Abstract:ABSTRACT BACKGROUND Despite great advances in assisted reproductive technology, poor ovarian response (POR) is still considered as one of the most challenging tasks in reproductive medicine. OBJECTIVE AND RATIONALE The aim of this systemic review is to evaluate the role of different adjuvant treatment strategies on the probability of pregnancy achievement in poor responders undergoing IVF. Randomized controlled trials (RCTs) comparing 10 adjuvant treatments [testosterone, dehydroepiandrosterone (DHEA), letrozole, recombinant LH, recombinant hCG, oestradiol, clomiphene citrate, progesterone, growth hormone (GH) and coenzyme Q10 (CoQ10)] were included. SEARCH METHODS Relevant studies published in the English language were comprehensively selected using PubMed, Embase and the Cochrane Central Register of Controlled Trials (CENTRAL) until 11 July 2018. We included studies that investigated various adjuvant agents, including androgen and androgen-modulating agents, oestrogen, progesterone, clomiphene citrate, GH and CoQ10, during IVF treatment and reported subsequent pregnancy outcomes. The administration of GnRH analogs and gonadotrophins without adjuvant treatment was set as the control. We measured study quality based on the methodology and categories listed in the Cochrane Collaboration Handbook. This review protocol was registered with PROSPERO (CRD42018086217). OUTCOMES Of the 1124 studies initially identified, 46 trials reporting on 6312 women were included in this systematic review, while 19 trials defining POR using the Bologna criteria reporting 2677 women were included in the network meta-analysis. Compared with controls, DHEA and CoQ10 treatments resulted in a significantly higher chance of clinical pregnancy [odds ratio (OR) 2.46, 95% CI 1.16 to 5.23; 2.22, 1.08–4.58, respectively]. With regard to the number of retrieved oocytes, HCG, oestradiol and GH treatments had the highest number of oocytes retrieved [weighted mean difference (WMD) 2.08, 0.72 to 3.44; 2.02, 0.23 to 3.81; 1.72, 0.98 to 2.46, compared with controls, respectively]. With regard to the number of embryos transferred, testosterone and GH treatment led to the highest number of embryos transferred (WMD 0.72, 0.11 to 1.33; 0.67, 0.43 to 0.92; compared with controls, respectively). Moreover, GH resulted in the highest oestradiol level on the HCG day (WMD 797.63, 466.45 to 1128.81, compared with controls). Clomiphene citrate, letrozole and GH groups used the lowest dosages of gonadotrophins for ovarian stimulation (WMD 1760.00, −2890.55 to −629.45; −1110.17, −1753.37 to −466.96; −875.91, −1433.29 to −282.52; compared with controls, respectively). CoQ10 led to the lowest global cancelation rate (OR 0.33, 0.15 to 0.74, compared with controls). WIDER IMPLICATIONS For patients with POR, controlled ovarian stimulation protocols using adjuvant treatment with DHEA, CoQ10 and GH showed better clinical outcomes in terms of achieving pregnancy, and a lower dosage of gonadotrophin required for ovulation induction. Furthermore, high-level RCT studies using uniform standards for POR need to be incorporated into future meta-analyses.

Abstract:By comparing standard antagonist regimen and agonist-antagonist protocol (AAP regimen), a combination of a microdose flare-up GnRH agonist with a GnRH antagonist in POSEIDON group 3 and group 4 patients with diminished ovarian reserve, this article aims to study if AAP regimen could improve the clinical outcomes in low prognosis patients. This is a retrospective study. The clinical data of 646 cycles of prospective poor ovarian response POR patients (POSEIDON group 3 and 4) who received in vitro fertilization and embryo transfer (IVF-ET) in Peking University Third Hospital reproductive medical center from January 2016 to May 2018 were retrospectively analyzed. The total number of APP cycle was 323, and the control group was selected from the database with 1:1 matching of prospective low prognosis patients (POSEIDON group 3 and group 4) with similar age and approaching date of oocyte retrieval. Patients' general information, ovarian hyperstimulation indicators and clinical outcomes were studied. AAP group had fewer antral follicle count (3.04±2.05 vs. 3.84±2.17, p<0.05) and similar AMH level (0.62±0.64 and 0.63±0.49, p>0.05) compared with control group. AAP group had shorter (9.84±2.59 vs. 10.31±2.23, p=0.015) and lower dosage (2754.18±973.37 vs. 3246.7±1044.20, p<0.05) of Gn using, and had similar number of oocytes obtained compared with control group (4.06±2.89 vs. 4.16±2.65, p=0.649). Under the same proportion of fertilization schemes (routine or ICSI methods), AAP group had higher fertilization rate (74.1% vs. 69.1%, p=0.004) and good quality embryo rate (62.6% vs. 56.9%, p=0.014), and ultimately had higher embryo implantation rate (22.3% vs. 15.8%, p=0.020) and cumulative clinical pregnancy rate (32.5% vs. 22.9%, p=0.018). For POSEIDON patients with low prognosis and poor ovarian reserve, controlled ovarian hyperstimulation with agonist-antagonist protocol had better clinical outcomes compared with conventional antagonist regimen.

Abstract:Spontaneous free perforation is an uncommon event in the natural history of Crohn's disease. It occurred in 21 of 1415 patients (1.5%) admitted with Crohn's disease to The Mount Sinai Hospital between 1960 and 1983. The mean duration from onset of Crohn's disease to occurrence of perforation was 3.3 years. Ten patients had small bowel perforation, ten patients had large bowel perforation, and one patient had simultaneous perforation of both ileum and cecum. The incidence of perforation in disease segments of small bowel was 1.0% (jejunum 6.0%, ileum 0.7%), and in the colon, 1.3%. Besides the 21 patients with spontaneous free perforation, an additional nine patients had spontaneous free rupture of an abscess into the peritoneal cavity. The mean duration from onset of Crohn's disease to rupture of abscess was 8.5 years. All 30 patients had surgery within 24 hours of perforation or rupture. All 21 patients with spontaneous free perforations survived, as did all but one of the nine patients with perforated abscess. The cornerstone of the treatment of ileocolonic lesions perforating into the general peritoneal cavity is proximal diversion with delayed reconstruction of intestinal continuity whenever possible. With perforation of the small bowel, primary reanastomosis is possible in selected patients.

Abstract:Objective The primary objective of the study was to assess traditional Chinese formula DKP supplementation in terms of efficacy and safety on reproductive outcomes of expected poor ovarian responder (POR, POSEIDON Group 4) undergoing in vitro fertilization-embryo transfer (IVF-ET). Design, Setting, and Participants Women eligible for IVF-ET were invited to participate in this randomized, double-blind, placebo-controlled, superiority trial at academic fertility centers of ten public hospitals in Chinese Mainland. A total of 462 patients (35–44 years) equally divided between DKP and placebo groups with antral follicle count (AFC) <5 or anti-müllerian hormone (AMH) <1.2 ng/ml were randomized. Interventions All participants were given DKP or 7 g placebo twice daily on the previous menstrual cycle day 5 until oocyte retrieval, which took approximately 5 to 6 weeks. Main Outcome Measure The primary outcome was the ongoing pregnancy defined as more than 20 gestational weeks of an intrauterine living fetus confirmed by pelvic ultrasonography. Results Demographic characteristics were equally distributed between the study populations. Intention-to-treat (ITT) analysis revealed that ongoing pregnancy rate (OPR) was not significantly different between DKP and placebo groups [26.4% (61/231) versus 24.2% (56/231); relative risk (RR) 1.09, 95% confidence interval (CI) 0.80 to 1.49, P = 0.593]. No significant differences between groups were observed for the secondary outcomes. The additional per protocol (PP) analysis was in line with ITT results: OPR in DKP group was 27.2% (61/224) versus 24.1% (55/228) in placebo group [RR 1.13, 95%CI (0.82 to 1.55), P = 0.449]. After subgroup analysis the findings concluded that POR population of 35–37 years had a significantly higher OPR after 5–6 weeks of oral DKP (41.8%, 33/79) versus placebo (25.4%, 18/71) [RR 1.65, 95% CI (1.02 to 2.65), P = 0.034, P for interaction = 0.028]. Conclusion This well-designed randomized controlled trial (RCT) offers new high-quality evidence to supplement existing retrospective literature concerning DKP performance in expected PORs. DKP could be recommended as a safe and natural remedy for expected PORs (aged 35–37 years) who fulfill the POSEIDON group 4 criteria. However, additional interventional clinical studies are undoubtedly required to be conducted in the future to validate this hypothesis. Efficacy of supplemented Er-xian decoction combined with acupoint application for poor ovarian response.

Abstract:This study aims to observe the efficacy of supplemented Er-xian decoction combined with acupoint application in treating poor ovarian response (POR). This study was a randomized controlled trial. A total of 80 patients, who were treated in the Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine from January 2016 to December 2017, were divided into two groups by tables of random numbers: experimental group (n = 40), and control group (n = 40). In the experimental group, patients orally received supplemented Er-xian decoction with acupoint application. In the control group, a Kuntai capsule was administered according to the course of treatment. The therapeutic effects in the two groups were observed and compared. In the experimental group, the total effective rate was 90%, the cure rate was 15% (six patients), the markedly effective rate was 35% (14 patients), the effective rate was 40% (16 patients), and the ineffective rate was 10% (four patients). In the control group, the total effective rate was 50%, the cure rate was 5% (two patients), the markedly effective rate was 15% (six patients), the effective rate was 30% (12 patients), and the ineffective rate was 50% (20 patients). The differences were statistically significant (P < 0.05). Definite efficacy was observed when a poor ovarian response was treated by supplemented Er-xian decoction combined with acupoint application. Improvements in perimenopausal symptoms, menstruation conditions, hormone levels, inhibin B (INHB), and antral follicle count (AFC) were markedly better in the experimental group than in the control group. In addition, the treatment was safe and had few side effects.
Moxibustion ameliorates ovarian function in premature ovarian insufficiency rats by activating cAMP/PKA/CREB to promote steroidogenesis in ovarian granulosa cells

Abstract:Premature ovarian insufficiency (POI) presents a substantial challenge to women's physiological and psychological well-being. Hormone replacement therapy, as the preferred therapeutic approach, involves solely exogenous supplementation of estrogen. Moxibustion, a traditional Chinese external treatment, has been investigated in our previous studies. It not only improves hormone levels and clinical symptoms in POI patients but also safeguards ovarian reserve. This study aims to explore the regulatory mechanisms by which moxibustion modulates hormone levels and restores ovarian function in POI. A POI rat model was established using cyclophosphamide, and moxibustion treatment was applied at acupoints "CV4" and "SP6" for a total of four courses. Subsequently, ovaries from each group were subjected to transcriptome sequencing (Bulk RNA-seq). Target pathways and key genes were selected through enrichment analysis and GSVA scoring, with validation using various techniques including electron microscopy, ELISA, Western blot, and immunohistochemistry. The results demonstrated that moxibustion restored the estrous cycle in POI rats, improved sex hormone levels, reduced the number of atretic follicles, and increased the count of dominant follicles (P<0.05). Bulk RNA-seq analysis revealed that moxibustion downregulated pathways associated with ovarian dysfunction, infertility, and immune responses, upregulated pathways related to follicular development and ovarian steroidogenesis. Furthermore, our data confirmed that moxibustion significantly increased the number of ovarian granulosa cells (GCs) and upregulated the expression of proteins related to steroidogenesis in GCs, including FSHR, P450 arom, cAMP, PKA, and CREB (P<0.05), with no significant effect observed on proteins related to steroidogenesis in theca cells. These outcomes aligned with the RNA-seq results. In conclusion, these findings propose that moxibustion enhances steroidogenesis in GCs through the activation of the cAMP/PKA/CREB pathway, consequently improving impaired ovarian function in POI rats. This study provides robust evidence supporting moxibustion as a targeted intervention for treating POI by specifically regulating steroidogenesis in GCs.
Effects of Different Pretreatments Prior to GnRH Ultrashort Agonist Combined with Antagonist Regimen on the Clinical Outcomes of Expected POR Patients

Abstract:To analyze the effect of different pretreatments on the IVF outcome of expected POR patients according to POSEIDON criteria, and to explore the appropriate treatment for POR patients. Retrospective study. Analysis of the clinical data of 364 cycles of expected POR patients who received IVF-ET in the Reproductive Medicine Center of the Peking University Third Hospital from January 2016 to May 2018.According to the pretreatment prior to IVF, the cycles were divided into oral contraceptive（OCP） group (A group, N = 167), estradiol valvate group (B group, N = 56) and no pretreatment group (C group, N = 141).The clinical data,ovarian stimulation indexes ,laboratory status and clinical pregnancy rate were compared among the three groups. The age of group A [(34.8+4.9) years old) was significantly younger than that of group B [(38.0+4.9) years old] and group C [(37.9+4.7) years old] (P=0.000).The BMI of group B [(21.9 +3.1) kg/m2] was significantly lower than that of group A [(23.5 +3.6) kg/m2] and group C [(23.2 +3.1) kg/m2] (P=0.014). There were significant differences in the number of antral follicles (AFC) among the group A (2.4±2.0), group B (4.1±1.9), and group C (3.5±2.0) (P=0.000). The proportion of anovulation in group A (32.9%) was significantly higher than that in group B (10.7%) and C (11.3%)( P=0.000). The number of IVF cycles in group C (3.0 + 1.7) was higher than that in group A (2.5 + 1.5)（P=0.017）. There was no significant difference in type of infertility, duration of infertility, basal FSH and AMH among the three groups (P > 0.05). The endometrial thickness on HCG in group A [(9.4 +1.9) mm] was thinner than that in group B [(10.6 +1.5) mm] and group C [(10.1 +2.0) mm] (P=0.000). The fertilization rate of group A (77.1%) and group B (77.6%) was significantly higher than that of group C (71.3%) (P=0.041). There were no significant differences in Gn dosage, days of stimulation, number of eggs obtained, ICSI proportion, cleavage rate, number of embryos available for transfer and number of good quality embryos among the three groups (P > 0.05). The implantation rate and clinical pregnancy rate in group A (26.2%, 36.1%) and group B (26.8%, 42.0%) were significantly higher than those in group C (14.5%, 21.2%) (P=0.014, P=0.014).There were no differences in embryo transfer cycle, number of embryos transferred, proportion of blastocyst transfer cycle, abortion rate and cycle cancellation rate among the three groups (P > 0.05). Pretreatment with OCP or estradiol valerate in luteal phase prior to GnRH ultrashort agonist combined with antagonist regimen can improve the clinical outcomes of patients with expected POR. Estradiol valerate pretreatment in luteal phase seems to be more effective.
Systematic review of acupuncture to improve ovarian function in women with poor ovarian response

Abstract:Objective: To determine the effect of acupuncture in treating poor ovarian response (POR). Methods: We searched MEDLINE (via PubMed), EMBASE, Allied and Complementary Medicine Database, CNKI, CBM, VIP database, Wanfang Database, and relevant registration databases from inception to January 30, 2023. In this review, both Chinese and English peer-reviewed literature were included. Only randomized controlled trials (RCTs) using acupuncture as an intervention for POR patients undergoing in vitro fertilization were considered. Results: Seven clinical randomized controlled trials (RCTs) were eventually included for comparison (516 women). The quality of included studies was generally low or very low. For the meta-analysis, seven studies showed that compared with controlled ovarian hyperstimulation (COH) therapy, acupuncture combined with COH therapy could significantly increase the implantation rate (RR=2.13, 95%CI [1.08, 4.21], p =0.03), the number of oocytes retrieved (MD=1.02, 95%CI [0.72, 1.32], p <0.00001), the thickness of endometrium (MD=0.54, 95%CI [0.13, 0.96], p =0.01), and the antral follicle count (MD=1.52, 95%CI [1.08, 1.95], p <0.00001), reduce follicle-stimulating hormone (FSH) levels (MD=-1.52, 95%CI [-2.41, -0.62], p =0.0009) and improve estradiol (E 2 ) levels (MD=1667.80, 95%CI [1578.29, 1757.31], p <0.00001). Besides, there were significant differences in the duration of Gn (MD=0.47, 95%CI [-0.00, 0.94], p =0.05) between the two groups. However, no statistical variation was observed in improving clinical pregnancy rate (CPR), fertilization rate, high-quality embryo rate, luteinizing hormone (LH) value, anti-mullerian hormone (AMH) value, or reducing the dose of gonadotropin (Gn) values between the acupuncture plus COH therapy group and the COH therapy group. Conclusion: Acupuncture combined with COH therapy is doubtful in improving the pregnancy outcome of POR patients. Secondly, acupuncture can also improve the sex hormone level of POR women, and improve ovarian function. Furthermore, more RCTs of acupuncture in POR are needed to be incorporated into future meta-analyses. Systematic review registration: PROSPERO, identifier CRD42020169560.

There were significant differences in the antral follicle count (AFC) among group A (2.4±2.0), group B (4.1±1.9) and group C (3.5±2.0) ( P<0.001). The proportion of anovulation in group A (32.9%) was significantly higher than that in group B (10.7%) and group C (11.3%) ( P<0.001). The number of IVF cycles in group C (3.0±1.7) was higher than that in group A (2.5±1.5)（ P=0.017）. The fertilization rate of group A (77.1%) and group B (77.6%) was significantly higher than that of group C (71.3%) ( P=0.041). There were no significant differences in Gn used dosage, duration of Gn stimulation, number of eggs obtained, intracytoplasmic sperm injection (ICSI) proportion, cleavage rate, number of embryos available for transfer and number of high-quality embryos among the three groups ( P>0.05). The implantation rate and the clinical pregnancy rate in group A (26.2%, 36.1%) and group B (26.8%, 42.0%) were significantly higher than those in group C (14.5%, 21.2%) ( P=0.014). Abstract:Refractory infertility is a relatively intractable issue regarding its treatment at present,In recent years,acupuncture-moxibustion therapy participate increasingly in treatment of refractory infertility and has achieved a certain therapeutic effect.It is discovered that acupuncture-moxibustion effectively improves ovarian function,promotes ovulation and improves ovum quality and endomentrial receptivity.Hence,this therapy has been widely applied to the treatment of infertility induced by primary ovarian insufficiency (POI),polycystic ovarian syndrome (PCOS),endometriosis (EM),intrauterine adhesion (IUA),etc..In the paper,the research status of this disease treated with acupuncture-moxibustion is reviewed.It is anticipated to determine the direction and lay the foundation for further research.

Abstract:OBJECTIVE To observe the effects of moxibustion preconditioning on ovarian function, fertility and ovarian granulosa cell apoptosis in rats with premature ovarian insufficiency (POI), so as to investigate its underlying mechanism in improving POI. METHODS Forty-two female SD rats with two complete estrous cycles were randomly divided into control group, model group and pre-moxibustion group, with 14 rats in each group. The pre-moxibustion group was pretreated with mild moxibustion for 14 days before POI model establishment at 1) "Guanyuan" (CV4) and "Zhongwan" (CV12) and 2) bilateral "Shenshu" (BL23) as two sets of acupoints on alternate days, once each day, for 10 min each acupoint. After 14-day mild moxibustion intervention, 75 mg·kg-1·d-1 tripterygium glycoside tablet suspension was administered to rats in the pre-moxibustion group and the model group by gavage, for 14 consecutive days, while equivalent saline was given to rats in the control group in the same way. After modeling, the effect of moxibustion preconditioning on ovarian reserve function was evaluated by the estrous cycles, pregnancy rate and embryo number, morphological changes of ovaries, and serum sex hormone levels. TUNEL staining was used to detect the rate of granulosa cell apoptosis in ovaries. Immunohistochemistry and real time quantitative PCR were used to detect the relative expression of Caspase-3 and Caspase-9 proteins and mRNA levels in ovaries. RESULTS Compared with the control group, the estrous cycles were disturbed; the pregnancy rate and number of embryos, the wet weight of ovary and ovarian index, the number of total follicles and different level of follicles, serum Estradiol (E2) and anti-mullerian hormone (AMH) levels were all significantly decreased (P<0.01,P<0.05), while the number of atretic follicles, serum follicule-stimulating hormone (FSH) and luteinizing hormone (LH) levels, the number of TUNEL-positive granulosa cells, the expression of ovarian Caspase-3 and Caspase-9 proteins and mRNAs were significantly increased (P<0.01) in the model group. Compared with the model group, the disordered estrous cycles were improved; the pregnancy rate, the embryo numbers, the wet weight of ovary, and the total follicle number and primary follicle number, serum AMH level were significantly increased (P<0.01，P<0.05), while the number of atretic follicles, serum FSH level, the number of TUNEL-positive granulosa cells, expression of ovarian Caspase-3 and Caspase-9 proteins and mRNAs were all significantly decreased (P<0.01,P<0.05) in the moxibustion group. CONCLUSION Moxibustion preconditioning could improve ovarian function and improve fertility of POI rats, which may be associated with reducing the apoptosis of ovarian granulosa cells.

A Novel Stimulation Protocol for Poor-Responder Patients: Combining the Stop GnRH-ag Protocol with Letrozole Priming and Multiple-Dose GnRH-ant: A Proof of Concept

Abstract:Objective: The objective of this study was to examine whether the combined Stop GnRH-agonist (GnRH-ag), letrozole priming, and multiple-dose GnRH-antagonist (GnRH-ant) protocol may improve in vitro fertilization/intracytoplasmic sperm injection cycle in poor ovarian responders (PORs). Design: This was a historical cohort, proof of concept study under tertiary setting at University affiliated Medical Center. Patients: Five PORs fulfilling the POSEIDON Group 4 criteria were included. Main outcome measures: Number of oocytes retrieved, number of top-quality embryos (TQEs), and controlled ovarian hyperstimulation (COH) variables were the main outcome measures. Results: The combined Stop GnRH-ag, letrozole priming, and multiple-dose GnRH-ant COH protocol revealed significantly higher number of follicles >13 mm on the day of hCG administration and higher number of oocytes retrieved, with non-significantly more TQEs and a reasonable clinical pregnancy rate. Conclusions: The combined Stop GnRH-ag, letrozole priming, and multiple-dose GnRH-ant COH protocol is a valuable tool in the armamentarium for treating POSEIDON Group 4 patients. Further large prospective studies are needed to elucidate its role in POR and to identify the specific characteristics of women (before initiating ovarian stimulation) that will aid both fertility specialists' counseling and their patients in adjusting the appropriate COH protocol.

Abstract:OBJECTIVE To observe the effect of moxibustion on Nrf2/HO-1 signaling pathway in rats with diminished ovarian reserve (DOR), and to explore the protective mechanism of moxibustion on ovarian reserve function. METHODS Forty SD rats were randomly divided into a blank group, a model group, a moxibustion group and a hormone group, 10 rats in each group. The rats in the model group, moxibustion group and hormone group were treated with intragastric administration of tripterysium glycosides turbid liquid to prepare DOR model. The rats in the blank group were treated with intragastric administration of sodium chloride solution with the same volume, once a day for 14 days. The rats in the hormone group were treated with hormone sequential therapy for 14 days from the day of modeling; the rats in the moxibustion group were treated with moxibustion at bilateral "Shenshu" (BL 23) or "Guanyuan" (CV 4) and "Zhongwan" (CV 12) from the day of modeling, and the two groups acupoints were alternated every other day, 10 min each time, for 14 consecutive days. The estrus cycle was observed every day by vaginal exfoliated cell smear, and the estrus cycle disorder rate in each group was calculated. After the intervention, the HE staining was used to observe the histological morphology of ovaries; ELISA was used to detect the contents of follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), anti Mullerian hormone (AMH), superoxide dismutase (SOD) and malondialdehyde (MDA); the protein levels of Nrf2 and HO-1 in ovarian tissue were detected by immunohistochemistry; real-time PCR (TaqMan probe method) was used to detect the expression of Nrf2 and HO-1 mRNA. RESULTS Compared with the blank group, the rate of estrus cycle disorder in the model group was increased (P<0.01); compared with the model group, the rate of estrus cycle disorder in the moxibustion group and hormone group was decreased (P<0.01). Compared with the blank group, the serum contents of FSH, LH and MDA in the model group were increased (P<0.01), and the serum contents of E2, AMH and SOD were decreased (P<0.01). Compared with the model group, the serum contents of FSH, LH and MDA in the moxibustion group and hormone group were decreased (P<0.01, P<0.05), and the serum contents of E2, AMH and SOD were increased (P<0.01). Compared with the blank group, the protein and mRNA expression of Nrf2 and HO-1 in the model group were decreased (P<0.01); compared with the model group, the protein and mRNA expressions of Nrf2 and HO-1 in the moxibustion group and hormone group were increased (P<0.01). CONCLUSION Moxibustion could reduce the rate of estrus cycle disorder, improve the level of serum sex hormones and antioxidant stress in DOR rats, and the mechanism may be related to the regulation of Nrf2/HO-1 signaling pathway.

Abstract:OBJECTIVE: Diminished ovarian reserve (DOR) refers to the decreased number and quality of oocytes in the ovary. Acupuncture and moxibustion has a certain effect on DOR; however, the number of studies and reports of research evidence are limited. This study aimed to conduct a scoping review of the clinical research status of acupuncture and moxibustion for treating patients with DOR.METHOD: PubMed, Cochrane Library, Excerpta Medica database, Allied and Complementary Medicine Database, Chinese Biological Medicine, China National Knowledge Infrastructure, VIP Database for Chinese Technical Periodicals, and Wanfang database were searched from January 2010 to May 2022 using keywords and medical subject heading terms. After applying the inclusion and exclusion criteria, relevant studies were selected. Structured tables and descriptive charts were made to visually express research features by using Excel, Original, IBM SPSS Model 18.0, Adobe Illustrator and other software packages. Report quality was evaluated for Cochrane bias using Review Manager 5.3.RESULTS: Overall, 851 studies were identified; of these, 90 met the inclusion criteria. The results extracted from these studies were classified into four categories: research characteristics, study type, acupuncture and moxibustion prescriptions, and efficacy observation.CONCLUSIONS: The quality assessment of acupuncture and moxibustion for DOR is not ideal. Therefore, standardisation and normalisation should be strengthened, and high-quality evidence is needed to further demonstrate the effectiveness of this approach. Due to heterogeneity in DOR diagnosis, the observation index should be updated with reference to the latest research to improve efficacy evaluation.

Abstract:A novel modified progestin primed ovarian stimulation (mPPOS) protocol (administrating medroxyprogesterone acetate (MPA) in the late follicular phase) has been described to adequately prevent premature ovulation and yield comparable number of retrieved and vitrified oocytes in oocyte donor cycle. The aim of the study was to test the hypothesis that a low-cost regimen of mPPOS is similarly effective or only slightly worse than GnRH-antagonist regimen in predicted poor ovarian responders. A prospective cohort study associated with a cost-effectiveness analysis. Infertile women with less than 10 antral follicles undergoing planned freeze-all cycles were recruited. Ovarian stimulation was started with 150-225IU of gonadotropin on the second day of cycle. Each eligible patient could choose whether to undergo either a single cycle of the mPPOS or GnRH antagonist regimen. MPA (10 mg) or GnRH antagonist (0.25 mg) (mPPOS group/GnRH antagonist group) was given daily when the leading follicle reached 14 mm and continued until the day of ovulation trigger. Viable embryos were cryopreserved for later frozen-thawed embryo transfer in both groups. Multiple linear regressions analysis was performed to search for a correlation between individual characteristics and the number of retrieved oocyte. Primary outcome was the number of oocytes retrieved. Secondary outcomes included the incidence of premature LH surge, duration of stimulation, total gonadotropin use, embryological laboratory outcomes and direct medical costs. We included 53 eligble women who had stimulation cycle performed with either a mPPOS (n=26) or an antagonist (n=27). The number of oocytes retrieved and viable embryos were similar between two groups (5.0±4.0 vs 4.8±3.6; 2.9±2.7 vs 2.4±2.1; P>0.05). No significant differences were observed in the duration of stimulation and total gonadotropin consumption. No premature LH surge occurred in either group. Only female age (p = 0.001) was associated with the number of retrieved oocytes in the multiple regression model. In the cost-effectiveness analysis, the mPPOS protocol was strongly dominant over the antagonist protocol. Progestin used later is feasible and effective in ovarian stimulation for predicted poor ovarian responders without intent to fresh embryo transfer. Current GnRH antagonist and mPPOS protocols were associated with similar results but the economic analysis was in favor of latter one. There was no statistically significant difference might be due to the sampling size. Large randomised controlled trials are needed to further confirm.
The correlation of acupuncture and moxibustion points for the treatment of diminished ovarian reserve and premature ovarian failure: A systematic review based on complex network

Abstract:Background: Infertility rates have risen in recent years, with diminished ovarian reserve (DOR) affecting 10% of infertile women, accounting for approximately 20% of ovarian disorders. This highlights the importance of exploring effective treatments for DOR. This study investigates acupoint compatibility rules in acupuncture and moxibustion therapies for DOR and premature ovarian failure (POF) using complex network analysis. It also provides insights into acupuncture and moxibustion approaches for managing these conditions. Methods: Clinical studies on acupuncture and moxibustion treatments for DOR and POF were retrieved from 5 databases, including China National Knowledge Infrastructure, SinoMed, Wanfang, CQVIP, and PubMed, covering the period from January 1, 2007, to September 15, 2024. Literature was screened based on inclusion and exclusion criteria to build a comprehensive database of acupuncture and moxibustion prescriptions for these conditions. Association rule analysis was performed using IBM SPSS Modeler 18.0, and complex network analysis was conducted with Gephi 0.9.2 software. Results: A total of 70 articles and 67 acupoints from 70 prescriptions were analyzed, with 665 occurrences of these acupoints. The most frequently used acupoint was Guanyuan (RN4), followed by Sanyinjiao (SP6), Zusanli (ST36), Shenque (RN8), Bladder 23 (BL23), and Zigong (EX-CA1). These acupoints were primarily related to the Ren (RN) and Bladder (BL) meridians. Jiaohui points, dorsal Yu points, and 5 Shu points were the most commonly utilized specific points. The combination of Sanyinjiao (SP6) and Guanyuan (RN4) had the strongest association. Complex network analysis revealed a small-world network effect, with 24 core acupoints, including Guanyuan (RN4) and Zusanli (ST36), serving as key nodes. Conclusion: Acupuncture treatments for DOR and POF primarily focus on the abdomen and lower back regions. Specific acupoints, such as rendezvous points, are frequently combined with distantly located points. These combinations are guided by therapeutic principles aimed at nourishing the kidney, strengthening the spleen, regulating liver qi, calming the heart, and promoting blood circulation.

Abstract:Management of women with reduced ovarian reserve or poor ovarian response (POR) to stimulation is one of the major challenges in reproductive medicine. The primary causes of POR remain elusive and oxidative stress was proposed as one of the important contributors. It has been suggested that focus on the specific subpopulations within heterogeneous group of poor responders could assist in evaluating optimal management strategies for these patients. This study investigated the effect of anti-oxidant treatment with coenzyme Q10 (CoQ10) on ovarian response and embryo quality in young low-prognosis patients with POR.
