Abstract:OBJECTIVE:To determine the quantitive and functional changes of circulating endothelial progenitor cells (EPCs) in dogs with dehydromonocrotaline-induced pulmonary artery hypertension (PAH).METHODS:Dehydromonocrotaline was injected into the canine right ventricle to induce pulmonary hypertension. Circulating EPCs were enumerated as AC+(113), KDR+ cells by fluorescence-activated cell sorter using counting beads, and the number and activity of EPCs after in vitro expansion were determined by acLDL uptake/lectin staining assay and in vitro tubule forming assay.RESULTS:Nine of the 10 beagles survived after dehydromonocrotaline injection. Six weeks later, mean pulmonary artery pressure increased from (11.3 +/- 2.0) mm Hg (1 mm Hg = 0.133 kPa) to (20.2 +/- 1.6) mm Hg (t =10.307, P < 0.01), and the AC+(133) and KDR+ cells decreased from (632.8 +/- 42.8) cells/ml to (206.1 +/- 26.8) cells/ml (t = 25.361, P < 0.01). UEA- I and DiLDL positive cells deceased from (41 +/- 6) EPCs/ x 200 field to (22 +/- 6) EPCs/ x 200 field (t = 6.510, P < 0.01). In addition, in vasculogenesis assay, PAH EPCs formed less quantitative (11.2 +/- 2.8 vs 21.1 +/- 2.8 tubules/ x 200 field, respectively, t = 7. 583, P <0. 01) and less qualitive tubules than baseline EPCs.CONCLUSION:The number and vessel forming ability of EPCs are impaired in this canine model of dehydromonocrotaline-induced pulmonary hypertension.
Dose–response of benazepril on biomarkers of the classical and alternative pathways of the renin–angiotensin–aldosterone system in dogs

Abstract:Angiotensin-converting enzyme inhibitors (ACEI) such as benazepril are commonly prescribed in both humans and dogs with heart disease to mitigate the renin–angiotensin–aldosterone system (RAAS); however, the dose-dependent effects of benazepril on comprehensive RAAS components remain unknown. In this study, nine purpose-bred healthy dogs received three different dosages of oral benazepril (0.125 mg/kg, 0.25 mg/kg, or 0.5 mg/kg) in a randomized crossover design following induction of RAAS activation by consuming a low-sodium diet. Blood samples were collected at serial time intervals after benazepril dosing to measure plasma benazeprilat (active metabolite of benazepril) and serum RAAS biomarkers. Blood pressure and echocardiogram were performed at baseline and after each benazepril administration. Time-weighted averages for RAAS biomarkers for 12 h post-dose and hemodynamic variables were compared between dosing groups using Wilcoxon rank-sum testing. Compared to the lowest dosage of benazepril (0.125 mg/kg), the highest dosage (0.5 mg/kg) resulted in lower time-weighted average values of angiotensin (Ang) II (− 38%, P = 0.004), Ang1-5 (− 53%, P = 0.001), ACE-S (surrogate for ACE activity; − 59%, P = 0.0002), and ALT-S (surrogate for alternative RAAS activity; − 22%, P = 0.004), and higher values of AngI (+ 78%, P = 0.014) and PRA-S (surrogate for plasma renin activity; + 58%, P = 0.040). There were no relevant differences between dosing groups for blood pressure or echocardiographic variables. Knowledge of dose-dependent alterations in biomarkers of the classical and alternative RAAS pathways could help inform clinical trials for dosage optimization in both dogs and humans.

Factors affecting the urinary aldosterone-to-creatinine ratio in healthy dogs and dogs with naturally occurring myxomatous mitral valve disease

Abstract:Abstract Background Chronic renin-angiotensin-aldosterone system (RAAS) activation in course of heart diseases contributes to cardiac remodeling and heart failure. Myxomatous mitral valve disease (MMVD) is characterized by different stages of severity and trend of RAAS activity during the course of the disease is still uncertain. The urinary aldosterone-to-creatinine ratio (UAldo:C) has been proven to reflect RAAS activation in dogs and might be a useful marker in monitoring therapy and disease progression, but data about this parameter need to be expanded. The objective of this study was to evaluate the UAldo:C in healthy dogs and dogs with naturally occurring MMVD, and to investigate the relationships between this parameter and clinical, echocardiographic and laboratory variables. Results The study population consisted of 149 dogs: 49 healthy and 100 MMVD dogs (45 stage B1, 13 stage B2 and 42 stage C). Urinary aldosterone-to-creatinine ratio was not significantly different among healthy and MMVD dogs of any stages. Breed, sex and age showed a significant impact on UAldo:C. In particular, Chihuahua and Cavalier King Charles spaniel showed significantly higher UAldo:C than other breeds, as well as intact females than other genders. In stage C dogs, UAldo:C appeared to be increased by spironolactone and was positively associated with furosemide dose ( P = 0.024). Aldosterone breakthrough (ABT) appeared to occur in 36% (8/22) of stage C dogs not receiving spironolactone. A significant positive association between UAldo:C and left atrium-to-aortic root ratio (LA/Ao) was found. Conclusions Individual factors such as breed, sex and age appeared to influence UAldo:C, and therapy seemed to add further variability. In the light of these results, comparing the UAldo:C of a single patient with a population-based reference value might lead to wrong interpretations and an individual monitoring should be considered. The prevalence of ABT in the present study (36%) was in line with those previously reported. However, due to the high individual variability of UAldo:C found in the study, even this result should be re-evaluated in the setting of an individual longitudinal approach. The positive association between UAldo:C and LA/Ao supports the mutual relationship between RAAS and cardiac remodeling.

Abstract:An angiotensin receptor/neprilysin inhibitor (ARNI), a heart failure treatment, is a combination drug made up of sacubitril, a neprilysin inhibitor, and valsartan, a vascular receptor blocker. No human or veterinary studies regarding the effect of ARNI on renal haemodynamics in the absence of cardiac or renal issues exist. Therefore, we investigated the effect of ARNI on renal haemodynamics in five healthy dogs. ARNI was administered to all five dogs at an oral dose of 20 mg/kg twice daily for 4 weeks. Renal haemodynamics were assessed on the day before ARNI administration (BL), on Day 7, and on Day 28. The glomerular filtration rate (GFR) significantly increased on Day 28 compared to BL and Day 7, whereas renal plasma flow increased on Day 7 and Day 28 compared to BL. Systolic blood pressure significantly decreased between BL and Day 28. Plasma atrial natriuretic peptide (ANP) concentrations increased on Day 7 compared to BL. Additionally, ANP concentrations increased on Day 28 in three of the five dogs. Different ANP concentrations were observed in the remaining two dogs. Both urine output volume and heart rate remained relatively stable and did not exhibit significant change. In conclusion, ARNI may enhance renal haemodynamics in healthy dogs. ARNI could be a valuable drug for treating both heart and kidney disease in dogs.

Abstract:Objective:To investigate the effect of angiotensin converting enzyme inhibitor(ACEI),angiotensin Ⅱ receptor blocker(ARB) and combined treatment on general and intrarenal renin-angiotensin system(RAS) in pants with chronic kidney disease(CKD). Methodology:Twenty-four CKD patients were randomly assigned to benazepril,valsartan or conmbined treatment group.During 8 weeks of benazepril treatment (20 mg/d),valsartan treatment (160 mg/d) or conmbined treatment (benazepril 10 mg/d and valsartan 80 mg/d),the blood pressure,serum creatinine,and proteinuria were detected, and the plasma renin activity (PRA),plasma and urinary angiotensinogen(AGT),angiotensin Ⅱ(Ang Ⅱ) and aldosterone were measured by RIA or ELISA. Results:After treatment of 8 weeks,the level of proteinuria [(0.61±0.25)g/24h vs (0.35±0.20)g/24h,P<0.05] and urinary AGT [(60.76±28.05)ng/(mg·Cr) vs (23.09±14.74)ng/(mg·Cr),P<0.05] was lower in benazepril group than that in baseline.In valsartan group, mean artery pressure was lower than baseline [(99.17±10.56)mmHg vs (84.63±9.33)mmHg,P<0.05] and PRA was higher than baseline[(1.33±0.76)ng/(ml·h) vs (6.02±2.59)ng/(ml·h),P<0.01].The primary end point was 87.5% in benazepril group,12.5% in valsartan group and 62.5% in combined treatment group.Compared with valsartan,the primary end point was reduced by benazepril (P<0.05). After 8 weeks treatment,PRA[(3.20±1.25)ng/(ml·h) vs (6.02±2.59)ng/(ml·h),P<0.05] and plasma Ang Ⅱ [(53.32±11.13)pg/ml vs (105.61±59.49)pg/ml,P<0.05] of benazepril group were lower than those of valsartan group. Conclusion:Short-term treatment with benazepril can reduce intrarenal agiotensin Ⅱ activity and urinary protein in patients with CKD. Valsartan may elevate plasma renin activity and angiotensin Ⅱ more significantly than benazepril.
Comprehensive characterization of the effect of mineralocorticoid receptor antagonism with spironolactone on the renin-angiotensin-aldosterone system in healthy dogs

Abstract:Objective To characterize the dose-exposure-response effect of spironolactone on biomarkers of the classical and alternative arms of the renin-angiotensin-aldosterone system (RAAS) in healthy dogs. Animals Ten healthy purpose-bred Beagle dogs. Procedures Study dogs were randomly allocated to 2 spironolactone dosing groups (2 mg/kg PO q24hr, 4 mg/kg PO q24hr). The dogs received 7-day courses of spironolactone followed by a 14-day washout period in a crossover (AB/BA) design. Angiotensin peptides and aldosterone were measured in serum using equilibrium analysis, and plasma canrenone and 7-α-thiomethyl spironolactone (TMS) were quantified via liquid chromatography-mass spectrometry/mass spectroscopy (LC-MS/MS). Study results were compared before and after dosing and between groups. Results Following spironolactone treatment, dogs had a significant increase in serum aldosterone concentration ( P = 0.07), with no statistical differences between dosing groups. Significant increases in angiotensin II ( P = 0.09), angiotensin I ( P = 0.08), angiotensin 1–5 ( P = 0.08), and a surrogate marker for plasma renin activity ( P = 0.06) were detected compared to baseline following spironolactone treatment during the second treatment period only. Overall, changes from baseline did not significantly differ between spironolactone dosages. RAAS analytes were weakly correlated (R < 0.4) with spironolactone dosage and plasma canrenone or plasma TMS. There were no adverse clinical or biochemical effects seen at any spironolactone dosage during treatment. Conclusions Treatment with spironolactone increased serum aldosterone concentration in healthy dogs and impacted other biomarkers of the classical and alternative arms of the RAAS. There was no difference in effect on the RAAS between 2 and 4 mg/kg/day dosing. Dosage of 4 mg/kg/day was safe and well-tolerated in healthy dogs.

Effect of telmisartan on microalbuminuria in patients with hypertension

Abstract:Objective:To investigate the effect of telmisartan on microalbuminuria in the patients with hypertension.Methods:81 cases of hypertension were randomly divided into two groups.41 patients in telmisartan group(A) orally took telmisartan 80mg,once a day,while 40 cases in mlodipine group(B) took oral amlodipine 5 mg,once a day.The antihypertensive effects and the change of microalbuminuria were observed and compared between the two groups.Results:After treatment,the systolic and diastolic blood pressures were significantly lower than those before treatment(P<0.05 or P<0.01).In the group A,systolic blood pressure decreased more significantly.After 1-month treatment,urinary albumin in both groups had significant difference compared with before treatment(P<0.05).However,no significant difference existed between the two groups(P>0.05);After 3-month treatment,the decrease of microalbuminuria in the group A was more obvious than that of the group B(P<0.05).Comparison of renal function before and after treatment in both groups showed no significant difference(P>0.05).Conclusion:Telmisartan and amlodipine can reduce microalbuminuria for short-term in hypertensive patients.But for the long-term observation of patients with hypertension,telmisartan is superior to amlodipine in reducing urinary protein.
Hypertension, retinopathy, and acute kidney injury in dogs: A prospective study

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Systemic hypertension (SH) is a potential complication of acute kidney injury (AKI) in dogs.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>To describe the prevalence of SH and hypertensive retinopathy in dogs with AKI, to investigate the relationship between SH and severity of AKI and to assess possible factors associated with SH.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Animals</h3><p>Fifty‐two dogs with AKI.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>Prospective observational study of dogs presenting to a tertiary referral center that fulfilled the International Renal Interest Society (IRIS) guidelines for the diagnosis of AKI. Systolic blood pressure measurement, urine protein/creatinine ratio (UPCR), urine output, presence of hypertensive retinopathy and fluid overload (FO), survival to discharge and duration of hospitalization were subsequently assessed. The prevalence of SH was calculated and the relationship between SH and recorded factors was examined by nonparametric statistics.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>The prevalence of SH (≥160 mm Hg) on admission or during hospitalization was 75% (39/52) and in 56% (22/39) of cases this was severe (≥180 mm Hg). Sixteen percent (7/43) of dogs had evidence of hypertensive retinopathy and 77% (24/31) dogs had UPCR >0.5. Forty‐two percent (22/52) dogs had FO on admission or during hospitalization. There was no association between SH and IRIS AKI grade, oligo/anuria, survival to discharge, duration of hospitalization or proteinuria. Dogs with FO on presentation were more likely to be hypertensive at admission compared to dogs without FO (<i>P </i> = .02). Dogs that did not survive to discharge were more likely to have FO (<i>P </i> = .007). </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions and Clinical Importance</h3><p>Systemic hypertension is common in dogs with AKI. Systemic hypertension might be associated with FO, which itself is associated with nonsurvival. Monitoring for SH and FO is therefore warranted in dogs with AKI.</p></section>

Influence of Telmisartan on peripheral calcineurin of rat with two kidneys one clip hypertension

Abstract:Aim To study the effects of telmisartan upon serum calcineurin.Methods 92 male SD rats with the same age were randomly divided into control group (N), sham operation group (S), 2K1C+distilled water group (K) and 2K1C+telmisartan group (T).S rats were performed the open-abdomen surgery without being restricted any renal artery, but the K and the T rats were restricted their left renal artery. Beginning from the third week after the surgery, the K rats started to be treated with the intragastric infusion of distilled water 10 ml·(kg·d)~(-1) , while the T rats with telmisartan 10 mg·(kg·d)~(-1) .And after being treated for 2, 4 and 8 weeks, rats were respectively measured the systolic blood pressure (SBP), the diastolic blood pressure (DBP) of the abdominal aorta. Before and after the operation, ultrasonography with probe of 7.5 MHz was used to obtain the structure and functional indexes, such as IVSd, IVSs, LVPWd, and serum calcineurin were evaluated by ELISA and colorimetric assay kit.Result Compared with the S group and the N group, ① the results of blood pressure (SBP, DBP) were significantly higher in K group (all P <0.01), after use of telmisartan, blood pressure was significantly reduced(P <0.01);② the thickness of interventricular septal and left ventricular posterior wall at the end of diastolic and systolic were significantly higher in K group (all P <0.01), after use of telmisartan, the thickness of those declined(all P <0.01);③ the level and activity of serum calcineurin were significantly higher in K group (all P <0.01), after use of telmisartan, the level and activity of calcineurin significantly fell(P <0.01).Conclusion The serum calcineurin of artery was also raised in the left ventricular remodeling. Telmisartan ameliorates ventricular remodeling effectively, which may be associated with decreasing the expression of artery serum calcineurin.
Diurnal pattern of sodium excretion in dogs with and without chronically reduced renal perfusion pressure

Abstract:In 5 conscious dogs the diurnal patterns of urinary sodium excretion (UNaV) were investigated, initially during 1 control day and, thereafter, during 4 days of servo-controlled reduction of renal perfusion pressure (rRPP). The individual dog's mean arterial blood pressure was reduced to 80% of the blood pressure on the control day. This value was always found to be below the threshold for the pressure-dependent renin release. During the entire study period urine was collected in 4-hour intervals and blood samples were taken every 4 h. The dogs were kept on a standardized high sodium and high water intake and were fed once daily at 8.30 h. On the control day, UNaV, urinary flow rate (UV), fractional lithium excretion (FELi) and fractional sodium excretion (FENa) had similar diurnal patterns. They peaked 4-8 h after food intake and decreased to low values during the night. On day 1 of rRPP, UNaV and FENa were maintained at very low levels in all collection periods, whereas the patterns of UV and FELi were unaltered compared with the patterns on the control day. On days 2-4 of rRPP, a clear-cut maximum in the patterns of UNaV and FENa recurred, comparable with the patterns on the control day. However, compared with the control day this maximum was shifted by 4 h towards the night. In contrast, the patterns of UV and FELi remained unchanged compared with the control day. The results indicate that UNaV has a typical time course in conscious, sodium- and water-replete dogs fed once daily. Endogenous stimulation of sodium reabsorption by means of rRPP results in a characteristic 4-hour shift of UNaV and FENa towards the night during rRPP days 2-4. This delay in UNaV seems to be evoked by processes in the distal tubule.

Abstract:Chronic low-grade inflammation is a key contributor to the progression of kidney disease. The release of cytokines and other pro-inflammatory proteins may further contribute to detrimental kidney health by increasing interstitial edema and renal fibrosis. The aim of the present study was to investigate the inflammatory markers in canines who developed renal disease naturally and were diagnosed with renal disease either during life or following necropsy, as assessed by a veterinarian. RNA was isolated from canine blood obtained at necropsy and stored as bioarchived samples from ten canines with renal disease (9.6–14.7 yr) and ten controls (10.1–14.8 yr). At the time of death, the mean blood creatinine concentration and BUN were elevated in dogs with renal disease compared to control (both p < 0.01). Samples were assessed for changes in gene expression using the Canine cytokine RT2 Profiler PCR Array for inflammation. There was a significant increase in C-C Motif Chemokine Ligand 16 (CCL16), C-X-C Motif Chemokine Ligand 5 (CXCL5), Interleukin 16 (IL-16), and Complement Component 5 (C5) (all p < 0.05 vs. con). In addition, there was also a statistically non-significant increase in 49 genes and a down-regulation in 35 genes from a panel of total 84 genes. Pro-inflammatory genes including CCL16, CXCL5, IL-16, and C5 can all contribute to renal inflammation and fibrosis through different signaling pathways and may lead to a progressive impairment of kidney function. Blockade of their activation may be important in ameliorating the initiation and/or the progression of renal disease.

Abstract:The present study compared renoprotective effects of angiotensin II type I receptor antagonist (AT(1)RA) with angiotensin converting enzyme inhibitor (ACEI), and their influence on the renin-angiotensin-system (RAS). Experimental nephrotic syndrome was induced in SD rats by repeated peritoneal injections of puromycin. Twenty-eight rats were randomly divided into four groups: normal control, nephrotic control, ACEI-treated, and AT(1)RA-treated groups. Serum, urine, and renal tissue were collected for study at the end of 12 weeks. Compared with those of the nephrotic control group, urinary protein was less and renal function was better in both treated groups. The glomerular and interstitial damage indexes of both ACEI- and AT1RA-treated rats were lower than those of nephrotic control rats, with no significant difference observed between the two treated groups. Local renal ACE activity and angiotensin II concentration were elevated in nephrotic rats (p < 0.01). However, there is no significant difference in circulating RAS, renal tissue renin, and aldosterone between the normal control and nephrotic control rats. As expected, enalapril inhibited the local renal ACE activity and significantly decreased angiotensin II (p < 0.01). Intrarenal ACE activity and angiotensin concentration returned to normal levels after treatment with irbesartan (p < 0.01). In conclusion, AT1RA and ACEI have comparable renal protective effects, and these protective effects were associated with the inhibition of intrarenal ANG II.
Effects of valsartan on angiotensinⅡreceptor distribution after myocardial infarction in dogs

Abstract:Objective To investigate the effects of valsartan on expression of angiotensin Ⅱ receptors in different regions of heart after myocardial infarction(MI).Metbods Canines were divided into sham-operated control group(n=7),infarction group(n=7)and Valsartan group(10 mg·kg-1·day-1 for 4 weeks after MI operation,n=7).Four weeks after operation,Dopplor tissue imaging(DTI)was used to evaluate reglonal ventricular function in the noninfarcted myocardium(apical and basal near to the infarction region).The mRNA and protein expressions of angiotensin Ⅱ type 1 receptor(AT1-R)and angiotensin Ⅱ type 2 receptor(AT2-R)on the corresponding regions were detected by competitive reverse-transcriptase polymerase chain reaction technique and immunohistochemical technique respectively.Results The protein and mRNA expressions of AT1-R were significantly increased in both apical and basal regions near to the infarction in dogs with MI compared with those in control group(P<0.05)which could be down-regulated by valsartan(P<0.05).AT2-R expressions were significantly upregulated in infarction group in both apical and basal regions compared with those in control group and valsartan further increased AT2-R expressions in both areas(P<0.05). Myocardial peak systolic velocity(Sm),myocardial peak early diastolic velocity(Em)and myocardial peak late diastolic velocity(Am)at both apical and basal regions near to the infarction regions were significantly lower in MI group than those in the control group which could be significantly improved by valsartan.Conclusion Both mRNA and protein expressions of AT1-R and AT2-R are upregulated in noninfarcted regions near MI,valsartan improved myocardial function via inhibiting AT1-R upregulation and enhancing AT2-R upregulation.

It also encourages nurse clinicians and nurse administrators to use the Cochrane Library and encourages nurse researchers to contribute to the Cochrane Library by conducting systematic reviews in the field of nursing care effectiveness.
Effects of Losartan on the Electrical Heterogeneity of Ventricular Wall During Acute Ischemia in Canine in Vivo

Abstract:Objective:To explore the effects of losartan on the electrical heterogeneity of ventricular wall during acute ischemia in canine in vivo. Methods: In canine acute ischemic model, 16 dogs were divided into 2 groups, control group( n = 8) and losartan group( n =8). losartan (5 mg.kg-1.d-1 ) was administered for 2 weeks before operation. With monophasic action potential (MAP) recording technique , the MAPs in epicardium (Epi), mid-cardium and endocardium (Endo) were recorded simultaneously during myocardial ischemia in two groups. MAPD, transmural dispersion of repolarization (TDR) and arrhythmias were monitored. Results: During acute myocardial ischemia, MAPD90 was shortened remarkably in the Epi and Endo, modestly in the Mid. The difference of the MAPD90 between Mid and Epi or Endo was significant ( P 0. 05). TDR in the three layers of ventricular myocardium were increased. Ventricular tachycardia or ventricular fibrillation occured in 4 dogs under the condition of acute ischemia, but no malignant arrhythmia occurred in all dogs before acute ischemia. The TDR in the losartan group was smaller than that in the control group ( P0.05). No ventricular tachycardia or ventricular fibrillation was found in the losartan group, the difference of the rate of malignant arrhythmia was significant between the two groups. Conclusion:The results suggest that losartan can suppress the TDR in acute ischemic myocardium, and decrease the development of malignant arrhythmias.

Abstract:Objective　To investigate the effects of angiotensin converting enzyme inhibitor (ACEI) and angiotensin II receptor antagonist (ATRA) on atrial natriuretic factor (ANF) and renin-angiotensin system (RAS) in patients with chronic renal failure (CRF).　Methods　The plasma angiotensin Ⅰ (AI), angiotensin Ⅱ (AII) and ANF levels were determined in 30 healthy volunteers and 40 patients with CRF after administration of losartan (Los) 50 mg once daily or fosinopril (Fos) 10 mg once daily in 0、4、8 and 12 weeks. Sit diastolic blood pressure (SiDBP) and blood urea nitrogen (BUN) and serum creatinine (SCr) were also measured at the same time. The data were analyzed by statistical software SPSS 7.0 .　Results　After 12 weeks, the levels of SiDBP were significantly decreased in both Los and Fos groups. In Fos group, aII was significantly inhibited, ANF and Scr were significantly increased (P<0.01). In Los group, aII was significantly increased (P<0.01), ANF reduced to normal level after it was increased at fourth week. SCr didn't increase.　Conclusion　There are significant effects of ACEI on intrarenal perfusion pressure. The change in ANF level may be used as a parameter for the effects. There are less effects of ATRA on intrarenal perfusion pressure than ACEI.

Abstract:The present study was conducted to investigate electrocardiographical (ECG) and haemato-biochemical changes in arrhythmia associated with renal diseases in dogs. The dogs with renal affections confirmed through appropriate diagnostic methods were selected and screened for arrhythmia. The ECG and haemato-biochemical parameters of twenty dogs with arrhythmia were compared with that of the control group and ECG parameters were statistically correlated with the haemato-biochemical parameters for correlation studies. It was found that the occurrence of arrhythmia was 51.2 per cent in renal diseases. Arrhythmia was more predominant in dogs with chronic kidney disease (CKD) followed by acute kidney injury (AKI). Sinus arrhythmia followed by first-degree AV block and wandering pacemaker were the common types of arrhythmias observed. A significant increase in R-R interval and a decrease in heart rate was noticed in comparison. The haemato-biochemical analysis revealed anaemia, leukocytosis with neutrophilia, elevated blood urea nitrogen (BUN), creatinine and magnesium level. A significant positive correlation was noticed between haemoglobin, volume of packed red cells (VPRC) and red blood cell count (RBC) with T amplitude and, creatinine and BUN levels with corrected Q-T interval. A significant negative correlation was noticed between VPRC, RBC and haemoglobin with the corrected QT interval. The present study revealed ECG and haemato-biochemical parameters had a significant role in renal diseases in dogs which might help in the early diagnosis and proper management of arrhythmia associated with renal diseases.
Effects of Telmisartan on Intraocular Pressure, Blood Pressure, and Ocular Perfusion Pressure in Normal and Glaucomatous Cats

Abstract:Purpose: To determine the effect of telmisartan on intraocular pressure (IOP), blood pressure (BP), and ocular perfusion pressure (OPP) in normal and glaucomatous cats. Methods: A four-week study was conducted in six normal adult cats, followed by a longer six-month study performed in 37 cats with spontaneous glaucoma and 11 age-matched normal cats. Telmisartan (1 mg/kg/day) or placebo-vehicle were administered orally once daily. IOP was measured by rebound tonometry. BP readings were obtained by oscillometric method. OPP was calculated as mean arterial pressure (MAP) - IOP. IOP and BP were obtained three times a week for the first study and weekly for the second study. Results: Baseline IOP was significantly higher, and OPP was significantly lower in glaucomatous cats than in normal cats (P < 0.0001). These differences between glaucomatous and normal cats persisted throughout the study, regardless of treatment (P < 0.001). No significant differences in IOP, BP, or OPP were detected between any study phases in the first, normal feline cohort or between telmisartan- and placebo-treated glaucomatous cats at any timepoint in the second study. Conclusions: Oral telmisartan was well tolerated and did not have a detrimental effect on BP or OPP in cats but did not lower IOP or improve OPP in cats with glaucoma. Translational relevance: While showing telmisartan could not be used as a sole therapy for IOP lowering, our data affirmed a lack of detrimental effects of telmisartan on BP and OPP in a translationally-relevant, spontaneous, large animal glaucoma model.

Antiproteinuric effects of angiotensin receptor blockers: telmisartan versus valsartan in hypertensive patients with type 2 diabetes mellitus and overt nephropathy

Abstract:Background: Renin-angiotensin system blockade reduces proteinuria and prevents nephropathy progression in patients with type 2 diabetes mellitus (T2D). Experimental evidence demonstrates that angiotensin receptor blockers (ARBs) possess anti-inflammatory potential, which might contribute to reducing proteinuria and providing renoprotection. Methods: We conducted a multicentre, double-blind, prospective, parallel-group non-inferiority study of 885 hypertensive [systolic blood pressure/diastolic blood pressure (SBP/DBP) >130/80 mmHg] patients with T2D, proteinuria (> or =900 mg/24 h) and serum creatinine (< or =3.0 mg/dl) who were randomized to once-daily telmisartan 80 mg or valsartan 160 mg; additional antihypertensive therapy was permitted. The primary endpoint was the change from baseline in the 24-h proteinuria after 12 months. Secondary endpoints included changes in 24-h albuminuria, estimated glomerular filtration rate (eGFR) and inflammatory parameters asymmetrical dimethylarginine (ADMA), high-sensitivity C-reactive protein (CRP) and urinary 8-iso-prostaglandin F(2alpha) (8-iso-PGF(2alpha)). Results: Telmisartan and valsartan produced comparable reductions in 24-h urinary protein excretion rates: geometric mean reduction (95% confidence interval) [telmisartan, 33% (27-39%); valsartan, 33% (27-38%)]. No significant differences between treatments were seen in changes from baseline in 24-h urinary albumin excretion rate and eGFR at 12 months. With both treatments, greater renoprotection was seen among patients with better blood pressure control. No significant changes in ADMA or CRP were noted in either group after 12 months, but urinary 8-iso-PGF(2alpha) levels decreased by 14% with telmisartan and by 7% with valsartan (P = 0.040). Conclusions: In patients with T2D, hypertension and overt nephropathy, the renoprotection afforded by telmisartan and valsartan appears similar, and the study was unable to show any effect beyond that due to blood pressure control. At doses used to treat hypertension, there is no evidence of inflammatory parameters being modified by ARBs in patients with more advanced kidney disease due to T2D.
