Abstract:Abstract 3-D gait analysis is the gold standard but many healthcare clinics and research institutes would benefit from a system that is inexpensive and simple but just as accurate. The present study examines whether a low-cost 2-D motion capture system can accurately and reliably assess adaptive gait kinematics in subjects with central vision loss, older controls, and younger controls. Subjects were requested to walk up and step over a 10 cm high obstacle that was positioned in the middle of a 4.5 m walkway. Four trials were simultaneously recorded with the Vicon motion capture system (3-D system) and a video camera that was positioned perpendicular to the obstacle (2-D system). The kinematic parameters (crossing height, crossing velocity, foot placement, single support time) were calculated offline. Strong Pearson’s correlations were found between the two systems for all parameters (average r = 0.944, all p < 0.001). Bland-Altman analysis showed that the agreement between the two systems was good in all three groups after correcting for systematic biases related to the 2-D marker positions. The test-retest reliability for both systems was high (average ICC = 0.959). These results show that a low-cost 2-D video system can reliably and accurately assess adaptive gait kinematics in healthy and low vision subjects.

A validation study demonstrating portable motion capture cameras accurately characterize gait metrics when compared to a pressure-sensitive walkway

Abstract:Digital quantification of gait can be used to measure aging- and disease-related decline in mobility. Gait performance also predicts prognosis, disease progression, and response to therapies. Most gait analysis systems require large amounts of space, resources, and expertise to implement and are not widely accessible. Thus, there is a need for a portable system that accurately characterizes gait. Here, depth video from two portable cameras accurately reconstructed gait metrics comparable to those reported by a pressure-sensitive walkway. 392 research participants walked across a four-meter pressure-sensitive walkway while depth video was recorded. Gait speed, cadence, and step and stride durations and lengths strongly correlated (r > 0.9) between modalities, with root-mean-squared-errors (RMSE) of 0.04 m/s, 2.3 steps/min, 0.03 s, and 0.05-0.08 m for speed, cadence, step/stride duration, and step/stride length, respectively. Step, stance, and double support durations (gait cycle percentage) significantly correlated (r > 0.6) between modalities, with 5% RMSE for step and stance and 10% RMSE for double support. In an exploratory analysis, gait speed from both modalities significantly related to healthy, mild, moderate, or severe categorizations of Charleson Comorbidity Indices (ANOVA, Tukey's HSD, p < 0.0125). These findings demonstrate the viability of using depth video to expand access to quantitative gait assessments.

Abstract:<div><p>Objective To replace camera-based three-dimensional motion analyzers which are widely used to analyze body movements and gait but are also costly and require a large dedicated space, this study evaluates the validity and reliability of inertial measurement unit (IMU)-based systems by analyzing their spatio-temporal and kinematic measurement parameters.Methods The investigation was conducted in three separate hospitals with three healthy participants. IMUs were attached to the abdomen as well as the thigh, shank, and foot of both legs of each participant. Each participant then completed a 10-m gait course 10 times. During each gait cycle, the hips, knees, and ankle joints were observed from the sagittal, frontal, and transverse planes. The experiments were conducted with both a camerabased system and an IMU-based system. The measured gait analysis data were evaluated for validity and reliability using root mean square error (RMSE) and intraclass correlation coefficient (ICC) analyses.Results The differences between the RMSE values of the two systems determined through kinematic parameters ranged from a minimum of 1.83 to a maximum of 3.98 with a tolerance close to 1%. The results of this study also confirmed the reliability of the IMU-based system, and all of the variables showed a statistically high ICC.Conclusion These results confirmed that IMU-based systems can reliably replace camera-based systems for clinical body motion and gait analyses.<br/></p></div>
Validation of wearable inertial sensor-based gait analysis system for measurement of spatiotemporal parameters and lower extremity joint kinematics in sagittal plane

Abstract:Wearable inertial sensor-based motion analysis systems are promising alternatives to standard camera-based motion capture systems for the measurement of gait parameters and joint kinematics. These wearable sensors, unlike camera-based gold standard systems, find usefulness in outdoor natural environment along with confined indoor laboratory-based environment due to miniature size and wireless data transmission. This study reports validation of our developed (i-Sens) wearable motion analysis system against standard motion capture system. Gait analysis was performed at self-selected speed on non-disabled volunteers in indoor ( n = 15) and outdoor ( n = 8) environments. Two i-Sens units were placed at the level of knee and hip along with passive markers (for indoor study only) for simultaneous 3D motion capture using a motion capture system. Mean absolute percentage error (MAPE) was computed for spatiotemporal parameters from the i-Sens system versus the motion capture system as a true reference. Mean and standard deviation of kinematic data for a gait cycle were plotted for both systems against normative data. Joint kinematics data were analyzed to compute the root mean squared error (RMSE) and Pearson’s correlation coefficient. Kinematic plots indicate a high degree of accuracy of the i-Sens system with the reference system. Excellent positive correlation was observed between the two systems in terms of hip and knee joint angles (Indoor: hip 3.98° ± 1.03°, knee 6.48° ± 1.91°, Outdoor: hip 3.94° ± 0.78°, knee 5.82° ± 0.99°) with low RMSE. Reliability characteristics (defined using standard statistical thresholds of MAPE) of stride length, cadence, walking speed in both outdoor and indoor environment were well within the “Good” category. The i-Sens system has emerged as a potentially cost-effective, valid, accurate, and reliable alternative to expensive, standard motion capture systems for gait analysis. Further clinical trials using the i-Sens system are warranted on participants across different age groups.
Development and Validation of 2D-LiDAR-Based Gait Analysis Instrument and Algorithm

Abstract:Acquiring gait parameters from usual walking is important to predict clinical outcomes including life expectancy, risk of fall, and neurocognitive performance in older people. We developed a novel gait analysis tool that is small, less-intrusive and is based on two-dimensional light detection and ranging (2D-LiDAR) technology. Using an object-tracking algorithm, we conducted a validation study of the spatiotemporal tracking of ankle locations of young, healthy participants (n = 4) by comparing our tool and a stereo camera with the motion capture system as a gold standard modality. We also assessed parameters including step length, step width, cadence, and gait speed. The 2D-LiDAR system showed a much better accuracy than that of a stereo camera system, where mean absolute errors were 46.2 ± 17.8 mm and 116.3 ± 69.6 mm, respectively. Gait parameters from the 2D-LiDAR system were in good agreement with those from the motion capture system (r = 0.955 for step length, r = 0.911 for cadence). Simultaneous tracking of multiple targets by the 2D-LiDAR system was also demonstrated. The novel system might be useful in space and resource constrained clinical practice for older adults.
Comparison of yellow dent and NutriDense corn hybrids in swine diets.

Abstract:Two experiments were conducted to verify the feeding value of NutriDense (ND) and Nutri-Dense Low-Phytate (NDLP) corn (Exseed Genetics LLC, BASF Plant Science, Research Triangle Park, NC) relative to that of yellow dent (YD) corn in swine diets. NutriDense corn is a high-protein, high-oil variety, and NDLP is a high-protein, high-oil, low-phytate variety. In Exp. 1, 315 nursery pigs that initially weighed 15.2 kg were used in a 21-d growth assay. Dietary treatments were arranged in a 3 x 3 factorial; main effects were corn source (YD, ND, and NDLP) and added fat (0, 3, or 6%, as-fed basis). Diets were formulated to contain 3.83 g of lysine/Mcal using calculated nutrient values. There were no corn source x fat interactions observed. Pigs fed YD, ND, and NDLP had ADG of 750, 734, and 738 g/d and G:F of 0.64, 0.66, and 0.65, respectively. No differences (P > 0.10) in ADG were observed among the three corn sources; however, pigs fed diets containing either ND or NDLP corn had decreased ADFI (P < 0.02) and improved G:F (P < 0.05) compared with pigs fed diets containing YD corn. Increasing dietary fat increased ADG (727, 746, and 748 g/d; linear, P < 0.04) and G:F (0.62, 0.66, and 0.68; linear, P < 0.01) and decreased ADFI (linear, P < 0.01). Using the NRC (1998) value for ME in YD corn, we calculated the energy value for ND and NDLP based on G:F differences compared with pigs fed YD corn. These data indicated the ME values for ND and NDLP corn are 4.5 and 2.5% greater (3,575 and 3,505 Kcal/kg), respectively, than for YD corn (3,420 Kcal/kg). In Exp. 2, 1,144 gilts (initial BW = 50.1 kg) were used in a commercial research facility to evaluate the effects of corn source (ND and YD) and added fat (0, 3, or 6%, as-fed basis) in a 2 x 3 factorial on pig performance and carcass traits. There was a corn source x fat interaction for ADFI and G:F. Increasing added fat resulted in greater changes in ADFI and G:F in pigs fed YD corn diets compared with those fed ND corn. Feeding ND corn increased ADG (main effect, P < 0.04), and greater percentages of added fat increased ADG (main effect; linear, P < 0.01). Results of Exp. 2 suggest that ND corn has 5.3% more ME than YD corn. The additional energy provided by ND corn improves G:F in both nursery and grow-finish pigs, and ND corn offers a means of formulating diets more concentrated in energy than YD corn.

Abstract:Kinematic analysis of human gait is an effective strategy to detect and assess an individual's gait to diagnose and develop and guide follow-on rehabilitation protocols. So, an accurate, objective gait analysis system has potential to facilitate rehabilitation process. System using smartphone-Kinovea represent an emerging technology for physical activity assessment and that may be relevant for gait analysis. The objective of this study was to determine gait displacement, speed and joint angle by using smartphone-Kinovea software system – to compare the normal gait with four distinct simulated gait abnormalities. Also, to assess validity of the proposed system by compared with QTM as gold standard. 30 participants completed an experiment in which they completed several gait trails on single day. Gait types were analyzed using statistical analysis (two-way MANOVA). As for validation assessment was analyzed using paired t-test by comparing proposed system with QTM. Results shows that joint angles for abnormal gaits are higher mean (Standard Deviation) compared to normal gait during HS and TO. While, normal gait exhibits higher mean (Standard Deviation) for d and s during both IDS and TDS phases compared to other four abnormal gaits in both genders. Also, there are significant different (p<0.05) of gait for all gait comparisons for all parameters, except hip angle of normal-HP with p=0.495. Moreover, there is some gait was similar with other gait due to they shared underlying kinematic aspects such as BA and DP. The validation of the system gives moderate result. These support that the smartphone-Kinovea system have potential in detecting and identifying abnormal gaits, and for future implementation in diagnosis and rehabilitation.
Validity and reliability of free software for bidimensional gait analysis

Abstract:Despite the evaluation systems of human movement that have been advancing in recent decades, their use are not feasible for clinical practice because it has a high cost and scarcity of trained operators to interpret their results. An ideal videogrammetry system should be easy to use, low cost, with minimal equipment, and fast realization. The CvMob is a free tool for dynamic evaluation of human movements that express measurements in figures, tables, and graphics. This paper aims to determine if CvMob is a reliable tool for the evaluation of two dimensional human gait. This is a validity and reliability study. The sample was composed of 56 healthy individuals who walked on a 9-meterlong walkway and were simultaneously filmed by CvMob and Vicon system cameras. Linear trajectories and angular measurements were compared to validate the CvMob system, and inter and intrarater findings of the same measurements were used to determine reliability. A strong correlation (rs mean = 0.988) of the linear trajectories between systems and inter and intrarater analysis were found. According to the Bland-Altman method, the angles that had good agreement between systems were maximum flexion and extension (stance and swing) of the knee and dorsiflexion range of motion and stride length. The CvMob is a reliable tool for analysis of linear motion and lengths in two-dimensional evaluations of human gait. The angular measurements demonstrate high agreement for the knee joint; however, the hip and ankle measurements were limited by differences between systems.
Establishing the Reliability of the GaitON® Motion Analysis System: A Foundational Study for Gait and Posture Analysis in a Healthy Population

Abstract:Background: Gait and posture analysis plays a crucial role in understanding human movement, with significant applications in rehabilitation, sports science, and clinical settings. The GaitON® system, a 2D motion analysis tool, provides an accessible and cost-effective method for assessing gait and posture. However, its reliability in clinical practice, particularly for intra-rater consistency, remains to be evaluated. This study aims to assess the intra-rater reliability of the GaitON® system in a healthy population, focusing on gait and posture parameters. Methods: A total of 20 healthy participants (10 males and 10 females) aged 18 to 50 years were recruited for the study. Each participant underwent gait and posture assessments using the GaitON® system on two separate occasions, spaced one week apart. Video recordings from anterior and posterior views were used to analyze gait, while images from anterior, posterior, and lateral views were captured to assess posture with markers placed on key anatomical landmarks. The reliability of the measurements was analyzed using intraclass correlation coefficients (ICC), a standard error of measurement (SEM), and the smallest detectable difference (SDD) method. Results: The GaitON® system demonstrated excellent intra-rater reliability across a wide range of gait and posture parameters. ICC values for gait parameters, including hip, knee, and ankle joint angles, ranged from 0.90 to 0.979, indicating strong consistency in repeated measurements. Similarly, ICC values for posture parameters, such as the head alignment, shoulder position, and ASIS alignment, were above 0.90, reflecting excellent reliability. SEM values were low across all parameters, with the smallest SEM recorded for the hip joint angle (0.37°), and SDD values further confirmed the precision of the system. Conclusion: The GaitON® system provides reliable and consistent measurements for both gait and posture analysis in healthy individuals. Its high intra-rater reliability and low measurement error make it a promising tool for clinical and sports applications. Further research is needed to validate its use in clinical populations and compare its performance to more complex 3D motion analysis systems.

Abstract:There is the need for the development of reliable and easy to use in clinical setting gait assessment tools. An open-access video analysis software that administers the calculation of kinematical and spatio-temporal characteristics of human movement is Kinovea® however, its repeatability as a gait analysis tool has not been well addressed. The purpose of the study was to examine the applicability and reliability of an objective, quantitative, low-cost and easy to use in the clinical setting, gait evaluation method, using Kinovea® software. Data collected from 44 healthy subjects recording gait in sagittal and frontal plane using two smartphones. Time consumption of the procedure was captured. Kinovea® software was used to calculate kinematical and spatial parameters. Intra- and inter-rater reliability of the video processing as well as intra-rater reliability of the measurement procedure represented good to excellent and there were less random measurement errors. There was no measurement error due to random variation for the most of the calculated parameters, except of the pelvis position. The results suggest that excepting low accuracy in calculation of pelvis position, gait evaluation using Kinovea® software is objective, quantitative, low-cost, reliable and easy to use in the clinical setting.

Abstract:Spatio-temporal parameters of human gait, currently measured using different methods, provide valuable information on health. Inertial Measurement Units (IMUs) are one such method of gait analysis, with smartphone IMUs serving as a good substitute for current gold-standard techniques. Here we investigate the concurrent validity of a smartphone placed in a front-facing pocket to perform gait analysis. Sixty community-dwelling healthy adults equipped with a smartphone and an application for gait analysis completed a 2-min walk on a marked path. Concurrent validity was assessed against an APDM mobility lab (APDM Inc.; Portland, OR, USA). Bland–Altman plots and intraclass correlation coefficients (agreement and consistency) for gait speed, cadence, and step length indicate good to excellent agreement (ICC2,1 > 0.8). For right leg stance and swing % of gait cycle and double support % of gait cycle, results were moderate (0.52 < ICC2,1 < 0.62). For left leg stance and swing % of gait cycle left results show poor agreement (ICC2,1 0.8). Thus we have a valid and reliable instrument for measuring healthy adults’ spatio-temporal gait parameters in a controlled walking environment.

Abstract:BackgroundGait is usually assessed by clinical tests, which may have poor accuracy and be biased, or instrumented systems, which potentially solve these limitations at the cost of being time-consuming and expensive. The different versions of the Microsoft Kinect have enabled human motion tracking without using wearable sensors at a low-cost and with acceptable reliability. This study aims: First, to determine the sensitivity of an open-access Kinect v2-based gait analysis system to motor disability and aging; Second, to determine its concurrent validity with standardized clinical tests in individuals with stroke; Third, to quantify its inter and intra-rater reliability, standard error of measurement, minimal detectable change; And, finally, to investigate its ability to identify fall risk after stroke.MethodsThe most widely used spatiotemporal and kinematic gait parameters of 82 individuals post-stroke and 355 healthy subjects were estimated with the Kinect v2-based system. In addition, participants with stroke were assessed with the Dynamic Gait Index, the 1-min Walking Test, and the 10-m Walking Test.ResultsThe system successfully characterized the performance of both groups. Significant concurrent validity with correlations of variable strength was detected between all clinical tests and gait measures. Excellent inter and intra-rater reliability was evidenced for almost all measures. Minimal detectable change was variable, with poorer results for kinematic parameters. Almost all gait parameters proved to identify fall risk.ConclusionsResults suggest that although its limited sensitivity to kinematic parameters, the Kinect v2-based gait analysis could be used as a low-cost alternative to laboratory-grade systems to complement gait assessment in clinical settings.

Abstract:Measurement of gait is becoming important as a tool to identify disease and disease progression, yet to date its application is limited largely to specialist centers. Wearable devices enables gait to be measured in naturalistic environments, however questions remain regarding validity. Previous research suggests that when compared with a laboratory reference, measurement accuracy is acceptable for mean but not variability or asymmetry gait characteristics. Some fundamental reasons for this have been presented, (e.g., synchronization, different sampling frequencies) but to date this has not been systematically examined. The aims of this study were to: 1) quantify a comprehensive range of gait characteristics measured using a single triaxial accelerometer-based monitor; 2) examine outcomes and monitor performance in measuring gait in older adults and those with Parkinson's disease (PD); and 3) carry out a detailed comparison with those derived from an instrumented walkway to account for any discrepancies. Fourteen gait characteristics were quantified in 30 people with incident PD and 30 healthy age-matched controls. Of the 14 gait characteristics compared, agreement between instruments was excellent for four (ICCs 0.913-0.983); moderate for four (ICCs 0.508-0.766); and poor for six characteristics (ICCs 0.637-0.370). Further analysis revealed that differences reflect an increased sensitivity of accelerometry to detect motion, rather than measurement error. This is most likely because accelerometry measures gait as a continuous activity rather than discrete footfall events, per instrumented tools. The increased sensitivity shown for these characteristics will be of particular interest to researchers keen to interpret "real-world" gait data. In conclusion, use of a body-worn monitor is recommended for the measurement of gait but is likely to yield more sensitive data for asymmetry and variability features.

Abstract:Desulfovibrio africanus ferredoxin III (Da FdIII) contains one [4Fe-4S](2+/1+) cluster and one [3Fe-4S](1+/0) cluster, bound by seven Cys residues, in which the [3Fe-4S] cluster is co-ordinated by the unusual sequence, Cys(11)-Xaa-Xaa-Asp(14)-Xaa-Xaa-Cys(17)-Xaa(n)-Cys(51)-Glu. The [3Fe-4S] core of this ferredoxin is so far unique in showing rapid bi-directional [3Fe-4S]<-->[4Fe-4S] cluster interconversion with a wide range of metal ions. In order to obtain protein for mutagenesis studies Da FdIII has been cloned, sequenced, and expressed as a hexa-histidine tagged (ht) polypeptide in Escherichia coli strain BL21(DE3) pLysS. Expression of ht Da FdIII, whether translated from a synthetic gene (pJB10) or from the native nucleotide sequence (pJB11), occurred at similar levels (approx. 6 mg.l(-1)), but without incorporation of metal clusters. The nucleotide sequence confirms the protein sequence reported previously [Bovier-Lapierre, Bruschi, Bonicel and Hatchikian (1987) Biochim. Biophys. Acta 913, 20-26]. Cluster incorporation was achieved using FeCl(3) together with cysteine sulphur transferase, NifS, plus cysteine to generate low levels of sulphide ions. Absorption and EPR spectroscopy show that both [3Fe-4S] and [4Fe-4S] clusters are correctly inserted. Thin-film electrochemistry provides evidence that the [3Fe-4S] cluster undergoes reversible cluster transformation in the presence of Fe(II) and Zn(II) ions with properties identical to the native protein. Nevertheless the protein has lower stability than native Da FdIII during chromatography. The one-dimensional 600 MHz NMR spectrum of the apoprotein indicates an unstructured protein with random coil chemical shifts whereas spectra of the reconstituted ht protein show secondary structural elements and 18 peaks shifted downfield of 9.6 p.p.m. The spectra are unique but have similarities with the shift patterns seen with 7Fe Desulfurolobus ambivalens Fd. The ht does not affect iron-sulphur cluster incorporation, but NMR evidence suggests that excess Fe binds to the tag. This may account for the lower stability of the ht compared with the native protein.

Today, human gait analysis is commonly used for clinical diagnosis, rehabilitation and performance improvement in sports. However, although previous research works in the literature address the use of motion capture systems by means of optoelectronic sensors, Inertial Measurement Units (IMUs) and depth cameras, few of them discuss their conception, guidelines and algorithms for measuring and calculating gait metrics. Moreover, commercially available motion capture systems, although efficient, are cost restrictive for most of the low-income institutions. In this research work, a new computer vision-based system (CVS) for gait analysis is developed and proposed. The aim is to close the gap found in the literature about the design and development of such systems by providing the requirements, considerations, algorithms and methodologies used to develop a gait analysis system with acceptable precision and accuracy, and at low cost. For this purpose, a linear computer vision method based on the non-homogeneous solution of the calibration matrix was used. The spatio-temporal and angular gait parameters were implemented in the proposed system, and compared with those reported in the literature. The denoising of the spatial gait trajectories and the strategies to detect gait events, are also presented and discussed. The results have shown that the proposed system is satisfactory for human gait analysis in terms of precision, computational performance and low cost.

Performance of a Mobile 3D Camera to Evaluate Simulated Pathological Gait in Practical Scenarios

Abstract:Three-dimensional (3D) cameras used for gait assessment obviate the need for bodily markers or sensors, making them particularly interesting for clinical applications. Due to their limited field of view, their application has predominantly focused on evaluating gait patterns within short walking distances. However, assessment of gait consistency requires testing over a longer walking distance. The aim of this study is to validate the accuracy for gait assessment of a previously developed method that determines walking spatiotemporal parameters and kinematics measured with a 3D camera mounted on a mobile robot base (ROBOGait). Walking parameters measured with this system were compared with measurements with Xsens IMUs. The experiments were performed on a non-linear corridor of approximately 50 m, resembling the environment of a conventional rehabilitation facility. Eleven individuals exhibiting normal motor function were recruited to walk and to simulate gait patterns representative of common neurological conditions: Cerebral Palsy, Multiple Sclerosis, and Cerebellar Ataxia. Generalized estimating equations were used to determine statistical differences between the measurement systems and between walking conditions. When comparing walking parameters between paired measures of the systems, significant differences were found for eight out of 18 descriptors: range of motion (ROM) of trunk and pelvis tilt, maximum knee flexion in loading response, knee position at toe-off, stride length, step time, cadence; and stance duration. When analyzing how ROBOGait can distinguish simulated pathological gait from physiological gait, a mean accuracy of 70.4%, a sensitivity of 49.3%, and a specificity of 74.4% were found when compared with the Xsens system. The most important gait abnormalities related to the clinical conditions were successfully detected by ROBOGait. The descriptors that best distinguished simulated pathological walking from normal walking in both systems were step width and stride length. This study underscores the promising potential of 3D cameras and encourages exploring their use in clinical gait analysis.
Validity and Reliability of Gait Speed and Knee Flexion Estimated by a Novel Vision-Based Smartphone Application

Abstract:Patients with knee osteoarthritis walk with reduced speed and knee flexion excursion in the early stance phase. A slow walking speed is also associated with falls in older adults. A novel vision-based smartphone application could potentially facilitate the early detection of knee osteoarthritis and fall prevention. This study aimed to test the validity and reliability of the app-captured gait speed and peak knee flexion during the initial stance phase of gait. Twenty adults (aged 23–68 years) walked at self-selected comfortable walking speeds while the gait speed and knee flexion were simultaneously measured using retroreflective sensors and Xsens motion trackers and the app in two separate sessions for validity and reliability tests. Pearson's r correlation and Bland–Altman plots were used to examine the correlations and agreements between the sensor- and app-measured outcomes. One-sample t-tests were performed to examine whether systematic bias existed. The intraclass correlation coefficient (ICC) was calculated to assess the test–retest reliability of the app. Very high correlations were found between the sensor and app measurements for gait speed (r = 0.98, p < 0.001) and knee flexion (r = 0.91‒0.92, all p < 0.001). No significant bias was detected for the final app version. The app also showed a good to excellent test–retest reliability for measuring the gait speed and peak knee flexion (ICC = 0.86‒0.94). This vision-based smartphone application is valid and reliable for capturing the walking speed and knee flexion during the initial stance of gait, potentially aiding in the early detection of knee osteoarthritis and fall prevention in community living locations.

Abstract:Abstract Despite its paramount importance for manifold use cases (e.g., in the health care industry, sports, rehabilitation and fitness assessment), sufficiently valid and reliable gait parameter measurement is still limited to high-tech gait laboratories mostly. Here, we demonstrate the excellent validity and test–retest repeatability of a novel gait assessment system which is built upon modern convolutional neural networks to extract three-dimensional skeleton joints from monocular frontal-view videos of walking humans. The validity study is based on a comparison to the GAITRite pressure-sensitive walkway system. All measured gait parameters (gait speed, cadence, step length and step time) showed excellent concurrent validity for multiple walk trials at normal and fast gait speeds. The test–retest-repeatability is on the same level as the GAITRite system. In conclusion, we are convinced that our results can pave the way for cost, space and operationally effective gait analysis in broad mainstream applications. Most sensor-based systems are costly, must be operated by extensively trained personnel (e.g., motion capture systems) or—even if not quite as costly—still possess considerable complexity (e.g., wearable sensors). In contrast, a video sufficient for the assessment method presented here can be obtained by anyone, without much training, via a smartphone camera.

The Development of the "Lab-in-shoe" System Based on an Instrumented Footwear for High-Throughput Analysis of Gait Parameters

Abstract:Acquisition of spatiotemporal parameters in gait outside the laboratory remains a difficult challenge in the current clinical setting. In this study, an instrumented footwear, namely "Lab-in-shoe" system, was implemented based on inertial (IMU) and distributed plantar pressure sensors, to facilitate high-throughput analysis of gait parameters. The system uses the plantar pressure sensor to determine the moment when the velocity of IMU reaches zero, and then uses the zero-velocity update algorithm (ZUPT) to mitigate sensor drift effects. In addition, simultaneous visualization of foot positioning and plantar pressure data in gait is achieved, which allows clinicians to obtain more intuitive information about gait alterations. To confirm the reliability and validity of the system, the estimates of spatiotemporal parameters were compared with the gold standard optical motion capture system (Vicon). Inter-system validity was assessed by Bland-Altman plots, and reliability was assessed by intra-class correlation (ICC 2, k). The results showed that "Lab-in-shoe" system has good consistency in stride length compared to the Vicon regardless of the walking speed (0.68-1.4 m/s), with an average error approximately at 1%. The overall correlation coefficients for ICC were larger than 0.961, indicating an almost perfect agreement. The results suggest that the reliability of spatiotemporal gait parameters with the "lab-in-shoe" system are acceptable and offered fundamental evidence that the system could be used to quantify the gait parameters in an open environment without the constraint of laboratory settings.
Vision-based motion capture for the gait analysis of neurodegenerative diseases: A review

Abstract:Background: Developments in vision-based systems and human pose estimation algorithms have the potential to detect, monitor and intervene early on neurodegenerative diseases through gait analysis. However, the gap between the technology available and actual clinical practice is evident as most clinicians still rely on subjective observational gait analysis or objective marker-based analysis that is time-consuming. Research question: This paper aims to examine the main developments of vision-based motion capture and how such advances may be integrated into clinical practice. Methods: The literature review was conducted in six online databases using Boolean search terms. A commercial system search was also included. A predetermined methodological criterion was then used to assess the quality of the selected articles. Results: A total of seventeen studies were evaluated, with thirteen studies focusing on gait classification systems and four studies on gait measurement systems. Of the gait classification systems, nine studies utilized artificial intelligence-assisted techniques, while four studies employed statistical techniques. The results revealed high correlations of gait features identified by classifier models with existing clinical rating scales. These systems demonstrated generally high classification accuracies and were effective in diagnosing disease severity levels. Gait measurement systems that extract spatiotemporal and kinematic joint information from video data generally found accurate measurements of gait parameters with low mean absolute errors, high intra- and inter-rater reliability. Significance: Low cost, portable vision-based systems can provide proof of concept for the quantification of gait, expansion of gait assessment tools, remote gait analysis of neurodegenerative diseases and a point of care system for orthotic evaluation. However, certain challenges, including small sample sizes, occlusion risks, and selection bias in training models, need to be addressed. Nevertheless, these systems can serve as complementary tools, equipping clinicians with essential gait information to objectively assess disease severity and tailor personalized treatment for enhanced patient care.
