Abstract:In this work, we utilize Machine Learning for early recognition of patients at high risk of acute respiratory distress syndrome (ARDS), which is critical for successful prevention strategies for this devastating syndrome. The difficulty in early ARDS recognition stems from its complex and heterogenous nature. In this study, we integrate knowledge of the heterogeneity of ARDS patients into predictive model building. Using MIMIC-III data, we first apply latent class analysis (LCA) to identify homogeneous sub-groups in the ARDS population, and then build predictive models on the partitioned data. The results indicate that significantly improved performances of prediction can be obtained for two of the three identified sub-phenotypes of ARDS. Experiments suggests that identifying sub-phenotypes is beneficial for building predictive model for ARDS.

Abstract:Abstract Background: Acute respiratory distress syndrome (ARDS) is a prevalent complication among critically ill patients, constituting around 10% of intensive care unit (ICU) admissions and mortality rates ranging from 35% to 46%. Hence, early recognition and prediction of ARDS are crucial for the timely administration of targeted treatment. However, ARDS is frequently underdiagnosed or delayed, and its heterogeneity diminishes the clinical utility of ARDS biomarkers. This study aimed to observe the incidence of ARDS among high-risk patients and develop and validate an ARDS prediction model using machine learning (ML) techniques based on clinical parameters. Methods: This prospective cohort study in China was conducted on critically ill patients to derivate and validate the prediction model. The derivation cohort, consisting of 400 patients admitted to the ICU of the Peking University Third Hospital(PUTH) between December 2020 and August 2023, was separated for training and internal validation, and an external data set of 160 patients at the FU YANG People's Hospital from August 2022 to August 2023 was employed for external validation. Least absolute shrinkage and selection operator (LASSO) and multivariate logistic regression were used to screen predictor variables. Multiple ML classification models were integrated to analyze and identify the best models. Several evaluation indexes were used to compare the predictive performance, including the area under the receiver-operating-characteristic curve (AUC) and decision curve analysis (DCA). S Hapley Additive ex Planations (SHAP) is used to interpret ML models. Results:400 critically ill patients were included in the analysis, with 117 developing ARDS during follow-up. The final model included gender, Lung Injury Prediction Score (LIPS), HepaticDisease, Shock, and combined Lung Contusion. Based on the AUC and DCA in the validation group, the logistic model demonstrated excellent performance, achieving an AUC of 0.836 (95% CI: 0.762-0.910). For external validation, comprising 160 patients, 44 of whom developed ARDS, the AUC was 0.799 (95% CI: 0.723-0.875). conclusion: Logistic regression models were constructed and interpreted using the SHAP method to provide a basis for screening high-risk groups for ARDS and to guide individualized treatment for different patients.

Abstract:Acute respiratory distress syndrome (ARDS) is a clinical syndrome defined by acute onset of hypoxemia and bilateral pulmonary opacities not fully explained by cardiac failure or volume overload. At present, there is no specific drug treatment for ARDS, and the mortality rate is high. The reason may be that ARDS has rapid onset, rapid progression, complex etiology, and great heterogeneity of clinical manifestations and treatment. Compared with traditional data analysis, machine learning algorithms can automatically analyze and obtain rules from complex data and interpret them to assist clinical decision making. This review aims to provide a brief overview of the machine learning progression in ARDS clinical phenotype, onset prediction, prognosis stratification, and interpretable machine learning in recent years, in order to provide reference for clinical.
Open-source machine learning pipeline automatically flags instances of acute respiratory distress syndrome from electronic health records

Abstract:Physicians could greatly benefit from automated diagnosis and prognosis tools to help address information overload and decision fatigue. Intensive care physicians stand to benefit greatly from such tools as they are at particularly high risk for those factors. Acute Respiratory Distress Syndrome (ARDS) is a life-threatening condition affecting >10% of critical care patients, and has a mortality rate over 40%. However, recognition rates for ARDS have been shown to be low (30-70%) in clinical settings. In this work, we present a reproducible computational pipeline that automatically adjudicates ARDS on retrospective datasets of mechanically ventilated adult patients. This pipeline automates the steps outlined by the Berlin Definition through implementation of natural language processing tools and classification algorithms. We train an XGBoost model on chest imaging reports to detect bilateral infiltrates, and another on a subset of attending physician notes labeled for the most common ARDS risk factor in our data. Both models achieve high performance: a minimum area under the receiver operating characteristic curve (AUROC) of 0.86 for adjudicating chest imaging reports in out-of-bag test sets, and an out-of-bag AUROC of 0.85 for detecting a diagnosis of pneumonia. We validate the entire pipeline on a cohort of MIMIC-III encounters and find a sensitivity of 93.5% (an extraordinary improvement over the 22.6% ARDS recognition rate reported for these encounters) along with a specificity of 73.9%. We conclude that our reproducible, automated diagnostic pipeline exhibits promising accuracy, generalizability, and probability calibration, thus providing a valuable resource for physicians aiming to enhance ARDS diagnosis and treatment strategies. We surmise that proper implementation of the pipeline has the potential to aid clinical practice by facilitating the recognition of ARDS cases at scale.

As a comparator, XGBoost models were developed for each of the 13 target labels. Model performance was assessed using the area under the receiver operating characteristic curve. Heat maps to visualize attention scores were generated to provide interpretability to the neural networks. Finally, cluster analysis was performed to identify potential phenotypic subgroups of patients with ARDS. Results: The single RNN model trained to classify 13 outputs outperformed the individual XGBoost models for ARDS prediction, achieving an area under the receiver operating characteristic curve of 0.842 on the external test sets. Models trained on an increasing number of tasks resulted in improved performance. Earlier prediction of ARDS nearly doubled the rate of in-hospital survival. Cluster analysis revealed distinct ARDS subgroups, some of which had similar mortality rates but different clinical presentations. Conclusions: The RNN model presented in this paper can be used as an early warning system to stratify patients who are at risk of developing one of the multiple risk outcomes, hence providing practitioners with the means to take early action.

Semisupervised Deep Learning Techniques for Predicting Acute Respiratory Distress Syndrome from Time-Series Clinical Data: Model Development and Validation Study

Abstract:Background A high number of patients who are hospitalized with COVID-19 develop acute respiratory distress syndrome (ARDS). Objective In response to the need for clinical decision support tools to help manage the next pandemic during the early stages (ie, when limited labeled data are present), we developed machine learning algorithms that use semisupervised learning (SSL) techniques to predict ARDS development in general and COVID-19 populations based on limited labeled data. Methods SSL techniques were applied to 29,127 encounters with patients who were admitted to 7 US hospitals from May 1, 2019, to May 1, 2021. A recurrent neural network that used a time series of electronic health record data was applied to data that were collected when a patient’s peripheral oxygen saturation level fell below the normal range (<97%) to predict the subsequent development of ARDS during the remaining duration of patients’ hospital stay. Model performance was assessed with the area under the receiver operating characteristic curve and area under the precision recall curve of an external hold-out test set. Results For the whole data set, the median time between the first peripheral oxygen saturation measurement of <97% and subsequent respiratory failure was 21 hours. The area under the receiver operating characteristic curve for predicting subsequent ARDS development was 0.73 when the model was trained on a labeled data set of 6930 patients, 0.78 when the model was trained on the labeled data set that had been augmented with the unlabeled data set of 16,173 patients by using SSL techniques, and 0.84 when the model was trained on the entire training set of 23,103 labeled patients. Conclusions In the context of using time-series inpatient data and a careful model training design, unlabeled data can be used to improve the performance of machine learning models when labeled data for predicting ARDS development are scarce or expensive.
Utilising intraoperative respiratory dynamic features for developing and validating an explainable machine learning model for postoperative pulmonary complications

Abstract:BACKGROUND: Timely detection of modifiable risk factors for postoperative pulmonary complications (PPCs) could inform ventilation strategies that attenuate lung injury. We sought to develop, validate, and internally test machine learning models that use intraoperative respiratory features to predict PPCs.METHODS: We analysed perioperative data from a cohort comprising patients aged 65 yr and older at an academic medical centre from 2019 to 2023. Two linear and four nonlinear learning models were developed and compared with the current gold-standard risk assessment tool ARISCAT (Assess Respiratory Risk in Surgical Patients in Catalonia Tool). The Shapley additive explanation of artificial intelligence was utilised to interpret feature importance and interactions.RESULTS: Perioperative data were obtained from 10 284 patients who underwent 10 484 operations (mean age [range] 71 [65-98] yr; 42% female). An optimised XGBoost model that used preoperative variables and intraoperative respiratory variables had area under the receiver operating characteristic curves (AUROCs) of 0.878 (0.866-0.891) and 0.881 (0.879-0.883) in the validation and prospective cohorts, respectively. These models outperformed ARISCAT (AUROC: 0.496-0.533). The intraoperative dynamic features of respiratory dynamic system compliance, mechanical power, and driving pressure were identified as key modifiable contributors to PPCs. A simplified model based on XGBoost including 20 variables generated an AUROC of 0.864 (0.852-0.875) in an internal testing cohort. This has been developed into a web-based tool for further external validation (https://aorm.wchscu.cn/).CONCLUSIONS: These findings suggest that real-time identification of surgical patients' risk of postoperative pulmonary complications could help personalise intraoperative ventilatory strategies and reduce postoperative pulmonary complications.

Predictive and Explainable Analysis of Post-operative Acute Kidney Injury in Children undergoing Cardiopulmonary Bypass: An Application of Large Language Models

Abstract:Motivation: Embedding large language models (LLMs) hold promise for predictive analytics due to their natively consistent numeric output. This study explores the utility of general-purpose LLMs for extracting interpretable and actionable insights from electronic health record (EHR) text columns in tabular data, focusing on paediatric cardiopulmonary bypass (CPB). Methods: We analysed data from 963 paediatric CPB operations in the UK (2019-2021), focusing on the severity of post-operative acute kidney injury (AKI) as a binary outcome, and using text columns documenting planned surgical procedures and patient diagnoses for each operation as features. We employed OpenAI's "text-embedding-3-large" for embeddings and "gpt-4-turbo" for generating descriptive labels of patient clusters that were formed by applying spherical k-means to embedding vectors. These "AI clusters" were compared against the "expert clusters" - based on the Partial Risk Adjustment in Surgery (PRAiS) v2 protocol - for 1) consistency using adjusted rand index (ARI) between the clusters and 2) predictive performance using area under ROC (AUC) of logistic regression models using cluster memberships as a categorical variable. Results: AI clusters showed statistically significant levels of consistency with expert clusters, evidenced by their ARIs of 0.31 and 0.32 for planned procedures and diagnoses, respectively. The clusters demonstrated comparable discriminative power in predicting severe post-operative kidney injury, with AUCs of 0.63 vs. 0.60 (planned procedures) and 0.56 vs. 0.58 (diagnoses) for AI clusters vs. expert clusters. Notably, AI clusters identified three interventions with significant odds ratios for AKI severity, highlighting potential areas for clinical focus. Replacing k-means clustering followed by logistic regression with k-nearest-neighbours - applied to LLM text embeddings - further improved AUC to 0.66 and 0.63, for planned procedures and diagnoses, respectively. Conclusions: Our findings affirm the potential of LLMs as effective tools in medical text analysis, facilitating both exploratory and predictive tasks. The integration of LLM-derived insights with traditional data analysis methods can significantly enhance risk stratification and outcome prediction in paediatric CPB, underscoring the value of AI-driven approaches in complex healthcare datasets.
[Study of the bactericidal effect of cefpodoxime using an in vitro pharmacokinetic model].

Abstract:Cefpodoxime is a new oral prodrug antibiotic. Following absorption from the proximal intestine, non-specific esterases hydrolyze this cleavable ester, releasing cefpodoxime, a new broad-spectrum third-generation cephalosporin with sustained plasma levels in humans. Cefpodoxime killing kinetics were studied using an in vitro model which simulates the pharmacokinetic profile obtained in healthy volunteers given a single oral dose of cefpodoxime proxetil providing 100, 200 or 400 mg active cefpodoxime. Cefpodoxime exhibited strong antibacterial activity against tested strains of Escherichia coli, Streptococcus pneumoniae and Staphylococcus aureus. These results suggest that use of two daily doses of 100 or 200 mg each are appropriate for the treatment of E. coli and S. pneumoniae infections in view of the pharmacokinetic properties of cefpodoxime. Less intensive therapy is probably adequate in uncomplicated community-acquired urinary tract infections. The bactericidal effect of cefpodoxime against S. aureus is prolonged due to a postantibiotic effect.
Development and validation of a deep learning-based framework for automated lung CT segmentation and acute respiratory distress syndrome prediction: a multicenter cohort study

Abstract:Background: Acute respiratory distress syndrome (ARDS) is a life-threatening condition with a high incidence and mortality rate in intensive care unit (ICU) admissions. Early identification of patients at high risk for developing ARDS is crucial for timely intervention and improved clinical outcomes. However, the complex pathophysiology of ARDS makes early prediction challenging. This study aimed to develop an artificial intelligence (AI) model for automated lung lesion segmentation and early prediction of ARDS to facilitate timely intervention in the intensive care unit. Methods: A total of 928 ICU patients with chest computed tomography (CT) scans were included from November 2018 to November 2021 at three centers in China. Patients were divided into a retrospective cohort for model development and internal validation, and three independent cohorts for external validation. A deep learning-based framework using the UNet Transformer (UNETR) model was developed to perform the segmentation of lung lesions and early prediction of ARDS. We employed various data augmentation techniques using the Medical Open Network for AI (MONAI) framework, enhancing the training sample diversity and improving the model's generalization capabilities. The performance of the deep learning-based framework was compared with a Densenet-based image classification network and evaluated in external and prospective validation cohorts. The segmentation performance was assessed using the Dice coefficient (DC), and the prediction performance was assessed using area under the receiver operating characteristic curve (AUC), sensitivity, and specificity. The contributions of different features to ARDS prediction were visualized using Shapley Explanation Plots. This study was registered with the China Clinical Trial Registration Centre (ChiCTR2200058700). Findings: The segmentation task using the deep learning framework achieved a DC of 0.734 ± 0.137 in the validation set. For the prediction task, the deep learning-based framework achieved AUCs of 0.916 [0.858-0.961], 0.865 [0.774-0.945], 0.901 [0.835-0.955], and 0.876 [0.804-0.936] in the internal validation cohort, external validation cohort I, external validation cohort II, and prospective validation cohort, respectively. It outperformed the Densenet-based image classification network in terms of prediction accuracy. Moreover, the ARDS prediction model identified lung lesion features and clinical parameters such as C-reactive protein, albumin, bilirubin, platelet count, and age as significant contributors to ARDS prediction. Interpretation: The deep learning-based framework using the UNETR model demonstrated high accuracy and robustness in lung lesion segmentation and early ARDS prediction, and had good generalization ability and clinical applicability. Abstract:Abstract Background To develop a machine learning model for predicting acute respiratory distress syndrome (ARDS) events through commonly available parameters, including baseline characteristics and clinical and laboratory parameters. Methods A secondary analysis of a multi-centre prospective observational cohort study from five hospitals in Beijing, China, was conducted from January 1, 2011, to August 31, 2014. A total of 296 patients at risk for developing ARDS admitted to medical intensive care units (ICUs) were included. We applied a random forest approach to identify the best set of predictors out of 42 variables measured on day 1 of admission. Results All patients were randomly divided into training (80%) and testing (20%) sets. Additionally, these patients were followed daily and assessed according to the Berlin definition. The model obtained an average area under the receiver operating characteristic (ROC) curve (AUC) of 0.82 and yielded a predictive accuracy of 83%. For the first time, four new biomarkers were included in the model: decreased minimum haematocrit, glucose, and sodium and increased minimum white blood cell (WBC) count. Conclusions This newly established machine learning-based model shows good predictive ability in Chinese patients with ARDS. External validation studies are necessary to confirm the generalisability of our approach across populations and treatment practices.

Abstract:Abstract Objective When patients develop acute respiratory failure (ARF), accurately identifying the underlying etiology is essential for determining the best treatment. However, differentiating between common medical diagnoses can be challenging in clinical practice. Machine learning models could improve medical diagnosis by aiding in the diagnostic evaluation of these patients. Materials and Methods Machine learning models were trained to predict the common causes of ARF (pneumonia, heart failure, and/or chronic obstructive pulmonary disease [COPD]). Models were trained using chest radiographs and clinical data from the electronic health record (EHR) and applied to an internal and external cohort. Results The internal cohort of 1618 patients included 508 (31%) with pneumonia, 363 (22%) with heart failure, and 137 (8%) with COPD based on physician chart review. A model combining chest radiographs and EHR data outperformed models based on each modality alone. Models had similar or better performance compared to a randomly selected physician reviewer. For pneumonia, the combined model area under the receiver operating characteristic curve (AUROC) was 0.79 (0.77–0.79), image model AUROC was 0.74 (0.72–0.75), and EHR model AUROC was 0.74 (0.70–0.76). For heart failure, combined: 0.83 (0.77–0.84), image: 0.80 (0.71–0.81), and EHR: 0.79 (0.75–0.82). For COPD, combined: AUROC = 0.88 (0.83–0.91), image: 0.83 (0.77–0.89), and EHR: 0.80 (0.76–0.84). In the external cohort, performance was consistent for heart failure and increased for COPD, but declined slightly for pneumonia. Conclusions Machine learning models combining chest radiographs and EHR data can accurately differentiate between common causes of ARF. Further work is needed to determine how these models could act as a diagnostic aid to clinicians in clinical settings.

Abstract:Background: Two acute respiratory distress syndrome (ARDS) subphenotypes (hyperinflammatory and hypoinflammatory) with distinct clinical and biological features and differential treatment responses have been identified using latent class analysis (LCA) in seven individual cohorts. To facilitate bedside identification of subphenotypes, clinical classifier models using readily available clinical variables have been described in four randomised controlled trials. We aimed to assess the performance of these models in observational cohorts of ARDS. Methods: In this observational, multicohort, retrospective study, we validated two machine-learning clinical classifier models for assigning ARDS subphenotypes in two observational cohorts of patients with ARDS: Early Assessment of Renal and Lung Injury (EARLI; n=335) and Validating Acute Lung Injury Markers for Diagnosis (VALID; n=452), with LCA-derived subphenotypes as the gold standard. The primary model comprised only vital signs and laboratory variables, and the secondary model comprised all predictors in the primary model, with the addition of ventilatory variables and demographics. Model performance was assessed by calculating the area under the receiver operating characteristic curve (AUC) and calibration plots, and assigning subphenotypes using a probability cutoff value of 0·5 to determine sensitivity, specificity, and accuracy of the assignments. We also assessed the performance of the primary model in EARLI using data automatically extracted from an electronic health record (EHR; EHR-derived EARLI cohort). In Large Observational Study to Understand the Global Impact of Severe Acute Respiratory Failure (LUNG SAFE; n=2813), a multinational, observational ARDS cohort, we applied a custom classifier model (with fewer variables than the primary model) to determine the prognostic value of the subphenotypes and tested their interaction with the positive end-expiratory pressure (PEEP) strategy, with 90-day mortality as the dependent variable. Findings: The primary clinical classifier model had an area under receiver operating characteristic curve (AUC) of 0·92 (95% CI 0·90-0·95) in EARLI and 0·88 (0·84-0·91) in VALID. Performance of the primary model was similar when using exclusively EHR-derived predictors compared with manually curated predictors (AUC=0·88 [95% CI 0·81-0·94] vs 0·92 [0·88-0·97]). In LUNG SAFE, 90-day mortality was higher in patients assigned the hyperinflammatory subphenotype than in those with the hypoinflammatory phenotype (414 [57%] of 725 vs 694 [33%] of 2088; p<0·0001). There was a significant treatment interaction with PEEP strategy and ARDS subphenotype (p=0·041), with lower 90-day mortality in the high PEEP group of patients with the hyperinflammatory subphenotype (hyperinflammatory subphenotype: 169 [54%] of 313 patients in the high PEEP group vs 127 [62%] of 205 patients in the low PEEP group; hypoinflammatory subphenotype: 231 [34%] of 675 patients in the high PEEP group vs 233 [32%] of 734 patients in the low PEEP group). Interpretation: Classifier models using clinical variables alone can accurately assign ARDS subphenotypes in observational cohorts. Application of these models can provide valuable prognostic information and could inform management strategies for personalised treatment, including application of PEEP, once prospectively validated. Abstract:Background: An early diagnosis model with clinical phenotype classification is key for the early identification and precise treatment of sepsis-associated acute respiratory distress syndrome (ARDS). This study aimed to: 1) build a machine learning diagnostic model for patients with sepsis-associated ARDS using easily accessible early clinical indicators, 2) conduct rapid classification of clinical phenotypes in this population, and 3) explore the differences in clinical characteristics, outcomes, and treatment responses of different phenotypes. Methods: This study is based on data from the Telehealth Intensive Care Unit (eICU) and Medical Information Mart for Intensive Care IV (MIMIC-IV). We trained and tested the early diagnostic model of sepsis-associated ARDS patients in the eICU. We used key predictive indicators to cluster sepsis-associated ARDS patients and determine the characteristics and clinical outcomes of different phenotypes, as well to explore the differences of in-hospital mortality among different the positive end-expiratory pressure (PEEP) levels in different phenotypes. These results are verified in MIMIC-IV to evaluate whether they are repeatable. Results: Among the diagnostic models constructed in 19,249 sepsis patients and 5,947 sepsis-associated ARDS patients, the AdaBoost (Decision Tree) model achieved the best performance with an area under the receiver operating characteristic curve (AUC) of 0.895, which is higher than that of the traditional Logistic Regression model (Z = −2.40, p = 0.013), and the accuracy of 70.06%, sensitivity of 78.11% and specificity of 78.74%. We simultaneously identified three sepsis-associated ARDS phenotypes. Cluster 0 ( n = 3,669) had the lowest in-hospital mortality rate (6.51%) and fewer laboratory abnormalities (lower WBC (median:15.000 K/mcL), lower blood glucose (median:158.000 mg/dl), lower creatinine (median:1.200 mg/dl), lower lactic acid (median:3.000 mmol/L); p < 0.001). Cluster 1 ( n = 1,554) had the highest in-hospital mortality rate (75.29%) and the most laboratory abnormalities (higher WBC (median:18.300 K/mcL), higher blood glucose (median:188.000 mg/dl), higher creatinine (median:2.300 mg/dl), higher lactic acid (median:3.900 mmol/L); p < 0.001). Cluster 2 ( n = 724) had the most complex condition, with a moderate in-hospital mortality rate (29.7%) and the longest intensive care unit stay. In Clusters 0 and 1, patients with high PEEP had higher in-hospital mortality rate than those with low PEEP, but the opposite trend was seen in Cluster 2. These results were repeatable in 11,935 patients with sepsis and 2,699 patients with sepsis-associated ARDS patients in the MIMIC-IV cohort. Conclusion: A machine learning diagnostic model of sepsis-associated ARDS patients was established. Three phenotypes with different clinical features and outcomes were clustered, and these had different therapeutic responses. These findings are helpful for the early and rapid identification of sepsis-associated ARDS patients and their precise individualized treatment.

Abstract:Chronic obstructive pulmonary disease (COPD) is one of the most common chronic illnesses in the world and the third leading cause of mortality worldwide. It is often underdiagnosed or not diagnosed until later in the disease course. Spirometry tests are the gold standard for diagnosing COPD but can be difficult to obtain, especially in resource-poor countries. Chest X-rays (CXRs), however, are readily available and may serve as a screening tool to identify patients with COPD who should undergo further testing. Currently, no research applies deep learning (DL) algorithms that use large multi-site and multi-modal data to detect COPD patients and evaluate fairness across demographic groups. We use three CXR datasets in our study, CheXpert to pre-train models, MIMIC-CXR to develop, and Emory-CXR to validate our models. The CXRs from patients in the early stage of COPD and not on mechanical ventilation are selected for model training and validation. We visualize the Grad-CAM heatmaps of the true positive cases on the base model for both MIMIC-CXR and Emory-CXR test datasets. We further propose two fusion schemes, (1) model-level fusion, including bagging and stacking methods using MIMIC-CXR, and (2) data-level fusion, including multi-site data using MIMIC-CXR and Emory-CXR, and multi-modal using MIMIC-CXRs and MIMIC-IV EHR, to improve the overall model performance. Fairness analysis is performed to evaluate if the fusion schemes have a discrepancy in the performance among different demographic groups. The results demonstrate that DL models can detect COPD using CXRs, which can facilitate early screening, especially in low-resource regions where CXRs are more accessible than spirometry. The multi-site data fusion scheme could improve the model generalizability on the Emory-CXR test data. Further studies on using CXR or other modalities to predict COPD ought to be in future work.

Abstract:The acute respiratory distress syndrome (ARDS) is a severe form of hypoxemic respiratory failure with in-hospital mortality of 35-46%. High mortality is thought to be related in part to challenges in making a prompt diagnosis, which may in turn delay implementation of evidence-based therapies. A deep neural network (DNN) algorithm utilizing unbiased ventilator waveform data (VWD) may help to improve screening for ARDS. We first show that a convolutional neural network-based ARDS detection model can outperform prior work with random forest models in AUC (0.95+/-0.019 vs. 0.88+/-0.064), accuracy (0.84+/-0.026 vs 0.80+/-0.078), and specificity (0.81+/-0.06 vs 0.71+/-0.089). Frequency ablation studies imply that our model can learn features from low frequency domains typically used for expert feature engineering, and high-frequency information that may be difficult to manually featurize. Further experiments suggest that subtle, high-frequency components of physiologic signals may explain the superior performance of DL models over traditional ML when using physiologic waveform data. Our observations may enable improved interpretability of DL-based physiologic models and may improve the understanding of how high-frequency information in physiologic data impacts the performance our DL model.
Machine learning-enabled risk prediction of chronic obstructive pulmonary disease with unbalanced data

Abstract:Background and objective: Since the early symptoms of chronic obstructive pulmonary disease (COPD) are not obvious, patients are not easily identified, causing improper time for prevention and treatment. In present study, machine learning (ML) methods were employed to construct a risk prediction model for COPD to improve its prediction efficiency. Methods: We collected data from a sample of 5807 cases with a complete COPD diagnosis from the 2019 COPD Surveillance Program in Shanxi Province and extracted 34 potentially relevant variables from the dataset. Firstly, we used feature selection methods (i.e., Generalized elastic net, Lasso and Adaptive lasso) to select ten variables. Afterwards, we employed supervised classifiers for class imbalanced data by combining the cost-sensitive learning and SMOTE resampling methods with the ML methods (Logistic Regression, SVM, Random Forest, XGBoost, LightGBM, NGBoost and Stacking), respectively. Last, we assessed their performance. Results: The cough frequently at age 14 and before and other 9 variables are significant parameters for COPD. The Stacking heterogeneous ensemble model showed relatively good performance in the unbalanced datasets. The Logistic Regression with class weighting enjoyed the best classification performance in the balancing data when these composite indicators (AUC, F1-Score and G-mean) were used as criteria for model comparison. The values of F1-Score and G-mean for the top three ML models were 0.290/0.660 for Logistic Regression with class weighting, 0.288/0.649 for Stacking with synthetic minority oversampling technique (SMOTE), and 0.285/0.648 for LightGBM with SMOTE. Conclusions: This paper combining feature selection methods, unbalanced data processing methods and machine learning methods with data from disease surveillance questionnaires and physical measurements to identify people at risk of COPD, concluded that machine learning models based on survey questionnaires could provide an automated identification for patients at risk of COPD, and provide a simple and scientific aid for early identification of COPD.

Abstract:RATIONALE: Computed tomography (CT) enables noninvasive diagnosis of usual interstitial pneumonia (UIP), but enhanced image analyses are needed to overcome the limitations of visual assessment.OBJECTIVES: Apply multiple instance learning (MIL) to develop an explainable deep learning algorithm for prediction of UIP from CT and validate its performance in independent cohorts.METHODS: We trained a MIL algorithm using a pooled dataset (n=2,143) and tested it in three independent populations: data from a prior publication (n=127), a single-institution clinical cohort (n=239), and a national registry of patients with pulmonary fibrosis (n=979). We tested UIP classification performance using receiver operating characteristic (ROC) analysis with histologic UIP as ground truth. Cox proportional hazards and linear mixed effects models were used to examine associations between MIL predictions and survival or longitudinal forced vital capacity (FVC).MEASUREMENTS AND MAIN RESULTS: In two cohorts with biopsy data, MIL improved accuracy for histologic UIP (area under the curve [AUC] 0.77 [n=127] and 0.79 [n=239]) compared to visual assessment (AUC 0.65 and 0.71). In cohorts with survival data, MIL UIP classifications were significant for mortality ([n=239, mortality to April 2021] unadjusted hazard ratio 3.1 95% confidence interval [CI] [1.96, 4.91] p<0.001, and [n=979, mortality to July 2022] 3.64 95% CI [2.66, 4.97] p<0.001). Individuals classified as UIP positive by the algorithm had a significantly greater annual decline in FVC than those classified as UIP negative (-88 ml/year versus -45 ml/year, n=979 p<0.01), adjusting for extent of lung fibrosis.CONCLUSIONS: Computerized assessment using MIL identifies clinically significant features of UIP on CT. Such a method could improve confidence in radiologic assessment of patients with interstitial lung disease, potentially enabling earlier and more precise diagnosis.
