Abstract:Background Inflammatory bowel disease (IBD) may impair patients‘ health-related quality of life (HRQOL) and impose burdens on caregivers. We aim to survey HRQOL including demographic characteristics of IBD patients and to evaluate the disease-related perception of their caregivers in East China. Methods Patients above 18 years old with established Crohn’s disease (CD) or ulcerative colitis (UC) from 5 medical centres in East China between December 2016 and July 2017 were enrolled. Patients‘ detailed demographic and clinical information were documented. Demographic data of the investigated regions were extracted from China Population and Employment Statistics Yearbook 2016 for comparison. Patients‘ HRQOL was assessed by short IBD questionnaire (SIBDQ) and patient-reported 0–10 score (IBD-10). The major caregiver of each patient was surveyed by IBD-10 to estimate patient‘s condition from caregiver’s viewpoint. Results A total of 601 IBD patients were included in this study. The proportion of post-secondary school education (51.4% vs 28.1%, p<0.0001) and the unemployment rate (31.8% vs 2.9%–4.0%, p<0.0001) was significantly higher in IBD cohort than in general population. Patients in active phase had remarkably lower SIBDQ and IBD-10 scores than those in remission, meanwhile, the scores declined along with the illness severity (table 1). Long disease duration, illness activity, unemployment and corticosteroid treatment were risk factors for poor HRQOL while high BMI provided a protective effect. There was a strong positive correlation between IBD-10 scores of patients and their caregivers (figure 1). Conclusions Higher educational attainment and unemployment rate exist among IBD patients in East China. Patients‘ HRQOL declines and correlates with multiple negative factors. Caregivers may form similar disease-related perception just as patients do.

Abstract:Abstract Background Self-care is an active process to maintain and promote health in chronic disease situations, and is effective in the results of many chronic diseases. So far, few studies have been conducted on the self-care of IBD. The study of self-care in IBD is also motivate, by the increase in prevalence in the coming years and the impact on health resources it could have. In addition, the study of self-care in relation to socio-sanitary features and habits within the population affected by IBD may be the beginning to identify areas for educational interventions by health care personnel. The primary objective of this study is to describe the levels of self-care maintenance, monitoring and management in patients with IBD. Methods Cross-sectional observational study to evaluate self-care in IBD patient. The questionnaire used was the Self-Care of Chronic Disease Index (SCCII). The SCCII investigates three aspects of self-care, maintenance, monitoring and management and the self-efficacy. The sociodemographic characteristics and some patient habits such as smoking habit, fatigue, exercise, sleep, nutrition and supplement use, were investigated. Ethics Committee approved Results We enrolled 85 patients with an average age of 44.8±16.8, of which 51.8% (female) and 50.6% (CD), 48.4% (UC). The level of self-care (SCCII) areas investigated ranged from score levels of 71.30 (Self-maintenance), 79.52 (self-monitoring), 68.94 (self-management) and 75.70 (self-efficacy). In the area of self-care maintenance the aspects that seem least considered by patients (never or rarely) are stress management (44.1%) and exercise (40%). Only 57.6% of patients often or always get enough sleep and only 22.3% of patients follow a healthy diet. The presence of fatigue seems to affect 68% of patients in the self-monitoring phase. In the area of self-management, only 41.2% of patients (often or always) try to rest during the day and only 52% of the sample succeeds continuously with the appearance of symptoms. In self-efficacy, it seems that 85.8% of the patients interviewed persist in following the treatment even when it is difficult. It seems that men have more significant levels of self-maintenance score 74. vs 68.8 (p>0.01) than women. Table 1 shows the average self-care scores in relation to the habits investigated. Conclusion From the results of our study it seems that patients with IBD have adequate levels of self-care (score ≥70), although sleep/rest, diet and exercise seem not to be highly considered by patients. Future studies may investigate which specific sociodemographic and clinical characteristics can be identified as predictors of maintaining self-care, self-care management and self-confidence, to improve areas of nurse educational intervention.

Abstract:Introduction Treatment non-adherence is common in young people with inflammatory bowel disease (IBD), yet support is lacking. A self-led self-management intervention supporting teens with IBD (ASSIST-IBD) is a new theory-based digital treatment adherence intervention, co-developed by young people living with IBD. ASSIST-IBD includes 10 short modules supporting adolescents to feel confident to follow their treatment plan, develop skills to overcome adherence obstacles, feel confident when talking to others about IBD and feel positive about the future. This research aims to determine the feasibility of implementing and measuring the effectiveness of ASSIST-IBD, using a single-arm mixed-methods feasibility trial. Methods and analysis 24 young people (aged 13–17) with IBD identified as being ≤80% adherent, and their parents, will use ASSIST-IBD for 6–12 weeks. For the primary endpoint of progression to randomised controlled trial, qualitative and quantitative data will be collected on; number of eligible members of the target population; number of recruited participants; reasons for non-participation and ineligibility; retention and follow-up rates; reasons for early withdrawal; completeness and utility of outcome measures; as well as further data on intervention acceptability, user experiences and user engagement. Secondary outcomes of preliminary effectiveness will include pre-intervention and post-intervention measures of treatment adherence (MARS-5), quality-of-life (IMPACT-III) and well-being (WEMWBS), and self-reported behaviour change success. Quantitative data will be analysed using descriptive statistics; qualitative data will be analysed thematically. An active patient and public involvement and engagement group will advise on the research throughout, including the development of the protocol. Ethics and dissemination The study has been granted ethical approval by Aston University's Health and Life Sciences Research Ethics Committee (ref:#HLS2112) and NHS Research Ethics Committee, Nottingham 1 Board (IRAS:#344918). Findings will be disseminated via peer-reviewed publications and lay summaries. Abstract:Introduction Digital health tools can be beneficial in the care of patients with chronic conditions and have the potential for widespread impact as readily scalable and cost-effective health interventions. However, benefits are often contingent on users sustaining their engagement with these tools over time. Sustained engagement with digital health tools can be challenging, and high rates of attrition from digital interventions are common. Inflammatory Bowel Disease (IBD) and irritable bowel syndrome (IBS) are prominent gastrointestinal conditions resulting in significant burdens for individuals and society. Emerging evidence suggests digital health tools can be beneficial for IBD and IBS management; however, it is not clear what barriers and enablers are experienced by people living with these conditions to sustaining their engagement with these tools, when necessary. Such knowledge could inform the tailoring of new and existing digital health tools to the needs of people living with IBD and/or IBS. This study will seek to identify the barriers and enablers of sustained engagement with digital health tools among adults living with IBD and/or IBS. Methods and analysis We will conduct semistructured interviews with a purposive sample of approximately 30 adults (>18 years) who (a) reside in Canada, (b) self-report that they have been diagnosed with IBD and/or IBS, (c) have ever used a digital health tool (ie, any application/platform) to manage their condition and (d) are capable of providing informed consent. Interviews will be audio and video recorded and transcribed verbatim. Data will be coded deductively and barriers and enablers to sustained engagement will be categorised in accordance with the Theoretical Domains Framework. Data analysis will be verified by a patient research partner. Ethics and dissemination The study has been approved by the Ottawa Health Science Network Research Ethics Board. The findings will inform the codevelopment of strategies to overcome modifiable barriers and leverage identified enablers of sustained engagement with digital health tools for IBD and IBS care. These strategies can inform the design of new, or modifications to existing, digital health tools for IBD and IBS care where sustained engagement is desirable. Strategies will be compiled into a guidebook and disseminated via the Inflammation, Microbiome and Alimentation: Gastro-Intestinal and Neuropsychiatric Effects (IMAGINE) Strategy for Patient Oriented Research chronic disease network in Canada.

Abstract:Background: Despite being in clinical remission, many people with inflammatory bowel disease (IBD) live with fatigue, chronic abdominal pain and bowel urgency or incontinence that limit their quality of life. We aim to test the effectiveness of an online self-management programme (BOOST), developed using cognitive behavioural principles and a theoretically informed logic model, and delivered with facilitator support. Primary research question: In people with IBD who report symptoms of fatigue, pain or urgency and express a desire for intervention, does a facilitator-supported tailored (to patient needs) online self-management programme for fatigue, pain and faecal urgency/incontinence improve IBD-related quality of life (measured using the UK-IBDQ) and global rating of symptom relief (0-10 scale) compared with care as usual? Methods: A pragmatic two-arm, parallel group randomised controlled trial (RCT), of a 12-session facilitator-supported online cognitive behavioural self-management programme versus care as usual to manage symptoms of fatigue, pain and faecal urgency/incontinence in IBD. Patients will be recruited through a previous large-scale survey of unselected people with inflammatory bowel disease. The UK Inflammatory Bowel Disease Questionnaire and global rating of symptom relief at 6 months are the co-primary outcomes, with multiple secondary outcomes measured also at 6 and 12 months post randomisation to assess maintenance. The RCT has an embedded pilot study, health economics evaluation and process evaluation. We will randomise 680 patients, 340 in each group. Demographic characteristics and outcome measures will be presented for both study groups at baseline. The UK-IBDQ and global rating of symptom relief at 6 and 12 months post randomisation will be compared between the study groups. Discussion: The BOOST online self-management programme for people with IBD-related symptoms of fatigue, pain and urgency has been designed to be easily scalable and implemented. If it is shown to improve patients' quality of life, this trial will enable clinicians and patients to make informed management decisions. This is the first trial, to our knowledge, focused on multiple symptoms prioritised by both people with IBD and health professionals. Trial registration: ISRCTN71618461 . Abstract:PURPOSE To examine the influence of two patellofemoral braces on pain response, patellar alignment, and patellofemoral joint contact area in persons with patellofemoral pain. METHODS Fifteen women between the ages of 18 and 45 yr with a diagnosis of patellofemoral pain participated. After the assessment of pain response using a visual analog scale, subjects underwent axial plane magnetic resonance imaging of patellofemoral joint at 0 degrees, 20 degrees, 40 degrees, and 60 degrees of knee flexion. Imaging was done with the knee extensors contracted (25% body weight) under three conditions: 1) no brace, 2) On-Track brace, and 3) Patellar Tracking Orthosis (PTO). Measures of mediolateral patellar displacement and tilt and medial and lateral facet contact area were obtained from the magnetic resonance images. RESULTS On average, the On-Track brace reduced symptoms by 50%, whereas the PTO reduced pain by 44%. When averaged across all knee flexion angles, the PTO and the On-Track brace significantly increased total patellofemoral joint contact area by 52.0 mm (21%) and 59.3 mm (24%), respectively, when compared with the no-brace condition. Bracing had no influence on lateral patellar tilt; however, small but significant changes in lateral patellar displacement were observed. CONCLUSION Large changes in pain and contact area occurred without sizable changes in patellar alignment. The results of this study suggest that changes in patellar alignment by itself may not be responsible for pain alleviation after patellar bracing.

Abstract:The Tat regulatory protein encoded by human immunodeficiency virus type 1 (HIV-1) induces high levels of transcription from the viral long terminal repeat (LTR) promoter element after interacting with a promoter proximal RNA target sequence. In the wild-type HIV-1 LTR, this activation is facilitated by the synergistic interaction of Tat with the NF-kappa B and, particularly, SP1 regulatory proteins that bind to DNA sequences within the LTR promoter element. Using a synthetic Tat responsive indicator construct, we here demonstrate that NF-kappa B and SP1 are not uniquely or even unusually competent to synergize with HIV-1 Tat. Instead, these proteins can be functionally replaced by several, but not all, of the heterologous cellular and viral transcriptional activators tested. Tat therefore shares the ability to functionally synergize with a range of transcriptional activators, which is characteristic of DNA-sequence-specific regulatory proteins.

Abstract:The combined effect of partial band jamming and imperfect channel estimation on the analysis of the performance of MC-FH/BFSK receiver over a Rayleigh fading channel is often neglected in the literature. In this study, this missing analysis is provided. The authors derive closed form bit error probability (BEP) expressions for studying the effect of partial band and broad-band jamming on the performance of a coherent uncoded MC-FH/BFSK receiver over a Rayleigh fading channel where the effect of channel estimation errors is not neglected. The considered model of the channel estimation errors does not assume independency between amplitude and phase estimates. Computer simulations are provided to validate the theoretical developments. It is shown that the smart partial band jammer should be present in all sub-bands of the system in order to be effective.

Abstract:Abstract Background Inflammatory Bowel Disease (IBD) management requires a multidisciplinary team, in which nurses have a pivotal role. Although patient care is the main focus of all clinical actions, satisfaction with healthcare professionals is an aspect of growing importance that has been related to positive clinical outcomes. In this setting, evaluating the satisfaction of the nursing provided could be a pivotal component for the satisfaction of the patient, particularly in the IBD unit, where patients in intravenous (iv) biological therapy spend a major amount of time compared to non-complicated acute hospitalization. The aim of the study is to measure the overall quality of nursing care perceived by IBD patients receiving biological therapy. Furthermore, the opinion of loyal and non-loyal patients was measured at the IBD center as a secondary objective Methods Cross-sectional observational study to assess nursing satisfaction through the Newcastel Satisfaction Nursing Scale (NSNS). The instrument consists of two separate scales: the Scale of Nursing Care Experiences ("A") and the Scale of Nursing Satisfaction ("B"). The patient who is followed by the center, at the time of administering the questionnaire, for a period of more than 12 months, will be considered in the arm long follow-up while all patients treated at the center for a period ranging from 0 to 12 months, in those short follow-up. The study protocol was approved by the Ethics Committee. Results Two hundred and thirty-four in biological therapy, questionnaires were administered, of these, with an excellent response rate, 200 (86.2%). The result was an overall level of satisfaction compared to the first scale "A", equal to 88.4%, while for the second scale "B" the total result was 90.7%. By analyzing the satisfaction of nursing care received, differentiating the patient between short and long follow-up, the results seem to confirm how the patient in the long follow-up positively affects satisfaction, significantly in the first scale "A" 90.5% vs 82.9% p = 0.0002 and in the second scale "B" 92.5% vs 86% p = 0.0001. The lowest average satisfaction scores were recorded in the doctor-nurse ratio and in the communicative aspects, while the highest scores are related to the empathic components and professional competence of nursing. Conclusion In conclusion, the level of satisfaction of patients in biological therapy is high and this seems to increase significantly with time and with patient loyalty to the center (Fig. 1). Further research projects could aim to fill gaps in knowledge and validate scales of assessment of nursing skills in the field of IBD, including by structuring studies with a control cohort.

Abstract:Peromelia, agenesia of the distal parts of the limbs, has been reported as a congenital defect in several animal species. In Angora goats, cases occur in a familiar pattern consistent with an autosomal recessively inherited defect. To obtain further evidence on the inheritance of peromelia in Angora goats, breeding results for goats being daughters of known carriers and which were then mated to a known carrier were analysed. Of 45 kids born in 1993 and 1994, five kids had peromelia. This corresponded to the expected 7:1 segregation. The difference between the number of affected male and female kids was not statistically significant. Peromelia affected kids occurred significantly more frequently among goats selected in the breeding study than among other goats in the respective herds. The study demonstrated that peromelia is inherited as an autosomal recessive defect. Based on knowledge obtained in a previous study, the genotype probably has varying degrees of expressivity.

Abstract:Up to 70% of people diagnosed with upper gastrointestinal (GI) tract or hepato-pancreato-biliary (HPB) cancers experience substantial reductions in quality of life (QoL), including high distress levels, pain, fatigue, sleep disturbances, weight loss and difficulty swallowing. With few advocacy groups and support systems for adults with upper GI or HPB cancers (i.e. pancreas, liver, stomach, bile duct and oesophageal) and their carers, online supportive care programs may represent an alternate cost-effective mechanism to support this patient group and carers. iCare is a self-directed, interactive, online program that provides information, resources, and psychological packages to patients and their carers from the treatment phase of their condition. The inception and development of iCare has been driven by consumers, advocacy groups, government and health professionals. The aims of this study are to determine the feasibility and acceptability of iCare, examine preliminary efficacy on health-related QoL and carer burden at 3- and 6-months post enrolment, and the potential cost-effectiveness of iCare, from health and societal perspectives, for both patients and carers.

Abstract:Introduction: Biologics are effective for the treatment of inflammatory bowel disease (IBD). However, unwarranted variation in processes and outcomes has been reported in the treatment of IBD. A care pathway for the treatment of IBD has the potential to reduce practice variation and improve outcomes. This study aims to compare the effect of a uniform care pathway for the treatment of patients with IBD with biologics to the current situation. Methods and analysis: IBD Value is a longitudinal multicentre non-randomised parallel cluster trial with a baseline period. The study takes place in eight centres in the Netherlands. The baseline period will run for 12 months, after which the care pathway will be implemented in 6 of the 8 participating hospitals during the implementation phase of 3 months. Hereafter, the effect of the care pathway will be assessed for 12 months. Total study period is 27 months. The primary outcome is the effect of the care pathway on disease control (IBD-Control questionnaire). Secondary outcomes are the effect of the care pathway on the other outcomes of the International Consortium of Health Outcomes Measurement IBD standard set, health-related generic quality of life, patient experiences and degree of variation; cost effectiveness of the care pathway; and the variation between hospitals in the aforementioned outcomes in the baseline period. Outcomes will be measured every 6 months. The study started on 1 December 2020 and a minimum of 200 patients will be included. Ethics and dissemination: The study was deemed not to be subject to Dutch law (WMO; Medical Research Involving Human Subjects Act) by the Medical Ethics Committee of the Erasmus MC, the Netherlands (registration number: MEC-2020-075) and a waiver was provided. Abstract:In response to the increasing public awareness of serious dye-contained wastewater contamination, we herein fabricated a novel anthracene-containing hyperbranched poly(ether amine) (hPEA-AN)/carbon nanotube (CNT) ultrathin membrane (UTM), which combined both the merits of the conventional dye adsorption strategy and membrane filtration process, to implement efficient selective adsorption of dye molecules and also the separation of dye mixtures by molecular filtration. Taking advantage of the π-π stacking interactions between anthracene and CNT sidewalls and hydrophobic interactions, CNTs were coated tightly with hPEA-AN to form the hPEA-AN@CNT complex, which can be well-dispersed very stably in water. The formation of the hPEA-AN@CNT complex was confirmed using X-ray photoelectron spectroscopy, Raman spectra, fluorescence spectra, and thermogravimetric analysis. Meanwhile, a simple filtration process was applied to prepare hPEA-AN@CNT UTMs with a thickness of 1.5 μm, which can be further cross-linked through photodimerization of anthracene moieties. The UTMs represented selective adsorption behaviors toward hydrophilic dyes even with similar backbones and the same charge states, namely, they showed high adsorption capacities (Qeq) toward eosin B, erythrosin B (ETB), 4',5'-dibromofluorescein, and Evans blue (EVB) dyes up to 300 μmol/g while showing low adsorption capacities toward calcein (Cal), methyl red, and Ponceau S dyes. On the basis of this unique selective adsorption, molecular filtration was then realized toward mixed ETB/Cal and EVB/Cal dyes, with a separation efficiency of up to 100% and regeneration without an obvious efficiency decrease.
eLIFEwithIBD: study protocol for a randomized controlled trial of an online acceptance and commitment therapy and compassion-based intervention in inflammatory bowel disease

Abstract:Background: Inflammatory bowel disease (IBD) entails physical, psychological, and social burden and holds a significant impact on quality of life. Experiential avoidance, cognitive fusion, shame, and self-criticism have been identified as possible therapeutic targets for improving mental health in people with IBD. Traditional face-to-face psychological therapy continues to provide obstacles for patients seeking assistance. Online psychological therapies centered on acceptance, mindfulness, and compassion have been shown to improve psychological distress in other populations. Objective: This paper presents the study protocol of a two-arm Randomized Controlled Trial (RCT) of an ACT and compassion-based, online intervention - eLIFEwithIBD - on the improvement of psychological distress, quality of life, work and social functioning, IBD symptom perception, illness-related shame, psychological flexibility, and self-compassion. Methods: The eLIFEwithIBD intervention is an adaptation of the LIFEwithIBD programme (delivered through an in-person group format) and entails an ACT, mindfulness, and compassion-based intervention designed to be delivered as an e-health tool for people with IBD. This protocol outlines the structure and contents of the eLIFEwithIBD intervention. Participants were recruited by an advertisement on the social media platforms of Portuguese Associations for IBD in January 2022. A psychologist conducted a brief interview with 80 patients who were interested in participating. Fifty-five participants were selected and randomly assigned to one of two conditions [experimental group (eLIFEwithIBD + medical TAU; n = 37) or control group (medical TAU; n = 18)]. Outcome measurement took place at baseline, post-intervention, and 4-month follow-up. All analyses are planned as intent-to-treat (ITT). Results: The eLIFEwithIBD intervention is expected to empower people with IBD by fostering psychological strategies that promote illness adjustment and well-being and prevent subsequent distress. The eLIFEwithIBD aims to gain a novel and better understanding of the role of online contextual behavioral interventions on improving the quality of life and mental health of people with IBD. Abstract:Abstract Background Inflammatory bowel disease (IBD) includes Crohn's Disease (CD) and Ulcerative Colitis (UC), and both lead to manifestations that affect the patient's quality of life (QoL). The issue of chronicitỳ makes the disease difficult to manage and requires a radical change in patients’ habits, leading to a decrease in QoL after diagnosis. The literature emphasises how spiritualitỳ positively influences the psychophysical well-being of chronic patients and consequently their QoL. This study aims to describe the spiritual needs of patients with IBD to promote holistic care and improve their QoL and aims to explore their eventual association with sociodemographic and clinical variables. Methods A cross-sectional study was carried out in the outpatient gastroenterology clinics of a university hospital, administering the validated Spiritual Needs Questionnaire (SpNQ) during outpatient follow-up visits to patients over 18 years of age after informed consent to participate in the study. The SpNQ consist of 27 items divided into four domains: Religious needs, Needs for Inner Peace, Existential needs, and Giving/Generativity Needs. Some clinical and sociodemographic data were also collected. Results A total of 103 completed the questionnaire: 53% had CD, 35% UC and 12% had undetermined IBD. 44% were males the median age was 45 years, and half of the sample was more than 10 years from IBD diagnosis. More than 40% were in biologics, 47% had a high educational level, 28% were smokers, and 4% had a stoma. 65% were Christian, 26% had no religion, and the frequency of their participation in religious gatherings varies from never (15%), to rarely (59%) and often (26%). They cultivate their spirituality mainly by conducting their existence in accordance with values that are fundamental to them (47.7%), staying in contact with nature (30.5%), praying (15.8%), engaging in introspection and self-knowledge (14.7%), volunteering (12.6%), and practising extreme sports (10.5%). The analyses showed statistically significant differences between sexes regarding the domains "Needs for Inner Peace" and "Religious Needs" (p<0.001) more relevant to women than men, and significant differences between ages regarding the domains "Existential Needs", more relevant to middle age (p=0.037), and "Giving/Generativity Needs", more relevant to middle age and to elderly (p=0.015). Conclusion Spiritual needs may play a major role in the care of patients with IBD, and nursing should consider this dimension to improve the holistic care of the patients. Research should be expanded through studies involving a larger sample size and evaluating the IBD outcome by acting on spiritual needs.

Chronic illness trajectory-based nursing intervention improves the self-care abilities of patients with inflammatory bowel disease

Abstract:Objective: To explore the effects of chronic illness trajectory model (CITM)-based nursing interventions on anxiety, depression, quality of life, medication adherence, and dietary compliance among patients with inflammatory bowel disease (IBD). Methods: A retrospective analysis was performed on 112 IBD patients admitted to Shandong Provincial Hospital Affiliated to Shandong First Medical University from January to December 2023. Patients were divided into two groups: a control group (n=62) receiving routine nursing care, and an observation group (n=50) receiving CITM-based nursing care. Assessments of anxiety, depression, self-care ability, daily living ability, and symptom severity were conducted before and after the intervention. Results: Post-intervention, the observation group demonstrated significantly higher quality of life scores at 1 and 3 months compared to the control group (both P<0.05). Additionally, the observation group showed improved medication adherence and lower symptom scores, with significant differences (both P<0.05). Anxiety and depression levels were also significantly reduced in the observation group compared to the control group (both P<0.05). Conclusion: CITM-based nursing intervention significantly enhances self-care abilities, quality of life, and compliance with medication and dietary regimens in IBD patients. Furthermore, it effectively alleviates anxiety and depression, supporting comprehensive management of this chronic disease.

Abstract:Abstract Background In recent years, given the change in the paradigm of management of Inflammatory Bowel Disease (IBD), the different societies have been working to ensure continuous, homogeneous, multidisciplinary social and health care with the patients and their autonomy as the central axis; promoting the creation of Integrated Care Units for IBD patients (ICU-IBD). In the latest analyses of our ICU, attention was drawn to the progressive increase in the demand for care from non-Spanish-speaking migrant patients. This prompted us to carry out this study, with the aim of finding out how this population uses our unit and how it responds to their needs. Methods A retrospective longitudinal descriptive observational study with analysis of demographic, clinical and care variables of all non-Spanish-speaking migrant patients followed in the ICU-IBD of our centre until 30 June 2023. Results Thirty patients of 14 different nationalities were included, with Moroccan being the most frequent (43.3%); only one of them (3.3%) was fluent in Spanish. Other clinical and demographic characteristics of note were: mean age at follow-up of 43.2 years, predominantly female (66.6%) and predominantly Crohn's disease (63.3%) (Table 1). A total of 86.7% of the patients were referred to our ICU-IBD from the Emergency Department, which they attended due to symptoms related to flare-ups of their intestinal disease. After the initial assessments, we observed that 73.3% did not attend for check-ups, 83.3% of the patients attended the emergency department in the event of a clinical flare-up, and 93.3% showed irregular adherence to treatment. In the subsequent follow-up, we observed a significant change, 76.7% regularly attended consultations, 76.7% consulted the unit in the event of an outbreak and 60% followed treatment regularly (Figure 1). These changes coincide with a socio-health intervention carried out by the specialised nurse; in which patients and their environment were provided with information regarding their disease, treatment and use of the UAI-EII; as well as addressing socio-health problems in order to facilitate their wellbeing and integration into the community. Conclusion In view of our results and the data published in other studies, we suggest that the language barrier, the social and healthcare culture of the country of origin, the socioeconomic situation and integration into the new social environment are factors to be taken into account in order to achieve adequate comprehensive care for this migrant population. The role of the IBD nurse is fundamental in the daily work of offering individualised, continuous and comprehensive care to both the patient and their environment; even more so in this population with marked socio-cultural barriers.

Developing an Online Program for Self-Management of Fatigue, Pain, and Urgency in Inflammatory Bowel Disease: Patients' Needs and Wants

Abstract:Background: Inflammatory bowel disease (IBD) is a lifelong relapsing-remitting condition, characterized by troublesome symptoms including fatigue, pain, and bowel urgency. These symptoms can persist even in clinical remission and have a debilitating impact on social, work-related and intimate domains of life. Symptom self-management can be challenging for some patients, who could potentially benefit from an online self-management tool. Aims: We aimed to understand patients' symptom self-management strategies and preferred design for a future online symptom self-management intervention. Methods: Using exploratory qualitative methods, we conducted focus group and individual interviews with 40 people with IBD recruited from UK clinics and from community-dwelling members of the Crohn's and Colitis UK charity; data were collected using a digital audio recorder, and transcribed and anonymized by a third party (professional) transcriber. We used framework analysis for focus group data and thematic analysis for interview data. Results: The data provided three core themes: ways of coping; intervention functionality; and intervention content. Participants attempt to manage all three symptoms simultaneously, recognizing the combined influence of factors such as food, drink, stress, and exercise on all symptoms. They wanted an accessible online intervention functioning across several platforms, with symptom and medication management, and activity-tracking features. Conclusions: Patients reported numerous ways of self-managing symptoms of fatigue, pain, and urgency/incontinence related to IBD and expressed their needs for content, design, and functionality of the proposed intervention. Based on this and existing intervention development literature, the IBD-BOOST online self-management intervention has now been developed and is undergoing testing.

Abstract:Abstract Background IBD affects the quality of life (QoL) of both the patient and their family members. Our previous systematic review and qualitative research study found that family members of IBD patients express a need for improved information about IBD, networking opportunities, and increased access to support services such as IBD nurses, counsellors, or psychologists. While existing studies recognise the challenges faced by family members, there is a lack of interventions to address their psychosocial and emotional burdens. Therefore, this study aims to co-design and develop an intervention to support family members’ psychosocial and emotional wellbeing and build familial and individual resilience. Methods The study employed co-design approach informed by Design Thinking to explore family members’ needs, prioritise problems, and generate ideas for developing the new intervention. The co-design process comprised two sequential online workshops with individuals with IBD (n=4), family members (n=9), and IBD nurses (n=2). Workshop data were recorded in both video and audio formats, and transcribed verbatim. Reflexive thematic analysis was used to analyse the data. Results The study identified two priority areas for supporting family members of individuals with IBD: information and psychosocial support. Within these domains, specific solutions were pinpointed: (1) Improving access to information related to IBD and facilitating easier adaptation to living with the condition — achievable through educational materials, online resources, or support groups. (2) Increasing opportunities for networking and peer support, feasible through online forums, support groups, or social events. (3) Enhancing access to professional support services, such as counselling or therapy, to provide coping mechanisms and necessary emotional support. Additionally, a logic model and a prototype for a web-based online resource, aimed at supporting psychosocial and emotional wellbeing, were developed. Conclusion This co-design process delineated support priorities for family members of people with IBD, as well as offering suggestions for interventions aimed at fostering family members’ psychosocial and emotional wellbeing while promoting familial and individual resilience. We now plan to test this novel intervention in a feasibility study.
