Abstract:The vast majority of volatile organic compounds (VOCs) are of biological origin and do not affect human health, while some VOCs or their oxidation products can damage the respiratory system, nervous system, digestive system and blood system after long-term inhalation by humans. There is limited evidence regarding the association of VOCs exposure with childhood asthma. In this study, we examined the associations between metabolites of VOCs (mVOCs) in urine and childhood asthma. We included a total of 1542 children aged 3–12 years who had information on urinary mVOCs, asthma and essential covariates in the current analyses. After controlling for covariates, we used logistic regression to assess the association between urinary mVOCs and childhood asthma. Then, we examined effect measure modification by child age, gender, race/ethnicity and serum cotinine. 2-Methylhippuric acid (xylene metabolites) (OR: 1.14; 95 % CI: 0.87, 1.59), N-acetyl-S-(benzyl)-l-cysteine (toluene metabolites) (OR: 1.15 95 % CI: 0.76, 1.71), N-acetyl-S-(2-carboxyethyl)-l-cysteine (acrolein metabolites) (OR: 1.09; 95 % CI: 0.61, 1.75), N-acetyl-S-(3-hydroxypropyl)-l-cysteine (acrolein metabolites) (OR: 1.10; 95 % CI: 0.66, 1.80), and N-acetyl-S-(3-hydroxypropyl-1-methyl)-l-cysteine (crotonaldehyde metabolites) (OR: 1.18; 95 % CI: 0.68, 2.01) were weakly associated with the prevalence of asthma in children. Among female children, 2MHA (2-methylhippuric acid) in urine was significantly associated with the prevalence of asthma (OR: 1.81 95 % CI: 1.07, 3.05). At the same time, BMA (N-acetyl-S-(benzyl)-l-cysteine) was significantly associated with the prevalence of asthma in non-Hispanic White (OR:2.09 95 % CI: 0.91, 4.66) and Black (OR:1.90 95 % CI: 0.96, 3.71) children. We found that gender modified the associations between urinary 2MHA and the odds of asthma (interaction term p value = 0.03). Urinary Metabolomic Profiling Reveals Biological Pathways and Predictive Signatures Associated with Childhood Asthma

Abstract:Background: Despite considerable efforts, the pathogenic mechanisms of asthma are still incompletely understood, due to its heterogeneous nature. However, metabolomics can offer a global view of a biological system, making it a valuable tool for further elucidation of mechanisms and biomarker discovery in asthma. Methods: GC-MS-based metabolomic analysis was conducted for comparison of urine metabolic profiles between asthmatic children (n=30) and healthy controls (n=30). Results: An orthogonal projections to latent structures discriminant-analysis model revealed a clear separation of the asthma and control groups (R-x(2)=0.137, R-y(2)=0.947, Q(2)=0.82). A total of 20 differential metabolites were identified as discriminant factors, of which eleven were significantly increased and nine decreased in the asthma group compared to the control group. Pathway-enrichment analysis based on these differential metabolites indicated that sphingolipid metabolism, protein biosynthesis, and citric acid cycle were strongly associated with asthma. Among the identified metabolites, 2-hydroxybutanoic acid showed excellent discriminatory performance for distinguishing asthma from healthy controls, with an AUC of 0.969. Conclusion: Our study revealed significant changes in the urine metabolome of asthma patients. Several perturbed pathways (eg, sphingolipid metabolism and citric acid cycle) may be related to asthma pathogenesis, and 2-hydroxybutanoic acid could serve as a potential biomarker for asthma diagnosis.

Abstract:(299/300) Concerns persist about the potential impact of prenatal exposure to bisphenols (BP) and their replacement analogues on childhood asthma and allergies. Previous studies on single and small cohorts had limited statistical power, few investigated analogues BPF and BPS, and even fewer examined atopic outcomes. Our objective was to assess whether prenatal exposures to individual environmental bisphenols (BPA, BPF, BPS) influence risk of childhood asthma, allergic rhinitis, and atopic dermatitis. Data from the the U.S. Environmental Influences on Child Health Outcomes (ECHO) consortium were harmonized on measures of prenatal urinary BPA, BPF and BPS and asthma and allergic rhinitis (ages 5-9 years) and atopic dermatitis (up to age 3 years) from 1905 mother-child pairs that were collected between 1998-2017. Across the 2012 federal ban of BPA from certain infant products, median BPA levels decreased from 1.11 ng/ml to 0.86 ng/ml; median BPF levels decreased from 0.51 ng/ml to 0.39 ng/ml; and median BPS levels increased from 0.23 ng/ml to 0.31 ng/ml (dilution adjusted; p<0.001 for all three median comparisons). Prenatal measures of BPA, BPF, and BPS were unrelated to the risk of childhood asthma, allergic rhinitis, or atopic dermatitis in the total population. Modest sex-dependent effects were observed: only among girls, second tertile levels of BPF was associated with a reduced odds of asthma (odds ratio (OR) 0.27, 95% confidence interval (CI) 0.08, 0.93); a continuous index of prenatal BPS was associated with reduced odds of atopic dermatitis (OR 0.64, 95% CI 0.44, 0.93). The ongoing and changing patterns of exposure to bisphenols in the U.S. population require further study with additional attention to time windows of exposure and co-occurring social determinants of health, to continue to inform current policies and evaluate the importance of limiting exposure to BPA and its analogues on childhood asthma, allergic rhinitis, and atopic dermatitis.

Urinary Biomarkers of Polycyclic Aromatic Hydrocarbons and Timing of Pubertal Development: The California PAH Study

Abstract:Background: Polycyclic aromatic hydrocarbons (PAHs) are endocrine-disrupting chemicals. Few studies have evaluated the association between pubertal development in girls and PAH exposures quantified by urinary biomarkers. Methods: We examined associations of urinary PAH metabolites with pubertal development in 358 girls 6-16 years of age from the San Francisco Bay Area enrolled in a prospective cohort from 2011 to 2013 and followed until 2020. Using baseline data, we assessed associations of urinary PAH metabolites with pubertal development stage. In prospective analyses limited to girls who at baseline had not yet started breast (N = 176) or pubic hair (N = 179) development or menstruation (N = 267), we used multivariable Cox proportional hazards regression to assess associations of urinary PAH metabolites with the onset of breast and pubic hair development, menstruation, and pubertal tempo (interval between the onset of breast development and menstruation). Results: We detected PAH metabolites in >98% of girls. In cross-sectional analyses using baseline data, PAH metabolites were not associated with the pubertal development stage. In prospective analyses, higher concentrations (≥ median) of some PAH metabolites were associated with two-fold higher odds of earlier breast development (2-hydroxy naphthalene, 1-hydroxy phenanthrene, summed hydroxy phenanthrenes) or pubic hair development (1-hydroxy naphthalene) among girls overweight at baseline (body mass index-for-age percentile ≥85) compared with nonoverweight girls with lower metabolites concentrations. PAH metabolites were not associated with age at menarche or pubertal tempo. Conclusions: PAH exposures were widespread in our sample. Our results support the hypothesis that, in overweight girls, PAHs impact the timing of pubertal development, an important risk factor for breast cancer.

Abstract:Introduction: Interpretation of findings from epidemiologic studies on ambient black carbon (BC) and asthma exacerbations among children has been hampered by uncertainties in exposure assessment and in the precise nature of the airway responses. Evidence linking personal BC exposure on acute respiratory symptoms and biomarker responses in both asthmatic and non-asthmatic children are relatively sparse. We examined associations of personal BC concentration with respiratory symptoms among asthmatic and non-asthmatic children in a large northeastern US city. We hypothesized that personal BC would be associated with respiratory biomarker responses and that effects would be more pronounced among asthmatic than non-asthmatic children. Methods: Daily lung function (FEV1/FVC and FEF2575/FVC), exhaled nitric oxide (eNO), a biomarker of oxidative stress (8-isoprostane) samples, and previous 24-hour personal BC data were collected among 30 asthmatic and 24 non-asthmatic 9/10 years old children. Repeated measures were collected at six different occasions over 3 to 4 weeks. The short-term associations between personal BC and respiratory biomarkers and lung function were assessed using linear mixed-effect models. Results: We found that the association between short-term exposure to BC and lung function decrements was modified by asthma status. Percent decreases in FEV1/FVC associated with each interquartile range (1.5 µg/m3) increase in 24hr average personal BC was 1.55 [95%CI: 0.34, 2.75] among non asthmatic children. We did not observe significant associations between personal BC and lung function among asthmatic children. We did not observe effects of BC on eNO in either subgroup. 8-isoprostane samples are currently being analyzed and findings will be presented. Conclusions: Our findings suggest short-term exposure to BC may have adverse effects on lung-function in non-asthmatic children. Greater variability in respiratory function among asthmatics may have limited power.

Abstract:BACKGROUND Although evidence suggests a weak association of single environmental risk factors with asthma and allergy risk in children, the influence of a combination of multiple environmental pollutants and household air pollution in the relationship of asthma and allergy remains unclear. We aimed to assessed the association of environmental composite quality index scores with asthma and allergy. METHOD Environmental composite quality indices were collected using the questionnaire that contains asthma and allergy, Chinese residential living habits, and outdoor environment items. Multivariable odds ratios (ORs) and 95% confidence intervals (CIs) were estimated using the binary logistic regression model after adjusting the most known asthma or allergy risk factors. RESULTS In this cross-sectional study, a total of 1636 girls and 1446 boys were included in the final analyses. Higher environmental quality index score was correlated with higher risk of asthma, eczema, and rhinitis. Comparing the top (≥5) to bottom group (0-2), the multivariable ORs were 2.47 (95% CI, 1.41-4.33, P trend = 0.004) for asthma, 1.51 for eczema (95% CI, 1.18-1.93; P trend = 0.002) and 1.83 for rhinitis (95% CI, 1.12-3.00; P trend = 0.017). Similar findings were observed in the stratified analyses. CONCLUSION The study showed that there is a harmful association between environmental quality and asthma with allergic diseases in children living in northeast China, and further research was needed to confirm the findings in other areas and clarify the specific mechanism.

Abstract:Background and aim: Studies suggest prenatal exposure to polycyclic aromatic hydrocarbons (PAHs) may influence wheezing or asthma in preschool-aged children. However, the impact of prenatal PAH exposure on asthma and wheeze in middle childhood remain unclear. We investigated these associations in diverse participants from the ECHO PATHWAYS multi-cohort consortium. Methods: We included 1,081 birth parent-child dyads across five U.S. cities. Maternal urinary mono-hydroxylated PAH metabolite concentrations (OH-PAH) were measured during mid-pregnancy. Asthma at age 8-9 years and wheezing trajectory across childhood were characterized by caregiver reported asthma diagnosis and asthma/wheeze symptoms. We used logistic and multinomial regression to estimate odds ratios of asthma and childhood wheezing trajectories associated with five individual OH-PAHs, adjusting for urine specific gravity, various maternal and child characteristics, study site, prenatal and postnatal smoke exposure, and birth year and season in single metabolite and mutually adjusted models. We used multiplicative interaction terms to evaluate effect modification by child sex and explored OH-PAH mixture effects through Weighted Quantile Sum regression. Results: The prevalence of asthma in the study population was 10%. We found limited evidence of adverse associations between pregnancy OH-PAH concentrations and asthma or wheezing trajectories. We observed adverse associations between 1/9-hydroxyphenanthrene and asthma and persistent wheeze among girls, and evidence of inverse associations with asthma for 1-hydroxynathpthalene, which was stronger among boys, though tests for effect modification by child sex were not statistically. Conclusions: In a large, multi-site cohort, we did not find strong evidence of an association between prenatal exposure to PAHs and child asthma at age 8-9 years, though some adverse associations were observed among girls.

Abstract:BACKGROUND:Few studies can be found on phthalate exposure in relation to childhood asthma and allergic symptoms from Mainland China, where a persistent increase in prevalence of childhood asthma and allergic disease has been observed.OBJECTIVES:This study aimed to assess the exposure levels to phthalates and its relationship with asthmatic and allergic symptoms among children in Shanghai, which has the highest prevalence of childhood asthma in Mainland China.METHODS:A follow-up study (2013-2014) of 434 children aged 5-10 years was conducted, based on the China, Children, Homes, Health (CCHH) study (2011-2012) in Shanghai, China. Information on asthmatic and allergic symptoms (wheeze, rhinitis, and eczema) were collected using validated questionnaires. Ten phthalate metabolites in morning urine samples were analyzed by high-performance liquid chromatography with triple quadrupole tandem mass spectrometry (HPLC-MS/MS). Multivariable logistic regression was used to estimate the associations between symptoms and urinary phthalate metabolites controlling for demographics, family history of allergic diseases and other covariates.RESULTS:Nine out of 10 phthalate metabolites were detected in all subjects (average detection rate of 93.2%). By multivariable logistic regression analyses, the 4th quartile of Mono‑n‑butyl phthalate (MnBP) (reference: 1st quartile) had adjusted prevalence odds ratios (aPORS) and 95% confidence intervals (95%CIs) of 2.27(1.06-4.88), 2.14(1.02-4.46) and 2.98(1.19-7.50) for wheeze, rhinitis and eczema, respectively, while those of Mono‑isobutyl phthalate (MiBP) were 2.23(1.08-4.62) and 2.96(1.02-8.60) for rhinitis and eczema, respectively. The highest quartile of mono‑2‑ethyl‑5‑hydroxyhexyl phthalate(MEHHP) and mono‑2‑ethyl‑5‑oxohexyl phthalate(MEOHP) had aPORS and 95%CIs of 3.10(1.10-8.74) and 2.63(1.02-6.80) for eczema, respectively. By summing up the 4 low molecular weight metabolites (∑4LMWP) and all 9 metabolites (∑9Total), the highest quartiles of ∑4LMWP and∑9Total were significantly associated with all symptoms. In most of the above associations, a significantly increasing trend from the 1st to the 4th quartile was observed. Subjects with 2 or 3 concomitant symptoms (reference: no symptoms) had significant positive associations with a higher level (the 4th quartile) of phthalate metabolites.CONCLUSIONS:Low MW metabolites such as MnBP and MiBP, high MW DEHP and the total amount of phthalate metabolites might have adverse health effects on asthma and allergic symptoms in Chinese children.
Pathways through which asthma risk factors contribute to asthma severity in inner-city children

Abstract:Background: Pathway analyses can be used to determine how host and environmental factors contribute to asthma severity. Objective: To investigate pathways explaining asthma severity in inner-city children. Methods: On the basis of medical evidence in the published literature, we developed a conceptual model to describe how 8 risk-factor domains (allergen sensitization, allergic inflammation, pulmonary physiology, stress, obesity, vitamin D, environmental tobacco smoke [ETS] exposure, and rhinitis severity) are linked to asthma severity. To estimate the relative magnitude and significance of hypothesized relationships among these domains and asthma severity, we applied a causal network analysis to test our model in an Inner-City Asthma Consortium study. Participants comprised 6- to 17-year-old children (n = 561) with asthma and rhinitis from 9 US inner cities who were evaluated every 2 months for 1 year. Asthma severity was measured by a longitudinal composite assessment of day and night symptoms, exacerbations, and controller usage. Results: Our conceptual model explained 53.4% of the variance in asthma severity. An allergy pathway (linking allergen sensitization, allergic inflammation, pulmonary physiology, and rhinitis severity domains to asthma severity) and the ETS exposure pathway (linking ETS exposure and pulmonary physiology domains to asthma severity) exerted significant effects on asthma severity. Among the domains, pulmonary physiology and rhinitis severity had the largest significant standardized total effects on asthma severity (-0.51 and 0.48, respectively), followed by ETS exposure (0.30) and allergic inflammation (0.22). Although vitamin D had modest but significant indirect effects on asthma severity, its total effect was insignificant (0.01). Conclusions: The standardized effect sizes generated by a causal network analysis quantify the relative contributions of different domains and can be used to prioritize interventions to address asthma severity.

Abstract:Age-related macular degeneration (AMD) is the leading cause of irreversible blindness. Because AMD adversely affects activities of daily living, rendering it more difficult to read, write, and drive, many affected individuals lose their independence in their retirement years. Age-related macular degeneration is estimated to affect more than 8 million individuals in the United States; the advanced form of the disease affects more than 1.75 million individuals. The population over age 85 years is expected to increase by 107% by the year 2020, so the prevalence of this disease will continue to rise dramatically. The only proven treatment available for the dry or nonexudative forms of this disease, which make up about 85% of the cases, is an antioxidant/mineral supplement that can slow the progression of the disease by 25% over 5 years. For the remaining 15% of cases, laser treatment and photodynamic therapy are the only therapies that have been demonstrated by randomized clinical trials to be beneficial for certain subgroups of these patients. Preventive measures are needed to reduce the burden of this disease. Smoking is the most consistently identified modifiable risk factor. Obesity, sunlight exposure, and factors including antioxidants and dietary fat intake may also affect AMD incidence and progression.Nonmodifiable risk factors include increasing age, gender, and family history of the disease. Although much progress has been made over the past decade, finding the causes and mechanisms of this condition remains a challenge.

Abstract:Background Exposure to polycyclic aromatic hydrocarbons (PAHs) is a potential risk factor for asthma prevalence. This study aims to explore whether PAHs exposure is associated with childhood asthma by altering microbial diversity and metabolic profiles. Methods Thirty children with asthma and 30 children as control in Nanjing, China were recruited. Urinary 1-hydroxypyrene (1-OHPyr) level was determined by UPLC–Orbitrap–MS as a PAHs exposure biomarker. Logistic regression was conducted to investigate the association between 1-OHPyr and childhood asthma. Microbial diversity was analyzed by 16S rRNA gene sequencing. Metabolic profiles were obtained by UPLC–Orbitrap–MS methods. Differential microbiota and metabolites were screened and selected as response biomarkers or intermediates. Mediation analysis was conducted to assess the association between PAHs and asthma mediated by intermediates. Results Participating children with and without asthma aged 6.43 ± 2.23 years. The urinary 1-OHPyr level ranged from 0.10 to 1.51 μmol/mol (creatinine corrected) in the participants. The urinary 1-OHPyr level was associated with childhood asthma (OR = 7.21, 95% CI: 1.03–50.42 per 1 μmol/mol unit). Microbial diversity was decreased in the group with asthma and there was a significant shift in the abundance of Proteobacteria (at the phylum level), Veillonella and Prevotella (at the genus level). The enrichment pathway analysis showed that differentially expressed metabolites were involved in purine metabolism, amino acid metabolism, and lipid and fatty acid metabolism. The urinary 1-OHPyr level was associated with the abundance of Actinomyces sp. oral clone IO076 and 7-methylguanine that showed a mediation effect on the association between urinary 1-OHPyr levels and childhood asthma by mediation analysis. Conclusions Urinary 1-OHPyr exposure was associated with childhood asthma, microbial diversity, and metabolic profiles. Microbial diversity and metabolic profiles may be intermediates as response biomarkers to PAHs exposure in childhood asthma. Further research is needed to confirm these study results and determine the underlying mechanism.
Modeling indoor particulate exposures in inner-city school classrooms.

Abstract:Outdoor air pollution penetrates buildings and contributes to total indoor exposures. We investigated the relationship of indoor to outdoor particulate matter in inner-city school classrooms. The School Inner City Asthma Study investigates the effect of classroom-based environmental exposures on students with asthma in the northeast United States. Mixed effects linear models were used to determine the relationships between indoor PM2.5 (particulate matter) and black carbon (BC), and their corresponding outdoor concentrations, and to develop a model for predicting exposures to these pollutants. The indoor-outdoor sulfur ratio was used as an infiltration factor of outdoor fine particles. Weeklong concentrations of PM2.5 and BC in 199 samples from 136 classrooms (30 school buildings) were compared with those measured at a central monitoring site averaged over the same timeframe. Mixed effects regression models found significant random intercept and slope effects, which indicate that: (1) there are important PM2.5 sources in classrooms; (2) the penetration of outdoor PM2.5 particles varies by school and (3) the site-specific outside PM2.5 levels (inferred by the models) differ from those observed at the central monitor site. Similar results were found for BC except for lack of indoor sources. The fitted predictions from the sulfur-adjusted models were moderately predictive of observed indoor pollutant levels (out of sample correlations: PM2.5: r2=0.68, BC; r2=0.61). Our results suggest that PM2.5 has important classroom sources, which vary by school. Furthermore, using these mixed effects models, classroom exposures can be accurately predicted for dates when central site measures are available but indoor measures are not available.

Abstract:We discuss our experience with antineoplastic drug extravasation. Between December 1988 and December 1990, 40 patients with cytostatic extravasation with lesions of varying seriousness were observed. In these patients, whenever possible, depending on the amount of time that elapsed since the accident and on the severity of the lesion, conservative therapy was done. The procedure consisted of local injection of a considerable amount of saline solution (20–90 ml, depending on the site of extravasation) and topical occlusive applications of corticosteroids locally. In all patients this was sufficient to avoid tissue necrosis. Treatment of a few patients deviated from these procedures because surgery was performed that ranged from the simple excision of infiltrated tissue to a more complex procedure of free flaps. On the basis of our experience, we discuss the role of early surgery when preventive measures and drug therapy are insufficient because of drug effects at the tissue level. In this series we did not perform early surgery.

Abstract:Background: Exposure to air pollution is associated with acute pediatric asthma exacerbations, including reduced lung function, rescue medication usage, and increased symptoms; however, most studies are limited in investigating longitudinal changes in these acute effects. This study aims to investigate the effects of daily air pollution exposure on acute pediatric asthma exacerbation risk using a repeated-measures design. Methods: We conducted a panel study of 40 children aged 8–16 years with moderate-to-severe asthma. We deployed the Biomedical REAI-Time Health Evaluation (BREATHE) Kit developed in the Los Angeles PRISMS Center to continuously monitor personal exposure to particulate matter of aerodynamic diameter < 2.5 μm (PM2.5), relative humidity and temperature, geolocation (GPS), and asthma outcomes including lung function, medication use, and symptoms for 14 days. Hourly ambient (PM2.5, nitrogen dioxide (NO2), ozone (O3)) and traffic-related (nitrogen oxides (NOx) and PM2.5) air pollution exposures were modeled based on location. We used mixed-effects models to examine the association of same day and lagged (up to 2 days) exposures with daily changes in % predicted forced expiratory volume in 1 s (FEV1) and % predicted peak expiratory flow (PEF), count of rescue inhaler puffs, and symptoms. Results: Participants were on average 12.0 years old (range: 8.4–16.8) with mean (SD) morning %predicted FEV1 of 67.9% (17.3%) and PEF of 69.1% (18.4%) and 1.4 (3.5) puffs per day of rescue inhaler use. Participants reported chest tightness, wheeze, trouble breathing, and cough symptoms on 36.4%, 17.5%, 32.3%, and 42.9%, respectively (n = 217 person-days). One SD increase in previous day O3 exposure was associated with reduced morning (beta [95% CI]: −4.11 [−6.86, −1.36]), evening (−2.65 [−5.19, −0.10]) and daily average %predicted FEV1 (−3.45 [−6.42, −0.47]). Daily (lag 0) exposure to traffic-related PM2.5 exposure was associated with reduced morning %predicted PEF (−3.97 [−7.69, −0.26]) and greater odds of “feeling scared of trouble breathing” symptom (odds ratio [95% CI]: 1.83 [1.03, 3.24]). Exposure to ambient O3, NOx, and NO was significantly associated with increased rescue inhaler use (rate ratio [95% CI]: O3 1.52 [1.02, 2.27], NOx 1.61 [1.23, 2.11], NO 1.80 [1.37, 2.35]). Conclusions: We found significant associations of air pollution exposure with lung function, rescue inhaler use, and “feeling scared of trouble breathing.” Our study demonstrates the potential of informatics and wearable sensor technologies at collecting highly resolved, contextual, and personal exposure data for understanding acute pediatric asthma triggers.

Indoor Allergen Exposure and its Association to Upper Respiratory Infections and Pulmonary Outcomes among Children with Asthma

Abstract:Background: Certain environmental allergen exposures are more common in disadvantaged communities and may contribute to differences in susceptibility to upper respiratory infections (URI). Objectives: To examine associations between indoor allergens and: 1) URI; 2) URI + cold symptoms; 3) URI + cold symptoms + pulmonary eosinophilic inflammation (FENO≥20 ppb); and 4) URI + cold symptoms + reduced lung function (percent predicted FEV1<80%). Methods: We used data from the Environmental Control as Add-on Therapy for Childhood Asthma study. Allergen concentrations were measured in air (mouse) and settled dust (mouse, cockroach, dog, and cat). URI was determined by testing nasal mucus for upper respiratory viruses. We evaluated associations between allergen concentrations and URI-associated outcomes accounting for age, sex, study month, season, health insurance, and household size. Results: 90 participants (92% Black, 92% public insurance) with 192 observations were included; 52 (27%) of observations were positive for URI. A doubling in cockroach allergen concentration increased the odds of a URI with cold symptoms by 18% (odds ratios (OR) =1.18, 95% CI, 0.99-1.40), the odds of a URI + cold symptoms + pulmonary eosinophilic inflammation by 31% (OR=1.31, 95% CI, 1.10-1.57), and the odds of a URI + cold symptoms + reduced lung function by 45% (OR = 1.45, 95% CI, 1.13-1.85). Mouse allergen concentrations were positively associated with all outcomes. Associations were suggestively stronger among children sensitized to pest allergens. Conclusions: Cockroach and mouse, but not dog or cat, allergen exposure may predispose children with asthma to URIs with colds and lower respiratory outcomes.

Abstract:Background: The National Asthma Education and Prevention Program guidelines emphasize environmental control as an integral part of asthma management; however, limited national-level data exist on how clinicians implement environmental control recommendations. Objective: We analyzed data on clinicians' self-reported use of recommended environmental control practices in a nationally representative sample (n = 1645) of primary care physicians, asthma specialists, and advanced practice providers from the National Asthma Survey of Physicians, a supplemental questionnaire to the 2012 National Ambulatory Medical Care Survey. Methods: We examined clinician and practice characteristics as well as clinicians' decisions and strategies regarding environmental trigger assessment and environmental control across provider groups. Regression modeling was used to identify clinician and practice characteristics associated with implementation of guideline recommendations. Results: A higher percentage of specialists assessed asthma triggers at home, school, and/or work than primary care or advanced practice providers (almost always: 53.6% vs 29.4% and 23.7%, respectively, P < .001). Almost all clinicians (>93%) recommended avoidance of secondhand tobacco smoke, whereas recommendations regarding cooking appliances (eg, proper ventilation) were infrequent. Although assessment and recommendation practices differed between clinician groups, modeling results showed that clinicians who reported almost always assessing asthma control were 5- to 6-fold more likely to assess environmental asthma triggers. Use of asthma action plans was also strongly associated with implementation of environmental control recommendations. Conclusions: Environmental assessment and recommendations to patients varied among asthma care providers. High adherence to other key guideline components, such as assessing asthma control, was associated with environmental assessment and recommendation practices on environmental control.

Abstract:Abstract Background In recent five years, air quality in China has been gradually improved; however, the prevalence of asthma in China has still increased significantly. This study aims to examine whether air pollution exposure is a significant contributor to childhood asthma and to explore the mechanisms using exposure and response biomarkers. Methods The generalized additive model was applied to evaluate the effects of air pollutants on asthma hospitalization. Urinary 1-hydroxypyrene (1-OHPyr) was determined as PAHs exposure biomarker. Metabolic profiles and microbial diversity were screened and selected as response biomarkers. Results Daily average concentrations of the six common air pollutants were not significantly associated with daily childhood asthma hospitalization. Urinary 1-OHPyr level showed large variability among asthmatic children and were positively correlated with the abundance of Prevotella (genus level) and purine metabolism. Conclusions The biomarker of PAHs exposure is associated with metabolic and microbiological biomarkers of childhood asthma. As the potential health risk, we suggest that PAHs should be included in the national monitoring system aiming to reduce the exposure.
