Abstract:BACKGROUND The aim of the study was the cross-cultural adaptation and validation of the Musculoskeletal Pain Intensity and Interference Questionnaire for Musicians. MATERIAL AND METHODS The process consisted of the following stages: forward translation, backward translation, analysis by committee of experts, pretest of Polish-language version of questionnaire, and cross-cultural validation: psychometric evaluation. In the last step, a convenience sample of 31 professional orchestra musicians (professional experience 16.7±9.5 years) was used to validate the new tool. The primary study outcome measurements were construct validity and internal consistency as measured by exploratory factor analysis (EFA) and Cronbach's alpha, respectively. RESULTS A final Polish version of the questionnaire was developed. Using EFA, the 2-factor structure (pain intensity and pain interference) was obtained, taking into account all 9 items, explaining about 76% of the total variance. The pain interference and pain intensity factors were characterized by high internal consistency (Cronbach's alpha 0.923 and 0.784, respectively). The lifetime prevalence of playing-related musculoskeletal disorders was 87%. CONCLUSIONS Translation and a cross-cultural adaptation of the Polish version of the questionnaire was successfully completed. The results obtained show a correctly carried-out validation process, but further testing of the tool is suggested. The Polish-language version of the validated tool can be used in scientific and clinical practice. The next step would be to estimate cutoff values for severity of health status in orchestra musicians.

Abstract:Background: The only complete and validated tool for evaluating professional orchestra musicians is the English-language Musculoskeletal Pain Intensity and Interference Questionnaire for professional orchestra Musicians (MPIIQM) questionnaire, which, in recent years, has been translated, adapted, and validated in other languages. The aim of the study was to validate the online version of the Polish version of the Musculoskeletal Pain Intensity and Interference Questionnaire for Musicians (MPIIQM-P). Materials and Methods: The group included 182 professional musicians. The respondents were asked to complete the MPIIQM-P, BPI, and QuickDash questionnaires twice within an interval of 4 days. The questionnaires were created in the web form creator—Google Form. Results: The EFA analysis showed a two-factor structure of the questionnaire consisting of factor 1—pain intensity and factor 2—pain interference. The internal agreement of the factors identified in the EFA analysis was measured by the α Cronbach index reaching from 0.813 to 0.913. The intraclass correlation coefficient of both factors ranged from 0.276 to 0.583. The analysis of convergent and divergent validity between the subscales of the questionnaire value ranged from 0.414 to 0.925. Conclusions: The online version of the MPIIQM-P questionnaire is a valid and reliable tool for the assessment of musculoskeletal pain and interference. The online MPIIQM-P questionnaire maintains the psychometric properties previously defined for the paper version.

Abstract:Scandinavian Journal of Public Health, Ahead of Print. Background:Orchestra musicians have a high risk of experiencing musculoskeletal problems. These problems may lead to sleep and psychological disturbance.Objectives:This study investigated the occurrence of musculoskeletal pain (MSP) among orchestra musicians and the coherence between pain and predictors such as gender, age and instrument. Further, the impact of pain on sleep and mental health was analysed to assess pain as a predictor threatening musicians' wellbeing.Methods:Some 358 musicians in eight different professional orchestras in Norway completed a questionnaire. Questions about pain experienced within the last 30 days were based on a national survey. Standardised screening tools like the Hopkins Symptom Checklist-25 (HSCL-25) and Bergen Insomnia Scale (BIS) were used to assess psychological distress and sleeping problems. Kruskal–Wallis tests were used to analyse the differences in the pain-related variables: experienced pain severity (EPS), number of pain areas, and work being perceived as the source of pain based on age. Mann–Whitney U tests were conducted to evaluate the statistical significance between gender and the pain-related variables. Spearman's rank correlations were used to examine the relationship between EPS and BIS, HSCL-25 and pain area. Additionally, simple linear regression models were employed to determine whether EPS can predict higher scores on the HSCL-25 and BIS scales.Results:The prevalence of MSP experienced in the last 30 days among orchestra musicians in Norway was 85%. Musicians experienced frequent pain regardless of their gender, age or instrument group. Neck, shoulders and upper back represented the most reported pain areas. Further, it was observed that increased EPS was correlated with an increase in the number of pain areas and in work being perceived as the source of pain. EPS significantly predicted sleep- and psychological distress.Conclusions:Orchestra musicians have a high risk of encountering MSP. Furthermore, their psychological distress and sleeping problems seem to be related to experienced MSP. To deliver optimal health, these problems should be handled simultaneously.

Abstract:Abstract Objective To evaluate prevalence of performance-related pain among musicians and compare pain characteristics, associated disability and approach to pain management, between music students and professional musicians. Methods A multicenter cross-sectional study was performed in a multi-stage stratified cluster random sample of 585 musicians, stratified by music students (294, among which 234 were at pre-university level and 60 at university level) and professional musicians (291). The main outcome was performance-related pain, measured by Performance-related Pain among Musicians Questionnaire (PPAM). Secondary outcomes were: pain management approach, physical activity levels, fatigue, distress, and health-related quality of life. Results The lifetime prevalence of performance-related pain was 55.0% (n = 322), being significantly higher among professionals (57.5% versus 42.5%, p < 0.001). Pain intensity scores were significantly higher in professional musicians (p < 0,05), but pain interference in performance was higher among music students, who also reported significantly higher levels of fatigue (p = 0.008) and distress (p < 0.001), and lower quality of life (p < 0.001). Regardless of the high levels of pain intensity and interference, fatigue, anxiety and depression, and low levels of quality of life, 33% musicians who developed pain had never looked for healthcare (this proportion is significantly higher for music students, 57%, p < 0.001). Conclusions We present the first multicenter study on performance-related pain among musicians with different backgrounds, using a validated tool, and distinguishing music students from professional musicians. Performance-related pain is a highly prevalent and disabling condition among musicians, however, its proper evaluation and management are still often underappreciated, contributing to significant impairments and reduced quality of life.

Methods Using Survey Monkey software, the German version of the 'TMD‐Pain‐Screener' was distributed electronically to professional and student musicians in Europe. Abstract:Musculoskeletal (MSK) conditions among professional musicians and music students are frequent and may have significant physical and psychosocial consequences on their lives and/or on their playing abilities. The Risk of Music Students (RISMUS) research project was set up in 2018 to longitudinally identify factors associated with increased risk of playing-related musculoskeletal disorders (PRMDs) in a large sample of music students enrolled in pan-European institutions. The aim of this cross-sectional study was to describe the prevalence of playing-related musculoskeletal disorders (PRMDs) in this novel population at baseline of the RISMUS project. A further goal was to begin to identify variables that might be associated with the self-reported presence of PRMDs among music students. Eight hundred and fifty students from fifty-six conservatories and music universities in Europe completed a web-based questionnaire on lifestyle and physical activity participation levels, musical practice habits, health history and PRMDs, psychological distress, perfectionism and fatigue. A total of 560 (65%) out of 850 participants self-reported a positive history of painful MSK conditions in the last 12 months, 408 (48%) of whom self-reported PRMDs. Results showed that coming from West Europe, being a first- or a second-year Masters student, having more years of experience and higher rates of perceived exertion after 45 minutes of practice without breaks were factors significantly associated with self-reported presence of PRMDs. According to the authors' knowledge, a large-scale multicentre study investigating prevalence and associated factors for PRMDs among music students at different stages of their education (from Pre-college to Masters levels) has not been conducted before. The high prevalence of PRMDs among music students, especially those studying at university-level, has been confirmed in this study and associated factors have been identified, highlighting the need for relevant targeted interventions as well as effective prevention and treatment strategies.
Brazilian Version of the Musculoskeletal Pain Intensity and Interference Questionnaire for Musicians (MPIIQM-Br): Cross-Cultural Adaptation and Psychometric Properties

Abstract:Objective: The present study aimed to adapt and validate the self-report tool named Musculoskeletal Pain Intensity and Interference Questionnaire for Musicians (MPIIQM-Br) into the Brazilian Portuguese language for professional orchestra musicians. Methods: The cross-cultural adaptation process was divided into nine steps. Five experts evaluated the equivalences among versions and the content validity ratio (CVR). The structural validity was verified by exploratory factor analysis (EFA). Construct validity was checked by convergent/divergent validity by comparing the results obtained by the MPIIQM-Br, the Brief Pain Inventory (BPI), and Disabilities of the Arm, Shoulder, and Hand Questionnaire-optional module (DASH-PAM), and the Kenny Music Performance Anxiety Inventory (K-MPAI). Finally, reliability was tested with internal consistency and test-retest reliability. Results: In total, 273 professional orchestra musicians (137 males) participated. The researchers found good equivalence among versions, and the CVR reached 0.99. EFA (n=212) showed that the MPIIQM-Br had two factors that explained 70% of the total variance. The convergent validity confirmed a strong positive correlation (r>0.7) and the divergent validity established a low correlation (r<0.4). Both internal consistency (Cronbach's a=0.90) and temporal stability (intraclass correlation coefficient=0.84) obtained high values. Discussion: The psychometric evaluation showed a two-dimensional structure, internal consistency, and temporal stability similar to the original tool and the German and Polish versions of the MPIIQM. However, the pain prevalence rates among Brazilian musicians were higher. Extreme informality and the growing demotivation may explain these findings. Conclusions: The MPIIQM-Br is a transcultural equivalent, reliable, and valid tool for the measurement and evaluation of pain and pain interference among professional orchestra musicians.
Health behaviours and work-related musculoskeletal pain among polish physiotherapists and nurses

Abstract:BACKGROUND: Physiotherapists and nurses play crucial roles in healthcare, often requiring them to adopt physically demanding positions. However, these positions can lead to joint stress and an increased risk of injury, as well as work-related musculoskeletal disorders (WRMSDs). These disorders can significantly impact the performance of their duties, which can negatively affect patient recovery. OBJECTIVE: This study assessed and compared health behaviour and work-related musculoskeletal pain among Polish physiotherapists and nurses. METHODS: The study encompassed a group of 304 participants. The methodology consisted of a survey addressing pain-related questions and the Health Behaviour Inventory (HBI). The HBI evaluates health behaviour in four categories. RESULTS: The average HBI score was 5.05 (±1.96) points for physiotherapists and 4.50 (±1.86) points for nurses (P = .01). When analysing the HBI components, a statistically significant difference was observed in positive mental attitude and health practices (P < .05). Regarding work-related musculoskeletal pain, 92% of nurses and 79% of physiotherapists reported experiencing pain while working (P < .01). Pain intensity, measured with the VAS, was 3.77 (±2.37) for physiotherapists and 4.73 (SD 2.04) for nurses (P < .01). Physiotherapists more often suffered from upper limb pain (57% vs 49%), while nurses experienced middle/lower back pain (82% vs 63%) and lower extremity pain (38% vs 23%) more frequently. CONCLUSIONS: Both professional groups experience WRMSDs, with nurses suffering more regularly and severely. In contrast, physiotherapists exhibit superior health behaviour practices, potentially mitigating musculoskeletal pain. Nonetheless, the prevalence of pain related to WRMSDs remains alarmingly high in both professions.

Abstract:OBJECTIVE: To evaluate the effect of a workplace injury prevention and wellness program compared to no intervention (control) on musicians' playing-related musculoskeletal pain intensity. The hypothesis was that musicians who completed the program would have a greater reduction in pain intensity than the control group. DESIGN: Pragmatic parallel randomized controlled trial. METHODS: Sixty-five orchestra musicians were recruited and completed patient-reported outcome measures (primary outcome: pain intensity over 11 months, using the Musculoskeletal Pain Intensity and Interference Questionnaire for Musicians; range: 0-40; lower is better) at baseline, 14 weeks (T1), and 11 months (T2). Following baseline assessment, participants were randomly assigned to the intervention (n = 33) or control (n = 32) groups, stratified by instrument and pain prevalence. The intervention group received a 14-week injury prevention and wellness program including education and exercise; the control group received no intervention. RESULTS: Pain intensity means (standard deviation) were 7.8 (6.2), 8.0 (7.5), and 8.6 (5.7) in the control group, and 9.0 (6.6), 5.0 (4.2), and 6.7 (6.6) in the intervention group at T0, T1, and T2, respectively. Using intention-to-treat analyses (3 dropouts, n = 65 analyzed), between-group differences in pain intensity (95% confidence interval) were T1-T0: -4.2 (-7.5, -0.9); T2-T0: -3.7 (-7.1, -0.3), type III (overall) P = .03, favoring the intervention group. No adverse events were reported. CONCLUSION: A workplace injury prevention and wellness program may have a clinically meaningful effect on reducing orchestra musicians' pain intensity. Work-related musculoskeletal disorders in Polish sonographers-A questionnaire study

Abstract:Purpose: The ultrasound specialists face a substantial issue with work-related musculoskeletal disorders (WMSDs), characterized by occupation-related pain, reaching an estimated frequency of up to 90.5%. The type and location of pain vary depending on the sonographers specialty and the specific anatomical areas being examined. Our study aimed to assess the prevalence and intensity of pain among Polish doctors from various specialties conducting ultrasound examinations. Materials and methods: The study has been performed between July and December 2023, involving 90 participants (51.9% women) actively practicing ultrasound diagnostics. The data collection process utilized a structured questionnaire developed by the researchers for this study. The questionnaire covered personal factors, including gender and age, and work-related aspects like workplace, specialty, sonography experience, and the types of examinations conducted. Statistical analysis involved both descriptive statistics and correlation analysis. Results: The age distribution ranged from 26 to 74 years, with mean (SD) of 43.1 (12.2) years. The leading specialties among physicians were radiology (22.2%) and internal medicine (22.2%). Among all physicians, 65.6% reported pain during or after ultrasound scans, with a mean (SD) pain intensity of 6.17 (2) in a 1-10 scale. The analysis indicated no correlation between age and gender and the occurrence of discomfort. Conclusions: WMSDs pose a significant risk to Polish sonographers, especially those specializing in vascular surgery, general surgery and obstetrics and gynecology specialties. The study underscores a notable deficit, with only 10% of the personnel reporting training in ergonomics for ultrasound work.
Lower thresholds and stronger adaptation to pain in musicians reflect occupational‐specific adaptations to contact heat stimulation

Abstract:Background Extensive audio‐motor training and psychological stress can cause professional musicians acute overstrain‐injury and chronic pain, resulting in damaged careers and diminished quality of life. It has also been previously shown that musicians might perceive pain differently than non‐musicians. Therefore, the aim of our study was to quantify differences between musicians and non‐musicians regarding their subjective responses to painful contact heat stimuli and assess how emotional traits might influence these responses. Methods Upon completing the StateTrait‐Anxiety‐Depression Inventory, 15 healthy musicians and 15 healthy non‐musicians from German universities received 15 noxious contact heat stimuli at the dorsal side of each hand and foot. After each stimulation, participants were asked to provide a pain rating from 0 to 10. Results Musicians not only reported significantly higher pain ratings after the first stimulation but also showed a significantly higher degree of habituation compared to non‐musicians. Additionally, musicians showed a significantly less pronounced difference regarding the pain rating of the hands compared to the feet than non‐musicians. Trait anxiety and trait depression scores had no effect on the pain rating or the habituation. Conclusion The more pronounced habituation of musicians might hint at a neuroplastic nociceptive alteration in musicians. The lack of significance between the psychological traits and their effect on the pain ratings is surprising but could be a result of both participant groups having stressful careers. Significance The findings of this report justify musicians' repetitive sensorimotor training as an important model for plasticity and contribute to a better understanding of pain perception in musicians.

Abstract:Introduction The neuro-biological side of chronic pain research has presented reliable evidence of distinct cortical and spinal alteration compared to healthy individuals. Furthermore, research suggests that musicians are especially vulnerable to pain, and recent neurological investigations into musicians' brain plasticity support this hypothesis. However, chronic pain is not acute pain plus time, but a separate condition, and little is known about musicians' chronic pain-related emotions and behaviors. This knowledge, however, is a crucial step in understanding how chronic pain is processed by musicians. Methods This study investigated pain catastrophizing as a critical pain-related behavior and emotional concept alongside six complementary variables: anxiety, depression, depersonalisation, burnout, coping strategies and professional identity. Results 103 under- and postgraduate students from various higher education institutions participated in an online survey. Students were allocated into three groups according to their main study subject and type of institution: music college musicians, university musicians and university non-musicians. A tree model confirmed the current chronic pain multifactorial model, suggesting a combination of several variables before catastrophizing pain. Group testing, however, showed that university non-musicians' pain catastrophizing was significantly worse especially when compared to music college musicians. Music college musicians and university musicians were less prone to maladaptive pain processes, despite perceiving pain for significantly longer. Discussion This novel finding indicates that chronic pain does not inevitably lead to dysfunctional pain processing for musicians and should be reflected accordingly to optimize pain-control. The biopsychosocio model of chronic pain provides a robust framework for future research in this population.

Abstract:Background: Musculoskeletal problems are common in musicians. Coronavirus 2019 (COVID-19) has brought with it lockdowns and quarantine periods, and as a result, caused physical and psychological problems. Objective: To evaluate musculoskeletal problems, attitudes and behaviors related to physical activity, and social media addiction in musicians during the COVID-19 pandemic. Methods: This cross-sectional study was conducted on a total of 137 musicians (string, keyboard, wind, percussion instrument players, and vocalists) online. Musculoskeletal problems with Cornell Musculoskeletal Discomfort Questionnaire (CMDQ), attitudes and behaviors related to physical activity with Cognitive Behavioral Physical Activity Questionnaire (CBPAQ), social media addiction with Bergen Social Media Addiction Scale (BSMAS) were evaluated. Results: In CMDQ, it was found that upper back, lower back, and neck problems were more common in all musicians. There were no differences between instrument groups in terms of CBPAQ sub-dimensions, total CBPAQ, and total BSMAS (p > 0.05). Conclusion: Considering the dramatic effects of COVID-19, it may be important not to ignore musicians' musculoskeletal problems. Abstract:Background: Inadequate protein intake and hypoalbuminemia, indicators of protein-energy wasting, are among the strongest mortality predictors in hemodialysis patients. Hemodialysis patients are frequently counseled on dietary phosphorus restriction, which may inadvertently lead to decreased protein intake. We hypothesized that, in hypoalbuminemic hemodialysis patients, provision of high-protein meals during hemodialysis combined with a potent phosphorus binder increases serum albumin without raising phosphorus levels. Playing-related musculoskeletal disorders among Chinese conservatoire piano students: prevalence, risk factors and preventive interventions

Abstract:Introduction: Both professional musicians and conservatoire students are at significant risk of developing playing-related musculoskeletal disorders (PRMDs) during their career life. With the growing number of students pursuing a conservatory degree and graduating from music conservatory in China, the aims of this study were: (1) to identify the nature of PRMD and explore the prevalence of PRMD in Chinese conservatoire students; (2) to determine the relevant risk factors with the presence of PRMD among Chinese conservatoire students; and (3) to suggest preventive interventions for young pianists at their early career stage. Methods: A self-reported online survey study was conducted among 363 Chinese conservatoire students who majored in piano performance. Results: Of all respondents, 82.6% reported having had at least one PRMD. The wrist was proved to be the most affected body site, followed by the shoulder, finger and arm. Respondents who experienced PRMD reported "pain," "fatigue," and "stiffness" as the most frequent symptoms. The main risk factors associated with PRMD included gender, years of playing experiences, practice hours, warm-up habits before practice, and break-taking during practice. Discussion: Female students, those with longer year of playing experience, those who practice longer hours, those who do not warm up before practice, and those who do not take breaks during practice were found to have more PRMD symptoms and higher level of severity. This study highlights the need to increase awareness of PRMD among conservatoire students and to understand the occurrence of PRMD; it is helpful for young pianists to prevent severe musculoskeletal disorders and implement preventive measures at early career stages. Further studies are suggested to follow up on music students who have had at least one PRMD at different stages of professional musical training.

Abstract:Abstract Background With the dynamic development of professional Paralympic sport, the prevalence of musculoskeletal pain and structural and/or functional disturbances in Para athletes constantly increases. The aim of the study was to evaluate the impact of internal compensatory mechanisms on selected aspects of body structure and function in elite sitting volleyball players. Methods The study included eighteen elite sitting volleyball players (male; n = 12, female; n = 6, age; 36.0 ± 6.1, body mass; 76.6 ± 16.1, body height; 179.3 ± 0.1) from the Polish national team. Retrospective and direct participatory observation methods were used in the study. NMQ-7 was used to assess the current prevalence and location of musculoskeletal pain. The evaluation of spinal curvature and pelvic inclination was performed using a non-invasive Medi Mouse method (Idiag M360) in three different trunk positions. All statistical analyses were performed using Statistica 13.3 software package. Results Lumbar hypolordosis was a predominant sagittal deviation of spinal curvature (n = 15;83%). Low back pain (LBP) and neck pain were the most frequent complaints (50%). Statistically significant differences in the values of thoracic kyphosis angle, pelvic inclination, and spine length (SL) in sagittal standing flexion and extension were found. However, there was no statistically significant difference in sagittal standing flexion for the lumbar lordosis angle with a simultaneous significant change in pelvic inclination (66.9°). Moreover, a tendency to interpenetration of relationships between variables that characterize (a) body structure and (b) function of the spine and musculoskeletal pain were observed. Shoulder pain correlated with SL (R = 0.6; p < 0.05) and body height (R = 0.5; p < 0.05). Pelvic inclination correlated with shoulder pain, LBP (R = 0.5; p < 0.05/R = 0.6; p < 0.01), and body trunk fat mass (R = − 0.6; p < 0.05). Conclusions Trunk fat mass induces internal compensatory mechanisms to maintain optimal pelvic inclination and sagittal spinal balance. Furthermore, the level of pelvic mobility may determine musculoskeletal pain in Para athletes with lower limb impairment.

Abstract:The sound in classical orchestral music is louder than noise emissions allowed by national rules in industry. We wanted to assess the audiologic status of professional musicians at different ages of their careers and to look for a coherence of declined hearing ability and the sound emissions in order to substantiate advices for hearing protection and occupational medicine in musicians. Data from questionnaires (anamnestic data on sound exposure in profession and leisure times, use of hearing protection, self-evaluation of hearing function and hearing deficits), audiometric data and amplitudes of OAE were evaluated from 109 professional musicians aged 30–69 years from three major German orchestras and from 110 students of an academy of music (aged 11–19 years). Sound emissions of the whole orchestra and of single instruments/instrument groups were measured at the orchestra stages and pits during rehearsals and performances. None of the musicians was engaged in noisy hobbies and only a few used hearing protectors regularly. More than 50% of the musicians had a hearing loss of 15 dB(A) and more. Highest losses were found among the strings and the brass players. DPOAE amplitudes coincidently declined with the duration of performing music in the orchestras. Professional musicians aged older than 60 years had a significantly greater hearing loss at 4 and 6 kHz than those aged 30–39 years. Among the strings in one orchestra a dominant hearing deficit in the left ears was observed. Musicians need the same health care for their hearing as workers in noisy industry. A better education on the hearing hazards (use of hearing protectors) as well as sound protection in the rehearsal rooms is necessary. Hearing loss in professional musicians should be accepted as an occupational disease.
