0.70 [0.53, 0.92]). Higher etoposide dose levels were associated with subsequent occurrences of AEs (OR: 5.97 [1.41, 30.5]) and neutropenia (OR: 3.55 [1.03, 13.3]). Dose adjustment did not affect overall survival as long as the patient completed the four-dose regimen treatment. For the second-line, fewer patients completed four treatment cycles and the most common reason of early discontinuation was tumour progression (n=72). Male patients experienced fewer AEs and better first line treatment response compared to females (csHR: 0.51 [0.25, 0.90]). High-risk patients (here defined as ECOG PS 2-3, or age over 75 years) with early discontinuation of therapy had survival outcomes similar to those who did not receive therapy. Conclusions: Our results indicate that SCLC therapies may benefit from more individualized dosing strategies. These strategies would aim to balance improved survival with reduced risk of AEs, particularly neutropenia. It would also be beneficial to assess the risk-benefit of treating specific subgroups, including patients receiving second line therapy. Real-world data are crucial for studying therapy response and risk-benefit of treating patient groups that are underrepresented in clinical trials.

Novel machine learning algorithm to predict response to immunotherapy in patients with small cell and non-small cell lung cancer.

Abstract:e13556 Background: Immune checkpoint inhibitors (ICI) are used to manage patients with both small cell (SCLC) and non-small cell (NSCLC) lung cancer. However, ICI response rates are often low, and identifying patients that will benefit from ICIs can be challenging. The value of biomarkers used to predict ICI response, such as PD-L1, Combined Positive Score (CPS) or tumor mutational burden (TMB) have been debated. Furthermore, the resources needed to assess these biomarkers may not be available in many centres. Developing more accurate and accessible tools that predict ICI responses could enable a precision medicine approach that improves patient outcomes. This study aimed to use a novel machine-learning (ML) algorithm to predict response to different ICI therapies in patients with lung cancer based on clinically available data. Methods: 334 eligible records were cleaned and reprocessed from textual to categorical data using one hot encoding. Complete datasets were available for 161 patients. Differences in the data distribution were handled using the Synthetic Minority Oversampling Technique. Six ML algorithms were trained, including Linear regression, Support Vector Classifier, XGBoost Classifier, Random Forest, Decision Tree, and Gaussian Naive Bayes Classifier. These algorithms used 80% of the training data, were tested on 20% of validation data and used the Grid Search Cross-Validation technique for hyperparameter optimization. Results: For the 161 patients included in the final analysis, the mean age was 68 years and 48% were female. 9% of patients had SCLC and 80% had NSCLC. Patients receiving Pembrolizumab, Nivolumab and Atezolizumab comprised 62%, 11% and 25%, respectively. The artificial intelligence (AI) algorithm predicted and stratified ICI response better than PD-L1 levels. Of the ML algorithms, XGBoost Classifier predicted response with the most accuracy, 64% (0.61 F1 score). This model found that good performance status (0-1), female gender and adenocarcinoma sub-type predicted response to ICI. On the other hand, M1, N2 staging, male gender, squamous cell carcinoma sub-type and receiving Atezolizumab were predictive of disease progression. Conclusions: This study developed multiple novel ML models to predict responses to ICIs in lung cancer. XGBoost Classifier used clinically available data to show that the type of ICI a patient receives, their histopathology sub-type and their TMN staging impact ICI response. Future work will aim to improve accuracy and predict ICI toxicity by including data from multiple centres, different cancer types and additional clinical variables. [Table: see text]

Abstract:84 Background: Lung cancer is often detected in advanced stages and can benefit from biomarker testing to identify targeted therapeutics. The IMPACT-NSCLC study was designed to identify barriers to equitable biomarker testing for lung cancer patients and develop targeted interventions to help improve biomarker testing rates using a digital personal health record and AI-enabled analysis. Methods: IMPACT-NSCLC unites the O'Neal Comprehensive Cancer Center (UAB) and Novellia, a leading personal health record tool, in a two-part single-institution study of adult NSCLC patients. Novellia uses proprietary technology and AI-enabled services to help patients digitally collect retrospective health records from providers and track health records prospectively, offering researchers a novel method of collecting and evaluating longitudinal health data. Part 1 of the study involves a retrospective study of patients with advanced NSCLC, with advanced data modeling to identify barriers and predictive factors in biomarker testing. Part 2 focuses on developing, implementing, and evaluating targeted interventions based on Part 1’s findings with the goal of improving biomarker testing rates in both academic and community settings. Results: In Part 1, data were collected from 35 patients diagnosed with NSCLC from 2020-2024. Most were white (80.77%) and male (53.84%). Median age was 65 (range 51-79). Most received care at an academic (NCI-designated) cancer center (74.28%). Most were not on commercial insurance plans (85.71%), with the most common form being BCBS/Medicare B and Viva Medicare. Most patients had Stage IV NSCLC (82.85%). More patients received liquid NGS testing at diagnosis (91.42%) than tissue NGS testing at diagnosis (68.57%). Nearly all patients (82.85%) received combination therapy, such as pemetrexed, and pembrolizumab, following biomarker testing. Oncologists initiated treatment while biomarker testing results were pending or incomplete more often in non-academic settings (55.55% of cases) vs. academic settings (26.92%). Conclusions: The pilot analysis found that patients with NSCLC typically received biomarker testing at diagnosis, more often with liquid biopsy. However, oncologists, especially in non-academic settings, often started treatment before full biomarker testing was completed. Comprehensive biomarker testing was frequently not done at progression. Further analysis leveraging machine learning and advanced modeling of Part 1 data may reveal more predictive factors for these variations, guiding interventions in Part 2. Developing a scalable best-practice guide for comprehensive biomarker testing could improve health system efficiencies and outcomes for patients with NSCLC.

Abstract:Background Small cell lung cancer (SCLC) is an aggressive and almost universally lethal neoplasm. There is no accurate predictive method for its prognosis. Artificial intelligence deep learning may bring new hope. Methods By searching the Surveillance, Epidemiology, and End Results database (SEER), 21,093 patients’ clinical data were eventually included. Data were then divided into two groups (train dataset/test dataset). The train dataset (diagnosed in 2010–2014, N = 17,296) was utilized to conduct a deep learning survival model, validated by itself and the test dataset (diagnosed in 2015, N = 3,797) in parallel. According to clinical experience, age, sex, tumor site, T, N, M stage (7th American Joint Committee on Cancer TNM stage), tumor size, surgery, chemotherapy, radiotherapy, and history of malignancy were chosen as predictive clinical features. The C-index was the main indicator to evaluate model performance. Results The predictive model had a 0.7181 C-index (95% confidence intervals, CIs, 0.7174–0.7187) in the train dataset and a 0.7208 C-index (95% CIs, 0.7202–0.7215) in the test dataset. These indicated that it had a reliable predictive value on OS for SCLC, so it was then packaged as a Windows software which is free for doctors, researchers, and patients to use. Conclusion The interpretable deep learning survival predictive tool for small cell lung cancer developed by this study had a reliable predictive value on their overall survival. More biomarkers may help improve the prognostic predictive performance of small cell lung cancer.

Abstract:In the absence of a method to ensure that crystals can be obtained for any given protein, the possibility of developing scaffolds for protein crystallisation becomes attractive. Among several approaches that could yield scaffolds, two are particularly promising: the first is based on immunoglobulin Fab fragments and immunoglobulin binding proteins while the second is based on fusion proteins. In the Fab based scaffold, the protein of interest is the antigen recognised by the antibody. In the second case, it is a protein fused to one of the scaffold components. The operational difference between the two methods is the existence of a flexible covalent tether compared to a highly specific interaction. The relative merits and disadvantages of each approach are discussed here. We also describe a lattice obtained through a combinatorial approach which appears to have the required properties to be considered a scaffold. The system makes use of an Fab derived from a rheumatoid factor and an Fc-fusion protein. The Fc-fusion system is ideal for enhanced expression of glycoproteins in mammalian cells and provides a useful tag for their purification. The molecular replacement shows a mode of binding for this rheumatoid factor that is not competitive with bacterial Fc-binding proteins. Hence it may be possible to generalize the method to include bacterial expression of fusion proteins with either protein A or protein G as the fusion partner.
Inferring the Evolution and Progression of Small-Cell Lung Cancer by Single-Cell Sequencing of Circulating Tumor Cells

Abstract:AbstractPurpose:Genomic analyses of small-cell lung cancer (SCLC) are limited by the availability of tumor specimens. This study aimed to investigate the suitability of single-cell sequencing of circulating tumor cells (CTC) as a method of inferring the evolution and progression of SCLCs.Experimental Design:Between July 1, 2011, and July 28, 2014, 48 consecutively diagnosed patients with SCLC were recruited for this study. CTCs were captured from each patient with CellSearch system. Somatic mutations and copy number alterations (CNA) were monitored by single-cell sequencing of CTCs during chemotherapy.Results:Single-cell sequencing of CTCs can provide a mutational atlas for SCLC. A 10-CNA score based on single CTCs was established as a classifier for outcomes of initial chemotherapy in patients with SCLC. The survival analyses demonstrated that patients with low CNA scores (<0) had significantly prolonged progression-free survival (PFS) and overall survival (OS) after first-line chemotherapy in comparison with those with high scores (≥0; PFS: 212 days vs. 110.5 days, P = 0.0042; and OS: 223.5 days vs. 424 days, P = 0.0006). The positive predictive value and negative predictive value of the CNA score for clinical subtype (refractory vs. sensitive) were 80.0% and 93.7%, respectively. By tracing allele-specific CNAs in CTCs isolated at different time points during chemotherapy, we showed that CNA heterogeneity might result from allelic losses of initially consistent CNAs.Conclusions:Single CTC-based sequencing can be utilized to depict the genomic profiles and evolutionary history of SCLC, thus offering the potential for clinical stratification of patients with SCLC.

Abstract:e13548 Background: Administrative claims provide valuable real-world insight into the care of cancer patients; however, claims data lacks cancer stage information. This limitation constrains research on the value of early diagnosis and treatment as well as on the costs and savings associated with increased cancer screening. In prior work, our team used the SEER-Medicare data to develop machine learning (ML) algorithms to stage non-small cell lung (NSCLC), colon (CC), and rectal (RC) cancer patients using clinical flags derived from claims data. These algorithms were 69% (RC), 78% (NSCLC), and 83% (CC) accurate at matching incident cancer patients with their SEER-recorded AJCC stage (SEER-stage) at diagnosis. This work sought to test whether these ML algorithms are sufficiently accurate for use in claim cost analyses. Methods: Incident NSCLC, CC, and RC patients were identified using 2016-2017 SEER-Medicare data and assigned a cancer stage using a claims-based predictive multinomial logistic regression model (R Statistical Software - v4.1.2 R Core Team 2021; nnet package - Venables and Ripley 2002). Patients’ cumulative medical and pharmacy costs were summarized for 12 months starting with patients’ index month. Patients’ Medicare index month was set equal to the month of their first claim with a cancer diagnosis. Patients’ SEER index month was set equal to the diagnosis month associated with their incident tumor record in SEER. Patients with each cancer type were then grouped two ways - by ML-stage and by SEER-stage. Median patient costs were compared between stage groups for each cancer type and differences tested for statistical significance using Wilcoxon Rank-Sum Testing. Results: For NSCLC and CC, raw differences in median 12-month claim costs between the ML- and SEER-stage cohorts were small (1%-4%). Cost differences for RC were higher (7%-17%). ML and SEER costs were not significantly different (p > 0.05) between later-stage cohorts (NSCLC stages 3 and 4, CC stages 2C/3 and 4, and RC stage 4); however, early-stage groups were always significantly different (p < 0.05). Conclusions: Although costs were not statistically equivalent across all stage groups, the similarity of ML and SEER costs across higher-stage cohorts and small raw differences in median costs for each NSCLC and CC group suggests that ML algorithms with higher accuracy may be used to develop costs from administrative data for stage shift modeling and cost tradeoff analyses. Abstract:Background: Precision therapies, such as targeted and immunotherapies, have substantially changed the landscape of late-stage non-small cell lung cancer (NSCLC). Yet utilization of these therapies is disproportionate across strata defined by race and socioeconomic status (SES), possibly due to disparities in molecular diagnostic testing (or "biomarker testing"), which is a prerequisite to treatment. Methods: We extracted a cohort of NSCLC patients from the Surveillance, Epidemiology and End Results (SEER)-Medicare linked data. The primary outcome was receipt of a molecular diagnostic test, based on claims data. The primary predictors were race and SES. Likelihood of receiving a molecular diagnostic test, and overall survival (OS), were investigated using logistic and Cox proportional hazards regression, adjusted for sex, age, residence, histology, marital status, and comorbidity. Results: Of the 28,511 NSCLC patients, 11,209 (39.3%) received molecular diagnostic testing. Compared to White patients, fewer Black patients received a molecular diagnostic test (40.4% vs 27.9%; p < .001). After adjustment, Black patients (ORadj [odds ratio]: 0.64; 95% CI [confidence interval]: 0.58-0.71) and those living in areas with greater poverty (ORadj: 0.85; 95% CI: 0.80-0.89) had statistically significant decreased likelihood of molecular diagnostic testing. Patients who did receive testing had a statistically significant decreased risk of death (HRadj [hazards ratio]: 0.74; 95% CI: 0.72-0.76). These results held in the stratified analysis of stage IV NSCLC patients. Conclusion: Disparities exist in comprehensive molecular diagnostics, which is critical for clinical decision making. Addressing barriers to molecular testing could help close gaps in cancer care and improve patient outcomes.

Abstract:Objective This study aimed to validate claims-based algorithms for identifying SLE and lupus nephritis (LN) in Medicare data, enhancing the use of the Lupus Index for geospatial research on SLE prevalence and outcomes. Methods We retrospectively evaluated the performance of rule-based algorithms using the International Classification of Diseases, 10th Revision (ICD-10) codes to identify SLE and LN in a well-defined prospective longitudinal cohort of patients with and without SLE from a South Carolina registry and rheumatology outpatient clinics. The analysis included comparison of algorithms based on Medicare fee-for-service claims data to these rigorously phenotyped populations. The primary classification for SLE cases was based on the American College of Rheumatology and Systemic Lupus Erythematosus International Collaborating Clinics criteria for SLE and LN. Algorithms were based on the number of ICD-10 codes with and without a 30-day separation in the observation period, including all of 2016–2018. Results The algorithm using two ICD-10 codes for SLE, with or without a 30-day separation, showed the best overall performance. For LN, specific ICD-10 codes outperformed combinations of SLE and renal/proteinuria codes that were found in ICD-9. Conclusions The findings of this study highlight the performance of specific ICD-10 code algorithms in identifying SLE and LN cases within Medicare data, providing a valuable tool for informing use of the Lupus Index. This index allows for improved geographical targeting of clinical resources, health disparity studies and clinical trial site selection. The study underscores the importance of algorithm selection based on research objectives, recommending more specific algorithms for precise tasks like clinical trial site identification and less specific ones for broader applications such as health disparities research.

Abstract:Background: Patients with non-small cell lung cancer (NSCLC) and patients with NSCLC combined with chronic obstructive pulmonary disease (COPD) have similar physiological conditions in early stages, and the latter have shorter survival times and higher mortality rates. The purpose of this study was to develop and compare machine learning models to identify future diagnoses of COPD combined with NSCLC patients based on the patient's disease and routine clinical data. Methods: Data were obtained from 237 patients with COPD combined with NSCLC as well as NSCLC admitted to Ningxia Hui Autonomous Region People's Hospital from October 2013 to July 2022. Six machine learning algorithms (K-nearest neighbor, logistic regression, eXtreme gradient boosting, support vector machine, naïve Bayes, and artificial neural network) were used to develop prediction models for NSCLC combined with COPD. Sensitivity, specificity, positive predictive value, negative predictive value, accuracy, F1 score, Mathews correlation coefficient (MCC), Kappa, area under the receiver operating characteristic curve (AUROC)and area under the precision-recall curve (AUPRC) were used as performance indicators to evaluate the performance of the models. Results: 135 patients with NSCLC combined with COPD, 102 patients with NSCLC were included in the study. The results showed that pulmonary function and emphysema were important risk factors and that the support vector machine-based identification model showed optimal performance with accuracy:0.946, recall:0.940, specificity:0.955, precision:0.972, npv:0.920, F1 score:0.954, MCC:0.893, Kappa:0.888, AUROC:0.975, AUPRC:0.987. Conclusion: The use of machine learning tools combining clinical symptoms and routine examination data features is suitable for identifying the risk of concurrent NSCLC in COPD patients.

Abstract:695 Background: Small cell carcinoma of the bladder (SCCB) is a rare variant of bladder cancer with limited data informing prognosis. We present a novel prognostic model for disease specific survival (DSS) for patients with non-metastatic (M0) SCCB, derived from the Surveillance, Epidemiology and End Results (SEER) database. Methods: The SEER database (18 registries/November 2020) was queried with the ICD-10 topography codes C67.0-C67.9 (bladder), and the morphologic code 8041 (SCCB). Eligible patients (adults, histologic confirmation, available follow-up/staging data, M0) were randomly divided into a training (TC) and a validation cohort (VC) (7:3 ratio). Variables significantly associated with DSS were identified with multivariate Cox regression and backwards stepwise conditional approach and inserted in the model. Points were assigned to each variable based on the formula: Probability of an event at time t=S 0 (t) exp(β 1 x 1 , β 2 x 2 ...) , (β = regression coefficients, x = observed covariate values, S0(t) = survival function). Cumulative risk points were assigned to each patient and three distinct risk categories for DSS were constructed. Harrel’s C-statistic with bootstrap resampling was used for internal (TC) and external (VC) validation. Model performance was compared to the AJCC 8 th edition (likelihood ratio test -LRT). Results: A total of 1039 patients were included (739 TC, 300 VC). No statistically significant differences in baseline characteristics/survival outcomes were identified between TC and VC. Age (years), T/N stage, radical cystectomy, chemotherapy, and radiation were significant variables inserted in the multivariate model (table). Patients were stratified to three risk groups (low, intermediate, high) based on the total cumulative risk points collected. Median DSS was NR (95%CI; NR, NR), 25 (95% CI; 18-32) and 9 months (95% CI; 7-11) for the low, intermediate, and high-risk category, log-rank p<.001. Harrel's C-statistic for the model was 0.70 (95%CI;0.67-0.73) in the TC and 0.66 (95%CI; 0.61-0.71) in the VC. In comparison, C-statistic for the AJCC 8 th edition was 0.56 (95% CI; 0.52-0.59) and 0.53 (95% CI; 0.51 to 0.57), respectively. The LRT demonstrated superior performance of the model compared to AJCC (p<.001). Conclusions: We present a novel prognostic model for DSS in patients with locoregional SCCB composed of variables readily available in daily practice, which may serve as a useful tool for individualized risk assessment of patients with this rare malignancy. Further validation in prospective studies/clinical trials is warranted. [Table: see text]
