Abstract:Objectives There is an increasing trend of pre-diabetes and diabetes mellitus (DM) among adolescents, and sub-Saharan Africa is no exception. However, few published data on pre-diabetes among adolescents in Sudan exist. We aimed to investigate the prevalence of and factors associated with pre-diabetes among adolescents in Eastern Sudan. Design A community-based cross-sectional study was conducted from August to October 2023. Settings This community-based study was conducted in Gadarif city, the capital of Gadarif state, Eastern Sudan. Participants Adolescents (within the ages of 10–19 years). Main outcome measures A questionnaire was used to collect socio-demographic information. Anthropometric and glycated haemoglobin (HbA1c) measurements were performed in accordance with standard procedures. Multivariate logistic regression analysis was performed. Results Of the 387 enrolled adolescents, 207 (53.5%) were female and 180 (46.5%) were male. The median (IQR) age was 14.0 (12.0–16.0) years. 39.5% of the participants' fathers were employed. The median (IQR) HbA1c was 5.5% (5.2%–5.8%). One-third (32.6%) of the adolescents had pre-diabetes or DM. Of the participants, 67.4%, 30.0% and 2.6% had no DM, pre-diabetes or type 2 DM, respectively. In the univariate analysis, the father's employment (OR=1.60, 95% CI=1.03 to 2.50) was associated with increased odds of pre-diabetes; age, sex, parents' education, the mother's occupation, body mass index z-score, cigarette smoking and a family history of DM were not associated with pre-diabetes. In the multivariate analysis, the father's employment (adjusted OR=1.70, 95% CI=1.03 to 2.50) was associated with increased odds of pre-diabetes. Conclusion Pre-diabetes is a significant public health problem among adolescents in Eastern Sudan. The introduction of early screening programmes for pre-diabetes at the community level is recommended to halt the progression of pre-diabetes to DM and to deal with existing DM among adolescents.

Abstract:Background: Malnutrition among adolescents is a major public health issue. This problem is particularly pressing in Sudan, an African country where there is scarce published data on the nutritional status of adolescents. In this study, we aimed to assess the nutritional status of adolescents in eastern Sudan. Methods: A community-based cross-sectional survey was carried out in Gadarif, eastern Sudan. A questionnaire was used to collect sociodemographic data, and the anthropometric measurements (weight and height) of adolescent participants were recorded. Height-for-age and body mass index-for-age Z-scores were calculated using the WHO anthropometric standards. Binary and multivariate multinomial regression analyses were performed. Results: A total of 388 adolescents were included in this survey, 207 (53.4%) were female, and 181 (46.6%) were male. The median (interquartile) age was 13.9 (12.0–16.0) years. The results showed that a total of 29 (7.5%), 93 (24.0%), 33 (8.5%), and 16 (4.1%) adolescents were stunted, thin, overweight, and obese, respectively. None of the investigated factors (age, sex, parents' education levels, and occupation) were associated with stunting. In the multivariate multinomial analysis, the male sex was associated with thinness (OR = 2.41, 95.0% CI = 1.47–3.94). Moreover, adolescents whose mothers had an education lower than secondary level were at a lower risk of overweight/obesity (OR = 0. 0.35, 95.0% CI = 0. 0.35). Conclusions: While both undernutrition and overnutrition exist in eastern Sudan, undernutrition is more common. Male sex and mothers' education levels are associated with malnutrition.

Prevalence of High Blood Pressure and Associated Factors among a Population of Apparently Healthy in-School Adolescents in Delta State, Nigeria: A Cross-Sectional Survey

Abstract:Background: Adolescence is a vulnerable phase when risky behaviours like smoking, poor diet, and physical inactivity set the stage for health problems like hypertension. Objective: This study assessed the prevalence of high blood pressure (HBP) and associated factors among apparently healthy inschool adolescents in Delta State, Nigeria. Methods: A cross-sectional study was conducted among apparently healthy in-school adolescents aged 10 to 19 years, using a multi-stage random sampling technique. HBP was defined using the 2016 European Society of Hypertension Guidelines. Factors associated with HBP were determined using binary logistic regression. A p-value less than 0.05 was taken as significant. Results: Of the 574 participants surveyed, 331 (57.7%) were females and 243 (42.3%) were males, with a female-to-male ratio of 1.36. The mean age of the study participants was 14.7 years. The mean systolic and diastolic blood pressure of the study participants were 118.81 (±12.703) and 70.16 (±9.972) mmHg respectively. Prehypertension and hypertension were present in 14.8% and 18.3% of the study population, respectively. Age 10-13 years (AOR = 7.70; 95% CI: 2.26-26.22; p = 0.001) and 14-16 years (AOR = 4.62; 95% CI: 1.40 -15.25; p = 0.001), upper socioeconomic status (AOR=1.19; 95% CI: 0.57-2.48; p = 0.020), and obesity (AOR = 2.14; 95% CI: 1.08-4.25; p = 0.039) were factors associated with HBP. Conclusion: The prevalence of hypertension among the study participants was significant. Factors associated with HBP include younger age (specifically 10-16 years old), higher socioeconomic status, and obesity.

Abstract:Background: Headache disorder is the second-highest cause of disability worldwide; however, data are scarce on headache among adolescents, especially in Africa. There has yet to be published data on headache among adolescents in Sudan, the third-largest country in Africa. This study aimed to assess the prevalence of primary headache disorders and associated factors among adolescents (10–19 years) in eastern Sudan. Methods: A community-based cross-sectional study was conducted in the city of Gadarif in eastern Sudan. Questionnaires were used to gather the adolescents' sociodemographic characteristics. Headache diagnostic questions were based on the beta version of the International Classification of Headache Disorders-III (ICHD-3). Multivariate analysis was conducted to assess the associated factors for primary headache disorders, and the results were expressed as risk ratios (RRs) and 95.0% confidence interval (CI). Results: Of the 401 enrolled adolescents, 186 (46.4%) and 215 (53.6%) were male and female, respectively. The median (IQR) age was 14.0 (12.1–16.2) years. Eighty-one (20.2%) of the 401 adolescents reported experiencing primary headache disorders, including migraine with aura in 16 (4.0%), migraine without aura in 33 (8.2%), tension-type in 14 (3.5%), and undifferentiated headache in 18 (4.5%) adolescents. The prevalence of primary headache disorders was significantly higher in females than in males [55/215 (67.9%) vs. 26/186 (32.1%), p = 0.004]. In the multivariate analysis, increased age ( RR = 1.09, 95.0 CI = 1.02–1.16) and being female ( RR = 1.75, 95.0 CI = 1.14–2.67) were associated with increased RR of primary headache disorders. Parents' education level and occupation, smoking/snuff use, and body mass index were not associated with primary headache disorders. Conclusion: One-fifth of the adolescents in eastern Sudan reported experiencing primary headache disorders, which was more common in females and with increased age.

Abstract:Background: Hypertension, a major global health concern, continues to affect millions of adults worldwide, leading to significant morbidity and mortality. The burden of hypertension is particularly pronounced in low- and middle-income countries, where limited healthcare resources and infrastructure pose unique challenges for effective prevention and management. Despite the pressing need for accurate data on hypertension prevalence and determinants, there is a dearth of research focusing on the Somali population. In this study, we aim to fill this critical knowledge gap by analyzing the latest available data from the Somalia Demographic Health Survey (SDHS) conducted in 2020. Method: The SDHS 2020 employed a three-stage stratified cluster sampling design to collect data from 48,796 individuals. Hypertension was measured through survey questions related to chronic diseases. Explanatory variables included age, sex, region, residence, tobacco use, wealth index, marital status, and body mass index (BMI). Statistical analysis involved univariate and multivariable logistic regression. Results: The prevalence of hypertension in Somalia was found to be 2.11%, lower than previous studies in Hargeisa and the SLHDS 2020 report. Females had a higher prevalence than males, and urban areas exhibited higher rates compared to rural areas. Age, BMI, region, and wealth index were identified as significant factors associated with hypertension. Discussion: The study's findings contribute to the understanding of hypertension in the Somali population and can inform public health interventions. The study benefits from its representative sample, multivariate analysis, and comparison with earlier studies. However, limitations include the cross-sectional design, limited information on lifestyle behaviors, and lack of data on treatment and control of hypertension. Conclusion: Hypertension is a public health concern in Somalia, and targeted interventions are needed to reduce its burden. Addressing factors such as age, gender, urban residence, BMI, and wealth index can contribute to hypertension management and control strategies. Future research should consider longitudinal studies and incorporate additional factors for a comprehensive assessment of hypertension etiology and management.

Abstract:Introduction: Hypertension is a complex disease with many contributing factors. The prevalence of hypertension in Southeast Asian countries (SEA) has shown a significant upward trend over the last two decades on a global scale especially in adolescents. Pooled data on its burden among SEA adolescents have not yet been synthesized. Objective: The objective of this study was to determine the prevalence of hypertension and to identify its risk factors among the adolescents in SEA. Method: A systematic search was conducted through PubMed, ScienceDirect, and Cochrane Library selecting observational studies until January 2024. Meta-analysis was conducted using Review Manager 5.4. Result: There were 12 included studies with total 15,967 patients with prehypertension and hypertension. Meta analysis showed that the overall pooled prevalence estimate of hypertension among adolescents in SEA was 12.37% [95% CI (8.43, 16.32), p<0.00001]. Among this, 11.39% [95% CI (5.71, 17.07), p<0.00001] of hypertension was reported in Indonesia, 16.95% [95% CI (-0.05, 33.95), p=0.05], in Malaysia, and 71.66% in other contries. Overall pooled prevalence estimate of prehypertension was 14.64% [95% CI (12.40, 16.87), p<0.00001]. Among this, 17.53% [95% CI (12.51, 22.5), p<0.00001] of prehypertension was reported in Indonesia, 13.21% [95% CI (8, 18.42), p<0.00001] in Malaysia, and 69,26% in other contries. The common risk factors that we found were female, ethnicity, smoking, alcohol, dyslipidemia, body mass index, physical activity, and psycosocial stress. Conclusion: Our findings reveal about 1 in 10 adolescents in SEA are hypertensive. This study encourages urgent primary and secondary prevention activities and have implications for policy and intervention development among policymakers.

The interplay between hypertension and BMI in Coptic teenagers

Abstract:Background: Elevated Body Mass Index (BMI) is a significant risk factor for developing hypertension (HTN). Other factors, including family history, smoking, and energy drinks consumption, can potentially increase the risk for HTN. Limited data are available on the risk factors for HTN in Coptic adolescent population. Hence, this study sought to investigate the association between these risk factors and HTN in Coptic adolescents. Methods: A screening survey was conducted on 140 high school students who attended church services. After signing a consent form, each participant was interviewed and asked to answer the survey questions. The blood pressure was measured twice after 15 minutes of rest using Omron device. Both weight and height were measured, and BMI was calculated using CDC BMI calculator. Descriptive statistics, univariate, and multivariable logistic regression analyses were used to explore the relationship between the HTN and other potential variables. Results: The cohort included 70 females (50%) and 70 males (50%) aged 16-21. The overall prevalence of HTN was 45.7% (73% in males and 18.6% in females). Participants were categorized by BMI into normal (n=78), overweight (n=44), and obese (n=18) groups. Compared to the normal weight group, the odds ratio (OR) for HTN in the overweight and obesity groups was 7.2 (95% C.I. 3.1 – 16.6, p<0.001) and 1.9 (95% C.I. 0.67 – 5.5, p=0.27), respectively. Among the total cohort, hypertensive individuals exhibited significantly higher BMI compared to normotensive individuals (26.3 [23.5, 28.3] vs. 22.5 [20.6, 25.7], p=0.001). However, there was no association between HTN and other covariates (p>0.05). In multivariable logistic regression, the association between HTN and elevated BMI remained significant (adjusted OR 3.8, 95% C.I. 1.6 – 9.3; p=0.002). Conclusion: our findings are consistent with accumulative evidence about the positive relationship between BMI and HTN, which is a part of the metabolic syndrome. Moreover, this is the first study to investigate the risk factor profile for HTN in Coptic adolescents. Larger scale epidemiologic studies are needed to assess the prevalence of HTN in this population and the best approach to manage the modifiable risk factors.

The Epidemiology of Hypertension in Uganda: Findings from the National Non-Communicable Diseases Risk Factor Survey

Abstract:BACKGROUND: Hypertension is an important contributor to global burden of disease and mortality, and is a growing public health problem in sub-Saharan Africa. However, most sub-Saharan African countries lack detailed countrywide data on hypertension and other non-communicable diseases (NCD) risk factors that would provide benchmark information for design of appropriate interventions. We analyzed blood pressure data from Uganda's nationwide NCD risk factor survey conducted in 2014, to describe the prevalence and distribution of hypertension in the Ugandan population, and to identify the associated factors.METHODS: The NCD risk factor survey drew a countrywide sample stratified by the four regions of the country, and with separate estimates for rural and urban areas. The World Health Organization's STEPs tool was used to collect data on demographic and behavioral characteristics, and physical and biochemical measurements. Prevalence rate ratios (PRR) using modified Poison regression modelling was used to identify factors associated with hypertension.RESULTS: Of the 3906 participants, 1033 were classified as hypertensive, giving an overall prevalence of 26.4%. Prevalence was highest in the central region at 28.5%, followed by the eastern region at 26.4%, western region at 26.3%, and northern region at 23.3%. Prevalence in urban areas was 28.9%, and 25.8% in rural areas. The differences between regions, and between rural-urban areas were not statistically significant. Only 7.7% of participants with hypertension were aware of their high blood pressure. The prevalence of pre-hypertension was also high at 36.9%. The only modifiable factor found to be associated with hypertension was higher body mass index (BMI). Compared to participants with BMI less than 25 kg/m2, prevalence was significantly higher among participants with BMI between 25 to 29.9 kg/m2 with an adjusted PRR = 1.46 [95% CI = 1.25-1.71], and even higher among obese participants (BMI ≥ 30 kg/m2) with an adjusted PRR = 1.60 [95% CI = 1.29-1.99]. The un-modifiable factor found to be associated with hypertension was older age with an adjusted PRR of 1.02 [95% CI = 1.02-1.03] per yearly increase in age.CONCLUSIONS: The prevalence of hypertension in Uganda is high, with no significant differences in distribution by geographical location. Only 7.7% of persons with hypertension were aware of their hypertension, indicating a high burden of undiagnosed and un-controlled high blood pressure. Thus a big percentage of persons with hypertension are at high risk of hypertension-related cardiovascular NCDs.

Abstract:Hypertension in adolescents has attracted increasing attention. However, large, well-designed studies accurately demonstrating epidemiological characteristics of adolescent hypertension remain insufficient. We, therefore, conducted a population-based multicenter study with repeated blood pressure visits to offer precise prevalence of it. We randomly recruited 42 025 adolescents aged 12 to 17 years in 6 centers throughout China from 2018 to 2019. The initial hypertension was estimated in all centers through one blood pressure measurement visit. Confirmed hypertension was evaluated in 3 blood pressure visits in 2 of 6 centers including 16 220 subjects. Hypertension was defined using American and Chinese guidelines and standardized by sex and age using 2010 China population census data. Standardized prevalence of initial and confirmed hypertension were 18.6% (95% CI, 17.3%–20.0%) and 5.9% (95% CI, 4.7%–7.2%) using 2017 American Association Pediatrics hypertension guidelines, respectively and which were 24.5% (95% CI, 23.0%–25.9%) and 8.8% (95% CI, 7.3%–10.3%) using 2018 Chinese hypertension guidelines. Male (odds ratio [OR], 1.275 [95% CI, 1.070–1.520]), parental hypertension history (OR, 1.387 [95% CI, 1.159–1.660]), higher heart rate (OR, 1.043 [95% CI, 1.037–1.049]), higher waist circumference (OR, 1.036 [95% CI, 1.027–1.046]), and adiposity indexes including overweight (OR, 2.211 [95% CI, 1.781–2.745]), and obesity (OR, 3.085 [95% CI, 2.330–4.084]) were identified as risk factors. In conclusion, the prevalence of hypertension was quite high evaluated by both guidelines with several risk factors, and a single blood pressure measurement visit leads to an overestimation of hypertension prevalence among adolescents.

Abstract:Hypertension is currently highly prevalent worldwide and serves as one of the significant risk factors for chronic diseases and mortality. Adult hypertension can be traced back to, as well as prevented starting in, childhood and adolescence. However, due to the lack of surveillance among children and adolescents, the prevalence and influencing factors of hypertension-related conditions have not been well described. Hence, a total of 67,947 children and adolescents aged 6 to 17 from China Nutrition and Health Surveillance (2015-2017) were enrolled to describe the weighted average blood pressure level and the weighted prevalence of hypertension, pre-hypertension, and their distribution and to analyze the risk factors for hypertension and pre-hypertension among Chinese children and adolescents at a nationwide level. In summary, the weighted mean values of systolic blood pressure and diastolic blood pressure were 111.8 (95% CI, 111.2-112.5) mmHg and 66.5 (95% CI, 66.0-67.0) mmHg, respectively. The weighted prevalence of hypertension and pre-hypertension was 24.9% and 17.1%, respectively. Moreover, general obesity, overweight, and central obesity served as risk factors for hypertension and pre-hypertension among Chinese children and adolescents. The current study indicated that the prevalence of hypertension and pre-hypertension in Chinese children and adolescents was at a high level. Moreover, blood pressure screening should be further intensified for children and adolescents at a high risk of being overweight or obese.

Abstract:Background: Hypertension is defined as two or more readings of systolic blood pressure measurement of 130 mm Hg or higher or diastolic blood pressure measurement of 80 mm Hg or higher. In the early stages, symptoms of HTN are often not detectable and thus many people with the disease are left undiagnosed. Undiagnosed HTN increases the risk of complications such as renal failure, myocardial infarction, heart failure, stroke and premature death. There was a high prevalence of undiagnosed HTN among adolescents. Objective: To assess the prevalence of undiagnosed hypertension and its associated factors among adult peoples living in Enemor Woreda, Ethiopia, 2021. Method: A community based cross-sectional study design was conducted from April to May, 2021. Data were collected using a structured self-administered English version questionnaire; which translated to the national Amharic language. The data were collected from 630 households using systematic random sampling technique at K=7. The data were entered to epi data version 3.4 and analyzed by SPSS version 26. Bivariate and multivariable logistic regression with 95% CI was hold to determine the significance of variables. Result: In this finding 260 (45.3%) of respondents were identified as having undiagnosed hypertension. Respondents having sedentary life style (AOR=2.24, 95%CI (1.37, 3.67)], BMI ≥25 (AOR=2.42, 95%CI (1.43, 4.09)], smokers [AOR: 0.45, 95%CI(0.28, 0.73)], health seeking behavior [AOR: 0.41, 95%CI(0.25, 0.66)], monthly income 25, having sedentary life style, cigarette smoking, health seeking behavior, dietary habit and monthly income were significance predictors of undiagnosed HTN; therefore, people should follow Life style modification and have health seeking behavior to early detect and manage undiagnosed HTN with in the community.

Abstract:Abstract Background: Hypertension is the leading cause of preventable deaths globally, yet there have been inconsistent reports on its burden and risk factors in rural Sub-Saharan Africa. This study aimed to assess the prevalence, awareness, and risk factors associated with hypertension among adults in a rural community in southwestern Uganda. Methods: A baseline survey was conducted as part of an ongoing implementation science cohort study in Ngango, a rural parish in the Mbarara district of southwestern Uganda. The study included adults aged 18-79 years from eleven villages. Research assistants and community health workers visited homes to enroll consenting adults. Data collection involved administering the WHO STEPS questionnaire, which gathered demographic information, behavioral characteristics, and lifestyle data, including tobacco and alcohol use, salt intake, fruit and vegetable consumption, and physical activity. Participants were also asked about prior blood pressure (BP) measurements. BP readings were taken three times, two minutes apart, along with anthropometric measurements. Hypertension was defined as BP >=140/90 mmHg, based on the average of the last two readings, or self-reported use of antihypertensive medication. The primary outcome was the prevalence and factors associated with hypertension, assessed using logistic regression. Secondary outcomes included hypertension awareness and the proportion of participants with controlled hypertension. Results: A total of 953 adults were enrolled. The median age was 43 years, with most participants being female (61.5%). Hypertension prevalence was 27.3% (260/953). Among those with hypertension, 61.5% were unaware, 27.7% were on treatment, and 65.3% had controlled BP. Despite 66.8% of participants reporting physical activity, 63.7% were overweight. Factors associated with hypertension were age > 40 years (OR 2.26, 95% CI 1.53-3.33; p <0.001), consuming <3 servings of fruit or vegetables per week (OR 1.62, 95% CI 1.11-2.35, p = 0.012), overweight (OR 1.57, 95% CI 1.05-2.34, p = 0.028), and obesity (OR 2.73, 95% CI 1.80-4.15, p <0.001). Conclusion: The prevalence of hypertension in rural southwestern Uganda is high, despite a relatively young and physically active population, indicating the need for targeted interventions.

Abstract:Hypertension in youth tracks into adulthood and has long-term negative health effects. In 2017, the American Academy of Pediatrics released new Clinical Practice Guidelines on pediatric hypertension. The guidelines updated blood pressure (BP) thresholds and age-sex-height percentile references for youth 1–17 years. Pediatric hypertension prevalence estimates since the new guideline are lacking. This study aims to provide the latest nationally representative prevalence estimates of elevated BP and hypertension in youth aged 8–19 years, total and stratified by age group, sex, race and ethnicity, and body mass index (BMI) category. For adolescents aged 13–17 years, both percentile-based thresholds and adult BP thresholds are available in the guidelines. We therefore compared adult BP thresholds for elevated BP (120–129/<80 mmHg) and hypertension (≥130/≥80 mmHg) with percentile thresholds in adolescents. We used data from 2017–March 2020 National Health and Nutrition Examination Survey to obtain a sample of 2600 youth after excluding those with no BMI or BP measures. Prevalence estimates of elevated BP and hypertension were 8.7% (95% CI: 6.9-10.6) and 5.4% (95% CI: 4.0-6.7), respectively. Estimates were higher among males and increased with age (Table). Youth with severe obesity had the highest prevalence of hypertension. Hispanic youth had the lowest prevalence of elevated BP. In adolescents, elevated BP prevalence was similar using percentile thresholds (11.9%, 95% CI: 9.3-15.1) and the adult threshold (10.8%, 95% CI: 8.5-13.7). Hypertension prevalence in adolescents was also similar for percentile thresholds (4.8%, 95% CI: 3.1-7.6) and the adult threshold (4.8%, 95% CI: 2.0-7.5). Our findings suggest ongoing concern of elevated BP and hypertension among youth in the US. Either percentile or adult based thresholds for determining high BP could be used among adolescents by researchers. Knowledge of current national estimates can inform policies and interventions to support cardiovascular health in youth.

Abstract:Abstract Background To investigate the regional prevalence of hypertension, overweight, and obesity among adolescents aged 11–17 years in Xuzhou. The relationships between overweight, obesity, abdominal obesity, age, gender, grades, and hypertension were studied. Methods The stratified cluster random sampling method was used and the middle school students from the first to the third grade of 10 junior and senior middle schools in Xuzhou were involved. Blood pressure, weight, height, and waist circumference were measured for each participant. Hypertension was diagnosed according to the standards of high blood pressure by gender, age, and height of children aged 3–17 in China in 2017. Overweight and obesity were defined based on the standards of the Chinese Obesity Working Group. Abdominal obesity was defined as waist-to-height ratio >0.46. Results A total of 10,004 students aged 11–17 years were enrolled with 4,774 females and 5,230 males. The prevalence of hypertension was 9.8% in all participants, 7.2% in females, and 12.2% in males. There were 1,495 students with obesity (14.9%), and 1,698 with overweight (17.0%). The prevalence of hypertension was 5.6%, 12.8%, and 25.4% in normal weight, overweight, and obese adolescents, respectively. A multivariate analysis showed that males (odd ratio, 95% confidence interval: 1.450, 1.256–1.674), age (1.770, 1.537–2.037), overweight (1.858, 1.465–2.357), obesity (4.010, 3.084–5.214), abdominal obesity (1.497, 1.184–1.892), and third grade (1.488, 1.250–1.772) were risk factors for adolescent hypertension. The risk of hypertension in male adolescents over 14 years old was 2.456 times than that of adolescents under 14 years old, while there was no significant difference in the risk of hypertension between female adolescents over and under 14 years old. In adolescents with normal weight, subjects with abdominal obesity had a much higher risk of hypertension compared with those without abdominal obesity (1.848, 1.334–2.559). Conclusions The prevalence of hypertension among adolescent school students in Xuzhou is high. Overweight and obesity are independent risk factors of hypertension in adolescents.

Abstract:The growing prevalence of overweight/obesity in adolescents highlights the significance of studying metabolic syndrome (MetS) in increasingly sedentary adolescents. To date, no study in Sudan has examined the association between MetS and physical activity (PA) among adolescents. This study aimed to assess the association between MetS and its components and PA among Sudanese early adolescents. A cross-sectional assessment was conducted from to 2018 to 2019 on a sample of 921 primary school students from Khartoum State, Republic of Sudan. MetS was defined according to the International Diabetes Federation criteria and a standardized questionnaire was used to assess PA. Metabolic equivalents of task were calculated, and levels of different intensities of PA (tertiles) were identified accordingly. The association between PA and MetS and its components was assessed using a logistic regression model. The participants comprised 388 boys and 533 girls with a mean age of 12.59 ± 1.21 years. The prevalence of MetS was significantly higher in subjects in the 1st PA tertile (least active) than in the 2nd and 3rd (most active) tertiles of PA, and this difference was observed in both boys and girls. After adjusting for other study factors, the odds of MetS among adolescents in the 1st PA tertile were 7 times higher than those in the highest PA tertile (adjusted odds ratio = 7.01, 95% confidence interval: 1.48, 32.99). A physically inactive lifestyle was associated with higher odds of MetS and its components, especially waist circumference and triglyceride levels, in Sudanese early adolescents. This study highlights the importance of promoting PA in this age group.

Abstract:Objectives: Hypertension is a health issue affecting adolescents. Accumulating evidence affirms that elevated blood pressure begins in childhood and tracks into adulthood. This cross-sectional study was conducted to determine the prevalence of elevated blood pressure and its predictors among secondary school students in Sarawak, Malaysia. Methods: A total of 2,461 secondary school students aged 12-17 years from 19 schools in Sarawak participated in the study. Questionnaire was used to obtain socio-demographic data, parental history of hypertension, and self-reported physical activity. Anthropometric and blood pressure measurements were taken. Data was entered and analysed using SPSS version 23.0. Results: The prevalence of adolescents with elevated blood pressure, overweight, central obesity, and overfat were 30.1%, 24.3%, 13.5%, and 6.7%, respectively. Multivariate logistic regression demonstrated the predictors significantly associated with elevated blood pressure among respondents: overweight (adjusted odds ratio=3.144), being male (adjusted odds ratio=3.073), being Chinese (adjusted odds ratio=2.321) or Iban (adjusted odds ratio=1.578), central obesity (adjusted odds ratio=2.145), being overfat (adjusted odds ratio=1.885), and being an older adolescent (adjusted odds ratio=1.109). Parental history of hypertension, locality, and physical activity showed no significant associations. Conclusion: The obesity epidemic must be tackled at community and school levels by health education and regulation of school canteen foods.

Abstract:Background: Emerging epidemiological data suggest that Hypertension (HTN) has become a significant public health challenge in sub-Saharan Africa. HTN in young adults is a problem lacking relevant attention because it is still erroneously considered a disease of the old. This study aimed to determine the prevalence of hypertension and its associated risk factors in undergraduate medical students at the University of Nigeria, Enugu Campus, Enugu State, Nigeria. Methodology: This was a cross-sectional study conducted between March and April 2021. This study recruited 279 consenting medical students (136 males and 143 females) aged 18-35 years. They were administered with a structured questionnaire. Data on sociodemographic information and risk factors for hypertension were collected. Blood pressure, waist circumference, weight, height, and body mass index were measured using standard methods. All data collected were carried out following the Institutional ethical guidelines and that of the Helsinki as revised in 2000. Data were analyzed using IBM Statistical Package for Social Sciences version 25, and statistical tools employed include descriptive statistics and Chi tests. Results were recorded as mean standard deviation, and statistical significance was taken at p<0.05. Results: This present study has shown a prevalence rate of 19.93% for hypertension. Isolated diastolic hypertension constituted a greater burden with a prevalence of 13.65% than systolic Hypertension (0.74%) and systolic-diastolic Hypertension 5.4%. The prevalence of prehypertension was 48.7%, with a higher incidence observed in females (25.8%), individuals aged 21-25 years (26.4), and those with normal BMI (35.1%). A significant association was observed between the stage of hypertension and gender (p = 0.005), and age category (p = 0.037). Of the examined cohort, 7.75% were underweight, 16.5% overweight, and 2.2% obese. Notably, systolic, and diastolic blood pressure, weight, as well as waist circumference showed significant (p = 0.01, p = 0.007, p =0.01 and p<0.0001 respectively) increases concomitant with advancing age. Conclusion: There is an increased prevalence of prehypertension and hypertension among young adults. This calls for a comprehensive national screening, public enlightenment, and targeted prevention programs that foster healthy lifestyle behaviours, physical activity, and healthy eating among students.

Abstract:Introduction: Several explanations might address the failure of antihypertensive medications: poor compliance, excessive salt ingestion, drug interactions, or an unsuspected secondary cause for hypertension. Resistant hypertension, as an additional failure cause, is relatively rare, but all patients should be considered medically treatable until proven otherwise. Therefore, we aim to explore the blood pressure readings and the current control status in hypertensive Sudanese patients. Methods: Utilizing the MMM 2022 Sudan screening data, participants’ BP >18 years was measured using a standard device by trained physicians for three consecutive measures, and a single mean value was calculated excluding the first reading. Participants with a history of hypertension were included in the analysis. Continuous data was presented in mean and standard deviation, and categorical data was presented in frequencies and percentages. According to current guidelines, several antihypertensive medications used were counted for each patient. All participants completed a structured questionnaire after obtaining their approval. Results: The study included 2,292 participants with an established diagnosis of hypertension. The mean age at diagnosis was 46±13 years, around 60% were females, and the median duration of hypertension was 5 (2, 10) years. Of these, 936 (40.9%), 790 (34.5%), and 543 (23.7%) had blood pressure exceeding SBP≥140 mmHg, DBP≥90 mmHg, and BP≥140/90 mmHg, respectively. Table 1 shows the baseline characteristics for each group. Despite the guideline recommendations to control hypertension, 78 (14.4%) were not adherent to medications, and 56 (10.3%) were taking three or more antihypertensive drugs. Conclusions: BP screening enables future decisions to control hypertension-related burden. Adherence to the internationally recommended guidelines will lead to achieving BP targets and prevent undesired outcomes. The subgroup of resistant hypertensive patients needs close monitoring and appropriate medical evaluation.

Abstract:PURPOSE: To determine the association between sociodemographic characteristics and blood pressure among a demographically diverse population-based sample of 10-14-year-old US adolescents.METHODS: We conducted cross-sectional analyses of data from the Adolescent Brain Cognitive Development Study (N = 4,466), year two (2018-2020). Logistic and linear regression models were used to determine the association between sociodemographic characteristics (sex, race/ethnicity, sexual orientation, household income, and parental education) with blood pressure among early adolescents.RESULTS: The sample was 49.3% female and 46.7% non-White. Overall, 4.1% had blood pressures in the hypertensive range. Male sex was associated with 48% higher odds of hypertensive-range blood pressures than female sex (95% confidence interval [CI], 1.02; 2.14), and Black race was associated with 85% higher odds of hypertensive-range blood pressures compared to White race (95% CI, 1.11; 3.08). Several annual household income categories less than $100,000 were associated with higher odds of hypertensive-range blood pressures compared to an annual household income greater than $200,000. We found effect modification by household income for Black adolescents; Black race (compared to White race) was more strongly associated with higher odds of hypertensive-range blood pressures in households with income greater than $75,000 (odds ratio 3.92; 95% CI, 1.95; 7.88) compared to those with income less than $75,000 (odds ratio 1.53; 95% CI, 0.80; 2.92).DISCUSSION: Sociodemographic characteristics are differentially associated with higher blood pressure in early adolescents. Future research could examine potential mediating factors (e.g., physical activity, nutrition, tobacco) linking sociodemographic characteristics and blood pressure to inform targeted interventions.
